Editorial
Afgelopen februari heeft de NeSECC haar 35 jarige bestaan gevierd met een lustrum. Het lustrum was een groot succes en wellicht had u nu al een jubileum editie verwacht. Hiervoor moet u echter nog even geduld hebben. Later in het jaar verschijnt er een speciale jubileum editie. In deze editie vindt u abstracts van de wetenschappelijke bijeenkomst in Nijmegen. Hier was het thema veiligheid en bij het middagprogramma kon een keuze worden gemaakt voor een excursie naar het dieren- en skillslab of kon er een calamiteitentraining worden gevolgd in combinatie met een bezoek aan het museum. De najaarsbijeenkomst vond plaats in Maastricht en het betrof een case report meeting: Extracorporeal Life Support (ELS). Een nieuwe variant op het griepvirus, H1N1, verbeterde apparatuur en niet te vergeten een uitgebreide studie van Dr. Peek doen de vraag naar dit soort extracorporale technieken toenemen. Diverse toepassingen van ELS kwamen voorbij en een ELS-patiënt deelde zijn persoonlijke ervaringen met de vele aanwezigen. Zoals altijd vond de nieuwjaarsbijeenkomst plaats in Nieuwegein. Dit jaar werd er gesproken over sport & doping en hemodilutie & normothermie. Verder vindt u in het blad de vaste rubrieken zoals u die gewend bent met onder andere nieuws vanuit Suriname van de voormalige hoofdredacteur. Tijdens het jubileum is er na tien jaar ook een eind gekomen aan het voorzitterschap van Kees Visser. De functie is overgenomen door de vicevoorzitter Jeroen van Hees. Hiermee komt de vacature van vicevoorzitter vacant. Hopelijk kan het bestuur hiervoor snel een nieuwe enthousiaste kandidaat werven. Op deze plaats wil ik Jeroen veel succes wensen met zijn nieuwe functie. Wij wensen u veel leesplezier toe, De redactie

Afgelopen februari heeft de NeSECC haar 35-jarig bestaan gevierd met een speciaal jubileum symposium. Door de inzet van vele leden waaronder een delegatie van de studiecommissie is dit een fantastisch congres geworden. Door de (mede) -organisatie van nationale wetenschappelijke bijeenkomsten zorgt de studiecommissie er voor dat NeSECC leden middels het bezoeken van deze wetenschappelijke bijeenkomsten hun kennis up to date kunnen houden. Het organiseren van dit soort bijeenkomsten is de kerntaak van de studiecommissie en vergt ook de meeste tijd van de studiecommissie. De onderwerpen van de bijeenkomsten komen ook steeds meer buiten het vakgebied van de cardiochirurgie te liggen. Hierdoor is het ook voor andere disciplines aantrekkelijk/interessant om deze bijeenkomsten te bezoeken. De bijeenkomst vorig jaar in Maastricht met als onderwerp ‘Extracorporeal Life Support’ is hier een goed voorbeeld van. Een respectabel aantal mensen die geen NeSECC lid waren hebben tegen betaling deze meeting bijgewoond. Daar waar vroeger deze bijeenkomsten uitsluitend bedoeld en bestemd waren voor klinisch perfusionisten zijn deze bijeenkomsten tegenwoordig ook voor andere disciplines interessant. Het vakgebied van de klinisch perfusionist is breder en beperkt zich niet alleen tot de hartchirurgie. Vorig jaar heb ik u geschreven dat de studiecommissie bezig is om haar contacten met de opleiding te verbeteren. Dit proces is in gang gezet en zal dit jaar verder uitgewerkt worden. De studiecommissie heeft afgelopen jaar ook een inventarisatie gedaan naar de salarisschalen van klinisch perfusionisten in opleiding. Dit laat een nogal wisselend beeld zien. Blijkbaar is er weinig bezwaar tegen de naamgeving van de ‘studiecommissie’ want tot op heden hebben we geen reactie ontvangen op de oproep om op zoek te gaan naar een nieuwe naam. Als voorzitter van de studiecommissie wens ik u dus veel leesplezier met deze NeSECC UPTODATE.

Extracorporal Membrane Oxygenation (ECMO) is a way of providing prolonged cardiopulmonary support in neonates with severe respiratory- or circulatory failure. emergencies Drs T. Antonius neonatoloog, UMC St. Radboud
Technical emergencies on ECMO can cause severe problems and have a mortality rate of 25%. Examples of these emergencies are pump- or artificial lung failure, damage to the tubing, clotting of the ECMO-system or air-embolus. Clinical emergencies include cardiac tamponade, severe bleeding, cannula dislocation and haematothorax. To adequately respond to these emergencies teams must be trained in the physiology and the principles of VA and VV-ECMO and must acquire the technical and non-technical skills (e.g. communication, leadership, teamwork) to react to these emergencies. ecmo simulation In our center, teams are trained using traditional didactic methods and hands-on training is performed using animals and water drills.
These types of training fail to deliver a realistic, interactive learning environment in which professionals are more likely to acquire the necessary skills. Therefore we initiated a new ECMO simulation training program and built a simulated ECMO environment. We modified a commonly used ECMO-system and connected a manikin so we can simulate different emergencies wirelessly. By controlling the patient and ECMO-monitors we provide the candidates with the necessary feedback and interaction. Team performance is captured on video and during a debriefing session led by an experienced instructor the technical and non-technical skills are reviewed using the recordings. The workshop will deal with the technical and non technical challenges of developing an ECMO teaching program and will demonstrate the ECMO-simulator.

Successful Double Lumen Veno-Venous Life Support With Adjuvant Plasmapheresis. A Tour De Force. Study objective We describe the use of plasmapheresis as an adjuvant treatment during venovenous extracorporeal life support (VVELS) complicated by heparin-induced thrombocytopenia (HIT). Methods and materials A novel double lumen catheter (27 Fr) which was percutaneously inserted in the right jugular vein, was used during VVELS in a 57-year-old male patient suffering from severe acute respiratory failure after abdominal aortic surgery due to a unknown immunomodulated phenomenon. Results Despite optimum conventional management, the patients condition deteriorated quickly (FiO2 100%, PaO2 5.3 kPa, SaO2 77%, pH 7.11). VV-ELS was inserted onsite with immediate improvement (PaO2 10.1 kPa, SaO2 97%), and subsequently the patient was transferred to our hospital. During VV-ELS (day 3), the patient developed a confirmed HIT, which forced a change in anticoagulation strategy from heparin to argatroban, maintaining target aPTT’s of 60-80 s during 3.5 L/min support.
The focus of the respiratory failure remained uncertain, but in combination with the developed HIT led to consider an immunologic problem. The patient underwent seven procedures of plasmapheresis, implemented on the ELS circuit, in ten days. Due to the induced shifts in clotting factors, and imbalanced coagulation, the ELS circuit was uneventfully changed seven times. After plasmapheresis, the clinical condition of the patient improved and ELS was successfully weaned after 23 days. Two weeks later, the patient was discharged from our hospital. Conclusion Double lumen VV-ELS in combination with adjuvent plasmapheresis therapy is a safe and effective method in management of severe respiratory failure complicated by HIT.

