WELCOME Dear Colleagues, It is our pleasure to welcome you to the XXVIth edition of the Belgian Week of Gastroenterology.

Enclosed you will find the complete program of this meeting. Several high quality abstracts, which illustrate the high level of both basic and clinical research in Belgium, have been selected for oral presentation and alternate with invited lectures by national and international experts during the scientific meetings. The best abstracts of the several scientific societies and research groups will be presented at the Plenary Session on Friday afternoon. High-quality abstracts that could not be selected as oral presentations, are presented as posters. A selection of best basic and clinical posters will be orally discussed during the oral poster sessions on Thursday and Friday. This edition of the Belgian Week starts already on Wednesday evening with a new initiative: a hands-on training course in abdominal ultrasound on models of hepatic and gastrointestinal diseases. A second session will be organized on Thursday evening.

As was the case last year, the meeting also includes an interactive case-report session on Friday, and a meeting of the IBD nurses. On Saturday we have the Seven Societies Postgraduate Course organized by the Royal Belgian Society for Surgery and entitled: “Bariatric and Metabolic Surgery / Cystic Lesions of the Pancreas”. This session has been granted accreditation points in Ethics & Economy.

Dit geneesmiddel is onderworpen aan aanvullende monitoring. Daardoor kan snel nieuwe veiligheidsinformatie worden fenobarbital en dus kunnen leiden tot lagere blootstelling en verlies van werkzaamheid van INCIVO. Raadpleeg de SamenvatPP: € 9098,66 - 375 mg - 168 tabs vastgesteld. Beroepsbeoefenaren in de gezondheidszorg wordt verzocht alle vermoedelijke bijwerkingen te melden. ting van de Productkenmerken van peginterferon alfa en van ribavirine voor een lijst met de contra-indicaties van deze producten, aangezien INCIVO in combinatie met peginterferon alfa en ribavirine gebruikt moet worden. Bijwerkingen: Samenvatting van het veiligheidsprofiel: Het totale veiligheidsprofiel van INCIVO is NAAM VAN HET GENEESMIDDEL: INCIVO 375 mg filmomhulde tabletten. KWALITATIEVE EN KWANTITATIEVE SAMENSTELLING: Elke filmomhulde tablet bevat 375 mg telaprevir. Hulpstof: 2,3 mg gebaseerd op gegevens uit fase 2- en fase 3-studies (zowel met als zonder controlegroep) met 3.441 patiënten die behandeld werden met INCIVO-combinatiebehandeling en op spontane postmarketnatrium per filmomhulde tablet. FARMACEUTISCHE VORM: Filmomhulde tablet. Gele, capsulevormige tablet van ongeveer 20 mm lang, waarop aan één kant ‘T375’ staat. Therapeutische indicaties: ingmeldingen. INCIVO moet worden toegediend met peginterferon alfa en ribavirine. Raadpleeg de betreffende Samenvatting van de Productkenmerken voor de bijwerkingen van die producten. De INCIVO, in combinatie met peginterferon alfa en ribavirine, is geïndiceerd voor de behandeling van genotype 1 chronische hepatitis C bij volwassen patiënten met gecompenseerde leverziekte (waaronder incidentie van bijwerkingen van minstens matige ernst (≥ graad 2) was in de INCIVO-groep hoger dan in de placebogroep. Tijdens de behandelingsfase met INCIVO of placebo waren de meest frequent cirrose): - die nooit eerder behandeld werden (therapienaïef); - die eerder behandeld werden met