In de voor u liggende editie van het Nederlands Tijdschrift voor Anesthesiologie vindt u de abstracts terug van de presentaties die tijdens de NVA Wetenschapsdag worden verzorgd door jonge onderzoekers die werkzaam zijn binnen een experimentele of klinische setting. Een van de grote uitdagingen voor onderzoek binnen de perioperatieve setting is het gebrek aan een uniforme en herkenbare patiëntenpopulatie en het kunnen focussen op een specifieke aandoening. In de afgelopen jaren is er daarom steeds meer focus binnen onze wetenschappelijke vereniging op samenwerking met verschillende onderzoekspartners. Anesthesiologen zijn daarmee in toenemende mate te vinden binnen samenwerkingsverbanden met preklinische afdelingen zoals fysiologie en celbiologie, en klinische afdelingen zoals heelkunde, cardiologie, cardio-thoracale chirurgie en intensive care geneeskunde. De toename in multidisciplinariteit en samenwerkingsverbanden is van groot belang voor de continuïteit van wetenschappelijk onderzoek, daar onderzoek steeds vaker alleen in aanmerking komt voor externe financiering, wanneer er sprake is van een nationaal samenwerkingsverband. En wanneer het onderzoek aansluit bij grote zorgthema’s. Samenwerking leidt tevens tot schaalvergroting, waardoor de uitvoering van grote prospectieve klinische studies mogelijk wordt. Op deze manier kunnen strate-
gische speerpunten worden gekozen, die een betere kans maken op financiering. De wetenschapscommissie van de NVA zal in toenemende mate aandacht besteden aan het ondersteunen van samenwerking binnen ons vakgebied en het stimuleren van samenwerking met onze klinische en preklinische partners. Het realiseren van samenwerking vormt een voorwaarde voor succesvolle financiering in de toekomst, en zal bijdragen aan het vergroten van de nationale impact van perioperatief wetenschappelijk onderzoek. Hierbij kan worden gedacht aan het oprichten van een nationaal anesthesiologisch onderzoeksplatform dat grote prospectieve klinische trials mogelijk maakt. Een dergelijke onderzoeksplatform is reeds zeer succesvol gebleken binnen andere vakgebieden, zoals de obstetrie en gynaecologie. Het verheugt het organiserend comité NVA Wetenschapsdag dat veel van de ingezonden abstracts zijn voortgekomen uit een samenwerkingsverband binnen of tussen ziekenhuizen. Het stimuleren van samenwerking van jonge wetenschappelijke onderzoekers met collega’s uit andere ziekenhuizen en disciplines buiten de perioperatieve setting zal naar verwachting leiden tot een nog sterkere positie van het anesthesiologisch onderzoek binnen de medische wetenschap. Conclusion MV induces diaphragm atrophy and reduces diaphragm fiber active and passive force generation, even after correction for CSA. Hypercapnia during MV prevents: diaphragm atrophy, reduction in passive force generation and MV induced inflammation in the diaphragm.

Background Exogenous erythropoietin (EPO) inhibits development of allodynia in experimental painful neuropathy due to its anti-inflammatory and neuroprotective properties at spinal, supraspinal and possibly peripheral sites. Here we assess the effect of a nonhematopoietic EPO analogue, ARA290, on tactile and cold allodynia in a model of neuropathic pain in rats and mice lacking the -common-receptor (cR-/-), a component of the receptor complex mediating tissue protection.
Methods 24h following spared nerve injury, rats, wild-type (WT) and cR-/- mice were injected with ARA290 or vehicle (five 30 g/ kg ip injections at 2-day intervals, followed by once/week, n = 8/group). In a separate group of 8 rats ARA290 treatment was restricted to 5 doses over the initial 2 weeks after surgery.

Results In rats, irrespective of treatment paradigm, ARA290 produced effective and longterm (up-to 15-weeks) relief of tactile and cold allodynia. Retreatment in week-16 of animals that had received only
the initial 5 doses of ARA290 did not further improve pain thresholds. Similarly, ARA290 was effective in WT mice producing significant relief of allodynia. In cR-/mice no effect of ARA290 was observed.

Introduction Life-saving care in the neonatal intensive care unit (NICU) entails daily repetitive exposure of neonates to up to 14 noxious routine procedures per day. Repetitive exposure to noxious stimulation during the critical postnatal period of fine-tuning of the vulnerable nociceptive network has been shown to affect nociception in later life. The objective of this study is to determine whether neonatal repetitive small painful procedures results in alterations in anatomy of the nociceptive network in later life.
Methods
Results
Neonatal Sprague-Dawley rat pups (N = 5) received four needle pricks per day to the left plantar hind paw during the first week of life. Control animals (N = 6) received four tactile non-noxious stimuli. Paw withdrawal thresholds (PWTs) to von Frey filaments were determined at the adult age of eight weeks. Quantitative immunohistochemistry was performed in adult animals to determine anatomy of primary afferents at the lumbar spinal cord level. A calcitonin-gene-relatedpeptide (CGRP) antibody was used to visualize peptidergic C/Ad-fibers, an isolectin-B4 (IB4) antibody for non-peptidergic C/Ad-fibers and a vesicular-glutamate-transporter-1 (VGLUT1) antibody for A-fibers.
At the age of eight weeks basal nociception was not affected by neonatal needle pricking, since there was no difference in PWTs to mechanical stimuli between tactile control and needle prick animals. There was no difference in IB4-immunoreactivity or VGLUT-1 measurements between tactile control and needle prick animals. However, CGRP-immunoreactivity was bilaterally increased in needle prick animals.
Conclusion Although repetitive small routine painful procedures did not alter basal nociception in later life, they can affect spinal nociceptive CGRP-positive primary afferent anatomy, which might affect responses to injuries in later life.

Introduction Dynamic preload indices are increasingly used to assess a patient’s fluid responsiveness. These indices include stroke volume variation (SVV), systolic pressure variation (SPV) and pulse pressure variation (PPV) and are obtained from the arterial pressure waveform. Furthermore, the plethysmographic variability index (PVI) is derived non-invasively via a finger clip. We compared the ability of these four indices to predict fluid responsiveness in patients undergoing major hepatic resection.
Methods After local IRB approval, 30 patients were included. Fluid was restricted to 6 ml/kg/ hr during resection. Thereafter, patients received a fluid challenge (FC) of 15 ml/kg
in 30 minutes. Patients were considered responders when stroke volume index (SVI) increased > 20 % above pre-FC values. SVV and SVI were measured by the FloTracVigileo system (Edwards Lifesciences, Irvine, USA), PVI by the Masimo Radical 7 pulse co-oximeter (Masimo Corp, Irvine, USA). Data were recorded using RugLoop software (Demed, Temse, Belgium) and
SPV and PPV were retrospectively calculated from the arterial pressure waveform. Areas under the ROC curve (AUC) were calculated to assess the ability of dynamic indices to predict fluid responsiveness.
Results 17/30 patients were fluid responsive. ROC analysis revealed an AUC for SVV, PVI,

abstracts 2011 PPV and SPV of 0.81, 0.77, 0.77 and 0.75, respectively, while the AUC of CVP was much lower (Figure 1). SVV, SPV and PPV decreased significantly after FC whereas PVI did not (Table 1). Furthermore, all non-dynamic hemodynamic variables except CVP remained unchanged.
Conclusion All dynamic preload indices predict fluid responsiveness during major hepatic resec-
tion to a similar extent, whereas the actual effects of fluid challenge were best tracked by PPV and SVV but not by PVI. Thus, it seems that the non-invasive plethysmographic variability index should be further evaluated before it can replace arterial pressure based dynamic variables.

The effectiveness of risk-dependent PONV prophylaxis in clinical practice: a before-after study T.H. Kappen, Y. Vergouwe, W.A. van Klei, L. van Wolfswinkel, C.J. Kalkman, K.G.M. Moons Universitair Medisch Centrum Utrecht, Utrecht
Introduction In a previous study, presenting a predicted risk of postoperative nausea and vomiting (PONV) in the anesthesia information management system resulted in a small increase in administration of PONV prophylaxis by anesthesiologists, without decreasing the PONV incidence. We hypothesized that the effect on administration of prophylaxis had been too small to effectively lower the PONV incidence. To this aim, we combined the presented PONV risk with a patient-specific recommendation on the number of antiemetics to be administered, and studied PONV prophylaxis prescription and PONV incidence.
Methods In the before period of a before-after study, care-as-usual and PONV outcomes within 24 hours postoperatively were documented (January-March 2010). During the intervention period (April-May 2010), anesthesiologists were presented with a risk-based recommendation on how many
prophylactic antiemetics to prescribe to their current patient. Patients undergoing elective, non-cardiac surgery as inpatients were eligible(n = 1,483). The effects of the intervention on PONV incidence was analyzed by multivariable logistic regression.
Results Baseline differences were too small to be considered as confounding. The crude PONV incidence was 49% in the care-as-usual group and 41% in the intervention group (p<0.05). The intervention led to a significant increase in the number of prophylactic antiemetics per patient (0.34 versus 0.96), particularly in high-risk patients (predicted risk >40%: 0.53 versus 1.5). Results of the regression analysis are shown in figure 1.
Conclusion In contrast to our previous study, providing anesthesiologists with patient-specific
recommendations increased administration of PONV prophylaxis, and now significantly lowered the PONV incidence, especially in high-risk patients. Concluding, this type of risk-tailored PONV prophylaxis is effective in clinical practice.

Introduction Exogenous erythropoietin (EPO) and the non-hematopoietic EPO analogue ARA290 prevent development of allodynia in animal models of painful neuropathy, a mechanism that is thought to relate to their anti-inflammatory and tissue protective properties. In this study we evaluate the analgesic and mood related effects of ARA290 treatment in patients with persistent severe neuropathic pain.
Methods Patients diagnosed with neuropathic pain related to sarcoidosis (n = 12) or diabetes mellitus type II (n = 12) were included in
the study. On day 1, 3 and 5 of the study patients received an intravenous infusion of ARA290 (2 mg). Pain and mood were quantified using the Brief Pain Inventory (BPI) and the Hospital Anxiety and Depression Scale (HADS) on the infusion days, and at 1 and 4 weeks after the end of the treatment week.
Results Baseline ‘Most pain’ visual analogue scale (VAS) scores were 8.2 0.4 cm with a predominance of pain in the lower extremities (lower legs and feet). One week ARA290 treatment resulted in a profound analgesic effect with > 50% reduction on the VAS for pain-related parameters (to 4.0 0.4 cm) in the sarcoidosis patients. Furthermore, a 33% reduction was observed in the absolute
scores of the HADS, which indicates that patients felt less depressed and anxious after treatment with ARA290. In patients diagnosed with type II diabetes mellitus no effect of ARA290 treatment on pain was observed.
Conclusion ARA290 effectively reduced pain, and improved mood in patients with neuropathic pain from sarcoidosis, without showing any side effects. The anti-inflammatory property of ARA290 may play a pivotal role in achieving this large analgesic effect. Absence of an effect in diabetes mellitus type II may suggest differences in the pathophysiologic pathways that underlie neuropathic pain.

