Voorwoord Stilstand betekent achteruitgang. Voor SIG Zorginformatie een reden te meer om flink aan de weg te timmeren om nog beter in te kunnen spelen op de informatie-behoefte die in het veld leeft. Dit heeft onder andere geresulteerd in een flink aantal nieuwe specialisme-gerichte publikaties van SIG Zorginformatie, samengesteld in nauwe samenwerking met de wetenschappelijke verenigingen. Elk van deze publikaties schetst de ‘grote lijnen’ van het specialisme bezien over een tijdsbestek van meerdere jaren. ‘Grote lijnen’ is inmiddels beschikbaar voor de specialismen kindergeneeskunde, orthopedie, interne geneeskunde, keel- neus- en oorheelkunde, cardiologie, heelkunde en neurochirurgie. Het is de bedoeling deze publikaties periodiek te verzorgen, waarbij steeds wisselende aspecten belicht kunnen worden.
De inhoud van deze informatie-bron, voor zowel de eerste- als de tweedelijns verloskunde in Nederland, is samengesteld in nauwe samenwerking met de Nederlandse Organisatie van Verloskundigen (NOV) en de Nederlandse Vereniging voor Obstetrie en Gynaecologie (NVOG). Om het toepassingsgebied van de publikatie te vergroten, is er voor gekozen om alle teksten behalve in het Nederlands, ook in het Engels weer te geven.
Om de herkenning en eenheid van specialismegerichte publikaties van SIG Zorginformatie te vergroten, is ervoor gekozen om ook bestaande publikaties in deze nieuwe lijn in te passen. Zo kreeg ook het jaarboek verloskunde een nieuw uiterlijk en een nieuwe naam: ‘Verloskunde in Nederland, Grote lijnen 1989-1993’.

Preface Stagnation implies decline. For SIG Health Care Information this is just one reason more to devote specific attention to this subject to even better anticipate on the information requirements in the healthcare field. This resulted a.o. in a substantial number of new publications on medical specialisms produced by SIG in close collaboration with scientific associations. Each of these publications sketches the ‘trends’ of a specialism viewed over a period of years. ‘Trends’ is now available for paediatrics, orthopaedics, internal medicine, ENT, cardiology, surgery and neurosurgery. It is the intention to provide these publications on a periodically bases, in which case changing aspects can be illustrated continuously. For better recognition of SIG’s publications concentrating on particular specialisms and for reasons of unity we decided to bring existing publications in line with this new format too. Therefore the obstetrics yearbook has also been given a new outlook and a new name: ‘Obstetrics in the Netherlands, Trends 1989 - 1993’.

Hoofdstuk 1 geeft algemene informatie over het Nederlandse verloskundige systeem, de scheiding eerste- en tweedelijns zorg, de representativiteit en de kwaliteit van de berichtgeving.
Chapter 1 provides general information on the Dutch obstetric system, the separation between primary and secondary care, and the representativeness and quality of the reporting process.
Hoofdstuk 2 presenteert informatie over bevallingen in de eerste en de tweede lijn in samenhang, overeenkomsten en verschillen.
Chapter 2 presents information on primary and secondary care deliveries in context, similarities and differences.
Meer specifieke informatie over de eerstelijns en de tweedelijns verloskunde is te vinden in respectievelijk hoofdstuk 3 en hoofdstuk 5. In hoofdstuk 4 wordt ingegaan op het ‘wanneer’ en ‘waarom’ van overdrachten tussen de eerste en de tweede lijn.
More specific information on primary and secondary care obstetrics can be found in Chapters 3 and 5 respectively. Chapter 4 examines ‘when’ and ‘why’ patients are referred between primary and secondary care.
In de hoofdstukken 2 t/m 5 worden onder meer kerngegevens gepresenteerd. Bij kerngegevens gaat het om de basiscijfers bij gebeurtenissen en handelingen betreffende de verloskundige zorg voor moeder en kind, zoals leeftijdsklasse van de moeder, pariteit, gewicht van het kind, zwangerschapsduur.

1 Verantwoording Responsibility 1.1 Het Nederlandse verloskundige systeem
1.1 Dutch obstetric system
Er bestaat binnen de Nederlandse verloskunde een duidelijke taakverdeling tussen de eerstelijns verloskundige zorg en de tweedelijns verloskundige zorg, gebaseerd op de scheidslijn tussen fysiologie en pathologie. De verloskundige en de huisarts in de eerste lijn begeleiden de gezonde zwangerschap en richten zich op de preventie en de selectie van pathologie. Uitgangspunt hierbij is dat zwangerschap, bevalling en kraambed in principe fysiologische gebeurtenissen zijn. Wanneer gedurende de zwangerschap, tijdens de baring of in het kraambed risico’s worden vastgesteld, consulteert de verloskundige of de huisarts de specialist in de tweede lijn. Na afweging van de risico’s (Verloskundige Indicatielijst), wordt bepaald of overdracht van zorg naar de tweede lijn wenselijk dan wel noodzakelijk is.
In the Dutch obstetric system there is a clear distribution of tasks between primary obstetric care and secondary obstetric care, based on the boundary between physiology and pathology. The independent midwife and the General Practitioner (GP) in primary care are responsible for a healthy pregnancy and aim at the prevention and the selection of pathology. This is based on the fact that in principle pregnancy, delivery and puerperium are physiological events. If, however, risks are identified during pregnancy, delivery or puerperium then the midwife or the GP consults the medical specialist in secondary care. After consideration of the risks (Obstetric Indication List/Verloskundige Indicatielijst), the decision is made whether referral of the patient to secondary care is desirable or necessary.

1.2 Scheiding eerste- en tweedelijns bevalling
1.2 Distinction between primary and secondary care delivery
In deze publikatie worden gegevens uit de LVR gepresenteerd van 1993 en in een aantal gevallen van 1989 tot en met 1993. In navolging van de jaarboeken van 1988 en 1991 worden de eersteen tweedelijns gegevens samen gepresenteerd. De gegevens van de eerste lijn (LVR-1) worden verstrekt door de deelnemende verloskundigen, van de tweede lijn (LVR-2) door de gynaecologen. De verloskundige zorg van de huisartsen wordt in de LVR slechts op beperkte schaal vastgelegd. De LVR gegevens van de huisartsen zijn alleen gebruikt in tabel 1.3a en bijlage A3.
This publication presents data from the National Perinatal Database of the Netherlands (LVR) from 1993 and, in a number of cases, from 1989 to 1993 inclusive. Following the pattern of the 1988 and 1991 yearbooks, primary and secondary care data are presented jointly. The data on primary care (LVR-1) has been provided by the participating independent midwives; the secondary care data (LVR-2) has been provided by the obstetricians. Obstetric care provided by the General Practitioners (GP’s) is only recorded in the LVR to a limited extent. LVR Data of GP’s are only used in Table 1.3a and Appendix A3.
Zwangerschap, bevalling en kraambed worden in Nederland beschouwd als fysiologische gebeurtenissen, totdat er complicaties optreden. Wanneer tijdens de zwangerschap, de baring of het kraambed risico’s worden geconstateerd, volgt een consult en/of overdracht naar de tweede lijn. Van de zorg die de verloskundige aan de vrouw biedt, worden gegevens in de LVR-1 vastgelegd. Ook wanneer de zorg slechts ten dele de zwangerschap, de baring en het kraambed betreft. Dit is het geval wanneer bijvoorbeeld tijdens de zwangerschap een vrouw wordt overgedragen aan de tweede lijn. Bij de tweede lijn worden de gegevens in de LVR2 alleen vastgelegd na de geboorte van een kind. De groep vrouwen die vóór, tijdens of direct post partum in de tweede lijn terechtkomt, kan dus twee keer in de LVR voorkomen, zowel in de LVR-1 als in de LVR-2. Om dubbeltellingen te voorkomen is het dus van belang om binnen de LVR een correct onderscheid te maken tussen eerste- en tweedelijns bevallingen.

