Geachte collega,
In de internationale wetenschappelijke literatuur wordt de nauwe relatie tussen de parodontologie en de implantologie steeds duidelijker beschreven. In de toekomstvisies van de wetenschappelijke verenigingen voor parodontologie, binnen en buiten Europa, zien wij al dat de parodontologie en implantologie als eenheid wordt gezien. Met het oog op deze toekomstvisies organiseert de Nederlandse Vereniging voor Parodontologie (NVvP) het najaarscongres met als titel: Parodontologie & Implantologie; onlosmakelijk verbonden. De meest ervaren wetenschappers op het gebied van risico-management en voorspelbaarheid van de resultaten van verschillende implantologische behandelingen zullen aan het woord komen. Uit het programma kunt u afleiden, dat het een buitengewoon interessante wetenschappelijke bijeenkomst wordt met een hoog gehalte aan informatie, die direct in de praktijk kan worden omgezet. Tijdens de preconference workshop op donderdagavond 15 november zullen medewerkers van ACTA (Afdeling Parodontologie en Afdeling Orale Functieleer, sectie prothetische tandheelkunde en orale implantologie) het congresthema in kleine kring inleiden. Op de hoofd congresdag, vrijdag 16 november, heeft U de mogelijkheid deel te nemen aan het parallelprogramma, waarin de begeleiding en nazorg van parodontitispatiënten met implantaten uitvoerig zal worden besproken. Tandheelkundige implantaten bij parodontitis patiënten

In deze voordracht zal de vraag besproken worden of tandheelkundige behandeling met implantaten bij patiënten met parodontitis of een verleden van parodontitis, het lange termijn succes kan beïnvloeden. Peri-implantair botverlies lijkt een vaker voorkomend fenomeen in personen die vatbaar zijn gebleken voor parodontitis. De beschikbare literatuur zal kort samengevat worden en gemeenschappelijke risicofactoren voor peri-implantair botverlies en parodontitis zullen besproken worden. De risicofactoren waar o.a. aan gedacht moet worden zijn microbiologische factoren, roken, en erfelijke factoren.

Botresorptie en remodelling van het kaakbot heeft er bij implantaatbehandelingen toe geleid dat guided bone regeneration (GBR) bijna tot een standaard is verheven. GBR kan verloren bot reconstrueren en in sommige gevallen buccale recessie voorkomen, maar in andere gevallen leiden tot papilverlies. Verstoring van de bloedvoorziening en doorklieven van de supracrestale vezels spelen hierbij een rol. Er bestaat echter een indicatiegebied waarbij een dusdanige hoeveelheid steunweefsel voorradig is, dat er kan worden overwogen om direct na extractie een implantaat te plaatsen en die van een tijdelijke kroon te voorzien. Hiervan zijn echter nog geen lange termijn onderzoeken beschikbaar. Omdat er op het gebied van replantatie bij avulsie van natuurlijke elementen wel zulke resultaten bekend zijn, kunnen die wellicht dienen als leidraad voor toekomstige behandelstrategiën.
Bij velen van onze zogenaamde paro-patiënten zijn er één of meerdere elementen verloren gegaan en lokt een vaste prothetische oplossing het meest. Maar is iedere paro-patiënt met een goede mondhygiëne geschikt voor een prothetische voorziening afgesteund op implantaten? Welke afwegingen kunnen er gemaakt worden? En, indien er gekozen wordt voor implantaten welke is dan de te bewandelen route? Aan de hand van casussen wordt een aantal aspecten rond deze problematiek belicht.

