Voorwoord Reeds een paar decennia leggen de beroepsbeoefenaren in het perinatale veld gegevens vast ten behoeve van kwaliteitsverbetering en onderzoek. In 1996 verscheen “Verloskunde in Nederland, Grote Lijnen 1989 – 1993, gebaseerd op gegevens uit twee perinatale deelregistraties: de Landelijke Verloskunde Registratie voor de Eerste Lijn (LVR-1) en de Landelijke Verloskunde Registratie voor de Tweede Lijn (LVR-2). Met deze uitgave werden zo gegevens, verzameld door verloskundigen en gynaecologen, gebundeld naar buiten gebracht. Als vervolg op deze ‘Grote Lijnen’ werd een overzicht over de jaren 1995-1999 samengesteld. Het doet ons veel plezier dat ook dit overzicht nu in druk is verschenen. Deze uitgave kon slechts tot stand komen dankzij de grote inzet van de verloskundigen en gynaecologen, die de gegevens hebben aangeleverd, en de medewerkers van Prismant, die deze gegevens bewerkt hebben. Het is een goede zaak dat nu ook in bredere kring van de resultaten van hun kennis en enthousiasme gebruik gemaakt kan worden.

Preface Data about pregnancy and labour have already been recorded for a considerable time in the Netherlands.An overview with obstetric data for the years 1989-1993 was created as the result of close cooperation with the Nederlandse Organisatie van Verloskundigen (Dutch Midwives’ Organisation, NOV) and the Nederlandse Vereniging voor Obstetrie en Gynaecologie (Dutch Association for Obstetrics and Gynaecology, NVOG). This overview was published in 1996 by SIG Zorginformatie (currently known as Prismant). Data, registered by midwives and gynaecologists were so presented jointly. You now have the sequel in front of you, a report of the years 1995-1999. We gladly present this overview in print.This edition was made possible through strong efforts of the midwives and obstetricians, who contributed their data and the members of Prismant, who were responsible for processing and analysing the data.We are pleased that the results of this collective enthusiasm and knowledge is spread more widely.

Structuur ‘Verloskunde in Nederland, Grote lijnen 1995 - 1999’ Hoofdstuk 1 geeft algemene informatie over het Nederlandse verloskundige systeem, de organisatie in eerste- en tweedelijns zorg, de representativiteit van de gegevens en de kwaliteit van de berichtgeving.
Meer specifieke informatie over de zorgverlening in de eerste lijn en in de tweede lijn is te vinden in de hoofdstukken 3 en 5. In hoofdstuk 4 wordt ingegaan op het ‘wanneer’ en ‘waarom’ van overdrachten tussen de eerste en de tweede lijn.
Hoofdstuk 2 beschrijft de overeenkomsten en verschillen van vrouwen (en hun kinderen) die zorg ontvangen in de eerste dan wel de tweede lijn.
Structure of ‘Obstetrics in the Netherlands, Trends 1995 - 1999’ Chapter 1 provides general information on the Dutch obstetric system, the organisation in primary and secondary care, the representativeness on the data and the quality of the reported data.
More specific information on primary and secondary care in Dutch obstetrics can be found in Chapters 3 and 5. Chapter 4 examines “when” and “why” patients are referred between primary and secondary care.
Chapter 2 describes the similarities and differences between women (and their children) who receive care by primary or secondary care professionals.

1 Verantwoordelijkheden 1.1 Het Nederlandse verloskundige systeem Verloskundige zorg in Nederland is gebaseerd op de uitgangspunten dat zwangerschap, bevalling en kraamperiode fysiologische processen zijn en dat bevalling en kraamperiode thuis kunnen plaatsvinden. De organisatie van de verloskunde is gebaseerd op het principe van risicoselectie, met een duidelijke taakverdeling en onderlinge samenwerking tussen de eerstelijns en de tweedelijns verloskundige zorgverlener. Laagrisico vrouwen krijgen eerstelijns zorg van (vrijgevestigde) verloskundigen en huisartsen.Vrouwen met een verhoogd risico ontvangen tweedelijns zorg door gynaecologen. Wanneer tijdens de zwangerschap, baring of in het kraambed verloskundige risico’s worden vastgesteld,
consulteert de eerstelijns verloskundig zorgverlener de specialist in de tweede lijn. Om de risicoselectie en verwijzing optimaal te laten verlopen, hebben de betrokken beroepsgroepen in het Verloskundig Vademecum richtlijnen voor overleg en overdracht geformuleerd.Als de indicatie voor verwijzing niet meer aan de orde is kan de tweedelijns zorgverlener de vrouw terugverwijzen naar de eerste lijn.1 Vrouwen met een laag risico hebben de mogelijkheid te kiezen tussen een thuisbevalling en een poliklinische bevalling, met begeleiding van hun eerstelijns zorgverlener. Vrouwen met een verhoogd risico bevallen onder begeleiding van een tweedelijns zorgverlener in een ziekenhuis.
1 Responsibility 1.1 Dutch obstetric system Obstetric care in the Netherlands is based on the concept that pregnancy, delivery and puerperium are physiological events and that both delivery and puerperium can take place at home. The organisation of obstetric care is based on the concept of risk selection, with a clear distribution of tasks and close mutual co-operation between professionals in primary and secondary obstetric. Lowrisk women receive primary care provided by (independent) midwives or general practitioners.Women with a high-risk obstetric profile receive secondary care, provided by obstetricians.
If obstetric risks are identified during pregnancy, delivery or puerperium, the primary care professional consults the professional in secondary care.To optimalize the risk selection and referral the professional organisations involved have formulated guidelines for consultation and referra, known as the ‘Obstetric Manual’. If the condition which prompted referral is no longer a risk factor, the secondary level care-provider can refer the woman back to primary care.2 Low-risk women may choose to give birth at home or in hospital with their primary care provider.Women evaluated as having an increased obstetrical risk deliver in hospital with a secondary care professional.