interferon alfa (gepegyleerd of niet gepegyleerd) als monotherapie of in combinatie met ribavirine, gemelde bijwerkingen met een ernst van minstens graad 2 in de INCIVO-groep (incidentie ≥ 5,0%) anemie, rash, pruritus, nausea en diarree. Tijdens de behandelingsfase met INCIVO of placebo waren waaronder patiënten met een recidief, partiële responders en patiënten zonder respons (null responders). Dosering en wijze van toediening: Behandeling met INCIVO dient te worden gestart en de meest frequent gemelde bijwerkingen met een ernst van minstens graad 3 in de INCIVO-groep (incidentie ≥ 1,0%) anemie, rash, trombocytopenie, lymfopenie, pruritus en nausea. Samenvattende gevolgd door een arts met ervaring in de behandeling van chronische hepatitis C. Dosering: INCIVO, 1.125 mg (drie filmomhulde tabletten van 375 mg) dient tweemaal daags (b.i.d.) oraal met voedsel te tabel van bijwerkingen: Bijwerkingen van INCIVO worden weergegeven in tabel 3. De bijwerkingen zijn ingedeeld naar systeem/orgaanklasse en frequentie: zeer vaak (≥ 1/10), vaak (≥ 1/100, < 1/10), worden ingenomen. Als alternatief kan 750 mg (twee tabletten van 375 mg) iedere 8 uur (q8h) oraal met voedsel ingenomen worden. De totale dagelijkse dosis is 6 tabletten (2.250 mg). Inname van soms (≥ 1/1.000, < 1/100) en zelden (≥ 1/10.000, < 1/1.000). Binnen elke frequentiegroep zijn de bijwerkingen weergegeven in volgorde van afnemende ernst. Tabel 3: Bijwerkingen van INCIVO INCIVO zonder voedsel of zonder het doseringsinterval in acht te nemen kan leiden tot verlaagde plasmaconcentraties van telaprevir, wat het therapeutisch effect van INCIVO kan verminderen. INCIVO (ingenomen in combinatie met peginterferon alfa en ribavirine) bij patiënten met een HCV-infectie in klinische studiesa en postmarketing Infecties en parasitaire aandoeningen: vaak: orale moet worden toegediend in combinatie met ribavirine en ofwel peginterferon alfa-2a ofwel -2b. Voor specifieke doseringsinstructies voor peginterferon alfa en ribavirine dient de Samenvatting van de candidiasis. Bloed- en lymfestelselaandoeningen: zeer vaak: anemie. vaak: trombocytopenieb, lymfopenieb. Endocriene aandoeningen: vaak: hypothyreoïdie. Voedings- en stofwisselingsstoornissen: vaak: Productkenmerken van deze geneesmiddelen geraadpleegd te worden. Behandelingsduur – Therapienaïeve volwassenen en patiënten met een recidief na een eerdere behandeling: De behandeling met hyperurikemieb, hypokaliëmieb. soms: jicht. Zenuwstelselaandoeningen: vaak: dysgeusie, syncope. Oogaandoeningen: soms: retinopathie. Maagdarmstelselaandoeningen: zeer vaak: nausea, diarree, INCIVO moet worden gestart in combinatie met peginterferon alfa en ribavirine en gedurende 12 weken worden aangehouden (zie figuur 1). - Patiënten met ondetecteerbaar ribonucleïnezuur van het braken, hemorroïden, proctalgie. vaak: anale pruritus, rectale hemorragie, anale fissuur. soms: proctitis. Lever- en galaandoeningen: vaak: hyperbilirubinemieb. Huid- en onderhuidaandoeningen: zeer hepatitis C-virus (HCV-RNA) (doel niet gedetecteerd) in week 4 en week 12 moeten additioneel nog 12 weken behandeld worden met alleen peginterferon alfa en ribavirine, voor een totale behandelingsvaak: pruritus, rash. vaak: eczeem, gezwollen gezicht, exfoliatieve uitslag. soms: geneesmiddelrash met eosinofilie en systemische symptomen (DRESS), urticaria. zelden: SJS, TEN, erythema