Introduction Measurement of mitochondrial oxygenation and oxygen consumption could be a potential tool for assessing aspects of mitochondrial function in vivo. This study was aimed at testing the feasibility of measuring mitochondrial oxygen tension (mitoPO2) and oxygen consumption in skin, by means of protoporphyrin IX (PpIX) triplet state lifetime measurement (PpIX-TSLM).
Methods Animals. A total of 4 anesthetized and mechanically ventilated male Wistar rats were used.
Measurements 5-aminolevulinic acid (ALA) cream was applied to the abdominal skin of rats to induce PpIX in the mitochondria. Photo-excitation with pulsed light was used to induce population of the PpIX triplet-state. The triplet state lifetime is related to mitoPO2 and was measured by delayed fluorescence. Oxygen consumption was determined by repeated mitoPO2 measurements while locally block-
ing oxygen supply. The latter was achieved by applying local pressure with the measurement probe. The mitoPO2 was recorded before, during, and after a 90 second compression. Oxygen consumption was derived from the decay of the mitoPO2 slope. Mitochondrial respiration was blocked by local application of cyanide cream.
Results Baseline levels of mitoPO2 were 4511mmHg. The slope of the mitoPO2 oxygen disappearance rate was -2.520.49mmHg/second (Figure 1a). After cessation of respiration, baseline mitoPO2 levels were 18034.1 mmHg and no oxygen disappearance was measured during application of pressure (Figure 1b).

Conclusion Our results show that PpIX-TSLM based mitochondrial respirometry is feasible in skin but accuracy of the measurement needs improvement. Further development of the PpIX-TSLM technique might ultimately enable clinical use for patients benefit. The ultimate goal is to provide clinicians technology that permits a look at oxygen supply and demand at the place where it matters most, the mitochondria.

Introduction Recent studies reconfirmed that patients still suffer from moderate to severe postoperative pain. The aim of this study was to develop procedure specific comparisons which may allow for an improved postsurgical pain therapy.
Methods To compare postoperative pain intensity, data of the QUIPS-project (Quality improvement in postoperative pain treatment) were analyzed. Patients completed the validated 15-item QUIPS questionnaire at the first postoperative day. Worst pain
intensities since surgery were measured using NRS (0-10). The type of surgical procedure was recorded using 21.000 codes of the German procedure codes. We defined 529 different surgical procedure groups. The data of 115.775 patients of 105 hospitals and 584 surgical wards were analyzed. Surgical groups with more than 20 patients were included for comparison. Worst pain intensities could be compared among 50.523 patients in 179 different surgeries. In addition, patients were grouped into those who received regional anesthesia and those who did not.
Results Orthopedic and traumatological surgeries were associated with the highest worst pain intensities. It may be noticed that a rather small portion of patients received regional anesthesia. In some surgeries tissue trauma and length of incision are not associated with higher pain intensities. Thus, patients
report higher pain intensities after open appendectomy (ranking 19), plastic reconstruction of hemorrhoids (ranking 23), and tonsillectomy (ranking 24) (all three: NRS median 6.0) than above-knee amputation (NRS median 4.0, ranking 115, open thoracotomy (NRS median 4.0, ranking 118) and mastectomy (NRS median 3.0, ranking 146)
Conclusion For the first time, this comprehensive procedure-specific comparison permits a quality analysis of everyday pain treatment. It is obvious that pain intensities after various procedures are underestimated. This may be one cause of insufficient postoperative pain treatment. In addition, the knowledge about optimal postoperative pain management should be employed forcefully such as the use of regional anesthesia.

Introduction Previously, we showed that diet-induced metabolic alterations influences sevoflurane-induced cardioprotection during ischemia in the rat heart, suggesting that metabolic alterations may modulate the sensitivity of the heart to stress. However, whether metabolic altered hearts are more sensitive to stress and anesthesia-related alterations in myocardial function is not known. The purpose of the present study was to explore the effect of diet-induced metabolic alterations on myocardial function during pharmacological induced stress and sevoflurane anesthesia.
Methods Male Wistar rats (n=8 per group) were exposed to a high fat diet (HFD) or control diet (CD). After 4 weeks, rats underwent an oral glucose tolerance test. Accordingly, rats underwent echocardiography to determine myocardial function during the administration of 2% sevoflurane at rest and during dipyridamole-induced hyperemia (20 mg/kg for 10 min).
Results Four weeks of HFD feeding resulted in impaired glucose tolerance compared to CD-fed rats (p<0.05). Systolic function as represented by fractional shortening was not altered after 4 weeks of HFD-feeding compared to CD fed rats under rest conditions (602% vs. 544%, ns). Administration of sevoflurane did not influence fractional
shortening (584% vs. 542%, ns); however, HFD reduced fractional shortening during hyperemia (681% vs. 592%, p<0.05). Moreover, sevoflurane administration during hyperemia revealed no differences in fractional shortening (642% vs. 612%, ns).
Conclusion Four weeks of high fat diet feeding impaired myocardial function during stress. Hearts exposed to this diet are not more sensitive for sevoflurane-related effects on myocardial function at rest and during hyperemia. These findings may result in new insights into the effect of high dietinduced metabolic alterations on myocardial function during stress and sevoflurane anesthetics.

Diabetes mellitus, independent of insulin use, is associated with an increased risk of cardiac complications after vascular surgery E.J. Bakker, T.M. Valentijn, K.M. van de Luijtgaarden, S.E. Background The Revised Cardiac Risk Index identifies insulin dependent diabetes mellitus as a risk factor for cardiac complications after noncardiac surgery. However, patients with non-insulin dependent diabetes also suffer from the atherosclerotic comorbities of insulin resistance, such as myocardial infarction. The current study aims to assess the risk of cardiac complications after vascular surgery in patients with insulin dependent and non-insulin dependent diabetes mellitus.
Methods 1.416 consecutive vascular surgery patients were included in this cohort study. Diagnosis of diabetes was based on the guidelines of the American Diabetes Association. Diabetes was categorized by insulin use. The primary endpoint was a composite of cardiovascular death, nonfatal myocardial infarction or myocardial ischemia, within 30 days of surgery. In multivariable analysis, corrections were made for comorbidities and glucose control. Troponin T measurements and electrocardiograms were routinely obtained at baseline, and on postoperative day 1, 3, 7, and 30.
The primary endpoint was reached in 171 (16%) non-diabetic, 55 (22%) non-insulin dependent diabetic, and 39 (40%) insulin dependent diabetic patients. After adjusting for age, high-risk surgery, and medical history, both insulin dependent (OR 2.36; 95%-CI 1.43 - 3.89) and non-insulin dependent (OR 1.53; 95%-CI 1.05 - 2.21) diabetes were associated with an increased risk of cardiac complications.
Conclusion Diabetes mellitus, independent of insulin use, is associated with an increased incidence of postoperative cardiac events.
Results Of a total of 1.416 patients, 98 (7%) patients had insulin dependent diabetes and 249 (18%) had non-insulin dependent diabetes.

Methods
Introduction Trans Cervical Resection of Myoma (TCRM) and Endometrium (TCRE) are safe hysteroscopic minimal invasive procedures. However, potentially life threathening venous and paradoxal embolism has been described. Nowadays, bipolar hysterscopic resection using saline for uterine distension is replacing monopolar resection using electrolyte free distension fluid. Fifty patints undergoing TCRM (n=36) or TCRE (n=14) were randomly allocated to undergo hysteroscopic surgery using either monopolar or bipolar diathermia. For detection of embolism, Trans Oesophageal Echography (TOE) was applied and the four chamber view was continuously monitored. Rating of intraoperative embolic events by a five stage classification (grade 1-5) was performed off-line by a blinded independent cardiologist.
Results The overall incidence of VGE was similar in the monopolar and the bipolar group (both 95,8 %). In both groups a positive
tendency was found between the amount of intravasation and the grade of VGE. Bipolar diathermia was associated with a higher incidence of VGE grade 4 (12,5% and 41,7 % respectively, P=0,031) at an average intravasation of 800 ml. Paradoxical embolism was observed in 2 patients (4,1 %), at intravasations of 300 ml and 2300 ml.
Conclusion Grade 1 VGE was seen in almost every patint using either monopolar or bipolar diathermia. More severe grade 3 and 4 VGE was seen in the bipolar group. Although paradoxical embolism was seen in only two patients, its potentially deleterious effects on vital organs raises concern. Irrespective of the type of distention fluid used, significantly more grade 4 VGE was observed when intravasation exceeded 1000 ml.

Introduction The incidence of postoperative myocardial infarction (POMI) after non-cardiac surgery remains high. Because patients often lack specific signs and symptoms, POMI is often not recognized and not treated. Routine postoperative measurement of troponin can be used as a screening instrument to detect POMI. The objective of this study was to determine the effect of routine postoperative troponin measurements on the occurrence of POMI and to determine the percentage of patients with possible POMI that otherwise would go undetected.
Methods This observational cohort study included two groups of non-cardiac surgery patients (age = 60 yrs) who underwent intermediate to high risk surgery. In the control group
(N=886, surgery Jan.-Mar. 2010) troponin was only measured if myocardial ischemia was suspected. In the intervention group (N=866, surgery Jan.-Mar. 2011) troponin levels were routinely measured on the first three postoperative days. If troponin was elevated, the attending ward physician was notified. The outcome POMI was defined as a troponin I level = 0.1ng/L nd ECG abnormalities or clinical symptoms.
ments identifies a considerable number of patients with elevated troponin after noncardiac surgery. This was accompanied by an increase in cardiology consultations and may facilitate adequate treatment in order to prevent cardiovascular events in the year following surgery. Still, adequate treatment for these patients is yet unknown, and is subject to further investigations.
Results Troponin levels were measured in 51 (5.6%) patients in the control group, and in 590 (68.1%) patients in the intervention group. Troponin levels were elevated in 22 (2.5%) and 73 (8.4%) patients, respectively. In 3 (0.3%) and 7 (0.8%) patients, respectively, POMI was diagnosed by a cardiologist. In-hospital mortality within 30 days after surgery occurred in 19 (2.1%) and 12 (1.4%) patients.

