BIJLAGE I Rapport in opdracht van de Staatssecretaris van Sociale Zaken en Volksgezondheid, dr. R. J. H. Kruisinga door het Rijks Instituut voor de Volksgezondheid opgesteld, rapp. No 120/69, dd. 21 oktober 1969, Laboratorium voor Toxicologie (hoofd dr. G. J. van Esch), betreffende toelaatbaarheid van het gebruik van cyclamaten als zoetstof in de voeding Inleiding Door de Joint FAO/WHO Expert Committee on Food Additives werd in 1967 een uitspraak gedaan over de toelaatbaarheid van Natrium- en Calciumcyclamaat, nadat de beschikbare toxiciteitsgegevens bestudeerd waren. De uitspraak luidde: "Estimate of acceptable daily intake (ADI) for man: 0-50 mg/kg bodyweight; temporary acceptance for 3 years" De reden waarom een temporary ADI werd gesteld was als volgt: "In view of the extensive studies in animal and man and widespread use in some countries, on the one hand and the gaps in information about certain biological effects on the other, a temporary acceptable daily intake is allocated with some further work specified below". Het onderzoek dat alsnog vereist werd en in een periode van 3 jaren verschaft diende te worden, werd als volgt omschreven: "1) Investigation into the metabolism to cyclohexylamine and other possible metabolites and full toxocological evaluation of these metabolites including any possible human genetic aspect. 2) Investigation into the exact mechanism of the laxative effect including any possible enteropathic cellular and subcellular changes. 3) Lifespan studies in a second species including observation on reproduction and development of several filial generations." Sinds de beoordeling van de twee cyclamaten door de Joint FAO/WHO Expert Committee zijn vele toxiciteitsonderzoekingen met cyclamaten en cyclohexylamine uitgevoerd. De gegevens van deze onderzoekingen zijn, voor zover die eind maart 1969 in onze dokumentatie aanwezig waren, samengevat in de twee bijgevoegde monografieën. De belangrijkste aspekten uit de literatuur, vermeld in de monografieën van de genoemde Expert Committee en van het Rijks Instituut voor de Volksgezondheid, zullen hieronder worden samengevat. Samenvatting van alle toxiciteitsonderzoekingen rover vermeld in het rapport van de Joint FAO/ WHO Expert Committee en in de bijgesloten bijlage over cyclamaten Onderzoek naar de biotransformatie van cyclamaat Uit de verschillende biotransformatieproeven met ratten blijkt dat na éénmalige of na herhaalde orale toediening van cyclamaat (in een aantal proeven gemerkt met 35 S en , 4 C)
± 15 - 30 pet. in de urine en in de faeces ± 7 0 - 8 0 pet. onveranderd cyclamaat wordt uitgescheiden. In bepaalde proeven wordt met 14 C-cyclamaat in het karkas 0,2 pet. en met 35 Scyclamaat 3 - 8 pet. van de toegediende radioactiviteit gevonden. Ongeveer 0,12 pet. van de 14 C activiteit wordt in de melk aangetoond. Na intraperitoneale en intraveneuze toediening blijkt dat na een éénmalige applicatie reeds na 5 uur pl.m. 70 pet. onveranderd in de urine is uitgescheiden. De organen longen, nieren, speekselklieren, bloed en placenta bij zwangere dieren toonden enige radioactiviteit. Na 7 uur was de radioactiviteit uit de placenta verdwenen, maar wordt dan in het darmkanaal (uitscheiding via de gal) en de nieren van de foetus gevonden. In een andere rattenproef, waarin gedurende 27—34 weken 0,5, 1,1 en 2,5 g van een mengsel cyclamaat/saccharine (10 : 1) per kg lichaamsgewicht per dag werd gegeven, werd de excretie van cyclohexylamine nagegaan. Uit de resultaten bleek dat hoe hoger de dagelijkse doses, des te meer deze metaboliet uitgescheiden wordt. Tevens bleek dat er dieren zijn die cyclamaat slechts voor een klein deel in deze metaboliet omzetten ("low converters"), terwijl andere dieren de zoetstof in grotere hoeveelheden in cyclohexylamine omzetten ("high converters"). In het laatste geval was dit gemiddeld 1 pet. maar bij één dier was dit ± 10 pet. van de toegediende hoeveelheid cyclamaat. Uitscheiding van dicyclohexylamine werd niet gevonden. Honden (na orale toediening) en konijnen (orale en intraperitioneale en intraveneuze toediening) scheiden met de urine grotere hoeveelheden onveranderd cyclamaat uit, nl. 80-90 pet. in 24 uur. In deze proeven was de hoeveelheid die in de organen aanwezig was zeer gering, nl. pl.m. 0,027 pet. van de totale dosis radioactief cyclamaat toegediend. Bij éénmalige of meerdere malen toediening van cyclamaat aan mensen werd (afhankelijk van doses) 31-79 pet. onveranderd in de urine uitgescheiden. Uit proeven met mensen en honden bleek dat naast de uitscheiding van cyclamaat ook cyclohexylamine in de urine wordt gevonden. Bij de mens voor pl.m. 0,12-0.8 pet. van de toegediende radioactieve hoeveelheid cyclamaat, terwijl dit bij honden slechts sporen waren. In een uitgebreid onderzoek met 40 vrijwilligers bleek dat 5 personen pl.m. 0,8 pet. (na 2-3 dagen) en één persoon 7.5 pet. van de toegediende hoeveelheid cyclamaat (na 17 dagen toediening) als cyclohexylamine uitscheidde. Een andere auteur vond zelfs één persoon met 40 pet. Uit deze gegevens blijkt als bij ratten, dat ook hier sprake is van „low" en „high converters". Bij patiënten met chronische nierziekten werd zowel na orale als na intraveneuze toediening geen duidelijk verschil in de excretie van cyclamaten gevonden in vergelijking met gezonde personen. 10 376

2 De vraag die gesteld kan worden is of het verschil in omzetting van cyclamaat in cyclohexylamine in „low converters" en „high converters" genetisch bepaald is. Een andere mogelijkheid is dat de microflora van de darm hierbij een rol speelt en mogelijk voor de verschillen tussen de 2 groepen verantwoordelijk is. Conclusie Bij verschillende diersoorten wordt cyclamaat bijna geheel binnen 48 uur onveranderd met urine en faeces uitgescheiden. Accumulatie in organen vindt niet of nauwelijks plaats. De hoeveelheden cyclamaat die in cyclohexylamine worden omgezet zijn in het algemeen gering (tot 1 pet.) maar bepaalde individuen of dieren scheiden met de urine hogere hoeveelheden uit. Er kan van zg. „low converters" en „high converters" worden gesproken. Mogelijk is dit genetisch bepaald of speelt een bepaalde darmflora hierbij een rol. De uitscheiding van dicyclohexylamine is (met de huidige analyse methoden) niet aangetoond. Het is niet bekend of onderzocht is in hoeverre andere stoffen een invloed hebben op de „detoxificatie" van cyclamaat. Onderzoekingen met mensen Uit het onderzoek met ratten, muizen, honden en apen is gebleken dat hoge doses cyclamaat diarrhee geven. Dit