Pregnancy, delivery and puerperium are regarded in the Netherlands as physiological events until complications occur. If risks are identified during pregnancy, delivery or puerperium, consultation and/or referral to secondary care follows. Data relating to care provided by the midwife to the woman are recorded in the LVR-1. Even if care only relates to a part of the pregnancy, delivery or puerperium. This is the case if, for example, a woman is referred to secondary care during pregnancy. In secondary care only data are recorded in LVR-2 after the birth of a child. The group of women referred to secondary care before, during or directly postpartum can occur therefore twice in the National Perinatal Database (LVR), in LVR-1 as well as in LVR-2. To avoid double counting it is therefore important to make a correct distinction in the LVR between primary and secondary care deliveries.

Eerstelijns bevallingen De bevallingen in de eerste lijn onder verantwoordelijkheid van de verloskundige zijn als volgt gedefinieerd: • er is door de verloskundige zorg verleend tijdens de baring; • er is geen overdracht naar de tweede lijn geweest vóór of tijdens de geboorte van het kind.
Tweedelijns bevallingen De selectie van bevallingen in de tweede lijn onder verantwoordelijkheid van de gynaecoloog is als volgt gedefinieerd: • de begeleiding door de gynaecoloog is gestart vóór of tijdens de geboorte van het kind.
Primary care deliveries Primary care deliveries under the responsibility of the independent midwife are defined as follows: • care has been provided by the midwife during delivery; • no referral to the secondary care has taken place before or during the birth of the child.
Secondary care deliveries The selection of secondary care deliveries under the responsibility of the obstetrician is defined as follows: • intervention by the obstetrician began before or during the birth of the child.
1.3 Representativiteit LVR
1.3 Representativeness of the LVR
In deze paragraaf zal een beeld worden gegeven van de representativiteit van de LVR-1 en LVR-2 en de deelnamegraad aan LVR. In de eerste plaats wordt het aandeel van de LVR t.o.v. het totaal aantal geborenen in Nederland en per provincie getoond. Daarna volgt de deelname van de verloskundigenpraktijken naar provincie, urbanisatiegraad en soort praktijk. Vervolgens wordt de deelname van de ziekenhuizen met het specialisme obstetrie/gynaecologie naar soort ziekenhuis en naar provincie behandeld.
This paragraph will illustrate the representativeness of the LVR-1 and LVR-2 and the participation degree in the LVR. In the first place the participation degree in the LVR is shown in comparison to the total number of births in the Netherlands and per province. This is followed by the participation of obstetric practices per province, the degree of urbanisation and the type of practice. Then the participation of hospitals with a obstetrics/gynaecology department is shown per type of hospital and per province.

De aantallen in tabel 1.3a betreffen levend- en doodgeborenen. Bij doodgeborenen worden de volgende criteria gehanteerd: door het Centraal Bureau voor de Statistiek (CBS) de doodgeborenen na een zwangerschapsduur van 28 weken1) en meer en door de LVR de doodgeborenen na een zwangerschapsduur van 16 weken en meer.

Om een schatting te kunnen maken van de verdeling eerstelijns en tweedelijns geborenen voor totaal Nederland, is gebruik gemaakt van zowel LVR als CBS gegevens.
To be able to make an estimation of the distribution of primary and secondary care births for the whole of the Netherlands, LVR data as well as CBS data are used.
Voor een gedetailleerde omschrijving van de gebruikte methode en berekeningswijze in tabel 1.3a, zie bijlage A3.

In figuur 1.3a is voor de jaren 1989 tot en met 1993 aangegeven hoe groot het (LVR) percentage geborenen is bij de verloskundigen en de gynaecologen.

Het LVR-aandeel van het totaal aantal geborenen bij verloskundigen is gestegen van 76% in 1989 naar 88% in 1993. In de tweede lijn is het LVR-aandeel in 1989 73% om vervolgens in 1993 te komen tot 87% van het totaal aantal geborenen bij gynaecologen.
The LVR share of the total number of births of midwife practices rose from 76% in 1989 to 88% in 1993. The LVR share in secondary care in 1989 was 73%, rising then to 87% of the total number of births attended by obstetricians in 1993.
In figuur 1.3b is per provincie gekeken naar het aantal geborenen dat in de LVR is geregistreerd en het totaal aantal geborenen. Er is hierbij geen onderscheid gemaakt in eerste- en tweedelijns geborenen.

Deelname van verloskundigen aan de LVR-1 Het is mogelijk de praktijken van de vrijgevestigde verloskundigen onder te verdelen naar provincie en urbanisatiegraad (bron: NIVEL1)). Aan de hand van het LVR-aandeel in de verschillende praktijksoorten kan bekeken worden of de LVR eerste lijn een doorsnede vormt van de praktijken in totaal Nederland wat betreft de genoemde parameters. Het totaal aantal praktijken in de volgende tabellen is kleiner dan het door NIVEL geregistreerde aantal van 475 per 01.01.93. Bij de LVR zijn in 1993 vijf praktijken geregistreerd die het NIVEL kent als tien aparte praktijken. In tabel 1.3b, tabel 1.3c en tabel 1.3d is daarom uitgegaan van een totaal aantal van 470 praktijken. De deelname van verloskundigenpraktijken van de provincies ligt tussen de 64 en 100%, waarvan Friesland met 64% het laagste en Flevoland en Zeeland met elk 100% het hoogste deelnamepercentage hebben. In landelijke gebieden is de deelname het hoogst met 90%, dan volgen de grote en kleine steden met respectievelijk 86% en 85%. De middelgrote steden zijn met 73% duidelijk ondervertegenwoordigd. De drie opleidingsklinieken voor verloskundigen in Amsterdam, Rotterdam, Heerlen en een drietal ziekenhuizen, waar verloskundigen in dienst zijn van het ziekenhuis, registreren wel in de LVR-1, maar zijn (alleen) in deze paragraaf buiten beschouwing, omdat het NIVEL deze klinieken niet als vrijgevestigde praktijken definieert. Het NIVEL beschikt over een registratie van werkzame verloskundigen. In deze registratie zijn alle verloskundigen opgenomen die vanaf 1970 werkzaam zijn (geweest). In deze registratie worden gegevens opgenomen als: adres, geslacht, leeftijd, afstudeerjaar, opleidingsinstituut, functie, praktijkadres, praktijkvorm e.d. De gegevens worden onder andere verkregen via jaarlijkse schriftelijke enquête onder alle werkzame verloskundigen.

Participation by midwives in the LVR-1 It is possible to classify independent midwife practices per province and per degree of urbanisation (source: NIVEL1)). Based on the LVR share in various types of practices it is possible to examine whether primary care LVR could form a profile of practices throughout the Netherlands with respect to the indicated parameters. The total number of practices in the tables following is smaller than the number of 475 per 01.01.93 recorded by NIVEL. In LVR five practices are recorded in 1993 which are regarded by NIVEL as ten separate practices. Tables 1.3b, 1.3c and 1.3d are therefore based on a total number of 470 practices. The participation of the midwife practices of the provinces lies between 64 and 100%, of which Friesland at 64% has the lowest participation percentage and Flevoland and Zeeland at 100% have the highest participation percentage. In rural areas the participation is the highest at 90%, followed by the small and big cities with 86% and 85% respectively. Medium-sized cities are clearly under-represented at 73%. The three teaching clinics for midwives in Amsterdam, Rotterdam and Heerlen and three hospitals where midwives are employed by the hospital do record data for the LVR-1, but are left out of account (only) in this paragraph because NIVEL does not define these hospitals as independent practices.