Introduction Several studies suggest that providing an actionable recommendation to physicians (directive approach) has a greater impact on physician behavior than only providing a risk assessment from a prediction rule (assistive approach). However, these studies did not compare both approaches within the same physician population. We compared prescription of prophylactic antiemetics using an assistive approach to implementation of a prediction rule for Postoperative Nausea and Vomiting (PONV) to a directive approach of the same rule in the same Anesthesiology Department.
Methods The assistive implementation study was performed as a cluster-randomized trial in 2006 and 2007 (57 physicians and 11.613 patients).
Physicians were randomized either to the intervention or to care-as-usual. Intervention group physicians were only provided with their patients’ predicted PONV risks without a recommendation. The directive study was a before-after study, conducted in 2010 (42 physicians and 1.483 patients). Care-as-usual was recorded during the before’-period. During the intervention period all physicians were provided with recommendations on how many prophylactic antiemetics would be required to sufficiently lower their patients’ PONV risks.
Results
Conclusion
Both approaches increased antiemetic prophylaxis in comparison to care-as-usual (Figure 1). When provided with a recommendation physicians administered significantly (p<0.001) more antiemetics per patient (0.96) than when only exposed to predicted PONV risks (0.34). This difference was greater for patients with a PONV risk >40% (1.5 vs. 0.53 antiemetics per patient, p<0.001).
When using a prediction rule, recommendations based on patient-specific PONV risks increased administration of prophylaxis by physicians. Although both implementation approaches were not compared head-tohead, this study suggests that physicians will better adhere to PONV guidelines, when an actionable recommendation on how to proceed is added to the risk assessment.

Objective Ischemic stroke is a serious complication of cardiac surgery with often adverse outcome. Preoperative cerebral hemodynamic impairment is an independent risk factor for stroke of both embolic and hemodynamic origin. The aim of the study was to determine whether a more detailed preoperative examination of cerebral hemodynamics and perioperative monitoring of cerebral oxygenation reduces stroke incidence and severity related to cardiac surgery.
Methods A historical cohort of 2.494 patients was analyzed. A study group of patients who underwent coronary artery bypass graft surgery after implementation of the Haga Brain Care Strategy was compared to a control group who had coronary artery bypass surgery before implementation. The strategy consists of an extended cerebral examination by preoperative transcranial Doppler and perioperative cerebral oximetry. Outcome measures were stroke rate and stroke severity measured with National Institutes of Health Stroke Scale (NIHSSS). Data were collected from archived patient files.
Results Stroke had occurred in 0.77% of the patients in the study group, compared to
1.67% in the control group. These data show a significant decline in stroke rate, with a odds ratio of 0.390 favoring the study group (95% confidence interval: 0.177 to 0.859; p = 0.019). There was a significant decline in stroke severity as well, with a mean NIHSS of 4.67 in the study group compared to 9.50 in the control group (p = 0.006).
Conclusion These retrospective data suggest that a more careful neurologic examination and monitoring, consisting of preoperative transcranial Doppler and perioperative cerebral oximetry, can reduce both incidence and severity of stroke related to coronary bypass surgery. Confirmation by a prospective randomized trial is warranted.

Introduction Propofol, a short acting hypnotic agent, is increasingly administered during diagnostic and therapeutic procedures to optimize patient wellbeing and satisfaction. Trained nurses often provide non-anesthesiologistadministered propofol (NAAP) sedation in selected patients (ASA 1-2). Patients are monitored using pulse oximetry and visual assessment of respiration (conventional monitoring). Although, evidence to what extent hypoxemia is harmful to patients is lacking, there is general consensus that hypoxemia during sedation should be avoided. Several studies suggest that
capnography in non-intubated patients, has additional value by early detection of alveolar hypoventilation. The medical team was then prompted of capnographic changes by a trained independent observer. We examined whether the use of capnography reduces the incidence of hypoxemia during NAAP sedation.
Methods An open randomized controlled trial was conducted in 426 healthy adult women during minor gynaecology procedures. Patients were randomly assigned to conventional monitoring with capnography or conventional monitoring alone. The primary endpoint was the incidence of oxygen saturations =90 %. Airway interventions were defined as chinlift or jawthrust.
Results The incidence of oxygen saturations =90 % was 25.6 % (55) in the capnography group and 25.6% (54) in the conventional monitoring group (P=0.99). The number of airway interventions was 105 (48.8%) in the capnography group versus 68 (32.2%) in the conventional monitoring group (P<0.001). Oxygen was administered in 27 (13%) patients in the capnography group versus 16 (7.6%) in the conventional monitoring (P=0.09).
Conclusion This study could not demonstrate benefit from capnography on the incidence of hypoxemia during NAAP sedation in healthy women, despite an increased number of airway interventions and oxygen administration.

Introduction Postoperative myocardial infarction is a major cause of mortality after non-cardiac surgery. In the perioperative period, myocardial ischemia generally occurs as STsegment depression on the ECG. Lead V3 to V5 of a 12-leads ECG are most sensitive to detect these changes. During recovery, however, patients are often routinely monitored using a 3-leads ECG. The aim of this study was to assess the ability of continuous 3-leads ECG ST-segment monitoring to detect postoperative myocardial ischemia.
Methods This observational cohort study included 7715 patients =60 years of age undergoing non-cardiac surgery. All patients were monitored during recovery by a Datex Ohmeda system. Data were obtained from an AIMS, that stores the median values per minute of all measured ST-segment levels of lead II. ST-segment depression was defined as a depression of =0.1 mV for at least 10 successive minutes. A troponin I level of =0.1ng/L was defined as a proxy for the outcome (post-
operative myocardial infarction). Troponin was only measured if myocardial ischemia was clinically suspected. We calculated the incidence of ST-segment depression, sensitivity and specificity, and the positive and negative predictive values.
Results Table 1 shows baseline characteristics. In 327 (4.2%) patients ST-segment depression of =10 minutes was detected, and in 229 patients (3.0%) STdepression =20 minutes (Table 2). The sensitivity and specificity of routine ST-segment monitoring were 23% and 96%. The PPV and NPV were 7% and 99%.

Conclusion ST-segment changes during recovery occur infrequently on a 3-leads ECG and routine 3-lead ECG ST-segment monitoring is not adequate in detecting myocardial ischemia. In order to detect and treat postoperative myocardial ischemia, anaesthesiologists should consider alternative strategies to monitor the occurrence of myocardial ischemia in patients undergoing non-cardiac surgery.

Introduction Spinal anesthesia (SA) is associated with hemodynamic changes due to inhibition of the autonomous nervous system. Here we investigated whether diabetic patients, who frequently develop autonomic function disturbances, differ in preoperative heart rate variability (HRV) and develop distinct hemodynamic changes after spinal anesthesia when compared to nondiabetics.
Methods In 19 diabetics and 36 non-diabetics, changes in mean arterial pressure (MAP), heart rate (HR) and cardiac output were recorded before and during SA using Nexfin HD (BMEYE, the Netherlands). HRV was measured before SA, and the very low frequency (VLF: <0.04 Hz), low frequency (LF: 0.04-0.12 Hz) and high frequency (HF: 0.12-0.40 Hz) domains were calculated.
Results Resuscitation volume and SA block height were similar among groups, whereas diabetics were older than controls (718 vs. 6513 years; P<0.05). When compared to con-
trols, diabetics had a lower HRV in the VLF (886(234-2875) versus 316(73-2254) ms2; P<0.05), LF (358(96-983) vs. 139(12-636) versus; P<0.01) and HF domain (220(34901) versus 56(30-372) ms2; P<0.01). Fifteen minutes after SA, diabetics tended to show a smaller decrease in MAP (from 10712 to 9525 mmHg (~10%)) than controls (from 10811 to 8618 mmHg (~21%)), but this was not statistically different. HR changes did not differ between groups, and diabetic and non-diabetic subjects equally maintained cardiac output at 15 minutes after SA (5.61.5 vs. 5.71.3 liter).

Introduction Sepsis or trauma-associated organ hypoperfusion is in some cases associated with hyperfibrinolysis, which has an unfavorable impact on patient outcome. This study investigated whether this hyperfibrinolytic state is also present during shock and hypoperfusion caused by out-of-hospital cardiac arrest (OHCA).
Methods Thirty OHCA patients without (suspected) pulmonary embolism and age > 18 years were included. Upon emergency department admission, total time of chest com-
pressions (CPR time), base excess (BE), pH, serum lactate, and partial oxygen and carbon dioxide pressure (pO2; pCO2) were measured. Furthermore, rotational thromboelastometry (ROTEM) clotting time, maximum clot firmness (MCF) and the lysis parameters maximum lysis (ML), lysis onset time (LOT) and lysis index at 30 and 45 minutes (LI30/LI45) were recorded. A 20% decrease of the MCF was defined as hyperfibrinolysis. Data represent mean SD.

Introduction Natriuretic peptides (NP) are secreted by cardiomyocytes and relate to prognosis in patients with chronic heart failure and acute coronary disease.This remains controversial in critically ill patients. In this prospective study we investigated the relation of N-terminal pro B-Type Natriuretic Peptide (NT-proBNP) concentration on admission and short- and long-term mortality in patients admitted to a mixed Intensive Care Unit (ICU).
Methods We included all consecutive patients admitted to our mixed ICU in a 9-monthperiod. NT-proBNP was measured in routinely collected blood on admission. Follow-up of the patients was 5 years.

Kaplan-Meier curve and ROC analysis was performed with the statistical program SPSS version 18.
Results The data of 298 patients were analyzed. 3 patients were lost to 5 year follow-up. Median age was 60 (45-71) years. On admission 23% of the patients were septic and 56,7% ventilated. Median NT-proBNP was 867 (167-3136) ng/l. 28-day-mortality and
groep 1 5-year-mortality was respectively 20,1% and 49,8%. Both were related to NT-proBNP with an area under the ROC curve (AUC) of 0,742 and 0,61(p<0,001). The group was divided in quartiles. Kaplan-Meier analysis showed significant difference in survival between the first and fourth quartile of NT-proBNP (74% vs 28,4%) (p<0,0001), see Figure 1a. In the patients who survived the first 28 days (n=235) 5-year-survival was 63%. AUC was 0,61 and survival in the first
quartile of NT-proBNP was 77,6% versus 50% in the fourth quartile (p=0,001). See Figure 1b.
Conclusion NT-proBNP concentration on admission predicts short- and long-term mortality in critically ill patients. Excluding patients with early mortality (before 28 days), NTproBNP still shows prognostic value for longterm (5 year) mortality.