aspekt is met behulp van 35S-cyclamaat bestudeerd en uit de proeven is gebleken dat de laxalieve werking berust op een increased bulk en secondair op een osmotische hypertonicity. In een aantal proeven met gezonde mensen en kinderen is nagegaan welke doses diarrhee of andere symptonen geven. De duur van deze proeven was van enkele weken tot 7 i maand. De doseringen lagen tussen 1 en 18 g/dag. Uit de proeven met volwassenen bleek dat 3 g/dag zonder verschijnselen werd verdragen terwijl bij 5-7 g de frekwentie van de darmbeweging is toegenomen en zachte faeces wordt waargenomen. Met doseringen van 10 g en hoger vertoonden de personen diarrhee. In een proef met 184 kinderen die 1,0 en 1,5 g/dag kregen gedurende 2 perioden van 12 weken, onderbroken door een periode van 2 weken waarin geen cyclamaat werd verstrekt, werd slechts bij enkele kinderen zachte faeces waargenomen. Geen diarrhee of andere effekten werden gezien. Naast deze onderzoekingen werden eveneens waarnemingen gedaan bij personen met lever-, nier-, maag-, darmziekten en bij diabetici. Patiënten (51) met chronische nierziekten, 17 patiënten met maagdarmziekten en 32 patiënten met chronische leverziekten kregen per dag voor perioden variërende tussen 6 weken en 61 maand hoeveelheden tot 5 g cyclamaat per dag. Slechts in 2 gevallen (één met nierziekte en één met maagdarmziekte) vertoonden diarrhee. Verder werden geen afwijkingen of invloed op het herstel van de patiënt gevonden. Diabetici (42) namen 0,2-3,2 g/dag op gedurende 6 tot 9 maanden. Geen invloed op darmkanaal en andere klinischchemische testen werd gevonden, behalve dan dat de activiteit van de serum alkalische fosfatase verlaagd en het gemiddelde (nuchter) bloedsuikergehalte gering verhoogd was. Conclusie Uit de proeven met volwassenen en kinderen blijkt dat resp. 3 en 1,5 g/dag geen invloed heeft op de darmbeweging en geen diarrhee geeft. Doses boven 5 g kunnen bij volwassenen in enkele gevallen zachte faeces geven terwijl 10 g en hoger diarrhee geeft. Uit het onderzoek met patiënten lijdende aan lever-, nier- en maagdarmziekten, blijkt dat cyclamaten geen duidelijke invloed hebben op het klinische verloop van de ziekte en dat de uitscheiding van cyclamaten niet verschillend is van gezonde personen. Bij diabetici werd een geringe invloed op het bloedsuikergehalte gevonden. Ook hier is de dosis die door laatst-
genoemde patiënten goed wordt verdragen gelijk aan die van gezonde mensen. Subacute, semichronische en chronische dierproeven In de diverse subacute en semichronische toxiciteitsproeven met ratten is natriumcyclamaat en/of calciumcyclamaat getest. Uit de gegevens blijkt dat 1 pet. en hoger groeiremming geeft en met 5 pet. worden histologische darmepitheelafwijkingen gevonden. Deze afwijkingen in de dunne darm kunnen als volgt worden omschreven: "Zwelling van de mucosa cellen met diepere crypten en cytoplasmatische vacuolisatie. In het ileum kwam oedeem voor, soms met atrofie van villi en hyperplasie van de zenuwcellen van de plexus van Auerbach. Histochemisch wordt een niet specifieke „slijmcel" hyperplasie met duidelijke enzymactiviteit beïnvloed waargenomen". In een andere proef, duur 11 maanden, worden met 1 pet. geen afwijkingen gevonden, maar met 2 en 3 pet. vertonen enige dieren diarrhee, echter geen invloed op groei, bloed en urinesamenstelling, lever en nier gewicht. Evenmin werd in deze proef een nadelige invloed op de fertiliteit (3 generaties) waargenomen. De diarrhee wordt naar alle waarschijnlijkheid veroorzaakt door de osmotische werking van het niet geresorbeerde cyclamaat in de darm. Apen krijgen 4 g per kg lichaamsgewicht gedurende 9 maanden. Geen afwijkingen werden gevonden, ook niet in de activiteit van leverenzymen, echter wel werd een vergroting van de eilandjes van Langerhans in de pancreas waargenomen. Met de electronenmicroscoop werden subcellulaire veranderingen waargenomen, waarvan de betekenis momenteel niet duidelijk is. Uit kortdurende konijnenproeven blijkt dat 1,2 g natriumcyclamaat per kg lichaamsgewicht een invloed heeft op bloedstolling, o.a. daling van de prothrombine-tijd werd gevonden. In een (slecht uitgevoerd) onderzoek met caviae gedurende 5 maanden, gaf 2 pet. een verhoogde mortaliteit en bij 0,5 en 2 pet. werden histologische leverafwijkingen waargenomen. Enige hondenproeven met een duur van 30 dagen tot omstreeks 15 maanden toonden dat 0,5 g/kg lichaamsgewicht in de 10de maand van de proef zachte faeces gaf, echter geen andere afwijkingen. Een tamelijk uitgebreide proef gedurende 15 maanden gaf geen afwijkingen te zien met 0, 0,2 en 0,4 pet. In een andere uitgebreide proef kregen honden 0,5, 1,0 en 1,5 g per kg lichaamsgewicht per dag (mengsel van Na-cyclamaat 4- saccharine ( 1 0 : 1 ) ) . Met deze dieren werd één generatie gefokt en deze jonge dieren kregen gedurende 1 jaar cyclamaat en werden daarna gedood. Geen afwijkingen werden gevonden. In een andere proef werd 0, 0,45, 0,9 en 1,35 g/kg lichaamsgewicht gedurende 6 maanden gegeven. De jonge dieren toonden geen afwijkingen. Naast deze kortdurende onderzoekingen zijn 2 chronische toxiciteitsonderzoekingen met ratten uitgevoerd. Hierbij moet opgemerkt worden dat dit onderzoek uitgevoerd werd in 1951. In de eerste proef werden de doseringen 0, 0,05, 0,1 en 1,0 pet. gedurende 18 tot 30 maanden aan de dieren gegeven. Tevens werden 3 generaties gefokt, maar alleen met de dosering 0,05 pet. Het onderzoek is voor die tijd uitgebreid uitgevoerd en toonde geen afwijkingen in groei, bloed- en urinesamenstelling, histopathologie en mortaliteit. De tumorfrekwentie komt overeen met die van de controles. Normale nesten werden geboren. In de tweede proef werd 0, 0,01, 0,1, 0,5, 1,0 en 5,0 pet. gedurende 24 maanden getest. In de groep 5 pet. werd groeiremming en diarrhee waargenomen. Mortaliteit, orgaangewichten, histopathologie en tumorfrekwentie niet verschillend van controledieren. Een speciaal reproduktie onderzoek is uitgevoerd, waarin drie doses cyclamaat in drinkwater en één dosis in het voedsel werd gegeven. De doses waren resp. 0, 150 en 280 mg per volwassen dier en 30 000 ppm ( ~ 1500 mg/kg lichaamsgewicht) in het voer. In Fi vrouwelijke dieren was in alle testgroepen de oestrus cyclus verstoord. Met de twee hoogste doses werden leververanderingen en in de hoogste dosis testes atrofie waargenomen. In deze twee doseringen was in de Fa generatie het aantal jongen zo gering dat de F3 generatie alleen met de laagste dosis kon worden verkregen (proef was beperkt van opzet).