In Nederland waren in 1993 137 ziekenhuizen met het specialisme gynaecologie en obstetrie (bron: NVOG). In tabel 1.3e zijn deze ziekenhuizen onderverdeeld naar soort ziekenhuis.
In the Netherlands in 1993 there were 137 hospitals with a gynaecology/obstetrics department (source: NVOG). Table 1.3e classifies these hospitals per type of hospital.
Van niet-universitaire opleidingsziekenhuizen neemt 100% deel aan de LVR. De universitaire ziekenhuizen nemen op één na allemaal deel, waardoor het deelnamepercentage 88% bedraagt. De niet-opleidingsziekenhuizen vormen numeriek de grootste groep en hebben een deelnamepercentage van 82%.

In de provincies Overijssel, Flevoland en Limburg nemen alle ziekenhuizen deel aan de Landelijke Verloskunde Registratie. In Groningen, Friesland, Drenthe, Gelderland, Zeeland en Noord-Brabant doen op een of twee na alle ziekenhuizen mee aan de LVR. Met name in Zeeland met slechts vier ziekenhuizen, levert het niet deelnemen van twee ziekenhuizen al snel een laag deelnamepercentage op (50%). Bij de overige drie provincies doen vier tot vijf ziekenhuizen niet mee aan de LVR.
In the provinces of Overijssel, Flevoland and Limburg all hospitals are involved in the National Perinatal Database (LVR). In Groningen, Friesland, Drenthe, Gelderland, Zeeland and Noord Brabant all hospitals are involved in the LVR with one or two exceptions. Particularly in Zeeland which has only four hospitals, the nonparticipation of two hospitals quickly results in a low participation percentage (50%). Conclusies representativiteit Wat betreft de verloskundigenpraktijken kan worden opgemerkt dat in 1993 de solopraktijken minder in de LVR vertegenwoordigd zijn dan de duo- en groepspraktijken, resp. 71%, 91%, 91%. Of dit invloed heeft op het ‘profiel’ van de totale cliëntenpopulatie van de verloskundigen en zo ja, welke invloed, is moeilijk te zeggen. De totale deelname van de verloskundigenpraktijken aan de LVR is 82% (tabel 1.3d). Het LVR-aandeel van de geborenen bij verloskundigen komt uit op 88% (tabel 1.3a). Een verklaring voor het verschil tussen deze beide percentages is gelegen in het feit dat duo- en groepspraktijken gemiddeld een hoger aantal bevallingen voor hun rekening nemen dan de solopraktijken. Van de ziekenhuizen met een obstetrische opleiding is het percentage dat deelneemt groter dan dat van de perifere ziekenhuizen. De niet-universitaire opleidingsziekenhuizen doen zelfs allemaal mee aan de LVR. Het feit dat er meer opleidings- dan perifere ziekenhuizen deelnemen zou kunnen betekenen dat zich binnen de LVR-2 ernstiger obstetrische problematiek bevindt dan binnen de totale tweede lijn in Nederland. Alleen wanneer alle ziekenhuizen deelnemen kan hierover uitsluitsel gegeven worden.

Conclusions on representativeness With respect to independent midwife practices it can be stated that in 1993 the one-personpractices were less represented in the LVR than twin and group practices, 71%, 91% and 91% respectively. It is difficult to say whether this affects the ‘profile’ of the total patient population of the midwives and if so, which influence. The total participation of midwife practices to LVR is 82% (Table 1.3d). The LVR share of the birth of midwife practices is amounting to 88% (table 1.3a). An explanation for the differences between these percentages is due to the fact that twin and group practices are taking in average a higher number of births on their account than one-person practices. The percentage of participating hospitals with an obstetric teaching department is higher than the percentage of peripheral hospitals. The non-university teaching hospitals are even all involved in the LVR. The fact that more teaching hospitals than peripheral hospitals are taking part could mean that there are more serious obstetric problems in LVR-2 than in the whole secondary care system in the Netherlands. Only when all hospitals will participate in the LVR, a definite answer can be given to this.

1.4 Kwaliteit van gegevens
1.4 Quality of data
Voor elke registratie is naast volledigheid van de gegevens, de kwaliteit van belang. Kwaliteit is een relatief begrip. Wanneer gestreefd wordt naar een registratie van goede kwaliteit, wordt getracht een gegevensbestand te creëren dat een juist beeld kan geven van ‘de werkelijkheid’. Dit kan op een aantal manieren geschieden. In de eerste plaats kan het bestand worden gecontroleerd op aperte fouten en onmogelijke waarden van verschillende variabelen. In de LVR wordt een deel van deze fouten reeds bij het opnemen van de gegevens in het bestand gedetecteerd (zie bijlage A1.2, Werkwijze LVR).
For each registration not only the completeness of the provided data but also the quality is important. Quality, however, is a relative phrase. When aiming at good quality registrations one tries to create a data file which can give an accurate image of the ‘reality’. This can be done in a number of ways. In the first place the data file can be checked for obvious errors and impossible values of different variables. In the LVR some of these errors are already detected when the data enters the file (see Appendix A1.2, LVR working method).
Echter, ook wanneer een gegeven binnen het bereik van de geaccepteerde waarden valt, en dus niet door een controleprogramma als ‘fout’ wordt herkend, kan het onjuist zijn. Ook in de toekomst zullen dergelijke bijstellingen waarschijnlijk worden uitgevoerd.

However, even if a figure fits within the range of accepted values, and therefore not recognised as an ‘error’ by a checking program, it can still be incorrect. This type of errors are much more difficult to detect and to correct, particularly because there is mostly no indication of what the ‘reality’ is from which the recorded value is deviating. There are a number of options to avoid this type of problems as much as possible. In the first place the data record process must be as easy as possible. Furthermore, it should be clear what exactly needs to be filled in. In the LVR a number of descriptions and codes have been adapted over the last years as a result of comments made by participants. Such adjustments will probably also take place in the future.

The quality of the recorded data is affected not least by the motivation of the participants. This motivation can be increased by showing the participants as much as possible the results of their own efforts. In this chapter a number of figures from the LVR-1 and the LVR-2 relating to deliveries are presented separately. For a number of variables the similarities and differences between those two parts of the LVR registration are presented. Also certain trends are shown at some places. The chapter concludes with the presentation of a number of figures on perinatal death. In the following paragraphs has been operated with data in LVR-1 being 61,204 women and 61,214 children. In LVR-2 this concerned in 1993 91,894 women and 94,746 children. At calculating the percentages in the tables the number of ‘unknowns’ has not been included.

This paragraph presents core data on the group of women who delivered, on whom data are recorded in the LVR. The individual sub-paragraphs provide insight in information on pregnancy and labour. A specification is made on the age distribution of the mother, the parity and the relationship between both. Then attention is given to the ethnic and geographical background of the mother, the period of time between the rupture of the membranes and the birth of the child, and the length of second stage. Finally, data relating to the perineum are presented.

2.1.5 Tijdsspanne breken vliezen en geboorte kind Het aantal uren tussen het moment waarop de vliezen breken en het geboortetijdstip van het kind, wordt in figuur 2.1.5a (primiparae) en 2.1.5b (multiparae) weergegeven voor zowel de LVR-1 als de LVR-2. Hierbij dient te worden opgemerkt dat van de tweedelijns bevallingen de primaire sectio’s buiten beschouwing zijn gelaten. Dit betreft 6,6% van de tweedelijns bevallingen.
2.1.5 Period between rupture of membranes and birth of child The period between the moment that the membranes rupture and the time of birth of the child, is shown in Fig. 2.1.5a (primiparae) and 2.1.5b (multiparae) for both the LVR-1 and the LVR-2. It should be noted that of the secondary care deliveries all planned Caesarean sections are left out of account in the table. This concerns 6.6% of the secondary care deliveries.

