Bestuurswisseling Tijdens het NVVC-Voorjaarscongres op 1 april 2011 zijn zowel voorzitter Marcel Daniëls als secretaris Paul van Dijkman teruggetreden als leden van het NVVCbestuur. Inkomend voorzitter Martin Schalij (LUMC Leiden) trad aan als de nieuwe voorzitter van de NVVC. De nieuwe inkomend voorzitter per 1 april 2011 is Victor Umans (Medisch Centrum Alkmaar), de nieuwe secretaris is Francisca Nijland (Lucas Andreas Ziekenhuis, Amsterdam).

Programma CVOI CAG Symposium
Vrijdag 27 mei 2011, Limburgs Museum, Venlo Het coronair angiogram speelt een centrale rol bij de diagnostiek van een groot deel van de cardiologische patiënten. De grote belangstelling en de enthousiaste reacties op de vijf in Venlo georganiseerde CAG symposia hebben de organisatoren in samenwerking met het CVOI doen besluiten dit symposium jaarlijks op alternerende locaties in Nederland te organiseren; teneinde alle cardiologen in opleiding in de gelegenheid te stellen tenminste eenmaal in hun opleiding aan dit symposium deel te nemen. Vorig jaar heeft het symposium in Leiden plaatsgevonden en voor dit jaar staat Venlo als locatie op het programma. Het doel van dit symposium is om alle aspecten van het coronair angiogram als diagnosticum, met hulp van een keur van sprekers aan bod te laten komen. Het programma heeft vele leermomenten voor cardiologen en cardiologen in opleiding. Tijdens het symposium zal een klapper met achtergrondinformatie en relevante literatuur worden verstrekt. De deelnemers kunnen de opgedane kennis toetsen door na het symposium elektronisch enkele meerkeuzevragen te beantwoorden waarbij de diverse programmaonderdelen van de gehele dag aan bod komen. J.G. Nieuwsbrief Tour d’horizon 10-11 juni 2011, Palace Hotel, Noordwijk De elfde Tour d’horizon wordt gehouden op 10-11 juni 2011 in het Palace Hotel in Noordwijk. Last year the study on Dilated Cardiomyopathy in the Netherlands (DiCaMiN-study) was linked to GENCOR (Neth Heart Journal 2010: 18 (4): 214). Here we report a summary of the initial findings. Introduction Approximately 20% to 50% of patients with idiopathic Dilated Cardiomyopathy (DCM) may have a genetic basis1 and familial DCM has been shown to be caused by rare variant mutations in more than 30 genes (www.genetests.org). However, mutations in the known genes only account for 35% of genetic causes leaving the majority of them unknown. Knoll et al. identified two new mutations in the integrin-interacting domain of the Laminin-α4 (LAMA4) gene, and one new mutation in the integrin-linked kinase (ILK) gene2. The initial objective of the DiCaMiN study was to ascertain the incidence of these new mutations in a cohort of familial DCM cases. Method In the multi-center study the inclusion of index patients with familial DCM was accomplished in each of the participating centers by a clinical geneticist. Patients only were included when diagnostic testing for prevalent DCM-genes has been performed. The collection of information on phenotype (e.g. data on diagnosis, ECG and echocardiography) and results of previously screened genes of the patients was coordinated by the GENCORcoordinator and added to GENCOR in each of the participating centers. Genomic DNA of patients was screened for both the p.P943L (exon 22) and p.R1073X (exon 24) LAMA4 mutations and the p.A262V ILK mutation (exon 9) identified by Knoll et al.2 Results 172 patients were included. Clinical information of these patients is stored in GENCOR. The mean age at time of diagnosis was 45.0 ± 13.9 years. Figure 1 gives an overview of genes previously screened in this cohort and the number of mutations found in each of these genes. The LAMA4 and ILK mutations were screened in DNA of 164 patients, no mutations were identified. Conclusion In this cohort of 164 DCM patients none of the novel mutations in exons 22 and 24 of LAMA4 and exon 9 of ILK were present. This suggests that they account only for a small percentage of familial DCM. The

In the RACE 3 study patients with early atrial fibrillation (AF) and early heart failure (HF) are randomized to conventional rhythm control or to an intensive ‘upstream’ rhythm control strategy including application of ACE-inhibitors and/or angiotensin II receptor blockers (ARBs), next to aldosterone receptor antagonists (ARAs), statins, cardiac rehabilitation and counseling. Currently upstream therapies for AF including inhibitors of the renin-angiotensin-aldosterone (RAAS) system are under attention, since adequate treatment of the underlying disease by upstream therapy may improve preservation of sinus rhythm and possibly prognosis in such patients.

In the recently published Atrial Fibrillation Clopidogrel Trial with Irbesartan for Prevention of Vascular Events (ACTIVE I) it was evaluated whether irbesartan, an ARB, would reduce the risk of cardiovascular events in patients with AF. The first coprimary outcome was stroke, myocardial infarction or death form vascular causes, the second endpoint was this composite outcome plus hospitalization for heart failure. A total of 9016 patients was randomized to receive either irbesartan (target dose 300mg) or double-blind placebo. Follow up was a mean of 4.1 years. The mean reduction of respectively systolic and diastolic blood pressure was 2.9 mmHg and 1.9 mmHg greater in the irbesartan group than in the placebo group. Although irbesartan did not significantly reduce the first and second coprimary outcomes, nor improve preservation of sinus rhythm, it was shown that irbesartan administration significantly reduced the number of first hospitalizations for heart failure (p<0.05). Furthermore irbesartan administration also reduced the composite of cerebrovascular and embolic events (p<0.05), reduced the overall number of hospitalizations for both cardiovascular and non-cardiovascular

events (both p<0.01) and also reduced hospital stay duration following cardiovascular events (p<0.001). The only modest difference in blood pressure between both groups could be caused by additional antihypertensive drugs prescribed to patients in the placebo group, as has also been observed in a recent meta-analysis of the largest trials involving ACE-inhibitors. The findings may suggest an effect independent of lowering of the blood pressure in improving clinical outcome. The RACE 3 study is unique in treating patients with early AF and HF with an intensive upstream therapy. An early AF patient group has never been studied before, and herein upstream therapy may have profound effects on atrial remodeling. n