multiforme. duur van 24 weken. - Patiënten met detecteerbaar HCV-RNA in week 4 of week 12 moeten additioneel nog 36 weken behandeld worden met alleen peginterferon alfa en ribavirine, voor een totale Nier- en urinewegaandoeningen: soms: creatinine in bloed verhoogdb, pre-renale azotemie met of zonder acuut renaal falen. Algemene aandoeningen en toedieningsplaatsstoornissen: vaak: perifeer behandelingsduur van 48 weken. - Voor alle patiënten met cirrose, ongeacht ondetecteerbaar HCV-RNA (doel niet gedetecteerd) in week 4 of 12, wordt een additionele behandelingsperiode van oedeem, productsmaak abnormaal. a aan de placebogecontroleerde fase 2- en fase 3-studies (samengevoegde gegevens) namen 1.346 HCV-geïnfecteerde patiënten deel. b incidentiepercentages zijn 36 weken met alleen peginterferon alfa en ribavirine aanbevolen, gebaseerd op meldingspercentages van ongewenste voorvallen voor een totale behandelingsduur van 48 weken. Figuur 1: Be(zie ook Laboratoriumafwijkingen hieronder). Bij de analyse van handelingsduur voor therapienaïeve patiënten en voor een additionele studie, Studie C211, was het veiligheidsprofiel van patiënten met een recidief na een eerdere behandeling. De combinatietherapie met INCIVO 1.125 mg tweemaal daags HCV-RNA-concentratie dient te worden gecontroleerd in week 4 vergelijkbaar met het veiligheidsprofiel voor patiënten die combien week 12 om de behandelingsduur te bepalen. In fase 3-studies natietherapie kregen met INCIVO 750 mg elke 8 uur. Er zijn geen werd een gevoelige, real-time PCR assay, met een kwantificatienieuwe veiligheidsgegevens geïdentificeerd. De HCV-RNA-concentratie dient te worden geconlow-density lipoproteïne: Graad 2: 6,9% (4,13-4,90 mmol/l 160troleerd in week 4 en week 12. Zie tabel 1 voor richtlijnen voor het 190 mg/dl); Graad 3: 2,5% (≥ 4,91 mmol/l ≥ 191 mg/dl); Graad 4: stopzetten van een behandeling met INCIVO, peginterferon alfa en nvt. creatinine: Graad 2: 0,9% (1,4-1,8 x ULN); Graad 3: 0,2% ribavirine. Alle patiënten: Aangezien het zeer onwaarschijnlijk is (1,9-3,4 x ULN); Graad 4: 0% (> 3,4 x ULN). Bij patiënten met HCV-RNA tussen en zijn dan vergelijkbaar met die waargenomen bij patiënten die 100 IE/ml en 1.000 IE/ml in week 12, bereikten 2/8 (25%) SVR. Bij alleen behandeld worden met peginterferon alfa en ribavirine. Bevoormalige null responders moet overwogen worden een extra schrijving van enkele specifieke bijwerkingen: Rash: Er zijn HCV-RNA-test uit te voeren tussen week 4 en week 12. Als de patiënten die behandeld werden met peginterferon alfa en ribaINCIVO-behandeling wordt stopgezet wegens bijwerkingen of virine. Meer dan 90% van de gevallen van rash was mild of matig wegens onvoldoende virologische respons, mag de behandeling van ernst. De rash die tijdens INCIVO-combinatiebehandeling werd met INCIVO niet opnieuw worden gestart. KWALITATIEVE EN KWANTITATIEVE SAMENSTELLING Eén maagsapresistente tablet van 20 mg en één maagsapresistente tablet van 40 mg bevatten respectievelijk 20 mg pantoprazol (als 22,6 mg pantoprazol-natrium-sesquihydraat) en 40 mg pantoprazol (als 45,10 mg pantoprazol-natrium-sesquihydraat). FARMACEUTISCHE VORM Maagsapresistente tablet. Een gele, ovale, biconvexe, filmomhulde tablet, aan één zijde bedrukt met “P20” (voor Pantomed 20mg) en “P40” (voor Pantomed 40mg) in bruine inkt. THERAPEUTISCHE INDICATIES PANTOMED 20 mg: - Behandeling van