3 Conclusie Uit de semichronische en chronische toxiciteitsproeven blijkt dat 1 pet. in het voer en hoger bij ratten groeiremming kan geven. Met 5 pet. worden histologische darmafwijkingen en diarrhee gevonden. In een proef werd met 2 pet. bij enige diarrhee gevonden. De twee chronische rattenproeven blijken uitgevoerd te zijn in 1951, waarbij bleek dat 1 pet. geen afwijkingen gaf. De 3de generatie kon alleen met de 0,05 pet. doseringsgroep gefokt worden. De tumorfrekwentie was als bij de controledieren. Bij apen werden met 4 g per kg lichaamsgewicht afwijkingen in de pancreas waargenomen, waarvan de betekenis niet bekend is. Bij konijnen 1,2 g/kg werd een invloed op de bloedstolling gevonden, terwijl bij caviae met 2 pet. in het voer verhoogde mortaliteit werd waargenomen. De cavia blijkt het gevoeligste proefdier te zijn. In hondenproeven bleek 1,5 g per kg lichaamsgewicht geen afwijkingen te geven. Evenmin werden afwijkingen gevonden in de jonge dieren, geboren in de 1ste generatie en gedurende 1 jaar op dit dieet gehouden. In een ander reproduktieonderzoek, 3 generaties, met ratten bleek pl.m. 375 mg/kg lichaamsgewicht geen afwijkingen, maar 700 mg een duidelijke invloed op de reproduktie te hebben. Teratogenesis en andere speciale onderzoekingen Met muizen, ratten, konijnen en honden werden teratogene proeven uitgevoerd. In de proeven werden verschillende doseringen (hoogste dosering was 500 mg) oraal aan zwangere muizen gegeven op verschillende momenten van de zwangerschap. Vijfhonderd mg op de 8-1 Ode dag toegediend gaf geen afwijkingen, terwijl toegediend op de 4-5de en 6—7de dag resorpties van embryo's of afgestorven embroyo's werden gevonden. In andere proeven waar in 0,5 en 10 g/kg lichaamsgewicht aan zwangere muizen werd gegeven op de 8ste dag, werden geen afwijkingen waargenomen. In de rattenproeven werd 50, 100, 150 en 250 mg (Nacyclamaat) per kg lichaamsgewicht gegeven op de 6de tot 15de dag. Geen teratogene afwijkingen of embryo toxische effekten werden gezien in vergelijking met de controlegroep. Hetzelfde resultaat werd verkregen bij konijnen met 1 g/kg oraal op de lste-8ste dag. Door de Food and Drug Administration is een onderzoek verricht waarin werd waargenomen dat cyclohexylamine chromosoom-breuk in de kiemcellen van de testes van de rat induceert. Normaal worden in ± 1 pet. van de cellen deze afwijkingen gevonden. Wanneer mannelijke ratten 50 mg/kg lichaamsgewicht per dag, gedurende vijf dagen, kregen werden deze afwijkingen gevonden in 16,5 pet. van de kiemcellen van de testes en in 12,5 pet. van de cellen van het beenmerg. Dagelijkse doses van 10 mg/kg lichaamsgewicht gaven nog een significante toename in chromosoomafwijkingen (nl. 3,2 pet.). In een experiment met vrijwilligers kon echter bij orale opname van 3-16 g cyclamaat (het waren personen die cyclohexylamine uitscheidden), geen invloed op chromosoomafwijkingen waargenomen worden. Conclusie De teratogene onderzoekingen geven geen duidelijk antwoord op de vraag of het cyclamaat teratogeen is. In één experiment werd na toediening van de stof op bepaalde momenten (4-5de en 6-7de dag van de zwangerschap) wel een invloed gezien maar in andere experimenten niet. Het onderzoek waarin na intraperitoneale toediening een toegenomen percentage chromosomen-breuken in de kiemcellen en cellen van het beenmerg wordt gezien, is moeilijk te beoordelen. In hoeverre vindt dit ook plaats na orale opname van de stof? De betekenis van dit onderzoek is niet duidelijk; het kan een zeer ernstig feit zijn, maar mogelijk ook niet. Verder onderzoek met vrijwilligers is dan ook van groot belang. Onderzoek naar de inductie van tumoren In een proef met ratten, die gedurende 98 weken 0,5 ml 15 pet. calciumcyclamaatoplossing in water 3x per week subcu-
taan kregen toegediend, werd bij 14/30 dieren plaatselijk sarcoma's geïnduceerd. Tumoren op afstand werden niet waargenomen. Met natriumcyclamaat, op dezelfde wijze getest, werden geen sarcoma's gevonden. De inductie van locale sarcoma's ontstaan na subacute injecties heeft zoals door de W.H.O. Scientific group in 1967 tot uitdrukking werd gebracht, de volgende betekenis: „ should not, alone, be considered significant evidence of a carcinogenic hazard: such a finding, however indicates the desirability of a thorough study for systemic manifestation of carcinogenicity by other parenteral or further oral investigations". Uitgebreid onderzoek naar de inductie van tumoren na orale toediening werd en wordt nog uitgevoerd. Uit een telexbericht van Mr. Craig van Abbott, Chicago (USA) bleek dat in een experiment is aangetoond dat implantatie van cholesteroltabletten die 20 pet. natriumcyclamaat bevatten in de urineblaas van muizen, blaastumoren induceert. Deze techniek wordt gebruikt om te zien of een stof als deze, zodanig in de blaas gekomen, tumoren kan induceren. Als controle worden bij een andere groep dieren alleen de cholesteroltabletten (zonder cyclamaat) geïmplanteerd. De inductie van blaastumoren met deze techniek was aanleiding om in een ander chronisch toxiciteitsonderzoek, waarbij een mengsel van cyclamaat en saccharine aan ratten werd gevoerd, na te gaan of ook in dit onderzoek blaastumoren aanwezig zouden zijn. Zover uit het telexbericht blijkt werden in 8 dieren blaastumoren gevonden waarvan minstens 3 carcinomen waren. In dit onderzoek werden zeer hoge doseringen gebruikt, ± 50 maal de dosis die door de Joint FAO/WHO Expert Committee wordt beschouwd als zijnde toelaatbaar voor de mens. Verdere details van dit onderzoek ontbreken momenteel nog. Aangezien in laatstgenoemd onderzoek een mengsel van cyclamaat en saccharine is getest en uit de literatuur bekend is dat saccharine in de vorm van cholesteroltabletten geimplanteerd in de urineblaas van de muis (the l l t h report of the Joint FAO/WHO Expert Committee on Food Additives 1967. WHO/Food. Add./68.33) eveneens blaastumoren geeft, moet de conclusie getrokken worden dat ook saccharine onder verdenking staat. Onderzoek naar de invloed van cyclamaten op de groei van ratten In het Nederlands Instituut voor Volksvoeding te Wageningen is een onderzoek verricht naar de invloed van cyclamaat op de groei van ratten. Uit dit oriënterende onderzoek blijkt dat natriumcyclamaat de eetlust bij ratten stimuleert en daardoor gewichtsvermeerdering geeft. In dit onderzoek werd alleen de voedselopname, groei en voedselconversie bepaald. Verder werd geen onderzoek gedaan. Conclusie Gezien de belangrijke beslissing over de toelaatbaarheid van cyclamaat en saccharine, is het van groot belang dat het onderzoek met het mengsel herhaald wordt, eventueel met verschillende doseringen, en dat daarnaast cyclamaat en saccharine apart getest worden op cancerogene eigenschappen. Samenvatting van alle gegevens van toxiciteitsonderzoekingen zover deze momenteel van cyclohexylamine ter beschikking zgn Konijnen scheiden na orale applicatie van HC-cyclohexylamine onveranderd cyclohexylamine en N-hydroxcyclohexylamine in de urine uit. Slechts 0,3 pet. van de radioactiviteit wordt als 14C02 uitgeademd. Een deel van de uitgescheiden verbindingen is geconjugeerd aan glucuronzuur. Mogelijk is ook de metaboliet cyclohexanon-oxim (tot 2,5 pet.) aanwezig (mogelijk artefact). Bij honden wordt in 24 uur cyclohexylamine grotendeels onveranderd uitgescheiden en 11-27 pet. in de vorm van een (nog onbekende) metaboliet.
