Deel A: ADMINISTRATIEVE BEPALINGEN. 1. 2. Voorwerp van de opdracht. Dit document beschrijft een opdracht in verband met het uitwerken van een Masterplan Informatica binnen de context van een Management Informatica Systeem, “MIS” en het ontwikkelen en implementeren van een MIS, inclusief het leveren van diensten omtrent organisatie en personeels gericht veranderingsmanagement, support en opleiding, alsook het correctief en evolutief onderhoud van de oplossing. Het MIS moet leiden tot het automatiseren van de bestaande gegevensstromen. Deze doelstelling dient onderbouwd te worden door een verdere afstemming van de informaticadiensten op de operationele behoeften van het ITG, ook bekend onder het concept “ICT to Business Alignment”. Het MIS dient de bestaande informaticasystemen te integreren, de communicatieprocessen elektronisch te laten verlopen zodat documenten centraal kunnen beheerd en geraadpleegd worden. Een documentbeheerssysteem is een elementair deel van de te implementeren oplossing om de informatiestromen elektronisch te ondersteunen. Een krachtige zoekmotor vormt daarin een meerwaarde. Dit vereist dat voorafgaandelijk de bedrijfsprocessen binnen en tussen de departementen, de automatisatie van de gegevensstromen en de impact op de bestaande taakomschrijvingen en procedures, in kaart dienen gebracht. Tevens moet de impact op de organisatie, op het personeel en op de stakeholders duidelijk gemaakt worden! De opdracht is het resultaat van de beslissing vermeld in de beleidsnota van Raad van Bestuur van 22 maart 2011. De doelstelling van de opdracht is het bereiken van een verder doorgedreven Bestuurlijke Efficiëntie, een verdere Optimalisatie van de intra- en inter-departementele Communicatie binnen het ITG, alsook een Optimalisatie van Communicatie met de Externe Partners (stakeholders) van het ITG
3. Percelen en posten De opdracht bestaat uit één perceel met 5 posten. De vijf posten zijn : 1. Analyse en Uitwerken van een Masterplan Informatisering, inclusief een doorgedreven “proof of concept” dat aantoont de doelstellingen van de opdracht te kunnen realiseren. 2. Ontwikkeling en de stapsgewijze implementatie van de oplossing, inclusief interfaces met andere toepassingen en training 3. “Change management”, hiermee wordt bedoeld het begeleiden van het ITG op organisatorisch vlak en het begeleiden van de organisatie, de gebruikers en de stakeholders ten einde een optimale aanvaarding van de oplossing en van de aangepaste procedures te bekomen 4. Ondersteuning van de gebruikers, software-update (onderhoud en actualisering) en correctie 5. Levering, installatie en onderhoud van de vereiste hardware

De kandidaturen hebben betrekking op alle posten; later door geselecteerde kandidaten in te dienen offertes dienen al deze posten te omvatten. De geselecteerde kandidaten moeten evenwel de verschillende posten afzonderlijk in hun offerte vermelden. De leverancier (indien er niet met onderaannemers wordt gewerkt) of de hoofdaannemer (indien met onderaannemers wordt gewerkt) zal als projectmanager optreden en zal rapporteren aan de opdrachtgever. De opdrachtgever kan waar nodig bijsturen met het oog op zaken zoals flexibiliteit en aanvaardbaarheid door het ITG-personeel en door de externe stakeholders, en met het oog op budgettaire, functionele en technische vereisten. Ook al wordt de opdracht in principe toegewezen in haar geheel, toch behoudt het ITG zich het recht voor om te allen tijde af te zien van het toevertrouwen van een of alle posten aan de leverancier en te beslissen om vermelde post of posten op te nemen in een nieuwe opdracht zonder dat de leverancier recht heeft op enige schadeloosstelling. De planning van de realisatie/van de implementatie van de oplossing, wordt door het ITG over een duurtijd van enkele jaren voorzien. De vermoedelijke startdatum van de dienstverlening voor post één zal het tweede kwartaal van 2012 zijn. Het voorzien van schaalbaarheid van de oplossing en van flexibiliteit in de graduele ontwikkeling en implementatie van de oplossing is een basisvereiste van het ITG.
4. Selectie van de kandidaten ITG zal de meest geschikte kandidaten selecteren volgens de hieronder vermelde selectiecriteria en behoudt zich het recht voor om niet alle posten toe te wijzen. In het kader van deze procedure zal het ITG maximum 5 kandidaten selecteren. Alleen de geselecteerde kandidaten ontvangen vervolgens het bijzonder bestek en zullen worden uitgenodigd om een offerte in te dienen.
a.
Uitsluitingscriteria
De kandidatuur wordt gesteld voor alle posten op straffe van nietigheid van de kandidatuur! De kandidaat geeft een schriftelijke verklaring dat hij niet verkeert in een van de onderstaande gevallen van uitsluiting. ITG behoudt zich het recht voor om de juistheid te controleren van deze verklaring op woord van eer vanwege de inschrijver wiens offerte het best wordt gerangschikt. Daartoe kan ITG aan de betrokken inschrijver vragen om de inlichtingen en bewijsstukken te verstrekken die toelaten zijn persoonlijke situatie te controleren met de snelste middelen en binnen de termijn die het ITG vaststelt voor de uiteindelijke gunning. Eerste uitsluitingscriterium § 1. De Belgische inschrijver die personeel tewerkstelt dat onderworpen is aan de wet van 27 juni 1969 tot herziening van de besluitwet van 28 december 1944 betreffende de maatschappelijke zekerheid der arbeiders, moet in orde zijn met betrekking tot zijn verbintenissen tegenover de Rijksdienst voor Sociale Zekerheid. Hij wordt geacht in orde te zijn met betrekking tot de voornoemde verbintenissen indien blijkt dat hij uiterlijk op de dag vóór de uiterste datum voor ontvangst van de offertes: 1. aan de Rijksdienst voor Sociale Zekerheid alle vereiste aangiften heeft bezorgd tot en met inbegrip van die welke betrekking hebben op het voorlaatste kalendertrimester dat verstreken is ten opzichte van de uiterste datum voor ontvangst van de offertes, en 2. voor deze aangiften geen schuld aan bijdragen heeft van meer dan 2.500 euro, tenzij hij voor deze schuld betalingstermijnen heeft verkregen die hij strikt nakomt.