lichte refluxaandoeningen en ermee geassocieerde symptomen (bv. pyrosis, zuurregurgitatie, pijn bij het slikken). - Langetermijnbehandeling en preventie van recidieven van refluxoesofagitis. - Preventie van gastroduodenale zweren geïnduceerd door niet-selectieve niet-steroïdale anti inflammatoire geneesmiddelen (NSAIDs) bij risicopatiënten die een continue behandeling met NSAIDs vereisen. PANTOMED 40 mg: - matige en ernstige refluxoesofagitis - bij patiënten met peptische ulcera in combinatie met twee geschikte antibiotica (zie “Dosering”) voor de eradicatie van H. pylori, met de bedoeling recidieven van duodenum- en maagulcus, veroorzaakt door dit micro-organisme, te verminderen. - duodenumulcus - maagulcus - Zollinger-Ellison syndroom en andere pathologische hypersecretoire aandoeningen. DOSERING EN WIJZE VAN TOEDIENING PANTOMED 20 mg: Aanbevolen dosering: Volwassenen en adolescenten ouder dan 12 jaar: Lichte refluxaandoeningen en ermee geassocieerde symptomen (bv. pyrosis, zuurregurgitatie, pijn bij het slikken) De aanbevolen orale dosering bedraagt één maagsapresistente tablet Pantomed 20 mg per dag. Verlichting van de symptomen wordt meestal bekomen binnen de 2 tot 4 weken. Voor de genezing van een ermee gepaard gaande oesofagitis is gewoonlijk een behandelingsduur van 4 weken vereist. Indien dit niet volstaat, zal genezing normaal bekomen worden na een bijkomende behandeling van 4 weken. Eens de symptomen zijn verdwenen, kunnen opnieuw optredende symptomen gecontroleerd worden door gebruik te maken van een “gebruik volgens noodzaak” regime van 20 mg éénmaal per dag, wanneer nodig. Een overschakeling naar een continue therapie kan overwogen worden wanneer er geen voldoende symptoomcontrole behouden kan worden met een “gebruik volgens noodzaak” regime. Langetermijnbehandeling en preventie van recidieven van refluxoesofagitis. Een onderhoudsdosis van één maagsapresistente tablet Pantomed 20 mg per dag wordt aanbevolen voor langetermijnbehandeling. Indien een recidief optreedt, zal deze dosis verhoogd worden tot 40 mg pantoprazol per dag. Pantomed 40 mg is hiervoor beschikbaar. Na genezing van het recidief kan de dosering opnieuw gereduceerd worden tot 20 mg pantoprazol. Volwassenen: Preventie van gastroduodenale zweren geïnduceerd door niet-selectieve niet-steroïdale anti inflammatoire geneesmiddelen (NSAIDs) bij risicopatiënten die een langdurige behandeling met NSAIDs vereisen. De aanbevolen orale dosering bedraagt één maagsapresistente tablet Pantomed 20 mg per dag. Kinderen onder de 12 jaar: Pantomed wordt niet aangeraden voor gebruik bij kinderen onder de 12 jaar wegens te weinig gegevens binnen deze leeftijdsgroep. Opmerking: Bij patiënten met een ernstige leverbeschadiging mag de dagelijkse dosis van 20 mg pantoprazol niet overschreden worden. Er is geen dosisaanpassing nodig bij ouderen of bij patiënten met een gestoorde nierfunctie. Algemene instructies: Pantomed 20 mg maagsapresistente tabletten mogen niet gekauwd of geplet worden en moeten in hun geheel met een hoeveelheid water ingenomen worden vóór een maaltijd. PANTOMED 40 mg: Aanbevolen dosering: Volwassenen en adolescenten ouder dan 12 jaar: Behandeling van matige en ernstige refluxoesofagitis. Eén tablet Pantomed per dag. In individuele gevallen mag de dosis worden verdubbeld (verhogen tot 2 tabletten Pantomed per dag), vooral wanneer andere behandelingen