HIV plan 2014-19 ACTIE 9: Serodiscordante koppels & condoomgebruik
28
Hoge Gezondheidsraad 2013 (HGR) (advies nr. 8902)
• Gebruik condoom blijft basis – preventie HIV en talrijke andere SOA‘s
• Toch is de HGR van oordeel:
29
• Beide partners een systematische screening – Hiv, HCV, syfilis, andere SOA's • rekening houdend incubatietijden voor de verschillende verwekkers;
31
• beide partners – sinds de SOA screening • geen andere seksuele partners hebben gehad;
• de seropositieve partner – ononderbroken zijn behandeling volgt;
• keuzevrijheid van de seronegatieve partner – altijd voorrang – eindbeslissing ligt altijd bij hem/haar;
32
• gebruik condooms – tijdens de menstruatieperiodes – Bij herpes – irritaties van het urogenitaal stelsel, enz.;
• informatie – verschil risico verschillende praktijken • anale of vaginale seksuele betrekkingen • duidelijk aangeven en definiëren waarover men spreekt.
33
Messages to patients • Hiv-therapy lowers risk – but no elimination
• Condoms – remain recommended
• Stopping hiv-therapy – Viral load peak possible
• Risk reduction transmission anal sex – Not well known
• High and consistant adherence hiv-therapy needed • VL undetectable – Can take months 34
Casus 1 discordant hetero-koppel • Vrouw 38 jaar – Hiv-positief: diagnose okt 2013 – VL > 200.000 – CD4 > 500
• Nieuwe partner sinds 6 maanden, 58 – Hij: hiv-negatief • Okt & dec 2013
• Uitgebreid gecounseld – Medisch/ sociaalverpleegkundige/ therapiecounelor – Geen erg spraakzame mensen 35
36
38
Casus 3 via Sensoa • MSM 50 jaar – Hiv-positief > 20 jaar – Ondetecteerbaar
• Grote verwarring...
Casus 4 ITG • Keniaanse vrouw, 30 jaar – – – –

Farmacologisch standpunt PrEP = zeer vroege PEP
Timing post exposure profylaxis • As soon as possible after accident • > 72 hour after exposure – No sense anymore – Contact with regional lymfnodes
PEP: does it work? • PEP-trials – Started end 1980’s • Prematurely ended – Nobody in placebo group
• Animal studies • Health care workers – Case-control study: monotherapy (Retrovir) • 80% reduction
PEP: does it work?
• Derived evidence – Mother to child transmission, PMTCT • First trial 1990’s – Monotherapy (Retrovir) – 80% reduction transmission risk – “Pharmaco-prevention works”
– PrEP = very early PEP

HIV drugs available
PEP drugs used • Tri-therapy= Standard – 2 Reverse transcriptase inhibitors – 1 Protease inhibitor
• Bi-therapy – 2 Reverse transcriptase inhibitors
Often used schedules • Reverse transcriptase inhibitors • ‘2 drugs in one’ – Truvada – Combivir
• Protease inhibitors – Kaletra – Reyataz – Darunavir
Integrase inhibitors Tivicay Isentress
Institute of Tropical Medicine/ UZA
• If index HIV-positive – & resistance profile known • Adaptation schedule
Institute of Tropical Medicine/ UZA • Practically – Often high rish exposures • Weekends • Night
– Starters-kit at emergency ward UZA • 4 days dose
– Consult ITM as soon as possible • Counselling • Follow-up dose
Proposal for prisons • Organise – 1. Starters-kit – 2. Ask advice • Local AIDS reference center – Next working day
Do not use • Viramune – Hepatotoxicity – Hypersensitivity
• Stocrin – In pregnant women
• Ziagen – hypersensitivity
Beware • Protease inhibitors – Anticonception pill not reliable
Communication double message • HIV – Often not very infectious
• But if infected – Early HIV-infection • Transmission rates extremely high
• Avoid sex • Use condoms • Do not get pregnant
Lab-tests • If patient in high risk group – Is patient already HIV-positive? • HIV-antibody test: quick test
• If HIV quick test – Positive: no PEP
Lab-tests • Baseline – – – – – – –
Hemato Transaminases HIV-antibody test HIV-antigen test Syfilis (RPR, TPPA) or (VDRL, TPHA) Hepatitis A, B, C-serology Women: pregnancy test
• Hepatitis-B-vaccination – Rapid schedule if not vaccinated
Lab-tests • 6 weeks, 3 months – HIV-antibody test – Syfilis (RPR, TPPA) or (VDRL, TPHA) – Hepatitis A, B, C-serology
• If accident in professional setting – Also testing at 6 months
PrEP
What is PrEP ? New prevention method for HIV(-) people to reduce risk of acquiring HIV
blocks HIV replication preventing HIV infection from establishing
Clinical Trials: Protective effect varies
PrEP • Ethically – Low income countries: no antivirals for all HIVpositives – High income countries: antivirals for HIV-negatives
• Economically – Cost-effectiviness – Who will pay? Health Insurance? – Generic drugs in low income countries?
• ….
Conclusions • ‘Pharmaco-prevention’ – It works, if you take your pills
• PEP – Window of opportunity: short – If PEP-start • Tri-therapy by preference
– Expert advice
• TASP – +/- 100% effective, biologically • Less effective – if sexual contact outside relationship – If index stops – If STI
