Is er een probleem ? • Ongeveer 1 op 5 van de volwassenen in Europa lijdt aan chronische pijn1 • 1 op 3 patiënten met kanker heeft neuropathische pijn3 • Prevalentie Neuropathische pijn (Europa): 7-8%4,5 • Jaarlijks 131.000 nieuwe gevallen van neuropathische pijn in Nederland6

Is er een probleem in de toekomst? Pijn bij ouderen • prevalentie van PIJN (meeste dagen pijn en > 3maanden) -jong volwassenen: 7-20% -leeftijd 50-65: 20-80% -oud oud (65-75) en oudste oud (>85): 25-60% • in rusthuizen, verpleeghuizen: tot 80% heeft pijn • prevalentie halveert in de patiëntengroep met verminderde cognitieve vermogens

• Bij 26% is er een invloed op het dagelijkse leven • 20% van de pijnpatienten heeft geen pijnstillers 58% krijgt onvoldoende analgetica

Grading Neuropathic Pain 1. Pain with a distinct neuroanatomically plausible distribution. 2. A history suggestive of a relevant lesion or disease affecting the peripheral or central somatosensory system. 3. Demonstration of the distinct neuroanatomically plausible distribution by at least one confirmatory test. 4. Demonstration of a relevant lesion or disease by at least one confirmatory test.
Grading system: Criterion 1 1. Pain with a distinct neuroanatomically plausible distribution. A region corresponding to a peripheral innervation territory or to the topographical representation of a body part in the central nervous system.

Grading system: Criterion 2 2. A history of a relevant lesion or disease affecting the peripheral or central somatosensory system. The lesion or disease is reported to be associated with pain, including a temporal relationship typical for the condition.

Grading system: Criterion 3 3. Demonstration of the distinct neuroanatomically plausible distribution by at least one confirmatory test. As part of the neurological examination, these tests confirm the presence of neurological signs concordant with the distribution of pain.
Confirmatory tests: • clinical neurological examination • quantitative sensory testing • electrophysiology - electroneurography (sensory) - evoked patentials (SEP, LEP)
Grading system: Criterion 4 4. Demonstration of the relevant lesion or disease by at least one confirmatory test. As part of the neurological examination, these tests confirm the diagnosis of the suspected lesion or disease. These confirmatory tests depend on which lesion or disease is causing neuropathic pain. Confirmatory tests: • IHS criteria of trigeminal neuralgia • CSF analysis • MRI • electrophysiology (motor/sensory electroneurography) • intraoperative neuroanatomical evidence • (biopsy)

Anti-epileptica (1) Carbamazepine • NNT (bij trigeminus neuralgie!): 1,7 (1,3-2,2) • Let op: potentieel ernstige bijwerkingen, geneesmiddeleninteractie (oudere patiënt) 1e lijnsbehandeling bij trigeminus neuralgie
Lamotrigine • NNT: 4,9 (3,5-8,1) • Wisselende studie-uitkomsten • Huidreacties (Stevens/Johson syndroom) 3e lijnsbehandeling
