Plánování ortognátních operací pomocí modelové operace Orthognathic surgery planing with the model surgery

Jsou hodnoceny mechanické a geometrické vlastnosti prakticky dostupných typů artikulátoru ajejich vlivná výsledek modelové operace. Je zdůvodněno použití a výhody artikulátoru vyššího typu a obličejových oblouků, podán jednoduchý návod pro volbu přístroje k modelové operaci a ke způsobu transferu modelové operace. Krátce je popsána vlastní metoda modelové operace vyvinutá na autorském pracovišti pro segmentální operace typu ostektomie dolní čelisti (Ortodoncie 2006,15, č. 2, str. 29-41). Abstract The report deals with the choice ofan articulator for model surgery prior to orthognathic surgery. Mechanical and geometrical features ofthe articulators available are discussed, as well as theirimpact on the model surgery result. Usage and advantages of articulators with face boware discussed. A simple instruction forthe selection of the device forthe model surgery is proposed. The means of model surgery transfer is also discussed. The method of model surgery for segmental body ostectomy ofthe mandible, developed at the Department is described in bríef (Ortodoncie 2006,15, No. 2, p. 29-41).
Klíčová slova: ortognátní chirurgie, ortodoncie, modelová operace, artikulátor, okludor Key Words: orthognathic surgery, orthodontics, model surgery, articulator, occludator Úvod Ortodonticko-chirurgická léčba spočívá v kombinaci operace na čelistních kostech a ortodontické léčby. Používáme ji u anomáliívětšího rozsahu s negativním (rušivým) vlivem na estetiku obličeje, které nemůžeme upravit pouze ortodontickou léčbou. Na stanovení plánu léčby se podílí specialista v čelistní a obličejové chirurgii a specialista v ortodoncii, v některých případech bývá nutná spolupráce i jiných odborníků. Diagnostika čelistních anomálií a obličejových deformit se opírá o klinické vyšetření včetně psychologického profilu pacienta. Nutná je analýza diagnostických fotografií, rtg dokumentace (kefalogramů, ortopantomogramů event. rtg ruky se zápěstím pro posouzení kostního věku) a analýza sádrových modelů chrupu, zastavených v okludoru nebo v případě potřeby v artikulátoru. www.orthodont-cz.cz

Introduction Orthodontic - surgical approach is used mainly in the management of extensive anomalies disturbing the esthetics of face which cannot be managed with orthodontic treatment only. The treatment plan is developed by an orthognathic surgeon and an orthodontist; cooperation with other experts may be required in more complicated cases. Diagnostics of jaw anomalies and facial deformities is based on clinical examination including psychological profile of a patient. The analysis of diagnostic photographs, radiographs (cephalograms and orthopantomograms; radiographs of a hand and wrist to judge the skeletal age, may be also required) is necessary, as well as the evaluation of model casts mounted on articulator. 29

Podle povahy anomálie je na základě této diagnostické dokumentace možné sestavit léčebný plán, posoudit technické možnosti ortodontické i chirurgické fáze léčby, předpovídat stabilitu výsledku léčby, výslednou estetiku obličeje a také časové i terapeutické zatížení pacienta. Před operací je dále pomocí těchto diagnostických prostředků možné simulovat chirurgický zákrok, tzv. modelovou operací. Kombinovaná ortodonticko-chirurgická léčba probíhá v těchto základních etapách: 1. Ortodontické předléčení se provádí téměř výlučně fixními aparáty. Jeho cílem je vytvořit dva pravidelné zubní oblouky, které budou po chirurgickém posunu v co nejlepší interkuspidaci.Protože jsou obvykle zuby skloněny tak, aby vadu kompenzovaly, je třeba tyto sklony odstranit, čili eliminovat dentoalveolární kompenzační mechanismus. Někdy je třeba provést tzv. dekompenzační extrakce zubů, naopak kompenzační extrakce jsou kontraindikovány. Tím se vada relativně před operací zhorší, což je třeba pacientovi vysvětlit. 2. Cílem vlastní ortognátní operace na čelistech je navodit pravidelný vzájemný vztah zubních oblouků pomocí repozice skeletálních čelistních bází do nové polohy. Technická proveditelnost i stabilita repozice záleží na vektoru změny polohy. Ortognátní operace na čelistech lze rozdělit podle čelisti, ve které se operuje na a) monomaxilárníoperace na mandibule, b) monomaxilární oprace na maxile, c) bimaxilární operace na obou čelistech a d) doplňkové operace typu genioplastiky bez vztahu k zubním obloukům. Podle polohy operačního řezu v čelisti lze rozeznávat a) segmentální operace, u kterých je řez veden v průběhu zubního oblouku (např. ostektomie těla mandibuly, přední nebo zadní alveolotomie maxily) a b) na operace, u kterých je řez veden mimo zubní oblouky a s těmito se manipuluje vcelku (např. sagitální osteotomie větve mandibuly nebo totální alveolotomie maxily v linii LeFort I). Volba konkrétního typu operace se určuje individuálně podle typu a rozsahu vady. Zásadní vliv na rozhodovací proces má výsledek modelové operace.

