HagaScoop
In samenwerking met de universitaire medische centra vervult het HagaZiekenhuis een belangrijke rol in toegepast medisch wetenschappelijk onderzoek. Gezien de aard van de patiëntenpopulaties (veelal anders dan die van de universitaire medische centra) is het HagaZiekenhuis bij uitstek geschikt voor participatie in grootschalig multicenter onderzoek en medical technology assessment (MTA) en is er gelegenheid voor promotieonderzoek van arts-assistenten en specialisten. In dit abstractenboek staan samenvattingen van wetenschappelijke onderzoeken beschreven van diverse specialismen. Het boek is tot stand gekomen dankzij de bijdragen van specialisten, arts assistenten, paramedici, stafmedewerkers en gespecialiseerd verpleegkundigen van 23 afdelingen van het HagaZiekenhuis.

‘Wetenschap verbindt’ We mogen gerust zeggen dat wetenschap leeft in het HagaZiekenhuis: in dit abstractenboek staan de wetenschappelijke bevindingen gepubliceerd van artsonderzoekers, paramedici, verpleegkundigen, beleids medewerkers en studenten die in het HagaZiekenhuis werken. Het boek dat meer dan 80 pagina’s bevat, wordt uitgereikt tijdens de jaarlijkse Wetenschapsdag van het HagaZiekenhuis.
Haga Wetenschapsdag Het is alweer de 38e keer dat de Wetenschapsdag van het HagaZiekenhuis wordt gehouden. Door menig onderzoeker wordt deze dag bestempeld als een bijzondere en stimulerende dag en wordt het als een belangrijk kanaal gezien om ideeën en informatie uit te wisselen. Naast de kennisuitwisseling is de wetenschapsdag ook bedoeld om aan elkaar te laten zien wat we aan wetenschappelijke activiteiten ontplooien en dat uit te dragen, bijvoorbeeld via posterpresentaties, voordrachten en publicaties. Het is een kans om elkaar te stimuleren en -waar mogelijkeen gemeenschappelijk component te vinden. Deze ‘kruisbestuiving of verbinding’ kan leiden tot een creatieve samenwerking. De wetenschapsdag is tevens een stimulans voor jonge onderzoekers om hun eerste onderzoeksresultaten te presenteren.
Onderzoek blijven stimuleren Het Wetenschapsbureau, onderdeel van het opleidings instituut de HagaAcademie, faciliteert en stimuleert wetenschappelijk onderzoek op een hoogwaardige en laagdrempelige wijze. Het is gericht op de ontwikkeling van wetenschap en op de uitvoering van toegepast onderzoek. Dat gebeurt door onderzoekers te begeleiden en te adviseren. Maar ook door de verbinding te zoeken tussen de onderzoekers binnen het HagaZiekenhuis. Dit alles met het beoogde resultaat om meer en beter onderzoek te kunnen ontwikkelen.

Dr. J.L. Chanfleury van IJsselsteinprijs, Prijs beste abstract en Afdelingsprijs De wetenschappelijke stafdag voor specialisten, arts assistenten, coassistenten, paramedici en verpleegkundigen is bedoeld om met elkaar te delen wat er aan onderzoek wordt gedaan, met elkaar in contact te komen en meer kennis op te doen. Dr. J.L. Chanfleury van IJsselsteinprijs Tijdens de jaarlijkse stafdag wordt de Dr. J.L. Chanfleury van IJsselsteinprijs uitgereikt. Het is een stimuleringsprijs die wordt gegeven aan de AIOS die in dat jaar het beste gepresteerd heeft met zijn/haar onderzoek. Dr. J.L. Chanfleury van IJsselstein (1819-1905) was de eerste hoogleraar in de DermatologieVenerologie in de periode 1867-1883 in Nederland. Hij koos voor Groningen als universi teitsstad. Hij promoveerde in 1844 tot doctor medicinae en in 1845 tot praktiserend arts. Hij vervolgde zijn opleiding in Parijs en Brussel en specialiseerde zich in de venereologie en urologie. In 1851 aanvaardde Van IJsselstein een functie als geneesheer in het Gemeenteziekenhuis in Den Haag die hij in 1865 uitbreidde met het directeurschap. In de tussentijd publiceerde hij veel over huidziekten en hun behandelingen. In 1867 nam hij afscheid als geneesheerdirecteur en bekleedde hij het hoogleraarschap in de dermato - venereologie. De eerste leerstoel als zodanig in Nederland. Hij combineerde dit professoraat tot 1883 met het voeren van een bloeiende praktijk voor huidziekten. Van IJsselstein overleed in 1905. De stichting HagaVrienden draagt het initiatief voor de prijs een warm hart toe en stelt hiervoor jaarlijks een geldbedrag ter beschikking.
Prijs beste abstract Deze prijs wordt jaarlijks uitgereikt. Zes onderzoekers zijn uitgenodigd hun abstract tijdens een posterwalk te presenteren. De prijswinnaar bevindt zich onder hen.
Afdelingsprijs De afdelingsprijs gaat naar het specialisme die dit jaar op grond van de ingestuurde posters wetenschappelijk kwalitatief het best gepresteerd heeft. Daarbij gaat het met name om onderzoek dat in het HagaZiekenhuis verricht is.

Influence of inhalation mode on aerosol lung deposition in patients with Cystic Fibrosis: a pharmacokinetic approach Background
Results
The therapeutic effect of inhaled antibiotics on lung infection in patients with Cystic Fibrosis (CF) is dependent on the aerosol deposition achieved in the lungs. It is assumed that slow and deep inhalation (TIM) of aerosolized fluid provides higher lung deposition compared to normal tidal breathing (TBM).
Mean Cmax and F were significantly increased for TIM compared to TBM. Frel was 1 or greater for all patients (mean=1.48) and mean Frel was significant higher than the value of 1 (p<0.0001, 95%CI=1.29-1.68). No interaction effects between inhalation mode and subgroup existed.
Objective To investigate the influence of two different inhalation modes (TIM and TBM) on tobramycin aerosol lung deposition in CF patients using pharmacokinetic parameters as a representation of lung deposition.
Methods Randomized, open-label, cross-over study of 18 adult CF patients. According to their lung function (%FEV1), patients were categorized in three subgroups. Each patient inhaled a tobramycin solution twice in random order: once in TBM and the other time in TIM mode. Blood samples were collected in order to model tobramycin pharmacokinetics. Individual pharmaco kinetic parameters were calculated: maximum serum level (Cmax) and bio availability (F). Relative bioavailability (Frel), defined as the ratio of FTIM to FTBM, was used to demonstrate relative change in lung deposition following TIM versus TBM inhalation.

Conclusion Slow and deep inhalation of aerosolized tobramycin resulted in a mean 48% higher lung deposition compared to normal tidal breathing. This result was independent from lung function category, which suggests that regardless the disease state of the CF patient, slow and deep inhalation always results in higher aerosol lung deposition compared to normal tidal breathing.

Topklinische diabeteszorg in het HagaZiekenhuis - voortgang
Achtergrond N.a.v. metingen in 2011, die bewezen dat gemiddeld slechts 15% van de in huis opgenomen patiënten met Diabetes Mellitus bekend waren bij de diabetesverpleegkundigen, en andere knelpunten in de zorg voor deze patiënten (zoals onbekendheid met de regiefunctie van Interne Geneeskunde voor DM-patiënten), zijn een aantal verbeteracties ingezet. We presenteren de verbeteracties en laten d.m.v. een toetsing zien dat de kwaliteit van zorg is vergroot.
Wat is kwaliteit? Het Amerikaanse Institute of Medicine (IOM) hanteert de volgende definitie: 'Quality of care is the degree to which health services for individuals and populations increase the likelihood of desired health outcomes and are consistent with current professional knowledge'1.
Voor Topklinische diabeteszorg kunnen we ook kwaliteitsdomeinen onderscheiden waarin juist de inzet van het diabetesteam dat doel bereikt (tabel 1)2.
Topklinische diabeteszorg Om de kwaliteit van de zorg te vergroten zijn een groot aantal activiteiten verricht. In tabel 2 een overzicht van die activiteiten met het kwaliteitsdomein.
Toetsing
opnieuw vastgesteld welk percentage patiënten onbekend is gebleven. Het percentage bekende Insulinegebruikende patiënten is gestegen van 34 naar 59%. Het percentage bekende orale middelengebruikende patiënten is gestegen van 0 naar 17%. Als nulmeting is destijds gemeten hoeveel klinische patiënten die orale diabetes medicatie of insuline gebruiken ook aangemeld zijn bij het diabetesteam, door het vergelijken van de lijst aangemelde patiënten met een lijst uit het Elektronisch Voorschrijf Systeem.

De thermometer in het VeiligheidsManagementSysteem (VMS) Achtergrond
Vraagstelling
Als onderdeel van het landelijke Veiligheidsprogramma zijn er 11 landelijke thema´s vastgesteld die gezamenlijk binnen ziekenhuizen de meeste onbedoelde schade aan patiënten veroorzaken. De verwachting van het landelijke programma was dat als de beschreven preventieve maatregelen genomen zouden worden de onbedoelde schade aan patiënten met 50% zou afnemen. Het HagaZiekenhuis heeft de thema´s omarmt en per thema is er een thema-eigenaar benoemd. De thema eigenaren zijn, vaak met een werkgroep van experts, met de thema´s aan de slag gegaan.
In hoeverre voldoet het HagaZiekenhuis in 2013 aantoonbaar aan de doelstellingen die per VMS/thema (tabel 1) zijn vast gesteld?
De directe aanleiding van dit onderzoek is het geplande bezoek van de NIAZ op 2 oktober om verdere vordering vast te stellen van de implementatie van de 11 VMS thema´s.

Conventional extracorporeal circulation circuits are commonly used to perform surgical coronary artery bypass grafting. However, these circuits are associated with a well-known variety of deleterious side effects leading to significant postoperative morbidity and mortality. The development of minimal extracorporeal circuits with substantial reduction of blood contact surface area led to reduced blood trans fusion, reduced inflammatory response and reduced organ injury. However, these studies are performed on patients undergoing elective isolated coronary arterial bypass surgery. Emergency patients are
more compromised and could benefit even more of the use of these miniaturized systems. Emergency coronary arterial bypass surgery is defined as survival intervention within 24 hours after coronary angiographic diagnosis or acute coronary event.

In vivo performance of two new continuous invasive extracorporeal saturation monitors Introduction

To maintain adequate perfusion a continuous inline device to monitor venous saturation and hematocrit may be utilized during extracorporeal circulation. The aim of this study was to compare the clinical performance of two continuous inline venous saturation and hematocrit monitors, the Datamaster and the BMU 40 with our routine laboratory blood gas analyzer the Rapidlab 865.
Before calibration the venous saturation values from the Datamaster as well as the values from the BMU 40 were rather consistent with the Rapidlab 865. After calibration the venous saturation values from the Datamaster were largely consistent with the Rapidlab 865, whereas values of the BMU 40 were rather incon sistent with the Rapidlab 865.(Figure 1a.)
Further development of inline monitors is warranted to improve the range for agreement as compared to the “Gold standard” the RAPID lab 865.
Thirty consecutive patients undergoing open heart surgery were randomized into two groups, the Datamaster or BMU 40. Venous blood gas samples were collected at the beginning of bypass, and 30 minutes later. Simultaneously with blood sampling, data was stored. The monitor was calibrated to the values obtained from the first sample of the blood gas analyzer.
Before calibration the hematocrit values from the Datamaster were unreliable (Figure 1b.). After calibration the hematocrit values were largely consistent with the Rapidlab 865. The hematocrit values from the BMU 40 were both before and after calibration largely consistent with the Rapidlab 865.

Computing Shortest Paths in Non-Invasive Imaging of Cardiac Activation Purpose
Results
Body Surface Potential Mapping (BSPM) is an emerging clinical mapping tool for guiding ablation. Current BSPM techniques calculate shortest paths (SP) to simulate the propagation of an excitation wave in a 3D heart model consisting of a grid of nodes and interconnecting spines. The major limitation of contemporary SP algorithms e.g. Dijkstra’s algorithm is the fact that simulated paths are confined to intersect all near nodes of the mesh (figure 1). Therefore, the true distance will always be overestimated. The aim of this study was to introduce the Line of Sight (LoS) algorithm rectifying this constraint and to demon strate its significance in a simulation study.
The LoS algorithm enabled the compu tation of pathways without having to intersect all nodes or travel along the splines (figure 2). LoS was also applicable to spherical models by flattening a suitable subset of the triangulated surface (figure 3 and 4). In this simulation study, the length of the SP was overestimated by a factor √2 when using Dijkstra’s algorithm. This means that travelling under a 45-degree angle may reduce the total travelling time by 40% compared to travelling along the edges of a mesh.
Method A simplified triangular 2D model was used to compare the conventional Dijkstra algorithm with the LoS algorithm.

Conclusion SP algorithms that use rectilinear pathways may not be able to identify the true SP in a mesh. Application of the LOS algorithm may improve the physical validity of the simulations and could increase the clinical value of BSPM.

Redo Catheter Ablation of Atrial Fibrillation – CMR Guided Noninvasive Identification of Electrical Gaps Purpose
Methods
Catheter ablation for atrial fibrillation (AF) is an established non-pharmacological therapy and has undergone rapid techno logical development. The majority of patients require multiple ablation proce dures. In addition, it is unclear which patients require only pulmonary vein isolation and which patients need additional modifications such as creation of linear lesions.
Patients scheduled for redo catheter ablation underwent 3D LGE examination using a 1.5 T scanner. Custom software was implemented to segment these images and generate scarmaps.
Late gadolinium enhanced cardiac magnetic resonance (LGE) is the gold standard for non-invasive tissue characterization. Recent progress in LGE imaging sequences enables identification and quantification of structural remodeling in cardiac tissue as thin as the atrial wall. These images, post processed with semi-automated algorithms, generate a projection of tissue characteristic information on an anatomical shell (scarmap). This may be used to identify potential electrical gaps and design patient specific ablation strategies. In addition, the scarmap may be fused with electro-anatomical mapping systems (EAM) to guide ablative therapy.

Results A scarmap was created in 5 patients. Figure 1 shows the workflow in a patient with previous ablation; a. EAM with sites of ablation, b. LGE demonstrated significant areas with non-ablated tissue, c. Scarmap with scar depicted as red. During the redo procedures, the area’s that required ablation corresponded with gaps identified by LGE.

Conclusion These preliminary data strongly suggest that 3D LGE can identify electrical gaps and the subsequent 3D scarmap can be fused with EAM to guide ablation.

Long-term clinical outcome of staged versus immediate complete percutaneous revascularization after primary pci for stemi in patients with multivessel disease Purpose
Results
To compare a 1-stage PCI to a 2-stage PCI strategy for complete revascularization in STEMI patients with Multivessel Disease (MVD).
Patients in the 1-stage PCI group were older (60±11yrs. vs. 65±15 yrs.; p=0.001), had less 3-vessel disease (52% vs. 33%, p=0.01) and TIMI 2-3 flow post-procedure (99% vs. 93%; p=0.02). Multivariate analysis for MACCE demonstrated the 1-stage PCI group as an independent predictor (HR 0.22; 95% CI 0.07-0.73, p= 0.01) and revealed an interaction between treatment strategy and gender (p=0.03). Compared to 2-stage PCI, women had a higher MACCE rate with 1-stage PCI (HR 5.43; 95% CI 1.02-28.94, p=0.048), whereas men had a lower MACCE rate with 1-stage PCI (HR 0.18; 95% CI 0.04-0.98, p=0.048). No significant difference was demonstrated between the groups for mortality.
Method Consecutive patients with STEMI and MVD (n=1119), presenting for primary PCI (PPCI) between January 2006 and January 2010, without shock or previous CABG, were retrospectively included (n= 832); 61 patients had PCI for all significant stenoses (1-stage PCI; 65±15 yrs., 18% women) and 123 patients (60±11 yrs., 23% women) had scheduled 2-stage PCI. Mortality, non-fatal re-infarction, unscheduled revascularization and cerebrovascular events (MACCE) were recorded during follow-up of 3.5 ± 1.5 yrs.

Conclusion Immediate complete percutaneous revascularization after PPCI for STEMI in patients with MVD may result in better long-term clinical outcome compared to a 2-stage PCI strategy for men, but not for women. Further research is needed to detect if a gender specific treatment strategy at PPCI is justified.