Zelfs indien de schuld aan bijdragen hoger is dan 2.500 euro wordt de inschrijver echter als zijnde in regel beschouwd indien hij aantoont, vóór de beslissing om de opdracht toe te wijzen wordt genomen, dat hij uiterlijk op de dag vóór de uiterste datum voor ontvangst van de offertes ten overstaan van een Opdrachtgever in de betekenis van artikel 4, § 1 en § 2, 1° tot 8° en 10° van de wet of van een openbare onderneming in de betekenis van artikel 26 van dezelfde wet, één of meer vaststaande en opeisbare schuldvorderingen heeft die vrij zijn van eender welke verbintenis tegenover derden, voor een bedrag dat ten minste gelijk is, op 2.500 euro na, aan het bedrag waarvoor hij te laat is met het betalen van bijdragen. § 2. De buitenlandse inschrijver moet uiterlijk op de dag vóór de uiterste datum voor ontvangst van de offertes: 1. in orde zijn met zijn verbintenissen betreffende de betaling van de socialezekerheidsbijdragen in overeenstemming met de wetsbepalingen van het land waar hij gevestigd is; 2. in orde zijn met de bepalingen van §1 indien hij personeel tewerkstelt dat onderworpen is aan de wet van 27 juni 1969 tot herziening van de besluitwet van 28 december 1944 betreffende de maatschappelijke zekerheid der arbeiders. 3. In gelijk welke fase van de procedure mag ITG met alle middelen die deze dienst nuttig acht informatie inwinnen over de situatie betreffende de betaling van de socialezekerheidsbijdragen van eender welke inschrijver. Tweede uitsluitingscriterium Wordt uitgesloten van deelname aan de toewijzingsprocedure, de dienstverlener die het voorwerp is geweest van een in kracht van gewijsde gegaan vonnis, waarvan de Opdrachtgever kennis heeft, wegens: 1. deelname aan een criminele organisatie, zoals bedoeld in artikel 324bis van het Strafwetboek; 2. omkoping, zoals bedoeld in artikel 246 van het Strafwetboek; 3. fraude in de betekenis van artikel 1 van de overeenkomst betreffende de bescherming van de financiële belangen van de Europese Gemeenschappen, goedgekeurd door de wet van 17 februari 2002; 4. witwassen van geld zoals bedoeld in artikel 3 van de wet van 11 januari 1993 tot voorkoming van het gebruik van het financiële stelsel voor het witwassen van geld en van de financiering van het terrorisme. Met het oog op de toepassing van deze paragraaf kan ITG, in geval van twijfel over de persoonlijke situatie van een dienstverlener, zich wenden tot de bevoegde Belgische of buitenlandse overheden om de informatie te verkrijgen die hij in verband hiermee noodzakelijk acht. Derde uitsluitingscriterium De inschrijver mag zich niet in een van de volgende gevallen bevinden: 1. in staat van faillissement of vereffening, zijn activiteiten hebben stopgezet of een gerechtelijk akkoord hebben verkregen of zich bevinden in eender welke analoge situatie als gevolg van een procedure van dezelfde aard die bestaat in de nationale wetgevingen en reglementeringen; 2. een faillietverklaring hebben ingediend, een procedure van vereffening of gerechtelijk akkoord hebben ingesteld of zich bevinden in een hangende procedure van dezelfde aard die bestaat in de nationale wetgevingen en reglementeringen. Vierde uitsluitingscriterium De inschrijver mag niet veroordeeld zijn geweest bij een in kracht van gewijsde gegaan vonnis voor een misdrijf dat afbreuk doet aan zijn beroepsintegriteit.

Vijfde uitsluitingscriterium De inschrijver mag op professioneel vlak geen ernstige fout hebben begaan die behoorlijk wordt vastgesteld door gelijk welk middel dat de aanbestedende overheden kunnen rechtvaardigen. Bovendien verbindt de inschrijver zich ertoe, door zijn offerte te ondertekenen, de normen na te komen die worden vastgelegd in de basisverdragen van de Internationale Arbeidsorganisatie (IAO) en in het bijzonder: 1. het verbod op dwangarbeid (verdrag nr. 29 betreffende de gedwongen of verplichte arbeid, 1930 en verdrag nr. 105 over de afschaffing van de gedwongen arbeid, 1957); 2. het recht op vakbondsvrijheid (verdrag nr. 87 betreffende de vrijheid tot het oprichten van vakverenigingen en tot bescherming van het vakverenigingsrecht, 1948); 3. het recht op organisatie en collectieve onderhandelingen (verdrag nr. 98 over het recht om zich te organiseren en collectief te onderhandelen, 1949); 4. het verbod op elke vorm van discriminatie op het vlak van arbeid en vergoeding (verdrag nr. 100 over het gelijke loon, 1951 en verdrag nr. 111 betreffende discriminatie (tewerkstelling en beroep, 1958); 5. de minimumleeftijd vastgesteld voor kinderarbeid (verdrag nr. 138 over de minimumleeftijd, 1973) evenals het verbod op de ergste vormen van kinderarbeid (verdrag nr. 182 over de ergste vormen van kinderarbeid, 1999). Het niet nakomen van de hierboven genoemde verdragen wordt dus beschouwd als een ernstige fout bij de beroepsuitoefening in de betekenis van artikel 69, 4° van het koninklijk besluit van 8 januari 1996. De bovenstaande bepalingen zijn van toepassing onverminderd de overige bepalingen die voorkomen in artikel 69 van voornoemd besluit. Zesde uitsluitingscriterium De inschrijver moet in orde zijn met betrekking tot zijn verbintenissen tegenover de directe belastingen van de B.T.W. Zevende uitsluitingscriterium De inschrijver mag zich niet op ernstige wijze schuldig hebben gemaakt aan valse aangiften bij het verstrekken van inlichtingen die in het kader van deze opdracht opeisbaar zijn.
b.
Selectiecriteria inzake financiële draagkracht van de inschrijver
Elke kandidaat moet bij zijn kandidatuur een ondertekende verklaring op erewoord voegen met betrekking tot: de totale omzet en het bedrijfsresultaat die werden gerealiseerd tijdens de laatste drie boekjaren (gelieve neergelegde balansen en resultatenrekeningen in bijlage toe te voegen); de omzet en het financiële resultaat (eventuele besparingen, meerkosten of bijkomende aanpassingen/”change requests”) betreffende de relevante en als referentie opgegeven projecten met betrekking tot Management Informatica Systemen. Om relevant te zijn dienen deze referenties aan de doelstellingen van het beoogde ITG-MIS, -zie hierboven-, te voldoen en dienen ze opgeleverd en geaccepteerd te zijn na 1 januari 2008; de jaarlijkse omzet per bedrijfssector en het aantal klanten in de respectieve sectoren na 1 januari 2008. De totale omzet gerealiseerd sinds 2008 voor de ontwikkeling en implementatie van de vermelde referentieprojecten gerealiseerd binnen de BeNeLux, bij drie verschillende klanten, –zie hieronder-, mag niet minder bedragen dan 1.000.000 euro. In geval kandidaten na toepassing van de hierna vernoemde selectiecriteria als nummer 5 en 6 ex aequo zijn geëindigd wordt de kandidaat met de grootste omzet geselecteerd.