Introduction Although intra-arterial blood pressure measurements are gold standard in pediatric hemodynamic management, their applicability is limited due to their invasive nature. Here we evaluated the level of agreement between non-invasive arterial blood pressure measurements using Nexfin HD with intra-arterial monitoring as alternative for hemodynamic measurements in children.
Methods The study included 41 children aged 2-18 years admitted for surgery or pediatric intensive care with an intra-arterial line as
part of their medical treatment. Systolic (SBP), diastolic (DBP) and mean arterial pressure (MAP) values were simultaneously obtained during 20 minutes, using an intra-arterial line and the non-invasive Nexfin HD monitor (BMEYE, Amsterdam, the Netherlands). Data were analyzed using Pearson correlations and Bland-Altman testing.
Results In thirteen cases the smallest finger cuff was used and this cuff had to be reapplied for several times to obtain a sufficient signal. Pearson correlation showed an association between invasive and non-invasive measurements for SBP, DBP and MAP of r=0.84, r=0.87 and r=0.88, respectively (all P<0.0001). Bland-Altman Analysis for SBP,
DBP and MAP showed a bias with limits of agreement (LOA) of 13.4 mmHg (-12.8 - 39.7), -0.21 mmHg (-13.2 - 12.8) and 2.61 mmHg (-12.5 - 17.4), respectively.
Conclusion The Nexfin HD is an easy to use continuous arterial blood pressure monitor in children. We found an underestimation of systolic blood pressure measurements by Nexfin HD in children, whereas the level of agreement of diastolic and mean arterial blood pressure with intra-arterial measurements was high. Our study suggests reevaluation of the finger cuff or Nexfin software technology to improve the agreement of non-invasive systolic blood pressure values with those obtained by intra-arterial measurements.

Introduction Intravenous cannulation can be challenging in children with a dark skin color. Visualization of veins with near-infrared (NIR) light might be helpful. Therefore, the effectiveness of a NIR vascular imaging system (VascuLuminator: VL) was investigated in facilitating intravenous cannulation in chil-
dren at the operating room in a population with predominantly dark skin color.
Methods All consecutive children (0-15 yr) in need for intravenous cannulation at the operating room of a primary general hospital in Curacao were included in this pragmatic cluster randomized clinical trial. The presence of the VL at the operating complex was randomized in clusters of a week.
Results The success at first attempt was 63% (27/43) in the VL Group vs. 51% (23/45) in the Control Group (p=0.27). There is a trend towards a shorter time to successful can-
nulation in the VL Group compared to the Control Group (159 38 s vs. 186 47 s; p=0.60). Subgroup analysis shows better results in children younger than 3 years of age, (success at first attempt VL Group: 61% (14/23) vs. Control Group: 35% (8/23); p=0.08) and in patients previously estimated to be difficult (success at first attempt VL Group: 50% (8/16) vs. Control Group: 8% (1/13); p=0.03).
Conclusion NIR vascular imaging shows to be promising in intravenous cannulation in children with a dark skin color, especially in young children < 3 yr and in children previously estimated to be difficult to puncture.

Retrospective evaluation of chest ultrasound performed by a Dutch helicopter emergency medical service R. Ketelaars, N. Hoogerwerf, G.J. Scheffer Universitair Medisch Centrum St. Radboud, Nijmegen
Introduction Because of advancements in technology, the use of a portable ultrasound (US) machine in the out-of-hospital setting becomes increasingly feasible. It has a diagnostic and therapeutic advantage, doesn’t impose a delay on the treatment of patients. It can be used in flight and can easily be taught to flight clinicians who have little experience with this modality.
Methods Since 2007 US is used by the Helicopter Emergency Medical Service of Nijmegen, The Netherlands. Data on every air medical
flight are routinely collected in a database. Flights between 2007 and 2010 on which an US examination of the chest was performed, were selected. Data on patient characteristics, properties of US examinations, US diagnoses and impact on medical treatment given were collected and subsequently analysed.
Results On a total of 2.572 air medical flights, 326 US examinations of the chest were performed on 281 (11%) patients. Among them were 15 (5%) children below the age of four. On average it took 2.77 minutes to perform an US examination and became less in the more recent years. Common diagnoses were: asystole, reduced contractility, suboptimal cardiac
filling and pneumothorax. After US the plan for treatment changed in 56 (20%) patients. In 7 (2%) we abandoned introduction of a chest tube. In 10 (4%) a different than devised destination for definitive care was selected. Because serious injuries could be excluded with US while still on the scene, a lower level hospital was more often selected than a higher one.
Conclusion Based on US examination in the out-ofhospital arena, treatment and destinations for definitive care are often changed. Invasive therapies are performed less often. Further prospective study will be needed to establish the value of out-of-hospital US in our region.

Background Studies on the efficacy of ketamine in the treatment of chronic pain indicate that prolonged or repetitive infusions are required to ensure prolonged pain relief. Few studies address ketamine-induced toxicity. Here we present data on the occurrence of ketamineinduced liver injury during repeated administrations of S(+)-ketamine for treatment of chronic pain in patients with complex regional pain syndrome type 1 as part of a larger study exploring possible time frames for ketamine re-administration.
Methods Six patients were planned to receive two continuous intravenous 100-h S(+)-ketamine infusions (infusion rate 10-20 mg/h) separated by 16 days. Three of these patients developed hepatotoxicity.
Results Patient A, a 65-year-old female developed an itching rash and fever during her second exposure. Blood tests revealed elevated liver enzymes (ALT, APT, AST and gamma-GT, all > 3 times the upper limit of normal) and modest increased eosinophilic leucocytes. Patient E, a 48-year-old female, developed elevated liver enzymes of similar pattern as Patient A during her second ketamine ad-
ministration and a weakly positive response to anti-nuclear antibodies. In a third patient, Patient F, a 46-year-old male, elevated liver enzymes (ALT and gamma-GT) were detected on the first day of his second exposure. In all patients the ketamine infusion was promptly terminated and the liver enzymes slowly returned to reference values within two months.
Conclusion Our data suggest an increased risk for development of ketamine-induced liver injury when the infusion is prolonged and/or repeated within a short time frame. Regular measurements of liver function are therefore required during such treatments.

Results Our data show that spinal GAP-43 expression is enhanced in the superficial dorsal horn of the L4-L5 spinal cord, peaking at 5 days after SNT and gradually returning to uninjured levels thereafter. GAP-43 was found to co-localize with peptidergic C fibers expressing calcitonin gene-related peptide (CGRP), but not with non-peptidergic C fibers (IB4), A fibers (VGLUT-1), spinal interneurons expressing PKC?, or
groep 1 astroglial cells (GFAP). Daily intrathecal bolus injections with the glial inhibitor PPF reduced mechanical hypersensitivity by 5 days after SNT. GAP-43 expression and its regulation in CGRP fibers remained unaffected by PPF treatment.
Conclusion On the basis of these data we conclude that spinal GAP-43 regulation in CGRP-expressing fibers, as a read-out of structural plas-
ticity in the spinal nociceptive network, is not a main player in the onset of mechanical hypersensitivity after peripheral nerve injury. As CGRP is classically regarded as a mediator of sensitization, the contribution of structural changes in CGRP-expressing fibers to the maintenance of neuropathyinduced pain hypersensitivity remains to be explored.

Sex differences in pain sensitivity have been consistently found but the basis for these differences is incompletely understood. The present study aimed to study how pain-related neural processing varies across the menstrual cycle in normally cycling, healthy females, and whether menstrual cycle effects represent fluctuating gonadal hormone levels.
Serum hormone levels confirmed that scans were performed at the appropriate cycle phase. No significant cycle phase differences were found for pain intensity or unpleasantness ratings of stimuli applied during fMRI scans. However, lower pressure pain thresholds were found in the follicular compared to other phases. Painful stimuli elicited significant brain activation in several regions traditionally associated with pain processing, including medial thalamus, anterior and mid-insula, mid-cingulate, foot primary somatosensory cortex,
Methods Fourteen subjects participated in four test sessions during their menstrual, mid-
secondary somatosensory cortex, cerebellum, and frontal medial regions. Inferior parietal lobule, occipital gyrus, cerebellum and several frontal regions demonstrated interaction effects between pain and cycle indicating differential processing of painrelated responses across different menstrual cycle phases.
Conclusion These results indicate that pain-related cerebral activation varies significantly across the menstrual cycle, even when perceived pain intensity and unpleasantness remain constant. The brain regions most affected suggest that motor and cognitive systems influenced by pain are the ones susceptible to menstrual cycle effects.

Introduction Previous studies show that the NMDAreceptor plays a role in the development of hyperalgesia [1] Administration of ketamine, an NMDA-receptor antagonist, results in a smaller area of hyperalgesia surrounding a surgical wound, and reduced opioid consumption [2]. Methadone has actions on the opioid and NMDA-receptors, and so the goal of this pilot study was to investigate if it is an effective perioperative analgesic, and if it has a beneficial effect on postoperative hyperalgesia and opioid consumption.
Methods In this observational study, 30 ASA1/2 patients undergoing laparoscopic cholecystectomy were included. All received TCI propofol/remifentanil anaesthesia according to a standard protocol. Before surgical incision either morphine (0,2mg/kg) or methadone (0,2mg/kg) was administered intravenously. The choice was made by the attending anesthesiologist, but was blinded for the investigators. Von Frey monofilaments were used to quantify postoperative hyperalgesia. Periincisional areas of hyperalgesia and tactile pain threshold were registered at various time points. Other outcome variables included postoperative morphine consumption, pain (NRS,NCPI) and sedation (OAA/S) scores.
Results The demographic, surgical and anesthetic characteristics of the groups were comparable. Mean duration of remifentanil infusion was 52 min in both groups. Mean(SD) remifentanil effect-site concentration was 6.63ng/ml (0.49) and 6.89ng/ml (0.90) in the morphine and methadone groups respectively (ns). The median[IQR] periincisional hyperalgesia area (xiphoid process) at 24h was similar (morphine group:1905mm2[780/3565]; methadone group 1435mm2[419/2207];ns). Median[IQR] 24h cumulative postoperative morphine consumption was also similar (morphine group:7[5/17], methadone group:2[0/17];ns). There were no significant differences in tactile pain threshold, pain and sedation scores.

Background A randomized controlled trial published in 2000 showed great efficacy of intrathecal (i.t.) methylprednisolone acetate (MPA) with lidocaine in patients suffering from intractable postherpetic neuralgia (PHN). (Kotani, 2000, N. Eng J. Med) Because the large treatment effect was never independently confirmed and there are ongoing concerns about its safety, i.t. MPA never became standard care for intractable PHN. We report results of a replication trial assessing safety and pain relief in patients with PHN.
Methods Out of the intended 39 PHN patients, 10 were randomized to active treatment with i.t. MPA 60 mg + lidocaine 60 mg (n=6) or control (i.t. lidocaine 60 mg; n=4). Patients received four injections at 7-day intervals. Visual analogue scores (VAS) for pain, allodynia and quality of life (QOL) were measured until 8 weeks follow-up (primary endpoint).
Results All 6 patients treated with MPA experienced an increase in global pain at 8 weeks, as compared to 1 of 4 patients in the control group. The allodynic area increased up to 20 fold in 4/6 MPA treated patients and decreased in 3/4 patients in the control group. QOL decreased in 5/6 MPA treated patients, compared to 2/4 in the control group. Given these results and those of a recent safety study on i.t. MPA in dogs, no additional patients were enrolled.