Depending on the character of the anomaly, it is possible to draw up a treatment plan, assess technical possibilities of individual orthodontic and surgical stages of the treatment, predict the stability of the treatment results, resulting esthetics of a face, and time as well as therapeutic demands put on the patient. With the help of the diagnostic instruments it is possible to simulate the operation by means of the so-called model surgery prior to the surgical intervention. Combined orthodontic-surgical treatment has the following stages: 1. Orthodontic pre-treatment is performed almost exclusively with fixed appliances. The aim is to form two regular dental arches that will show the best possible intercuspidation after the surgical movement. In general, teeth incline to compensate for the anomaly; therefore, it is inevitable to eliminate the inclination, i.e. to eliminate dentoalveolar compensatory mechanism. Sometimes it is necessary to perform the so-called de-compensatory extractions of teeth. On the other hand, compensatory extractions are contraindicated. Thus, the defect is relatively worsened prior to the surgery. It is therefore crucial to explain to patient the procedure in detail. 2. The aim of the orthognathic surgery is to establish regular relationship of dental arches with the help of reposition of skeletal jaw bases in a new position. Technical feasibility and stability of reposition depend on the vector of the change of position. Orthognathic interventions in jaws may be subclassified according to the jaw in which the surgery is performed: a) monomaxillary surgery in the mandible, b) monomaxillary surgery in the maxilla, c) bimaxillary surgery in both jaws, d) additional interventions such as genioplasty without relation to dental arches. According to the position of surgical cut performed in the jaw it is possible to speak about a) segmental surgery - the cut is inside the dental arch (e.g. ostectomy of the mandible body), and b)interventions in which the cut goes outside dental arches, while the arches are moved as the whole (e.g. sagittal osteotomy of the mandibular ramus). The individual approach is chosen according to the type and extent of the anomaly. The model surgery result has the principle impact on the decision-making process.