Parodontitis is een multifactoriale aandoening, bij peri-implantitis is dat niet anders. Met het toenemen van het aantal implantaten wordt ook deze aandoening een serieus probleem. Het verhelpen van peri-implantitis is tot op heden vooral symptomatisch, de ene keer succesvoller dan de andere. De aandacht van de implantologie is echter tot op heden nog steeds vooral gericht op het aanbrengen van het implantaat ter verkrijging van osseointegratie. Dit moet sneller, eenvoudiger en beter. Diverse fabrikanten passen hun implantaatontwerp aan om een versnelde integratie in het botweefsel te krijgen, van ruw naar glad en visa versa. Maar wat mogen we verwachten voor de peri-implantaire weefsels? De invloed van het implantaat op de peri-implantaire gezondheid zal in deze presentatie worden bediscussieerd aan de hand van klinische voorbeelden in reflectie tot de literatuur. Aan de hand van een aantal casussen worden aspecten rond deze problematiek belicht.
of the patients with implants demonstrate progressive bone loss, at one ore more implants after 10 years. It is demonstrated that mucositis (probing depth ≥4 mm and bleeding on probing without progressive bone loss) is common around implants if the oral hygiene is neglected. In a recent paper by RoosJansåker et al. (2006a) 48 % of the implants and 77 % of the patients’ demonstrated mucositis and 16 % of the patients and 6.6%of the implants demonstrated peri-implantitis (defined as bone loss ≥3 threads (a minimum bone loss of 1.8 mm. Fransson et al. (2005) reported progressive bone loss in 28 % in patients with a documented function time of at least 5 years. If a patient develop biological complications around implants, several implants are often affected in the same patient. Accordingly patient related risk factors such as an altered immune response, smoking and genetic factors have been proposed to be related to the development of peri-implantitis. Several studies have highlighted the fact that periodontitis patients is at higher risk for developing peri-implantitis and smoking has also been associated with presence of periimplant disease (van der Weijden et al. 2005, Roos-Jansåker et al. 2006 b). On a local level presence of plaque and the amount of keratinized mucosa have also been implicated to be of importance. In conclusion, mucositis and peri-implantitis are common findings ten years after implant therapy. Previous periodontitis patients and smokers seams to be more prone to develop peri-implant infections. Therefore, in the every day clinic, it is important to access risk factors before implant installation and to carefully monitor the patients after treatment in order to observe early signs of infections. Early detection of signs of disease, reinforcement of oral hygiene measures and smoking cessation programs is recommended.

Evidence-based periodontology is the comprehensive integration of literature evidence, clinician skills, and patient preference and views. The more advanced the disease, the less likely the best evidence is available to define the limit required for tooth extraction. The practitioner has to recommend treatment, while the patient has to decide whether to go along with the treatment. Thus, patient preference is critical to the decision-making process for advanced periodontal disease. The aim of this presentation is to give a comprehensive overview of the expanding field of periodontal decisionmaking, through challenging clinical cases.

Over the last decades, dental implants have become a commonly used treatment alternative to removable dentures in patients that have lost their teeth. Several longitudinal studies have reported survival rates around 90-95 % over periods of 5-10 years (Berglundh et al. 2002). The high clinical survival rate even among partially dentate individuals has led to an increased use of dental implants. Previously long tem follow up studies on implant therapy merely presented survival or so called success rates and only few long time studies reported on the incidence of peri-implantitis. However, biological complications do occur around implants and recently a few papers have presented data indicating that as many as 15-20%

Infecties rond tandvervangende implantaten vertegenwoordigen een nieuw fenomeen in de tandheelkunde. Epidemiologische gegevens duiden op een toename van peri-implantitis. Deze infectie vertoont grote overeenkomsten met parodontitis en er bestaan overeenkomstige risicofactoren voor beide aandoeningen. Deze zullen worden besproken in het licht van diagnose en prognose. Kennis van de microbiologische component van peri-implantitis lijkt eveneens essentieel voor een juiste diagnose en een effectieve behandelingsplanning

This presentation will focus on the similarities and the differences between periodontitis and peri-implantitis. It will cover definition of the clinical entities, clinical diagnosis, epidemiology, risk factors, as well as prevention and treatment. It will also discuss the best available evidence of the differential risk of recurrent/progressing periodontitis and peri-implantitis in the context of treatment planning for cases with severely compromised teeth.