Extracorporeal Life Support for Cardiac and/or Pulmonary failure. Results of 30 ECLS-procedures introduction

In the period November 2004 till November 2010 we performed 30 Extra Corporeal Life Support (ECLS) procedures on 28 patients suffering from acute cardiac and/or pulmonary failure. The age of the patients varied between 16 and 76 years of age (mean age 42.9 ± 17.7). 17 Female patients (mean age 38.8 ± 18.7) and 11 male patients (mean age 48.5 ± 16.2). Two patients were supported twice in 1 year. They suffered from ischemic heart failure and received ECLS as bridge to bridge (Heartmate II implantation). Both patients finally underwent heart transplantation. ecls system Since 2007 the ECLS system consists of the PLS-set (Maquet) with Rotaflow centrifugal pump and Quadrox-D oxygenator, Bioline coated. Percutaneous femoral cannulas used for veno-arterial access were 17fr Biomedicus arterial cannula Carmeda coated (Medtronic) and 21fr Biomedicus venous cannula Carmeda coated (Medtronic). For perfusion of the distal part of the leg a super arrow flex percutaneous cannula 6fr (Arrow) was used. For the veno-venous access a 23fr or 27fr Bi-Caval dual lumen cannula (Avalon) was used. If necessary, to unload the left atrium and to prevent pulmonary edema a 21fr transseptal cannula (TandemHeart, Cardiac Assist) was inserted. indications The indications for ECLS were divided into 4 groups: 1. post-operative heart/ lung failure, 2. myocarditis, 3. ischemic heart failure and 4. miscellaneous group consisting of cardiac (1 hypothermia after drowning, 1 late rejection HTX, 1 cardio-
myopathy 6 months post-partum, 1 cardiomyopathy e.c.i.) and pulmonary.

results The overall survival rate was 63.3%. The mean age of the survivor group (19 patients) was 38.7 ± 15.5 years in comparison to the mean age of the non-survivor group which was 50.1 ± 19.8 years. The mean ECLS-duration for all 30 patients was 6.3 ± 5.0 days (minimum 1 day, maximum 22 days). The survivor group had a longer mean ECLS-duration in comparison to the non-survivor group, 7.6 ± 5.5 (1-22 days) vs. 4.1 ± 3.3 (1-11 days) days. Also resulting in a longer ICU-stay for the survivor group, 21.9 ± 11.1 (8-45 days) vs. 16.2 ± 33.6 (1-117 days) days and longer hospital stay, 56.2 ± 31.8 (23-124 days) vs. 16.4 ± 33.6 (2-117 days) days. In the post-operative heart/lung failure group, ECLS was mainly used as bridge to recovery for 11 out of 12 patients. For 1 patient ECLS was bridge to heart transplantation. Also in the myocarditis group 3 out of 4 patients needed ECLS as bridge to recovery. One patient first received a Heartmate II and underwent heart trans-

plantation later that year. For the ischemic heart failure group, 3 patients out of 5 received ECLS as bridge to Heartmate II. All 3 patients underwent heart transplantation later that year. All 9 patients in the miscellaneous group needed ECLS as bridge to recovery. Table 1 shows the survival to ECLS, 30 days survival and survival to hospital discharge for all 4 groups.
conclusion From our results we can conclude that ECLS is a useful treatment that allows for an additional window of therapeutic opportunity.

Quantitative Assessment Of Cardiac Load-responsiveness During Extracorporeal Life Support: Case And Rationale Abstract

We describe a case of a patient assisted by extracorporeal life support, in which data from the pulmonary artery catheter became unavailable during cardiac reloading. As a result, the assessment of cardiac load-responsiveness became limited to transesophageal echocardiography, and central venous and radial artery pressure monitoring. In addition, we obtained the dynamic filling index, a validated measure for venous volume during extracorporeal life support, and used this index to assess cardiac load-responsiveness during acute reloading. While reloading, the obtained findings on cardiac pump function by the dynamic filling index in absence of the pulmonary artery catheter were supported by transesophageal echocardiography and standard pressure measurement. This suggests that the dynamic filling index can be used to support the monitoring of cardiac load-responsiveness when pulmonary artery pressure and cardiac output monitoring are unavailable during extracorporeal life support.