louky. Tuto tendenci podporují i tzv. „neextrakční přístupy" k ortodontické léčbě a teorie o relaci zubních oblouků a čelistního kloubu. Sagitálníosteotomie větve mandibuly je v současnosti nejrozšířenějším typem ortognátní operace, která značně překročila své původní indikace [2] na úkor ostektomie těla mandibuly. Po zjištění faktu, že sagitálníosteotomie větve mandibuly jsou málo stabilní a zvláště v případech korekce skeletálního otevřeného skusu anomálie recidivují, hledaly se jiné možnosti, mezi nimiž se prosadily zejména bimaxilární operace. Bimaxilární operace jsou částečně stabilnější než sagitální osteotomie samy o sobě [3], představují však zvětšené operační riziko a zvýšené zatížení pacienta a jsou z tohoto hlediska v rozporu se zásadou volby nejjednoduššího operačního výkonu, kterým se dosáhne úpravy deformity nebo anomálie, a který zajistí výsledek léčby očekávaný pacientem [4]. Nestabilita a nezbytnost dlouhodobého ortodontického doléčování u sagitálních osteotomiía bimaxilárních operací typu sagitální osteotomie + totální alveolotomie v linii LeFort, vedoucí často k značné nespokojenosti léčených pacientů s výsledkem léčby, motivovala autorská pracoviště k realizaci operací typu ostektomie těla mandibuly [5], a to zvláště v případech vertikálních čelistních anomálií typu skeletálního otevřeného skusu [6-14]. Při dlouhodobých kontrolách léčených jedinců lze pozorovat dobrou stabilitu výsledků léčby, přičemž základním nedostatkům segmentálních operací (zbytkové poextrakční mezery, přechodné parestézie mentálních nervů a pooperační jizvy pod dolní hranou mandibuly) nepřikládají sledovaníjedinci v dlouhodobém sledování prioritní význam. Dlouhodobé sledování vybraných dentálních a skeletálních parametrů potvrzuje vynikající skeletální stabilitu výsledku operace, kvalitativně průkazně překračující úroveň stability značně rozšířených alternativních monomaxilárních typů operace otevřeného skusu, a dále velmi dobrou dentální stabilitu dosaženého výsledku léčby [15-19]. Modelová operace Významnou součástí plánování a realizace kombinované chirurgicko-ortodontické léčby je modelová operace před chirurgickým výkonem [20-26]. Cílem modelové operace na sádrových modelech je nalézt vhodný typ ortognátní operace. Modelová operace umožňuje plánovat pooperační interkuspidaci a potřebné zábrusy, u segmentálních operací stanovit rozsah ostektomií, u bimaxilárních operací zhotovit repoziční dlahy. and to preference of interventions manipulating with the whole dental arches. The tendency is supported by the so-called ,,non-extraction approaches" to orthodontic treatment as well as by theories on the relationship of dental arches and temporo-mandibular joint. Nowadays, sagittal osteotomy of the mandible ram us is the most frequent orthognathic surgery that has already significantly gone beyond its original indications [2]. This is at the expense of the mandible body ostectomy. After it was found out that sagittal osteotomy of the mandible ramus is less stable, and - especially in correction of skeletal open bite - that the anomalies tend to relapse, other possibilities were sought. Thus, bimaxillary interventions asserted themselves. Bimaxillary surgery is more stable than sagittal osteotomies [3]. However, they pose increased risk and more demands on the patient. Therefore, they are against the principle of the choice of the simplest intervention leading to deformity or anomaly improvement, and to treatment results expected by the patient [4]. Instability and the need of a long-time orthodontic post-treatment in sagittal ostectomies and bimaxillary interventions (mandibular sagittal osteotomy + LeFort I line maxillary surgery) when patients were quite often dissatisfied with the treatment results and length, became the motif for the authors to perform ostectomy of the mandible body [5], especially in vertical jaw anomalies of skeletal open bite [6-14]. In the course of the long-time follow-up of our patients we can see a good stability of the treatment results. The patients do not take the basic disadvantages of segmental interventions (residual spaces after extractions, temporary paraestesia of mental nerves, postsurgical scars under the lower edge of the mandible) as the priority. The long-time follow-up of specific dental and skeletal parameters proves outstanding skeletal stability of the results of intervention, the quality of which is evidently much better than the stability of extensively used alternative monomaxillary surgeries of open bite. It also proves a very good dental stability of the treatment results [15-19]. Model surgery The model surgery prior to the surgery is an important part of surgical-orthodontic planning [20-26]. The aim of the model surgery performed on model casts is to decide about the appropriate orthognathic technique. The model surgery makes it possible to plan the post-operative intercuspidation and necessary selection grindings, in case of segmental surgery it helps to establish the extent of ostectomy, in bimaxillary interventions it makes it possible to prepare repositioning splints. With the help of cephalogram it is possible to plan the extent and vector of bone movements outside alveolar ridges of jaws as well as changes in soft 31