niet doeltreffend waren. Volwassenen: Eradicatie van H. pylori in combinatie met twee geschikte antibiotica. Bij Helicobacter pylori positieve patiënten met maag- en duodenumzweren, dient eradicatie van de bacterie bekomen te worden via een combinatietherapie. Afhankelijk van het resistentiepatroon, kunnen de volgende combinaties aangeraden worden voor de eradicatie van H. pylori: a) tweemaal daags 1 tablet Pantomed + tweemaal daags 1.000 mg amoxicilline + tweemaal daags 500 mg clarithromycine b) tweemaal daags 1 tablet Pantomed + tweemaal daags 500 mg metronidazol + tweemaal daags 500 mg clarithromycine c) tweemaal daags 1 tablet Pantomed + tweemaal daags 1.000 mg amoxicilline + tweemaal daags 500 mg metronidazol. Indien de combinatietherapie geen optie is, bv. de patiënt is Helicobacter pylori negatief, dienen de doseringsaanbevelingen voor Pantomed monotherapie gevolgd te worden: Behandeling van maag- en duodenumulcus. Eén tablet Pantomed per dag. In individuele gevallen mag de dosis worden verdubbeld (verhogen tot 2 tabletten Pantomed per dag), vooral wanneer andere behandelingen niet doeltreffend waren. Zollinger-Ellison syndroom en andere pathologische hypersecretoire aandoeningen. Bij een langetermijnbehandeling van het Zollinger-Ellison syndroom en andere pathologische hypersecretoire aandoeningen, zullen de patiënten hun behandeling starten met een dagelijkse dosis van 80 mg (dit zijn 2 tabletten Pantomed 40 mg). Daarna kan de dosis zoals nodig verhoogd of verlaagd worden aan de hand van zuurtegraadbepalingen. Wanneer de dagelijkse dosis meer dan 80 mg bedraagt, dient deze over 2 innames per dag gespreid te worden. Een tijdelijke verhoging van de dosering boven 160 mg pantoprazol is mogelijk, maar deze mag niet langer dan nodig voor een adequate controle van de zuurtegraad toegediend worden. Bij het Zollinger-Ellison syndroom en andere pathologische hypersecretoire aandoeningen is de behandelingsduur niet beperkt en dient aangepast te worden aan de klinische behoeftes. Kinderen onder de 12 jaar: Pantomed wordt niet aangeraden voor gebruik bij kinderen onder de 12 jaar wegens te weinig gegevens binnen deze leeftijdsgroep. Speciale populaties: Bij patiënten met een ernstige leverbeschadiging dient de dosis gereduceerd te worden tot 1 tablet (40 mg pantoprazol) om de 2 dagen. Daarenboven zullen bij deze patiënten tijdens een behandeling met Pantomed de leverenzymen regelmatig gecontroleerd worden. In geval van gestegen leverenzymen dient de behandeling met Pantomed stopgezet te worden. De dagelijkse dosis van 40 mg pantoprazol mag niet overschreden worden bij bejaarde patiënten of bij patiënten met een gestoorde nierfunctie. Een uitzondering hierop is de combinatietherapie voor H. pylori eradicatie, waar ook bij oudere patiënten gedurende 1 week de normaal voorziene dosis pantoprazol (2 x 40 mg/dag) zal toegepast worden. Algemene instructies: Pantomed tabletten mogen niet gekauwd of geplet worden en moeten in hun geheel met een hoeveelheid water ingenomen worden 1 uur vóór het ontbijt. In de combinatietherapie voor eradicatie van Helicobacter pylori wordt de tweede tablet Pantomed vóór het avondeten ingenomen. De combinatietherapie duurt in het algemeen 7 dagen en kan verlengd worden tot maximaal 2 weken. Indien, teneinde de genezing van de ulcera te verzekeren, een verdere behandeling met pantoprazol geïndiceerd