c.
Selectiecriteria m.b.t. de technische competentie en draagkracht van de inschrijver
In zijn kandidatuur moet de kandidaat – op duidelijke wijze – het bewijs leveren van het feit dat hij over de technische draagkracht beschikt om de opdracht tot een goed einde te brengen. De kandidaat moet beschikken over aantoonbare expertise met betrekking tot een aanbod van diensten zoals vermeld in alle posten, inclusief het change management (post 3). De kandidaat levert het bewijs van competentie en draagkracht door het afleveren van: 1) Een lijst van minimum 3 soortgelijke opdrachten gerealiseerd binnen de BeNeLux, gedurende de voorbije 3 jaren bij 3 verschillende klanten met opgave van: de naam van de klant (publiek- of privaatrechtelijke instantie) evenals de contactperso(o)n(en) die officieel de klant mogen vertegenwoordigen (zie verder); de beschrijving van de dienst met vermelding van het gedeelte dat de kandidaat zelf heeft uitgevoerd en het gedeelte dat werd uitbesteed; de gebruikte methodologie voor het projectmanagement, het servicemanagement en het changemanagement; een beschrijving van de manier waarop de procedures van qualitymanagement werden toegepast; duurtijd van de implementatie; het budget van inbedrijfstelling en de recurrente kosten; de geïmplementeerde architectuur (software/hardware). De totale omzet gerealiseerd voor de ontwikkeling en implementatie van de drie vermelde referentieprojecten uitgevoerd in de BeNeLux, mag niet minder bedragen dan 1.000.000 euro. Elke referentie moet, op straffe van niet-ontvankelijkheid, de referentie en de telefoon- en e-mailgegevens bevatten van een bevoegd persoon bij de klant met wie contact kan worden opgenomen. Indien blijkt dat de opgegeven contactpersoon bij de klant niet bevoegd is om de klant te vertegenwoordigen of indien de klant niet bereid is om als referentieproject te fungeren dan wordt de referentie als onontvankelijk aanzien en vervalt de kandidatuur! (* Als contactpersonen die officieel de klant mogen vertegenwoordigen worden volgende profielen aanzien: een persoon met officiële tekenbevoegdheid (cfr. Staatsblad) een directielid een lid van het “Steering Committee” die langs de kant van de klant een leidende rol speelt/heeft gespeeld in de realisatie van een referentieproject) 2) Een beschrijving van de methodologie en aanpak die de kandidaat in het vooruitzicht stelt voor het uitvoeren van het project en van de diensten. Meer bepaald de aanpak van de analyse van de behoeften en de inhoud van een Masterplan Informatisering, inclusief de projectmanagement-, de servicemanagement- en de changemanagement-aanpak. Onder changemanagement wordt verstaan de begeleiding van het ITG als organisatie en de begeleiding van de organisatie, van de gebruikers en van de stakeholders om de veranderingen die een MIS-project met zich meebrengt, optimaal te aanvaarden en te benutten. 3) Een cv van minstens 6 medewerkers die de Nederlandse taal volledig machtig zijn en die aan deze opdracht kunnen meewerken, met een overzicht van hun diploma's, bijkomende vormingen, ervaring

en rol in soortgelijke projecten, alsook hun informaticakennis. (onder medewerker wordt verstaan zowel vast personeel als vast personeel van een subcontractor, alsook freelancepersoneel dat op geregelde basis door de kandidaat-leverancier wordt ingehuurd). 4) Een beschrijving van de werking van de ondersteunende diensten. Voor de aangeboden hardware moet een 24 x 24 contract geboden worden, 7 op 7 met een interventietermijn van 4 uur onsite. Voor softwaregerelateerde problemen is er een tweedelijns help desk met kennis van het Nederlands vereist met een tijdsvenster van 8.00 tot 18.00 uur gedurende weekdagen. 5) De kandidaat zal een verklaring doen aangaande het aantal werknemers van zijn onderneming tijdens de voorbije 3 jaren, alsook de relevante functionele competentiedomeinen waarin het dienstverlenend personeel actief is. Er wordt verwacht van de kandidaat dat het gemiddeld aantal werknemers over de laatste vier jaren minstens 50 bedroeg (exclusief personeel op freelance- of interimbasis te werk gesteld).
d.
Toepassing van de selectiecriteria
Voor de kandidaten die niet verkeren in een van de gevallen van uitsluiting (zie hierboven) en die tevens bewijs hebben geleverd dat zij voldoen aan de vereiste financiële draagkracht zoals vermeld, wordt de beoordeling op basis van de selectiecriteria als volgt toegepast: Criterium De gemiddelde klantentevredenheid van de referentieprojecten, aangegeven door de contactpersonen* (zie p 7) Beschrijving en relevantie van diensten per referentieproject Methodologie op het vlak van projectmanagement, servicemanagement (m.b.t. de werking van ondersteunende diensten), en qualitymanagement Change-methodologie: naar de organisatie, naar de gebruikers en naar de stakeholders Kwaliteit van aanpak van analyse en inhoud van een Masterplan Informatisering (zie post 1) Competentie van de medewerkers op basis van cv en op basis van de relevante aangegeven competentiedomeinen