Introduction Previously we showed that K+ Cl- cotransporter 2 (KCC2) protein levels are downregulated in painful experimental peripheral neuropathy and consequently lead to dysfunctional GABAA receptor-mediated inhibition. Spinal cord stimulation (SCS)-mediated analgesia is known to increase extracellular GABA levels. From this we expect decreased intracellular GABA levels in responders to SCS but not in non-responders. Furthermore if GABAA receptor-mediated inhibition contributes to SCS-induced analgesia, reduced KCC2 and/or GABAA receptor levels might be expected only in non-responders to SCS. Here we explored the mechanisms underlying SCS-induced alterations in extracellular
GABA levels and GABAA receptor-mediated disinhibition both in responders and nonresponders to SCS.
Methods Sprague Dawley rats (n=54) were subjected to peripheral nerve injury followed by 30 min of SCS. Withdrawal thresholds to von Frey monofilaments were determined during the development of neuropathic pain and SCS per se. Responders to SCS were characterized by an increase of = 2 filaments at the end of SCS. Sham-stimulated neuropathic rats served as controls. Ipsilateral cryosections were immunostained to quantify intracellular GABA levels, the number of GABAergic interneurons and GABAA receptor protein levels in spinal dorsal horn Rexed laminae 1-3. Dorsal horn KCC2 protein levels were measured by immunoblotting.
Results 1) Intracellular GABA levels were decreased in responders to SCS compared to nonresponders and sham SCS. 2) No differences were observed in the number of GABAergic cells or GABAA receptor concentrations in all groups (sham SCS, responders SCS, non-responders SCS). 3) KCC2 protein levels were equally decreased in all groups.
Conclusion These data clearly demonstrate that the pain-relieving effect of SCS involves release of spinal dorsal horn intracellular GABA. Alterations in the number of GABAergic interneurons and GABAA receptor-mediated inhibibition do not contribute to the analgesic properties.

Introduction The incidence of acute post-operative pain (acute pain) after day-case surgery is 2040%. However, little is known about health care consumption due to acute pain. This study evaluates the relation of acute pain and health care consumption during the first four days after day-case surgery.
Methods During eighteen months, 1256 patients undergoing ambulatory surgery were prospectively included. All patients received a prescription with pain medication and
completed a questionnaire with regard to preoperative status one week before surgery. Another questionnaire was completed the fourth day after surgery to determine health care consumption (visits to GP, specialist or emergency unit). Acute pain was measured by an 11-point numeric rating scale (NRS). NRS > 4 was considered clinically relevant.
Results Of all included patients, 248 (20%) experienced moderate to severe post-operative pain (NRS > 4), of whom 47 patients (19%) visited the GP, specialist or emergency unit for 78 times during the first four days after surgery. In the group with mild pain (NRS = 4; n = 1008) on the other hand, 55 patients
(5%) made 69 visits to health care (p < 0.001). Besides that, in the group with mild pain, 640 patients (63.5%) did not return to their regular daily activities on the fourth day, compared to 213 patients (85.9%) in the group with moderate to severe pain (p < 0.001).
Conclusion Our study showed that acute pain was associated with a significant increase of health care consumption after day-case surgery during the first four postoperative days. Besides, significantly more patients with moderate to severe pain did not return to their regular daily activities compared to patients with mild or no postoperative pain.

Om hernieuwd optreden van neuromusculaire blokkade te voorkomen, dienen de aanbevolen doses van sugammadex te worden gebruikt. Een verhoogd risico op bloedingen kan niet worden uitgesloten bij patiënten: met erfelijke vitamine K afhankelijke stollingsfactorgerelateerde deficiënties; met reeds bestaande coagulopathiën; die coumarinederivaten gebruiken en met een INR boven 3,5; die antistollingsmiddelen gebruiken en een dosis van 16 mg/kg sugammadex ontvangen. Sugammadex mag niet worden gebruikt voor opheffing van blokkades geïnduceerd door niet‑steroïde neuromusculair blokkerende stoffen en door steroïde neuromusculair blokkerende stoffen, anders dan rocuronium of vecuronium. Indien de neuromusculaire blokkade wordt opgeheven onder voortzetting van de anesthesie, dienen aanvullende doses van het anestheticum en/of opioïd te worden gegeven op geleide van de klinische indicatie. Indien hernieuwde neuromusculaire blokkade is vereist vóór het verstrijken van de aanbevolen wachttijd van 24 uur, dient een niet‑steroïde neuromusculair blokkerende stof te worden gebruikt. Sugammadex is niet onderzocht bij patiënten, die rocuronium of vecuronium krijgen op de Intensive Care. Mogelijke interacties: In situaties waar mogelijke verdringingsinteracties verwacht kunnen worden, dienen patiënten (na parenterale toediening van een ander geneesmiddel binnen 6 uur na toediening van sugammadex) zorgvuldig gecontroleerd te worden op tekenen van hernieuwd optreden van een blokkade (voor maximaal ongeveer 15 minuten). In situaties waar mogelijke bindingsinteracties kunnen optreden wordt de arts geadviseerd om het geneesmiddel opnieuw toe te dienen of de toediening van een therapeutisch gelijkwaardig geneesmiddel en/of niet farmacologische interventies te overwegen (zie rubriek 4.5*). Nierfunctiestoornis: Het gebruik van sugammadex bij patiënten met een ernstige nierfunctiestoornis wordt niet aanbevolen. Leverfunctiestoornis: Patiënten met een ernstige leverfunctiestoornis moeten met grote voorzichtigheid worden behandeld. Vertraagd herstel: Aandoeningen waarbij sprake is van een verlengde circulatietijd, zoals cardiovasculaire aandoeningen, gevorderde leeftijd of oedeemvorming kunnen gepaard gaan met langere hersteltijden. Geneesmiddelenovergevoeligheid: Artsen moeten voorbereid zijn op de mogelijkheid van overgevoeligheidsreacties op geneesmiddelen (waaronder anafylactische reacties) en de nodige voorzorgsmaatregelen treffen. Natriumbeperkt dieet: Indien er meer dan 2,4 ml oplossing moet worden toegediend, dient hier rekening mee te worden gehouden bij patiënten met een natriumbeperkt dieet. Verlenging van het QTc‑interval: De routinematige voorzorgsmaatregelen voor de behandeling van aritmie moeten in overweging worden genomen. Pediatrische populatie: De interacties en waarschuwingen voor volwassenen gelden ook voor kinderen. Interacties: Voor toremifeen kunnen verdringingsinteracties niet worden uitgesloten. Het gebruik van fusidinezuur in de preoperatieve fase kan het herstel van de T4 /T 1‑ratio tot 0,9 enigszins vertragen. Voor hormonale anticonceptiva kon een klinisch relevante bindingsinteractie niet worden uitgesloten. In het algemeen interfereert sugammadex niet met laboratoriumtests, met als mogelijke uitzondering de progesteronbepaling in serum. Doseringen van 4 mg/kg en 16 mg/kg sugammadex kunnen de geactiveerde partiële tromboplastinetijd (aPTT) en protrombinetijd (PT) voor korte duur (< 30 minuten) licht verlengen. Bijwerkingen: De veiligheid van sugammadex is beoordeeld op basis van een geïntegreerde veiligheidsdatabase van ongeveer 1700 patiënten en 120 vrijwilligers. Zeer vaak: Dysgeusie (metalen of bittere smaak), werd vooral waargenomen na doses van 32 mg/kg sugammadex of hoger. Vaak: Complicaties bij anesthesie, indicatief voor herstel van neuromusculaire functie. Soms: Overgevoeligheidsreacties op geneesmiddelen zijn waargenomen bij een aantal patiënten en vrijwilligers in klinische onderzoeken; de postmarketingfrequentie waarin deze reacties optreden is niet bekend. Deze reacties, die varieerden van geïsoleerde gevallen van huidreacties tot ernstige systemische reacties (anafylaxe, anafylactische shock), zijn ook voorgekomen bij patiënten die niet eerder blootgesteld waren aan sugammadex. Symptomen die geassocieerd kunnen zijn met deze reacties zijn: bloedstuwing, urticaria, erythemateuze huiduitslag, (ernstige) hypotensie, tachycardie en zwelling van de tong en keelholte. Na behandeling met sugammadex zijn enkele gevallen van awareness gerapporteerd. Hernieuwd optreden van een blokkade: De incidentie van het hernieuwd optreden van een blokkade was 2% na gebruik van sugammadex en 0% in de placebogroep. Vrijwel al deze gevallen kwamen voor in dose‑finding onderzoeken met suboptimale doses (minder dan 2 mg/kg) (zie rubriek 4.4*). Verkorte productinformatie Sevorane® (aug 2010) Naam en samenstelling: Sevorane®, vloeistof voor inhalatiedamp, bevat als actief bestanddeel sevofluraan. Daarnaast bevat Sevorane 300 ppm water. Indicaties: Inductie en onderhoud van algemene anesthesie voor klinische en poliklinische chirurgische ingrepen bij volwassenen en kinderen. Contraindicaties: overgevoeligheid voor sevofluraan of andere gehalogeneerde middelen, bekende of vermoede genetische aanleg voor maligne hyperthermie, icterus en/of onverklaarbare koorts bij een eerdere toepassing van sevofluraan of van een ander gehalogeneerd anestheticum. Waarschuwingen: Sevorane dient alleen te worden toegediend door personen die getraind zijn in de algemene anesthesie. Voor het openhouden van de luchtwegen, kunstmatige beademing, zuurstoftoediening en reanimatie dienen hulpmiddelen onmiddellijk beschikbaar te zijn. Alleen toedienen met behulp van een speciaal voor sevofluraan gecalibreerde verdamper zodat de diepte van de anesthesie gemakkelijk en snel kan worden veranderd. Hypotensie en ademdepressie nemen toe met de anesthesiediepte. Bij de inductiefase kan hoesten voorkomen. Tijdens gebruik van sevofluraan kan in geïsoleerde gevallen een AV-junctioneel ritme ontstaan, met name wanneer tevoren een vagolyticum gegeven is, zoals atropine. Tijdens de onderhoudsfase van de anesthesie geeft een verhoging van de sevofluraanconcentratie een dosisafhankelijke daling van de bloeddruk. Een excessieve bloeddrukverlaging kan gerelateerd zijn aan de diepte van anesthesie. Deze verlaging kan gecorrigeerd worden door de ingeademde sevofluraanconcentratie te verlagen. Het ontwaken dient zorgvuldig te worden vastgesteld voordat de patiënt de verkoeverkamer verlaat. Kleine stemmingsveranderingen kunnen aanhouden gedurende een aantal dagen na toediening. Patiënten moeten geadviseerd worden dat het uitvoeren van handelingen die oplettendheid vereisen, tijdelijk verminderd kunnen zijn. Een ontwakingsdelier komt ongeveer 2-3 keer zo vaak voor bij kinderen onder de 6 jaar, dan bij volwassenen. Agitatie bij het ontwaken is bij jonge kinderen vaker gerapporteerd bij anesthetica met een korte ontwakingsduur zoals sevofluraan in vergelijking met anesthetica met een langere ontwakingsduur. Met voorzichtigheid toepassen bij patiënten met kans op verhogingen van de intracraniële druk. Hierbij dient sevofluraan samen met handelingen, die de intracraniële druk verlagen, zoals hyperventilatie te worden toegepast. Sevofluraan dient met voorzichtigheid te worden toegepast bij patiënten met coronaire hartziekten, omdat sevofluraan een verwijdend effect heeft op het systemisch en coronair arterieel systeem, en bij patiënten met myasthenia gravis, omdat deze patiënten zeer gevoelig zijn voor geneesmiddelen die de ademhaling onderdrukken. Bij gevoelige patiënten kunnen inhalatie anesthetica een hypermetabole toestand van de skeletspieren veroorzaken welke leidt tot een hoge zuurstofbehoefte en tot het klinische syndroom maligne hyperthermie. Behandeling omvat het staken van sevofluraan, intraveneuze toediening van dantroleennatrium en ondersteunende therapie. Later kan nierfunctieverlies optreden; de urineproductie dient te worden gevolgd en zo mogelijk in stand gehouden. Het gebruik van inhalatie anesthtica is in zeldzame gevallen in verband gebracht met verhogingen van de serumkaliumspiegels die in de postoperatieve fase bij pediatrische patiënten hebben geleid tot hartaritmieën en de dood. Patiënten met een sluimerende of actieve neuromusculaire aandoening, met name de ziekte van Duchenne, zijn kwetsbaar. Tevens is het gebruik van succinylcholine hiermee in verband gebracht; bij deze patiënten trad een verhoging van het serumcreatinine-kinase op, en in enkele gevallen met myoglobinemie. Bij optreden van hyperkaliëmie en/of ritmestoornissen wordt aanbevolen deze snel en doeltreffend te behandelen. Bij verdenking op een neuromusculaire aandoening dient nadere evaluatie plaats te vinden. Sevofluraan dient met voorzichtigheid te worden toegepast bij patiënten met nierinsufficiëntie. Na anesthesie met gehalogeneerde anesthetica werden stoornissen van de leverfunctie, icterus en fatale levernecrose gemeld. Een cirrose of andere anomalie kan een reden zijn om een ander anestheticum te kiezen. Zeldzame gevallen van extreme warmte, rook en/of spontaan vuur in de anesthesiemachine zijn gerapporteerd tijdens het gebruik van sevofluraan samen met uitgedroogd CO2absorbens. Een ongewoon vertraagde stijging of onverwachte afname van de ingeademde sevofluraan concentratie kan samenhangen met een excessieve verwarming van het CO2-absorbensreservoir. Bij vermoeden van uitgedroogd zijn van het CO2-absorbens, dient het te worden vervangen voor toediening van sevofluraan. CO2-absorbentia dienen routinematig te worden vervangen onafhankelijk van de staat van de kleurindicator. Bijwerkingen: Sevofluraan kan een dosisafhankelijke hart- en ademdepressie veroorzaken. Zeer vaak (≥1/10) komen voor: misselijkheid, braken, hoest, hypotensie. Vaak (>1/100-<1/10) komen voor: fluorosis, agitatie (komt zeer vaak voor bij kinderen), slaperigheid, duizeligheid, hoofdpijn, bradycardie (komt zeer vaak voor bij ouderen), tachycardie, hypertensie, ademhalingsmoeilijkheden, larynchospasmen, speekselvloed, koude rillingen, koorts, hypothermie. Soms (>1/1.000<1/100) komen voor: verhoogd bloedglucose, abnormale leverfunctietests, toename van aantal witte bloedcellen. Zelden (>1/10.000-<1/1.000) komen voor: anafylactische reactie, anafylactoïde reactie, convulsie, dystonie, bronchospasmen, hepatitis, leverfalen, levernecrose, pruritus, huiduitslag, urticaria, maligne hyperthermie (deze bijwerkingen werden spontaan gemeld na registratie van Sevorane en werden vrijwillig gemeld uit een onbekende populatie. Sevoflurane attenuates microvascular recruitment in the human myocardium C.S.E. Bulte, J. Slikkerveer, O. Kamp, S.A. Loer, C. Boer, R.A. Bouwman VU Medisch Centrum, Amsterdam
Introduction The influence of general anesthetics on the myocardial microvasculature is unknown, partly due to the lack of a noninvasive intraoperative imaging technique. Myocardial contrast echocardiography allows measurement of the myocardial relative blood volume (rBV), reflecting microvascular density, and absolute myocardial blood flow (MBF). Using myocardial contrast echocardiography, we aimed to investigate anesthesia-related alterations in myocardial microvasculature and myocardial blood flow.
Methods Thirteen cardiovascular healthy patients (6 women; age 4311 years) scheduled for general anesthesia were included. Contrast
echocardiography was performed before and during the administration of 1.0 MAC sevoflurane and included relative blood volume and myocardial blood flow measurements at rest and during adenosineinduced hyperemia, causing recruitment of functional microvascular density. Flow reserves were calculated by MBFhyperemia/ MBFrest. Data are presented as meanSD.
Results Preoperatively, basal rBV increased from 0.100.02 ml/ml to 0.140.03 ml/ml (p<0.001). In contrast, we observed no increase in rBV during intraoperative hyperemia. Furthermore, intraoperative basal rBV (0.080.02 ml/ml) and hyperemic rBV (0.080.03 ml/ ml) were significantly lower compared to preoperative measurements (both p<0.05). In the preoperative setting, basal MBF increased from 1.00.3 ml/min/g to 3.50.7 ml/min/g during hyperemia (p<0.001). Preoperative and intraoperative baseline MBF
were comparable. In response to hyperemia, MBF was lower in the intraoperative setting reflected by an intraoperative flow reserve of 2.30.8 versus 3.81.7 preoperatively (p=0.0008).
Conclusion These data show that sevoflurane reduces functional microvascular density under basal conditions, whereas microvascular recruitment during adenosine-indcued hyperemia is attenuated. Interestingly, intraoperative basal myocardial blood flow is preserved suggesting intact coronary autoregulation. In contrast, hyperemic myocardial blood flow, and thus myocardial flow reserve, is reduced in the intraoperative setting. Whether these observations are the result of hemodynamic changes, direct effects of sevoflurane on the myocardial microvasculature or an interaction between sevoflurane and adenosine remains to be elucidated.

