1. Nederlandse samenvatting Het EWING-2008 protocol is het vervolg van Euroewing 99. In opzet is er veel analogie met het Euroewing 99 protocol. De inclusie criteria zijn gelijk. Ook de groepsindeling na de eerste 6 VIDE kuren is eensluidend. Voor patiënten die voor randomisatie 1 in aanmerking komen wordt nu gevraagd of ze voor bisfosfonaat als adjunct therapie gerandomiseerd willen worden. In de toekomst zal mogelijk nog een randomisatie met fenretinide worden toegevoegd. De patiënten die voor randomisatie 2 in aanmerking komen, zullen op een zelfde manier worden behandeld als in Euroewing 99. Deze patiënten zullen geanalyseerd en gerapporteerd worden samen met de gelijksoortig behandelde patiënten in Euroewing 99. De patiënten in randomisatie 3 zullen worden nu worden behandeld met aansluitend metronomische therapie. Het protocol heeft de Duitse gremia doorlopen en is daar goed gekeurd. In Nederland is er een intentie van de internist-oncologen in Leiden, Amsterdam AMC, Groningen en AvL om te participeren. Internationaal is er geen samenwerking meer met de SFCE en de UKCOG. Deze samenwerkingsverbanden hebben echter momenteel nog geen protocol voor de patiënten met deze aandoening. De samenwerking met Oostenrijk en Zwitserland blijft echter bestaan. De Tsjechen en de NOPHO beraden zich momenteel nog over eventuele participatie. Intern binnen SKION is er goedkeuring van de protocolcommissie Ewing-tumoren en de ziektecommissie beentumoren. Het protocol aan de centra is aangeboden om add-on studies toe te voegen en de verschillende disciplinegroepen hebben hun fiat gegeven. Dec 2009

Advies van Taak- en disciplinegroepen 2.1 Taakgroep Supportive care Basisprotocol ondersteunende maatregelen bij kinderoncologische behandeling versie 210408 Opgesteld door SKION taakgroep Supportive care bestaande uit: WJE Tissing, MD vd Wetering, AYN Schouten-van Meeteren, A. Mavinkurve, M. Bruin, F. Abbink, E. Michiels, L. Ball, en C. van den Hoed – Heerschop. Inleiding De behandeling van maligniteiten vergt een aantal maatregelen in de ondersteunende behandeling. Deze worden ingegeven door de betreffende medicatie, de toedieningsweg, toedieningsperiode en de dosering. Bij radiotherapeutische behandeling zijn het bestralingveld, volume, de dosis en de fractionering bepalend voor de noodzakelijke ondersteunende therapie. De basale preventieve ondersteunende maatregelen treft u navolgend aan. Een deel van de maatregelen is niet gerelateerd aan een specifiek onderdeel van de behandeling maar geldt als ondersteunend in algemene zin. Deze zijn het laatste onderdeel van deze paragraaf. Uiteraard bestaat er een breed spectrum aan bijwerkingen en complicaties van elk betreffend medicament. Deze zijn onder andere terug te vinden in het Farmacotherapeutisch Kompas en kinderoncologische handboeken. Overigens wordt verwezen naar het werkboek “Ondersteunende behandeling in de kinderoncologie”, onder redactie van W.A. Kamps, M.C. Naafs-Wilstra, A.Y.N. Schouten-van Meeteren en W.J.E. Teratogeniciteit

2.2 Taakgroep Late Effecten Juni 2009, Opgesteld door Stuurgroep LATER Richtlijnen betreffende de zorg voor 5-jaars overlevenden van Kinderkanker (Richtlijn SKION LATER) zijn vanaf eind 2009 beschikbaar via www.SKION.nl. Deze richtlijnen zijn met financiële ondersteuning van Zonmw ontwikkeld in een samenwerkingsverband van de 7 kinderoncologische centra in Nederland. Op basis van de oncologische behandeling en de hiermee samenhangende potentiële late schadelijke gevolgen wordt aangegeven welke zorg deze kinderen en volwassenen minimaal nodig hebben. Voor de zorg tussen het einde van de behandeling en 5 jaar na diagnose is nog geen richtlijn beschikbaar. In afwachting van de ontwikkeling van een richtlijn voor deze periode kan voorlopig de SKION LATER richtlijn dienen als uitgangspunt voor de zorg.

3. Studiemedicatie
Verkrijgen van medicatie Treosulfan: Dient via het eigen bestelsysteem van het ziekenhuis besteld te worden. Met het speciale Treosulfan aanvraagformulier dat via de SKION website is te downloaden, kan na invullen door de arts, de apotheek gratis medicatie krijgen van de firma Lamepro. Dit formulier dient dan ook via de apotheek van het desbetreffende ziekenhuis naar de firma te worden verstuurd. Treosulfan kan bij bestelling voor 15.00 uur de volgende dag geleverd worden. Treosulfan kan voor 1 patiënt op voorraad besteld worden bij Lamepro. (Dit op verzoek van de apotheek van het UMCG). Zometa : Toedieningsmogelijkheden: 1.
In het ziekenhuis: Zometa moet via de eigen ziekenhuisapotheek geleverd worden. Kosten van de toediening drukken dan op het budget van de afdeling.