5. Informatiesessie Tot uiterlijk donderdag 18 augustus 2011 om 16:00 uur kunnen alle kandidaten hun vragen schriftelijk richten aan de vertegenwoordiger van het ITG, zoals vermeld hieronder. Alle kandidaat-inschrijvers worden bij deze, met een maximum van 3 personen per kandidaat, uitgenodigd op een informatiesessie die doorgaat op woensdag 24 augustus 2011 in de lokalen van het ITG, in het Auditorium, van 10.00 tot 12.00 uur. Uiterlijk om 09.30 uur dienen de kandidaten, die aanwezig wensen te zijn, zich op de receptie van het ITG, Nationalestraat 155 (hoofdingang) aan te melden. Tijdens deze informatiesessie zal men trachten de gestelde vragen zo duidelijk mogelijk te beantwoorden. Nadien kunnen bijkomende opmerkingen en vragen gesteld worden, tot uiterlijk woensdag 31 augustus 2011. De antwoorden op deze vragen zullen worden verstuurd naar alle kandidaten op uiterlijk maandag 5 september 2011.

a.
Indienen en openen van de kandidatuurstellingen.
De kandidatuurstelling moet in het Nederlands opgemaakt worden en moet opgesteld worden in overeenstemming met de in deel C. van dit bijzondere document vermelde eisen en model. De kandidatuurstelling dient in één (1) ondertekend exemplaar op papier ingediend te worden, en in zes (6) digitale exemplaren op elektronische drager, volgens de modaliteiten bepaald in art. 103 tot en met 105 van het KB van 08.01.1996 betreffende de overheidsopdrachten. De kandidatuurstellingen worden vóór de opening van de kandidatuurstellingen ofwel per schrijven (een aangetekend schrijven wordt aanbevolen) opgezonden naar, ofwel persoonlijk neergelegd bij het ITG. De kandidatuurstellingen worden aanvaard voor zover de zitting van opening van de kandidatuurstellingen niet voor geopend is verklaard. De kandidatuurstelling worden in een gesloten omslag gestoken. Op deze omslag dienen de volgende vermeldingen te worden aangebracht: het nummer van het oproep met name:. ITG-MIS-001; de datum en het uur van de opening van de kandidatuurstellingen.

Christoph Laureys - aankoper Nationalestraat 155 B-2000 Antwerpen Op dinsdag 20 september 2011 om 10 uur zal in het Instituut voor Tropische Geneeskunde, in Zaal Broden, in openbare zitting worden overgegaan tot de opening van de kandidatuurstellingen ingediend voor deze opdracht. Maximaal twee vertegenwoordigers van elke geselecteerde kandidaat die beslist heeft een kandidatuur in te dienen, mogen de zitting bijwonen. In het raam van het onderzoek van de kandidatuurstellingen door het ITG, worden de inschrijvers erop gewezen dat zij het bezoek aan hun installaties door afgevaardigden van het ITG moeten toelaten.

6. Wetgeving. Onderhavige opdracht is onderworpen aan de algemene reglementering betreffende de overheidsopdrachten voor aanneming van werken, leveringen en diensten. Elke inschrijver wordt verondersteld de bepalingen te kennen en te aanvaarden die in volgende wettelijke en reglementaire teksten opgenomen zijn: - De wet van 24 december 1993 betreffende de overheidsopdrachten en sommige opdrachten voor aanneming van werken, leveringen en diensten (BS van 22 januari 1994); - Het koninklijk besluit van 8 januari 1996 betreffende de overheidsopdrachten van werken, leveringen en diensten en de concessies van openbare werken (BS van 26 januari 1996) ; - Het koninklijk besluit van 26 september 1996 tot bepaling van de algemene uitvoeringsregels van de overheidsopdrachten en van de concessies voor openbare werken + bijlage : algemene aannemingsvoorwaarden voor de overheidsopdrachten voor aanneming van werken, leveringen en diensten en voor de concessies voor openbare werken (BS van 18 oktober 1996); - Alle wijzigingen aan de wet en de voormelde besluiten die van toepassing zijn op de dag van de opening van de offertes;

7. Mogelijke extra informatie met betrekking tot de inschrijver Het ITG behoudt zich het recht voor alle nuttige inlichtingen, van morele of financiële aard, over de inschrijvers in te winnen.
8. Laattijdige uitvoering. Van de bepalingen van art. 75 van de bijlage bij het KB van 26.09.1996 inzake boeten wordt hier afgeweken gezien het kritieke karakter van de automatisering in zijn totaliteit. Het overschrijden van de geplande realisatiedatum van de fases geldt als een aanmaning voor de dienstverlener. De boeten zullen worden bepaald in het lastenboek en zullen verder worden vastgelegd in het bestek.