na průsvitky kefalogramů a stanovit reorientaci roviny okluze vzhledem k lebce a obličeji. Modelová operace na sádrových modelech a kefalogramech má význam tehdy, pokud opravdu imituje stav zubních oblouků a čelistí dosažený vlastní operací. Proto se při modelových operacích využívá k plánování ortognátní operace různých přístrojů a pomůcek, které napomáhají co nejvěrněji reprodukovat výchozí stav. Mezi tato zařízení patří artikulátor. Tradičním a dlouhodobě ověřeným způsobem je plánovat operaci pomocí modelové operace na sádrových modelech fixovaných v artikulátoru spolu s plánováním kostních posunů na kefalogramech. Druhou možností je využití různých počítačových simulací operace na základě počítačových modelů lebky, čelistí a zubů. Některé z těchto programů se již používají v praxi [27], většina z nich má ale ráz experimentální. Tyto prostředky jsou používány ve dvou koncepcích provedení operace. U první možnosti je řídícím hlediskem interkuspidace sádrových modelů po modelové operaci, přičemž realizovatelnost míry kostních posunů a změny měkkých tkání se ověřují na kefalogramů. Takto se postupuje u monomaxilárních operací a někdy také u bimaxilárních operací. Druhá koncepce se primárně odvíjí od plánování kostních posunů vzhledem k lebce a obličeji na průsvitce kefalogramů nebo na počítačovém modelu a modelovou operací se ověřuje interkuspidace, popřípadě se na modelech zhotoví dlaha. Takto část ortodontistů a chirurgů postupuje u bimaxilárních operací. Tato metoda bývá zpravidla zatížena větší chybou než první, protože vychází z dvourozměrného zobrazení trojrozměrného objektu. The model surgery on plaster casts and cephalograms is justified only when it really reflects the situation of dental arches and jaws arrived at through the intervention. Therefore, in model surgery to plan orthognathic operation various equipment and tools are used that help to reproduce the initial situation. A dental articulator belongs to such equipment. The model surgery performed on model casts mounted on articulator, together with the planned bone movements on cephalograms, is a traditional and well proved way to plan the surgical intervention. Another possibility is to use different computer simulations based on computational models of the skull, jaws and dentition. Some of the programmes are used in practice [27], however, majority are still of an experimental character. The instruments mentioned above are used in two model surgery techniques. In the first, the main aspect is the intercuspidation of model casts after the model surgery. The extent of bone movements and changes in soft tissues are considered in cephalograms. This approach is used in monomaxillary surgery, sometimes in bimaxillary interventions. The second approach is based primarily on the planned bone movements with regard to the skull and face. The therapy is planned with the help of cephalogram or computational model. The model surgery serves to check the intercuspidation; a splint may be prepared with the help of models. Some orthodontists and surgeons use this approach in case of bimaxillary interventions. The second approach is usually subject to greater error than the first one because it is based on bidimensional representation of three-dimensional object. Vztah okluzních rovin ke kloubu je neurčitý a chyba tohoto vztahu bývá u tzv. korunkových okludorů ještě zvětšena neanatomickou polohou kloubní osy okludoru. Rozsah této chyby je do značné míry eliminován u tzv. okludátorů, přístrojů s jednoduchými šarnýrovými klouby, které respektují Bonwillův trojúhelník (obr. 1). V průměrných artikulátorech (obr. 2) jsou modely zastaveny opět v maximální interkuspidaci. Přístroj omezeně umožňuje dynamickou artikulaci. Vztah dolní okluze ke kloubní ose a dráha zavírání jsou nastaveny podle průměrných fyziologických hodnot u populace. Vztah okluzní roviny k lebce zde nelze postihnout. Vseřiditelných artikulátorech (obr. 3-5) je možno individuálně postihnout situaci. V závislosti na dokonalosti jejich mechanického uspořádání lze částečně nebo plně nastavit pohyby v kloubu a vzájemný vztah zubních oblouků při dynamické artikulaci, podle toho se rozlišují artikulátory částečně seřiditelné a plně seřiditelné. Je možné simulovat okluzní vztahy při maximální interkuspidaci i v centrálním vztahu čelistí a vztahy zubních oblouků k lebce a kloubu. Vztah okluze k lebce a kloubu je do přístroje přenesen pomocí obličejového oblouku. U modelové operace se používají tzv. obličejové oblouky určující vztah horní okluze k obličeji a lebce pomocí tří bodů určité referenční roviny na lebce. Referenční rovinou je zpravidla Frankfurtská horizontála. Poloha kloubní hlavice se určuje nepřímo. Průměrná odchylka od skutečné polohy kloubu nepřesahuje u průměrných anatomických hodnot populace 5 milimetrů. www.orthodont-cz.cz

