VOORWOORD Terwijl ons vakgebied begonnen is door toepassing van het weinig wetenschappelijke concept van trial-and-error, waarbij vaak bij toeval de bedwelmende werking van verschillende gassen werd ontdekt, is ons vak geëvolueerd tot een hoogwaardig specialisme waarin wetenschappelijk onderzoek aan de basis staat van nieuwe ontwikkelingen. Want, de anesthesiologie leent zich als geen ander vak tot het doen van wetenschappelijk onderzoek voor, tijdens en na de chirurgische ingreep. Nederland heeft altijd een belangrijke rol gespeeld in de wetenschappelijke ontwikkelingen binnen de Anesthesiologie. Dat geldt nog steeds, ook vandaag de dag publiceren Nederlandse anesthesiologen regelmatig in de top anesthesiologische- en pijntijdschriften, zoals Anesthesiology, Pain en de British Journal of Anaesthesia. Daar kunnen we niet anders dan trots op zijn met dank aan de visionaire figuren uit het heden en verleden die de weg hiervoor hebben vrijgemaakt. De Wetenschapsdag 2015 wordt door de afdeling Anesthesiologie van het Leids Universitair Medisch Centrum georganiseerd in een nieuwe systematiek waarin de opleidingsklinieken elkaar zullen afwisselen in de organisatie van deze dag. Volgend jaar is het ErasmusMC aan de beurt. Ook dit jaar hebben we weer een groot aantal inzendingen van hoge kwaliteit ontvangen. We hebben moeilijke keuzes moeten maken maar de meeste inzendingen komen aan bod, ofwel als lange of korte presentatie ofwel als ePoster. De wetenschappelijke inzendingen bevestigen het beeld dat in Nederland nog steeds vernieuwend onderzoek kan worden uitgevoerd door vaak jonge onderzoekers. Het thema van deze Wetenschapsdag is dan ook van aios, voor aios. We hebben dit jaar twee bijzondere gastsprekers weten te strikken, en voor het eerst twee vrouwen. Iets wat ons zeker niet zal verbazen gezien het feit dat de geneeskunde snel feminiseert. Professor Nadine Attal, neuroloog en pijndokter uit Parijs zal praten over haar inzichten in de pathofysiologie, diagnostiek en behandeling van neuropathische pijn. Professor Attal wordt wereldwijd gezien als een eminent wetenschapper op het gebied van chronische pijn met recente publicaties in Brain en Lancet Neurology. Professor Trudy Dehue is hoogleraar wetenschap aan de Universiteit Groningen en zij zal praten over de complexiteiten van het doen van wetenschappelijk onderzoek. WETEN VERGT MEER DAN METEN Trudy Dehue Meten, tellen, rekenen en (hersen)scannen kunnen van groot belang zijn in de wetenschap, maar ze zijn onvermijdelijk ingebed in ontwerpen, indelen, definiëren, beslissen en interpreteren. Wie armoede onderzoekt, moet eerst een definitie van armoede kiezen en vervolgens meetmethoden afleiden uit deze definitie. En wie een antidepressivum test op een rat of muis, moet eerst bepalen wat telt als een (niet langer) depressieve rat of muis en daarvoor tests ontwerpen. Eigenlijk maken dergelijke verhalen áchter de feiten wetenschappelijk onderzoek ook het meest moeilijk en bijzonder. Zolang ze echter verborgen blijven, bevragen we de wetenschap er niet op. Als we denken dat wetenschappelijke feiten de natuur weerspiegelen, maken de beslissingen waarop ze berusten de realiteit vóór anderen maar niet met hen.
Trudy Dehue is wetenschapstheoreticus en wetenschapshistoricus. Ze is als hoogleraar verbonden aan de faculteit Psychologie van de Universiteit Groningen. Naast Engelstalige artikelen en een boek bij Cambridge University Press (1995) publiceerde ze de Nederlandstalige boeken De depressie-epidemie. Over de plicht het lot in eigen hand te nemen (Augustus, 2008) en Betere mensen. Over gezondheid als keuze en koopwaar (AtlasContact, 2014).

NEUROPATHIC PAIN: IMPROVING OUTCOME THROUGH OPTIMIZED THERAPEUTIC STRATEGIES Prof. Nadine Attal

Professor Nadine Attal directs the Centre for the Evaluation and Treatment of Pain at the Ambroise Paré Hospital in Boulogne, France. After she specialised in neurology and pain she focused her scientific research primarily on neuropathic pain. In 2011 she was appointed associate professor for pain. Since 2001, she is member of the U 987 INSERM Research Unit on pain (directed by D. Bouhassira). Professor Attal is a member of several scientific societies, including the Society for Neuroscience, the International Association for the Study of Pain (IASP) and the French Society on Pain. In addition, she is member of the management committee for the Neuropathic Pain Special Interest Group of the IASP (NeuPSIG). Professor Attal recently co-chaired the updated NeuPSIG recommendations on the pharmacological treatment of neuropathic pain with Nanna Finnerup (Denmark), chaired the prior EFNS guidelines on pharmacotherapy and was involved in several NeuPSIG recommendations on the assessment of neuropathic pain. Professor Attal has authored over 100 refereed journal articles, more than 50 book chapters and contributed to several other publications on neuropathic pain.
Neuropathic pain is a significant medical and socio-economic burden. Epidemiological surveys have indicated that many patients with neuropathic pain do not receive appropriate treatment for their pain. A number of pharmacological agents have been found effective in neuropathic pain on the basis of randomized controlled trials and evidence-based recommendations for the pharmacotherapy of neuropathic pain have recently been updated (1). However, all meta-analyses indicate that only a minority of patients with neuropathic pain have adequate response to drug therapy. Several reasons may account for these findings, including a modest efficacy of the active drugs, a high placebo response, the heterogeneity of diagnostic criteria for neuropathic pain and an inadequate classification of patients in clinical trials (1, 2). Improving the current way of conducting clinical trials in neuropathic pain should contribute to reduce therapeutic failures and may have an impact on future therapeutic algorithms. The present presentation will emphasize the treatments available for neuropathic pain, the updated therapeutic recommendations and the strategies to optimize therapeutic response.