Long term follow-up of 4 treatment strategies in Multivessel Disease (MVD) following Primary Percutaneous Intervention (PPCI) for ST-Elevation Myocardial Infarction (STEMI) Aims
Results
To evaluate the optimal treatment strategy for non-culprit lesions (n-CL) treatment in MVD STEMI patients following primary PCI, we compared clinical outcome of 4 current strategies.
Patients in the 2-stage PCI group were younger (p<0.001) and had less previous AMI compared to the MT group (p=0.03). At long-term FU (mean 3.4yrs±1.2yrs) CR strategies showed significantly lower MACCE (HR 0.66; 95% CI 0.49-0.89, p=0.007) compared to the MT group. Furthermore, 1-stage PCI and CABG were independent predictors for MACCE at long-term follow-up (HR 0.36; 95% CI 0.17-0.77, p=0.008 and HR 0.61; 95% CI 0.38-0.96, p=0.03), whereas only 1-stage PCI showed a trend for lower mortality (HR 0.16; 95% CI 0.02-1.18, p=0.07). Repeat revascularization was more frequent in the 2-stage PCI group (p<0.001). No differences were found at long term FU for re-AMI or cerebrovascular accidents.
Methods Of 1472 consecutive patients presenting with STEMI for PPCI from 2006 to 2010 without prior bypass surgery (CABG) or shock, MVD was present in 832 patients (56%). Patients were treated by; one-stage complete revascularization (CR) at PPCI in 61 patients (1-stage PCI; 65±15 yrs.), planned staged CR by PCI in 123 patients (2-stage PCI; 60±11 yrs.) or surgery in 116 patients (CABG; 65±11 yrs.) and culprit only PCI followed by medical therapy in 532 patients (MT; 65±13 yrs.). End-points were MACCE (mortality, re-AMI, repeat revascularization, non-fatal cerebrovascular accidents) and all-cause mortality at long-term follow-up (FU).
Conclusion For patients with STEMI and MVD admitted for PPCI, complete revascularization strategies had better clinical outcome at long-term follow-up.

Doel Het ondergaan van gewrichtsvervangende operaties van heup en knie leidt bij een groot deel van de mensen tot een goed resultaat op het gebied van gezondheid en gezondheidsgerelateerde kwaliteit van leven. Een kleine groep heeft echter een minder goede resultaten1. Het scannen op preoperatieve risicofactoren kan er toe bijdragen dat men patiënten met een risico op een minder goede uitkomst voortijdig kan identificeren en daar al bij het zorg traject rekening mee kan houden. Vraagstelling van dit onderzoek: Kan er een individueel predictiemodel gemaakt worden om de ligduur, het fysiek functioneren, het krijgen van een delier en de verandering van de woonsituatie van een patiënt te voorspellen op basis van preoperatieve factoren?’

Er zijn momenteel rond de 150 patiënten gescreend. Over de effecten op de (mid) lange termijn (3 maanden en langer) zijn nog geen uitspraken te doen. De eerste korte termijn resultaten op basis van 60 patiënten laten een significante matige tot goede relatie zien van de ligduur met het fysiek functioneren (TUG, FAC, knijpkracht) en een significante redelijke relatie met het psychisch welbevinden.

Minor Neurological Dysfunction (MND) kan nadelige gevolgen hebben voor de motorische ontwikkeling1,2 en de participatie van het kind in het dagelijks leven.3,4 Met het ‘Minor Neurological Dyscfunction-assessment’ (MND-assessment) wordt MND in kaart gebracht. In de praktijk is niet duidelijk bij welke klinische bevindingen het afnemen van deze test geïndiceerd is. De Movement Assessment Battery for Children-II (MABC-II) wordt veelvuldig afgenomen. Zou het scoren van de kwalitatieve items op de MABC-II gebruikt kunnen worden als indicatie voor neurologisch testen op het MND-assessment?
De kwalitatieve items gescoord op de hele MABC-II hebben een zwakke, niet signifi cante correlatie met het MND-assessment. De kwalitatieve items gescoord op het domein handvaardigheid van de MABC-II hebben een sterke maar net niet signifi cante correlatie.
Vraagstellingen
Mogelijk levert onderzoek bij een grotere groep een significante correlatie tussen de kwalitatieve items van het domein handvaardigheid en het MND-assessment op, zodat duidelijk wordt wanneer het MND-assessment uitgevoerd moet worden.

Toetsing van het BiBo-risicomodel bij patiënten die een totale knie- of heupprothese krijgen Doel Chirurgische ingrepen als een totale heup- of knieprothese gaan bij kwetsbare ouderen gepaard met verlies van zelf redzaamheid en kwaliteit van leven en een toename van morbiditeit en zelfs sterfte. Er zijn indicaties dat hoe fitter en beter voorbereid patiënten een operatie ingaan, hoe sneller het functioneel herstel na de operatie gaat1,2. Dit wordt het zogenaamde ‘better in, better out’(BiBo) concept genoemd. Het gebruikte risico model binnen het BiBo-concept is echter nog maar in één ziekenhuis getoetst. Vraagstelling voor dit onderzoek is: Is het predictiemodel gepresenteerd in het BiBo-project toepasbaar op de populatie van het HagaZiekenhuis? Met andere woorden, is de gemiddelde ligduur van de patiënten uit de risicogroep (gedefinieerd in BiBo) minimaal 1 dag langer dan de gemiddelde ligduur van een patiënt buiten deze risicogroep?
Hierbij worden de volgende preoperatieve risicofactoren gemeten: geslacht, BMI, leeftijd, Charnley-score, TUG. Postoperatief wordt de opnameduur geregistreerd. Externe validatie van het BiBo-risicomodel vindt plaats, waarbij er wordt gekeken of de risicogroep één ligduurdag verschilt ten opzichte van de niet-risicogroep.
Resultaten Er zijn momenteel rond de 150 patiënten gescreend. Meer dan de helft van de patiënten blijkt binnen de risicogroep te vallen. De ligduur laat op dit moment nog geen significant verschil zien tussen de risicopatiënten en de niet risicopatiënten (Heup: P = 0.15, Knie: P = 0.63).

Kinderfysiotherapeutische follow-up van prematuren: alleen voor de allerkleinsten? Resultaten
Hoe vroeger zuigelingen geboren worden, hoe meer risico ze lopen op motorische problemen.1-4 Wanneer deze problemen vroeg in kaart worden gebracht, kan tijdig interventie worden ingezet. Onduidelijk is of prematuur geboren zuigelingen (PG zuigelingen, zwangerschapsduur 32-37 weken), net als erg prematuur geboren zuigelingen (EPG zuigelingen, zwangerschapduur <32 weken), een risico hebben op motorische problemen en daarmee opgenomen moeten worden in de kinderfysiotherapeutische follow-up.
Twintig zuigelingen zijn geïncludeerd en getest op de TIMP. Zes van de tien PG zuigelingen hadden een risico op motorische problemen. Van de tien EPG zuigelingen waren dat er vijf.
Vraagstellingen Is er op de gecorrigeerde leeftijd van zes weken een verschil in motorische ontwikkeling tussen PG en EPG zuigelingen en hebben PG zuigelingen een risico op motorische problemen?
Methode
Conclusie Er is geen significant verschil in motorische ontwikkeling gevonden tussen PG en EPG zuigelingen op de gecorrigeerde leeftijd van zes weken en volgens deze studie hebben beide categorieën zuigelingen een risico op motorische problemen. Indien uit longitudinaal vervolgonderzoek naar het motorische ontwikkelingsniveau van prematuren bij een grotere populatie dezelfde conclusies voortkomen, wordt geadviseerd PG zuigelingen op te nemen in de kinderfysiotherapeutische follow-up.

IJzerstatus bij kinderen geboren na een zwangerschapsduur van 32 tot 37 weken Inleiding Prematuur geboren kinderen hebben een verhoogd risico op een ijzerdeficiëntie. IJzersuppletie wordt geadviseerd aan premature kinderen geboren na een zwangerschapsduur van 32 weken of minder. In hoeverre een soortgelijk advies wenselijk is voor kinderen geboren na 32 tot 37 weken zwangerschap is onbekend.
Doel Inzicht te verkrijgen in de ijzerstatus van kinderen geboren na een zwangerschapsduur van 32 tot 37 weken.
Methode Prospectief cohort onderzoek bij kinderen geboren na een zwangerschapsduur van 32 tot 37 weken. De ijzerstatus werd bepaald in de eerste week na de geboorte en op de leeftijd van 1½, 4 en 6 maanden.
Resultaten 200 kinderen werden geïncludeerd waarvan tot op heden 156 kinderen zijn vervolgd tot de leeftijd van 6 maanden. Een lage ijzervoorraad (ferritine <12 µg/L) was aanwezig bij 1, 7 en 7 kinderen op de respectievelijke leeftijd van 1½, 4 en 6 maanden. Kinderen met een lage ijzervoorraad in de eerste 6 maanden,

hadden lagere ferritine waarden na de geboorte en kregen langer borstvoeding dan kinderen met een normale ijzer voorraad (respectievelijk 101,5 µg/L (+76,9 µg/L) en 215,5 µg/L (+122,5 µg/L), p 0.001 en 16 weken (+9 weken) en 10 weken (+9 weken), p 0.03).
Conclusie Een lagere ijzervoorraad bij de geboorte is geassocieerd met een lagere ijzer voorraad op de leeftijd van 1½, 4 en 6 maanden. Daarnaast suggereren deze resultaten dat het ijzergehalte in borstvoeding onvoldoende voorziet in de verhoogde ijzerbehoefte van kinderen geboren na een zwangerschapsduur van 32 tot 37 weken in de eerste 6 maanden na de geboorte.

Interpretatie van ijzerstatus parameters
Inleiding
Resultaten
Er bestaat geen consensus over de definitie van ijzerdeficiëntie bij kinderen. Naast ferritine en hemoglobine (Hb) wordt in de literatuur vaak meerdere ijzerstatus parameters bepaald, waarbij ijzerdeficiëntie gedefinieerd wordt als >2 afwijkende waarden. Over de correlaties tussen verschillende parameters is weinig bekend waarmee de toegevoegde waarde van deze methode boven het uitsluitend bepalen van het ferritine en Hb discutabel is.
Tabel 1 toont de correlaties tussen de ijzerstatus parameters. Het ferritine is gecorreleerd met het hepcidine maar niet met het MCV, Ret-Hb, Hb en de sTfR. Deze laatste parameters zijn onderling wel met elkaar gecorreleerd.

parameters verschillende stadia van ijzerdeficiëntie weergeven. Ferritine en hepcidine zijn een maat voor de ijzer voorraad terwijl het Hb, Ret-Hb, MCV en de sTfR een maat zijn voor het ijzer beschikbaar voor de erythropoïese. Als een actieve infectie is uitgesloten geven ferritine en Hb goed inzicht in de ijzerstatus van jonge kinderen.
Conclusie De correlatie tussen het ferritine en hepcidine enerzijds en het Hb, Ret-Hb, MCV en de sTfR anderzijds laat zien dat deze

Gebruik van de Abdominale VAC in het HagaZiekenhuis
Inleiding
Methode
Primaire en/of secundaire peritonitis al dan niet in combinatie met een abdominaal compartiment syndroom (ACS) gaan gepaard met een hoge mortaliteit. Recentelijk is het focus van behandeling gericht op snelle ‘broncontrole’ tesamen met openbuik behandeling met VAC therapie (AbVac) en streven naar zo snel mogelijk sluiting van de buik. Volgens de literatuur kan een mortaliteit tot 18% worden bereikt en 89% sluiting van de buik. De ratio, techniek en de resultaten van inplementatie van deze techniek en strategie in het HagaZiekenhuis worden beschreven.
Alle 13 opeenvolgende patiënten die zijn behandeld met AbVac zijn geanalyseerd. Er werd gekeken naar de invloed van aantal VAC wissels op de uitkomst van mortaliteit, ligduur, complicaties en mogelijkheid om buik te sluiten, mede in het licht van kosten.
Resultaten Patiënten met een primair focus (sepsis, perforatie) hadden een significant hogere mortaliteit (67%) dan de secundaire peritonitis (25%), 8 versus 12 ligdagen IC, 34 versus 29 dagen VAC. Er trad 1x vorming van enterocutane fistel op. Negen patienten konden worden gesloten met behulp van en MESH en vier patienten primair. De kosten van 2 weken VAC Therapie zijn ongeveer 4725 Euro per patient, jaarlijks 84.000 Euro versus 135.000 Euro Jaarlijks voor secundaire littekenbreuk correctie, doch worden mede bepaald door de kosten van de evt. gebruikte mesh.
Conclusie Vroegtijdige en intensieve open buik behandeling met Abdominale Vac therapie en zo vroeg mogelijk sluiting van de buik is mogelijk, is echter een zeer intensieve therapie met nog steeds hoge mortaliteit en kan leiden tot minder kosten op de langere termijn. Elke indicatie zal echter kritisch moeten worden bekeken.

Bij Video Assisted Thoracoscopische ingrepen (VATS) is een betrouwbare een long ventilatie met een double lumen tube (DLT) van groot belang. Bij de klassieke DLT is fiberscopische controle ter bevestiging van een juiste positie noodzakelijk. Deze controle kan slechts intermitterend worden uitgevoerd. Recent is een DLT (Viva Sight®) op de markt gekomen met een ingebouwde camera. Hiermee wordt een continue video controle van de positie van de DLT mogelijk. Wij onderzochten de tijd die met het gebruik van beide typen DLT gepaard ging en de bijkomende kosten.

Conclusie Het gebruik van de Viva Sight® leidt tot een verkorting van de tijd die voor inleiding en positionering noodzakelijk is. Daarbij is het gebruik goedkoper en is een continue bewaking van de juiste positie van de tube gewaarborgd. Of dat laatste leidt tot betere resultaten dient nader onderzocht te worden.
Een retrospectieve analyse van de intubatiemethodes Viva Sight vs Fiberscoop van alle VATS operaties tussen 1-1-2013 en 22-8-2013. Inleidingstijd en tijd van positioneren van patiënt werden vergeleken.

De Nuss bar operatie als operatie van pectus excavatum: goede resultaten bij een multidisciplinaire aanpak Figuur 1: Spreiding van de patiëntengroep
Achtergrond Van de congenitale thoraxwand deformi teiten is 90% een pectus excavatum (PE). PE wordt gekenmerkt door een ingevallen borstwand. De incidentie is 1:400-1000 en het komt vier keer vaker voor bij een man dan bij een vrouw. Er zijn twee chirurgische methodes voor de correctie van PE, de modificaties van de Ravitch methode en de minimaal invasieve Nuss bar methode.

53 minuten in 2012. De mediaan van de operatieduur voor de plaatsing van twee bars was 107 minuten in 2011 en 73 minuten in 2012. Er zijn in totaal vier complicaties opgetreden, zowel peri- als postoperatieve complicaties. Deze konden alle goed behandeld worden.

Materiaal en methoden Een retrospectief statusonderzoek is verricht in het Hagaziekenhuis. Geïncludeerd zijn alle patiënten met PE waarvoor een Nuss bar operatie is verricht in de periode van 2011 en 2012 in het Hagaziekenhuis.
Resultaten In totaal zijn er in de onderzoeksperiode 35 Nuss bar operaties verricht. 77% was man en 23% vrouw. De gemiddelde leeftijd van de totale groep was 20,5 jaar. Van de patiënten presenteerde zich 35% met lichamelijke klachten, 21% psychosociale klachten en 44% met zowel lichamelijke als psychosociale klachten. De mediaan van de operatieduur voor de plaatsing van één bar was 72 minuten in 2011 en 28

Conclusie De Nuss bar operatie is een succesvolle, minimaal invasieve methode voor de correctie van PE met een zeer hoge patiënttevredenheid. Echter, het is een operatie met potentieel ernstige complicaties, zowel peri- als postoperatief. Een multidisciplinaire aanpak essentieel. Een ervaren, goed samenwerkend team bestaand uit een chirurg, een anesthesist, een fysiotherapeut en verpleegkundigen leidt tot goede resultaten. Centralisatie van de Nuss bar operatie in gespecialiseerde ziekenhuizen zou de kwaliteit van de zorg voor patiënten kunnen verbeteren.