4 Aan een vrijwilliger werd de met 14C gemerkte stof gegeven. Hiervan werd 98 pet. onveranderd in de urine teruggevonden. Uit de verkregen gegevens bleek dat de „half-life" in plasma van cyclohexylamine voor de mens, rat en hond ongeveer gelijk is, nl. 2-4 uur. Uit de farmacologische proeven is gebleken dat de stof een indirekt werkend sympathicometische stof is en wat werking betreft op de stof thyramine lijkt. Cyclohexylamine geeft cardiovasculaire effekten, mogelijk doordat het catecholamine vrij maakt en/of door directe stimulering van «-adrenerge receptoren. De stof bleek bij dieren en mensen sterk huidirriterend en zwak sensibiliserend te zijn. De acute orale LDso van cyclohexylamine is voor ratten 230-350 mg per kg lichaamsgewicht. De stof geeft lever-, nier- en hersenbeschadiging. Uit het interim rapport van de Food and Drug Administration van Amerika blijkt uit de eerste resultaten van het chronisch toxiciteits-onderzoek en het reproduktieonderzoek, dat bij ratten met 15 mg/kg lichaamsgewicht na 1 jaar groeiremming werd gevonden. Bij honden werd dit met dezelfde dosering niet waargenomen. Uit het reproduktieonderzoek blijkt dat 15 mg/kg lichaamsgewicht bij ratten er. konijnen geen invloed had op de fertiliteit en reproduktie. Evenmin werden teratogene afwijkingen gevonden. Wel werd bij de 15 mg een grotere sterfte waargenomen. Bij de konijnen werd met 15 mg een groter aantal resorpties, kleinere nesten en verhoogde mortaliteit waargenomen. Deze gegevens zijn echter in hetzelfde en in een ander laboratorium slechts ten dele, nl. de verhoogde sterfte, bevestigd. Conclusie Uit de onderzoekingen blijkt dat cyclohexylamine gemakkelijk wordt opgenomen en grotendeels onveranderd wordt uitgescheiden. Over het voorkomen van metabolieten is weinig bekend. Belangrijk is dat dit metabolisme-onderzoek wordt voortgezet en speciale aandacht wordt gegeven aan het eventueel voorkomen van dicyclohexylamine, een mogelijk zwak carcinogene verbinding. Alvorens een meer definitieve conclusie over de toxiciteit van cyclohexylamine gegeven kan worden, zullen de resultaten van het toxiciteitsonderzoek dat gaande is. afgewacht moeten worden zodat dan eventueel met behulp van de no-effect level een berekening van toelaatbare hoeveelheden gemaakt kan worden in vergelijking met de hoeveelheid die mogelijk door de mens wordt opgenomen of geproduceerd. Discussie en conclusie Uit de proefdiergegevens en de tamelijk uitgebreide mensengegevens blijkt dat de ADI. gesteld door de Joint FAO/WHO Expert Committee, zijnde 0-50 mg/kg lichaamsgewicht, met de tot op dit moment beschikbare toxiciteitsgegevens nog steeds overeenstemt. Deze ADI komt voor een mens van 60 kg en een kind van 25-30 kg resp. overeen met een opname van 3.0 en 1.25-1.5 gram per dag. Hierbij dient opgemerkt te worden dat bij de vaststelling van deze hoeveelheden geen conventionele veiligheidsfactor is toegepast. Hoe is nu de situatie in Nederland? D.w.z. hoeveel cyclamaat wordt door de consument dagelijks opgenomen? Door deze hoeveelheid te vergelijken met de hoeveelheid berekend per mens/dag op basis van de ADI, is het mogelijk een beeld van de situatie te krijgen. In Nederland zijn volgens de Warenwet zoetstoffen toegelaten onder de volgende voorwaarden: Artikel 3 quater: lid 1: „In eet- en drinkwaren mogen geen andere kunstmatige zoetstoffen aanwezig zijn dan orthobenzoëzuursulfimide (saccharine), N-cyclohexytsulfaminezuur en onschadelijke zouten dezer stoffen". In het 2de lid staat vermeld dat op de verpakking van eeten drinkwaren, waarin één der in het eerste lid genoemde kunstmatige zoetstoffen aanwezig is, de vermelding „bevat kunstmatige zoetstof" aangebracht dient te zijn.
In het 3de lid staat o.a. vermeld dat hetgeen in het 2de lid staat, niet van toepassing is voor waren waarvoor in enige ander algemene maatregel van bestuur krachtens de Warenwet met betrekking tot het aanbrengen van bedoelde vermelding anders is bepaald. Tegenover deze algemene aanwijzing van zoetstoffen staat dat diverse Warenwetbcsluiten een bepaling bevatten, waarin het gebruik van zoetstoffen in de betreffende waren niet toegestaan is. Een uitzondering vormen de toestemming genoemd in het Visbesluit van het gebruik van zoetstoffen in halfconserven; in het Bierbesluit het gebruik van saccharine in bier tot een gehalte van 15 mg per liter zonder vermelding op het etiket en het gebruik van saccharine in donker bier tot een gehalte van 60 mg per liter met vermelding. Waren bestemd of geschikt voor lijders aan suikerziekte worden geregeld voor ieder produkt afzonderlijk bij Koninklijke toestemming onder voorwaarden. In deze voorwaarden wordt o.a. het gebruik van zoetstoffen in deze waren geregeld. Uit het bovenstaande volgt dat cyclamaten niet zijn toegestaan in limonades, softdrinks etc. Dit houdt in dat de belangrijkste bron waarmede cyclamaten opgenomen zouden worden, zoals bv. in de U.S.A.. in Nederland niet aanwezig is. In welke vorm en welke hoeveelheid worden in Nederland de cyclamaten dan wel gebruikt? Volgens de gegevens van de Werkgroep Zoetstoffen van de firma's Bayer en Kon. Zout Organon N.V.. zou 90 pet. van de zoetstoffen worden verwerkt in zoetstoftabletten en de resterende 10 pet. in levensmiddelen waarvan nog een groot deel in de dieetartikelen. Uit de gegevens van de genoemde firma's blijkt dat het consumptiepatroon in Nederland in 1968 als volgt is: Omgezet werden: zoetstoftabletten op basis van cyclamaat 158 ton zoetstoftabletten op basis van saccharine en cyclamaat 32 ton verbruik in andere vorm 25 ton totaal verbruik in cyclamaat

(Het „verbruik in andere vorm" betreft de toepassing als smaakcorrigens in farmaceutica en de toepassing in levensmiddelen volgens artikel Iquarter, lid 2, resp. artikel 9. lid 1. van het Algemeen Besluit Warenwet.) Wanneer de 25 ton cyclamaat, verbruikt in farmaceutica en levensmiddelen, door alle Nederlanders wordt geconsumeerd dan zou dit betekenen een dagelijkse opname van 5,7 mg cyclamaat per persoon. In bepaalde gevallen zal dit zeker enkele tientallen malen hoger zijn. bv. in het geval dat speciale diëten worden geconsumeerd. De opgenomen hoeveelheid zal echter ver onder de 1.5 of 3,0 g per dag blijven. Het gebruik van zoetstoftabletten is veel hoger, zoals uit de tabel blijkt namelijk totaal 190 ton. En hier is het niet mogelijk te stellen dat de gehele bevolking deze hoeveelheid consumeert (Wanneer dit wel het geval was dan zou dit betekenen een gemiddelde opname van 43 mg per dag per persoon). Bepaalde personen zullen echter geheel geen tabletjes gebruiken, terwijl anderen dit wel zullen doen. De verbruikers van zoetstoftabletten kunnen globaal in 2 groepen verdeeld worden, te weten diabetici en andere patiënten naast ,.gewichtsbewusten". Volgens schatting van de firma's Bayer en Kon. Zout Organon (dd. 17-3-1969) zouden diabetici gemiddeld 1,5 g cyclamaat per dag opnemen, terwijl het gemiddelde van ,.gewichtsbewusten" pl.m. 750 mg zou zijn. Gezien het feit dat uit de nieuwe gegevens blijkt dat de stof cancerogeen is voor ratten, zijn in Amerika de volgende beslissingen genomen: In het telexbericht van 20-10-1969 van mr. Craig (Abbott, Chicago) staat vermeld dat mr. R. H. Finch van het Department of Health, Education and Welfare heeft verklaard:" . . . . the use of cyclamate in the production general prupose foods and beverages be discontinued forthwith. As for general
