– Verbetering door glucose toe te dienen
Functioneel pancreas NET Gastrinoom (Zollinger Ellison Syndroom) – Gastrine overproductie – Verlaging pH maag en darm • Diarrhee, buikpijn, ulcera, perforaties darm/maag

Enucleatie of Resectie? • Meer enucleaties in pancreaskop • Meer enucleaties in functionerende tumoren • Meer pancreas fistels na enucleatie • meer ernstige fistels na resecties • Overall geen verschil opname duur, morbiditeit/mortaliteit en survival. • Wel een verschil tussen enucleatie pancreaskop versus PPPD Pitt et al, J Gastrintest Surg 2009
Enucleatie pancreasNET • Wel enucleatie: – Insulinomen – Geen contact met ductus pancreaticus – Pancreaskop laesies – Gastrinomen in het pancreas
Neoadjuvante behandeling pancreas NETs • Geen vergelijkende studies • Weinig tumorafname volgens RECIST met octreotide analoga, sunitinib of everolimus • Mogelijk enig effect peptidereceptor radionuclide therapy PRRT (Lutetium) voor lokaal irresectabele pancreastumoren zonder metastasen • Serie Rotterdam: – 3/48 patiënten ondergingen een PPPD (7%) na 177Lu-octreotate – 3/12 patiënten (25%) met alleen locale irresectabiliteit voor behandeling met 177Lu-octreotate