2.1.6 Duur actief meepersen De ‘duur actief meepersen’ geeft inzicht in het aantal minuten dat verstrijkt vanaf het allereerste begin van het actief meepersen tot en met het geboortetijdstip van het kind. Dit betekent dus dat de vrouw lang niet altijd gedurende die volledige duur ook daadwerkelijk actief aan het persen is. Hierin wordt bijvoorbeeld ook de transporttijd van huis naar ziekenhuis meegerekend. In figuur 2.1.6a en figuur 2.1.6b wordt de duur actief meepersen weergegeven voor vrouwen aan wie zorg is verleend in de eerste en tweede lijn, opgesplitst naar primiparae en multiparae. Bij de tweedelijns bevallingen zijn de primaire sectio’s buiten beschouwing gelaten.
2.1.6 Length of second stage The ‘length of second stage’ gives an indication of the number of minutes that pass from the very start of active pushing up to the moment the child is born. This does therefore not always mean that the woman is really engaged with active pushing for this entire period. In this figures, for example, is included the transportation time from home to the hospital. Figures 2.1.6a and 2.1.6b show the length of second stage for women in primary or secondary care, divided into primiparae and multiparae. At the secondary care deliveries the planned Caesarean sections are left out of account.

2.1.7 Perineumletsel In welke mate er tijdens de perinatale zorgverlening perineumletsel ontstaat, wordt voor de eerste en voor de tweede lijn weergegeven in figuur 2.1.7a (primiparae) en 2.1.7b (multiparae). Wanneer er sprake is geweest van zowel een ruptuur als een episiotomie zijn de vrouwen geclassificeerd bij de eerst voorkomende categorie van de volgende lijst: ‘totaal-ruptuur’, ‘subtotaal-ruptuur’, ‘episiotomie’ en ‘ruptuur’. Bij de tweede lijn zijn alle sectio’s buiten beschouwing gelaten.

2.1.7 Perineum damage To what extent during perinatal care perineum damage occurs is shown for primary and secondary care in Fig. 2.1.7a (primiparae) and 2.1.7b (multiparae). Whenever a rupture as well as an episiotomy occurs, the women are classified according to the initial category that occurs on the following list: ‘third degree tear, including sphincter’, ‘third degree tear, sphincter not completely torn’, ‘episiotomy’, and ‘rupture’. At secondary care deliveries, all Caesarean sections are left out of account.

2.2 Geboren kinderen in de LVR
2.2 Children born in the LVR
In deze paragraaf worden kerngegevens gepresenteerd van de groep kinderen waarvan gegevens zijn vastgelegd in de LVR. In afzonderlijke subparagrafen wordt inzicht gegeven in de zwangerschapsduur, het geboortetijdstip, het geboortegewicht, het geslacht, de ligging bij geboorte en de Apgarscore. Verder wordt aandacht besteed aan het aantal meerlingen en aan de relatie tussen aangeboren afwijkingen en de leeftijd van de moeder.
This paragraph presents core data on the group of children on whom data are recorded in the LVR. The individual sub-paragraphs give indications of the gestational age, time of birth, birth weight, gender, presentation at birth and Apgar score. Furthermore, attention is given to the number of multiple gestations and to the relationship between congenital abnormalities and the mother’s age.

2.2.8 Relationship between congenital abnormalities and mother’s age In Table 2.2.8 the presence of congenital abnormalities of the child is related to the mother’s age. It should be noted in this respect that certain congenital abnormalities (e.g. heart defects) only become apparent some time after the birth of the child. For this reason it is possible that this information is not recorded in the LVR and is therefore missing in the table below.
In tabel 2.2.8 wordt de aanwezigheid van aangeboren afwijkingen bij het kind gerelateerd aan de leeftijd van de moeder. Hierbij dient te worden opgemerkt dat bepaalde aangeboren afwijkingen (bijv. hartafwijkingen) zich pas enige tijd na de geboorte manifesteren. Hierdoor is de kans aanwezig dat dit gegeven niet in de LVR wordt vastgelegd en dus in de onderstaande tabel ontbreekt.

2.3 Perinatal mortality
De cijfers over perinatale sterfte, zoals in deze publikatie gepresenteerd, dienen met voorzichtigheid te worden geïnterpreteerd waar het de totaalpercentages betreft. Ze zijn niet of slecht te vergelijken met de cijfers van het Centraal Bureau voor de Statistiek (CBS). De CBS gegevens zijn gebaseerd op doodgeborenen met een zwangerschapsduur van 28 weken en meer, bij de LVR daarentegen worden alle geboorten vastgelegd vanaf 16 weken zwangerschapsduur. M.i.v. 1 juli 1991 worden weliswaar door CBS doodgeborenen vanaf 24 weken zwangerschapsduur geregistreerd, maar bij een onderverdeling naar o.a. ‘plaats van geboorte’ wordt uitgegaan van een zwangerschapsduur van 28 weken en meer.
The figures on perinatal mortality presented in this publication should be interpreted with caution as far as the total percentages concern. They are not or only poorly comparable with the figures of Statistics Netherlands (CBS). The CBS data are based on stillbirths with a gestational age of 28 weeks onwards, in the LVR however all births with a gestational age of 16 weeks onwards are recorded. From 1 July 1991 onwards the CBS records stillbirths from a gestational age of 24th weeks onwards, but at the subdistribution of o.a. ‘place of birth’ a gestational age of 28 weeks onwards is applied.
De LVR-cijfers zoals hier gepresenteerd, kunnen tevens geen totaalcijfer geven van perinatale sterfte omdat het aandeel van de huisartsen ontbreekt. Verder is de registratie van ‘eerste-weeksterfte’ in de LVR niet volledig. Hoe later een kind in de eerste levensweek overlijdt, hoe groter de kans dat dit gegeven in de LVR niet meer wordt vastgelegd. Dit betreft veelal kinderen die zijn overgedragen aan de kinderartsen. Mede daarom is de Landelijke Neonatologie Registratie (LNR) een waardevolle en noodzakelijke aanvulling op de verloskunde registratie. De cijfers zoals hier gepresenteerd kunnen dan ook geen sluitende impressie geven van de perinatale sterfte in Nederland, geregistreerd in LVR.

The LVR figures as presented here are also incapable of providing a total figure for perinatal mortality because the GP figures are missing. Furthermore, registration of deaths in the first week are incomplete in the LVR. The later a child dies in the first week of life, the greater the chance that this will no longer be recorded in the LVR. This concerns in particular children who have been referred to paediatric care. For this reason too the National Neonatal Database [Landelijke Neonatologie Registratie - LNR] is a valuable and necessary supplement to the National Perinatal Database. The figures as presented here can therefore give no conclusive illustration of perinatal mortality in the Netherlands, as recorded in the LVR.

2.3.1 Intra-uteriene vruchtdood De sterfte in de tweede lijn is ten dele toe te schrijven aan gevallen, waarbij het kind op het moment dat de moeder onder de zorg kwam van de gynaecoloog, reeds intra-uterien was overleden. Hierbij kan sprake zijn van een overname uit de eerste lijn. Ook kan het zijn dat de vrouw rechtstreeks bij de gynaecoloog onder controle kwam. Een en ander wordt weergegeven in tabel 2.3.1.
2.3.1 Intrauterine foetal death Mortality in secondary-line care is in part attributable to cases in which the child had already died in the uterus when the mother came under the care of the gynaecologist. This can be described as a transfer from the primary line. It is also possible that the woman was attended from the start by the gynaecologist. Both are shown in Table 2.3.1

Voor de LVR-1 wordt in hoofdstuk 4, tabel 4.2.2 en tabel 4.3.2 het aantal vrouwen geregistreerd, dat door de verloskundige is overgedragen wegens intra-uteriene vruchtdood. Deze aantallen zijn niet zonder meer in relatie te brengen met de in de LVR-2 geregistreerde vrouwen die vanuit de eerste lijn zijn overgedragen met intrauteriene vruchtdood (tabel 2.3.1). Doordat niet alle LVR-1-overdrachten terecht komen bij deelnemende klinieken aan LVR-2 en niet alle LVR-2 overnamen afkomstig zijn van aan de LVR-1 deelnemende verloskundigenpraktijken, sluiten LVR-1 en LVR-2 niet volledig op elkaar aan. Gegevens van de huisartsen over dit onderwerp ontbreken totaal.