5 purpose foods and beverages already produced, my act will pcrmit orderly withdrawal of the products from the market place. In the case of beverages which contain a higher level of cyclamates, we are requiring a recall to be completed by January 1ste 1970. All other artificially sweetened foods contain a lower level of cyclamates and pose a very minimal risk according to the advice I have from top scientists, both in and of the government. These products will be phased out of the market by February lst 1970. I should emphasize also that my order does not require the total disappearance from the market place of soft drinks, foods and non-prescription drugs containing cyclamates. These products will continue to be available to persons whose health depends upon them, such as those under medical care for such conditions as diabetes or obesity. I expect that in the future these products will be labeled as drugs, to be consumed on the advice of a physician." f Door dr. H. L. Ley jr. werd gesteld dat: „in accordance with the instructions of the secretary, with which I heartily concur, I have taken the following action. 1. Cyclamate will be removed from the gras list effective with publication in the federal register monday. 2. Artificial sweeteners containing cyclamate are being transferred to drugs status to permit their use under medical supervision. 3. General purpose food and beverages and drug products containing cyclamates will be treated in the following order. a. Production of such items will be required to be discontinued now. b. Beverages containing cyclamate, including packaged mixes, will be required to be removed from the market place by January lst, 1970. c. Other food products containing cyclamate will be required to be removed from the market by February lst, 1970. d. Drug products containing cyclamate will be required to be removed from the market by July lst, 1970.
Hoewel de details van de Amerikaanse onderzoekingen momenteel nog ontbreken, mag uit de recentelijk bekend geworden verklaring van het Amerikaanse departement van Volksgezondheid en de Food and Drug Administration, steunend op de beoordelingen van het wetenschappelijk onderzoek door het „National Cancer Institute", de „National Research Council" en de „National Academy of Sciences" in de Verenigde Staten, afgeleid worden dat cyclamaat na orale opname (in hoge doseringen) een cancerogene verbinding voor ratten is. Het feit dat tumoren bij ratten worden geïnduceerd, houdt niet in dat de stof ook cancerogeen is voor de mens. Uitgaande echter van het principe dat potentieel cancerogene stoffen niet in onze voeding aanwezig mogen zijn, betekent dit dat cyclamaat niet langer toelaatbaar is als toevoeging aan onze voeding, met inbegrip van de cyclamaat houdende zoetstoftabletten ter vervanging van suiker. Het hierbedoelde risico zal moeten worden afgewogen tegen de therapeutische voordelen bij de behandeling van bepaalde groepen patiënten. Nadere bestudering, bevestiging en uitbreiding van het verrichte onderzoek is noodzakelijk. Addendum Hieronder volgt een korte omschrijving van h u toxicologische onderzoek dat momenteel gaande is. (13) Urine and feces After a pretreatment of the sample, the cyclamate was determined in a methylene extract by a gas-chromatographic method. (21) Biochemical aspects Metabolism Cyclohexysulfamic acid probably resists metabolism because it is highly ionized at physiological pH values and may not penetrate the sites where it could be metabolized. (9) Calcium cyclamate had no sig. effect either in vivo or in vitro, in a concentration of 20 mg/ml, on the type or rate of growth of the microbiological flora of the stool. (8) Rat Because of the discrepancy between the reports of Miller (1966) and of Der.se and Daun (1966), (both reports are summarized in Joint FAO/ WHO report 1967) a study duplicating the work of Derse and Daun was carried out. Groups of ten 23 days-old male rats weighing 45-50 g each were placed on diets containing 5 calcium cyclamate for 22 weeks. Three-day cyclamate balance studies were perform ed at the eighth, fourteenth, and twenty-second week on two rats from each group. At the beginning of each three-day period, the rats were dosed orally with an aqueous solution of uniformly labeled 14 C-cyclamate. The excreta from each rat were collected over the three-day period and homogenized.
The homogenate was analysed by gaschromatography, according to the procedure of Derse and Daun, and by measurement of radioactivity. Results The average percent of cyclamate recovered in the homogenate of the feces of the three balance studies was for two dose groups: 94 pet. by GLC and 97 - 99 pet. by 14 C. So the elimination of cyclamate in the rat by the feces is nearly quantitative. The animals with 10 pet. cyclamate had lower weights. Unformed stools were observed at the onset of the study in 10 pet. group. Gradually these stools became formed, but still were very Ioose. Rats with 5 pet. had soft but formed stools. The adrenals in all groups were morphologically and histologically within normal limits (in contrast to the report of Nees and Derse, Nature 211 (1967), 1191, summarized in Joint FAO/WHO report (1967). (25) Toxicological data Oral Rat A litter mate, paired-feeding study was established on nutriënt conversion, growth, skeletal development, and carcass composition. Dosage levels were 0, 0,5, 1,0 and 2,0 pet. of calcium cyclamate. Length of testing period not mentioned. Results Growth and skeletal development were comparable among test and control groups. Food consumption was increased. No gastrointestinal effects of the type previously associated with cyclamate administration have been noted. (No detailed report at our disposal.) (3) Subcutaneous Rat Rats were injected subcutaneously with 0,5 ml of a 15 pet. aqueous solution of calcium cyclamate 3 times per week for 10 weeks. A connective tissue response was observed which became recognizably abnormal: collageneous bands and foei of astypical fibroblastic proliferation. (No detailed report available.) (24) Thirty rats were injected subcutaneously with 0,5 ml of a 15 pet. aqueous solution of calcium cyclamate 3 times per week for 98 weeks. Sarcomata developed in 14 out of 30 rats surviving until tumour-bearing age. Ten of these 14 sarcomata were succesfully transplanted into weanling rats. In contrast no sarcomata were found when sodium cyclamate was administered in the same way and in the same dosage as calcium cyclamate. (No detailed report available.) (24) Observations in man 28 men took daily for 14 days 7-12 g Ca-cyclamate in solution in soft drinks or partly in solution and partly in dry capsules. Increase in bowel movements of uncertain significance in 4 of 28 men were reported. The group means indicated no sign. effect on the frequency of bowel movements by any dose up to 12 g. Softer stools accompanied by an increase in mean stool weight in the 12 g group occurred in 12 of the 21 men. A combination of Ca-cyclemate and saccharin given daily in doses up to 4,9 g of Ca-cyclamate and 0,5 g of saccharin had no effect on the mean gastrointestinal motility in 25 men.