Parodontologie heeft als basis insteek het behoud van de natuurlijke elementen. Met een volledige diagnostiek en combinatie van klinische en röntgenologische bevindingen, moet er binnen haalbare doelstellingen voor de patiënt, voldoende elementen resteren voor een functioneel optimale
situatie. Dit alles zonder (vroeg)tijdig offeren van prothetisch goed te restaureren dentities ten gunste van de “hype” der prothetische hulpmiddelen zoals implantaten. Om de natuurlijke dentitie voorspelbaar te houden, is dan wel een goed gestructureerde “after sales” of te wel nazorg, van groot belang, individueel toegesneden op elke patiënt. Met de toename in de afgelopen jaren van geplaatste implantaten komen de bijbehorende problemen in de tijd ook aan de oppervlakte. Naast “failure” van de prothetische componenten is de ontsteking component van de weefsels om de implantaten een belangrijke factor voor het al of niet slagen op langere termijn van implantaat gedragen constructies. Mucositis als equivalent voor gingivitis en paro-implantitis voor parodontitis rond natuurlijke elementen, zijn afwijkingen die bij voorkeur vroegtijdig gediagnosticeerd moeten worden om erger te voorkomen. In Deel 1 zal Dick Barendregt u handvatten bieden om de diagnostiek van afwijkingen rond de implantaten zo effectief mogelijk uit te voeren en indien nodig preventieve maatregelen toe te passen. Fridus van der Weijden zal u in Deel 2 inzicht geven in de huidige opties ter behandeling van de peri-implantitis ter eliminatie van de ontsteking, mogelijke regeneratie en de voorspelbaarheid van voornoemde therapieën.

Dick Barendregt studeerde in 1988 af als tandarts aan de Rijksuniversiteit van Groningen. Hij is daarna ondermeer werkzaam geweest als tandarts bij de Koninklijke Marine en heeft gedurende 3 jaar in diverse algemene praktijken waargenomen. In 1991 startte hij met zijn Post Academische opleiding in de parodontologie aan het ACTA. Deze opleiding rondde hij in 1994 af. Vervolgens startte hij als tandarts-parodontoloog (NVvP) in de Kliniek voor Parodontologie Amsterdam en de Praktijk voor Parodontologie Utrecht. In 1996 werd door hem de Kliniek voor Parodontologie Rotterdam opgericht. Daar houdt hij zich naast de parodontologie bezig met de implantologie als Implantoloog (NVOI). De laatste 9 jaar is in dat kader de nadruk komen te liggen op patiënten behandeling in een interdisciplinair teamverband met collega tandartsen zoals orthodontisten, endodontologen, prothetisten, kaakchirurgen en tandtechnici. Dick Barendregt is verder verbonden aan de afdeling parodontologie van het ACTA. Daarnaast is hij betrokken bij verschillende postacademische cursussen.

Prof. dr. Arie Jan van Winkelhoff studeerde Biologie, met microbiologie als specialisatie, aan de Rijks Universiteit Utrecht en de Universiteit van Gainesville Florida, USA. In 1986 promoveerde hij op de Vrije Universiteit (proefschrift: Black-pigmented Bacteroides in human oral infections). Sinds 1986 is hij medewerker van de Afdeling Orale Microbiologie en hoofd van het Klinische Laboratorium voor Parodontale Microbiologie. In zijn onderzoek houdt hij zich o.a. bezig met de rol van bacteriologie voor diagnose en behandeling van parodontitis en het gebruik van antibiotica bij de behandeling

Dr. Fridus van der Weijden studeerde in 1984 af als tandarts aan de Rijksuniversiteit van Utrecht waar hij ook zijn opleiding in de parodontologie heeft genoten. In 1990 werd hij erkend als tandarts parodontoloog door de Nederlandse Vereniging voor Parodontologie. Zijn proefschrift met als titel ‘The use of models and indices in plaque and gingivitis trials’ verdedigde hij in 1993 en in 2000 werd hij senior onderzoeker van de Interuniversitaire Onderzoekschool Tandheelkunde (IOT). Recent ontving hij zijn erkenning als tandartsimplantoloog van de Nederlandse Vereniging voor Orale implantologie. Sinds 1989 verdeelt hij zijn tijd tussen de Paro Praktijk Utrecht en de vakgroep Parodontologie van het ACTA. Van der Weijden heeft veel onderzoek verricht op het gebied van de mondverzorging en de parodontale therapie. Daarnaast is hij regelmatig docent bij postacademische cursussen. Hij is auteur van het boek ‘De stille kracht van ultrasoon’ en auteur en co-auteur van meer dan 50 Nederlandstalige en 100 internationale publicaties.