Efficacy And Economic Assessment Of Conventional Ventilatory Support Versus Extracorporeal Membrane Oxygenation For Severe Adult Respiratory Failure (CESAR). A Multicentre Randomised Controlled Trial. Background
Findings
Severe acute respiratory failure in adults causes high mortality despite improvements in ventilation techniques and other treatments (eg, steroids, prone positioning, bronchoscopy, and inhaled nitric oxide). We aimed to delineate the safety, clinical efficacy, and cost-effectiveness of extracorporeal membrane oxygenation (ECMO) compared with conventional ventilation support.
766 patients were screened; 180 were enrolled and randomly allocated to consideration for treatment by ECMO (n=90 patients) or to receive conventional management (n=90). 68 (75%) patients actually received ECMO; 63% (57/90) of patients allocated to consideration for treatment by ECMO survived to 6 months without disability compared with 47% (41/87) of those allocated to conventional management (relative risk 0.69; 95% CI 0.050.97, p=0.03). Referral to consideration for treatment by ECMO treatment led to a gain of 0.03 quality-adjusted life-years (QALYs) at 6-month follow-up. A lifetime model predicted the cost per QALY of ECMO to be £19252 (95% CI 7622-59200) at a discount rate of 3.5%.
Methods In this UK-based multicentre trial, we used an independent central randomisation service to randomly assign 180 adults in a 1:1 ratio to receive continued conventional management or referral to consideration for treatment by ECMO. Eligible patients were aged 18-65 years and had severe (Murray score >3.0 or pH <7.20) but potentially reversible respiratory failure. Exclusion criteria were: high pressure (>30 cm H2O of peak inspiratory pressure) or high FiO2 (>0.8) ventilation for more than 7 days; intracranial bleeding; any other contraindication to limited heparinisation; or any contraindication to continuation of active treatment. The primary outcome was death or severe disability at 6 months after randomisation or before discharge from hospital. Primary analysis was by intention to treat. Only researchers who did the 6-month followup were masked to treatment assignment. Data about resource use and economic outcomes (quality-adjusted life-years) were collected. Studies of the key cost generating events were undertaken, and we did analyses of cost-utility at 6 months after randomisation and modelled lifetime costutility. This study is registered, number ISRCTN47279827.
Interpretation We recommend transferring of adult patients with severe but potentially reversible respiratory failure, whose Murray score exceeds 3.0 or who have a pH of less than 7.20 on optimum conventional management, to a centre with an ECMO-based management protocol to significantly improve survival without severe disability. This strategy is also likely to be cost-effective in settings with similar services to those in the UK. De eerste openhartoperatie werd uitgevoerd door de afro-amerikaanse arts Daniel Hale Williams. In de zomer van 1893 kreeg hij een 18-jarige man, met een steekwond in de borst, hevig bloedend en in shock op de operatietafel. Dokter Williams besloot om de borstkas te openen, en het letsel te overhechten. Na 51 dagen verliet deze jongeman in goede gezondheid het ziekenhuis. Er zijn andere chirurgen van wie gezegd wordt dat ze de eerste openhart operatie uitgevoerd hebben, maar Daniel Hale Williams is de enige die wordt bezongen. Door Stevie Wonder nog wel, in zijn nummer ‘Black Man’ uit 1976. Het hart was in die tijd een ontoegankelijk gebied voor de operateur: niet alleen stond de kennis van het fysiologie van het hart nog in de kinderschoenen (waardoor men angst had voor een verstoring van de hartcontracties, en dus de circulatie), maar ook het gegeven dat er zo’n 4 à 5 liter bloed per minuut door het hart stroomt, maakte een operatie op het hart een zeer bloederige, en daardoor zeer kortdurende aangelegenheid. Bigelow Een van de personen die hierin verandering heeft gebracht was Wilfred (Bill) Bigelow, een Canadese hartchirurg. Tijdens zijn artsenopleiding behandelde hij een jonge Canadees met bevriezingsverschijnselen, en tot zijn verbazing waren bevriezingsverschijnselen in de medische wetenschap nog amper onderzocht. In de tweede wereldoorlog was dokter Bigelow werkzaam in Engeland en Noordwest Europa, en heeft hij veel soldaten behandeld die te kampen hadden met onderkoeling en bevriezingen. Na zijn terugkeer naar Canada kreeg hij een ingeving: misschien was onderkoeling wel de methode om
hartoperaties mogelijk te maken. Hij was ervan overtuigd dat onderkoeling het metabolisme verlaagde, de hartslag vertraagde, en zo de chirurg de tijd zou kunnen geven een operatie uit te voeren. Dit ging wel geheel in tegen de gangbare opvatting dat onderkoeling juist het metabolisme verhoogde, en zijn onderzoek naar hypothermie werd dan ook sceptisch ontvangen. Jarenlang is onderzoek uitgevoerd naar de winterslaap van bosmarmotten, omdat Bigelow dacht dat hierin antwoorden te vinden waren die openhart chirurgie mogelijk zouden maken. Helaas was deze onderzoekslijn niet succesvol, maar het andere onderzoek van Bigelow leidde er wel toe, dat in september 1952 de eerste openhart operatie onder hypothermie uitgevoerd kon worden door de Amerikaanse artsen John Lewis en Richard Varco. Hypothermie gaf de chirurgen (net) voldoende tijd om het hart open te maken, een eenvoudig congenitaal defect te repareren, en het hart weer te sluiten. Vaak echter werd, na het openen van het hart, een complexe afwijking gevonden, die niet operabel bleek binnen de tijd dat de bloedcirculatie veilig stil kon staan. Lillehei Een van de assisterende artsen bij deze eerste hypotherme openhartoperatie was Walton C. Lillehei, en hij vond de 6 tot 12 minuten waarbinnen veilig geopereerd kon worden niet voldoende. Hij bedacht, samen met dr. Varco, de ‘controlled crosscirculation’, waarbij de oxygenatie van de pediatrische patiënt werd overgenomen door een volwassene met dezelfde bloedgroep, meestal een van de ouders. Deze cross-circulation werd bereikt door bloed uit beide cavae van het patiëntje te leiden naar de vena femoralis van de donor, en geoxygeneerd bloed uit de a. femoralis

van de donor te leiden naar de a. carotis van het patiëntje. Deze techniek werd voor het eerst toegepast in maart 1954, en in 15 maanden werden 45 patiëntjes met complexe ventriculaire defecten geopereerd. Alhoewel briljant en succesvol, was er veel kritiek op deze methode. Deze techniek is in die tijd wel beschreven als ‘de enige operatie met kans op 200% mortaliteit’, en is alleen in de kliniek van Lillehei uitgevoerd. heartpump Tegelijk met al deze ontwikkelingen waren er ook onderzoeksgroepen bezig met de ontwikkeling van machines die de hart- en longfunctie van een hartpatiënt konden overnemen. In 1952 werd in Detroit de heartpump gebruikt tijdens een openhartoperatie, een bloedpomp ontwikkeld door dr. Dodrill en General Motors (de heartpump lijkt ook wel op een 12 cilinder motor). De oxygenatie tijdens deze operatie werd verzorgd door de patiënt’s eigen longen. Al snel bleek dat er geavanceerdere machines in ontwikkeling waren dan de heartpump, en in 1954 werd de heartpump gedoneerd aan het museum The Smithsonian Institution. Gibbon De eerste machine waarmee succesvol de hart- en longfunctie werd overgenomen, was door dr. John Gibbon en IBM ontwikkeld. Om preciezer te zijn: de IBM type III, de derde versie van deze hartlongmachine, werd op 6 mei 1953 gebruikt in het Thomas Jefferson Universiteitsziekenhuis in Philadelphia om een atriumseptum defect te repareren bij een 18-jarige vrouw. Deze eerste operatie verliep niet zonder hobbels: de ontstolling van de patiënte bleek niet voldoende te zijn, en door het ontstaan van stolsels lieten de interne sensoren de hartlongmanchine stoppen. Voor de rest van de operatie is de hartlongmachine handmatig aan de gang gehouden, waarbij al het bloed en vloeistoffen die maar voorhanden waren, gebruikt zijn om de hartlongmachine functionerend
te houden. Het postoperatieve beloop van deze patiënte was ongecompliceerd. De twee daaropvolgende patiënten overleden aan deze hartlongmachine, waardoor dr. Gibbon pas weer wilde opereren met de hartlongmachine als er meer bekend was over de effecten van extracorporele circulatie op de fysiologie van de patiënt. De ironie wil dus dat de eerste succesvolle openhart operatie met de hartlongmachine ook meteen dr. Gibbons laatste is geweest. bubble oxygenator Andere artsen gingen wel door met de risicovolle openhart operaties aan bypass, en de ontwikkeling van de hartlongmachine, en in 1955 werd door de doktoren Lillehei en Richard A. DeWall de bubble oxygenator geintroduceerd. Relatief goedkoop, herbruikbaar en veel praktischer dan de tot dan toe gebruikte plaatoxygenatoren. Dit type oxygenator heeft het mogelijk gemaakt dat de hartchirurgie explosief kon groeien, en de bubble oxygenator is standaard gebleven tot laat in de 70er jaren. Toen in de jaren ‘60 bleek dat de resultaten van openhart operaties met cardiopulmonale bypass bij kinderen drastisch beter waren als de operaties onder hypothermie werden uitgevoerd, werden deze beide technieken voortaan gecombineerd. terug naar normothermie Dus waarom bestaat er, sinds een jaar of tien, de trend om meer normotherme cardiopulmonale bypass te gebruiken bij openhart operaties? Ten eerste hebben er enorme verbeteringen plaatsgevonden in het extracorporele circuit: sinds de jaren ‘70 worden de kleinere hollowfiber oxygenatoren toegepast, en sinds de jaren ‘90 zijn er diverse coatings op de markt. Vooral hierdoor zijn de nadelige effecten van het extracorporaal circuit op de patiënt sterk verminderd, en is het postoperatieve beloop significant verbeterd. Dit heeft ertoe geleid dat de noodzaak tot het sterk verminderen van het metabolis-