Tables 4.2.2 and 4.3.2 in Chapter 4 record for the LVR-1 the number of women referred by midwives because of intrauterine foetal death. These numbers cannot be straightforwardly correlated with the women recorded in the LVR-2 who were referred from the primary line because of intrauterine foetal death (Table 2.3.1). Since not all LVR-1 referrals end up at hospitals taking part in the LVR-2 and not all patients accepted in the LVR-2 have come from midwife practices participating in the LVR-1, the LVR-1 and LVR-2 do not match completely. There are no data at all from GPs on this subject.

3.1 General
De zorg van de verloskundige aan de cliënte kan worden onderverdeeld in drie te onderscheiden perioden: zwangerschap, baring en kraambed. In elk van deze perioden is een consult van of een overdracht aan de tweede lijn mogelijk. De verschillende perioden waarin zorg is verleend zijn aangegeven in tabel 3.1a. Dit kan zowel zorg zijn gedurende de gehele periode of een gedeelte daarvan, bv. in geval van overdracht. Elke situatie wordt hierna kort toegelicht.
Obstetric care for the patient can be divided into three different periods: pregnancy, labour and puerperium. In each of these periods consultation with or referral to secondary care is possible. The different periods during which care is provided are outlined in Table 3.1a. This can be both care during the entire period or a part thereof, e.g. in the case of a referral. Each situation is briefly described furtheron.

a - zorg tijdens zwangerschap, baring en kraambed Het grootste deel van de zorggevallen wordt gevormd door de groep vrouwen waarbij door de verloskundigen zorg is verleend vanaf zwangerschap tot en met kraambed. Hierin zijn inbegrepen de overdrachten tijdens de baring, waarna de zorg voor het kraambed weer door de verloskundige wordt overgenomen.
b - zorg tijdens zwangerschap en baring Dit is de groep vrouwen die tijdens de baring wordt overgedragen aan de tweede lijn en gedurende de kraambedperiode niet meer terugkomt in de eerste lijn.
c - zorg tijdens zwangerschap Deze groep bestaat voor het grootste gedeelte uit vrouwen die tijdens de zwangerschap worden overgedragen aan de tweede lijn en ook na de baring niet meer in de eerstelijns zorg terugkeren. Voor de rest zijn het vrouwen waarbij de zorg om andere redenen beëindigd wordt, b.v. in verband met een abortus of een verhuizing.
d - zorg tijdens de zwangerschap en het kraambed Deze groep vrouwen bestaat uit die zorggevallen die tijdens de zwangerschapsperiode worden overgedragen aan de tweede lijn en voor het kraambed weer onder de zorg van de verloskundige vallen.
e+f - zorg tijdens baring of zorg tijdens baring en kraambed Dit zijn de uitzonderingsgevallen waarbij pas op het moment van de baring de hulp van de verloskundige wordt ingeroepen.
a - care during pregnancy, labour and puerperium The major share of care cases is formed by the group of women attended by midwives from pregnancy to puerperium. This includes referrals during labour, after which care during puerperium is taken over again by the midwife. b - care during pregnancy and labour This is the group of women which is referred to secondary care during labour and do not return to primary care during the puerperium period. c - care during pregnancy This group consists for most part of women referred to secondary care during pregnancy and not returned to primary care again after labour. The rest are women for whom care is ended for other reasons, e.g. in connection with an abortion or migration. d - care during pregnancy and puerperium This group of women consists of those care cases which are referred to secondary care during pregnancy and then return to the midwife for puerperium care. e+f - care during labour or care during labour and puerperium These are exceptional cases where the midwife is called just at the time of labour. g - care during puerperium This group is known as ‘unconnected’ puerperia. Care during pregnancy and labour are provided in secondary care. These women then return to primary care for puerperium care.

3.2 Pregnancy, labour and puerperium in primary care; no referral to secondary care
Van de in totaal 81.592 vrouwen (groep a in tabel 3.1b) zijn 58.239 vrouwen vanaf de zwangerschap tot en met het kraambed in de eerste lijn gebleven. Deze vrouwen werden dus niet overgedragen aan de tweede lijn. In de navolgende subparagrafen wordt deze groep vrouwen nader in beeld gebracht. Achtereenvolgens wordt aandacht besteed aan de pariteit, de plaats van de baring en de procentuele verdeling van de plaats van de baring per provincie. Tot slot wordt de pediatrische betrokkenheid weergegeven.
Of the total of 81,592 women (group a in Table 3.1b) 58,239 women remained in primary care from pregnancy up till puerperium inclusive. These women were thus not referred to secondary care. This group of women is illustrated in greater detail in the following sub-paragraphs. Insight is given, subsequently, to parity, the place of labour and the distribution percentage of the place of labour per province. Finally paediatric intervention is shown.

4.1 Admission and discharge in primary care
Tabel 4.1 geeft zowel de in- als de uitstroom weer van de vrouwen die zich hebben aangemeld in de eerste lijn tijdens de zwangerschap, tijdens de baring of tijdens het kraambed, met daarbij een onderscheid naar pariteit. Hierbij zijn de percentages bij uitstroom vanwege ‘abortus’ en ‘beëindiging zorg’ berekend ten opzichte van de ‘start’-groep. De overige percentages zijn steeds genomen ten opzichte van het aantal vrouwen dat tijdens de betreffende periode in zorg was.
Table 4.1 shows both the admission and discharge of the women forwarded to primary care during pregnancy, labour or the puerperium, separated by parity. Here the discharge percentages because of ‘abortion’ and ‘termination of care’ are included in the ‘start’ group. The other percentages are always taken in comparison to the total number of women in care during the relevant period.
De groep ‘nieuw in zorg gekomen tijdens de baring’ heeft betrekking op die vrouwen die zich pas tijdens de baring bij de verloskundige hebben aangemeld. ‘Weer in zorg gekomen tijdens de baring’ heeft betrekking op de groep vrouwen waarbij tijdens de zwangerschap een overdracht naar de tweede lijn is geregistreerd, in combinatie met zorgverlening tijdens de baring in de eerste lijn. Deze baring wordt beschouwd als zijnde een baring in de tweede lijn. Een grafische weergave van de in- en de uitstroom wordt getoond in de figuren 4.1a en 4.1b.
The ‘newly admitted to care during labour’ group refers to those women who have only registered at the midwife during labour. ‘Returned to care during labour’ concerns the group of women where during pregnancy a referral to secondary care has been registered, in combination with treatment in primary care during labour. This labour is regarded as a secondary care labour. A graphic illustration of admission and discharge is shown in Figs. 4.1a and 4.1b. The following sub-paragraphs examine in greater detail the when and why of referrals.

Van de totale groep van 101.408 vrouwen die de zorg startte bij een verloskundige is dit 20%. 14.149 (71%) hiervan keerden terug naar de verloskundige voor de zorg tijdens het kraambed (zie tabel 4.1).
4.2 Referral during pregnancy During pregnancy 19,891 women were referred to secondary care. Of the total group of 101,408 women which started care at a midwife this amounts to 20%. 14,149 (71%) of these women returned to the midwife for care during puerperium (see Table 4.1).