1.2 Registratie van eerste- en tweede-lijns verloskundige zorgverlening In deze publicatie worden vooral gegevens uit de Landelijke Verloskundige registratie (LVR) uit het jaar 1999 gepresenteerd, soms over de jaren van 1995 tot en met 1999. In navolging van de jaarboeken van 1988 3, 19914 en 19935 worden de eerste- en tweedelijns gegevens samen gepresenteerd. Van de zorg die de verloskundige aan de cliënte biedt, legt zij gegevens vast in de LVR-1. Zij registreert ook wanneer zij slechts een deel van de zorg tijdens zwangerschap, de baring en het kraambed verleent. Bij een overdracht tijdens de zwangerschap registreert zij de zorg die ze aan de vrouw verleend heeft tot en met het
moment van overdracht. Bij een overdracht tijdens de baring registreert zij eveneens gegevens van het kind. In de tweede lijn worden de gegevens dan in de LVR-2 vastgelegd na de geboorte van het kind. De vrouwen die vóór, tijdens of direct na de baring verwezen worden naar de tweede lijn, kunnen twee keer in de LVR voorkomen (zowel in de LVR-1 als in de LVR-2). Om dubbeltellingen en vertekeningen in de landelijke cijfers te voorkomen is het van belang een correct onderscheid aan te brengen tussen eerste- en tweedelijns bevallingen (deze publicatie) en/of een accurate koppelingsmethodiek toe te passen.6
1.2 Registration of primary and secondary obstetric care This publication presents data from the National Perinatal Database of the Netherlands (LVR) from 1999 and, in a number of cases, from 1995 to 1999 inclusive. As in the yearbooks of 1988, 1991 and 1993, primary and secondary care data are combined. Midwives record data relating to care provided to her clients in the LVR-1. She records even if she only provided care during part of the pregnancy, delivery or puerperium. In case of a referral during pregnancy she registers data of the provided care up till the moment of referral. In case of referral during delivery she also registers data on the newborn child.

In secondary care data for the LVR-2 is included after the birth of the child. Women referred to secondary care during their pregnancy, delivery or directly postpartum can therefore be included twice in The Netherlands Perinatal Registry (that is, in LVR-1 as well as in LVR-2). To avoid double counting of a ‘case’ and biases in the national data it is important to make a correct distinction between primary and secondary care deliveries (this publication) and/or to make use of adequate linking of registries.6
Huisartsen leggen hun verloskundige zorg slechts op beperkte schaal vast in de LVR. Daarom blijft de door hen verstrekte verloskundige zorg in deze publicatie buiten beschouwing.

Eerstelijns bevallingen onder verantwoordelijkheid van de eerstelijns verloskundige zijn als volgt gedefinieerd: • de eerstelijns verloskundige was tijdens de gehele baring verantwoordelijk voor de zorg; • de eerstelijns verloskundige heeft de zorg op geen enkel moment vóór of tijdens de geboorte van het kind overgedragen aan de tweede lijn.
Bevallingen door huisartsen worden in deze publicatie buiten beschouwing gelaten.
De tweedelijns bevallingen onder de verantwoordelijkheid van de gynaecoloog zijn als volgt gedefinieerd: • de gynaecoloog was verantwoordelijk voor de zorg tijdens een (gedeelte van) de baring de gynaecoloog heeft de begeleiding voor de zorg gestart vóór of tijdens de geboorte van het kind.
General Practitioners (GPs) record the obstetric care they provide to a limited extent only.Therefore obstetric care which is provided by GP’s is not included in this this publication.

1.3 Representativiteit LVR De data voor deze publicatie zijn vastgelegd door verloskundigen (LVR-1) en gynaecologen (LVR-2). In deze paragraaf wordt de deelnamegraad aan LVR-1 en LVR-2 beschreven, evenals de representativiteit van de deelnemers. Eerst worden de in de LVR geregistreerde geboortes getoond naar eerste- of tweedelijns verantwoordelijkheid, zoals beschreven in paragraaf 1.2. Dit wordt vergeleken met het totaal aantal levend- en doodgeboren kinderen met een zwangerschapsduur van 24 weken of meer (tabel 1.3a).
Daarna volgt de deelname van de verloskundige praktijken naar provincie, naar urbanisatiegraad en naar soort praktijk (tabel 1.3b/d).Vervolgens wordt de deelname van de maatschappen obstetrie-/gynaecologie getoond naar soort ziekenhuis en naar provincie (tabel 1.3e/f). In alle tabellen wordt de deelname afgezet tegen het totaal aantal praktijken of maatschappen in de desbetreffende categorie.
1.3 Representativeness of the LVR The data for this publication are recorded bij midwives (LVR-1) and gynaecologists (LVR-2).This paragraph describes the degree of participation in the LVR-1 and LVR-2 as well as the representativeness of the participants. First the number of births which are registered in the LVR will be shown according to the primary or secondary professional, as described in chapter 1.2.This will be compared to the total number of live- and stillbirths after a gestational age of 24 weeks or more.

This is followed by the participation of independent midwifery practices per province, per degree of urbanisation and per characteristics of the practice (table 1.3b/d).The participation of partnerships obstetrics / gynaecology is shown per characteristics of hospital and per province (1.3e/f). Participation will be compared to the total number of practices or professional partnerships in the respective category.
De LVR-2, waarin de verleende zorg wordt geregistreerd, neemt alle doodgeborenen op die geboren worden na een zwangerschapsduur van 16 weken en meer.
De aantallen in tabel 1.3a betreffen levend- en doodgeborenen. Bij tabellen over doodgeborenen worden verschillende definities gehanteerd: het CBS registreert doodgeborenen die geboren worden na een zwangerschapsduur van 24 weken en meer. Deze ondergrens is een wettelijke grens.
Bij iedere tabel zal duidelijk aangegeven worden welke ondergrens is toegepast.

The numbers in Table 1.3a refer to live- and stillbirths. For stillbirths different criteria are applied: the Statistics Netherlands (CBS), registers all stillbirths after a gestational age of 24 weeks or more.This lower limit is a legal criterium.
Calculation taking account of LVR/CBS
The LVR-2 registry, which is a registry of the care provided, includes all stillbirths that occur after a gestational age of 16 weeks or more. Every table will clarify which lower limit is used.

Aantal geborenen geregistreerd in LVR
In figuur 1.3a is voor de jaren 1995 tot en met 1999 aangegeven hoe groot het percentage geborenen is dat verloskundigen en gynaecologen hebben geregistreerd in respectievelijk de LVR-1 en de LVR-2. Werd in 1995 ongeveer 80% van alle pasgeborenen geregistreerd in de LVR, in 1999 was dat percentage gestegen naar 90%.
Om een schatting te kunnen maken van de verdeling eerstelijns en tweedelijns geborenen voor Nederland, is gebruik gemaakt van zowel LVR als CBS gegevens. Voor een gedetailleerde omschrijving van de gebruikte methode en berekeningswijze zie bijlage A3

Numbers of births registered in the LVR In order to be able to make an estimation of the distribution of primary and secondary care births for the Netherlands, LVR data as well as CBS data are used. Please see Appendix A3 for a detailed description of the applied method and the calculation formula