Dr. Erik Blom is in 1989 afgestudeerd als tandarts aan de UvA, alwaar vanaf 1985 zijn bijzondere interesse in de implantologie is ontstaan. Naast een aantal jaren algemene tandheelkunde in diverse praktijken in binnen- en buitenland is hij vanaf 1994 verbonden aan de afdeling implantologie van het ACTA. Vanaf die tijd heeft hij zich tevens verdiept in de esthetische en restauratieve tandheelkunde in een privé kliniek in het Gooi. Na zijn promotie in 2001 heeft hij zijn MSc in de implantologie aan de
UvA gehaald. Momenteel is hij als docent, opleider en onderzoeker werkzaam bij de afdeling implantologie van het ACTA, daarnaast geeft hij regelmatig nascholing en heeft zijn eigen tandheelkunde kliniek in Blaricum. Peet C. van Gils, MSc. Na te zijn afgestudeerd als tandarts in 1988 is Peet van Gils geëmigreerd naar Italië waar hij op Sardinië te Cagliari tot 1998 praktijk hield en zijn voorliefde voor de parodontologie ontwikkelde. Terug in Nederland heeft hij van 1999 tot medio 2001 gewerkt in een gespecialiseerd centrum voor implantologie en parodontologie, waar hij met name suprastructuren op implantaten vervaardigde. Daarnaast volgde hij de Master of Science opleiding Parodontologie te Amsterdam welke met succes in 2002 werd afgerond. Sindsdien is hij werkzaam te Alkmaar in het Centrum voor Parodontologie en Implantologie. Daarnaast is hij een dag per week verbonden aan de MSc-opleiding Parodontologie ACTA als wetenschappelijk medewerker. Tevens houdt hij er een verwijspraktijk op na voor parodontologie & implantologie te Amstelveen. Prof. dr. Bruno G. Loos Prof. dr. Bruno Loos heeft in 1981 zijn tandartsdiploma behaald aan de Vrije Universiteit. Onder andere door het enthousiasme van Hans Rodenburg was de belangstelling voor de parodontologie gewekt. In het laatste jaar van zijn studie heeft hij toen als student-assistent onder leiding van Hans Rodenburg een aanstelling gekregen. Na 1 jaar algemene praktijk heeft Loos van 1982-1985 een MSc-opleiding in de parodontologie gevolgd aan de Loma Linda University in Californië, VS. Vervolgens heeft hij aan de State University of New York in Buffalo, VS, van 1986 tot 1992 een opleiding in Oral Biology gevolgd, promotie-onderzoek verricht en dit afgesloten met een PhD thesis. Na nog een jaar als postdoctoral fellow bij dezelfde universiteit, is hij in 1993 begonnen als universitair hoofddocent bij de afdeling parodontologie aan het ACTA. In 1996 is hij daarbij ook aangesteld als coördinator van het MSc programma parodontologie. Hij heeft grote belangstelling voor vraagstukken omtrent de vatbaarheid voor parodontitis, met name doet hij onderzoek naar de rol van genetische en immunologische aspecten. In 2005 is hij benoemd tot hoogleraar parodontologie in het bijzonder de immunobiologie van parodontale infecties aan het ACTA – sectie Parodontologie. Paul M. Sipos, MSc. Paul Sipos studeerde in 1993 af als tandarts aan het Academisch Centrum Tandheelkunde Amsterdam (ACTA). Hij is daarna ondermeer werkzaam geweest als tandarts bij de Koninklijke Marine en heeft in diverse algemene praktijken waargenomen. In 1998 startte hij met zijn Post Academische opleiding in de parodontologie aan het ACTA. Deze opleiding rondde hij in 2001 af. Hij heeft gewerkt als parodontoloog bij de Koninklijke Marine en bij de Kliniek voor Parodontologie Rotterdam en Praktijk voor Parodontologie en Implantologie Arnhem. Hij is nu wetenschappelijk medewerker bij de afdeling Parodontologie van het ACTA en werkzaam in zijn eigen praktijk in het Centrum Implantologie Parodontologie Amstelveen.