4.2.2 Naar redenen van overdracht In tabel 4.2.2 worden de redenen voor overdracht tijdens de zwangerschap aangegeven, gesplitst naar primi- en multiparae. Een klein aantal keren werd hier ten onrechte ‘intra-uteriene vruchtdood bij baring’ als reden opgegeven. Dit aantal is in tabel 4.2.2 opgeteld bij ‘intra-uteriene vruchtdood in deze zwangerschap’.
4.2.2 By reasons for referral Table 4.2.2 indicates the reasons for referral during pregnancy, divided into primiparae and multiparae. ‘Intrauterine foetal death during labour’ is incorrectly given here as the reason in a small number of cases. This number is included in Table 4.2.2 under ‘Intrauterine foetal death in this pregnancy’.

4.3 Referral during labour, third stage and directly postpartum
Tijdens de baring, het nageboortetijdperk en direct post partum werden er 22.7351) vrouwen overgedragen aan de tweede lijn. Dit is 27% van de groep die in die periode in zorg was van de verloskundige. 21.396 (94%) hiervan keerden terug naar de verloskundige voor de zorg tijdens het kraambed (zie tabel 4.1).

4.3.2 Naar redenen van overdracht
4.3.2 By reasons for referral
De redenen die het meest voorkomen bij een overdracht tijdens de baring worden weergegeven in tabel 4.3.2. Het item ‘dreigende vroeggeboorte’ is een probleem van de zwangerschap. Toch kan het voorkomen dat dit gecodeerd wordt als reden van overdracht tijdens de baring. Te denken valt daarbij aan die gevallen waarin de vrouw a.h.w. overrompeld wordt door dit probleem, al in partu is en dan wordt overgedragen. Het woord dreigend is dan overbodig.
The most frequent occurring reasons for referral during labour are given in Table 4.3.2. The item ‘threatened preterm birth’ is a pregnancy problem. It could occur, however, that this is coded as a reason for referral during labour. This can be the case if the woman is caught by this problem at being already in partu and is referred then. The word ‘threatened’ is thus superfluous.

5 Verloskunde in de tweede lijn Obstetrics in secondary care Dit hoofdstuk presenteert een deel van de zorgverlening door de tweede lijn. Als eerste is te zien wanneer in de zwangerschap vrouwen onder de zorg van de gynaecoloog kwamen en hoe de baring plaatsvond. Vervolgens wordt aandacht besteed aan de kunstverlossingen voor verschillende groepen vrouwen. En tot slot wordt ingegaan op een aantal zaken rondom meerlingen. Voor deze publikatie is ‘zorgverlening in de tweede lijn’ als volgt gedefinieerd: de gynaecoloog draagt de verantwoording voor de zorg vanaf het moment van eerste onderzoek (tijdens zwangerschap of baring) tot en met de voltooiing van de baring. In 1993 werd aan 91.894 vrouwen zorg verleend door gynaecologen, die deelnamen aan de LVR. Gevallen waarbij de vrouw na de geboorte van het kind werd doorverwezen naar de tweede lijn, zijn in dit hoofdstuk buiten beschouwing gelaten.

This chapter presents a part of the care provision in secondary care. Firstly it shows when during the pregnancy women started to receive care of the obstetrician and how labour took place. Subsequently attention is paid to instrumental deliveries for various groups of women. Finally, notice is taken of a number of matters with respect to multiple gestations. For this publication ‘secondary care provision’ is defined as follows: the obstetrician has the responsibility for care from the first examination (during pregnancy or labour) onwards till the completion of labour. In 1993 91,894 women were attended by obstetricians participating in the LVR. Cases where the woman was referred to secondary care after the birth of the child are not included in this chapter.

Het grootste deel van de vrouwen in de LVR-2 komt tijdens de zwangerschap onder de zorg van de gynaecoloog. Bij 27% begint de zorgverlening in de tweede lijn tijdens de baring. Dit kunnen zowel thuis, als ook in het ziekenhuis gestarte bevallingen zijn onder leiding van verloskundige of huisarts. De overname in relatie tot de aanvang van de zorg is te zien in tabel 5.1b. The major group of the women in the LVR-2 come under the responsibility of the obstetrician during pregnancy. For 27% secondary care begins during labour. 5.2 Gestational age at start of care
Het eerste moment van zorgverlening door de gynaecoloog kan ook worden weergegeven naar de zwangerschapsweek waarin dit plaatsvindt. Dit wordt gepresenteerd in onderstaand overzicht (tabel 5.2).
The first moment of care provided by the obstetrician can also be indicated by the week of pregnancy in which this occurs. This is presented in the table below (Table 5.2).

5.5 Caesarean sections
Nederland heeft vergeleken met andere westerse landen een laag percentage sectio’s. Het landelijk percentage in Nederland was in 1993 8,5% (CBS/LMR).
Compared with other western countries, the Netherlands has a low percentage of Caesarean sections. The national percentage in the Netherlands was 8.5 % in 1993 (CBS/LMR).

In dit hoofdstuk betreft het totaalpercentage van 15,1% sectio’s alleen de tweedelijns ziekenhuisbevallingen. Hieronder vallen niet de bevallingen in ziekenhuizen die onder verantwoordelijkheid van een verloskundige of huisarts plaatsvinden. In de volgende subparagrafen worden de sectio’s afzonderlijk belicht. Bij een primaire sectio is geen vaginale bevalling geprobeerd. Bij een secundaire sectio is wel een vaginale bevalling geprobeerd. Een primaire sectio blijft een primaire sectio ook als de vrouw onverwacht in partu komt. In de tabellen 5.5.1 tot en met 5.5.6 tellen de percentages in horizontale richting per jaar op tot 100%.
In this chapter the total percentage of 15.1% Caesarean sections refers only to secondary care hospital deliveries. Not included are deliveries in hospitals that are supervised by a midwife or GP. Caesarean sections are discussed separately in the following sub-paragraphs. With a planned Caesarean section no vaginal delivery has been attempted. With a emergency Caesarean section a vaginal delivery has been attempted. A planned Caesarean section remains a planned Caesarean section even if the woman unexpectedly starts being in labour. In sub-paragraphs 5.5.1 to 5.5.6 inclusive the percentages per year in a horizontal direction total 100%.

5.6 Multiple gestations

National Perinatal Database A1.1 Opzet en ontwikkeling 1982-1993 De Landelijke Verloskunde Registratie (LVR) is tot stand gekomen op initiatief van de Nederlandse Vereniging voor Obstetrie en Gynaecologie (NVOG) en in nauwe samenwerking met de Nederlandse Organisatie voor Verloskundigen (NOV), de Inspectie Gezondheidszorg (IGZ) en SIG Zorginformatie. Het is een continu informatiesysteem op landelijke niveau, opgezet voor de registratie van gegevens over verloskundige zorg verleend door verloskundigen, huisartsen en gynaecologen. De doelstellingen van de Landelijke Verloskunde Registratie zijn: • het stimuleren van een permanente vorm van kwaliteitscontrole voor individuele zorgverleners en beroepsgroepen betrokken bij de verloskundige zorg; • het stimuleren van een continue intercollegiale toetsing voor zorgverleners in de eerste- en tweedelijns gezondheidszorg; • het verzamelen en beschikbaar stellen van verloskundige gegevens ten behoeve van beleidsbepaling en -evaluatie, wetenschappelijk onderzoek en onderwijs. In 1982 is gestart met de registratie van tweedelijns bevallingen, waaraan in 1993 door 85% van de ziekenhuizen met gynaecologie werd deelgenomen. In 1985 is begonnen met de registratie van de eerstelijns bevallingen, zowel thuis als in de kliniek en alle overige zorgverleningen (incl. overdrachten), waarbij een verloskundige is betrokken. In 1993 registreerde 82% van de verloskundigenpraktijken in de LVR eerste lijn. A1.1 Establishment and development 1982-1993 The National Perinatal Database of the Netherlands (LVR) was set up as an initiative of the Dutch Association for Obstetrics and Gynaecology (NVOG) and in close collaboration with the Organisation of Midwives in the Netherlands (NOV), the Inspectorate of Haelth Care [Inspectie Gezondheidszorg - IGZ] and SIG Health Care Information. It is an continuous information system at a national level, set up to register data on obstetric care provided by midwives, GP’s and obstetricians. The objectives of the National Perinatal Database are: • to stimulate a consistent form of quality control for individual care providers and professional groups involved in obstetric care; • to stimulate peer review between colleague care providers in primary and secondary care; • to collect and to make available obstetric information for policy making and evaluation, scientific research and education. In 1982 has been started to register secondary care deliveries, to which in 1993 85% of the hospitals with a obstetrical speciality were participating. In 1985 has been started with the registration of primary care deliveries, at home as well as in the hospital and of all other care provisions (incl. referrals), in which a midwife is involved. In 1993 82% of midwife practices registered data for LVR-1 primary care. Together, midwives and obstetricians registered 80% of the total number of deliveries in the Netherlands in 1993.