Fig. 1.3a indicates the rate of births which midwives and obstetricians have registered in the LVR-1 and LVR-2 for the period 1995 to 1999 inclusive. While in 1995 about 80% of all births were registered in LVR, the rate of registered births was increased to 90% in 1999.
De LVR heeft in Groningen met 96% het hoogste deelnamepercentage; de provincies Drenthe, Overijssel, Gelderland, Utrecht, Noord-Brabant en Noord-Holland hebben een deelnamepercentage van 90 - 95%; de provincies Flevoland, Limburg en Zuid-Holland 80 - 95%; en Friesland heeft een deelnamepercentage van 79%. Zeeland heeft met 53% het laagste deelnamepercentage. Daar het aantal geborenen in deze provincie vrij klein is, is de invloed van dit deelnamepercentage op het totale deelnamepercentage gering.
In 1995 werd 88% van de pasgeborenen geboren onder verantwoordelijkheid van de tweede lijn, geregistreerd in de LVR-2. In 1999 was dat percentage gestegen naar 97%. In figuur 1.3b is per provincie gekeken naar het percentage geborenen dat in de LVR is geregistreerd en het totaal aantal geborenen. Er is hierbij geen onderscheid gemaakt in eerste- en tweedelijns geborenen.

In 1995 88% of all births under responsibility of obstetricians in secondary care were registered in the LVR-2. In 1999 this share of registered births was increased to 97%. Fig. 1.3b shows the rate of births registered in the LVR and the total number of births per province. No distinction is made between primary and secondary care births.
The LVR’s highest participation occurs in the province of Groningen with 96%.The provinces of Drenthe, Overijssel, Gelderland, Utrecht, North Brabant and North Holland have a participation rate of 90 - 95%. Finally, the provinces Flevoland, Limburg and SouthHolland participate for 80 - 95%, and Friesland has a participation rate of 79%.At 53%, Zeeland has the lowest rate of all. Participation by midwives in the LVR-1 The practices of independent midwives can be characterized per domicile (province and level of urbanisation) and number of midwives working in the practice. Based on characteristics of midwifery practices that participate in the LVR-1 it is possible to analyse whether the participating practices are representative for all midwifery practices.
Het totaal aantal praktijken in de onderstaande tabellen is kleiner dan het door NIVEL geregistreerde aantal van 458 per 01.01.99.7 Aan de praktijkcodes van aan de LVR deelnemende praktijken is te zien dat een aantal praktijken in 1999 hebben samengewerkt, maar zich ondanks de vergaande samenwerking bij het NIVEL als aparte praktijken hebben laten meetellen. In tabel 1.3b, tabel 1.3c en tabel 1.3d is daarom uitgegaan van een totaal aantal van 458 praktijken.
Verder komen in 1999 13 van 422 praktijken met een specifieke LVR- praktijkcode niet als aparte praktijk voor in de registratie van het NIVEL. Hierdoor is het aantal LVR praktijken in tabel 1.3b in twee provincies hoger dan het aantal NIVEL praktijken.

Deelname van gynaecologen aan de LVR-2 In Nederland waren in 1999 118 ziekenhuizen met het specialisme gynaecologie en obstetrie. In tabel 1.3e zijn deze ziekenhuizen onderverdeeld naar soort ziekenhuis. In 1999 nemen zowel maatschappen obstetrie-/gynaecologie in universitaire als in niet-universitaire opleidingsziekenhuizen voor 100% deel aan de LVR-2.
Het merendeel van de maatschappen obstetrie-/gynaecologie is werkzaam in niet-opleidings-ziekenhuizen. 95% van hen neemt deel aan de LVR-2. In de provincies Overijssel, Flevoland, Limburg, NoordBrabant, Gelderland, Noord- en Zuid-Holland en Friesland nemen alle ziekenhuizen deel aan de Landelijke Verloskunde Registratie. Most of the partnerships of obstetrics are based in nonteaching hospitals. 95% of them participates in de LVR-2. All hospitals in the provinces of Overijssel, Flevoland, North Brabant, Gelderland, North Holland, South Holland and Friesland are involved in the National Perinatal Database (LVR).With one or two exceptions, all hospitals in Groningen, Drenthe, Utrecht and Zeeland are involved in the LVR.
Conclusies representativiteit In 1999 neemt 92% van de verloskundige praktijken deel aan de LVR-1 (tabel 1.3d). Solopraktijken nemen minder deel aan de LVR dan de duo- en groepspraktijken (resp. 82%, 93%, 99%). Óf en zo ja, welke invloed dit heeft op de representativiteit van de in de LVR-1 geregistreerde cliëntenpopulatie, is moeilijk te zeggen.
Het aantal levend- en doodgeborenen dat in de LVR-1 is geregistreerd t.o.v. het totaal aantal geborenen in de eerste lijn komt uit op 79% (tabel 1.3a). Dit is te verklaren door het feit dat in tabel 1.3a alle pasgeborenen zijn opgenomen, inclusief degenen bij wie de huisarts de bevalling heeft begeleid. Van de ziekenhuizen met een obstetrische opleiding neemt 100% deel aan de LVR-2 registratie. Dit percentage ligt hoger dan bij de niet-opleidingsziekenhuizen (95%). Het feit dat er meer opleidings- dan niet-opleidingsziekenhuizen deelnemen kan gevolgen hebben voor de representativiteit. Mogelijk is de ernstiger obstetrische problematiek over-vertegenwoordigd in de geregistreerde problematiek.Alleen wanneer alle ziekenhuizen deelnemen aan de LVR-2 kan hierover uitsluitsel gegeven worden.
Conclusions on representativeness In 1999 92% of all midwifery practices participate in the LVR-1 (table 1.3d). Solo practices show less participation than the twin and group practices, (82%, 93% and 99%, respectively). It is hard to say whether this affects the representativeness of the patient population which is registered in the LVR-1, and if so, what the impact is.
The number of live and still births registered in the LVR compared to the total births in primary care amounts to 79% (table 1.3a).This can be explained by the fact that this table 1.3a includes all births in primary care, including the ones by GPs. 100% of the teaching hospitals participate in the LVR-2 registry.Their participation rate is higher than the one of non-teaching hospitals participating in the registry (95%). This might influence the representativeness of the registry. Maybe the serious obstetric problems are overrepresented in the registry.Whether this is the case or not can only be answered when all hospitals are participating in the LVR-2.