Colorectaal carcinoom HagaZiekenhuis 2009-2012 Uitkomstindicatoren en landelijke spiegelinformatie
Patiënten en methoden Alle patiënten die in HagaZiekenhuis werden geopereerd voor een colorectaal carcinoom werden geïncludeerd. Gekeken werd naar het percentage re-interventies en re-operaties, na colorectale chirurgie.
Resultaten Jaarlijks werden in het HagaZiekenhuis tussen de 139 en 168 colorectale resecties verricht. Het percentage aantal re-inter venties varieert tussen de 13% en 9%,
In DICA wordt de spiegelinformatie en de indicatoren per ziekenhuis jaarlijks gerapporteerd. De geleverde funnelplots geven inzicht in de relatie tusssen aantal behandelde patiënten per ziekenhuis, het percentage reinterventies en re operaties, en de betrouwbaarheidsintervallen. De bijgaande figuren uit de DICA rappor tage, genummerd 5 en 6, geven deze funnelplots voor 2012 weer voor de reinterventies en re operaties. Het Haga Ziekenhuis wordt in rood weergegeven.
Nauwgezette lokale registratie van de uitkomsten van zorg bij colorectale chirurgie en invoer in de landelijke audit (DICA) geven inzicht in de kwaliteit van zorg, het verloop in de tijd, en de ver houding van eigen kwaliteit ten opzichte van het landelijk gemiddelde.

Sedert 2009 wordt door de Nederlandse GE-chirurgen een aantal belangrijke uitkomstparameters van de geopereerde patiënten voor colorectale maligniteiten gerapporteerd in de DICA (Dutch Institute for Clinical Auditing). De GE chirurgen van het HagaZiekenhuis participeren sinds de start van de Audit. In de huidige analyse is gekeken naar de kwaliteitsparameters in de loop van de afgelopen jaren, gespiegeld aan de benchmark van het gemiddelde resultaat in Nederland.

Vijf jaar schildwachtklierprocedures bij mammacarcinoom in HagaZiekenhuis: resultaten en consequenties 2007-2012 Inleiding De schildwachtklierprocedure kan bij patiënten met een mammacarcinoom een onnodige okselklierdissectie en de morbiditeit hiervan voorkomen. Een tumorvrije schildwachtklier betekent met 95-98% betrouwbaarheid dat de overige okselkieren ook tumorvrij zijn. Wanneer de schildwachtklier wel tumor bevat wordt de oksel wel behandeld. De huidige analyse omvat de bevindingen van okselklierstadiering en -behandeling van het Mammateam HagaZiekenhuis over 2007-2012
Patiënten en Methoden Alle patiënten die tussen juli 2007 en juli 2012 zijn geopereerd in verband met een KT1/2N0 mammacarcinoom gecombineerd met schildwachtklierprocedure werden geincludeerd. De analyse omvat de bevindingen, en de vervolgbehandelingen van deze patiënten.
Resultaten In totaal werden 777 schildwachtklier procedures verricht. Bij 4 patiënten werd peroperatief de schildwachtklier niet gevonden en werd primair een okselklierdissectie verricht: deze patiënten bleken allen uitgebreide kliermetastasering te hebben (3 x N2 en 1 x N3).

Bij 200 patiënten werd een positieve schildwachtklier gevonden. 119 patiënten ondergingen een directe okselklierdissectie na peroperatief onderzoek van de schildwachtklier. Bij 70 patiënten werd geen additionele kliermetastasering meer gevonden in het okselklierdissectie preparaat. Bij 49 (40%) werden additonele kliermetastasen gevonden (28x 1-3 klieren 21x >3 klieren). Bij 29 positieve schildwachtklieren werd de oksel bestraald in het kader van de AMAROS/EORTC10981 trial. Bij 52 patiënten (27%) was er sprake van discordantie van het peroperatief en definitieve PA-rapport. Bij 47 van hen werd een aanvullende okselklierdissectie verricht. Bij 9 (17%) van hen werden additionele kliermetastasen (1 of 2) gevonden. Bij 5 patiënten werd de oksel behandeld met radiotherapie. Bij 38 van de 200 positieve schildwacht klieren betrof het een mikrometase. 23 ondergingen een okselklierdissectie. Bij drie (13%) van hen werd 1-2 additionele kliermetastasen gevonden. De overige werden adjuvant behandeld (RT/ systemisch)
Conclusie Onze resultaten ondersteunen de huidige visie dat okselklierdissectie bij tumor positieve schildwachtklierbevindingen op statische gronden en de huidige stand van de wetenschap niet bij iedere patiënte vanzelfsprekend is.

Uitkomstindicatoren en Spiegelinformatie Mammacarcinoom behandeling HagaZiekenhuis 2011-2012
Prestatieindicatoren voor mamma carcinooom worden door vele partijen reeds jaren van de ziekenhuizen gevraagd. Sedert 2011 worden de gegevens verzameld een aangeleverd aan DICA (Dutch Institute for Clinical Auditing). Daarmee is homologatie van de parameters verkregen alsmede de mogelijkheid de eigen kwaliteitsparameters te spiegelen aan de landelijke data. Door de gespecialiseerd verpleegkundigen, de verpleeg kundig specialisten, en de afdeling Beleid en Kwaliteit worden de data gecontroleerd op juistheid en consistentie.
In 2011 en 2012 werden 233 resp 212 patienten met een mammacarcinoom geopereerd. Vrijwel alle patiënten werden pre- en postoperatief multidisciplinair besproken. Het percentage patiënten dat binnen normtijd na diagnose wordt geopereerd ligt hoog (Tabel1).
Patienten en methoden Van de patiënten die in 2011 en 2012 in het HagaZiekenhuis werden geopereerd voor een mammacarcinoom werden logistieke en inhoudelijke kwaliteits parameters geanalyseerd en vergeleken met landelijke data.
Voor de patiënten die met een borst sparende operatie worden behandeld is een radicale resectie bij eerste operatie een belangrijke parameter. In 2011 en 2012 werden 142 resp 117 patiënten voor een invasieve tumor sparend behandeld. De radicaliteit bij eerst resectie worden getoond in de funnelplots. Het Haga Ziekenhuis wordt in rood weergegeven. (figuur 4 en figuur 7; Data van DICA.NL).
Conclusie Nauwgezette dataregistratie en controle maakt betrouwbare toelevering van lokale kwaliteitsparameters mogelijk. Deze kunnen vervolgens worden gespiegeld aan de vastgestelde norm en de gemiddelde in Nederland.

Multidisciplinaire behandeling en -bespreking en protocoladherentie Melanoomteam HagaZiekenhuis: resultaten 2011-2012
Inleiding

Conclusie
De incidentie van het melanoom neemt in Nederland toe. De protocollaire behandeling is in recente jaren uitgebreid met lymfogene stadiering (schildwachtklier procedure bij >1mm Breslow dikte (SNB)) en nieuwe (dure) systemische middelen. In het HagaZiekenhuis functioneert sinds 2011 een Melanoom-team en voor trials (MSLTII) en behandeling met systemische middelen zijn er afspraken gemaakt met NKI/Avl en LUMC. Binnenkort zal een landelijke registratie voor melanomen stadium III en IV worden gestart (DICA). Multidisciplinaire bespreking van het behandelbeleid is hierin een belangrijke indicator.
In 2011 en 2012 werden 104 patiënten behandeld voor hun melanoom (78 primair, 26 recidivering). Een primair melanoom werd bij 78 patiënten gediagnosticeerd. De primaire excisie werd 34 keer verricht door de afdeling dermatologie, 18 keer door de heelkunde, 14 keer door de plastische chirurgie, en 12 keer door een huisarts. De Breslowdikte van het melanoom was meer dan 1 mm bij 45 patiënten. In totaal werden 10 patiënten niet multi disciplinair besproken en geen snb procedure verricht (2011 n=2; 2012 n=8). Elf patiënten zagen na voorlichting af van een schildwachtklierprocedure. Bij 24 patiënten werd een SNB-procedure verricht. Hierbij werden bij 6 patiënten een positieve SNB gevonden.
In 2012 werden twee maal zoveel patiënten met een melanoom behandeld. Deze toename ging gepaard met een minder goede opvolging van de protocollaire behandeling en pre- en postoperatieve MDO bespreking. Het melanoomteam heeft de gemaakte werkafspraken daarop opnieuw besproken en bevestigd.
Doel Het inzichtelijk maken van de diagnostiek en behandeling van patiënten met een melanoom in het Hagaziekenhuis 2011-2012.
Patiënt en Methoden Alle patiënten die in de periode 2011 (N=33) en 2012 (N=71) werden geregistreerd met de diagnose Melanoom (PALGA) werden geanalyseerd naar methode van diagnose, en behandeling.

Totaal werd bij 26 patiënten een recidief melanoom (in-transit metastase(n) (N=11) of lymfklier-metastase(n) (n=15)) gevonden. Een PET CT scan toonde bij 18 van hen ook metastasen op afstand. Patiënten die hiervoor in aanmerking kwamen werden adequaat verwezen naar de samenwerkende centra.

MRI Mamma: een retrospectieve analyse naar de diagnostische opbrengst bij high risk screening en bij diagnostische ‘problem solving’ Inleiding Een MRI van de mamma wordt voor een aantal indicaties gemaakt. Screening bij patiënten met een hoog risico op mammacarcinoom, diagnostische dilemma’s bij conventionele diagnostiek, en respons evaluatie bij neo adjuvante chemotherapie zijn de belangrijkste. Van de MRI mammae die in het Hagaziekenhuis werden gemaakt werd de diagnostische opbrengst retro spectief geanalyseerd.
Patiënten en methoden Alle MRI’s van de patiënten die in de jaren 2011 en 2012 een MRI mammae onder gingen in het HagaZiekenhuis werd de indicatie, vraagstelling, bevindingen en de eventuele wijziging van het diagnostisch en/of behandelbeleid geanalyseerd. Follow up MRI’s werden separaat geanalyseerd van de MRI’s die in het kader van diagnostische dilemma werden aangevraagd na MammaMDO.
‘Eerst verrichte MRI’s’ die werden gemaakt naar aanleiding van een diagnostisch dilemma na bespreking in het MDO werden 46 maal gemaakt. De gemiddelde leeftijd in deze groep was 54,5 jaar (range 34-79jaar). De indicatie voor deze eerste MRI’s was bij 27 een lastig beoordeelbare mammografie, bij 5 een discrepantie tussen de mammografische en pathologische bevindingen (ptie) en 14 overige indicaties. 9 van deze MRI’s toonden afwijkingen (maligne) die het behandelbeleid veranderden (19,6%)
Conclusie In de huidige praktijk van voor het maken van MRI voor high risk patiënten wordt bij 3,5% van de onderzoeken een maligniteit gevonden in twee jaar. De MRI’s die gemaakt worden in het kader van ‘problemsolving’, leveren vaak extra informatie. In een groot aantal gevallen wordt het diagnose- of behandelbeleid hierdoor beïnvloed.
Resultaten In de jaren 2011-2012 werd bij 130 vrouwen een MRI mammae gemaakt. 84 MRI’s waren follow up MRI (tweede of volgende) bij high risk screening, slecht beoordeelbare mammografieën, of te pijnlijke mammografie. Bij deze 84 MRI ’s werden 3 maligniteiten gevonden (3,5%). HagaWetenschap | Abstracten HagaZiekenhuis

Galblaasoperaties HagaZiekenhuis 2008-2012: complicatieregistratie als kwaliteitsinstrument
Inleiding In HagaZiekenhuis worden ongeveer 350 cholecystectomieen per jaar verricht. De huidige standaard hiervoor is de laparoscopische verwijdering van de galblaas. De ernstigste complicatie van galblaaschirurgie is gallekkage. Bij peroperatieve complicaties of ernstige infiltratie kan conversie naar open proce dure nodig zijn. In de huidige analyse worden de complicaties bij laparoscopische galblaaschirurgie in het HagaZiekenhuis vergeleken in drie jaren 2008-2010-2012, en gespiegeld aan literatuurdata.
Patiënten en methoden Van de galblaasoperaties in de jaren 2008, 2010, en 2012 werden complicatieregistratie-data geanalyseerd. Het percentage conversies, wondinfecties, gallekkages en de mortaliteit van de (laparoscopische) cholecystectomie werden geanalyseerd.
Resultaten Tabel 1 toont het aantal galblaasoperaties in opeenvolgende jaren. In 2008 werden nog 18 cholecystectomieen primair open verricht. In 2012 werden alle cholecystecomieen in opzet laparoscopisch verricht. Het percentage conversies nam

hierbij niet toe. Het percentage wond infecties, en gallekkages is globaal stabiel, en in de orde van grote die de literatuur vermeldt. De patiënten die overleden gerelateerd aan de cholecystectomie waren allen patiënten met ernstige comorbiditeit en/of zeer hoge leeftijd.
Conclusie Alle cholecystectomieën worden heden ten dage laparoscopisch in opzet verricht. Noch het percentage conversies naar open procedure, noch het percentage ernstige complicaties neemt hierbij toe. De percentages complicaties komen overeen met die gerapporteerd in de literatuur.

Implantatie van Port-a-Cath systemen voor intraveneuze toegang Resultaten HagaZiekenhuis 2010-2011: complicaties, levensduur en vergelijking met literatuur Inleiding Een Port-a-Cath is een volledig onderhuids implanteerbaar systeem voor herhaalde lijke intraveneuze toegang. Voor de behandeling van patiënten met hemato logische, of solide maligniteiten, of andere reden voor herhaaldelijke toegang is er een indicatie voor een dergelijk systeem. De complicaties van de ingreep en de levensduur van het cathetersysteem werden geanalyseerd.
Patiënten en methoden Van alle patiënten die in de jaren 2010 en 2011 een Port-a-Cath systeem kregen werd uit status en complicatieregistratiesysteem data geanalyseerd over de ingreep, direct postoperatieve complicaties, complicaties op langere termijn, en de levensduur van de catheter.
Resultaten In 2010 en 2011 werd bij 45 patiënten een Port-a-Cath systeem geïmplanteerd. De mediane leeftijd was 45 jaar (range 17-71). 18 maal bij patiënten met een solide tumor (40%), 20 maal bij hemato logische ziekte (44%), andere indicaties 7 maal (16%). Er waren 14 complicaties (1 binnen 24hr, 10 binnen 180 dagen, 3 na 180 dagen). Bij 37 patienten (72,5%) trad geen complicatie op. De meest voorkomende compli catie was infectie (8%).
De catheter werd bij 30 patiënten verwijderd. Het einde van de behandeling was de reden bij 18 verwijderingen. De overige 12 werden verwijderd naar aanleiding van complicaties (infectie, malfunctie, thrombose).

In de hele groep bedroeg het gemiddeld aantal catheterdagen 296 (range 0-1034). Er zijn vier grotere series in de literatuur over deze problematiek (range: n=228 – n=1500). Het gemiddeld aantal catheter dagen in meta analyse van deze studies bedraagt 237-284 dagen. (tabel 1). Het gemiddelde percentage ongecompliceerde Port-a-Cath plaatsingen bedraagt in deze series 76%-88%.

Conclusie Het aantal catheterdagen van de in Haga geïmplanteerde Port-a-Cath’s komt overeen met die in de literatuur. Het aantal complicaties is relatief hoger dan in de literatuur. Dit wordt vooral veroorzaakt door een wat hoger percentage infecties. Of de toediening van pre-operatieve antibiotica profylaxe was gebeurd, bleek niet bij alle patiënten naspeurbaar. Dit werd hierom opgenomen in het operatieve protocol implantatie Port a Cath.

Polyneuropathie op basis van een IgM-paraproteinemie: verbetering na behandeling met Rituximab Introductie
Achtergrond
De incidentie van polyneuropathie in Nederland wordt geschat op 3.6%, 12-27% hiervan wordt geduid als idiopathische neuropathie. 10% van deze neuropathieën blijkt te berusten op IgM-paraproteinemie, welke kan lijden tot ernstig hinder.
De etiologie van polyneuropathie op basis van een IgM-paraproteinemie is grotendeels onbekend. Enerzijds kan het M-proteine binden aan het MAG-eiwit, waardoor er een anti-MAG neuropathie ontstaat. Anderzijds is dit anti-MAG niet altijd aantoonbaar en kan een polyneuropathie op basis van een IgM paraproteinemie zich voordoen als een chronisch inflammatoire demyeliniserende neuropathie. Zonder behandeling een progressieve aandoening. Twee middelen lijken positief effect te hebben: Fludarabine & Rituximab. Van beide middelen is het effect nog niet bewezen in een dubbelblind placebo-gecontroleerd onderzoek. Beide middelen verhogen de kans op infectie, maar in tegenstelling tot Fludara bine heeft Rituximab geen pancytopenie als bijwerking.
Casus Het betreft een 53-jarige man met een blanco voorgeschiedenis die zich presenteert met klachten van branderige pijn in de benen, paraesthesieën en een afnemende gevoel in de benen. Het EMG toonde een distaal overheersende polyneuropathie aan armen en benen met forse motorische en sensorische component en ook een demyeliniserende component, het laboratorium onderzoek toonde een M-proteine 2g/l. Geconcludeerd wordt dat er sprake is van een polyneuropathie op basis van een IgMparaproteinemie. Behandeling met Rituximab wordt gestart, waarop de klachten afnamen en het EMG verbeterde.