is, dienen de doseringsaanbevelingen voor de behandeling van duodenum- en maagulcera gevolgd te worden. Een duodenumulcus geneest doorgaans binnen de 2 weken. Indien een behandeling gedurende 2 weken niet voldoende is, zal in vrijwel alle gevallen volledige genezing bekomen worden na een bijkomende behandeling van 2 weken. Voor de behandeling van maagulcera en refluxoesofagitis is meestal een behandelingsduur van 4 weken vereist. Zoniet zal volledige genezing gewoonlijk bekomen worden na een bijkomende therapie van 4 weken. CONTRA-INDICATIES PANTOMED 20 mg: Pantomed 20 mg mag niet gebruikt worden in geval van gekende overgevoeligheid voor één van de bestanddelen van Pantomed 20 mg. Pantoprazol, zoals andere PPIs, mag niet tegelijkertijd met atazanavir worden toegediend. PANTOMED 40 mg: Pantomed mag in het algemeen niet gebruikt worden in geval van gekende overgevoeligheid voor één van de bestanddelen van Pantomed of voor één van de combinatieproducten. Pantomed zal in de combinatietherapie voor eradicatie van H. pylori bij patiënten met matige tot ernstige lever- of nierinsufficiëntie niet gebruikt worden, aangezien tot nog toe geen gegevens beschikbaar zijn over de werking en de veiligheid van Pantomed in de combinatietherapie bij deze patiënten. Pantoprazol, zoals andere PPIs, mag niet tegelijkertijd met atazanavir worden toegediend. BIJWERKINGEN (Vaak (≥ 1/100 tot < 1/10), Soms (≥ 1/1.000 tot < 1/100), Zelden (≥ 1/10.000 tot < 1/1.000), Zeer zelden (< 1/10.000), niet bekend (kan met de beschikbare gegevens niet worden bepaald). Onderzoeken: Zeer zelden: Verhoogde leverenzymen (transaminasen, -GT); verhoogde triglyceriden; verhoogde lichaamstemperatuur; Bloed- en lymfestelselaandoeningen: Zeer zelden: leukopenie; trombocytopenie; Zenuwstelselaandoeningen: Vaak: hoofdpijn – Soms : duizeligheid; stoornissen van het gezichtsvermogen (wazig zien); Maagdarmstelsel aandoeningen: Vaak: hoogdigestieve pijn, diarree, constipatie, flatulentie – Soms: nausea / braken – Zelden: droge mond; Nier- en urinewegaandoeningen: Zeer zelden: interstitiële nefritis; Huid- en onderhuidaandoeningen: Soms: Allergische reacties zoals pruritus en huidrash – Zeer zelden: urticaria; angio-oedeem; ernstige huidreacties zoals het syndroom van Stevens-Johnson, erythema multiforme, syndroom van Lyell; fotosensibiliteit; Skeletspierstelsel- en bindweefselaandoeningen: Zelden: Artralgie – Zeer zelden: myalgie; Algemene aandoeningen en toedieningsplaatsstoornissen: Zeer zelden: Tromboflebitis op de plaats van injectie, Perifeer oedeem; Immuunsysteem-aandoeningen: Zeer zelden: Anafylactische reacties, inclusief anafylactische shock; Lever- en galaandoeningen: Zeer zelden: Ernstige hepatocellulaire beschadiging leidend tot icterus, met of zonder leverfalen; Psychische stoornissen: Zelden: Depressie, hallucinatie, desoriëntatie en verwardheid, met name bij vatbare patiënten, zowel als verergering van deze symptomen indien voorafbestaand. FROM ANTWERP, GHENT, BRUGES, OSTEND: Drive toward the Brussels ring R0 and follow in the direction of Namen/Namur. Keep following the ring R0 and drive straight on (via the tunnel under the crossing where R0 crosses the motorway toward Namur–Luxembourg). Follow for about 6 km (4 miles) and take the Genval–Hoeilaart-Brussels exit. Cross the first traffic light and turn left toward Genval/La Hulpe at the second light.