1.4 Kwaliteit van gegevens Voor elke registratie is -naast volledigheid- de kwaliteit van de gegevens van belang. Voor een registratie van goede kwaliteit is een gegevensbestand nodig dat een juist beeld kan geven van ‘de werkelijkheid’. Dit kan op een aantal manieren. In de eerste plaats kan het bestand worden gecontroleerd op aperte fouten en onwaarschijnlijkheden. In de LVR wordt een deel van deze fouten reeds bij het invoeren en/of opnemen van de gegevens gedetecteerd (zie bijlage A1.2, Werkwijze LVR).
Echter, ook wanneer een gegeven binnen het bereik van de geaccepteerde waarden valt, en niet door een controleprogramma als ‘fout’ wordt herkend, kan het gegeven niet overeenkomen met de werkelijkheid. Dit soort onjuistheden is veel moeilijker te ontdekken en te corrigeren: meestal is onbekend wat ‘de werkelijkheid’ is waar het geregistreerde gegeven van afwijkt. Om dit laatste probleem zoveel mogelijk te vermijden, moet het invulproces zo gemakkelijk mogelijk zijn. Verder moet er geen twijfel kunnen ontstaan over wat er gevraagd wordt en wat moet worden ingevuld. In de LVR is in de loop der jaren aan de hand van commentaren van de deelnemers, een aantal omschrijvingen en coderingen om deze reden aangepast. Dit zal ook in de toekomst nodig blijven.
1.4 Quality of data For each registry, not only the completeness of the provided data but also the quality of the data is important. For a registry of good quality, one needs a data file which can give an accurate image of the “reality”.This can be done in a number of ways. In the first place, the data file can be checked for obvious errors and improbabilities. In the LVR, some of these errors are already detected as soon as the data enter the file (see Appendix A1.2, LVR working method).

However, even if a value fits within the range of accepted values, and is not recognised as an “error” by a checking program, the data can still be incorrect.These types of errors are harder to detect and correct: usually there is no indication of the “real situation” from which the recorded value is deviating.To avoid this type of problems the data recording process must be as easy as possible. Furthermore, there shouldn’t be any doubt at all what is asked for and what needs to be filled in.A number of descriptions and codes in the LVR has been adapted over the last years as a result of comments made by participants.These kind of adjustments will be needed in the future as well as in the past
Niet in de laatste plaats wordt de kwaliteit van het ingevulde beïnvloed door de motivatie van de deelnemer. Deze kan worden verhoogd door de deelnemer te laten zien wat zijn/haar inspanningen oplevert. Daarom ontvangen deelnemers jaarlijks praktijkoverzichten en worden de ruwe data verwerkt in voor de populatie gecorrigeerde data (VOKS, Verloskundige Onderlinge Kwaliteitsspiegeling). De gegevens uit de LVR zijn verder beschikbaar voor het uitvoeren van onderzoek, wat door de betrokken beroepsorganisaties wordt gestimuleerd.
Last but not least, the quality of the recorded data is affected by the motivation of the participants.This motivation can be increased by showing the participants the results of their own efforts.Therefore participants receive an annual report on the data of their own practice (rough data) and data which is adjusted for population differences (Obstetric Peer Review, the VOKS). The data in the LVR are available for further research, which is being stimulated bij all professional organisations involved.

2 Primary and secondary care deliveries: similarities and differences This chapter gives an overview of the similarities and differences between women and children registered in the LVR-1 and the LVR-2.At some places some trends are shown. The chapter concludes with the presentation of a number of figures on perinatal death.
In the following paragraphs, the data in LVR-1 (1999) include 66.331 women and 66.337 live and stillborn children. In the 1999 LVR-2 110.130 women and 113.870 live and stillborn children are included. The number of ‘unknowns’ has not been included in the calculation of the percentages in the tables.

2.1.5 Tijdsspanne breken vliezen en geboorte kind
In de LVR een aantal etnische en geografische kenmerken van de betrokken vrouwen geregistreerd.Tabel 2.1.4 geeft de verdeling weer van vrouwen naar etniciteit, zoals geregistreerd in de LVR.
Het aantal uren tussen het moment waarop de vliezen breken en het geboortetijdstip van het kind, wordt in figuur 2.1.5a (primiparae) en 2.1.5b (multiparae) weergegeven voor zowel de LVR-1 als de LVR-2. In beide figuren zijn de geplande sectio’s buiten beschouwing gelaten. Geplande sectio’s vormen 7,5% van de tweedelijns bevallingen.

In the LVR some ethnic and geographical data on the included women are registered.Table 2.1.4 shows the distribution of women to ethnicity, as recorded in the LVR.
The period between the moment that the membranes rupture and the time of birth of the child is shown in fig. 2.1.5a (primiparae) and 2.1.5b (multiparae) for both the LVR-1 and the LVR-2. Of deliveries the planned Caesarean sections are excluded from both figures. Planned caesarean sections add up to 7.5% of the secondary care deliveries.

2.1.6 Duur actief meepersen port meegerekend bij de totale duur van de uitdrijving, ook al zijn ze op dat moment niet actief aan het persen. Ook voor vrouwen bij wie tijdens de uitdrijving wordt besloten tot een sectio kan een deel van de geregistreerde ‘duur actief meepersen’ bestaan uit voorbereiding en transport in plaats van actief persen. In beide figuren zijn de primaire sectio’s buiten beschouwing gelaten.
De ‘duur actief meepersen’ geeft inzicht in het aantal minuten dat verstrijkt vanaf het allereerste moment van actief meepersen tot en met het geboortetijdstip van het kind. In figuur 2.1.6a en figuur 2.1.6b wordt de duur actief meepersen weergegeven voor primiparae en multiparae opgesplitst naar de eerste en tweede lijn. Voor vrouwen die tijdens de uitdrijving van huis naar ziekenhuis worden verwezen wordt de duur van het trans-

2.1.6 Length of second stage The “length of second stage” gives an indication of the number of minutes that pass from the very start of active pushing up to the moment that the child is born. Figs. 2.1.6a and 2.1.6b show the length of second stage for primiparae and multiparae, divided into in primary or secondary care. Transportation time is included for those women who are referred from a home delivery to the hospital during
the second stage, even though they are not engaged in active pushing during transportation. For those women who are indicated for a Caesarean section during second stage, part of their ‘length of second stage’ will consist of induction and transportation time instead of active pushing. All planned Caesarean sections in secondary care deliveries are excluded from both figures.