Conclusie Bij patiënten met een polyneuropathie op basis van een IgM-paraproteinemie, welke ernstig hinder ondervinden in het dagelijks leven, kan behandeling met Rituximab (4 weken 1x/week 375mg/ m2) overwogen worden.

Track en trace bij bloedtransfusie
De CBO consensus schrijft voor dat bloedproducten in de gehele bloed transfusieketen voor 100% traceerbaar moeten zijn. Tot nu toe werd daar niet volledig aan voldaan. Bij de laatste fase van de keten, de toediening van het bloedproduct, ontbrak tot nu toe een betrouwbare verslaglegging. Dit is, vanzelfsprekend, een heel belangrijke schakel in de keten. Om een volledige traceerbaarheid te bewerkstelligen werden de volgende stappen ondernomen: 1. In de eerste fase van het project is de invoering van polsbandjes bij alle patiënten gerealiseerd. Op de pols bandjes staat een barcode met het patiënt identificatie nummer (PIN). 2. In de tweede fase is de personeel badge van alle verpleegkundigen voorzien van een barcode met personeelnummer. 3. In de derde fase wordt het track en trace systeem ingevoerd. Het programma is een internet applicatie met een “real time” connectie met het laboratorium informatie systeem. Als webapplicatie is het via de internet browser te benaderen. Op de ver nieuwde afdelingen wordt gebruik gemaakt van Computers On Wheels waarmee draadloos aan het bed de controle tussen patiënt en bloed product kan worden uitgevoerd.
De belangrijkste verbeteringen zijn: risico toediening onjuist bloedproduct geminimaliseerd ■■ 100% traceerbaarheid bloedproducten ■■ gehele keten eenduidig door identifi catie van patiënt en product met barcodes ■■ volledige procedure inzichtelijk en vastgelegd ■■ kostenbesparing door minder administratie ■■ uitbreiding patiëntinformatie (trans fusieadvies, transfusie historie) ■■ kostenbesparend, 1 verpleegkundige kan de transfusie veilig uitvoeren (voorheen 2) ■■ terugdringen papierwerk ■■
Na de klinische hematologie, waar al geruime tijd wordt gewerkt met het programma, zal het programma binnenkort ook op alle andere afdelingen worden geïntroduceerd.

Presence of medication taken by blood donors in plasma for transfusion Background
Method
The TRIP national hemovigilance and biovigilance office receives reports on side effects and incidents associated with the transfusion of labile blood products. The findings are publicly reported in annual hemovigilance reports. The category of anaphylactic reaction, defined as allergic reactions with systemic features such as airway obstruction, fall in blood pressure, vomiting or diarrhoea, accounts for the largest number of serious transfusion reactions. In most cases no cause is found for these reactions. TRIP data show that anaphylactic reactions occur relatively frequently during or after transfusion of plasma or (plasma rich) platelet con centrates. Data from blood services show that approximately 10% or more of plasma donors regularly use medication which is permitted under donation guidelines. It is conceivable that medication taken by the donor, or its metabolites in plasma for transfusion, could cause an anaphylactic transfusion reaction in the recipient.
In a large hospital, samples (5 ml) were taken from thawed quarantine fresh frozen plasma units which had to be rejected for transfusion because of leaks or length of time after thawing. Up until screening the samples were stored frozen at 70°C. Only samples for which the blood supply organisation confirmed that the donor had consented to research use were included. The samples were analysed for approxi mately 800 drugs or drug metabolites using ITox screening. All detectable substances that were recognised as drug or drug metabolite were considered a positive screening result.

Results Currently 87 samples have been collected and analysed. ITox screening was positive in 14 samples, in eleven samples only one substance was determined and in three samples the presence of two or three drugs was measured. Results screening in table 2.

In 16% of the analysed quarantine fresh frozen plasma units drug or drug metabolites could be determined. This is in line with the expectation that 10 to 20% of the plasma donors use medication on a regular basis.

The found concentrations do not exceed the concentrations expected under normal use of that particular medicine. All of the found drugs are permitted under donor guidelines. Unexpected was the finding of methylparaben, a substance widely used as preservative by cosmetic and pharmaceutical industries. Parabens are also used as food additives.
After freezing, quarantine, storage en thawing of fresh frozen plasma units it is possible to detect medication in effective concentrations in donor plasma. Even when taking into account the diluting effect that occurs with transfusion, it cannot be excluded that a sensitized patient could get an anaphylactic reaction due to administration of plasma with this level of drug concentration. Further investigation of the effects of donor medication in the recipients of plasma or plasma rich blood products is recommended.

Niet elke pneumonie is wat het lijkt
Casusbeschrijving Een 72-jarige vrouw, bekend met een indolent T-cel lymfoom, komt bij haar huisarts vanwege dyspnoe en een nietproductieve hoest. Deze schrijft onder verdenking van een pneumonie doxycyline voor. Na uitblijven van klinische ver betering wordt vervolgens claritromycine voorgeschreven gericht op “een atypische verwekker". Twee maanden later bezoekt zij de SEH vanwege aanhoudende klachten. Er is sprake van hypoxemie en desaturatie bij inspanning. De X-thorax toont een consolidatie in de rechter middenkwab en enkele preexistente noduli (zie figuur 1). Een bacteriële pneumonie, niet reagerend op zowel een tetracycline als een macro lide, werd uiterst onwaarschijnlijk geacht,

bovendien maakte patiënte geen acuut zieke indruk en had zij geen koorts. Aanvullend onderzoek toonde dat er slechts sprake was van mild verhoogde ontstekingswaarden. Differentiaal diagnostisch werd progressie van het bekende NonHogdkin lymfoom (type; not otherwise specified) of een primair longcarcinoom bij haar nicotineabusus (28 “pack years”) vermoed. Het lymfoom was voorheen jaren stabiel, vrijwel asymptomatisch en er was in overleg met patiënte gekozen voor een expectatief beleid. Aanvullend onderzoek werd nu aangewezen geacht. De PET-CT scan toonde uitgebreide lymfadenopathie in de hals, het media stinum en de rechter hilus. Daarnaast werden meerdere consolidaties in de longen en pleurale tumordeposities waargenomen. Cytologisch onderzoek van een halsklier en pleuravocht toonden geen aanwijzingen voor een primair longproces. Aanvullend beenmergonderzoek was niet bijdragend (geen lokalisatie van bekende T-cel lymfoom). In verband met aan houdend hypoxemie werd een conven tionele longfoto herhaald waarbij pro gressie van het longbeeld en pleuravocht werd gezien (zie figuur 2). Er volgde pleuradrainage, waarbij eveneens de diagnostiek herhaald werd. Nu werd een beeld passend bij lokalisatie T-cel lymfoom gevonden. Histologisch onderzoek van een halsklier toonde lokalisatie van een T-cel lymfoom met opvallende EBV-positieve B-cel component. Differentiaal diagnostisch werd gedacht aan een reactieve proliferatie secundair aan het T-cel lymfoom (opvlamHagaWetenschap | Abstracten HagaZiekenhuis

ming door disfunctionering van T-helpercellen) of een EBV-gedreven maligne ontaarding van de B-lymfocyten. De PCR op EBV was sterk positief (4.0 log copies/ mL, 9.481 copies/mL). Uiteindelijk werd geconcludeerd dat er sprake was van een tweede Non-Hodgin Lymfoom; naast het eerder vastgestelde T-cel lymfoom nu ook een EBV geassocieerd B-cel lymfoom. Patiënte wordt behandeld met chemo therapie (R-CHOP). De respons was klinisch en radiologisch goed en inmiddels is er een partiële remissie opgetreden. Deze casus illustreert de noodzaak om bij patiënten die zich presenteren met een (atypische) pneumonie, alternatieve overwegingen in de differentiaal diagnose op te nemen, zeker indien klinische respons op gerichte antibiotisch behandeling uitblijft. 39

Inleiding
Resultaten
Vanaf 1988 is het stamceltransplantatie traject actief op de afdeling hematologie. Dit traject begint bij afname (aferese) van hematopoїetische progenitor cellen (HPC) bij patiënten in remissie na stamcel mobilisatie. De HPC(A)’s worden bewerkt, gecryopreserveerd en opgeslagen in vloeibare stikstof. Na conditionering, bestaande uit chemotherapie en eventueel bestraling, worden deze hematopoїetische progenitor cellen na ontdooien weer teruggeven aan de patiënten middels infusie (reïnfusie).
CD34 aantal en vitaliteit voor en na cryopreservatie (zie tevens grafiek): De mediane recovery van CD34 cellen na ontdooien gecorrigeerd voor de vitaliteit is 70% (61 – 79%). Ongecorrigeerd is de mediane recovery 87% (76-98%); (zie figuur 1).
De cryopreservatie- en ontdooiprocedure zijn kritieke punten voor de kwaliteit van het product. Er zijn criteria vastgesteld waar aan voldaan moet worden om zo de kwaliteit van het product te garanderen.
Principe Van alle patiënten in het stamceltransplantatie traject van 2012 (n=36) zijn de gestelde kwaliteitscriteria geëvalueerd en vergeleken met de voorafgestelde eisen en literatuur. De volgende criteria spelen een cruciale rol bij het kwaliteit van het product: CD 34+ cellen,vitaliteit en CFU-gm voor en na cryopreservatie en repopulatie na reïnfusie.

CFU-gm per CD341: Voor invriezen is het aantal CFU-gm / CD34 mediaan 0.08 (0.05 – 0.14) en na ontdooien 0.05 (0.03 – 0.08). Of anders geformuleerd voor invriezen leveren 12.5 CD34 cellen 1 CFU-gm op en na invriezen leveren 20 CD34 cellen 1 CFU-gm op. Repopulatie: Op dag 20 hadden 28 (78%) patiënten > 0.5 x 10e9/L granulocyten en 31 (86%) > 20 x 10e9/L trombocyten. Op dag 100 hadden alle patiënten een WBC getal van > 2.0 x 10e9/L en hadden slechts drie patiënten een trombocytengetal < 100 x 10e9/L (70, 94 en 96).
Conclusie Het cryopreservatie proces, inclusief ontdooien, zoals uitgevoerd door de sub afdeling IHSCU van de afdeling hemato logie, voldoet aan de vastgestelde eisen. De productkwaliteit kan gegarandeerd worden.

Coma door pregabaline, kan dat?
Introductie Pregabaline, of (s)-3-aminomethyl-5-methyl-hexaanzuur is een GABA-agonist met anti-epileptische, analgetische, anxiolytische en antiemetische werking. Het heeft een effect op de spanningsafhankelijke calciumkanalen. Hierdoor neemt de calciuminflux af en worden de zenuwen minder prikkelbaar1. Therapeutische bloedspiegels zijn tussen de 1 en de 6mg/l. Vanuit expert opinion worden spiegels boven de 12 mg/l als toxisch beschouwd. De belangrijkste neurologische bijwerkingen zijn hoofdpijn, duizeligheid, ataxie en slaperigheid2. Coma werd eenmaal eerder beschreven na een intoxicatie met 8 gram Pregabaline3. Niet eerder is beschreven dat een patiënt langdurig een EMV van 3 had en daarbij beademd moest worden. Dit case report beschrijft een casus van een pregabaline intoxicatie waarbij patiënte langdurig comateus was na inname van vermoedelijk 1,5 gram in combinatie met benzodiazepinen.
Case details Het betreft een vrouw van 65 jaar, met depressieve klachten sinds het overlijden van haar partner. Ze heeft een tentamen suicide gedaan met 1500mg pregabaline, perindopril, valeriaan, temazepam, oxazepam. Ze presenteerde zich op de SEH
met een EMV van 3 en werd geïntubeerd. Bij opname op de Intensive Care was er een geschatte spiegel van de pregabaline van 40mg/l, met een marge van 30mg/l tot 50mg/l. Na 30 uur was de spiegel 1.6mg/l. Uitgaande van de halfwaardetijd van 6 uur zou de spiegel tot niet toxisch moeten zijn gedaald na 14 uur, bij een normale nierfunctie. Echter de patiënte scoorde een EMV van 3 de eerste 12 uur, de 12 uur daaropvolgend verbeterde de EMV van 3 naar 10, maar pas na 32 uur was patiënte neurologisch voldoende hersteld om geectubeerd te worden.
Conclusie

Symptomen zijn na een tentamen suicidale met pregabaline doorgaans mild3-4-5. Beschreven casuïstiek overautointoxicaties met pregabaline zijn daarom beperkt3. Patiënten zijn meestal chronische gebruikers van het middel waarmee zij een autointoxicatie doen3-4-5 Symptomatologie is uiteenlopend, maar zo’n ernstig bewustzijnsverlies werd slechts eenmaal eerder beschreven3. In dit case-report beschrijven wij een comateus beeld bij een lage overdosering pregabaline, echter wel in combinatie met benzodiazepinen. Het coma kan enerzijds verklaard worden door synergistische effecten van pregabaline en benzidiazepinen, anderzijds door niet chronisch gebruik in combinatie met een eenmalige toxische ingestie.

Mortality of patients readmitted to the ICU
Introduction We studied our ICU database to see whether patients who require readmission to the ICU during the same hospital admission have a worse prognosis as compared to patient who are not readmitted.
Patients and methods We included all patients admitted to the ICU of the HagaZiekenhuis between January 1st, 2010, and December 31st, 2012, in the study. The ICU is a level 3, intensivist led, 16-bed mixed medical surgical unit. All specialties including cardiothoracic and neurosurgery are available in our hospital. We extracted all relevant patient data including APACHE IV score, expected mortality and hospital mortality from our database. The study endpoint was hospital mortality.

During the study period a total of 4,492 patients were admitted to the ICU, 4,280 of whom were admitted only once while 212 patients were admitted more than once during the same hospital admission (Table 1). From table 1 we learn that hospital mortality is twice as high in patients readmitted to the ICU as compared to
Table 2 shows that readmission to the ICU is an independent risk factor for hospital mortality.

patients with only one ICU admission (25.5% vs 12.0%). This might be explained by the fact that patients readmitted to the ICU were sicker at the first ICU admission (mean APACHE IV score 75.5 vs63.4, mean expected hospital mortality 28.3 vs 16.2%). To correct for illness severity on admission we performed logistic regression analysis (Table 2).

Niet-operatieve behandeling van een door een thoraxdrain geïnduceerd letsel van de arteria pulmonalis Introductie Het inbrengen van een thoraxdrain is een frequente procedure op de spoed-eisende hulp(SEH), operatiekamer, intensive care (IC) en soms zelfs pre-hospitaal bij thoraxtrauma maar heeft mogelijke complicaties als letsel van het diafragme, longparenchym, intercostale arterie of zelfs perforatie van het hart. Insertie in de arteria pulmo nalis is een zeer zeldzame maar levens bedreigende complicatie. We beschrijven een casus welke conservatief zonder operatie succesvol is behandeld.
Casus Een 63-jarige patiënt presenteerde zich op de SEH met een septische shock en somnolentie. Zes maanden eerder was een pT1N0M0 adenocarcinoom van de rechter bovenkwab van de long verwijderd (ontdekt bij analyse voor aortakle stenose en mitraalkle- insufficentie) gevolgd door een aortaklepvervanging. Mitraal klep-plastiek was niet mogelijk door verklevingen na de lobectomie. Intubatie en opname op de IC volgde en bij X-thorax werd vocht rechts gezien dat bij proefpunctie pus bleek te zijn. Zonder voorafgaand verder onderzoek gezien de septische situatie
van patiënt werd een 16F thoraxdrain rechts ingebracht waarbij direct 2000 ml bloed afliep en patiënt hypotensief werd. De drain werd afgeklemd en patiënt werd succesvol geresusciteerd. Verdere analyse met een CT-thorax toonde de thoraxdrain gelegen in de resterende rechter onderkwab (met hierin pneumonie) en op gekruld in de arteria pulmonalis Theoretisch zou de oplossing zijn om via een thoracotomie de drain te verwijderen en het empyeem te behandelen. Gezien de beperkte longfunctie-reserve kon patiënt echter geen eventuele rest-pneumo nectomie aan. Gezien de bekende verklevingen leek een thoracotomie met centrale exploratie en afklemmen van de art. pulmonalis sowieso al een uitdaging. In multidisciplinair overleg werd besloten de drain dagelijks stapsgewijs terug te trekken met 2-3 cm. Op de 4e dag werd de drain zo definitief verwijderd zonder bloedverlies. Op de 5e dag kon patiënt gedetubeerd worden. De naar inmiddels bleek met Staphilococcus Aureus geïnfecteerde thoraxholte werd succesvol antibiotisch behandeld met Flucloxaciline en Ceftriaxon en patiënt kon op de 21e dag ontslagen worden. Dexamethason induced Pneumocystis carinii pneumonia
Introduction A 73-year old male presented to the emergency department with fever since 2 days. There were no chills or specific symptoms except for some dyspnea. Recently he had been diagnosed with cerebellar metastasis of an unknown primary tumour. This was treated with dexamethasone 8mg/day with response regarding his headache and disturbed gait. In the next six weeks dexamethasone was tapered to 3mg/day. Physical examination showed a respiratory rate of 30/minute. His blood pressure was
125/70 mmHg, the pulse 100/minute and temperature 38.4C. A chest X-ray revealed interstitial infiltrates which was confirmed by chest CT-scan showing bilateral, predominantly basal consolidations with surrounding ground glass. An atypical pneumonia was con sidered most likely, especially because patient had a parrot at home. Doxycycline was started. A broncho-alveolar lavage (BAL) was performed during admission to rule out Pneumocystic jirovecii pneumonia (PCP) after which the patient was dis charged as he improved and was afebrile.