De verloskundige zorg van de huisartsen wordt in de LVR slechts op beperkte schaal vastgelegd. In deze publikatie zijn de LVR-gegevens van de huisartsen gebruikt om een schatting te maken van het aantal kinderen dat in het ziekenhuis wordt geboren onder de verantwoordelijkheid van de huisarts. Obstetric care provided by GPs is only recorded in the LVR to a limited extent. In this publication LVR data of GP’s is used to make an estimation of the number of children born in the hospital under the responsibility of a GP. See appendix A3. Fig. A1.1 shows the growth of the LVR from the first years onwards separately for primary and secondary care based on the number of births.

A1.2 Werkwijze LVR
A1.2 LVR working method
Per kalenderjaar wordt de informatie geregistreerd, waarbij de geboortedatum van het kind bepalend is. Het vastleggen van de gegevens voor de LVR vindt plaats in de praktijken van de deelnemende verloskundigen en gynaecologen. Voor de registratie wordt gebruik gemaakt van een formulier of van een geautomatiseerd systeem. De vast te leggen gegevens zijn voor een groot deel voor de eerste en de tweede lijn gelijk. Het resterende deel bevat gegevens die meer specifiek zijn voor het handelen van hetzij de verloskundigen, hetzij de gynaecologen. Sinds 1989 kunnen binnen de LVR de overdrachten tussen de eerste en de tweede lijn worden vastgelegd. Dit biedt enerzijds mogelijkheden om binnen de LVR te komen tot een goede scheiding van eerste- en tweedelijns bevallingen, anderzijds om onderzoek te verrichten naar de overdrachten van eerste naar tweede lijn. Om inzicht te krijgen in de te registreren gegevens is in bijlage A2 een overdruk opgenomen van zowel het 1e lijns als het 2e lijns formulier.
Per calendar year data are registered of which the child’s date of birth is determining. The registration of the data for the LVR takes place in the practices of the participating midwives and obstetricians. For the registration a form or an automated system is used. The data to be recorded are to a large extent similar for primary and secondary care. The remaining part contains data that are more specific for the performance of either midwives or obstetricians. Since 1989 it has been possible within the LVR to record referrals between primary and secondary care. This offers opportunities, on the one hand, to come to a separation within the LVR between primary and secondary care deliveries and, on the other hand, to research referrals from primary to the secondary care. Offprints of the forms for both primary and secondary care are included in Appendix A2 to give an idea of the type of data recorded.
De door de deelnemers aangeleverde gegevens worden bij de SIG gecontroleerd en opgeslagen in een computersysteem. De eenmaal vastgelegde gegevens kunnen meervoudig worden gebruikt. In 1993 legde 46% van de deelnemende gynaecologische praktijken gegevens vast door middel van een computersysteem. The data supplied by the participants are checked by SIG and stored in a computer system. The participants have the opportunity to correct errors which are identified by SIG. Also evident impossible situations occurring in the obstetric field and detected by SIG are submitted to the participating practices for assessment. The liability of the data is receiving a lot of attention by which the aim is to make the completion of the LVR forms to a routine matter as part of the daily activities. The once recorded data can be used for a number of purposes. In 1993 46% of the participating obstetrical practices recorded data by using a computer system. Secondary care practices can purchase a software package via SIG which has been specially developed for this purpose.

The prompt control by this software package supports an adequate recording of the data. The fact that these data, besides for the LVR, can also be used for (discharge)-letters in contacts with other healthcare providers, is providing an extra check. The data are used also for the production of personal overviews.
De inhoudelijke en beleidsmatige aspecten van de registratie worden behartigd door de Registratiecommissie LVR, waarin voor de deelnemers de NOV, de NVOG en de LHV zitting hebben. Ook de IGZ heeft een vertegenwoordiging in de Registratiecommissie. De SIG participeert in de Registratiecommissie LVR als houder en bewerker van de LVR.
The content and policy aspects of the registration are looked after by the LVR Registration Committee in which the NOV, NVOG and LHV are representing the participants. Also the IGZ has representation in the Registration Committee. SIG is participating in the Registration Committee as holder and processor of the LVR.

Uit de LVR worden vier soorten informatie vervaardigd: 1. invoer-ondersteunende informatie; 2. jaarlijkse praktijkanalyses; 3. informatie op landelijk niveau; 4. analyse- en onderzoeksbestanden.
Four types of information are produced from the LVR: 1. input-supporting information; 2. yearly practice analysis; 3. information at national level; 4. analysis and research files.
De invoer-ondersteunende informatie bestaat uit een verslag aan de betreffende deelnemer over de kwaliteit van de door de deelnemer aangeleverde gegevens op basis waarvan de deelnemer de gegevens zonodig kan corrigeren. De praktijkanalyse bevat informatie op praktijkniveau, waarbij relaties worden gelegd naar voorgaande jaren en naar landelijk niveau. De overzichten worden voor de deelnemende praktijken vervaardigd. Van de LVR informatie op landelijk niveau is deze publikatie een voorbeeld. Op verzoek kunnen er analyse- en onderzoeksbestanden worden samengesteld.
The input-supporting information consists of a report to the relevant participant on the quality of the data supplied by the participant, on the basis of which the participant can correct the data if necessary. The yearly practice analysis contains information at practice level in which relations are identified with previous years and with national level. The overviews are produced for the participating practices. This publication is an example of the LVR information at a national level. Analysis and research files can be composed on request.

A1.4 Data management and information supply
In de LVR worden gegevens over bevallingen vastgelegd. Deze gegevens worden anoniem opgenomen. De identiteit van moeder en kind is alleen bij de individuele deelnemer (verloskundige of gynaecoloog) bekend. Voor het beheer van en omgaan met de gegevens zijn de rechten en plichten van de deelnemer en van de SIG vastgelegd in een privacyreglement LVR. Het privacyreglement is opgesteld in overleg me de privacycommissie LVR. Deze commissie bestaat uit twee vertegenwoordigers namens de verloskundigen en twee vertegenwoordigers namens de gynaecologen. Verstrekking van gegevens kan slechts plaatsvinden na positief besluit van de privacycommissie en/of op basis van een schriftelijke machtiging van de betreffende berichtgever(s) aan de SIG.
The LVR records data on deliveries. These data are recorded anonymously. The identity of mother and child is only known to the individual participant (the midwife or obstetrician). The rights and obligations of the participant and SIG with respect to controlling and handling the data are laid down in the privacy regulations for LVR. These privacy regulations have been established in concertation with the LVR Privacy Committee. This committee consists of two representatives on behalf of the midwives and two representatives on behalf of the obstetricians. The supply of data can only happen after a positive decision thereof by the Privacy Committee and/or on the basis of a written authorisation from the concerning provider(s) of data to SIG.