Introduction Anesthesia Information and Management Systems (AIMS) are useful as a resource for research databases. However, AIMS cannot decide whether or not a certain value is a true’ value or an artifact. Such unreliable estimates may influence research results. We assessed the reliability of AIMS data for research purposes by measuring the occurrence rate of artifacts in vital parameter values recorded in our AIMS.
Methods This prospective observational study included 86 patients, who were monitored by a Datex Ohmeda system. In order to prevent capturing artifacts, data are recorded in the AIMS and displayed real-time only after applying a filter. For HR, saturation, ST-segment and IBP the median value per minute is calculated and stored. The NIBP is recorded every time it is measured. In addition to the AIMS collecting data,

data were collected manually in the operating room. All possible artifacts were verified with the attending anesthesiologist as to whether it concerned a true value or an artifact.
Results The study included 86 patients, of which 84 (98%) underwent general anesthesia. HR, NIBP, saturation and ST-segment were measured in all patients, whereas IBP was

measured in twelve. In total 9.534 minutes of anesthesia time were recorded (Table 1) and the incidence of true artifacts ranged from 0.02% (95% CI: 0.01-0.08%) for HR to 13.49% (95% CI: 12.20-14.89%) for IBP measurements. The incidence of artifacts in IBP
groep 1 measurements was often (53%) caused by relocation of the pressure sensor.
Conclusion Storing a median value per minute to filter capturing of continuous vital parameter
values in an AIMS database provides reliable data for HR and saturation, with artifact rates below 0.5% and reasonable reliability for ST segment and NIBP (artifact rates 4.7% and 2.3%, respectively).

Helium induced preconditioning protocols in neonatal ventricular rat myocytes D. van de Vondervoort, B. Preckel, M.W. Hollmann, N.C. Hauck-Weber Academisch Medisch Centrum, Amsterdam
Introduction Noble gases without anaesthetic properties, such as helium, can induce cardioprotection. A previous in vivo study showed that protection is mediated through activation of prosurvival-signalling-kinases. A study on helium-induced preconditioning and postconditioning showed GSK-3 phosphorylation during ischemia was reduced after helium application. Yet, involvement of ERK1/2 and Akt could not be confirmed on a biochemical level, even though ischaemia/ reperfusion has previously been shown to activate the Reperfusion-Injury-SalvageKinase (RISK) pathway. To further elucidate these pathways, ERK1/2 phosphorylation was studied after helium administration in
vitro. We hypothesized that helium is able to phosphorylate ERK1/2 in isolated cardiomyocytes. Furthermore, we aimed to optimise the in vitro protocol by studying different durations of helium administration.
Methods Cardiomyocytes were isolated from 1-3 day old Wistar rat pups. Cells were treated for the duration of either 3x5 minutes or 15 minutes with either helium (5% CO2, 25% O2, 70% helium) or control gas (5% CO2, 25% O2, 70% N2) or left untreated. Culture medium was refreshed after each period of helium or control gas application to ensure complete washout. After completion of the protocol cells were harvested for Western Blot analysis.
Results Both helium (0,190,14) and control-gas (0,180,09) administration for 3x5 minutes
demonstrated an increase in ERK1/2 phosphorylation (in mean phospho/total ratioSD) vs. untreated (0,120,02). Pre-treatment with helium did not increase phosphorylation more than control-gas. Both helium (0,640,29) and control-gas (0,650,30) administration for 15 minutes demonstrated an increase in ERK1/2 phosphorylation vs. untreated (0,390,25). Pre-treatment with helium did not augment phosphorylation more than control-gas.
Conclusion There is a difference between treated and untreated groups. No difference is found between helium- vs. control-gas administration, suggesting that even changing of the media influences ERK phosphorylation. Our data suggest that the administration of helium in vitro needs further optimization.