Three days later, he was readmitted because of progression of dyspnea and recurrence of fever. PCP was now strongly considered; high dose trimethoprimsulfamethoxazole was started. Because of progressive respiratory failure, the patient was transferred to the Intensive Care Unit for non-invasive ventilation. He died a few days later. The PCR as well as specific stainings of the BAL were positive for PCP. An HIV-test was negative.
Discussion Systemic glucocorticoid therapy is associated with dose-dependent increased risk of infection, especially common pathogens. Opportunistic infections, like PCP, are less common, and are mainly a concern in patients taking other immunosuppressive agents or with diseases causing immunocompromise. Glucocorticoid use in the absence of any other form of immunosuppression seldomly causes PCP. However, when given to old people at high dose for extended period, as in this patient, PCP should always be considered in case of fever and respiratory symptoms.

Levamisol in cocaïne: onverwachte oorzaak van agranulocytose
Heden ten dage bevat ongeveer 64% van cocaïne in Nederland levamisol1. Dit middel, immuunmodulerend en een anthelminthicum, is in het verleden gebruikt in de behandeling van reumatoïde arthritis en als chemotherapeuticum bij onder andere coloncarcinoom. Wegens het optreden van ernstige bijwerkingen, waaronder het ontstaan van agranulocytose, is het middel in het begin van deze eeuw van de markt gehaald. Het gebruik van levamisol als versnijdingmiddel bij cocaïne is de afgelopen jaren sterk toegenomen. Deze trend is wereldwijd omdat het middel in de bronlanden reeds wordt toegevoegd. De reden van het versnijden van cocaïne met levamisol berust op de werking: het nabootsen of versterken van het cocaine-effect. Daarnaast spelen de lage kosten van het middel en de chemische eigenschappen een rol: het is wit van kleur en smelt op gelijke temperatuur waardoor het nauwelijks op te merken is.

Deze casus betreft een 56-jarige man die anamnestisch reeds 45 jaar cocaïne snuift. Hij werd opgenomen vanwege een necrotiserende pneumonie van de rechter bovenkwab, mogelijk op basis van een Enterobacter cloacae (figuur 1). Het bloedbeeld toonde een agranulocytose. A female with extremely painful skin lesions: what is your diagnosis? Introduction Fenprocoumon is a commonly used anticoagulant. We describe a patient who presented with a rare but very serious, cutaneous side effect.

A 43-year-old obese female with a history of unprovoked deep vein thrombosis as well as an episode of thrombophlebitis presented at the emergency department with extremely painful skin lesions. Her family history was notable with thrombosis associated with factor V Leiden mutation and prothrombin gene-mutation. Recently, she developed a second episode of thrombophlebitis and low molecular weight heparin (LMWH) as initial treatment was prescribed. Four days after stopping LMWH and starting Fenprocoumon she developed areas of dark discoloration surrounded by an erythematous lesion on both thighs as well as on the skin of the epigastric region and the lower abdomen (picture 1). The pain was accompanied by paraesthesia. Haemorrhagic blisters or clear signs of gangrene were not present. An initial concern regarding a possible hematoma or abscess was dismissed because of the absence of trauma, fever and/or increased inflammatory parameters. Laboratory investigation showed normal platelet count, prothrombin time 49.7 seconds (ref range: 9.0-12.0 sec), activated partial thromboplastin time 32 seconds

(24-34 sec), factor V activity 120 %. Because the diagnosis “coumadin induced skin necrosis” was considered, the patient was admitted and the Fenprocoumon was stopped immediately. Vitamin K was administered for ten days. The pain resolved over three days, the patient was treated with non-steroidal antiinflammatory drugs. All lesions resolved after four weeks during follow up in the outpatient clinic. The level of protein C and S turned out to be low, respectively 68 % (70-140%) and 48 % (60-140 %). This increases the susceptibility to this complication.
Discussion Skin necrosis is caused by a localized hypercoagulable state due to the initiation of vitamin K antagonists as these drugs inhibit both the synthesis of vitamin K-dependent clotting factors (factors II, VII, IX and X)as well as the production of natural anticoagulant protein C and S. As a consequence of a shorter half-life than the clotting factors, rapid depletion of protein C and S leads to a relative procoagulant state during the initial 24 to 48 hours of Fenprocoumon which in susceptible individuals may lead to thrombotic occlusions of the micro vasculature with subsequent skin necrosis. Therefore, it is important to identify the warning signs. If the skin necrosis is identified and treated early on, like in our case, the affected tissue usually heals well. Otherwise, noticeable scars can form.

Een patiënt met hepatitis C genotype 2k/1b
Inleiding Het hepatitis C virus (HCV) is een enkelstrengs DNA virus met grote genetische heterogeniteit en wereldwijd geschat 170 miljoen dragers. Therapie voor HCV heeft de laatste jaren een enorme vlucht genomen. Therapieregimes worden voornamelijk gebaseerd op genotype, deze casus onderstreept het belang hiervan.
Casus Een 47-jarige Russische man, in de voorgeschiedenis depressieve klachten, met bij presentatie leverchemieafwijkingen (AST150, ALT250) en bij echografie hepatomegalie zonder tekenen van portale hypertensie. Virusdiagnostiek bevestigde chronische HCV genotype-1b (sequentieanalyse NS5b), virusRNA 6,5 log(10) HCV IU/ ml. Leverbiopsie toonde gevorderde fibrose en verdenking cirrose. Op dit moment bestond indicatie tot behandeling van HCV. Gezien de psychiatrische voorgeschiedenis en bijwerkingen van PEG-interferon werd besloten tot behandeling in studieverband met PEG-interferon vrije therapie.
Bij aanmelding voor de HCVerso studie (Fase III trial Faldaprevir /Ribavirine) werd genotypering extern herhaald. Aldaar bleek er sprake van genotype 2a/2c. Duidelijk werd dat het externe laboratorium een ander gen voor genotypering gebruikt resulterend in genotype 2a/2c (LiPA HCV). Deze discrepantie wordt gezien bij een recombinant HCV 2k/1b. Dit specifieke genotype is uitsluitend beschreven in patiënten met Russische achtergrond. Ongeschikt voor de HCVerso studie werd patiënt behandeld als genotype 1b met PEG-interferon, Ribavirine en Telaprevir.
Conclusie Deze casus toont het belang van nauw keurige genotypering bij HCV. Foutieve genotypering kan lijden tot onderbehandeling en een slechtere uitkomst voor de patiënt. Het is van belang kennis te nemen van de in het ziekenhuis gebruikte tests en de beperkingen hiervan. Bij HCV patiënten met een Russische achtergrond dient men bedacht te zijn op recombinant HCV 2k/1b.

Afwijkende vervormbaarheid van erytrocyten met een hemoglobinopathie
Inleiding Erytrocyten van patiënten met erfelijke hemolytische anemieën zijn zwak en hebben een verkorte levensduur. De verminderde vervormbaarheid van zwakke erytrocyten is in een ektacytometer meetbaar. Optimale flexibiliteit is essentieel voor een functionele erytrocyt in de kleine haarvaten. De zeefwerking in de milt is mede hierop gebaseerd. Oude cellen zijn minder vervormbaar en niet in staat om de poriën van de sinussen te passeren en worden weggevangen. In onze studie wordt de relatie tussen de vervormbaarheid, het type mutatie en de ernst van de anemie bij hemo globino(Hb)-pathieën onderzocht.
Methode Meting vervormbaarheid van erytrocyten in een ektacytometer met de maximale Deformabiliteit Index (DI-max) als maat. DNA analyse en capillaire elektroforese. Patiënten met een Hb-pathie, die geen bloedtransfusie hebben gehad.
Resultaat Cellen van α-thalassemie patiënten met één of twee α-globine deleties hebben een grotere DI-max dan cellen met deleties

van drie α-globine allelen (HbH). Heterozygote β-thalassemie cellen hebben een betere vervormbaarheid / grotere DI-max dan intermediaire β-thalassemie cellen. Bij sikkelcellen is de vervormbaarheid van de dragers (AS) minder aangedaan dan bij homozygote SS. Een toenemende hoeveelheid HbF bij SS-cellen zorgt voor een betere vervormbaarheid.
Conclusie Cellen van patiënten die doorgaans een milde anemie hebben, waaronder één of twee α-deleties, heterozygote β-thalassemie en AS-drager, laten een verlaagde maar relatief kleine afwijking in hun vervormbaarheid zien. Mutaties die een ernstige anemie kunnen veroorzaken, waaronder HbH, intemediaire β-thalassemie en SS, hebben een sterk verminderde vervormbaarheid. Een verhoogd HbF heeft bij SS patiënten een klinisch gunstig effect op het sikkel proces en is meetbaar als een verbeterde vervormbaarheid. De ektacytometer is een analyser die de conditie van de erytrocyten binnen verschillende vormen van Hb-pathie goed kan bepalen.

Leeftijdsafhankelijke afkapwaarden voor de d-dimeer test bij verdenking op een veneuze trombo-embolie – zo klaar als een klontje? waarde van 500 µg/l fibrinogeen equiva lente eenheden (FEU) vallen. Een recente, retrospectieve analyse suggereert dat leeftijdsafhankelijke afkapwaarden (LAAW) leiden tot vermindering van vervolgonderzoek (CT-thorax of compressie-echografie) zonder noemenswaardig verlies aan sensitiviteit (>50 jaar dan afkapwaarde = 10 µg/l FEU * leeftijd).1 Een multidisciplinaire inventarisatie (hematologie, vasculaire geneeskunde en klinische chemie) binnen het Haga ziekenhuis was geïndiceerd.
D-dimeer assays worden toegepast in combinatie met een lage Wells score om veneuze trombo-embolieën (VTE) uit te sluiten bij patiënten. Echter, d-dimeer concentraties stijgen met de leeftijd waardoor oudere patiënten regelmatig ‘onterecht’ boven de aanbevolen afkap

In het HagaZiekenhuis worden zo’n 1300 d-dimeer testen per jaar verricht waarbij 55% van de resultaten >500 µg/l FEU zijn (Figuur 1). Bij LAAW zou 46% positief zijn, een theoretische reductie van maximaal 100 vervolgonderzoeken per jaar (d-dimeer test wordt immers ook gebruikt voor andere indicaties). Bij patiënten >80 jaar is een evidenter effect (91% vs. 70% positief) zichtbaar. Echter, deze hoge percentages geven ook het belang aan van een juiste

indicatiestelling. Verder is het effect van LAAW op een klinisch relevante populatie bekeken met twee gerenommeerde d-dimeer testen, Tina-quant en VIDAS (Tabel 1). Door gebrek aan standaardisatie zijn er grote verschillen tussen de testen: invoering van LAAW zou bij de Tina-quant zelfs tot (onacceptabele?) verlaging van de sensitiviteit kunnen leiden. Onze inventarisatie laat zien dat LAAW zeker (nog) niet onomstreden zijn. Zo is een, bij voorkeur, prospectieve studie met de ‘eigen’ d-dimeer test gewenst om te beoordelen of met LAAW op een veilige wijze VTE kunnen worden uitgesloten. Verder blijft, zeker bij ouderen, een juiste selectie vóór het aanvragen van eminent belang. 1 Schouten et al. Changes in antimicrobial susceptibility in adult CF patients with chronic Pseudomonas aeruginosa infection treated with daily inhalation of tobramycin or colistin Chronic lung infection with Pseudomonas aeruginosa is the most common bacterial infection in patients with Cystic fibrosis. Inhalation of antibiotics is a standard treatment. Early studies already showed a decline of susceptibility to tobramycine after treatment with inhaled tobramycin in an alternate month cycle. Tobramycin for inhalation is prescribed in our centre for continuous use > 10 years. We hypo thesize that this treatment does not alter susceptibility of P. aeruginosa to tobramycine.
Objective To test the susceptibility of P. aeruginosa isolates in CF patients after continuous inhalation of tobramycin over 3 to 5 years. Susceptibility at the start of this period and at the end will be compared.
Methods In this prospective study with retrospective control measurements we collected sputum samples over 3 to 5 years. All samples collected from 60 CF patients with chronic P. Aeruginosa infection

and treated with Tobramycin or Colistin inhalation therapy. For the analysis of the sputum samples we used the high-range Etest strips. We applied the MENSURA criteria for predicting the susceptibility to tobramycine. All measurements are performed in our microbiology department.
Results From 53 patients sputum samples were collected. Early analyses of data show no alteration of the susceptibility of P. Aeruginosa. Mean MIC for tobramycin changed from 21,4 in year one to 14,1 in year five. Analysis of intra-individual changes of MIC is conducted at this moment.
Conclusion Early analysis showed no increase of MIC for tobramycine in patients treated continuously with inhalation antibiotics. We feel that further analysis of data will support our hypothesis.

Temporal bone pneumatization in Cystic Fibrosis related to genotype?
Paranasal sinus pneumatization in patients with Cystic Fibrosis is smaller compared to the general population and is correlated to CF genotype. Interestingly, in CF patients temporal bone pneumatization (TBP) is larger compared the general population. It is debated whether TBP is influenced environmentally or genetically.

The aim of the present study was to investigate TBP in patients with CF and to correlate TBP with genotype and paranasal sinus pneumatization.
Temporal bone pneumatization in CF patients is not related to CF genotype or to paranasal sinus volume. Hypothetically pneumatization of the temporal bone is under a different, possibly genetic, influence than the paranasal sinus pneumatization.
Methods
Of the 31 patients with mild CF 71% had large TBP and of the 73 patients with severe CF 82% had large TBP. TBP did not differ significantly for CF genotype. Moreover no correlation between TBP and paranasal sinus volume was found.
In 104 adult CF patients a CT scan of the temporal bone and the paranasal sinuses was performed. Temporal bone pneumatization was graded using a validated scoring system and paranasal sinus volume was measured. Patients were divided into two groups, mild and severe CF, based on their CFTR mutations.

In patient 1, a female of 32 years old, the maximum concentration (Cmax) of tobramycin was 0.027 mg/L. This Cmax was reached 30 minutes after the nasal lavage with tobramycin (tmax). In total 0.20% (0.62 mg) of the tobramycin was systemically absorbed. In patient 2, a male of 36 years old, the Cmax was 0.029 mg/L. The tmax was 45 minutes and in total 0.16% (0.51 mg) of tobramycin was absorbed.

Methods
Recent studies showed that the paranasal sinuses can constitute a niche for bacteria. To date no effective treatment for these bacteria is available. Off label administra tion of nasal antibiotics may be an option. However, first safety of this treatment has to be established.

Conclusions Nasal lavage with 320 mg tobramycin did not result in toxic serum levels. The results of two patients showed a fast absorption of tobramycin and a slow elimination. Approximately 0.20% of the tobramycin was absorbed by the sinonasal mucosa.

