1.Versnelde afbraak van bloedcellen 2.Te kort aan bloedplaatjes 3.Slechte nierfunctie – nierfalen Stolsels / kleine bloedpropjes in de bloedvaatjes van de nier = thrombotische micro-angiopathie

Meestal kinderen < 5 jaar , ouderen > 65 jaar, zomer-nazomer Per jaar ongeveer 20 -30 kinderen met STEC-HUS RIVM 1250 meldingen STEC infectie Acute fase : 2-5% overlijden, restverschijnselen na doormaken HUS 5-30 %, Komt niet meer terug Komt ook niet weer terug na een niertransplantatie

Kan op elke leeftijd voorkomen, elk moment van het jaar Vaak voorafgegaan door infectie Luchtweg-infectie Maag darm infectie , ook diarree (meestal niet bloederig)

In 10-15% aHUS in families Prognose slechter dan bij STEC-HUS Groot deel blijft dialyse afhankelijk Kan terugkomen, aHUS kan ook weer in niertransplant terugkomen

Oorzaak van aHUS = ontregeld complement systeem
Wat doet het complement systeem Bestaat uit meer dan 30 verschillende eiwitten belangrijk in de eerste afweer tegen bacterien, het in gang zetten van de witte bloedcellen Kettingsreactie van eiwitten die elkaar activeren Activatie van systeem komt op gang door verschillende soorten prikkels: bacterien, virussen stress auto-antistoffen Inactiveren van systeem door specifieke eiwitten = regulatoren

• Op tijd herkennen • Op tijd bloedafnames naar oorzaken van vorm HUS • Bij verdenking aHUS al starten met plasma-behandeling • Plasma-infusie • Plasma-aferese net zo lang doorgaan tot beeld ‘rustig’ is (korte periode – levenslang)