me tijdens laag risico openhart operaties minder urgent is. Ten tweede heeft normothermie zelf gedurende de operatie ook nog een aantal voordelen: thrombocytenfunctie wordt beter behouden (en dus de postoperatieve stolling), het risico op een hyperthermie van het brein tijdens het opwarmen (aantoonbaar nadelig) wordt verkleind, en de tijd aan de hartlongmachine (en de OK-tijd) wordt verminderd. hematocriet Maar normothermisch opereren heeft toch als nadeel dat het metabolisme van de organen tijdens de operatie niet verlaagd wordt. Komt de zuurstofvoorziening van gevoelige organen zoals de hersenen en de nieren niet in gevaar door de hemodilutie? Verschillende studies hebben gekeken naar het laagste hematocriet gedurende normotherme (tot 34 ºC) cardiopulmonale bypass en het optreden van postoperatieve complicaties. Het blijkt dat bij een flow van 2.4-2.6 l*m2*min-1 een hematocriet onder de 22-24% samengaat met verhoogde postoperatieve complicaties, zoals nierfalen of CVA. Een alternatief zou kunnen zijn een hogere bloedflow aan te houden (een cardiac index van bijvoorbeeld 2,8 of 3,0), het probleem hiervan is echter dat de mogelijkheid bestaat dat door de hemodilutie shunting optreedt van bloed door de organen. Een laag hematocriet verstoord de microcirculatie, en resulteert in het dichtvallen van capillairen. Dit zou kunnen leiden tot een lokaal zuurstoftekort, bij ruim voldoende systemische bloedflow. Wat wel in het achterhoofd gehouden moet worden, is dat deze studies retrospectief en observatief zijn, dat wil zeggen dat er geen oorzaak en gevolg vast te stellen is. Al deze studies hebben bijvoorbeeld als complicerende factor dat patiënten met een laag hematocriet in het algemeen ook meer bloedproducten toegediend krijgen, en vaak ook meer comorbiditeiten hebben dan gemiddeld. Bovendien zullen
de effecten van een lager of hoger hematocriet alleen klinisch zichtbaar worden, als de hemodilutie ook daadwerkelijk de zwakste schakel is in de keten. Is bijvoorbeeld het postoperatieve beleid niet optimaal, dan kunnen de complicaties die hierdoor optreden het effect van een optimalere perfusie teniet doen. Desalniettemin lijkt het raadzaam om de hemodilutie te beperken tot een hematocriet >22-24%, in het bijzonder bij normotherm-achtige operaties. Sport Is Te Mooi Voor Doping
Het woord doping is een verzamelterm voor alle stoffen en methoden die verboden zijn in de sport. Van origine komt het woord van inheemse stammen, die bepaalde dranken gebruikten om feller te kunnen vechten of om in trance te raken voor religieuze rites. Bij de opkomst van de georganiseerde sportbeoefening in de negentiende en twintigste eeuw was het gebruik van doping aanvankelijk toegestaan. Sinds de jaren 1960 geldt er echter een wereldwijd verbod op dopinggebruik in de sport. Sinds 2004 is de Wereld AntiDoping Code van kracht, die nauwkeurig bepaalt aan welke eisen dopingreglementen moeten voldoen. Middelen en effecten De meest gebruikte dopingmiddelen zijn de anabole steroïden1. Deze middelen zijn afgeleid van het mannelijk geslachtshormoon testosteron en stimuleren de spiergroei terwijl ze de spierafbraak remmen. Ze zorgen ervoor dat iemand sterker en sneller wordt. Helaas zorgen ze ook voor vele andere effecten in het lichaam, waaronder acne, een hogere bloeddruk, een ongunstigere verhouding van het cholesterol, leverfunctiestoornissen, ongewenste veranderingen van de geslachtsorganen en een verhoging van de agressie2. Het is op voorhand onvoorspelbaar welke bijwerkingen bij welke personen zullen ontstaan. Voorbeelden van anabole steroïden zijn nandrolon, stanozolol en methandië non. In de topsportwereld zijn hiernaast het hormoon erytropoëtine (EPO) en de methode van bloeddoping verleidelijk. Beide zorgen voor een grotere hoeveelheid rode bloedcellen, welke verantwoordelijk zijn voor het zuurstoftransport. Op deze wijze wordt het duurvermogen aanzienlijk ver-
beterd3. De risico’s hiervan zijn gerelateerd aan het extra bloed: het wordt dikker, en het risico op hart- en vaataandoeningen neemt exponentieel toe. Bij sporters die trainen in sportscholen en fitnesscentra gaat het meestal niet zozeer om het duurvermogen, maar om het fysieke uiterlijk. Ook hier zijn anabole steroïden populair, samen met allerlei middelen om de effecten daarvan extra te beïnvloeden. Het meest gebruikt worden echter afslankmiddelen zoals efedrine, amfetamines of schildklierhormonen4. Deze middelen verhogen de stofwisseling, en zorgen naast vetverbranding ook voor een hoge bloeddruk en hartritmestoornissen. Amfetamines zijn bovendien zeer verslavend. Verboden middelen rondom operaties Op de dopinglijst staan een aantal producten en methoden die perfusionisten regelmatig zullen tegenkomen. Erytropoëtine en bloeddoping (bloedtransfusies voor niet-medische doeleinden) zijn al genoemd, maar ook sterke pijnstillers zoals fentanyl, morfine en oxycodon zijn verboden, en er zijn strenge regels voor het al dan niet mogen toedienen van infusen. Deze laatste regels veranderen nogal eens per jaar (de dopinglijst wordt jaarlijks vastgesteld), maar de meest recente regel is dat intraveneuze infusies zijn verboden, tenzij ze legitiem worden gegeven bij ziekenhuisopnames of klinisch onderzoek5. Dit wordt geoperationaliseerd als maximaal 50 ml vloeistof per 6 uur. In 2011 is voor het eerst het achtereenvolgens afnemen, manipuleren en opnieuw toedienen van volbloed in de bloedsomloop verboden, wat gericht is tegen zoge-

noemde ozontherapie. Niet dat deze therapie als prestatieverhogend wordt gezien, maar het ondergaan van deze therapie geniet een opkomende populariteit onder bepaalde groepen sporters, en het Wereld Anti-Doping Agentschap wilde deze nietmedische praktijk een halt toeroepen. Een topsporter die toch één van deze stoffen moet krijgen of een verboden methode moet ondergaan, kan daarvoor dispensatie aanvragen bij de sportautoriteit waar hij of zij onder valt6. Bij noodsituaties kan de dispensatie uiteraard ook achteraf nog geregeld worden.
prestatie te verbeteren10. Dit komt neer op ongeveer 8% van sportend Nederland, welke met name te vinden is in de nietcompetitieve fitnesscultuur. De exacte gevolgen voor de volksgezondheid van dit dopinggebruik zijn vooralsnog onbekend, maar de risico’s lijken aanzienlijk te zijn1. Hoewel dopinggebruik en de gevolgen hiervan dus onze aandacht verdient, dient iedereen die zich mengt in discussies over dit onderwerp zich er terdege van bewust zijn dat de overgrote meerderheid van sporters, zowel competitieve als nietcompetitieve, hun hobby of werk uitoefenen zonder doping te gebruiken.