Nasal airway resistance in adult patients with Cystic Fibrosis
Body plethysmography is used to measure airway resistance. The airway resistance can also be measured through the nose resulting in a nasal airway resistance (NAR). NAR in patients with CF can be increased because of the high prevalence of rhino sinusitis and nasal polyps.
Objective Investigation of nasal airway resistance in adult patients with Cystic Fibrosis.
Methods In 104 adult patients with CF, NAR was measured using body plethysmography. The subject was placed in a body box with a CPAP mask covering mouth and nose. Three measurements followed; breathing through the mouth (Raw), breathing through the nose (Rn) and a second measurement through the nose after administration of the decongestant xylometazoline HCl 0.1% (Rnxylo). Nasal airway resistance was calculated by: Rn-Raw. The effect of xylometazoline was calculated by: Rn-Rnxylo.
Results The mean age of 104 patients was 34.8 years (range 19-63) and 52.9% was male. Since no normal values exist for NAR, only results from within one patient were assessed. Surprisingly, NAR had a negative value in 17 patients, indicating that the resistance through the mouth was higher than through the nose. The NAR after xylometazoline had a negative value in 39 patients. These results could indicate the absence of a decongesting effect of xylometazoline. But more important, these negative results could also indicate a limited value of body plethysmography in measuring NAR in CF patients.
Conclusions The results of measuring NAR in CF patients were difficult to interpret. Measuring NAR using a body plethysmograph seems to have limited value in routine care of CF patients.

Prevalence of bronchial colonisation and postoperative pulmonary complications in major non-infective pulmonary surgery Introduction Bacterial colonisation of the lungs is frequently reported in patients with lung cancer and is thought to have a potential impact on postoperative complications and prognosis. Bacterial colonization may be a result of local bronchial obstruction or as a cause of immunosuppression or smoking. This study aimed to define the prevalence of bronchial colonisation and possible association with infectious complications after major pulmonary surgery in our hospital.
Methods Between January 2009 and January 2011 a total of 114 patients underwent surgery
for non-infective pathology and were included in this retrospective analysis. Postoperative Pulmonary Complications (PPC) were scored using the Melbourne Group Scale (MGS).

Out of the 114 surgery’s, 73 patients underwent surgery for lung cancer, and 38 for other indications (pneumothorax, diagnostics). 39 patients had a PPC, of which 26 patients had Hospital acquired Pneumonia or Empyema. The other 13 patients had woundinfections (1), urinary tract infections (3), bronchitis (6) and febris e.c.i. (3). Mortality rate was 0,88 % (1 patient). Preoperative bacterial coloni sation was with 31,5 % poorly documented.

Conclusion Pulmonary infections remain a serious medical complication following thoracic surgery. In this retrospective study we found a complication rate of 34 %, which is comparable to documented rates in previous studys. This study does not provide enough information to make a statement about prevalence of bacterial or an association with developing PPC.

The impact of sexual abuse in patients undergoing colonoscopy A multicentered survey study Background Sexual abuse has been linked to strong effects on gastrointestinal health. Colonoscopy can provoke intense emotional reactions in patients with a sexual abuse history and may lead to avoidance of endoscopic procedures.
Objective To determine whether care around colonoscopy needs adjustment for patients with sexual abuse experience, thereby exploring targets for the improvement of care around colonoscopic procedures.
Methods Questionnaires were mailed to patients (n=1419) from two centers within 11 months after colonoscopy. Differences in experience of the colonoscopy between patients with and without a SA history were assessed and patients’ views regarding physicians’ inquiry about sexual abuse and care around endoscopic procedures were obtained.
Results
Patient born in a non-western country (n=67) reported more sexual abuse: 17.9% of 39 females and 10.7% of 28 males. Discomfort during colonoscopy was indicated on a scale from 0 to 10, mean discomfort rate of patients with sexual abuse experience was 4.78(±3.47) versus 3.54(±3.11) in patients that did not report sexual abuse (p=0.007). Of patients indicating to have experienced sexual abuse, 53.8% believed the gastroentero logist should ask about sexual abuse and 43.4% said they would be helped with deeper sedation during colonoscopy.
Conclusions Sexual abuse is prevalent in patients presenting for colonoscopy and these patients experience more discomfort during the procedure. Special attention for sexual abuse patients during colonoscopy could diminish their distress.

High frequency of additional cerebral involvement in adrenomyeloneuropathy X-linked adrenoleukodystrophy (X-ALD), is a devastating inherited disorder of peroxisomal metabolism caused by mutations in the ABCD1 gene, resulting in damage to central and peripheral nervous system and endocrine organs. For decades, adrenomyeloneuropathy (AMN) was considered a relatively mild phenotype with only accidental additional cerebral demyelination in contrast to childhood cerebral adrenoleukodystrophy (CCALD). In 2001 it was reported that 13/68 male patients with AMN (19.1%) developed additional brain involvement during a follow-up interval of 9,5 +/- 5,5 years. We studied the risk of developing additional cerebral demyelination in the Dutch cohort of AMN patients.
Methods consecutive male AMN patients from the Dutch X-ALD cohort without cerebral demyelination, visiting the Amsterdam Medical Center between January 1st 1992 and January 1st 1999, were included. Primary endpoints were demonstration of brain involvement, death, or the end of our study. Three levels of certainty were used to demonstrate cerebral demyelination; confirmation by MRimaging, detailed information from other threating physicians, or information

obtained from their families. Results were compared with a study carried out at the Kennedy Krieger Institute, Baltimore, MD, in 2001, using 95% confidence interval of the difference between the studies.
Results 27 AMN patients were included, 17 (63%) of them developed additional cerebral demyelination with a mean interval between onset of symptoms due to myelo(neuro)pathy and brain involvement 10.2 ± 6.9 years. The mean survival after demonstration of cerebral demyelination was 3.4 ± 2.9 years. additional brain involvement was higher in the Dutch AMN cohort than in the study published in 2001 (difference 44%, 95% CI of the difference 0.23 – 0.64).
Conclusion Or study confirms that the incidence of additional cerebral demyelination in AMN patients much higher than previously reported. Survival is as poor as in the boys with CCALD. Therapies that can halt the frequently occurring relentlessly progressive cerebral demyelination in AMN patients are badly needed.

Mimics of acute subdural hematoma in the emergency department Introduction Diagnosing the true etiology of a subdural fluid collection is challenging in the acute setting of an emergency department. In case of a subdural fluid collection at CT - imaging, the possibility of a subdural hematoma (SDH) will be considered at once. However, other causes must be considered. Three patients who presented with a history of progressive headache, aphasia and a right-sided hemiparesis in whom a isodense subdural fluid collection was demonstrated with CT-imaging. Initially, all three patients were diagnosed with a SDH. However, the final diagnosis turned out to be different.
Cases
breast cancer are the most frequent ones1,2. Subdural empyema mostly evolves from paranasal sinusitis. Other causes are meningitis, ear nose throat or dental infections, postsurgical complications or bacteremic seeding of a previous SDH3,4.
Conclusion Both the clinical as well as the radiological picture of Pachymeningitis hemorrhagica interna with dural metastases and subdural empyema can mimic those of a SDH. In case of doubt Gadolinium-enhanced MR Imaging should be performed.

Results
Utilization is defined as an executive function disorder in which patients grab or use objects and use it appropriately but in the inappropriate setting. In utilization there is a difficulty to resist the impulse to manipulate or use the object. It may indicate impaired frontal functioning. We investigated a simple utilizationtest by offering sunglasses. We questioned if utilization of sunglasses is only seen in cognitive impaired patients or also in cognitively healthy controls.
Of the 50 control (cognitively healthy) patients not a single patient put on the offered sunglasses (50 out of 50 with a negative test). Of the 50 patients with cognitive complaints 30 had a positive test. The mean MMSE of the 30 patients who put the glasses on was 21.9 (out of 30). Follow up of the 30 patients who had a positive test showed that 28 had objective cognitive dysfunction (most frequent Alzheimers disease). In the other 20 patients with a negative test the mean MMSE was 26.3 (out of 30). Of these 20 patients who did not put the glasses on 7 appeared to have cognitive dysfunction.
Methods We tested utilization in 50 patients in our neurology out patient ward who were referred for cognitive problems (‘cognitive patients’) and we compared them with 50 patients who were referred for other neurological complaints (‘general patients’). We tested utilization by offering sunglasses without any instructions. When the patient put on the sunglasses, the test was considered positive. When the patient ignored the glasses, asked what to do or otherwise responded in an adequate way the test was considered negative. All cognitive patients had a general work-up including an Mini Mental State Examination (MMSE), MRI-cerebrum and neuropsychological.

Sensitivity of the sunglass test to detect cognitive dysfunction is 80% and specificity is 97%.
Conclusion The sunglass test is a simple test to screen for utilization and cognitive dysfunction. Patients who put them on are more likely to have cognitive dysfunction and further investigations may be warranted.

Sensitiviteit en specificiteit van de K-SNAP voor dementia screening In de wetenschappelijke literatuur worden diverse cognitieve screeningstests gebruikt voor het screenen van dementie. Bij de meeste tests ontbreken gegevens over de sensitiviteit en specificiteit of de normering laat te wensen over. Wij hebben retrospectief onderzoek uitgevoerd met de Nederlandstalige Kaufman-Short Neuropsychological Assessment Procedure (K-SNAP). Wij vergeleken de prestaties van patiënten met een dementiesyndroom en Mild Cognitive Impairment (MCI) met die van gezonde leeftijdsgenoten (de normgroep van de test). De patiënten zijn in dit ziekenhuis onderzocht bij de Geheugenpoli. De K-SNAP bestaat uit een eenvoudige Mentale Status, een visuele perceptietest, een aandachtstest en een planningstest. Er worden 4 subtest scores berekend en een maat voor algeheel cognitief functioneren en een maat die aanduidt of verder onderzoek gewenst is.
Uit de statistische analyses bleek dat de patiënten met de ziekte van Alzheimer (n = 91) en patiënten met een gemengde dementie (n = 13) op bijna alle tests en beide indices slechter presteerden dan de gezonde ouderen (n = 386) en MCIpatiënten (n = 34). De MCI-patiënten en de patiënten met een frontotemporale dementie (FTD, n = 21) presteerden beter dan de Alzheimer patiënten, maar slechter dan de controlegroep (met name op de planningstest). De sensitiviteit van de subtest en indices varieert per dementiesyndroom: de beste resultaten worden gevonden voor de mentale Status en de planningstest, bij de ziekte van Alzheimer en de gemengde dementie. Bij FTD is de sensitiviteit lager. Deze bevindingen betekenen dat men de K-SNAP niet als enige test kan gebruiken bij dementieonderzoek.

Postoperatieve pijn na verschillende schouderoperaties: een prospectief beschrijvende studie Doel

Conclusie
Ervaren vrouwen ouder dan 50 met een laag normaal BMI (BMI 18-25) hogere postoperatieve pijn na schouderoperaties dan vrouwen ouder dan 50 met een hoog BMI (BMI>25)? Secundair wordt post operatieve pijn na verschillende schouderoperaties vergeleken. Risicofactoren voor het optreden van ernstige postoperatieve pijn worden onderzocht.
Gemiddelde pijnscore is postoperatief lager dan preoperatief. Dag 1 postoperatief is een pijnterugslag te verwachten, waarna de pijn steeds verder afneemt. Tussen vrouwen ouder dan 50 met een laagnormaal BMI en vrouwen ouder dan 50 met een hoog BMI is geen significant verschil. Verschillende operatiegroepen geven geen significant verschil in post operatieve pijn in de eerste 3 weken postoperatief. Mannelijk geslacht en cuff repair operaties hebben een grotere kans op het ontwikkelen van ernstige post operatieve pijn.
Er is geen significant verschil in post operatieve pijn tussen vrouwen ouder dan 50 met een laagnormaal BMI en hoog BMI. Ook tussen de verschillende operatie groepen is geen significant verschil. Mannelijk geslacht en cuff repair operaties geven een verhoogde kans op het ontwikkelen van hoge postoperatieve pijn.

Patiënten ouder dan 18 die een schouderoperatie ondergaan worden gevraagd mee te doen. Er zijn 167 patiënten geïncludeerd. De verschillende operaties zijn onder verdeeld in 4 groepen; rotator cuff repairs, schouderinstabiliteit, AC-gewrichts ingrepen en overige patiënten. Pijnscores worden gedurende de eerste week elke dag en op dag 14 en 21 middels een Numeric Rating Scale (NRS-score) bijgehouden. Patiënten met een compli catie of taalbarrière werden geëxcludeerd. Leeftijd, BMI, operatieduur, geslacht, soort anesthesie worden onderzocht als mogelijke risicofactoren.

Het gebruik van Duel-mobility cups (avantage) bij heuprevisie
Inleiding Luxaties zijn een veelvoorkomend probleem na revisie van een totale heup prothese. Incidentiecijfers van 8% en hoger worden beschreven. De dual-mobility cup wordt in de literatuur geprezen vanwege zijn lage dislocatie aantallen. De primaire uitkomstmaat voor dit onderzoek was het aantal luxaties die optraden na revisie chirurgie met plaatsing van de dualmobility cup. Daarnaast werd gekeken naar kwaliteit van leven. Dual-mobility cup (producent: Biomed)
Methode Alle patiënten die een dual-mobility cup kregen tijdens een heuprevisie werden geïncludeerd. Onderzocht werd of een luxatie na plaatsing van de dual-mobility cup had plaatsgevonden. Reden van revisies werd genoteerd. Patiënt tevredenheid werd gescoord middels de Oxford heupscore, SF-12 en VAS pijnscore
37 patiënten bleken inmiddels te zijn overleden. De eerste 39 respondenten gaven op de vragenlijst een Oxford heupscore van gemiddeld 75 (range 17-100) aan. Een gemiddelde SF-12 (fysiek) van 39,28 (range 16,6-56,6) en (mentaal) van 54,8 (range 26,2-70,5). Een gemiddelde VAS score van 2 (range 0-9). Bij 3 patiënten (1.6%) werd na plaatsing van de dualmobility cup een revisie uitgevoerd vanwege loslating van de cup. In 1 geval bleef de heup luxeren na plaatsing van de cup door volledig ontbreken van stabiliserende musculatuur.
Conclusie Dual-mobility cups geven de heup goede stabiliteit en een lage kans op luxaties op de midden-lange termijn. Daarnaast is de patiënt tevredenheid gemiddeld hoog Verdere follow-up is noodzakelijk om te evalueren of luxaties en loslating van het component niet vaker zullen optreden.
Resultaten
AP opname na plaatsing van de dualmobility cup
186 patiënten werden geïncludeerd met een gemiddelde follow-up van 50 maanden (range 10-82). Reden voor revisies was: loslating (72%), infectie (7%), luxaties (21%).

Combining femoral and acetabular morphology in femoroacetabular impingement: the Omega Zone Background Classification of hip morphotypes in femoral acetabular impingement (FAI) is presently done with separate parameters for acetabular and femoral morphology on planar images. Correlation between these isolated parameters and symptoms or development of degenerative changes appears is weak. Acetabular morphology may to some degree “compensate” femoral deformities and vice versa, e.g. minor acetabular dysplasia can decrease or delay impingement of the head-neck junction with the acetabular rim. Further, proximal femoral parameters such as version and neck-shaft angle also influence femoral head position within the acetabulum and thereby impingement.
software (Articulis™). Combining alpha and center-edge angles, acetabular and femoral version and neck-shaft angle, we defined a novel parameter in range of motion of the femoral head (Omega zone) that characterizes the severity of FAI.
Results The Omega zone was significantly smaller in patients with FAI, but could also differentiate between symptomatic and symptom-free FAI.

The omega zone differs between groups with signs of FAI with or without complaints. Symptomatic FAI patients have a larger impingement region that occurs earlier in flexion.

Zesde extensor loge herstel van de pols: Indicatie, behandeling en resultaten Doel
Resultaten
Het beschrijven van de indicatie, operatieve techniek en resultaten van de operatieve correctie van de zesde extensor loge van de pols met behulp van een vrije retina culum flap.
Er zijn 6 mannen en 8 vrouwen geïncludeerd. Ten tijde van de ingreep was de gemiddelde leeftijd 45 jaar (uitersten: 26 - 63). Bij een mediane follow-up van 26 maanden (uitersten: 13 - 36) werd een hoge patiënttevredenheid gerapporteerd, met een mediane VAS van 8. De mediane PRWHE pijnscore was 9,5/50 en mediane algemene DASH en PRWHE scores waren respectievelijk 7,87/100 en 18/100. Betreffende de beweeglijkheid werd een significante vermindering in supinatie gevonden. Daarnaast was er geen verschil in knijpkracht tussen de aangedane en gezonde extremiteit. Er werden geen complicaties gemeld.
Opzet Retrospectieve en beschrijvende studie
Methode Gegevens zijn verzameld van 13 patiënten met klachten van subluxatie van de extensor carpi ulnaris (ECU), die tussen 2010 en 2012 een operatieve reconstructie van de zesde extensor loge zijn ondergaan. Gegevens zijn verzameld uit de statussen van de participanten. Alle patiënten kregen postoperatief enkele gestandaardiseerde en een tevredenheidvragenlijst toe gestuurd waarmee het functioneren van de aangedane extremiteit en de patiënt tevredenheid in kaart kon worden gebracht. Tevens zijn de knijpkracht en beweeglijkheid van de pols in kaart gebracht tijdens een poliklinische controle afspraak. Er waren 6 patiënten die niet voor controle konden komen.

