Evidence voor richtlijn • Meta-analyses: • Avenell, 2004 (J Hum Nutr Dietet, 2004) (follow up >=1 yr)
• Anderson, 2000 (Am J Clin Nutr, 2001) (follow up >=2 yr)
• Gewichtsdaling 5 kg (3.5 kg-7kg) in een jaar

Meta-analyse in American Psychologist, 2007 Diets are not the answer •
(niet in richtlijn!)
• 1/3 – 2/3 regain more weight than they lost on their diets • There is little support for the notion that dieting leads to lasting weight loss 12
Mann et al: bias ‘successful weight loss maintenance’ • Waar moet je bij RCT op letten? - Gemeten vs zelf-gerapporteerd gewicht? - % drop out? - Duur van follow-up - % regain of lost weight?
13
>1 yr vs >2 yr of langer • Tabel: 1 yr follow up Avenell et al., 2004: • Vb. Swinburn et al., (Diabetes Care, 2001) (-3.3 kg na 1 yr) • Onvermeld blijven Swinburn’s resultaten van 5-yr follow-up
• compared with the control group… the greatest weight difference was found at 1 year (-3.3 kg) • diminished at subsequent follow up (…-1.6 kg at 3 years) • and was no longer present by 5 years (+1.1 kg)
14
Diets are not the answer • Lijnen bestrijdt symptoom (gewicht)

Overgewicht en EE niet in DSM-5 • Maar....... • EE sterk gelieerd aan BED • (Binge Eating Disorder) • BED wél in DSM-5

DSM-5: Binge Eating Disorder • A. Recurrent episodes of binge eating. An episode of binge eating is characterized by both of the following: • 1. eating, in a discrete period of time (e.g., within any 2-hour period), an amount of food that is definitely larger than most people would eat in a similar period of time under similar circumstances • 2. a sense of lack of control over eating during the episode (e.g., a feeling that one cannot stop eating or control what or how much one is eating)
DSM-5: Binge Eating Disorder • B. The binge-eating episodes are associated with three (or more) of the following: • 1. eating much more rapidly than normal • 2. eating until feeling uncomfortably full • 3. eating large amounts of food when not feeling physically hungry
DSM-5: Binge Eating Disorder • 4. eating alone because of being embarrassed by how much one is eating • 5. feeling disgusted with oneself, depressed, or very guilty after overeating • C. Marked distress regarding binge eating is present.
DSM-5: Binge Eating Disorder • D. The binge eating occurs, on average, at least once a week for three months. • E. The binge eating is not associated with the recurrent use of inappropriate compensatory behavior (i.e., purging) and does not occur exclusively during the course of bulimia nervosa or anorexia nervosa.
Overgewicht en EE niet in DSM-5 • EE hangt sterk samen met BED • (Binge eating disorder)
• EE hangt samen tekort aan hersendopamine in dorsale striatum
