Gevalsbespreking Therapie in neurologie: hoofdpijn

• 39-jarige vrouw
• voorgeschiedenis: depressie
• migraine zonder aura vanaf de menarche
• middelengebruik: geen
• progressieve toename in hoofdpijn frekwentie sedert 1 jaar
• medicatiegebruik: 2 dagen per week paracetamol / paracetamol codeïne / zolmitriptan
• dagelijkse hoofdpijn sedert drie maanden

Behandeling van chronische migraine
Preventieve behandeling van chronische migraine: algemene beginselen
Behandeling van chronische migraine 1. Hygienische maatregelen: ingrijpen op 1. gebrek aan beweging 2. alcohol/caffeine overconsumptie 3. Onregelmatige maaltijden. 4. Slaap (zowel teveel als te weinig) 5. Tabagisme
2. Comorbiditeit behandelen: 1 angst/depressie 2. obesitas 3. OSAS
3. Profylaxe:
•
Profylaxe kan het aantal migraine hoofdpijndagen, en de ernst en de invaliditeit ervan reduceren. Geneesmiddel selecteren in functie van comorbiditeit
•
Combinatie van profylactica kan noodzakelijk zijn.
•

• Beperk acute behandeling: - simpele analgetica minder dan 15 dagen/maand - combinatie analgetica / 5-HT1 agonisten minder dan 10 dagen/maand - combinatie van verschillende klassen minder dan 10 dagen/maand.
