Inleiding / epidemiologie Aspecifieke nekklachten • Incidentie: 1,2 – 213 / 1000 personen / jaar • Prevalentie (jaar) 30-50% • Voor de huisarts: 16 / 1000 patiënten / jaar • Voor de fysiotherapeut: 33% is middels directe toegang
Inleiding / epidemiologie Specifieke nekklachten • cervicale radiculopathie (5%) • 20-25% veroorzaakt door hernia Neck Pain Task Force: Joint & Bone Decade 2000-2010
Diagnostiek • Doel: om mensen met specifieke onderliggende pathologie op te sporen • Anamnese • Rode vlaggen • Prognostische factoren • Lichamelijk onderzoek • Provocatie testen • Beeldvormend onderzoek
Anamnese Rode vlaggen (0.1%) • Nachtelijke pijn, trauma met geheugenverlies, kanker in familie of anamnese, etc: • Onduidelijk welke relevant zijn (validiteit, betrouwbaarheid)
Anamnese Prognostische factoren: • Vragenlijsten (NDI, VAS etc) zijn betrouwbaar in inventarisatie van factoren • Met name pijn intensiteit is voorspellend
Lichamelijk onderzoek • Range of motion, pijnpunten, dysfunctie • Validiteit en betrouwbaarheid is laag • Aanvullende waarde op anamnese is gering
Lichamelijk onderzoek Provocatietesten • Upper limb tension test, Spurling test, compressie test etc • ULTT enigszins valide, met name in uitsluiten van cervicale radiculopathie
Beeldvormende technieken • Voor nekklachten zonder uitstraling: • Geen bewijs voor enig nut • Voor nekklachten met uitstraling: • Onvoldoende bewijs voor enig nut
Beeldvormende technieken Consistente bevindingen in: • anamnese (radiculaire uitstraling) + • ULTT + • MRI • Voorspellende waarde om cervicale radiculopathie te diagnosticeren (en uitsluiten)
Behandeling Doel: • meer mensen, of mensen sneller dan natuurlijk beloop te laten herstellen
Behandeling Behandel keuzes 1e lijn: • Afwachtend beleid • Advies en pijnstilling • Paramedische zorg • Fysiotherapie • Manuele therapie • Verwijzing 2e lijn

Conclusie • Ongeveer 95% patiënten heeft aspecifieke klachten • Natuurlijk beloop is gunstig (5070% herstelt) • Alleen de combinatie van anamnese, provocatie test en MRI nuttig in uitsluiten (of bevestigen) van cervicale radiculopathie
Conclusie • Advies om actief te blijven zeer gunstig, evt met korte begeleiding fysiotherapeut of manuele therapie • Bij cervicale radiculopathie is afwachten in eerste instantie aangeraden • Gebruik van predictiemodellen is hoopvol, maar nog in ontwikkelfase