Bij dopingcontroles onder topsporters test ongeveer 1-3% van de sporters positief 7. Wat dit percentage zegt over de daadwerkelijke prevalentie van dopinggebruik is niet eenduidig. Dopingcontroles worden niet iedere dag uitgevoerd, dus is het mogelijk om als kwaadwillende sporter de gok te wagen en in bepaalde periodes toch iets te gebruiken. Bovendien zijn niet alle middelen even goed detecteerbaar, en het lijkt dan ook gerechtvaardigd om het aantal dopinggebruikers hoger in te schatten. Alle gegevens (van controles en enquêtes, maar ook van politie en justitie) wijzen er echter op dat dopinggebruik minder voorkomt dan menigeen denkt8. Schattingen dat meer dan de helft van de sporters op Olympische Spelen doping gebruiken lijken schromelijk overdreven, ook gezien de prestaties op specifieke krachtnummers die in de afgelopen 20 jaar zijn afgenomen9. Het idee is dat dopingcontroles werken, al blijft het gissen naar het precieze effect ervan en naar het exacte aantal dopinggebruikers in de topsport.

An In Vitro Comparison Of Bubble Elimination In Quadrox And Capiox Oxygenators E.W.M. Bakker EKP, K. Visser EKP Dept. of extracorporeal circulation, Medical Centre Leeuwarden, the Netherlands.
Abstract The generation of gaseous microemboli still remains an issue in cardiopulmonary bypass. We report the testing of the air handling capacity of the Capiox FX25, the Quadrox i (both with integrated arterial filter) and the Quadrox D plus separate Quart arterial filter. Therefore we introduce air in an extracorporeal circuit in a water filled laboratory setup. Number of emboli, bubble size and the total volume of air are measured with the GAMPT BCC200. In a first test we measure bubbles after introduction of a 50 ml bolus of air just prior to the oxygenator. In a second test we apply a subtle stream of micro emboli in a reproducible way. In the first test we find a total volume of 0.041, 0.173 and 4.215 μl measured distal to the arterial filter for the Capiox, Quadrox i and Quadrox D respectively. In Test 2 we compare bubble counts, air volume reduction, reduction in number of emboli and mean diameter index for the oxygenator/filter combinations. We find volume reductions of 99.8%, 88.5% and 93% respectively. Therefore we conclude that the Capiox may be superior to the Quadrox D and Quadrox i concerning air handling. Clinical testing and greater data collection are required to prove the significance of our findings. Introduction Gaseous microemboli (GME) have always been an issue in cardiopulmonary bypass. Their correlation with organ dysfunction after cardiac surgery has been widely studied and discussed1,2,3. Although GME are often undistinguishable from other factors as a cause of worse patient outcome, its contribution is assumed to be significant and most likely based on ischaemia caused by partial occlusion of blood ves-
sels and endothelial damage4. The extra corporeal circuit (ECC) may contribute to the generation of GME5,6 as well as surgical interventions7. Since decades, efforts have been made to minimize the amount of air entering the patient’s bloodstream, by the use of certain ECC components and by changing their design. When considering removal of GME from the ECC, the arterial filter and the oxygenator appear to be components of great interest8. The venous reservoir removes most visible air, but still allows GME to enter the arterial side of the circuit. Although an oxygenator is not designed for air removal, its characteristics play an important role in their capability to remove air9. The last obstacle for air before reaching the aorta is, in most cases, the arterial line filter. The filters used today are screen filters. Their capability to remove air depends mainly on a critical bubble point pressure, in its turn depending on the pore size of the filter. Smaller pore sizes increase their effectiveness10, eventually limited by the size of cellular components of the blood. Besides generation of microembli, massive air embolism remains a point of concern, although its incidence has fallen from once every 2500 cases in 1970 to once every 30,000 cases according to the most recent national survey in the USA11. Massive air embolization is mainly caused by operator error. When altering ECC components and perfusion practice, development has not always been focussed on their influence on GME generation and removal. Nevertheless, some new techniques may introduce new sources of emboli. Vacuum assisted venous drainage (VAVD) has shown to be a significant source of gaseous microemboli in the arterial line during extracorporeal circulation12. It also re-introduces

the possibility of massive air emboli13,14. Recently Terumo developed an oxygenator with integrated screen filter. The company states that this product handles air efficiently, even though the screen filter exists of only one layer wrapped around the oxygenation fibres. It is said to remove air via the microporous polypropylene hollow fibres of the oxygenator. We tested the air handling capacity of this product in comparison with an oxygenator and filter we use in our daily routine and an oxygenator with integrated filter. Methods Three oxygenator/filter combinations (1: Capiox FX 25 microporous polypropylene hollow fibre oxygenator with integrated 32 μm polyester screen filter (Terumo), 2: Quadrox-i microporous polypropylene hollow fibre oxygenator with integrated 40 μm arterial filter (BE-HMO 71000, Maquet) and 3: Quadrox-D polypentene diffusion membrane oxygenator (Maquetb) and Quart 40 μm arterial filter (BE-HMOD 2000, Maquetb), respectively called CAP, Qi and QD in this article, are tested in a laboratory setup for their air handling capacity. Therefore the oxygenator/filter combinations are built into a closed ECC system, connected to a pseudo patient (50 liter container, filled with water at room temperature). The extracorporeal circuit (ECC), customized and sterilized by Maquetb consists of a tubing set (Raumedic), collapsible reservoir (JVR 1900, Maquetb), centrifugal pump (Rotaflow-RF32, Maquetb) and hard shell cardiotomy reservoir (HC 2821, Maquetb). The Capiox FX 25 has an Xcoating (Terumoa), the collapsible reservoir has a Safeline coating (Maquetb) and all other components are Bioline coated (Maquetb). Figure 1 shows a schematic picture of the ECC. One by one the Cap, Qi and QD are built Terumo, Leuven, Belgium Maquet, Hirrlingen, Germany Raumedic Münchberg, Germany