individual parameters of the situation within the mouth, i.e. in their grade of adjustability [28].
The simple-hinge articulator (so-called occludator) is the simplest type of articulator. It does not allow lateropulse movements; it allows only the hinge closing movement. The casts in a simple-hinge articulator are in the relationship of the maximum intercuspidation. The relationship of both occlusal planes to the skull is random in all the three planes of space. The upper occlusal plane is roughly parallel with the upper arm of the device. The relationship of occlusal planes to the joint is indistinct and the error of the relationship is often extended in the so-called "crown occludators" with the non-anatomical position of the joint axis of the device. The extent of this error is largely eliminated in case of the so-called occludators, devices with single hinge joints that respect the Bonwill triangle (Fig. 1). In the average value articulators (Fig. 2) the models are set in the maximum intercuspidation as well. The device allows dynamic articulation to some extent. The relationship of the lower occlusion to the joint axis and the closing path are set according to the mean physiological values of the population. The relationship of occlusal plane to the skull cannot be represented. In adjustable articulators (Fig. 3-5) it is possible to represent the individual patient's situation. Depending on the quality of the mechanical set-up, it is possible to set the movements in the joint and the relationship of the dental arches during movement. Thus, we may distinguish between partially adjustable articulators and fully adjustable articulators. It is possible to simulate occlusal relationships in the maximum intercuspidation as well as in the centric relation, and relationships of dental arches to the skull and joint. The relationship of the occlusion to the skull and joint is transferred to the device with the help of face bows. They define the relationship of the upper occlusion to the face and skull with the help of three points of a reference plane in the skull. Frankfurt horizontal is usually the reference plane. The position of the joint is defined indirectly. The mean deviation from the real po33

Léčebný protokol pro volbu přístroje k modelové operaci Kritéria pro volbu přístrojeje pro klinické použití možno vyjádřit třemi otázkami, které lze považovat za vhodný základ návrhu léčebného protokolu modelové operace: 1. Co je možné určitým artikulátorem zjistit ? 2. Co pro daný typ operace potřebujeme vědět ? 3. Co chceme chirurgovi sdělit ? Kritéria pro volbu přístroje pro modelovou operaci a odůvodněnost indikace k uplatnění artikulátorů vyššího typu a obličejových oblouků vyplývají z mechanických a geometrických vlastnosti dvou prakticky dostupných typů artikulátorů, které se mohou projevit na výsledku modelové operace. Vadná geometrie okludorů se může při modelové operaci manifestovat zvláště chybou odhadu operační autorotace dolní čelisti. Dráha tohoto pohybu je vý-

Treatment protocol for the choice of the device for model surgery The criteria of the equipment choice for the clinical use may be expressed with three questions. These questions are considered an appropriate basis of the proposed treatment protocol for the model surgery: 1. What can be found out with the given articulator? 2. Whatdowe need toknowforthe particular intervention? 3. What do we want to inform a surgeon about? The criteria for the choice of the articulator appropriate for the model surgery and the reason of the indication for articulators of a higher level and facebowes result from the mechanical and geometrical features of the main types of articulators available. Their characteristic features may influence the result of the model surgery, of course.

Vertikálně vadná poloha zavírací osy přístroje nepříznivě ovlivní horizontální vztah oblouků. Horizontálně vadná poloha osy má vliv na vertikální vztah. Vadná dráha zavírání se projevuje zejména v rovině sagitální. Odchylka může být, zvláště u segmentálních operací horní čelisti, z hlediska výsledku operace a prognózy pooperační ortodontické léčby klinicky významná. U operací na těle nebo větvi dolní čelisti je horní okluze zachována v původní relaci k lebce i kloubu. Dolní okluzní rovina a mezičelistní vztahy vzhledem ke kloubu se nemění libovolně ale podle zachovalé horní okluzní roviny. Seřiditelný artikulátor s obloukem zde neposkytne pro budoucí relace kostí žádné podstatné informace a stačí operace v okludoru. Při pochybnostech o pooperační dynamické artikulaci, je vhodné si pooperační artikulaci ověřit v průměrném artikulátoru.