2.2 Geboren kinderen in de LVR In deze paragraaf worden kerngegevens gepresenteerd van de groep levend- en doodgeboren kinderen geboren na een zwangerschapsduur van 16 weken, waarvan gegevens zijn vastgelegd in de LVR. In afzonderlijke subparagrafen worden de volgende kenmerken beschreven: de zwangerschapsduur, het geboortetijdstip, het geboortegewicht, het geslacht, de ligging bij geboorte en de Apgarscore.Verder wordt aandacht besteed aan het aantal meerlingen en aan de relatie tussen aangeboren afwijkingen en de leeftijd van de moeder.
Alle figuren en tabellen worden onderverdeeld naar geboren onder verantwoordelijkheid van de eerste of de tweede lijn.
2.2 Children born in LVR This paragraph presents key figures on the group of live and stillborn children born after a gestational age of 16 weeks or more whose data are included in the LVR. In the subsequent subparagraphs all births are characterized according to their gestational age, time of birth, birth weight, gender, presentation at birth and Apgar score. Other subparagraphs focus on the number of multiple gestations and the relationship between

2.2.8 Relationship between congenital abnormalities and mother’s age The prevalence of children born with congenital abnormalities is related to the mother’s age in table 2.2.8. It should be noted in this respect that certain congenital abnormalities (for example, heart defects) only become

apparent some time after the birth of the child. Therefore it is most likely that not all congenital abnormalities are included in the LVR. 1)

2.3 Perinatale sterfte De cijfers over perinatale sterfte zoals in deze publikatie gepresenteerd, dienen met voorzichtigheid te worden geïnterpreteerd. Ze zijn niet of slecht te vergelijken met de cijfers van het Centraal Bureau voor de Statistiek (CBS). De CBS gegevens over perinatale sterfte zijn gebaseerd op doodgeborenen met een zwangerschapsduur van 24 weken en meer. In de LVR worden echter alle geboorten vastgelegd vanaf 16 weken zwangerschapsduur. De LVR-cijfers zoals hier gepresenteerd, kunnen tevens geen totaalcijfer geven van perinatale sterfte omdat het aandeel van de huisarts ontbreekt.
Ten derde is de registratie van late perinatale sterfte, aan het einde van de eerste week, niet volledig in de gecombineerde LVR-1 en LVR-2. Hoe later een kind in de eerste levensweek overlijdt, hoe groter de kans dat dit gegeven in de LVR niet meer wordt vastgelegd. Dit betreft veelal kinderen die zijn overgedragen aan kinderartsen. Mede daarom is de Landelijke Neonatologie Registratie (LNR) een noodzakelijke en essentiële aanvulling op de verloskunde registratie.
2.3 Perinatal mortality The figures on perinatal mortality presented in this publication should be interpreted with caution.They are either not at all, or only poorly, comparable with the figures of the Central Bureau of Statistics [Centraal Bureau voor de Statistiek - CBS]. The CBS data of perinatal mortality are based on stillbirths with a gestational age of 24 or more. However, in the LVR, all births with a gestational age of 16 weeks or more are included. Because the data on care by GP’s are missing, the LVR figures as presented here are not covering all data on perinatal mortality.

Thirdly, registration of late perinatal death, at the end of the first week after birth, is incomplete in the LVR.The later a child dies in the first week of life, the more probable that this death will not be recorded in the combined LVR-1 and LVR-2.This concerns, in particular, children who have been referred to paediatric care. Therefore the National Neonatal Database [Landelijke Neonatologie Registratie - LNR] is a necessary and essential supplement to the National Perinatal Database.
In de navolgende subparagrafen wordt nader ingegaan op de perinatale sterfte.Allereerst wordt inzicht gegeven in het percentage intra-uteriene vruchtdood bij eerste onderzoek in de tweede lijn. Daarna wordt de perinatale sterfte achtereenvolgens gerelateerd aan de zwangerschapsduur, het geboortegewicht, meerlingen en de leeftijd van de moeder.Ten slotte wordt de perinatale sterfte gepresenteerd onder kinderen die naar de kinderarts zijn verwezen.
De cijfers zoals hier gepresenteerd kunnen geen sluitende indruk geven van de in de LVR geregistreerde perinatale sterfte en zeker niet van de perinatale sterfte in Nederland. Rekening houdend met de beperkingen zoals hierboven beschreven wordt in tabel 2.3 weergegeven hoe vaak in 1999 perinatale sterfte is geregistreerd in de LVR-1 en de LVR-2.

The figures as presented here can give no conclusive impression of perinatal mortality as recorded in the LVR, and even less on the perinatal mortality in the Netherlands. Given the constraints explained above table 2.3 shows the perinatal mortality as registered in 1999 in the LVR-1 and LVR-2.
The next sub-paragraphs deal with perinatal mortality in greater detail. First of all, the rate of intra-uterine foetal mortality at the first examination in secondary care is presented. Perinatal mortality is subsequently related to gestational age, birth weight, multiple gestations and the mother’s age. Finally, the perinatal mortality of children who are referred to a paediatrician is presented.

2.3.1 Intra-uteriene vruchtdood bij eerste onderzoek in de tweede lijn De sterfte in de tweede lijn is ten dele toe te schrijven aan gevallen, waarbij het kind op het moment dat de vrouw onder de zorg kwam van de gynaecoloog, reeds intra-uterien was overleden. Hierbij kan sprake zijn van
een verwijzing vanuit de eerste lijn. Ook kan het zijn dat de vrouw rechtstreeks bij de gynaecoloog onder controle kwam. Een en ander wordt weergegeven in tabel 2.3.1.

2.3.1 Intrauterine foetal death at the first examination in secondary care Mortality in secondary care is, in part, attributable to cases in which the child had already died in uteri when the mother came under the care of the gynaecologist. This can be the case after a referral by primary care. It is

also possible that the woman was attended from the start by the gynaecologist. Both possibilities are shown in Table 2.3.1.
Voor de LVR-1 wordt in hoofdstuk 4, tabel 4.2.2 en tabel 4.3.2 het aantal vrouwen geregistreerd dat door de verloskundige is overgedragen wegens intra-uteriene vruchtdood. Deze aantallen komen niet zondermeer overeen met het aantal in de LVR-2 geregistreerde vrouwen die vanuit de eerste lijn zijn overgedragen met intra-uteriene vruchtdood (tabel 2.3.1). Omdat niet alle LVR-1-overdrachten terecht komen bij deelnemende klinieken aan LVR-2 en niet alle LVR-2 overnamen afkomstig zijn van aan de
LVR-1 deelnemende verloskundigenpraktijken, sluiten LVR-1 en LVR-2 niet volledig op elkaar aan. Daarnaast ontbreken de gegevens over de verwijzing van de huisartsen vanwege intra-uteriene vruchtdood.
Tables 4.2.2 and 4.3.2 in Chapter 4 record for the LVR-1 the number of women referred by midwives because of intrauterine foetal death. These numbers are not consistent with the number of women recorded in the LVR-2 who were referred from primary care due to intrauterine foetal death (table 2.3.1). Since not all LVR-1 referrals end up at hospitals taking part in the LVR-2, and not all patients accepted in the LVR-2 have come from midwife practices participating in the LVR-1, the LVR-1 and LVR-2 are not completely comparable. Last, data on the referral by GPs because of intrauterine foetal death are not included.