Deel B. TECHNISCHE SPECIFICATIES. 1. Voorstelling van het ITG Het Instituut voor Tropische Geneeskunde (ITG) is een stichting van openbaar nut waarvan de oprichtingsakte dateert van 1931. Het ITG is sinds 1906 actief als opleidings-, onderzoeks- en referentiecentrum in de tropische geneeskunde en de gezondheidszorg in de ontwikkelingslanden. Tot 1960 ressorteerde het ITG onder het Ministerie van Koloniën, nadien onder het Ministerie van Nationale Opvoeding. Bij de defederalisering van het onderwijs in 1988 werd de financiering overgeheveld naar de Vlaamse Gemeenschap, departement onderwijs, waar het een onderdeel werd van het Vlaamse hoger onderwijs. In het decreet op de universiteiten wordt het ITG omschreven en betoelaagd als een 'Instelling voor Hoger Academisch Onderwijs'. In 1993 werden de statuten en de samenstelling van de Raad van Bestuur grondig aangepast. De Vlaamse regering erkent het ITG met het Decreet van 18 mei 1999 als 'Postinitiële Instelling van Openbaar Nut‟ en geeft het ITG een jaarlijkse toelage voor het inrichten van postinitieel en postacademisch onderwijs, het opleiden van doctorandi en het uitvoeren van wetenschappelijk onderzoek en dienstverlening. De wederzijdse rechten en plichten werden vastgelegd in een hernieuwbare vijfjarige beheersovereenkomst met de minister van Onderwijs. Het ITG verbindt zich in deze beheersovereenkomst onder andere tot het voorleggen en uitvoeren van doelgerichte beleidsplannen, het aanbieden van opleidingsplaatsen, het invoeren van interne en externe kwaliteitsbewaking en samenwerking met universiteiten en buitenlandse instellingen. Het ITG heeft bovendien een belangrijk programmaovereenkomst met het Ministerie van Buitenlandse zaken, directie ontwikkelingssamenwerking voor de financiering van de samenwerking met partners in het zuiden en het organiseren van specifieke opleiding voor internationale experten in de gezondheidszorg of dierengeneeskunde.
2. De opdracht van het ITG De opdracht van het ITG omvat drie luiken: Onderwijs: de vorming van medici en paramedici in o.m. tropische geneeskunde en diergeneeskunde, het verlenen van internationale masters in volksgezondheid, ziektebestrijding en tropische diergeneeskunde of veeteelt, het begeleiden van universitaire doctorandi, etc. Onderzoek: het verrichten van fundamenteel en toegepast onderzoek op de belangrijkste tropische en andere infectieziekten bij mens en dier, de organisatie van humanitaire en veterinaire gezondheidsdiensten in ontwikkelingslanden, etc. Hiertoe wordt nauw samengewerkt met binnen- en buitenlandse onderzoeksinstellingen. Dienstverlening: het verrichten van curatieve en preventieve raadplegingen en adviezen, het uitvoeren van laboratoriumanalyses. Verder voert het ITG voor het Ministerie voor Ontwikkelingssamenwerking een programma uit van vorming, operationeel onderzoek en capaciteitsversterking. Het ITG is erkend als enig nationaal referentiecentrum voor tropische geneeskunde. Deze opdracht wordt gerealiseerd in drie (3) wetenschappelijke departementen. Zeer recent onderging het ITG een belangrijke wijziging en ging van 5 naar 3 wetenschappelijke departementen. Bovendien zijn er zoals in elke organisatie een aantal “Ondersteunende Diensten”: Aankoop, Personeelsdienst, Boekhouding, Technisch Beheer, Grafische Dienst, Studentendienst, Communicatiedienst en Informatica.

5. Scope van het project Het doel is om een partner te selecteren voor het ITG die een strategie kan ontwikkelen en een Masterplan Informatisering zal afleveren voor de implementatie van een softwareplatform. Dit moet toelaten om de bestaande informatica-systemen te integreren en de communicatieprocessen elektronisch te laten verlopen. Documenten moeten centraal kunnen beheerd worden en eenvoudig te raadplegen zijn. De partner moet in staat zijn het project in al zijn fases te begeleiden en te realiseren, vanaf de functionele analyse tot en met de implementatie en de dienstverlening en ondersteuning nadien. Dit houdt in dat, beginnend bij de departementen en de ondersteunende diensten van het ITG, de verschillende bedrijfsprocessen moeten in kaart gebracht worden. Concreet moet worden omschreven hoe de geautomatiseerde informatiestromen zullen verlopen en welke de impact is op de bestaande taakomschrijvingen en procedures. Tevens moet de impact op de organisatie duidelijk gemaakt worden. De doelstelling van het project Management Informatica Systeem, “MIS”, moet leiden tot een efficiëntere organisatie en kostenbesparingen gebaseerd op de integratie van de bestaande informaticatoepassingen en tot het automatiseren van de bestaande informatiestromen. Deze doelstelling zal onderbouwd worden door een verdere afstemming van de informaticadiensten op de operationele behoeften van het ITG, ook bekend onder het concept “ICT to Business Alignment”. Vanuit deze visie moet assistentie verleend worden om een beheersstructuur op te zetten binnen het ITG om te verzekeren dat de doelstellingen gerealiseerd worden daar dit project een diepgaande invloed gaat hebben op de bestaande procedures en organisatie. Een documentbeheerssysteem is een elementair deel van de te implementeren oplossing om de informatiestromen elektronisch te ondersteunen. Een krachtige zoekmotor is een essentieel onderdeel hiervan. Een datawarehouse-insteek zal eveneens vereist zijn voor het omzetten van gegevens naar informatie door de definitie van metadata. De totale oplossing zal vermoedelijk uit een combinatie van databases en databasemanagementtools bestaan die gebruikt worden om het beheer van de besluitvorming te ondersteunen.
6. Overzicht van functionele behoeften (niet exhaustief – indicatief)) a. Belangrijke algemene behoeften Zonder de ambitie om volledig te zijn volgt hier een lijst van de meest belangrijke functionele behoeften: Onderwijs: o
Consolidatie en harmonisatie, binnen de verschillende onderwijssecties, van alle “administratieve” en “academische gegevens” van de studenten.

Genereren van diploma‟s en diplomasupplementen.

Harmonisatie en standaardisatie “leren op afstand”
o
Ondersteuning van de academische planning: opleidingsroosters, beschikbaarheid van docenten en infrastructuur (zalen,aula,..).
o
Een geïntegreerde financiële opvolging van de kosten van het onderwijs zodat de fullcost van de studie kan bepaald worden.

Ondersteuning voor de kwaliteitszorg binnen het departement onderwijs. Het evaluatieproces van de diverse cursussen dient gestandaardiseerd en geharmoniseerd worden
o
Mogelijkheid om een “community of expertise “ op te bouwen binnen het departement enerzijds en met de studenten anderzijds.
o
Harmonisatie van de accreditatie van de verschillende onderwijssecties.
Onderzoek: o
Opvolging van de administratieve cyclus voor het behalen van PhD-graad.
o
Opzet van een “lerende organisatie”. Hiermee wordt bedoeld dat men nood heeft aan instrumenten waarmee men contact kan houden en gegevens uitwisselen tussen medewerkers van het ITG enerzijds en externe collega‟s en andere stakeholders anderzijds in verband met projecten en onderzoeken.