in between the centrifugal pump and the Quart arterial filter. During measurements with the Cap and Qi the Quart filter is clamped at both the inlet and outlet and the shunt bypassing the Quart filter is opened. During measurements with the Quadrox D the shunt is closed and the water is led through the Quart arterial filter. So, as CAP and Qi contain an integrated filter, all measurements are performed using one oxygenator and one filter. The arterial probe of the bubble counter is positioned 8 cm distal to the filter, and the venous probe is located just between the centrifugal pump and the oxygenator, following the instructions of the manual15. The scale of the bubble counter is set to 20-500 μm. Pressure (measured at the top of the Quart arterial filter) and flow are maintained at 150 mmHg and 5 l/min respectively, running the pump at 3000 rpm’s and partially clamping the arterial side 15 cm distal to the arterial bubble counter probe. The water level is kept at 300 ml in the cardiotomy reservoir, which is positioned higher than the collapsible reservoir. Venous flow from the pseudo patient into the venous line was controlled by siphon drainage. TEST 1 In Test 1 a 50 ml bolus of air is rapidly injected, just before the oxygenator (‘arterial air injection’ in figure 1). During four minutes thereafter, bubbles are counted

distal to the arterial filter (‘arterial bubble probe’ in figure 1) and at the point just prior to the air injection site (‘venous bubble probe’ in figure 1). TEST 2: In Test 2 the same amount of air is slowly introduced just before the venous reservoir (‘venous air injection’ in figure1), during a period of 150 seconds, using a syringe pump (Perfusor® fm B.Braun). In this test, emboli are counted between the centrifugal pump and oxygenator, as well as after the arterial filter, during a four minutes time period. Each test is performed twice for every oxygenator/filter combination. Between measurements the circuit is de-aired until no emboli are detected by the bubble counter (BCC200, GAMPT). Results For each testing method (Test 1 (acute arterial) and Test 2 (continuous venous introduction of air) the results are displayed in tables 1 and 2 respectively. For Test 1 we display the total amount of air passing through the oxygenator/filter combination (Vol out) and the maximum embolic load (max emboli/s). Furthermore the total number of emboli (Num Out) and their size (Diameter Out) is shown. For Test 2 the amount of air entering the oxygenator is displayed as well (Vol B.Braun, Germany GAMPT, Zappendorf, Germany
In, Num In and Mean emboli/s). Mean number of emboli/s are displayed instead of the maximum. Volume reduction (VolRed), number reduction (NumRed) and mean diameter index (MDI) are parameters calculated from the measured data of Test 2. Results of TEST 1 Re-entry effect: In Test 1 we measured bubbles proximal to the air injection site. After air injection we found some bubble activity that could only be explained as a re-entry effect. The air passing the oxygenator and filter was not entirely removed by the pseudo patient and the venous reservoir. The bubbles detected at the venous site were therefore considered ‘extra’ . This re-entry effect was 1-6% of the total arterial bubble volume, increasing with larger arterial bubble volumes. As most of this re-introduced air will be removed by the oxygenator and the filter (88.5%-99.8% as shown in the results of Test 2), the arterial bubble measurements will be influenced by this effect in only a minor way (up to 0,7%). As the precise influence of this effect per test is not known, and the effect is not of major influence it is not corrected for. Figure 2 shows how the GAMPT bubble counter presents its results after acute introduction of 50 ml of air (Test 1) directly before the Capiox oxygenator. The red line (arterial probe) in figure 2 presents the bubbles measured distal to the Capiox. It shows a reduction of emboli in the first minute after air introduction, down

to a minimum of approximately 1 bubble per second. For Qi and QD we found the arterial emboli generation to remain at a higher level (approx. 10 bubbles per second) during the four minutes of measurement (figure 3). The blue lines in figures 2 and 3 represent the emboli measured before the oxygenator. These emboli originate from the 50 l container as a re-entry effect. This re-entry effect is 2%, 1% and 6% of the total air volume measured at the arterial site for Cap, Qi and QD respectively.

The total number of bubbles, measured at the arterial side is 1243, 2044 and 8738, for Cap, Qi and QD respectively. The average bubble size passing the oxygenator and filter is 35, 48 and 69 μm for Cap, Qi and QD respectively. The diagram in figure 5 expresses the number and size of the bubbles per oxygenator/filter combination. No bubbles >500 μm (presented as ‘over range’) were detected.

The total volume of air passing the oxygenator and the filter is different in al three tested oxygenator/filter combinations. This is clearly visible in figure 4. We found 41 nl (Cap), 173 nl (Qi) and 4215 nl (QD) passing oxygenator and filter. Worth mentioning is the big difference between

emboli/s (max 76), with an average bubble size of 46 μm (max 299). Ninety percent of the bubbles are between 20 and 80 μm, as shown by the blue lines in the diagram called ‘Number’ in figure 6. The differences in total volume of air measured at the arterial side are pointed out in Figure 7. Respectively 0.9 vs 65.4 vs 39.6 nl of air passed Cap, Qi and QD (Vol out, table 2). As the amount of air presented at the oxygenator inlet is comparable in all tests, the air reduction follows the same, but inversed, pattern (99.8% vs 88.5% vs 93.0% for Cap, Qi and QD resp.). Figure 8 shows the number and size of the bubbles measured after the filter. Again the Capiox handles the air more efficiently. This accounts for the total number of bubbles as well as the average size and the maximum size of the bubbles passing oxygenator and filter. Numbers are displayed in table 2.

Discussion In this experiment we searched for possible differences in air handling capacity of the Capiox FX 25, compared to the Quadrox i and Quadrox D with Quart arterial filter. Our results show the Capiox FX 25 to remove air more efficiently than the Quadrox oxygenators with Quart filter.

When searching for an explanation for the relatively big differences, we found first of all the pore size of the filters used. The fact that the Capiox FX 25 has a filter with 32 μm pores, while the Quart and the Quadrox i have 40 μm pores may be considered a significant difference9. Another difference is the type of fibres. The Capiox FX 25 and the Quadrox i both have micro porous oxygenation fibres, but the Quadrox D has a diffusion membrane consisting of polypentene (closed) fibres. This may account for the great difference found in Test 1, where massive air is introduced into the oxygenator. Inside the Capiox FX25 or the Quadrox i the air may purge through the porous fibres. The Quadrox D has a deairing membrane at the top of the oxygenator where massive
air should be removed. The efficiency of this membrane may be less than the efficiency of microporous fibres. The air inside the oxygenator may disperse bubbles for a longer period of time after air injection, as seen in figure 3 compared to figure 2. The Quadrox i does have porous fibres but seems not as effective at removing air as the Capiox. The incorporation of the one layer screen filter close to the hollow fibres seems to create a situation where bubbles are easily led through the porous fibres of the oxygenator. Despite the fact that only a single oxygenator per test setup has been used, homogeneity of the product is presumed sufficient to trust our conclusions to be useful. However, we tested under non clinical conditions, using water instead of blood, 20 instead of 37 ºC, and a 50 liter container instead of a patient. These factors are of significant influence when measuring gaseous emboli8, so clinical performance may not correlate with the results from our experiment. Especially the use of water instead of blood may be of significant impact. Blood has a lower surface tension, diminishing bubble point pressure and thereby reducing the capability of screen filters to trap air. Furthermore when using blood, membrane surfaces will be coated with proteins, affecting the air handling capacity of the oxygenators and filters. F. de Somer and colleagues16 tested the accuracy of the GAMPT BC200. They concluded the maximal measurable bubble count to be ± 200 counts/s, as the minimum time interval between bubbles is 5000 μs. In Test 1 we find maximum bubble counts of 135, 114, 332 counts/s for Cap, Qi, QD resp. The fact that we did find 332 counts/s may be explaned by difference in software versions. According to F. de Somer the measurements of the GAMPT BC200 may underestimate the real embolic load. In this view Test 1 shows bubble counts that may be underestimated, especially for the higher embolic