Background Testing the effect on breathing of newly developed opioid analgesics should be part of safety-screening in phase I and II trials. No specific guidelines are available on the methods of choice.
Methods In a group of 12 healthy male volunteers, the effect of an oral experimental opioid was assessed on resting respiratory variables (respiratory rate (F), end-tidal PCO2
(PETCO2), oxygen saturation (SpO2) and ventilation (Vi)), the ventilatory response to CO2 and iso-hypercapnic Vi. The iso-hypercapnic Vi data was further analyzed using a PK/PD approach to provide the temporal profile of the opioid in terms of its plasma concentration.
Results The opioid had no systematic effects on resting F, PETCO2 and SpO2 while its effect on resting Vi was relatively small (30% depression) occurring at varying intervals following intake. In contrast, large systematic effects were observed on the Vi-CO2 response (depression 75%, time constant 0.8 0.2 h) and iso-hypercapnic Vi (depression
45%, time constant 1.2 0.1 h). The PK/PD analysis revealed an onset/offset time similar to that of morphine with a 2000-times greater potency.
Conclusion Data obtained from breathing with artificially increased end-tidal PCO2 was most sensitive and informative on the effect of the opioid on the ventilatory control system. We therefore advocate the use of these tests when assessing the safety of an opioid or any other respiratory depressant in phase I and II trials. However, further studies such as on reversibility of respiratory depression are needed to get the full characterization of the opioid’s safety.

Introduction After opening of the aortic valve, a pulse pressure wave propagates through the blood vessels. This is called the vascular transit time (VTT) and is a measure of arterial resistance. As it is impossible to reliably measure VTT, we measure the pulse transit time (PTT). PTT is the time between the R-wave on ECG, and the foot of the pulse wave on the photoplethysmography (PPG) signal. Pre-ejection period (PEP) is a component of PTT; this is the time between electrical activation of the ventricles (R-wave) and opening of the aortic valve. In formula: PTT=PEP+VTT. PTT can be affected by a change either in PEP or VTT. The aim of this study is to measure the variability of PEP in rest with echocardiography as a golden standard, in order to evaluate the contribution of PEP to PTT.
Methods In this study 20 healthy volunteers (18-65 yrs) without cardiovascular diseases were included. The participants were in left lateral position, ECG electrodes were placed for Rwave detection to calculate the PTT and for ECG-registration during echocardiography. Four PPG-sensors were placed, 2 at the index fingers and 2 at the big toes. Echocardiography was performed simultaneously with the PTT-measurements during 10 heartbeats. Subsequently PEP and PTTs of the 4 locations were calculated for the corresponding heartbeats.

Results We included 20 participants (27.1yr, ssd 4.1) with mean PEP 58.5ms, sd13.0 The intra-subject PEP-variation was 2.0-5.9ms. The contribution of PEP is approximately 21% of PTTfinger and 16% of PTTtoe.
Conclusion Variability of PEP in rest is small. Therefore a change in PTT is due to the vascular component of the PTT rather than a change in PEP

Introduction Proinflammatory cytokines, predominantly KC and IL-1, have been shown to play an important role in the pathogenesis of ventilator-induced lung injury (VILI). Caspase-1 is activated within the inflammasome and required for the processing of the inactive precursor pro-IL1. Interleukin-1-converting enzyme (ICE) knockout mice are lacking the gene coding for caspase-1.
Methods A group of wild-type (WT) mice and a group of ICE knockout (-/-) mice were ventilated using clinically relevant ventilator settings (tidal volume 8 ml/kg). Lungs and plasma were collected in order to determine blood gas values, cytokine profiles, leukocyte influx and histopathological features.
Results Ventilated mice showed increased IL-1, KC and TNFa concentrations in lung homogenate, increased IL-6, IL-10, KC and TNFa in plasma and increased leukocyte influx in the lungs.
No significant differences were observed between the WT group and the ICE-/group.
Conclusion The inflammasome appears not to be involved in the production of IL-1 in VILI. With regard to the observed strong increase in KC production and neutrophil influx in ventilated mice, it appears plausible that IL-1 production in VILI is mediated by neutrophil factors like proteinase-3 and neutrophil elastase.