Voor de polikliniek moet een elektronisch patiëntendossier geïnstalleerd worden waarvan de implementatie zal plaatsvinden in de loop van 2012. Een selectie hiervoor werd reeds uitgevoerd.

Ondersteuning voor het opstellen, documenteren en verspreiden van alle administratieve procedures.

De integratie van de budgetopvolging met het financiële pakket

Ondersteuning voor het managen van de inventaris van toestellen en het onderhoud, de keuring en kalibratie ervan.

Documenten kunnen bijvoorbeeld per project worden gegroepeerd, ongeacht de fysieke locatie waar de documenten zich bevinden.

Reductie van de tijd om werkmappen met documenten ten bate van een specifieke taak samen te stellen.

Bindingsmiddel tussen processen: op het moment dat verschillende op zich staande items worden opgeroepen kan een document als bindingsmiddel fungeren. Ketenintegratie van processen en systemen: door de informatie aan het begin van het proces te digitaliseren kunnen andere processen en systemen deze informatie gebruiken en is ketenintegratie veel gemakkelijker,. Voorkomen dubbele invoer: voorkomt dat dezelfde informatie op meerdere plaatsen in de organisatie wordt vastgelegd. Veiligheid: een digitaal archief kan eenvoudig elders opgeslagen worden voor backup. Beveiliging: authenticatie en autorisatie op gebruik van vertrouwelijke documenten. Opgeslagen documenten kunnen op verschillende niveaus beveiligd worden. Mappen, maar ook complete archieven kunnen beveiligd worden Kostenbesparing:voorkomen dat een document vele malen gedupliceerd wordt. Geen kopieën naar elke verschillende belanghebbende. Één document digitaal opgeslagen kan door iedereen geraadpleegd worden. Eenvoudige dossiervorming: een antwoord op een (ingescande) brief kan vanuit het DMS met Word of Excel gemaakt worden en is direct gekoppeld aan het betreffende document (linken van documenten). Meer flexibiliteit: complete sets van gegevens op een laptop of CD meenemen kan eenvoudig. Alle informatie binnen handbereik. Workflowmanagement: door workflowmanagement kunnen taken geautomatiseerd worden, zoals het doorsturen van documenten naar de juiste medewerker(s) ten bate van verdere verwerking of controle

Introduction Aim Currently, the controlled documents are managed by the electronic system Webiso. This URS describes the requirements for the new document management system, a software system that must be able to electronically review, approve, consult, fill in and print documents. The system must provide a complete document life cycle and version management. These requirements are the basis for: The supplier who delivers the system The qualification of the system: IQ, OQ and PV.
Scope The new system must have improved features for uploading and editing documents, forms, attachments etc. In addition, the possibility to incorporate figures and schemes into documents must be straightforward. In order to track the reading of documents (training), the system must be able to deal with “reading confirmations”. A search engine and possibilities for defining custom queries and reports should also be present. Further, the system must also be able to handle periodic reviews of documents.

Description of the requirement GENERAL REQUIREMENTS System Operating System Compatibility – the system must have a provision for running on common Microsoft Operating Systems such as Windows 2003, 2007 and 2010 Server, XP, XP Pro, Vista, Windows 7. If possible, it should also run on Mac Operating Systems. Web Server Based Access – all the systems functions must be easily accessed via a web browser distributed interface. The system must allow to structuralize documents based on metadata. It must be possible to give all folders a name. After the folder is given a name, it must also be possible to change its name. The system shall allow for user-friendly dragging and dropping of documents from one folder to another folder. It must be possible to hide folders for different user groups (eg. readers, administrators). Links between documents should be possible. If there are links between one or more documents in the same or in different folders, all these links should be found by the system when asked for linked documents. Links must not be created automatically. If the file structure is adapted or if documents are moved from one folder to another, the present links in the documents should be updated automatically. The system must have an easy to access inbox for users to view all active tasks. It must be possible to adapt the lay-out of the screen and information on it to the individual needs of the users (eg. display of graphical overviews, list of actions, etc). The graphical lay-out of the system should preferably fit as closely as possible to the ITM home-style. When creating or importing a document, it must be possible to choose the type of document (vision/mission, quality manual, general procedure, specific procedure, method, regulation, working instruction, form for completion, attachment) and corresponding document template. It must be possible to include the following document information: title of the document, number of the document, group of users/readers who can consult the document, department/unit/service, author, approvers, time span for execution of periodic reviews… If not all mandatory information is entered when creating the document, the user must get a warning for each of these items and must not be able to continue. Both portrait and landscape set-ups must be possible in one document. The system should generate document numbers automatically. Manual adaptation of this number and manual numbering by specified users/groups of users should also be possible. The system has to be compatible with common windows based applications for the purpose of controlling attached file storage and viewing. Applications should include but not be limited to: (MS Word, MS Excel, PowerPoint, MS Access, Word Perfect, PDF, Visio, drawing, video, audio, and other common file applications/extensions). It must be possible to change the file type of a document (eg. if a version of a document is created in word, it must be possible to create a later version of the same document in excel). The system must allow for the insertion of figures, schemes, etc into documents by normal office features (copy/paste, insert file, etc) After creation of the document, it must be possible for the author to insert content into the document. After the initial creation or import of the document, the status of the document should be „DRAFT‟ and the version number 0.0 has to be shown in the system. The system should work using the check in and check out of documents in order to prevent that more than one user can change a document at the same time. If a document is checked out of the system by the author, it must not be accessible for other users. It must be possible to create forms for completion that can be filled in electronically or manually on a print out of the form. If a form for completion is filled in electronically, it must be possible to temporarily save the form and its content when not completely filled in in order to complete the form for example on the next working day. It must also be possible for different users to work on the same form. The document administrator and authors must be able to import one or a group of documents into the library/database together with the corresponding document information. The method for importing documents should be easy and user-friendly. An imported document should follow the same life cycle as a document that is drawn up in the application itself. The system shall allow for the capability to convert common file extension type to Portable Document Format (PDF) It must be possible for the document administrators to track the documents for which a working cycle has started but that are not processed between specific time periods. B. Modifying documents It must be possible to create a new draft from a document with status EFFECTIVE/APPROVED/REJECTED. When a new draft of a document exists, this should be indicated in the current effective version, both in the reader and administrator sections of the system. When checking in, the version of the document must increase by 0.1. When checking in a document, the author must list the changes made. After check in, it must be possible to send the document for review/coauthoring/approval. C. Review/co-authoring of documents The author must have the possibility to send out a document for review/co-authoring in parallel to a group of reviewers/co-authors or to one individual reviewer/co-author. This should only be possible in the status DRAFT. When the document is sent out for review/co-authoring, the status of the document must stay DRAFT. Each reviewer/co-author has to be notified of their review/co-authoring task. The reviewer/co-author has 10 working days to perform the review/co-authoring. If no action is performed within this time period, the action must become overdue and a notification has to be sent to the person. The reviewer/co-author must have the possibility to consult the document, to change the 32