Binnen de LVR worden eerstelijns bevallingen gedefinieerd als bevallingen die plaatsvinden onder verantwoordelijkheid van een verloskundige of huisarts; tweedelijns bevallingen onder verantwoordelijkheid van een gynaecoloog.
Within the LVR primary care deliveries are defined as deliveries that take place under the responsibility of a midwife or GP; while secondary care deliveries are those under the responsibility of an obstetrician.
In LVR 1993 bedroeg het aantal geborenen onder de verantwoordelijkheid van de eerste lijn (verloskundige) 61.214; onder de verantwoordelijkheid van de tweede lijn (gynaecoloog) 94.746.
In the LVR in 1993 the number of births were as follows 61,214 under the responsibility of primary care (midwife); 94,746 under the responsibility of secondary care (obstetrician).
Dit suggereert dat het deelnamepercentage in 1993 van de LVR t.o.v. totaal Nederland • voor de eerste lijn (verloskundigen) 67% is, nl. 61.214 / 91.278 * 100, en • voor de tweede lijn 106% is, nl. 94.746 / 89.588 * 100. De CBS cijfers zijn gebaseerd op o.a. het gegeven ‘hulp verleend tijdens de baring’ dat in de meeste gevallen door de vader bij de Burgerlijke Stand wordt opgegeven, wanneer hij aangifte doet van de geboorte van het kind. In de gevallen waar de verloskundige een groot deel van de begeleiding doet en de gynaecoloog in een later stadium te hulp wordt geroepen, kan het voorkomen dat de vader alleen de verloskundige opgeeft als degene die ‘hulp verleende tijdens de baring’. Ook daar waar een verloskundige in dienstverband is van het ziekenhuis en/of de maatschap gynaecologie, kan een ‘tweedelijns geborene’ (LVR) bij het CBS zijn geregistreerd als geborene in ziekenhuis bij verloskundige. In de LVR wordt deze geboorte echter als tweedelijns vastgelegd. Het CBS gegeven ‘geborenen in ziekenhuizen onder verantwoordelijkheid van verloskundigen’ is dus gebaseerd op andere intelkriteria dan de LVR-definitie ‘eerstelijns geborenen onder verantwoordelijkheid van verloskundigen in ziekenhuizen’. This suggets that the participation percentage in the LVR in 1993 compared to the total of the Netherlands • is 67% for the primary care (midwives), i.e. 61,214 / 91,278 * 100, and • 106% for the secondary care, i.e. 94,746 / 89,588 * 100. The number of births under supervision of medical doctors in hospitals (89,588) also contains the small number of births in hospitals under the responsibility of GPs. The CBS figures are based a.o. on the data ‘assistance provided during labour’ which in most cases is declared by the father at the Registry Office when official registering the birth of the child. In cases where the midwife performs a major part of the assistance and the obstetrician is called at a later stage, it is possible that the father only declares the midwife as the person who ‘provided assistance during labour’. In addition, where a midwife is employed by the hospital and/or the obstetric department, a ‘secondary care birth’ (LVR) can be recorded by CBS as a child born in hospital with a the intervention of a midwife. In the LVR, however, this birth is recorded as a secondary care event. The CBS data ‘births in hospitals under the responsibility of midwives’ are therefore based on different recording criteria from the LVR definition of ‘primary care births under the responsibility of midwives in hospitals’.

Om een landelijke schatting te kunnen maken van de verdeling eerstelijns en tweedelijns geborenen is gebruik gemaakt van zowel LVR als CBS gegevens. Daarbij zijn wij ervan uitgegaan dat: • het percentage thuis- versus ziekenhuisgeborenen bij verloskundigen in de LVR-1 betrouwbaar is en representatief voor alle verloskundigen in Nederland, • de plaats van de geboorte (thuis of ziekenhuis), CBS-gegeven, betrouwbaar is, • het aantal geborenen, CBS-gegeven, volledig is, • gynaecologen geen thuisbevallingen doen. Uit de LVR-1 1993 blijkt dat 64% van alle geboorten onder verantwoordelijkheid van de verloskundige thuis plaatsvindt. Volgens het CBS zijn er in 1993 44.560 kinderen thuis geboren onder de leiding van de verloskundige. Dit betekent dat het totaal aantal kinderen geboren onder verantwoordelijkheid van de verloskundige in 1993 44.560 / 64,3742 * 100 = 69.220 bedraagt. Van deze 69.220 kinderen zijn 44.560 thuis geboren en 24.6601) in het ziekenhuis. Het aandeel van verloskundigen in de LVR-1 t.o.v. totaal Nederland is dan 61.214 / 69.220 * 100 = 88,4%. Een vergelijkbare berekening is ook toegepast bij het schatten van het aantal ziekenhuisgeborenen bij huisartsen. In de LVR-1 1993 is het percentage kinderen dat thuis, onder verantwoordelijkheid van de huisarts, wordt geboren 87,0%. Volgens het CBS zijn er in 1993 15.864 kinderen thuis geboren onder de leiding van een huisarts (CBS: geneeskundige). Een ruwe schatting van het totaal aantal kinderen dat geboren is onder verantwoordelijkheid van de huisarts is 15.864 / 87,0 * 100 = 18.226. 1)

In order to provide a national estimate of the distribution of primary and secondary care births, both LVR and CBS data are used. In this we have assumed that: • the percentage in the LVR-1 of home- versus hospital births with the intervention of midwives is reliable and representative for all midwives in the Netherlands, • the place of birth (home or hospital), CBS data, is reliable, • the number of births, CBS data, is complete, • obstetricians perform no home deliveries. From the 1993 LVR-1 it appears that 64% of all births of primary care take place at home under the responsibility of a midwife. According to CBS 44,560 children were born at home in 1993 with the intervention of a midwife. This means that the total number of children born under the responsibility of a midwife in 1993 is 44,560 / 64.3742 * 100 = 69,220. Of these 69,220 children 44,560 were born at home and 24,6601) in hospitals. The group deliveries of midwives in the LVR-1 compared to the total of the Netherlands is therefore 61,214 / 69,220 * 100 = 88.4%. A comparable calculation has also been applied for estimating the number of children born at hospitals with the intervention of a GP. In 1993 LVR-1 the percentage of children born at home under the responsibility of a GP is 87.0%. According to CBS in 1993 15,864 children are born at home under the supervision of a GP (CBS: medical doctor). A rough estimation of the total of children born under the responsibility of the GP is 15,864 / 87.0 * 100=18,226.

Het aandeel van de gynaecologen is gestegen van 53% in 1989 naar 56% in 1993. Bij de verloskundigen daalde het aandeel van 36% naar 35%1). Ook het aandeel van de huisartsen daalde van 11% in 1989 naar 9%2) in 1993. Voor het eerst is een schatting gemaakt van het aantal tweedelijns bevallingen dat door de gynaecoloog en de verloskundige samen is gedaan. Het gaat hier met name om bevallingen waarbij tijdens de baring de vrouw door de verloskundige is overgedragen aan de gynacoloog.

The number for obstetricians has increased from 53% in 1989 to 56% in 1993. For midwifes the number decreased from 36% to 35%1). Also the GP number decreased from 11% in 1989 to 9%2) in 1993. For the first time an estimation was made on the number of secondary care deliveries with the intervention of obstetricians and midwifes. This particularly concerns deliveries referred by the midwife to the obstetrician during labour.