load using the Quadrox D with separate arterial filter. The technique we used to introduce bubbles in Test 2 creates a slow stream (30 counts/s) of relatively small (46 μm) bubbles with good reproducibility. Maximum arterial bubble counts are between 3-27 counts/s. Measurements by the GAMPT BCC200 of this kind of embolic load are expected to be accurate. M. Jirschik and colleagues found the Quadrox i to reduce 75% of the total volume of air during clinical application with 76 patients. In our non clinical setting we found a volume reduction of 88.5%. Jirschik concluded the Quadrox i to be superior to the Polystan Safe Maxi with a volume reduction of 46%. Weitkemper and colleagues found the Quadrox i to be the best of six, when testing oxygenators for their air handling capacity18. Although conditions in our experiment are different, the volume reduction of the Capiox being 99.8% is striking. A study with clinical setup and greater data collection is required to prove practical and statistical significance of our findings. We tested only air handling, so these results represent only one aspect of the performance of the oxygenators and filters. Pressure drop, compliance, oxygen- and carbon dioxide transfer rate or other important characteristics were not tested. 17
This experiment focuses on the removal of air by oxygenators and filters only. Jones and colleagues have shown that other components may also play a significant role in the removal of air from the ECC8. They showed that components from different companies have very different characteristics, concerning air removal. For optimal air removal venous air filtration19 or an arterial bubble trap20 could be used, but this will of course, increase complexity of the circuit. In this experiment, the equipment we
used did not entirely remove all of the air from the extracorporeal circuit. Furthermore Test 2 imitated a possible clinical situation (air entering the venous line) for only 4 minutes. During the longer time period of an operation the total amount of air presented to the ECC may be higher. This means that professionals should still focus on the prevention of air entering the ECC. Conclusion The air handling capacity of the Capiox FX25 is superior when introducing a large bolus of air, as well as after slow introduction of micro-emboli, compared to Quadrox i and the Quadrox D/Quart oxygenator/filter combinations. When comparing the Quadrox i with a Quadrox D with a separate Quart arterial filter, we find the Quadrox i to handle massive air in a better way than the Quadrox D. On the contrary the Quadrox D may handle micro bubbles somewhat better than the Quadrox i. Hernieuwde Kennismaking Isala Klinieken Zwolle
fusie ziekenhuis De Isala klinieken zijn in 1998 ontstaan door een fusie tussen het Sophia Ziekenhuis en Ziekenhuis De Weezenlanden. Beide ziekenhuizen zijn meer dan 100 jaar oud en hebben een rijke geschiedenis. De twee locaties samen zullen in een nieuw groot ziekenhuis komen. In de Isala-klinieken werken meer dan 5.000 medewerkers en er zijn bijna 1.000 beschikbare bedden. Per jaar worden meer dan 522.000 polikliniekbezoeken verzorgd en ruim 93.000 klinische opnames en dagbehandelingen. Daarbij staan de Isala klinieken voor kwaliteit, aandacht en een persoonlijke band met de patiënt. De Isala klinieken bieden alle basisvoorzieningen die een patiënt van een ziekenhuis mag verwachten. In de loop der jaren hebben de Isala kliniekenwsteeds meer hooggespecialiseerde functies toegewezen gekregen, zoals hart- en neurochirurgie, dialyse, stamcel- en beenmergtransplantaties. Op die en andere terreinen kunnen de Isala klinieken zich meten met academische ziekenhuizen. Deze hooggespecialiseerde zorg noemen we topklinische zorg. De Isala Klinieken is een van de grootste, zoniet het grootste topklinische ziekenhuis van Nederland.

Na jaren van onderhandelen met de Isala klinieken is de maatschap ECCare tot stand gekomen. ECCare is opgericht in september 2009 en het eerste grote contract met de Isala Klinieken is getekend in Zwolle in juni 2010. ECCare is een maatschap van zes klinisch perfusionisten die diensten aan medische instellingen en bedrijven verleent op het gebied van extracorporale circulatie ondersteuning ten behoeve van open hart chirur-

gische ingrepen en bloedbesparende- en behandelings-technieken bij chirurgische ingrepen in het algemeen. Daarnaast is ECCare ook verantwoordelijk voor harten longondersteuningssystemen bij de behandeling van patiënten met hart en/of longfalen. Op jaarbasis verzorgt ECCare bij meer dan 1300 openhart chirurgische ingrepen de extracorporale circulatie ondersteuning en worden er circa 1000 bloedmanagement procedures uitgevoerd onder andere bij orthopedische ingrepen. Voor de Isala-klinieken is ECCare tevens verantwoordelijk voor de praktijkbegeleiding en ondersteuning in de theoretische opleiding van klinisch perfusionisten. Verder worden er consultancy activiteitenvoor diverse bedrijven georganiseerd en worden er desgewenst trainingen gegeven aan klanten. De ervaring is niet alleen groot in termen van aantallen behandelde patiënten maar ook ten aanzien van de grote diversiteit aan toe te passen technieken, te gebruiken materialen en apparatuur. ECCare heeft een ruime ervaring met mini ECCsystemen en maakt voor de “de Isala-klinieken” routinematig gebruik van een zelf ontworpen mini systeem wat desgewenst voor elke operatie uitgebreid kan worden. Diverse technieken zoals ECC zonder cardiotomiezuigers en -reservoir, mini bloedcardioplegie, diepe hypothermie met circulatiestilstand in combinatie met
antegrade cerebrale perfusie, kinetisch ondersteunde veneuze drainage en diverse andere technieken worden dagelijks toegepast. Logo Het logo van ECcare representeert het belangrijkste aspect van het vakgebied nl het in stand houden van de lichaamscirculatie met behulp van een hartlongmachine. De ononderbroken cirkels stellen het arteriële en veneuze deel van het menselijk vaatsysteem voor die, in elkaar verweven als de cirkels, er voor zorgen dat de menselijke weefsels van zuurstof en voedingsstoffen worden voorzien en dat afvalstoffen worden afgevoerd. De cirkels zijn zodanig in het logo geplaatst dat er meerdere harten zichtbaar zijn. Het hart is de drijvende kracht die nodig is om de eigen lichaamscirculatie in stand te houden en ziekten en aandoeningen van het hart zijn de belangrijkste reden van het bestaan van de beroepsgroep. Een berichtje uit Paramaribo