3 Verloskunde in de eerste lijn: zorg door de verloskundige 3.1 Algemeen De zorg van de verloskundige kan worden onderverdeeld in drie te onderscheiden perioden: zwangerschap, baring en kraambed. In elk van deze perioden is een consult van of een overdracht aan de tweede lijn mogelijk. De verschillende perioden waarin verloskundigen zorg hebben verleend zijn aangegeven in tabel 3.1a. In deze tabel is te zien aan hoeveel vrouwen verloskundigen zorg hebben verleend. Dit kan zowel zorg zijn gedurende de
gehele periode of een gedeelte van de desbetreffende periode (tot aan een overdracht naar de tweede lijn). Elke situatie wordt hierna kort toegelicht.
3 Obstetrics in primary care: by a midwife 3.1 General Obstetric care by a midwife can be divided into three different periods: pregnancy, labour and puerperium. In each of these periods, consultation with or referral to secondary care is possible. The different periods during which care is provided are outlined in table 3.1a.This table shows to how many women midwives provided care.Their care can involve care during the entire period as well as care during a
part of a certain period (i.e. untill the moment of referral to secondary care). Each situation is briefly described below.

3.2 Zorg voor zwangerschap, baring en kraambed door de verloskundige in de eerste lijn zonder overdracht naar de tweede lijn Van de in totaal 94.052 vrouwen (groep a in tabel 3.1b) zijn 60.967 vrouwen in de eerste lijn gebleven. Zij zijn op geen enkel moment tijdens zwangerschap, baring of kraambed overgedragen aan de tweede lijn. In de navolgende subparagrafen wordt deze groep vrouwen nader in beeld gebracht.Achtereenvolgens wordt aandacht besteed aan de pariteit, de plaats van de baring en de procentuele verdeling van de plaats van de baring per provincie.Tot slot wordt de pediatrische betrokkenheid weergegeven.
3.2 Pregnancy, labour and puerperium in primary midwifery care without a referral to secondary care From the total of 94,052 women (group a in Table 3.1b), 60,967 women remained in primary care.They weren’t for any moment during pregnancy, labour or puerperium, referred to secondary care. In the following sub-paragraphs this group of women is described in greater detail. 4 Overdracht; wanneer en waarom 4.1 In- en uitstroom bij eerste lijn Tabel 4.1 geeft zowel de in- als de uitstroom weer van de vrouwen die zich hebben aangemeld bij de verloskundige in de eerste lijn tijdens de zwangerschap, tijdens de baring als tijdens het kraambed, met daarbij een onderscheid naar pariteit. Hierbij zijn de percentages voor uitstroom vanwege ‘abortus’ en ‘beëindiging zorg, overige redenen’ berekend ten opzichte van de ‘start’-groep.
De overige percentages zijn steeds genomen ten opzichte van het aantal vrouwen dat tijdens de betreffende periode in zorg was.
4 Referral; when and why 4.1 Admission and discharge in primary care Table 4.1 shows both the admission and discharge of women in midwifery primary care during pregnancy, labour or the puerperium, subdivided by parity. The rate of discharge because of “abortion” and “termination of care, other reasons” are a proportion of the “start” group.
All other percentages are always taken as a proportion of the total number of women in care during the relevant period.

De groep ‘nieuw in zorg gekomen tijdens de baring’ heeft betrekking op die vrouwen, die zich pas tijdens de baring bij de verloskundige hebben aangemeld. ‘Weer in zorg gekomen tijdens de baring’ heeft betrekking op de groep vrouwen waarbij een verwijzing naar de tweede lijn tijdens de zwangerschap is geregistreerd in de LVR, maar in combinatie met zorgverlening tijdens de baring in de eerste lijn. Deze baring wordt beschouwd als zijnde een baring in de tweede lijn.
Een grafische weergave van de in- en de uitstroom wordt getoond in de figuren 4.1a en 4.1b. In de navolgende subparagrafen wordt nader ingegaan op het moment en de reden van de overdrachten.

4.2 Overdracht tijdens zwangerschap In 1999 droegen verloskundigen in totaal 30.435 vrouwen tijdens de zwangerschap over aan de tweede lijn. Van de totale groep van 122.913 vrouwen die de zorg startte bij een verloskundige is dit 25%. 25.285 (83%) hiervan keerde terug naar de verloskundige voor de zorg tijdens het kraambed (zie tabel 4.1).
Tabel 4.2.1 laat van de vrouwen die tijdens de zwangerschap werden overgedragen een onderverdeling zien naar pariteit.Tabel 4.2.2 geeft de redenen van overdracht weer.
4.2 Referral during pregnancy In 1999 midwives referred a total of 30,435 women during pregnancy to secondary care.This amounts to 25% of the total group of 122,913 women who started care at a midwife. 25,285 (83%) of them returned to the midwife for care during puerperium (see Table 4.1).

Dit is 33% van de groep die in die periode in zorg was van de verloskundige.Van de in deze fase verwezen vrouwen keerden er 30.512 (98%) terug naar de verloskundige voor de zorg tijdens het kraambed (zie tabel 4.1).
In tabel 4.3.1 wordt de groep vrouwen die werd overgedragen tijdens de baring uitgesplitst naar primiparae en multiparae. De meest voorkomende reden voor overdracht tijdens de baring worden gepresenteerd in tabel 4.3.2. 1)

4.3 Referral during labour, third stage and directly postpartum In 1999 midwives referred 31.077 women were referred to secondary care during labour, the third stage and directly postpartum This represents 33% of the group receiving care from the midwife during that period. 30.512 (98%) of the women who were referred during labour returned to the midwife for care during puerperium (see Table 4.1).
In table 4.3.1 the group of women referred during labour is divided into primiparae and multiparae.The most frequent reasons for referral during labour are presented in table 4.3.2.

4.4 Overdracht tijdens kraambed In 1999 droegen verloskundigen 285 vrouwen tijdens het kraambed over aan de tweede lijn.Ten opzichte van de totale groep (135.365 vrouwen) die bij de verloskundige in zorg was tijdens het kraambed is dit 0,2%.
In de navolgende subparagrafen wordt een verdeling naar pariteit en naar redenen van overdracht gepresenteerd.
4.4 Referral during puerperium In 1999 midwives referred 285 women during their puerperium to secondary care.This is 0.2% of the total group (135,365 women) with primary obstetric care during puerperium.