7 One man did have a sign. incrcasc in bowel frequency. The consistency of the stool was unchanged. (8) 28 men took daily for 21 days 5-7 g of Ca-cyclamate in solution in soft drinks or part in solution and part dry in capsules. Tests on blood (erythrocytes leucocytes) and urine (specific gravity, glucose, albumen, sediment) gave normal results during this period, except one man, who started the experiment with 2+ glucose in his urine; his urine became negative for glucose during the period of reduced sugar intake. One man showed a sign. increase in frequency of bowel movements as the intake was increased to 7 g per day. (8) A man took daily six packages containing calcium saccharine and calcium cyclamate. When he was awake he got a bad taste in his mouth and shortly after intake there was a diuretic effect present. When awakening for reason of nocturia there was an unsteadiness and swaying, but his way back to bed was normal. Upon arising the next morning there were no ill effects save the bad taste. Systemic pruritis, small patches of a fine pinheaded rash especially on the elbow. and itch on these rashes were observed. After a period of abstinence hee took half the dose. The sweetening was reduced beyond expectations, diuresis still obvious. There was some itching and very faint rash in small areas. No bad taste and no imbalance. Then a package at each meal was taken. After 4 weeks a fluttering in the left ear canal took place. He stopped the treatment and after 5 days the ear symptoms disappeared. An occasional mild itch and a slightly receding induration are disappearing. (10) Special investigations Photosensitization Human A 40-year-old negro woman drank daily for about 2 months six ca 350 gram cans of diet soda. She showed burnings and pruritic eruptions on the skin, where she had been exposed to the sun. An experiment was started with the same woman: a) Influence of the sun After 2 weeks abstinence from diet soda the woman took daily 72 tablets of Ca-cyclamate saccharin combination (equiv. to the amount of cyclamate and saccharin in six ca 350 gram cans of diet soda) in 6 equal doses. After 4 days dermatitis was clearly seen and 1 month after treatment was ended she showed postinflammatory hypopigmentation. After about half a year she was entirely asymptomatic and there were only a few hypopigmented maculae left. b) Influence of a quartz lamp The woman was exposed to a hot quartz lamp with and without window glassfiltcr while she was taking diet soda and suffering from dermatitis. No diminished minimal erythema dose was demonstrated, nor was there any abnormal reaction to a delayed erythema dose (10 x min. eryth. dose) even when this dose was repeated 3 x at the same site several days apart. c) Influence of a xenon are lamp (200 Watt xenon are with KB-1 and infrared blocking filters: range 295-420 nanometers and approximating summer sun at an angle of 60°). Baseline studies were performed on the same woman after 3 months of abstinence from artificial sweeteners and on several normal negro women using the inner forearm. The symptoms were for 30 seconds exposure - barely perceptible erythema: after 24 hours maximum erythema; after 48 hours — erythema disappeared. The 45- and 60-second exposure sites also reddened and erythema disappeared within 48 hours without residual pigmentation. Then the test woman took in 6 equal doses 72 tablets of the Ca-cyclamate-saccharin combination daily far 4 days. Distinct differences with the baseline studies appeared on all exposed sites (30 seconds and longcr). Within 24 hours distinct indu-
ration appeared lasting for about one week. Within 48 hours minute papules appeared, lasting also for one week, in which the exposed spots seemed eczematous. The erythema seen at 24 hours was replaced slowly during this week by a brownish pigmentation which faded slowly, but still could be seen at the 60-second site one month after exposure. Separate experiments with Ca-cyclamate or saccharin were carried out. 360 mg/day of saccharin alone for 4 days caused no other erythema than was seen in the baseline studies. 3600 mg/day (equiv. to six ca 350 gram cans of diet soda) caused the same unusual eczematous reaction and subsequent prolonged pigmentation as with the ingestion of the combination of Ca-cyclamate and saccharin. The auther (a white man) used himself also as an experimental subject and treated himself in the same manner as the negress. No change in minimal erythema dose occurred and no induration, papulation or pigmentation appeared. In these tests no other side-effects were noted. (20) Sodium Cyclamate Manufacture The most common method of synthesis is the reaction of sulfuric chlorhydrin on cyclohexylamine. Then the cyclohexylammoniumsalt of sufuric chlorhydrin in formed and transformed into sodium cyclamate. (17) Physical and chemical properties Solubility in grams/ 100 ml solvent at 25° C. After extraction of the amine with chloroform, an alcoholic solution of 1,4-benzoquinone is added and this mixture is maintained at a temperature of 50° C during 2 hours. Cyclohexylamino-1,4 -benzoquinone is formed. This method permits colorimetric determination of concentrations below 0.025 pet. (18) After reduction of sulfamate by means of a 10 pet. sodiumnitrite solution the sulfate is determined by means of a 10 pet. bariumchloridesolution. 1 mg bariumsulfate ~ 0,8621 mg sodiumcyclamate. One hour before the test and 1, 3 and 5 hrs after administration blood (in plasma: total base, bicarbonate, Na+, Ca++, K+ and Mg+ + ) was examined. No sign. changes were observed. No details available. (15) Subcutaneous Rabbit Rabbits were given 250 mg/kg of sodium cyclamate orally and subcutaneously. Subacute toxicity Oral Rat A group of 8 male and 8 female rats (ca 110 g each) received by gastric intubation 1 g/kg of a mixture of sodium cyclamate and sodium saccharin (10: 1) in a dose volume of 10 ml water for 16 days. A control group of 3 rats of each sex was treated in the same way with distilled water. Survival, food consumption and growth were recorded daily. Results Small depression in weight gain but no untoward reactions were observed. Necropsy of the test animals after 16 days showed no gross lesions. (28) Groups of 14 male and 14 female rats (75 - 100 g) received diets with 0, 0,5 pet. sodium cyclamate or 0,5 pet. sodium cyclamate/sodium saccharin (10 : 1) mixture for 38 days. Survival, behaviour, general physical appearance and stoolconditions were recorded daily. On day 39, the surviving animals were killed and sections of renal and hepatic tissues were examined microscopically. Results Marked reduction in growth rate and food intake in both test groups, but more in cyclamate group than in the mixture group. Effect of food utilization was comparable among groups, as was survival. Some diarrhoea was noted in all groups. A number of inflammatory and hydropic lesions in the liver and kidneys of the rats of both test groups was found. These may have been associated either with the physical effects of diarrhoea and