The reviewer/co-author must have the possibility to compare previous drafts with the current version. All changes have to be traceable. Both (co)-authors and reviewers must be able to consult which person has made a change, what has been changed and when. As long as a reviewer/co-author has checked out a document for review/co-authoring, it must not be possible for another person to change the document. When checking in a document by the co-author/reviewer, they must list their changes made to the text. When checking in, the version of the document must increase by 0.1. After review/co-authoring, the reviewer/co-author must have the possibility to confirm that the review/co-authoring task was performed. When the last review/co-authoring task is completed, the author must be notified. When the review is performed, the status of the document should be changed to REVIEWED. During the review/co-author phase, the author must have the possibility to change the document. Unless the document is checked out by a reviewer/co-author. When the author checks out the document for changing, the status of the document must be changed to DRAFT again. The author must have the possibility to incorporate into the text all changes made by the reviewers/co-authors. After all changes are incorporated into the text, the author must be able to check in the document. After checking in the document, it must be possible to send out the document again for review or for approval. D. Approval of documents The author must have the possibility to send out the document for approval in parallel to a group of approvers or to an individual approver. This should be possible both from the status DRAFT and REVIEWED. As soon as the document is sent for approval, the status of the document must change to IN APPROVAL. Each approver must be notified of the approval task. The approver has 20 working days to perform the approval. If no action is performed within this time period, the action must become overdue and a notification has to be sent to the approver. The approver must have the possibility to consult the sent document (read-only). The approver must have the possibility to compare previous drafts with the current version. The approver must have the possibility to approve or reject the document. When rejecting a document, the system must oblige the approver to fill in a reason for rejection. It must be possible for the approver and document administrator to delegate an approval task to another authorized person. The system must ask for a reason for delegation. The approver must have the possibility to send out an additional approval task to another authorized person. When the status of the document is IN APPROVAL, it must be possible to change the document information/metadata. When the last approval task is completed, the author must be notified. When the last approval task is completed, the status of the document must be changed to APPROVED. When a document is rejected, the author and other approvers must be notified with the reason for rejection. The task must then disappear from the Inbox of the approvers that have not yet approved. When a document is rejected, the author must be able to draw up a new draft version of the document which can be sent out for review or approval. E. Publishing of documents When the status of the document is APPROVED, the author must have the possibility to change the status to EFFECTIVE. When performing this change of status, it must be possible to link this to the document group(s) or individual persons who have access to read the document. document administrator. As from the effective date, the document must be available for the readers as far as access is permitted. When a new version (a.0) of a document becomes EFFECTIVE, version (a-1.0) must get the status ARCHIVED. Only the effective version of the document should be available for the readers. Archived versions shall be moved to an archived status for purposes of viewing and storage access. The archived version of the document needs to be available for (co)authors, reviewers and approvers. F. Documents not in use For documents with status EFFECTIVE, document administrators must be able to send out a request for approval in order to change the status of the document to “NOT IN USE” for documents that may be removed or have been replaced. It must be possible for the document administrator to give a reason for removal of a document. Each approver must be notified of the approval task to change the status of the document from EFFECTIVE to NOT IN USE. The reason for removal should be included in the notification. The approver must have the possibility to consult the sent document (read-only). The approver must have the possibility to approve or reject the request. When rejecting the request, the system must ask the approver to fill in a reason for rejection. It must be possible for the approver and document administrator to delegate the approval task to another authorized person. The system must ask for a reason for delegation. It must not be possible to change the document and document information/metadata during this process flow. When the approval task is completed, the document administrator must be notified. When approval is done, the status must be changed to NOT IN USE. When the request is rejected, the document administrator must be notified with the reason for rejection. When the status is changed to NOT IN USE, the document must no longer be available for the readers. The documents with status NOT IN USE must still be available for document administrators. It must be possible for the document administrators to track the documents that have been removed. G. Periodic reviews of documents The system must be able to deal with periodic revisions of EFFECTIVE documents. The responsible persons should be notified by the system of their revision tasks. In the notification a link to the periodic review task in the system has to be present and the statement „check for possible updates of the document template‟ should be present. It should be possible to select when the notification should be send (eg. 1 month before the document needs to be revised). It must be possible to document in the system that the periodic review has been performed. If the document needs to be modified, a new version of the document should be initiated and published. The version of the document must increase by 0.1 and the author should be obliged to list the changes made. It must be possible to set a different review period for different document types. The system must be able to process a report with the status of the periodic reviews. In this report should be listed: which tasks have to be done before a specific date or within a period of time, which tasks have been performed, which tasks are ongoing, which tasks are to be started, persons responsible, due date. H. Reporting During review/co-authoring, the (co)-author and reviewers must have the possibility to consult to whom the document is sent for review/co-authoring and in which stadium of the flow it is situated. The same must be possible for approval. In each stage of the document life cycle, it must be possible for the document administrator to consult and print out for each document a document history. Herein must be listed all actions that are performed on the document, by whom and when. At each stage of the document life cycle, the document administrator must have the possibility to see the different versions of the documents from the department/unit. The system must allow for generating an overview list of the current effective versions of all documents with document number, title, version number, effective date and/or other metadata like status, department, etc. Authorized users must be able to create custom reports and queries based on the systems data. The system should have the capability to support the reporting of audit trail activities within the system. It must be possible to process a report that lists all overdue tasks for a specific unit/department and one that lists all overdue tasks for all units/departments. The