The Direct Anterior Approach: Beyond the Learning Curve Recently, total hip arthroplasty (THA) with patient in supine position has become more popular. The supine direct anterior approach (DAA) combines a stable pelvic position with a muscle sparing technique, were other current used approaches in the lateral decubitus position, involve transection of tendons and/or muscles. In the Haga hospital we have been performing the DAA for more than 5 years. The purpose of this study was to report the short-term results of the DAA without traction table “beyond the learning curve”. Specifically we examined intraoperative parameters (operating time, blood loss) and parameters during hospital stay (day of mobilization, length of stay). Further, we examined the overall complication rate and pre- and one year postoperative OXFORD hip scores. All patients scheduled for primary THA through the DAA in 2011 in the HAGA hospital were included retrospectively. 561 total hip prostheses were placed in 555 patients in 2011.

Mean operating time was 74 minutes (SD 20, range 26 - 145). Mean intraoperative blood loss was 442 mL (SD 288, range 30 2000). 95% Of the patients started mobilizing within one day after surgery. Mean length of hospital stay was 3.7 days (SD 1.8, range 1-16). Overall orthopedic related complication rate was 4.6%. One year after surgery the mean OXFORD score (87 SD 16, range 0 – 100) showed significant improvement compared to the pre-operative OXFORD score (59 SD 17, range 0 – 92). We conclude that, once through the learning curve, the DAA is a reliable approach with peri-operative outcomes that are not inferior to outcomes from widely accepted approaches.

Where is the Neck? Alpha angle measurement revisited Background and Purpose
Results
The alpha angle is the most used measurement to classify concavity of the femoral head-neck junction. It is not only used for treatment decisions for hip impingement, but also in cohort studies relating hip morphology and osteoarthritis. Alpha angle measurement requires identification of the femoral neck axis, the definition of which may vary between studies. Furthermore, depending on the method used, the alpha angle may or may not account for other morphological characteristics such as head-neck offset. We compared how two methods of alpha angle measurement ('anatomical' and 'three-point') characterize proximal femoral morphology.
Discrepancies up to 13 degrees were seen between the two methods. The three-point method had an “equalizing effect” by disregarding femoral head position relative to the neck: in femora with high alpha angles, it resulted in lower values than anatomical measurement, and vice versa in femora with low alpha angles. Using the anatomical method, a reference interval for the alpha angle of 30-66 degrees was derived.
Methods Alpha angles were measured using both methods, on 59 cadaver femora and 83 cross-table lateral radiographs of asymptomatic subjects, and results compared using Bland-Altman plots.
Interpretation

Anatomical method (A.2 and B.2) defines the femoral neck axis by connecting the centers of 3 circles projected over the neck contour. The axis is translated to the center of the femoral head if necessary, to measure the alpha angle. In this example alpha angle A.1 = 64˚, A.2 = 73˚. Angle B.1 and B.2 are both 30˚, while the femoral head is positioned central on the femoral neck.
We recommend the anatomical method because it also reflects the position of the femoral head on the neck. Consensus and standardization of technique of alpha angle measurement is warranted, not only for planar, but also for CT or MRI-based measurements.

Open reduction and internal fixation of an un-united longitudinal cuboid fracture: a case report Introduction
Case
Isolated cuboid fractures are rare and may result in considerable disability due to the involvement of the cuboid in all intrinsic movements of the mid- and hind foot. Cuboid fractures generally occur in the transverse plane. Only three longitudinal cuboid fractures (in the sagittal plane) have been reported before. The management and the outcomes of operative treatment of a non-union of a longitudinal cuboid fracture have not been described before.
A 53-year old man reported disabling pain at the medial side of his left ankle, three months after a supination trauma at work. Physical examination only revealed pain on palpitation at the medial malleolus and swelling at the lateral side of the foot. The radiologist described the radiographs and a MRI scan of the foot as normal. Finally, a bone scintigrafy revealed a non-displaced, non-united longitudinal fracture of the cuboid (Figure 1).

Surgery was performed eight months after the injury. To compress the fracture, three AO 4.0-mm cancellous screws were
inserted in the cuboid via the lateral approach (Figure 2). Their position was verified fluoroscopically, because direct vision on the fracture was not feasible. Finally, ten holes were drilled through the cuboid with a 1.0 mm Kirschner wire, to enhance fusion. Consolidation occurred and the screws were removed three months after surgery.
Outcome Three months after removal of the screws, the patient experienced considerable pain reduction, returned to his work and was able to walk several kilometers. His Foot Function Index was 21 (maximum disability 100).

Difference in postoperative stiffness and pain after arthroscopic rotator cuff repair: bio-absorbable versus non-absorbable implants Background
Results
Bio-absorbable suture anchors are frequently used in arthroscopic rotator cuff repair (ACR). However we experience more early postoperative stiffness (e.g. synovial reaction resulting in decreased external rotation) and pain compared to using Titanium- and PEEK-anchors. In this study we compare postoperative pain and clinical outcome following arthroscopic rotator cuff repair through a randomized controlled trial.
Fifty patients were analyzed in the preliminary evaluation (bio-absorbablegroup (n=18, age 59±12 years), titaniumgroup (n=18, age 58±12 years) or PEEKgroup (n=14, age 54±11 years). For the first 6 months postoperatively no significant differences were seen in the VAS or functional outcomes.
Methods ACR was performed in patients with a partial or complete tear of the supra spinatus / infraspinatus tendon. Patients were randomized to the bio-absorbablegroup, titanium-group or PEEK-group. Postoperative pain was measured using the Visual Analog Scale (VAS) daily in the first week and at 2, 3, 6, 12, 24 weeks. Functional outcome: external rotation (ER) glenohumeral abduction (GA), total active elevation (TAE), external rotation abduction (ERAB) and Oxford Shoulder Score (OSS) were measured at 2, 3, 6, 12, 24 weeks.

Conclusion The preliminary results showed no difference in postoperative stiffness between the groups at first six weeks to 6 months after arthroscopic rotator cuff repair. However, analysis of the complete data set is required to draw definite conclusions.

A comprehensive approach to decrease the infection rate in arthroplasty in a high volume orthopaedic ward Introduction Prosthetic joint infection (PJI) can be a devastating complication, with con siderable morbidity and high cost. We reviewed our peri-operative pathway for total hip and knee arthroplasty to further reduce PJI-rate.
Methods We reviewed best available evidence for strategies to reduce PJI-rate. A multidisciplinary task force decided which measures could be improved reliably, reproducibly and cost-effectively.
Results We identified four main areas for improvement. First, analysis of PJI revealed more than half was caused by microorganism’s resistant/ insensitive to the standard peri-operative antibiotic. Therefore high-risk patients (see below) now receive two antibiotics. Second, we identified 7 patient-related risk factors, of which revision surgery is the most important. All revisions and patients combining two or more risk factors now receive cutaneous disinfection 4 days preoperatively.

Third, all steps of total joint implantation were analysed for potential loss of sterility. Here, we introduced door-movement counters, stricter glove and suction tip changes, improved tissue and instrumenthandling, povidone-iodine wound lavage, patient-temperature registration and tranexamic acid to reduce wound bleeding. Fourth, wound dressing and handling were changed in the ward. Presentations were held about the reasons and measures of this project, called (reflecting our ambition) “the Zero Rules for Zero infections”.
Discussion Started in early 2013, we have as yet no data to support the effect on PJI-rate. We implemented all measures simul taneously because randomized studies of specific measures are not feasible, requiring thousands of patients. The effectiveness of “Zero Rules” will be assessed by comparing PJI-rate for cohorts treated with and without these measures.

An observational study comparing arthrodesis with hemi-arthroplasty of the MTP-1 joint in hallux rigidus patients Introduction
Results
Comparison of failure rates and long term results of the arthrodesis and hemi-arthroplasty of the first metatarsalphalangeal (MTP-1) joint in hallux rigidus patients.
No differences were found in failure rates (p=0.714) and time to failure (p=0.337). Five (9.4%) prosthesis operations failed at a median time of 12.1 months (3.5 – 72.3 months) and 19 (13.3%) arthrodesis failed with a median time of 8.4 months (3.7 – 84.0 months). The collected questionnaire data showed no significant differences between the groups.
Methods A retrospective study was conducted between January 2005 and March 2013 and included 140 arthrodesis patients (HAGA) and 50 Biopro MTP-1 hemi-arthroplasty patients (MCH). The primary outcome of the study, was failure. Failure in the prosthesis group was defined as having performed a second operation on the joint due to persistent pain or infection and in the arthrodesis group as having performed revision arthrodesis due to non union, improper toe position, or removal of plate because of infection. All patients were asked to complete questionnaires containing the Foot and Ankle Outcome Score (FAOS), the Foot Function Index (FFI) score, Numerical Rating Scale for pain (NRS) and binominal questions about satisfaction and if they would chose the operation again.
Conclusion Our study shows that the long term results for Biopro MTP-1 hemi-arthroplasty and arthrodesis are not different, but because the follow-up period is only median 36.6 months (4.7 - 96.2 months) in prosthesis patients and 39.3 months (5.4 - 155.7 months) in arthrodesis patients, further research is required to determine whether Biopro MTP 1 hemi-arthroplasty can be an alternative for MTP-1 arthrodesis in the long term.

The use of platelet rich plasma on self reported pain and function in patients with chronic elbow pain; a pilot study Background
Methods
Failure of conservative treatment to reduce pain and restore mobility in patients with chronic elbow complaints may lead to explore new technologies. Platelet rich plasma (PRP) is a new technology in which platelets and leukocytes are isolated and concentrated from patients own blood. Injection of these platelets and leukocytes on the injury side of the elbow provide a release of various growth factors that participate in tissue repair processes and promote the native healing process in the injured area. The aim of this study is to evaluate the use of PRP injections in patients with chronic elbow complains.
52 patients met the inclusion criteria for this pilot study. The impact of the chronic elbow complaints on function, social participation and pain were measured using Oxford Elbow Score Sheets, which were measured over a time period of six months.
Results Over a period of 6 months the total elbow score increased significantly from 15 to 26 points (Fig 1). This may indicate a reduction in chronic elbow complaints from severe to moderate.

Patients improved over a period of 6 months on all three separate domains, resulting in significant increased function (from 7 to 10), social ability (from 3 to 8) and severity of pain (from 5 to 8; Figure 2).
Conclusion The results of this study indicate that patients with chronic elbow complaints may benefit from autologous platelet rich plasma by reduction in pain and restoration of mobility after 6 months.

Transpopliteal recanalization of femoral occlusions in patients with critical limb ischemia using a 4F system Purpose
Results
To assess the results of transpopliteal recanalization of superficial femoral artery (SFA) occlusions by means of a 4 French system in patients with critical limb ischemia (CLI).
Technical success rate was 83% (ten out of twelve limbs). Two periprocedural complications occurred, both successfully treated. During six months follow-up there were no major amputations, three patients died due to non-related causes. Duplex restenosis (>50%) rate at six months was 50%.
Methods and Methods During a 24 months period, 11 patients, four male and seven female, with a mean age of 77 years (range 49-91) underwent 12 attempts of transpopliteal recanalization of long SFA occlusions (TASC C/D). All patients had CLI (Rutherford 4 -6) and were non-operable due to poor general condition. Indications for transpopliteal access were proximal SFA occlusion in six limbs, failure of antegrade recanalization in four and infected fem-fem crossover in two. The popliteal artery was punctured under ultrasound guidance and a 4 F radial sheath (Cordis, Johnson & Johnson) was introduced. The occlusions were recanalized subintimally, predilated with a 5 – 6 mm Passeo-18 PTA balloon, stented with 5 – 6 mm self-expandable nitinol Pulsar-18 stents (Biotronik) and postdilated. Technical success was defined as a patent SFA with less than 30 percent residual stenosis.
Conclusions Transpopliteal recanalization and stenting of an occluded SFA in CLI for a temporary bypass is feasible by means of a 4F system. Both technical results and clinical results are promising. Long term follow-up needs to be determined.

Resultaten
Voor langdurige behandeling van een obstructief ureter probleem kan een dubbel J katheter (JJ) gebruikt worden. Veelal worden deze retrograad via cysto scopie geplaatst. Het is echter zeer goed mogelijk om radiologisch percutaan een JJ antegraad te plaatsen, zeker indien er reeds eerder via een nefrostomiekatheter toegang tot het pyelocalicieel systeem verkregen is.
130 procedures bij 100 unieke patiënten werden opgezet om een JJ antegraad te plaatsen. 109 keer was reeds een nefrostomiedrain in situ. De indicaties waren divers en staan vermeld in tabel 1.
Doel Inventarisatie van de indicaties, het succespercentage en de complicaties van antegraad geplaatste JJs.
Patiënten en Methode Via statusonderzoek werden retrospectief gegevens verzameld van patiënten bij wie tussen 2005 en 2013 (HagaZiekenhuis) of tussen 2008 en 2013 (LUMC) antegraad een JJ geplaatst werd door een interventieradioloog.
125 keer was de procedure succesvol. In de overige 5 procedures (bij 4 unieke patiënten) lukte het niet een ureterstenose te passeren of de ureter tot in de blaas te katheteriseren. Dertien keer was het noodzakelijk een ureterstenose te predilateren met een PTA ballon voordat de JJ kon passeren. Bij 21 patiënten had een niet succesvolle poging tot retrograde plaatsing plaats gevonden. Bij al deze patiënten was de antegrade JJ plaatsing geslaagd. Acht procedure gerelateerde complicaties zijn vermeld (Zes infecties, 1 fausse route, 1 malpositie).
Conclusie Het antegraad, percutaan plaatsen van een JJ is een goed alternatief voor de retrograde benadering, zeker wanneer een patiënt reeds een nefrostomiekatheter in situ heeft en/of retrograde benadering niet succesvol was.

Overzicht van de endovasculaire behandeltechnieken bij portale hypertensie. Bekendheid van TIPSS en BRTO als behandeling vergroten. Eerste ervaringen in LUMC met BRTO behandelstrategie.
Achtergrond Levercirrose met portale hypertensie komt ook in Nederland relatief veel voor. Als gevolg van de complicaties, zoals (bloedende) varices, ascites, spontane bacteriële peritonitis, splenomegalie of heptorenaal syndroom, is het een belang rijke doodsoorzaak tussen 45-65. Primaire behandeling is medicamenteus en endoscopisch, maar bij therapiefalen, zoals refractaire ascites of varices bloeding is endovasculaire behandeling effectief. In het Westen wordt traditioneel de TIPSS-procedure gebruikt voor het verlaging van de portale tensie en indirect daarmee de druk in en de bloedingskans van de varices verlaagd. In Azië wordt veelal scleroseren van varices via trans veneuze benadering als directe behandeling van de bloedingsbron toegepast. De laatste jaren wordt directe behandeling van met name gastrische varices ook in Europa en Amerika meer gewaardeerd. In LUMC recent eerste ervaringen opgedaan.
Details procedures TIPSS: Transjugulair en via de rechter levervene, punctie van de rechter vena portae. Aansluitend gecoverd stenten van het traject. Voordeel: directe verlaging van de portale druk. Nadeel: verminderde leverfunctie door hypoperfusie en hepatische encephalopathie. BRTO: Transveneuze katheterisatie van een portosystemische shunt (meestal gastrorenaal) Shuntocclusie met ballon en retrograad scleroseren van de varices. Voordeel: directe behandeling van de bloedingsoorzaak. Toename leverperfusie. Symbiose met endoscopische behandeling. Nadeel: op termijn toename oesophagusvarices.

It is unclear whether advanced airway management in the prehospital setting improves outcomes following out-ofhospital cardiac arrest (OHCA) compared with conventional bag-valve-mask ventilation. Time spent for endotracheal intubation in the field may be better spent focusing on the provision of optimal cardiopulmonary resuscitation (CPR), including chest compressions, performing defibrillation and administering medications.