9 waterloss or with same endemic disease in the rats. Three rats in each of the groups had died during the test period. (28) Rabbit A group of 6 rabbits (3-5 kg) received drinking water with 2 pet. sodium cyclamate (1,2 mg/kg bodyweight) for 4 weeks. Then they received for 3 weeks normal drinking water foliowed by a period of 4 weeks with 2 pet. cyclamate-drinking water. Before the test and after 4 and 11 weeks determinations of blood clotting are made (thrombocytes, heparinoid-tolerance test, partial thromboplastine-time, thrombine-time, prothrombine, prothrombine-time factor V, factor VII, serum prothrombine, fibriniogen, thrombelastogram = TEG (r, K, Ma), thrombine generationtest, platelet factor 3. Results The prothrombine and factor VII were decreased where as the r- and K-time of TEG were lengthened. The platelet factor 3 was markedly decreased. These changes were most evident in the second period of cyclamate administration after 3 weeks pause. (16) Rabbit A group of 6 rabbits (3-4 kg) received drinking water with 3-9 mg Marcoumar©for several weeks. During the last weeks also 0,5 pet. sodium cyclamate was added to the water. A marked decrease of the prothrombine-time was found. As 0,5 pet. sodium cyclamate alone does not cause this effect, pretreatment with Marcoumar has a cumulative effect. (16) Subcutane Rat Rats were injected subcutaneously with 0,5 ml of a 15 pet. aqueous solution of sodium cyclamate 3 times per week for 10 weeks. A connective tissue response due to the injected agent was found. (24) Semicronic toxicity Oral Rat Two groups of 16 male and 16 female rats received diets containing 0,5 or 2 pet. of a sodium cyclamate/sodium saccharine (10 : 1) mixture ad libitum for 790 days. A control group of 8 rats of each sex received only the basal ration. Food consumption was determined weekly. Records were also kept of survival, general appearance, behaviour efficiency of food utilization, body weights (3 times a week for the initial 15 weeks and weekly thereafter), haemograms (at week 16 and 27) and gross and microscopic examination of visceral tissues in a group of animals killed after week 16 and in the remainder killed at termination after 27 weeks on the test diet. Results Two rats of the 0,5 pet and four of the 2 pet group failed to survive the study, with deaths in the low dose group occurring on days 29 and 87 and in the high dose group on days 22, 105, 121 and 176. Necropsy of these animals revealed that death was due to debilitation and some form of respiratory disease. In all other respects, the majority of the animals in both testgroups were normal. One rat in the 0,5 pet. group and two in the 2 pet. group evidenced some diarrhoea, but this lasted only for 24-48 hrs and regressed spontaneously. Growth and blood picture (Hb, Leu, Lymph./neutroph. ratio) were normal. No gross or microscopic changes were seen in the liver, kidney, gonads, intestines, stomach or pancreas. (28)
Oral Guinea pig Number of animals: three groups (A, B and C) of 7 c? animals (approx. 500 g) each. Dosage group A: 2 pet. Na-cyclamate in the drinkingwater, ad libitum, (water intake 30 ml/animal). group B: 0,5 pet. Na-cyclamate in the drinkingwater: 30 ml/24 hrs/animal. group C: 0,3 pet. NaCl in the drinkingwater: 30 ml/24 hrs/ animal. Testingperiod: approx. 120 days. Every time an animal died it was replaced. Examinations made: blood (on 4 animals of groups A and B each and on 6 animals of group C; alkaline phosphatase, SGPT, LDH), Microscopic examination (liver in 4 animals of group A and B). Abnomalities: SGPT and LDH values of groups A and B were clearly increassed and SAP lcvels were slightly increased. In the livers of group A brightened livercells, vacuolization of the protoplasma and local necrosis were seen. In group B the same alteration in the liver were seen but to a minor extent. (16) Subcutaneous Rat Rats were injected subcutaneously with 0,5 ml of a 15 pet. aqueous solution of sodium cyclamate 3 times per week for 98 weeks. No sarcomata were found. (In contrast calcium cyclamate caused sarcomata) (24) Reproduction studies Rat Groups of 6 pups (first litter of Fi) the folIowing number of animals were available: Control 5<J and 9 ? ; 0,8 g group: 5 <J and 0 9; 1,6 g group; 3
10 it was noticed that the fat was more fluid. Two animals from 30 000 ppm group showed atrophy of testis. Microscopic examination revealed hypoplasia of the seminiferous epithelium with localised disappearance of seminiferous cells. In some tubules calcium deposits were found. There was also hyperplasia of the interstitial cells. During the test period the two animals with 30 000 ppm showed loss of hair when their weight was 57 and 77 g. The kidneys of these animals showed a slight granular degeneration of the epithelium of the tubuli contorti and a rich yellow-orange pigmentation after coloration. Summarising it can be said that sodium cyclamate in the used dosages, has a marked effect on the development of the suckling pups. Their mortality and growth are affected. In the 30 000 ppm group marked alterations in the testis were found both by macroscopical and microscopical examination. Rat Groups of 35
Rat Groups of 20-24 femal rats were given daily doses of resp. 0, 50, 100 and 250 mg/kg of sodium cyclamate in 5 ml/kg of water by stomach tube from the sixth to fifteenth day of pregnancy. As regards the litter size, the resorption rate, and the mean weight of foetuses, there were no significant differences. No malformations occurred in the testgroups. Assessment of the developing skeleton (Alizarin Red S staining) did not reveal significant deviations from the controls. (26) Observations in man Oral 5 subjects, who had already been used in an earlier experiment (Leaky et al. Fd. Cosmet. Toxicol. 5 (1967) 447; incorporated in Toxicological Evaluation of some Flavouring Substances and non-nutritive sweetening agents. F.A.O. Nutrition Meetings Report Series No. 44 A, WHO (Food Add/68. 33, 1967), received 1 gr sodium cyclamate daily for 17 days. 24-hr samples were collected at 3, 10 and 17 days after the first dose. Only one subject excreted cyclohexylamine in amounts that appeared to increase with length of time on dose (this subject was the only one who showed positive results in the previous study). The equivalent of 0,7, 2,3 and 7,5 pet. of the daily dose of cyclohexylamine, appeared in the urine on day 3, 10 and 17 resp. The presence of dicyclohexylamine could not be detected in the urine. In the faeces prior to and after the experiment cyclohexylamine was not detected. There was no in vitro evidence that sodium cyclamate could be metabolised to cyclohexylamine by faecal bacteria. (14) 26 men and 9 women received single oral daily doses of 1 gr sodium cyclamate. Urine was collected for 24 hrs before the first dose and thereafter for three successive 24 hr periods after the first dose. Four subjects (2 $ and 2

Rernark concerning the ADI of the WHO The work of warden (no report available shows that people who uscd cyclamates in the recommended doses (0-50 mg/kg/ day = ADI temporaly set in Geneva) soon excreted cyclohexylamine in quantities varying from 0,8 - 7,5 pet. of the ingested amount of cyclamate. (4) Toxicology Biochemical aspects Metabolism Rabbits Cyclohexylamine, when orally administered to rabbits at a dose of 0,2 g/kg, gives rise to unchanged cyclohexylamine and N-hydroxycyclohexylamine in the urine. Rabbits received orally 0,17 g/kg 14C-labeled cyclohexylamine. 68 pet of the radioactivity could be recovered from the urine in 60 hrs; 13 pet. in the first 10 hrs; 39 pet. in the next 20 hrs, and 16 pet. in the last 30 hrs. The excretion curve showed a small peak at 3 hrs. and a second larger peak at 32 hrs. A small amount (0,5 pet.) was found in the breath (0,3 pet. as 14C02 and 0,2 pet. as ethanolsoluble material). 45 pet. of the administered dose was shown to be excreted in the urine as unconjugated cyclohexylamine, 0,2 pet. as N-hydroxycyclohexylamine, in conjugated form, and 2,5 pet. as cyclohexanone oxim, which is probably and artifact arising from the glucuronide of N-hydroxycyclohexylamine in the hydrolysis procedure. Thin-laer chromatography of all fractions of the radioactive urine reveated 3 metabolites with Rf values resp. 0,53, 0,37 and 0,59. The last one is corresponding in position to cyclohexylamine. (13) Dog One dog received during 16 days oral doses of 80 mg/kg bodyweight/day cyclohexylamine as the hydrochloride salt dissolved in water. In the urine of this dog neither aniline nor oxyaniline or other metabolites were found. (12) One dog received during 7 days subcutaneous injections of 125 mg/kg bodyweight/day cyclohexylamine as the hydrochloride salt dissolved in water. In the urine of the dag no unchanged cyclohexylamine nor aniline or oxyaniline were found. (12) Rat, dog, human Tests were carried out with uniformly labeled 14C-cyclohexylamine . The results in 4 rats and 2 dogs suggest cyclohexylamine is excreted almost totally in the urine within 24 hours and that the major portion of this is unchanged cyclohexylamine. From 11-27 pet. of the recovered dose was excreted as a metabolite. Further studies revealed that in one human subject greater than 98 pet. of the recovered dose of 14C-cyclohexylamine was excreted unchanged. The half life in plasma in the rat, dog and man was found to be about 2,5, 3 and 4 hours resp. Tissue distribution of cyclohexylamine at equilibrium in rats shows that cyclohexylamine readily penetrates the tissues. The highest concentrations were found in the lungs, spleen, liver