The path of the movement is significant in the interventions aimed at the vertical change in the relationships of jaws. Due to non-anatomical position of the joint axis, the path of hinge closing is steeper than in practice. Vertically defective position of the closing axis of the device affects horizontal relationship of arches. Horizontally defective position of the axis shows negative influence on the vertical relationship. Defective closing path manifests itself especially in sagittal plane. From the viewpoint of the surgery result and prognosis of the post-operative orthodontic treatment, the deviation may be clinically significant, especially in segmental approach in the maxilla.

Especially in the management of open bite-when the vertical orientation of jaws is changed considerably - it is necessary to consider autorotation of the mandible when planning the new position of the maxilla and relationships between the jaws. To state correct path of autorotation it is necessary to know the distance of the lower occlusion from the joint. This is possible to find out only with the help of the adjustable articulator with a face bow. The aim of a bimaxillary intervention is to distribute movements of bones and dental arches between both jaws, either from the viewpoint of the stability of the result, or because of better occlusion after intervention, sometimes also to enhance a physical appearance of the retrognathic face. Both occlusal planes lose their original relationship to the skull; the maxilla may be moved in all directions, and the mandible may be adapted with regard to the maxilla, usually in the relationship of the maximum intercuspidation. The resulting movement of the mandible will be influenced by the position of maxillary incisors after the ope35

Modelová operace podle Kamínka u segmentálních chirurgických zákroků Pro operace typu osteotomie, zejména na dolní čelisti, byla na autorském pracovišti vyvinuta vlastní metoda plánování ortognátních operací pomocí modelové operace. Tato metoda je používána hlavně při léčbě čelistních anomálií a obličejových deformit typu skeletální III. třídy s otevřeným skusem. Při chirurgické korekci progenních anomálií spojených s otevřeným skusem se na dolní čelisti užívají dva základní typy operací - sagitální osteotomie větve a ostektomie v těle mandibuly. Při sagitální osteotomii větve dochází k posunu mandibuly dorzálně, ale pokud má být vyřešena i úprava otevřeného skusu, je nutný posun dolních řezáků kraniálně. Tím však dojde k posunu distální části dolní čelisti kaudálně a natažení svalové kličky, což může vést k nestabilním výsledkům. Má-li být výsledek osteotomie stabilní, je nutný zákrok i na horní čelisti typu LeFort I downfracture. Při ostektomii dochází k vytětí části těla dolní čelisti v místě premolárů nebo molárů a posunu frontálního segmentu dorzálně a kraniálně, čímž je současně korigován obrácený i otevřený skus a dolnítřetina obličeje je zkrácena. K posouzení vhodnosti typu operace a k simulaci dosažených změn slouží modelová operace, jejíž výsledky se přenáší na rtg dokumentaci a po prokreslení slouží k naplánování chirurgického zákroku. Modelová operace se provádí na modelech pacienta vložených do okludoru či artikulátoru (obr. 69). Dolní model má zvýšenou bázi, na kterou se zaznačí dolníokraj mandibuly, paralelnívertikálnílinie před aza zubem indikovaným k extrakci a jednu až dvě horizontální linie na alveolárním výběžku. Po vytětí frontálního segmentu a jeho umístění do nové plánované pozice (obr.10-14) lze zjistit ze změn na těchto liniích velikost ostektomie a rozsah rotace mandibuly ke korekci otevřeného skusu (obr.15-16). Na nákresech dolní čelisti se přenášejí změny pro pravou a levou stranu zvlášť, jejich průměr potom na překreslený kefalogram. Změny v poloze zubů, kostí a kožní brady se přenášejí v plném rozsahu, změny v profilu rtů jsou asi o třetinu menší. Tento způsob modelové operace spolehlivě znázorní rozsah plánovaných změn při segmentálních operacích v dolní i horní čelisti. Transfer modelové operace Důležitou součástí procesu plánování ortognátní operace je způsob provedení transferu léčebné dokumentace včetně modelové operace z ortodontického na chirurgické pracoviště. Zajištění optimálního způsobu transferu léčebné dokumentace plánované operace mezi oddělenými pracovišti specialistů je potřeba ktomu, aby bylo možno opakovaně spolehlivě repro36