5 Verloskunde in de tweede lijn 5.1 Aanvang zorg naar pariteit Dit hoofdstuk presenteert de zorgverlening door de tweede lijn.Als eerste is te zien in welke periode van de zwangerschap vrouwen onder de zorg van de gynaecoloog kwamen en hoe de baring plaatsvond. Vervolgens wordt aandacht besteed aan de kunstverlossingen voor verschillende groepen vrouwen. En tot slot wordt ingegaan op een aantal zaken rondom meerlingen.
In 1999 werd aan 110.130 vrouwen zorg verleend door gynaecologen die deelnamen aan de LVR. Zorg aan vrouwen die na de geboorte van hun kind werden doorverwezen naar de tweede lijn, zijn in dit hoofdstuk buiten beschouwing gelaten.
Voor deze publikatie is ‘zorgverlening in de tweede lijn’ als volgt gedefinieerd: de gynaecoloog draagt de verantwoording voor de zorg vanaf het moment van eerste onderzoek in de tweede lijn (tijdens zwangerschap of baring) tot en met de voltooiing van de baring.
5 Obstetrics in secondary care 5.1 Start of care by parity This chapter presents the care provision in secondary care. Firstly, it shows during which period of their pregnancy women started to receive obstetric care and how labour took place. Subsequently, attention is paid to instrumental deliveries for various groups of women. Finally, notice is taken of a number of matters with respect to multiple gestations.
In 1999, 110,130 women were attended to by obstetricians participating in the LVR. Care to women who were referred to secondary care after the birth of their child are not included in this chapter.
For this publication,“secondary care provision” is defined as follows: the obstetrician has the responsibility for care from the first examination in secondary care (during pregnancy or labour) until the completion of labour.

Het grootste deel van de vrouwen in de LVR-2 komt tijdens de zwangerschap onder de zorg van de gynaecoloog. In 1999 komt 28% van de vrouwen die tweedelijns zorg krijgen, tijdens de baring in tweedelijns zorg. Dit zijn vrouwen die (thuis of in het ziekenhuis) met de eerstelijns verloskundige of huisarts aan hun bevalling zijn begonnen.

5.5 Sectio’s Het landelijk percentage sectio’s was in 1999 11%. Dit percentage is gestegen met 2,6% ten opzichte van het percentage in 1993 (8,5%). Van de bevallingen onder verantwoordelijkheid van de tweede lijn vindt 19,2% plaats via een sectio. Hieronder vallen niet de eerstelijns bevallingen (al dan niet in ziekenhuizen) die onder de verantwoordelijkheid van een verloskundige of huisarts plaatsvinden. In de navolgende subparagrafen worden de sectio’s afzonderlijk belicht.
Bij een primaire sectio is geen vaginale bevalling geprobeerd. Bij een secundaire sectio is wel een vaginale bevalling geprobeerd. Een primaire sectio blijft een primaire sectio ook als de vrouw onverwacht in partu komt. In de tabellen 5.5.1 tot en met 5.5.6 tellen de percentages in horizontale richting per jaar op tot 100%.
5.5 Caesarean sections The national percentage of Caesarean section in the Netherlands was 11% in 1999. Compared to 1993 the caesarian rate was increased by 2.6% (coming from 8.5%). From all deliveries in secondary care and under responsibility of the obstetrician 19.2% were performed via Caesarean sections. Deliveries in hospitals that are attended by a primary care midwife or GP are not included. Caesarean sections are discussed separately in the following sub-paragraphs.

Opzet en ontwikkeling 1982-1999
De Landelijke Verloskunde Registratie (LVR) is tot stand gekomen op initiatief van de Nederlandse Vereniging voor Obstetrie en Gynaecologie (NVOG) in nauwe samenwerking met de Koninklijke Nederlandse Organisatie van Verloskundigen (KNOV), de Inspectie Gezondheidszorg (IGZ) en Prismant (voorheen SIG
Zorginformatie). Het is een continu informatiesysteem op landelijk niveau, opgezet voor de registratie van gegevens over verloskundige zorg, verleend door verloskundigen, huisartsen en gynaecologen.
Appendix A1 National Perinatal Database A1.1 Establishment and development 1982-1999 The National Perinatal Database of the Netherlands (LVR) was set up as an initiative of the Dutch Association for Obstetrics and Gynaecology (NVOG) in close collaboration with the Royal Dutch Organisation of Midwives (KNOV), the Inspectorate of Health Care (IGZ) and Prismant Health Care Information. It is a
continuous information system at a national level, set up to register data on obstetric care provided by midwives, GPs and obstetricians.

De doelstellingen van de Landelijke Verloskunde Registratie zijn: • het stimuleren van een permanente vorm van kwaliteitscontrole voor individuele zorgverleners en beroepsgroepen betrokken bij de verloskundige zorg; • het stimuleren van een continue intercollegiale toetsing voor zorgverleners in de eerste- en tweedelijns gezondheidszorg; • het verzamelen en beschikbaar stellen van verloskundige gegevens ten behoeve van beleidsbepaling en -evaluatie, wetenschappelijk onderzoek en onderwijs.
In 1982 is gestart met de registratie van tweedelijns bevallingen, waaraan in 1999 door 97% van de ziekenhuizen met obstetrie werd deelgenomen. In 1985 is begonnen met de registratie van de eerstelijns bevallingen, zowel thuis als in de kliniek en alle overige zorgverleningen (incl. overdrachten), waarbij een verloskundige is betrokken. In 1999 registreerde 92% van de verloskundige praktijken in de LVR eerste lijn.
The objectives of the National Perinatal Database are: • to stimulate a consistent form of quality control for individual care providers and professional groups involved in obstetric care; • to stimulate peer review between colleague care providers in primary and secondary care; • to collect and to make available obstetric information for policy making and evaluation, scientific research and education.
In 1982,secondary care delivery registration was begun. In 1999, 97% of the hospitals with an obstetrical speciality were participating. Registration of primary care deliveries, at home as well as in the hospital, and of all other care provisions (incl. referrals), in which a midwife is involved began in 1985. In 1999, 92% of midwife practices registered data for LVR-1 primary care.