and G.I. tract. The calculated apparent volume of distribution corrected for plasma protein binding (8 pet. which is bound) was 2,7 1/kg. (11) Toxkological data Dermal Laboratory animals A 100 pet. solution of cyclohexylamine is placed by patch on the skin of the back and remained for 48 hours. The material is reapplied to the skin af ter a waiting period of 14 days. Cyclohexylamine causes a severe irritating effect and a moderate sensitizing effect. (3) Human A 25 pet. solution of cyclohexylamine is placed by patch on the skin of the back and remains for 48 hours. The material is reapplied to the skin after a waiting period of 14 days. Cyclohexylamine causes a servere irritating effect and a slight sensitizing effect. (3) Inhalation Max. permissible concentration for vapors of cyclohexylamine is 0.001 mg/l. (6) Intraperitoneal Laboratory animals LD.,» is 200 mg/kg. Cyclohexylamine is highly toxic and causes severe shock. Brains, liver and kidney show pathological changes. (3) Rat Male rats received daily for five days injections with different doses of cyclohexylamine into the abdominal cavity. Twenty-four hours after te last injection chromosomes of the rats' germ cells and bone marrow cells were studied for the incidence of breaks over and above the control level (1-2 pet.). 10 mg/kg body weight produced chromosome breaks in 3.2 pet. of the germinal cells. With 50 mg/kg body weight 16,5 pet. of germ cell chromosomes showed damage. This dose gave 12,5 pet. breaks among bone marrow chromosomes. (16) (17) Pharmacology In vitro Cyclohexylamine is toxic to HeLa tissue cells in concentrations of 0,015 pet. (7) In vivo Cat. The pharmacological actions of cyclohexylamine on the autonomie nervous system were tested using the cats' blood pressure and nictitating membrane. Cyclohexylamine (5 mg/ kg) had no effect upon the adrenergic cholinergic or histaminic responses but demonstrated a pressor action. This pressor activity was blocked bij the a-receptor blocking agent phenoxybenzamine. Prior treatment of the animals with reserpine abolished the cyclohexylamine pressor response, which could then be restored with norepinephrine infusion. Thus, cyclohexylamine appeared tot be an indirect acting sympathicomimetic amine which closely resembles tyramine. Pretreatment of animals with the monoamine oxidase inhibitor pargyline (25 mg/kg) resulted in a pontentiation of the tyramine responses tested. This same treatment did not alter the pressor responce of cyclohexylamine. This observed lack
13 of interaction was most likely due to the fact that cyclohexyU amine is not a substrate of monoamine oxidase as is tyramine. (2) Cat 0,4 mg/kg cyclohexylamine was injected in cats. CycIohexylamine produced a significant increase in heart frequency and a slight increase of blood pressure. A very long time of effect is observed. (5) Strong sympathicomimetic effects can be seen in the cat after administration of cyclohexylamine i.v. (3,7 mg/kg) as well as enterally though only at large doses of 100 mg/kg. Extirpation of the adrenal glands in cats had no influence on the effects caused by i.v. injected cyclohexylamine. So it is proved that cyclohexylamine does not work "über freigesetztes Epinephrin". Fretreating the animals with cocaïne caused a remarkable loss of the effects of i.v. injected cyclohexylamine. A similar inhibition was observed after pretreatment with reserpine but the exogenous supply of norepinephrine may largely reinstall the effects of cyclohexylamine. In normal animals and even more so in animals treated with reserpine cyclohexylamine enhances the effect of norcpinephrine supplied exogenously. From these findings it is concluded that cyclohexylamine has to be classified with the indirectly acting neuro-sympathicomimetics. (9) Guinea pig, rabbit, cat Intravenous injection of 1,0, 3,0 and 10,0 mg/kg cyclohexylamine in cats caused a pressor response and tachycardia of approximately 20 minutes duration in anesthetized, bilaterally vagotomized cats. In cats under artificial respiration with opened chests, cyclohexylamine produced an increase in myorcardial contractile force. This positive inotropir effect could also be demonstrated in isolated perfused rat and guinea pig hearts and in isolated atria of the rabbit. Higher doses of cyclohexylamine caused a vasode pressor effect accompanied by brachycardia and negative inotropism. Cyclohexylamine probably produces its cardiovascular effects through release of endogenous catecholamines as well as by direct stimulation of a-adrenergic receptors. (1) (10) Rat, guinea pig, cat When cyclohexylamine is given i.v. in the form of ammoniumchloride, in amounts that are produced endogenously from cyclamate in humans, an increase of the arterial pressure and the pulse rate are observed in rats, guinea pigs and cats. Investigations in spinal cats, in cats pretreated with azomethionium, phentolamine and l-(3-methylphenoxy)-hydroxypropyl-3—isopropyl-ammonium-hydrochloride (Kö 592), and investigations on the nictitating membrane have shown that cyclohexylamine belongs to the sympathicomimetic substances. The behavior of the glucose and the electrolyte metabolism confirm that classification. Cyclohexylamine is less comparable to epinephrine, than to norepinephrine. Onderzoek opdracht (Projekt nr. 50 Tox) met betrekking tot de toelaatbaarheid van het gebruik van zoetstoffen in de voeding Aanvullend toxicologisch onderzoek opgedragen door de heer Staatssecretaris van Sociale Zaken en Volksgezondheid, dr. R. J. H. Kruisinga aan de directeur-generaal van hei Rijks Instituut voor de Volksgezondheid, met betrekking tot de toxiciteit van kunstmatige zoetstoffen. In het onderzoek (Projekt nr. 50 Tox) zal de toxiciteit van cyclamaat, saccharine en een mengsel van cyclamaat en saccharine verder fundamenteel onderzocht worden. Hierbij zal in het bijzonder aandacht worden besteed aan de carcinogene en teratogene werking van zoetstoffen. De mogelijkheid van cumulatief effect van saccharine en cyclamaat zal hierbij in het bijzonder moeten worden nagegaan.
Nadere specificatie en projektbeschrijving zullen plaatsvinden in overleg tussen de hoofdinspecteur van het Staatstoezicht op de Volksgezondheid, belast met het toezicht op levensmiddelen en de keuring van waren en de directeur-generaal van het Rijks Instituut voor de Volksgezondheid. De specificatie en de projektbeschrijving van bovenbedoeld onderzoek dienen uiterlijk 10 november aan dr. Kruisinga ter goedkeuring te worden voorgelegd. Interimrapportering omtrent het onderzoek aan de Staatssecretaris zal uiterlijk 15 februari 1970 dienen plaats te vinden.