Ik kom net uit het zwembad, heerlijk vanuit het werk lekker zwemmen in het buitenbad en in je korte broek weer naar huis. Het blijft een heerlijkheid om geen enkel moment een jas te hoeven dragen, je hoeft er niet eens aan te denken. Het enige waar je hier wel aan moet denken is een paraplu, vooral nu want we zitten vol in de regentijd. Met enige regelmaat staan de straten vol water en moet je rustig rijden. Het gaat momenteel niet zo goed met de Surinaamse ziekenhuizen. Er is in 2008 een start gemaakt met een functiewaarderingssysteem. Hierdoor zijn de salarissen gestegen en daardoor zijn de uitgaven flink toegenomen. Helaas zijn de betalingen aan het ziekenhuis niet aangepast. Alle landsambtenaren zijn verzekerd via het staatsziekenfonds en deze moet de kosten van de behandelingen en opnames betalen. Helaas is dit een ‘all-in’ tarief wat inhoudt dat er een bedrag per ligdag wordt betaald zonder rekening te houden met de behandeling en ook nog eens een bedrag dat jaren geleden bedongen is. Je begrijpt dat het ziekenhuis hier niet van kan rondkomen. Het heeft dan ook een direct effect op alle bestellingen die geplaatst worden. Wanneer hier bestellingen geplaatst worden bij een firma moet eerst een deel betaling plaats vinden voordat het artikel daadwerkelijk komt. Omdat vervolgens het artikel vaak ook nog uit het buitenland moet komen, Nederland of USA, is het flink geduld hebben. In september is er groot onderhoud uitgevoerd op de OK. Dit was erg hard nodig. Het gebouw staat al 45 jaar en alleen echt noodzakelijke reparaties werden uitgevoerd. De gaspunten zijn vervangen, nieuwe stopcontacten aangelegd, tegelwerk gerepareerd en nog andere klussen. Het was een flinke bende hier op de OK. De spoedpatiënten werden met een ambulance naar een ander ziekenhuis gebracht. Gelukkig was binnen een week alles gereed en konden we op een mooie schone OK aan de slag. Eind vorig jaar hebben we vier hartlongmachines uit Leiden gekregen met een aantal gasblenders en daar zijn we heel blij mee. Er werd nog steeds gedraaid met een COBE hartlongmachine die ooit met een hartmissie meegekomen was. De twee perfusionisten grijpen gelijk de kans aan om de machine anders op te bouwen en het systeem te verkleinen. Mochten er klinieken zijn die spullen over hebben, bijvoorbeeld bijna verlopen ballonnen voor IABP (34 cc) of apparaten die boekhoudkundig worden afgeschreven, wij houden ons warm aanbevolen. Een heel extreem voorbeeld van dingen die hier heel hard nodig zijn maar waar gewoon geen geld voor is, zijn ziekenhuisbedden. Het is soms schrijnend om te zien hoe de bedden er uit zien. Gelukkig hebben we nog niet zo lang geleden een groot aantal matrassen gekregen, de patiënten liggen dus wel zacht. Met warme groeten uit Paramaribo, Anjo

Per oktober 2010 is een nieuwe groep klinisch perfusionisten in opleiding gestart. Omdat er voor de maart groep 2010 niet voldoende aanmeldingen waren is de startdatum verlegd naar oktober 2010. Tijdens de Adviescommissie vergadering van oktober 2011 kwam het ter sprake dat er mogelijk alweer in maart 2011 gestart zou kunnen worden, maar na inventarisatie bleek dat niet zo te zijn. Na al een aantal toezeggingen te hebben ontvangen en mede omdat in september 2011 het Opleidingsfonds tot uitbetaling overgaat, is het de verwachting dat we voor oktober 2011 voldoende aanmeldingen zullen ontvangen.

Het 35-jarige jubilieum van de NeSECC is een succes waar iedereen binnen de NeSECC trots op mag zijn. Het PION is er als commissie binnen de NeSECC erg blij mee dat er een beroepsvereniging is, waardoor er extra verdieping aan het beroep gegeven kan worden. Vanaf 2003 vallen wij officieel binnen de NeSECC en kunnen daardoor o. a. themadagen organiseren om extra verdieping en verbreding aan de opleiding te geven. Aansluitend op de wetenschappelijke dag in Nijmegen heeft het PION 24 en 25 september het themaweekend ‘Noodprocedures’ in Zwolle georganiseerd. Voor de eerstejaars was dit een leuke gelegenheid om kennis te maken met medestudenten. We zijn ontvangen in de Isala klinieken en na het welkomstwoord van de divisie manager heeft Cees Maas een presentatie gehouden over noodprocedures. Aan de hand van een case report heeft hij een boeiende presentatie gegeven over calamiteiten tijdens ECC. Peter Segers heeft vervolgens een presentatie gehouden over de oxygenator functie, functie monitoring en falen. Als laatste heeft Carolien Menting, tweede jaars in opleiding in het Catharina ziekenhuis, een case report gehouden met betrekking tot arteriële pompfalen. Daarna hebben we de theorie van die ochtend in de praktijk gebracht. Er werd een situatie nagebootst waarbij er massaal veneuze lucht optrad. Met behulp van een AV-shunt moesten we dit probleem zien te verhelpen. Dit was lastiger dan gedacht. We hebben de KVAD (kinetische veneuze assist device) toegepast en er was de opdracht een oxygenator te vervangen. Het is niet helemaal een waterballet geworden, maar het was lastiger dan gedacht. Het themaweekend is een leerzaam en gezellig weekend geweest. Het is goed dat we als perfusionisten in opleiding ons er van bewust zijn wat er allemaal mis kan gaan tijdens de ECC, maar vooral hoe het opgelost kan worden. Met dank aan de Isala klinieken in Zwolle is dit weekend een succes geworden! Donderdag 13 januari 2011 heeft PION in Leiden een themadag georganiseerd, met als thema ‘Bloedstollend’. Een erg actueel thema als gekeken wordt naar het toegenomen gebruik van stollingsdiagnostiek binnen de cardiochirurgie. Ter introductie heeft Benoitte Genette een presentatie gegeven over de stolling en het Heparine Management Systeem (HMS). Carla Megens heeft een interessante presentatie gegeven over haar afstudeeronderzoek wat betrekking had op het heparine management tijdens ECC. Vanwege omstandigheden met de laatste spreker ging het laatste praatje over thromboelastografie (TEG) en klassieke stollingstesten helaas niet door. De themadag was een duidelijke weergave over het complexe onderwerp stolling. Met een nieuwe voorzitter, Henri van Heugten, hebben we ook dit jaar weer een aantal leuke thema’s op het programma staan. 20 en 21 mei gaan we het themaweekend ‘Transplantatie’ in Groningen organiseren. Daarnaast hebben we nog een aantal ideeën die misschien zelf interessant zijn voor in het opleidingsprogramma. Maar als een kliniek interesse heeft om een themadag mee te organiseren of ideeën heeft dan horen we dat graag! NeSECC, op naar de volgende 35 jaar!