Werkwijze LVR
De informatie wordt per kalenderjaar geregistreerd, waarbij de geboortedatum van het kind bepalend is. Het vastleggen van de gegevens voor de LVR vindt plaats in de praktijken van de deelnemende verloskundigen en gynaecologen. Voor de registratie wordt gebruik gemaakt van een formulier of van een geautomatiseerd systeem. De vast te leggen gegevens zijn voor een groot deel voor de eerste en de tweede lijn gelijk. De door de deelnemers geleverde gegevens worden bij Prismant gecontroleerd en opgeslagen in een computersysteem. Deelnemers hebben de mogelijkheid door Prismant geconstateerde fouten te corrigeren. Ook gesignaleerde onwaarschijnlijke situaties op verloskundig specialistisch gebied worden door Prismant ter beoordeling aan de deelnemende praktijken voorgelegd. De betrouwbaarheid van de gegevens heeft grote aandacht, waarbij ernaar gestreefd wordt het invullen van de LVR-gegevens een routinehandeling te laten zijn, die deel uitmaakt van de dagelijkse werkzaamheden. De eenmaal vastgelegde gegevens kunnen meervoudig worden gebruikt.
Sinds 1994 leggen alle deelnemende gynaecologische praktijken de gegevens vast door middel van een computersysteem. De automatiseringsgraad in de 1e lijns praktijken groeit gestaag. In 1999 gebruikte ongeveer 45% van de deelnemende verloskundige praktijken een computer-systeem voor vastlegging van de LVR1-gegevens.
The data supplied by the participants are checked by Prismant and stored in a computer system.The participants have the opportunity to correct errors identified by Prismant. Also evidently impossible situations occurring in the obstetric field, and detected by Prismant, are submitted to the participating practices for assessment. The reliability of the data is receiving a lot of attention. The aim is to make the completion of the LVR data a routine part of the daily activities. Once recorded, the data can be used for a number of purposes.
All participating obstetrical practices have recorded data by using a computer system.The degree of automation in the primary care practices is gradually increasing. In 1999, 45% of the participating midwifery practices used a computer system for storing LVR1 data.

The input-supporting information consists of several reports to the relevant participant on the quality of the data supplied by the participant, on the basis of which the participant can correct the data, if necessary. The annual practice analysis contains information, at the practice level, which identifies relations with previous years and with the national level. This publication is an example of the LVR information at a national level. Analysis and research files can be composed on request.

Tot 1 januari 1994 werd bij elke geboorte de ‘aard van de verloskundige hulp’ (begeleiding door verloskundige, huisarts of gynaecoloog) en de ‘plaats van de bevalling’ (thuis of in het ziekenhuis) vastgelegd door het Centraal Bureau voor de Statistiek (CBS). Met ingang van 1995 zijn deze gegevens niet meer beschikbaar. Het totaal aantal geborenen wordt wel nog geregistreerd door het CBS. Met behulp van gegevens uit de LVR en het CBS is een ruwe schatting gemaakt van het aandeel eerstelijns en tweedelijns geborenen. Door het CBS wordt als definitie voor het aantal levend geborenen gehanteerd: alle geborenen die enig teken van leven hebben getoond (ademhaling, spieractiviteiten, hartaktie), ongeacht de zwangerschapsduur. In 1999 registreerde het CBS 201.389 geborenen in Nederland, inclusief doodgeborenen.

Information about the type of obstetric assistance (by midwife, GP or obstetrician) and place of birth was gathered by the Central Bureau for Statistics Netherlands (CBS) until 1 January 1994.After that date, this kind of information is no longer available.The total number of births is, though, still registered by the CBS. The distribution of births over primary and secondary care can be derived from LVR1 and LVR2. The CBS definition of the number of live births is all children born who have shown any sign of life (breathing, muscular activity or heartbeat), regardless of the duration of pregnancy. According to CBS, 200,445 children were born in 1999 .

Beschrijving berekeningsmethode eerstelijns- /tweedelijns geborenen. Voor het verkrijgen van inzicht in de verdeling van het aantal geborenen tussen de eerste lijn (de verloskundige en de huisarts) en de tweede lijn (het ziekenhuis) is de volgende aanpak gekozen. Bij de berekening is in eerste instantie uitgegaan van de gegevens in de tweede lijn (LVR-2). Deze keuze is gebaseerd op het feit dat de LVR-2 een relatief hoge representativiteit (hogere nauwkeurigheid) kent (zie hoofdstuk 1). Ook de zelfstandig gevestigde verloskundigen praktijken in de LVR-1 hebben een hoog deelnamepercentage. Door het ontbreken van gegevens van de huisartsen is de representativiteit van de totale eerste lijn echter lager dan die van de tweede lijn.
Description of the method used for the distribution of births over primary and secondary care The following approach was used to get information about the distribution of births over primary (midwife and GP) and secondary care (hospital). The number of births in secondary care was used (LVR2) as the baseline.This choice was based on the fact that the LVR-2 has a much higher representation (a better accuracy) than the primary registration (see Chapter 1). The independently established midwives’ practices in the LVR-1 also have a high participation rate. However, due to the lack of information from GPs, the representativeness of primary care in total is lower than the one of secondary care.
Om voor de ontbrekende data uit de LVR-2 een 100% representativiteit te bereiken is de LVR2 aangevuld met gegenereerde data voor de ontbrekende ziekenhuizen. Met behulp van geboorteaandelen per gemeente zijn de ontbrekende data van niet-deelnemende ziekenhuizen gesimuleerd. Deze data zijn alleen gebruikt om te komen tot een schatting van het eerstelijns en tweedelijns aandeel en zijn verder in deze publicatie buiten beschouwing gebleven. Het totaal aantal geborenen in de tweede lijn wordt op deze manier geschat op 11.509, zijnde 58,3% van het totaal. Nadat de tweedelijn zo volledig mogelijk is herberekend kan de eerste lijn in beeld worden gebracht. Het totaal aantal geborenen (CBS) minus het geschat aantal geborenen in de tweede lijn (LVR2 aangevuld met de gegenereerde gegevens) levert vervolgens het aantal geborenen in de eerste lijn op. Dit betreft dan zowel geborenen bij de verloskundigen als bij de huisartsen.
To approach a 100% representation of the LVR), the actual registration was completed with generated data for missing hospitals. Missing data from non-participating hospitals were simulated through local birth percentages.These data were used only to estimate the primary and secondary care rates and were further excluded from this publication.This method resulted in an estimated number of 11,509 children (58.3% of the total) born in the secondary care. Comment:
Bij deze berekeningsmethode moet worden bedacht dat het gaat om een ruwe schatting.Verdergaand onderzoek is nodig om een nauwkeuriger beeld te kunnen schetsen. Deze methodiek houdt geen rekening met eventuele dubbeltellingen in beide registraties, hetgeen zou kunnen leiden tot een onderschatting van het aandeel van de eerste lijn. Bij een enkele regio heeft de optelling van het aantal geborenen (LVR1 en LVR2) volgens deze berekeningswijze geleid tot iets meer dan 100%. Dit kan een indicatie zijn van mogelijke doublures in de registraties.
While using this method, it must be kept in mind that this is a rough estimation. More in-depth research is needed in order to outline a more precise description. This methodology does not include any possibility of a double inclusion of data, in both registrations.This could lead to an underestimation of the share of primary care. For some regions, using this method of calculation can lead to numbers of births (LVR1 and LVR2) that are a bit more than 100%.This can be an indication for possible duplication in the registry.