ration. The resulting position of maxillary incisors is very difficult to state in the model casts or in the actual segment. If the plan is based on the incorrectly oriented models and on the inaccurate closing path, then the error is transferred on the repositioning splint as well, and the resulting position of the maxilla and the mandible may be different than originally stated and expected. The adjustable articulator and face bow is appropriate for the above mentioned model surgery to reproduce the original orientation of the casts to the joint and to the skull. The procedure also makes iteasierto manufacture repositioning splints, as multiple casts are easily mounted and changed. Model surgery for segmental surgery according to Kamfnek For the mandibular osteotomy surgery our department developed our own method of orthognathic planning with the help of the model surgery. This method is used particularly in the treatment of skeletal Class III open bite anomalies. In the correction of Class III anomalies accompanied with the open bite, two basic procedures are applied in the mandible - sagittal osteotomy of ramus and ostectomy in the mandible body. In sagittal osteotomy of ramus, the mandible is moved dorsally. If we aim to correct the open bite as well, it is necessary to move mandibular incisors cranially. Thus the distal portion of the mandible is moved caudally, and the muscular sling is stretched, which may lead to unstable results. To achieve stable results of the osteotomy, the LeFort I down fracture is necessary in the maxilla. But during mandibular body ostectomy an only part of the mandible body is cut at the place of premolars or molars and the frontal segment is moved dorsally and cranially. Thus the overjet as well as open bite are corrected at the same time and the lower part of the face is shortened. To choose the appropriate technique and to simulate the results achieved, the model surgery is used. The results of the model surgery are transferred onto cephalometric tracings. After they are drawn in detail, they serve as the basis for the plan of surgical intervention. The model surgery is performed in a patient's plaster casts mounted on a simple articulator (Fig. 6-9). The lower model has the heightened basis onto which the lower mandibular border is marked, as well as parallel vertical lines in front and beyond the tooth indicated for extraction, and one or two horizontal lines on the alveolar bone. After the frontal segment is cut and located in the new planned position (Fig.10-14), it is possible to find out in the lines the extent of ostectomy and the extent of the mandibular rotation to correct the open bite (Fig. 15-16). In the drawings of the mandible, the changes in the right and left sides are transferred separately, their mean values then onto the drawn cephalogram. Z hlediska transferu modelové operace je nejsnazší doručit modely bez přístroje. "Modelová operace" bez přístroje v ruce je snadno a rychle proveditelný způsob modelové operace, ale ze sdělitelných informací z bývá minimum - jen odhadovaná pooperační interkuspidace a odhad potřeby zábrusů. V užším slova smyslu se tedy o žádnou modelovou operaci nejedná. Okludátor a průměrný artikulátor postačují pro operace na těle a větvi mandibuly, akceptovatelný je transfer modelů v přístroji bez ztráty dosažené informace, tj. transfer mezi pracovišti s modely zastavenými v přístroji. Pořízení přístroje je bez vysokých finančních nároků. Nevýhodami jsou snížené možnosti zpětné kontroly výchozího stavu chirurgem a ztráta části informace z modelové operace po vyjmutí sádrového modelu z přístroje. Určitou nevýhodu je také nepohodlný transfer léčebné dokumentace včetně přístroje, ve kterém byla modelová operace provedena.

In the operating conditions of Czech departments there are three available ways of transfer of the model surgery: "Model surgery" at hand (without any device), model surgery in a simple-hinge or average value articulator, and model surgery in semi-adjustable articulator with face bow. From the viewpoint of the transfer of model surgery the easiest way is to deliverthe models without an apparatus. "Model surgery" without an apparatus in hand is an easy and quick way of model surgery, however, there is a minimum of information to share - expected occlusion after surgery, the need of grindings. In fact, we cannot speak about the model surgery in the strict sense. Simple articulators are appropriate for the interventions in the mandibular body and ramus. The transfer of models in the device is acceptable, the information is not lost, i.e. there is the transfer of models mounted on the device between the departments. The device is not costly. However, disadvantage may be seen in the lower possibility of the check-up of the initial condition by a surgeon, and the partial loss of information achieved during the model surgery after the models are removed from the device. Uncomfortable transfer of the documentation including the device is also a partial disadvantage.