Conclusion Prehospital advanced airway management in patients with out-of-hospital cardiac arrest is associated with decreased neurological outcome/ survival, compared to bag-valve-mask ventilation.
Clinical bottom line The studies above provide an opportunity to reconsider the approach to prehospital airway management in patients with an OHCA. Further research needs to be done to see whether this results can be applied to the Netherlands and to investigate whether there are circumstances under which advanced airway management is beneficial.

Incidentie, oorzaken, behandeling en uitkomsten van epistaxis op de Spoedeisende hulp
Inleiding Epistaxis is een probleem dat regelmatig op de spoedeisende hulp afdeling (SEH) wordt gezien. In de literatuur is relatief weinig bekend over de incidentie, oorzaak, behandeling en het aantal recidieven na behandeling van epistaxis op de SEH.
Methode Een retrospectief dossier onderzoek van alle patiënten die op de SEH behandeld werden voor epistaxis in de jaren 2011en 2012 in een groot opleidingsziekenhuis. Zowel demografische als ziektespecifieke data zijn in een database gerangschikt en geanalyseerd met SPSS 17.0.
Resultaten In totaal zijn 252 patiënten met epistaxis op de SEH van het Hagaziekenhuis behandeld. Het merendeel van de patiënten gebruikte anticoagulantia (53,6%). In 48,3% werd de patiënt zonder hemostase behandeling retour gestuurd. Het overgrote deel van
de patiënten werd behandeld door SEH behandelaren (92,5%). Slechts 19 van de 252 (7,5%) werd primair gezien door KNO behandelaren. De meest gebruikte behandeling was tamponade. Cauterisatie werd slechts bij een minderheid van de gevallen, 4 van de 252 (1,6%), toegepast en alleen door KNO behandelaren. Een significant deel van behandelde patiënten (34,5%) werd binnen een week terug gezien met een recidief epistaxis. Bij 5,6% was een klinische behandeling noodzakelijk.
Conclusie Gemiddeld werd elke 3 dagen een patiënt met epistaxis gezien. Behandeling bestond voornamelijk uit tamponade. Desondanks kreeg een significant deel van de behandelde patiënten een recidief epistaxis na ontslag van de SEH. Optimaliseren van de diagnostiek en behandelprotocollen zou mogelijk kunnen leiden tot een vermindering van deze recidieven.

De invloed van ervaring en supervisie op de herkenning en behandeling van ernstig sepsis op de spoedeisende hulp afdeling Doel
Resultaten
Snelle herkenning en agressieve behandeling zijn de belangrijkste strategische pijlers ter verbetering van de behandeling van patiënten met een ernstig sepsis op de afdeling Spoedeisende hulp (SEH). Doel van dit onderzoek was om inzicht te krijgen in de invloed van werkervaring en supervisie op de herkenning en behandeling van patiënten met dit klinisch syndroom.
Totaal werden 49 patiënten met een ernstig sepsis geincludeerd. Er waren geen verschillen in het behalen van de resuscitatiebundel tussen de groepen. (H)AIOS gaven minder vaak zuurstof (p = 0,023), ANIOS overlegde significant sneller met hun supervisor (p = 0,030) en specialisten pasten meer volume resuscitatie toe (p = 0,037). Mortaliteit was niet verschillend tussen de groepen en bedroeg in totaal 20,4%.
Methoden Prospectief, observationeel cohort onderzoek. Alle patiënten met ernstig sepsis op de SEH werden geincludeerd gedurende 3 maanden. Zij werden verdeeld in 3 groepen obv primaire behandelaar; ANIOS (minst ervaren), (H)AIOS (gemiddeld ervaren) en specialist (meest ervaren). Demografische en klinische gegevens werden uit het elektronisch dossier gehaald. Primaire uitkomstmaat was het behalen van de resuscitatiebundel. Secundaire uitkomstmaat was mortaliteit.

Conclusie De behandeling van patiënten met een ernstig sepsis op de spoedeisende hulp door artsen met verschillende kennis niveaus en ervaringsjaren, had geen invloed op het behalen van de resuscitatie bundel. Mogelijk is dit te verklaren doordat meer onervaren artsen sneller overleggen met een supervisor.

Audit of the Pain Management Guideline for adult patients at the Emergency Department of the Haga Teaching Hospital Background Over 80% of patients who visit the Emergency Department (ED) suffer from pain. International studies show that pain is undertreated. Many studies on pain management (PM) in the ED report data that is solely derived from patient records. Studies aiming to improve PM have not used baseline statistics for feedback to ED personnel.
Objective Our objective was first to investigate the extent to which the Pain Management Guideline (PMG) was implemented in everyday practice and second to improve the implementation with an educational intervention.
Methods This is an intervention study with a pre-post design (baseline / immediately after the intervention). Data was collected through observation of triages and review of patient records. Patients >16 years were included if they arrived at the ED using their own means of transportation and presented with pain at triage. Nurses were observed by the same research student during triage and the frequencies with which they followed the PMG was documented. The patient records were subsequently reviewed for total amount of analgesics administered. During the educational intervention nurses
and physicians were instructed on the PMG and were presented with the baseline results to raise awareness.

Results
Total amount of analgesics distributed increased (Acetaminophen +16,2% CI 9,9-22,4% and Diclofenac +10,4% CI 5,4-15,3%). Results are shown in the diagrams below.
1002 triages were observed, 844 were included (409 at baseline, 435 after intervention. 87% of patients presented pain at triage. There were no significant differences in baseline characteristics between the groups. There was a significant increase in the frequency with which the PMG was followed.
Conclusion The PMG was insufficiently implemented in this ED. An educational intervention including feedback on the current state of PM resulted in a significant and major improvement. Regular feedback on PM practices may strongly improve its implementation and therefore patient satisfaction.

Risk Stratification of Chest Pain Patients in the Emergency Department
In order to improve patient quality of care and ED logistics, a clinical pathway was introduced for patients presenting with chest pain suspected for acute coronary syndrome (ACS) at the emergency department (ED) of the Haga hospital.
Objective This study evaluates the modified TIMI risk score (M-TIMI, table 1) in combination with the clinical suspicion of the treating physician, in its ability to predict an ACS. All M-TIMI parameters were collected including cardiac markers at 6 and 10 hours. ACS or death within 30 days of first visit when returning to the ED was primary endpoint. The judgement of the physician if there was a persisting suspicion on ACS was recorded. M-TIMI 0 was considered low risk and M-TIMI > 3 was considered high risk of ACS. For diagnostic accuracy the sensitivity and specificity of the M-TIMI in combination with physician judgment was calculated. Length of stay was calculated as process outcome parameter.
Results Group 1 contained 475 patients; group 2 contained 847 patients. Groups were comparable for age. A total of 226 of 1422 had a final diagnosis of ACS (16%). Of all patients 47% were admitted. For low risk patients with M-TIMI 0 and without persisting suspicion of ACS, sensitivity and specificity were respectively 100% and 24,5%. For high risk patients with M-TIMI > 3 and a persisting suspicion on ACS, sensitivity and specificity were respectively 86,1% and 83,6 %. There was no significant difference in mean length of stay between groups (group 1 2:49:32 and group 2 2:49:02 hrs).
Conclusion The M-TIMI has a high sensitivity for excluding ACS in low risk patients with M-TIMI 0 in combination with low suspicion of the physician. The high-risk category with M-TIMI > 3 in combination with the physician’s judgement is not very specific for ACS for the patient population at the HAGA hospital. The clinical pathway did not improve length of stay. This is expected since for only 33% of patients a clinical pathway was (partly) used. More research is necessary to validate low risk M-TIMI for excluding ACS. Even more important is to prioritize the clinical pathway for ED staff.

Weerstanden en motivatoren van het zorgpad ‘Pijn op de borst’ op de spoedeisende hulp van het HagaZiekenhuis Achtergrond
Resultaten
Conclusie
Om het zorgproces bij patiënten met pijn op de borst beter te structureren, de risicostratificatie te standaardiseren en de patiënten op basis van de best beschikbare evidence te helpen, is op de SEH van het HagaZiekenhuis in 2011 een zorgpad ‘pijn op de borst’ opgestart. Echter er is sprake van een implementatieprobleem; het zorgpad wordt niet bij alle patiënten met pijn op de borst gebruikt. Doel van deze studie is om 1) de weerstanden en motivatoren van het gebruik van dit zorgpad te achterhalen en 2) door middel van de proces- en uitkomstparameters (1e ECG < 10 minuten, 2e ECG< 60 minuten, toedienen van ASA < 1 uur, totale verblijfsduur op SEH, goede correlatie risicostratificatie, <1% van de patiënten retour met toch ACS) weer te geven of het zorgpad inhoudelijk doet wat het zou moeten doen.
Een verscheidenheid aan belemmerende en bevorderende factoren van invloed op de implementatie van het zorgpad zijn met deze studie geïdentificeerd. Hoewel medische hulpverleners de voordelen van het zorgpad in de praktijk kunnen benoemen, zijn zij over het algemeen niet overtuigd van de meerwaarde van het zorgpad. Er is een gebrek aan kennis over het doel en de resultaten van het zorgpad. Andere veel genoemde weerstanden zijn: het zorgpad wordt op papier aangeboden, gebrek aan motivatie onder de artsen en verpleegkundigen, de perceptie dat zij het zorgpad niet nodig hebben en dat er te weinig aandacht wordt besteed aan het zorgpad door supervisoren bij overlegmomenten. Daarnaast werden er veel belemmerende factoren genoemd op het gebied van de organisatorische omgeving. De gemiddelde verblijfsduur op de SEH in de groepen waarbij het zorgpad wel, deels en niet werd gebruikt, was respectievelijk 2:49:32 uur, 2:56:12 uur en 2:49:02 uur. Deze verschillen zijn niet statistisch significant. De overige procesparameters zijn door een gebrek aan tijdsregistratie niet te beoordelen. Voor patiënten met een M-TIMI score van 0 en geen persisterende verdenking op ACS was de sensitiviteit 100% en de specificiteit 24,5%. De negatief voorspellende waarde was 100%. Hoe hoger de M-TIMI score, des te groter de kans op een ACS. In 0,2% van de gevallen is er bij een M-TIMI score van 0 toch sprake van een ACS.
Uit deze studie komt naar voren dat het belangrijk is om een aantal dingen op verschillende niveaus te verbeteren: hulpverleners overtuigen van de meerwaarde van het zorgpad, hen motiveren om het zorgpad te gebruiken, het zorgpad digitaliseren en meer informatievoor ziening over doel, resultaten en gebruikswijze van het zorgpad. Het zorgpad verbetert niet de doorlooptijden op de SEH. De M-TIMI score lijkt goed te correleren met de uitkomst ACS. Door de motivatoren te versterken en de weer standen aan te pakken, kan het implementatieprobleem worden aangepakt.
Methode Om de weerstanden en motivatoren te identificeren werden 15 semigestructu reerde diepte-interviews gehouden met medische hulpverleners uit zoveel mogelijk verschillende functiegebieden. Hierbij werd gevraagd wat de positieve en negatieve punten van het zorgpad zijn en wat ze graag anders zouden willen zien. De transcripten werden kwalitatief geanalyseerd, gebaseerd op de gefun deerde theoriebenadering. De proces- en uitkomstparameters werden door middel van kwantitatieve analyse berekend.

Comparison Of Tap Water Versus Chlorhexidine For Uncomplicated Traumatic Wound Irrigation In The Emergency Department Study objective
Results
The purpose of this study was to compare wound infection rates for irrigation with tap water versus chlorhexidine before closure of uncomplicated traumatic wounds in the emergency department.
379 patients were enrolled. Follow-up data were obtained in 342 of enrolled subjects, 5 patients were excluded, because of preventively prescribed antibiotics, and 32 were lost of follow-up. 177 patients were enrolled in the Chlorhexidine group and 165 in the tap water group. The wound infection rates were comparable in both groups: 5 (2.8%) of the subjects in the Chlorhexidine group and 7 (4.2%) of the subjects in the tap water group (p=0.477). However, five patients in the tap water group had infections of the suture puncture wounds, and four patients of the Chlorhexidine groep had these kind of infection(p=0.656).
Methods The study was a prospective, single blind, randomized controlled trial at the Emergency Department of a large urban teaching hospital. Subjects were a consecutive sample of adults presenting with acute traumatic skin defects, i.e. wounds, requiring sutures or tissue adhesives. Exclusion criteria included immuno compromise or current use or need for antibiotics. Subjects who consented were randomized to irrigation with tap water or chlorhexidine. Wounds were closed according to standard protocol. Subjects were asked to return to the emergency department for suture removal and evaluation in five to seven days.
Conclusion There were no clinically important differences in infection rates between Chlorhexidine and Tap water. Tap water may be an effective alternative to Chlorhexidine for wound irrigation.

Syncope: an ED approach to risk stratification Background
Conclusion
Syncope is a sudden loss of consciousness with loss of postural tone with spon taneous and complete recovery. Syncope accounts for approximately 1-2 % of emergency department (ED) presentations.
When there is a life threatening event precipitating the syncope, there will be clues in history, physical exam and ECG. If after evaluation the cause is uncertain: risk stratify. Risk factors are: older age
and associated comorbidities, abnormal ECG, hematocrit < 30 (if obtained), history or presence of heart failure, coronary artery disease and structural heart disease. Disposition is determined by the presence of risk factors that predict short-term adverse events.

Methods Databases MEDLINE, EMBASE and Cochrane Library were searched form inception till august 2012. 1173 Articles were retrieved and selected by two independent reviewers. 199 Articles were included for critical appraisal.
Results Emergency physicians always start with the ABC approach. Focused history and physical examination including an ECG will provide the clues to a life-threatening cause. An ECG is essential in the evaluation of syncope. Other diagnostic tests need to be directed by findings in history and physical exam. If after initial evaluation the cause is uncertain, a disposition decision is directed by risk stratification. Several decision rules for syncope have been developed. None is sensitive and specific enough to use in the ED setting. They give a good overview of existing risk factors that predict short-term adverse events.

Locatie Juliana Kinderziekenhuis
Locatie Leyweg
Naast de locatie Sportlaan ligt de locatie Juliana Kinderziekenhuis. Dit is het enige niet-academische kinderziekenhuis van Nederland. In het Juliana Kinderziekenhuis zijn alle voorzieningen afgestemd op kinderen. Voor onderzoek en behandeling hoeven kinderen dus vrijwel nooit naar een afdeling voor volwassenen, zoals in de meeste andere kinderziekenhuizen wel het geval is. Dit zorgt ervoor dat het Juliana Kinderziekenhuis voor zowel de kinderen als hun ouders een zeer prettige omgeving is. In 2015 verhuist het Juliana Kinderziekenhuis naar de nieuwbouw op locatie Leyweg.
De locatie Leyweg in de wijk Leyenburg is de grootste vestiging van het HagaZiekenhuis. Hier ligt het accent op spoedeisende en complexe zorg. Zo zijn daar een omvangrijk operatiecomplex, een moderne Intensive Care en een grote afdeling Spoedeisende Hulp ondergebracht. De SEH wordt bij rampen in de regio Haaglanden ook als traumacentrum gebruikt. Daarnaast is het Hartcentrum op de Leyweg gevestigd. In 2015 is de nieuwbouw op deze locatie gereed en huisvest het Juliana Kinderziekenhuis, Moeder- en Kindcentrum, dagbehandeling voor volwassenen, het OK-complex, de Interventie Radiologie, de polikliniek Hematologie en Anesthesiologie, de Jutters, het Haga Juliana Geboortecentrum, het Ronald Mc Donald huis en de afdeling Fysiotherapie.
Locatie Sportlaan
Locatie Zorgplein Westland Op het Zandeveltplein in 's-Gravenzande bevindt zich het Zorgplein Westland. Het HagaZiekenhuis houdt hier spreekuur in apotheek De Klipper. Voor diverse specialismen kunnen inwoners van het Westland hier terecht.
De locatie Sportlaan ligt tegen de duinen van de Haagse Vogelwijk aan. Op deze locatie ligt het accent op de planbare zorg. Er bevinden zich onder meer een geboortehotel, Gynaecologie & Verloskunde, Urologie met Bekkenbodem centrum en een orthopedisch centrum op de Sportlaan. Bijna alle specialismen houden hier spreekuur.
Locatie Buitenpolikliniek Wateringse veld De Buitenpolikliniek Wateringse Veld van het HagaZiekenhuis ligt in de Vinex-wijk Wateringse Veld in Den Haag Zuid. Een aantal specialismen houdt hier spreekuur. Hierdoor biedt het HagaZiekenhuis voor deze wijk een aantal basisvoorzieningen dicht bij huis.