Acute intoxicatie met alcohol, hypnotica, centraal werkende analgetica, opioïden of psychotrope middelen. Gebruik van MAO-remmers, ernstige leverfunctiestoornissen, epilepsie die niet voldoende onder controle is door middel van behandeling. Speciale waarschuwingen: ZALDIAR en ZALDIAR BRUIS worden niet aanbevolen bij patiënten met ernstige nierinsufficiëntie (creatinineklaring < 10 ml/min) of bij ernstige ademhalingsinsufficiënte. Niet gelijktijdig gebruiken met andere paracetamol of tramadol bevattende geneesmiddelen zonder een arts te raadplegen. Epilepsiepatiënten die met behandeling onder controle zijn of patiënten die ontvankelijk zijn voor aanvallen, mogen alleen met ZALDIAR of ZALDIAR BRUIS worden behandeld als dat absoluut noodzakelijk is. Gelijktijdig gebruik van opioïd-agonistenantagonisten (nalbufine, buprenorfine, pentazocine) wordt niet aangeraden. Patiënten die een onderhoudsbehandeling met opioïden ontvangen, gebruiken minstens 60 mg orale morfine per dag, minstens 25 microgram transdermale fentanyl per uur, minstens 30 mg oxycodon per dag, minstens 8 mg orale hydromorfon per dag of een equianalgetische dosis van een ander opioïd gedurende één week of langer. Dosering en wijze van toediening: De behandeling dient te worden ingesteld door en onder toezicht te blijven van een arts met ervaring in de behandeling van kankerpatiënten met opioïden. De neusspray in verpakking voor eenmalig gebruik bevat slechts één dosis en mag niet van te voren worden getest. Voor verdere informatie, zie de volledige IB-tekst. Contra-indicaties: Overgevoeligheid voor het werkzame bestanddeel of voor één van de hulpstoffen. Gebruik bij opioïdnaïeve patiënten. Ernstige respiratoire depressie of ernstige obstructieve longaandoeningen. Eerdere faciale radiotherapie. Terugkerende epistaxisaanvallen. Bijzondere waarschuwingen en voorzorgen bij gebruik: Zoals bij alle krachtige opioïden kan er klinisch significante respiratoire depressie optreden bij gebruik van fentanyl en moeten patiënten geobserveerd worden op deze effecten. Patiënten met pijn die een chronische opioïdbehandeling ontvangen, ontwikkelen een tolerantie voor respiratoire depressie en dus is het risico op respiratoire depressie bij deze patiënten lager. Gelijktijdig gebruik van geneesmiddelen die het centrale zenuwstelsel onderdrukken kan het risico op respiratoire depressie verhogen. Bij patiënten met chronische obstructieve longaandoeningen kan fentanyl ernstigere bijwerkingen hebben. Bij deze patiënten kunnen opioïden de ademhalingsprikkel onderdrukken en de weerstand van de luchtwegen verhogen. Fentanyl dient met zorg te worden toegediend aan patiënten met matige tot ernstige lever- of nierfunctiestoornissen. De invloed van lever- en nierfunctiestoornissen op de farmacokinetiek van Instanyl is niet geëvalueerd; wanneer fentanyl echter intraveneus wordt toegediend, is de klaring gewijzigd als gevolg van lever- en nierfunctiestoornissen, veroorzaakt door veranderingen in metabole klaring en plasma-eiwitten. Fentanyl dient met zorg te worden gebruikt bij patiënten met tekenen van een verhoogde intracraniale druk, een verminderd bewustzijn of coma. Instanyl dient met zorg te worden gebruikt bij patiënten met een hersentumor of hoofdletsel. Fentanyl kan bradycardie veroorzaken. Fentanyl dient daarom met zorg te worden toegediend aan patiënten met bradyaritmie. Opioïden kunnen hypotonie veroorzaken, vooral bij patiënten met hypovolemie. Instanyl dient daarom met zorg te worden gebruikt bij patiënten met hypotonie en/of hypovolemie. Als de patiënt herhaalde episodes van epistaxis of nasaal ongemak ervaart bij het gebruik van Instanyl, dient een andere toedieningsvorm voor de behandeling van doorbraakpijn overwogen te worden. De totale blootstelling aan fentanyl bij personen met een verkoudheid, zonder eerdere behandeling met een nasale vasoconstrictor, is vergelijkbaar met die bij gezonde personen. Tolerantie en lichamelijke en/of psychische afhankelijkheid kan optreden na herhaald gebruik van opioïden zoals fentanyl. Iatrogene verslaving na therapeutisch gebruik van opioïden komt echter zelden voor bij de behandeling van kankergerelateerde pijn. Ontwenningsverschijnselen kunnen optreden door de toediening van stoffen met opioïd-antagonistische activiteit, bijv. naloxon, of door analgetica met een gecombineerde agonistische en antagonistische werking (bijv. pentazocine, butorfanol, buprenorfine, nalbufine). Wanneer de behandeling met Instanyl wordt gestart, dienen andere toedieningsvormen overwogen te worden voor gelijktijdige behandeling van samenlopende ziektes die via de neus kunnen worden behandeld. Bijwerkingen: Met het gebruik van Instanyl kunnen typische opioïd-gerelateerde bijwerkingen worden verwacht. Vaak zullen na voortgezet gebruik van het geneesmiddel de meeste van deze bijwerkingen verdwijnen of in intensiteit afnemen. De meest ernstige bijwerkingen zijn respiratoire depressie (mogelijk leidend tot apneu of ademstilstand), circulatoire depressie, hypotensie en shock. Alle patiënten dienen nauwlettend te worden geobserveerd op deze bijwerkingen. De volgende categorieën worden gebruikt om de bijwerkingen volgens frequentie van voorkomen te rangschikken: zeer vaak (≥1/10); vaak (>1/100 en <1/10); soms (>1/1.000 en <1/100); zelden (>1/10.000 en <1/1.000); en zeer zelden (<1/10.000), niet bekend (kan met debeschikbare gegevens niet worden bepaald). Binnen elke frequentiegroep worden bijwerkingen gerangschikt naar afnemende ernst. Psychische stoornissen: Soms: afhankelijkheid, insomnia. Niet bekend: hallucinaties. Samenstelling: Elke QUTENZA-huidpleister bevat 179 mg capsaïcine (640 μg/cm2, 8% g/g). Elke tube reinigingsgel (50 g) bevat 0,2 mg/g butylhydroxyanisol (E320). Therapeutische indicatie: QUTENZA is geïndiceerd voor de behandeling van perifere neuropathische pijn bij niet-diabetische volwassenen, als monotherapie of in combinatie met andere analgetica. Dosering en wijze van toepassing: QUTENZA dient door een beroepsbeoefenaar te worden toegediend. Het behandelgebied, dient vóór het aanbrengen van QUTENZA met een lokaal anaestheticum te worden voorbehandeld. Er mogen maximaal vier pleisters worden gebruikt. QUTENZA dient op intacte, niet-geïrriteerde, droge huid te worden aangebracht gedurende 30 minuten op de voeten en op andere plaatsen 60 minuten. Behandeling met QUTENZA kan elke 3 maanden worden herhaald, afhankelijk van de persistentie of terugkeer van de pijn. Contra-indicaties: Overgevoeligheid voor het werkzame bestanddeel of voor één van de hulpstoffen. Niet gebruiken: Indien de patiënt allergisch (overgevoelig) is voor capsaïcine, chilipepers of één van de andere bestanddelen. QUTENZA mag op geen enkel deel van het hoofd of het gezicht van patiënten worden gebruikt. QUTENZA niet op beschadigde huid gebruiken. QUTENZA of andere materialen die met behandelde gebieden in contact zijn geweest niet aanraken, omdat dit een branderig en prikkend gevoel kan veroorzaken. Ogen, mond of andere gevoelige gebieden niet aanraken. Snuiven of inademen dicht bij de QUTENZA-huidpleister kan hoesten en niezen veroorzaken. Bijwerkingen: De volgende bijwerkingen kunnen optreden op de aanbrengplaats: Zeer vaak (≥ 1/10): pijn en erytheem; Vaak (≥ 1/100, < 1/10) pruritus, papels, vesikels, oedeem, zwelling en droogheid. De bijwerkingen waren van voorbijgaande aard en waren meestal licht tot matig van ernst. Verder kwamen Soms (≥ 1/1000 en < 1/100) voor dysgeusie, hypo-esthesie, branderig gevoel, herpes zoster, oogirritatie, eerstegraads atrioventriculair (AV-)blok, tachycardie, palpitaties, hypertensie, hoesten, keelirritatie, misselijkheid, pijn in extremiteit en spierspasmen. De reinigingsgel voor QUTENZA kan lokale huidreacties (bijv. contacteczeem) of irritatie van de ogen en slijmvliezen veroorzaken. Voorzorgsmaatregelen voor gebruik: Altijd nitrilhandschoenen dragen bij het hanteren van QUTENZA en het reinigen van behandelde gebieden. Wettelijke categorie: Uitsluitend receptgeneesmiddel. Verkorte productinformatie Lyrica (oktober 2010) Samenstelling: Lyrica 25, 75, 150 en 300 mg harde capsules bevatten respectievelijk 25, 75, 150 en 300 mg pregabaline. Indicaties: Behandeling van perifere en centrale neuropathische pijn bij volwassenen, adjuvant-therapie bij volwassenen met partiële epilepsie met of zonder secundaire gegeneraliseerde aanvallen en behandeling van gegeneraliseerde angststoornis (GAS) bij volwassenen. Farmacotherapeutische groep: Anti-epileptica, ATC code: N03AX16. Contra-indicaties: Overgevoeligheid voor het werkzame bestanddeel of voor één van de hulpstoffen. Waarschuwingen en voorzorgen: Diabetische patiënten die in gewicht toenemen tijdens de pregabalinebehandeling, kunnen een aanpassing van hun bloedsuikerverlagende geneesmiddelen nodig hebben. Er zijn post-marketing meldingen geweest van overgevoeligheidsreacties, inclusief gevallen van angio-oedeem. Pregabaline dient onmiddellijk te worden gestaakt indien zich symptomen van angio-oedeem voordoen, zoals zwellingen in het gezicht, rondom de mond of in de bovenste luchtwegen. Bij de behandeling met pregabaline zijn duizeligheid en slaperigheid opgetreden, wat het optreden van toevallige verwondingen (door vallen) bij oudere patiënten kan doen toenemen. Er zijn ook post-marketing meldingen geweest van verlies van bewustzijn, verwardheid en geestelijke achteruitgang. Daarom dient patiënten aangeraden te worden om voorzichtig te zijn tot ze vertrouwd zijn met de mogelijke effecten van het geneesmiddel. Bijwerkingen van het gezichtsvermogen: in gecontroleerde onderzoeken werd bij een groter gedeelte van de patiënten die met pregabaline werden behandeld in vergelijking met de patiënten die placebo kregen wazig zicht gemeld, dat in een meerderheid van de gevallen bij het voortzetten van de dosering vanzelf verdween. In klinische studies waarin oogheelkundige testen werden uitgevoerd, was de incidentie van verminderde gezichtsscherpte en veranderingen van het gezichtsveld groter in de patiëntengroep die met pregabaline werd behandeld dan in de placebogroep. De incidentie van fundoscopische veranderingen was groter in de placebogroep. Tijdens de postmarketingervaring zijn ook bijwerkingen van het gezichtsvermogen gemeld, inclusief verlies van het gezichtsvermogen, wazig zicht of andere veranderingen van de gezichtsscherpte, waarvan de meeste tijdelijk waren. Het staken van de behandeling met pregabaline kan resulteren in het verdwijnen of verbeteren van deze visuele symptomen. Gevallen van nierfalen zijn gemeld en in sommige gevallen vertoonde het stoppen met pregabaline reversibiliteit van deze bijwerking. Er zijn onvoldoende gegevens bekend met betrekking tot het stopzetten van antiepileptische co-medicatie na het bereiken van een controle van de aanvallen met pregabaline in de combinatietherapie, met als doel monotherapie met pregabaline te bereiken. Na stopzetting van korte- en langetermijnbehandelingen met pregabaline zijn onttrekkingsverschijnselen waargenomen bij sommige patiënten. De volgende voorvallen zijn gemeld: slapeloosheid, hoofdpijn, misselijkheid, angst, diarree, griepsyndroom, zenuwachtigheid, depressie, pijn, hyperhidrose en duizeligheid. De patiënt dient hiervan op de hoogte gebracht te worden bij het begin van de behandeling. Convulsies, inclusief status epilepticus en tonisch-clonische aanvallen, kunnen voorkomen tijdens het gebruik van pregabaline of kort na het stopzetten van de behandeling met pregabaline. Wat betreft de stopzetting van een langdurige behandeling met pregabaline zijn geen gegevens beschikbaar over de incidentie en ernst van de abstinentieverschijnselen in verhouding tot de gebruiksduur en de dosis van pregabaline. Er zijn post-marketing meldingen geweest van congestief hartfalen bij een aantal patiënten die pregabaline kregen. Pregabaline dient met voorzichtigheid te worden gebruikt bij oudere cardiovasculair gecompromitteerde patiënten. Tijdens de behandeling van centrale neuropathische pijn veroorzaakt door letsel van het ruggenmerg was de incidentie van bijwerkingen in het algemeen, CZS-gerelateerde bijwerkingen en in het bijzonder slaperigheid verhoogd. Dit kan toe te schrijven zijn aan het bijkomend effect van gelijktijdig toegediende geneesmiddelen (bijv. geneesmiddelen tegen spasticiteit) die nodig zijn voor deze aandoening. Dit dient overwogen te worden bij het voorschrijven van pregabaline bij deze aandoening. Het optreden van suïcidale ideevorming en - gedrag is gemeld bij patiënten die behandeld werden met anti-epileptica bij verschillende indicaties. Een meta-analyse van gerandomiseerde placebogecontroleerde onderzoeken met anti-epileptica laat ook een kleine toename van het risico zien op suïcidale ideevorming en gedrag. Het mechanisme achter dit risico is niet bekend en de beschikbare gegevens sluiten de mogelijkheid van een toegenomen risico voor pregabaline niet uit. Patiënten moeten daarom nauwkeurig gecontroleerd worden op tekenen van suïcidale ideevorming en gedrag en een geschikte behandeling dient te worden overwogen. Patiënten (en hun verzorgers) moeten erop gewezen worden dat indien er zich tekenen van suïcidale ideevorming of - gedrag voordoen er medisch advies ingewonnen moet worden. Er zijn post-marketing meldingen van voorvallen die verband houden met een verminderde werking van het onderste gedeelte van het maagdarmkanaal (bijv. darmobstructie, paralytische ileus, constipatie) wanneer pregabaline gelijktijdig werd toegediend met geneesmiddelen die constipatie kunnen veroorzaken zoals opioïde analgetica. Wanneer pregabaline en opioïden samen gebruikt worden, kunnen maatregelen overwogen worden om constipatie te voorkomen (vooral bij vrouwelijke patiënten en ouderen). Gevallen van misbruik zijn gemeld. Voorzichtigheid dient te worden betracht bij patiënten met een geschiedenis van drugsmisbruik. Deze patiënten dienen gecontroleerd te worden op symptomen van pregabalinemisbruik. Gevallen van encefalopathie zijn gemeld, voornamelijk bij patiënten met een onderliggende aandoening die het optreden van encefalopathie kan bespoedigen. Lyrica bevat lactosemonohydraat. Patiënten met zeldzame, erfelijke problemen van galactose-intolerantie, Lapp-lactase deficiëntie of glucosegalactose malabsorptie mogen dit geneesmiddel niet innemen. Interacties: Aangezien pregabaline voornamelijk onveranderd wordt uitgescheiden in de urine, nagenoeg niet wordt gemetaboliseerd bij de mens, in vitro het geneesmiddelenmetabolisme niet remt en niet aan plasma-eiwitten wordt gebonden, is het onwaarschijnlijk dat het farmacokinetische interacties teweeg zou brengen of er onderhevig aan zou zijn. Pregabaline kan de effecten van ethanol en lorazepam versterken. Tijdens post-marketing ervaring zijn er meldingen geweest van ademhalingsstilstand en coma bij patiënten die pregabaline en andere CSZ antidepressieve geneesmiddelen gebruikten. Pregabaline lijkt een additieve werking te hebben op de verstoring van de cognitieve en algemene motorische functie, veroorzaakt door oxycodone. Er zijn geen specifieke farmacodynamische interactiestudies uitgevoerd bij oudere vrijwilligers. Interactiestudies zijn alleen bij volwassenen uitgevoerd. Bijwerkingen: In het klinisch onderzoeksprogramma waren de bijwerkingen meestal mild tot matig in intensiteit. De opgesomde bijwerkingen kunnen ook verband houden met het onderliggende ziektebeeld en/of gelijktijdig toegediende geneesmiddelen. De meest voorkomende bijwerkingen die resulteerden in stopzetting van de pregabalinebehandeling waren duizeligheid en slaperigheid. Zeer vaak (≥ 1/10): duizeligheid en slaperigheid. Vaak (≥ 1/100, < 1/10): toegenomen eetlust, euforie, verwarring, geïrriteerdheid, afgenomen libido, desoriëntatie, slapeloosheid, ataxie, abnormale coördinatie, tremor, dysartrie, geheugenstoornis, concentratiestoornis, paresthesie, sedatie, evenwichtsstoornis, lethargie, wazig zien, diplopie, vertigo, braken, droge mond, constipatie, flatulentie, erectiele disfunctie, abnormale gang, een dronken gevoel hebben, vermoeidheid, perifeer oedeem, oedeem en gewichtstoename.