system must allow the export of reports and the results of queries to excel. I. Management of workflows & security and delegation Document administrators must have the possibility to change the author of a document. Document administrators must be able to re-assign tasks of one document administrator to another document administrator. Document administrators must have the possibility to delegate a task to another person. For delegation a reason must be filled in. It should be possible for the document administrators to delegate in 1 step all tasks from one person to another person. For delegation a reason must be filled in. Document administrators must be able to determine and change the group of persons or individual persons that have access to a document. This must be changeable during the entire life cycle of a document. Document administrators must be able to determine and change the date when a document becomes EFFECTIVE. If the different languages in which a form for completion is created are in one common document, it must be possible to select only one language for the print-out. K. Notification of assigned tasks A notification has to be sent for all co-author, review, approval and periodic review tasks. This notification must be sent directly by the system to all relevant persons for each new task. In the notification a link to the task in the system has to be present. The text of the notification should preferably be in the language of the user (Dutch, English, French). It must be clearly mentioned if a delegation of tasks has taken place and the reason for this. It should be obvious from the title of the notification which task has to be performed. The notification must include the task (for example review, approval,…) and the details of the document (document number, title, version). Description of the requirement ISO 17025:2005 4.3 Document control 4.3.2 Document approval and issue 4.3.2.1 All documents issued to personnel in the laboratory as part of the management system shall be reviewed and approved for use by authorized personnel prior to issue. NEN-EN-ISO 9001:2008 4.2 Documentation requirements 4.2.1 General The quality management system documentation shall include a) documented statements of a quality policy and quality objectives; b) a quality manual; c) documented procedures and records required by this International Standard, and d) documents, including records, determined by the organization to be necessary to ensure the effective planning, operation and control of its processes. NOTE 1 Where the term “documented procedure” appears within this International Standard, this means that the procedure is established, documented, implemented and maintained. A single document may address the requirements for one or more procedures. A requirement for a documented procedure may be covered by more than one document. NOTE 2 The extent of the quality management system documentation can differ from one organization to another due to a) the size of organization and type of activities, b) the complexity of processes and their interactions, and c) the competence of personnel. use, e) to ensure that documents remain legible and readily identifiable, f) to ensure that documents of external origin determined by the organization to be necessary for the planning and operation of the quality management system are identified and their distribution controlled, and g) to prevent the unintended use of obsolete documents, and to apply suitable identification to them if they are retained for any purpose. 4.2.4 Control of records Records established to provide evidence of conformity to requirements and of the effective operation of the quality management system shall be controlled. The organization shall establish a documented procedure to define the controls needed for the identification, storage, protection, retrieval, retention and disposition of records. Records shall remain legible, readily identifiable and retrievable. FDA 21CFR Part 11 compliance Subpart B--Electronic Records Sec. 11.10 Controls for closed systems. Persons who use closed systems to create, modify, maintain, or transmit electronic records shall employ procedures and controls designed to ensure the authenticity, integrity, and, when appropriate, the confidentiality of electronic records, and to ensure that the signer cannot readily repudiate the signed record as not genuine. Such procedures and controls shall include the following: (a) Validation of systems to ensure accuracy, reliability, consistent intended performance, and the ability to discern invalid or altered records. (b) The ability to generate accurate and complete copies of records in both human readable and electronic form suitable for inspection, review, and copying by the agency. Persons should contact the agency if there are any questions regarding the ability of the agency to perform such review and copying of the electronic records. (c) Protection of records to enable their accurate and ready retrieval throughout the records retention period. (d) Limiting system access to authorized individuals. (e) Use of secure, computer-generated, time-stamped audit trails to independently record the date and time of operator entries and actions that create, modify, or delete electronic records. Record changes shall not obscure previously recorded information. Such audit trail documentation shall be retained for a period at least as long as that required for the subject electronic records and shall be available for agency review and copying. (f) Use of operational system checks to enforce permitted sequencing of steps and events, as appropriate. (g) Use of authority checks to ensure that only authorized individuals can use the system, electronically sign a record, access the operation or computer system input or output device, alter a record, or perform the operation at hand. (h) Use of device (e.g., terminal) checks to determine, as appropriate, the validity of the source of data input or operational instruction. (i) Determination that persons who develop, maintain, or use electronic record/electronic signature systems have the education, training, and experience to perform their assigned tasks. (j) The establishment of, and adherence to, written policies that hold individuals accountable and responsible for actions initiated under their electronic signatures, in order to deter record and signature falsification. (k) Use of appropriate controls over systems documentation including: (1) Adequate controls over the distribution of, access to, and use of documentation for system operation and maintenance. Subpart C--Electronic Signatures Sec. 11.100 General requirements. (a) Each electronic signature shall be unique to one individual and shall not be reused by, or reassigned to, anyone else. (b) Before an organization establishes, assigns, certifies, or otherwise sanctions an individual's electronic signature, or any element of such electronic signature, the organization shall verify the identity of the individual. (c) Persons using electronic signatures shall, prior to or at the time of such use, certify to the agency that the electronic signatures in their system, used on or after August 20, 1997, are intended to be the legally binding equivalent of traditional handwritten signatures. (c) Persons using electronic signatures shall, upon agency request, provide additional certification or testimony that a specific electronic signature is the legally binding equivalent of the signer's handwritten signature. Sec. 11.200 Electronic signature components and controls.
(a) Electronic signatures that are not based upon biometrics shall: (1) Employ at least two distinct identification components such as an identification code and password.

(i) When an individual executes a series of signings during a single, continuous period of controlled system access, the first signing shall be executed using all electronic signature components; subsequent signings shall be executed using at least one electronic signature component that is only executable by, and designed to be used only by, the individual.