Toedienen in de thuis situatie, door Novanurses. Novanurses geeft per voorschrijvende arts een eigen inlogcode en wachtwoord voor hun internetsite. Via deze site geeft de arts aan wie en hoeveel medicatie gegeven moet worden. Novanurses declareert aan de ziektekostenverzekeraar en de kosten komen niet ten laste van het ziekenhuisbudget.

Zoledroninezuur kan tijdens de consolidatie cycli gelijktijdig met de chemotherapie (a 21 dagen) gegeven worden. Tijdens de consolidatie cycli wordt zoledroninezuur i.p. op dag 1 voorafgaand aan de chemotherapie gegeven. Als het tijdstip van de start van de kuur niet synchroon loopt met het 21 dagen interval van zoledroninezuur (omdat de kuur bijvoorbeeld is uitgesteld), wordt het zoledroninezuur in de thuissiutatie via Eurocept a 21 dagen toegediend. Na deze consolidatiecycli wordt zoledroninezuur a 28 dagen gegeven.

3.1 Zometa (Zoledronizuur) Deze medicatie wordt gegeven middels thuistoediening. De behandelend arts heeft een persoonlijke inlognaam en wachtwoord nodig voor het Extranet van Eurocept Homecare , om deze medicatie aan te vragen en de patiënt te kunnen volgen. Datamanagers hebben ook een inlognaam en wachtwoord nodig om de doses Zoledroniczuur te kunnen registreren in de MARVIN database.
Zolindronic acid PLEASE NOTE: For further and updated information, please refer to exemplary package insert: http://novartispharma.com Mechanism of action: Class II and III bisphosphonates induce apoptosis of cancer cells and prevent bone metastases by inhibition of the mevalonate (HMG CoA reductase) pathway and consecutive loss of prenylation of GTPases as RAS, RAP1, Rho, RAC, cdc42. Bisphosphonates have been shown to induce apoptosis in Ewing tumor cells in vitro and in vivo. Application: intravenously, at least 15 minute iv infusion in a line separate from all other drugs. Zolindronic acid must not be mixed with calcium-containing solutions. Metabolism: Zolindronic acid is not metabolised. 24 hours following application 36 + 16% is excreted renally. Terminal half-life: Triphasic elimination with early biphasic elimination with t½ of 0.24 and t1/2 of 1.87 hours and long term elimination with t½ of 146 hours. No accumulation was observed after repeated application of zolindronic acid. 3.2 Treosulfan
Treosulfan L-Threitol 1,4-Bis(Methanesulfonate) PLEASE NOTE: For further and updated information, please refer to exemplary package insert: http://www.medac.de Mechanism of action: Prodrug of a bifunctional alkylating agent. Treosulfan induces a preferential Guanin-N7 alkylation of DNA and DNA cross linking and irreversible DNA strand brakes Application: IV: as a single hour infusion. Maximum tolerated dose of high-dose treosulfan is 47 g/m². Oral application: Capsules of 1 and 5 g are available. The oral formulation is not recommended for high dose treosulfan prior to autologous stem cell re-infusion. Metabolism: Treosulfan is converted non-enzymatically, but pH dependent, to alkylating monoepoxide intermediates and L-diepoxibutane (Feit, 1961). Dose independent, 25% of the total treosulfan dose is eliminated renally. Terminal half-life: In intravenously administered treosulfan up to 10g/m² terminal half-life was 1.9 h in adults. Administration of high dose treosulfan results in a linear correlation of AUC and Cmax with the administered dose. Contraindication: Hypersensitivity to treosulfan Side effects: The listed site effects are a compilation of published side effects. Please note that the side effects occurred in combined modality high dose regimens different from the regimen proposed in this protocol. 1. Introductie en rationale Ewing sarcoma is the second most frequent primary malignancy of the bones, following osteosarcoma. In comparison to osteosarcoma, Ewing sarcoma is characterized by a relative simple karyotype with only a few numerical and structural aberrations. Most consistently, a chromosomal translocation between chromosome 11 and 22 is present. Initial preoperative treatment of most patients with Ewing sarcoma in the Netherlands consists of vincristine, ifosfamide, doxorubicin and etoposide (EuroEWING 99 and coming protocol). After recovery from this chemotherapy, patients proceed to surgical resection. After surgery, patients continue treatment with chemotherapy, with bone marrow transplantation in poor responders. The histological response of the tumor is determined by evaluating the percentage of tumor necrosis in the removed tumor. The response to preoperative chemotherapy was defined „good‟ if more than 90% necrosis was found in the resected tumor and „poor‟ in the remaining cases. Most larger studies reported that disease-free survival of the patient is significantly correlated with the histological response as poor response to chemotherapy predicts a high chance of treatment failure. With current treatment regimens, about two third of patients with localized disease can be cured, whereas survival rates of patients with metastatic disease are very poor. A poor response to chemotherapy is the dominant risk factor for poor survival. A better knowledge of the mechanisms responsible for the poor response on the current chemotherapeutic regimens may lead to the development of more effective chemotherapeutic approaches. There is growing evidence that more insight into the mechanisms of drug resistance and the genetic determinants of drug response will lead to increased efficacy and decreased toxicity of the therapy (Pharmacogenetics/Pharmacogenomics). Especially, genes and the genetic variants of drug-metabolizing enzymes, drug transporters or drug targets are of interest for determining the optimal treatment. Pharmacogenomics, which studies the role of inheritance in individual variation in drug disposition and response, could be useful to further improve outcome in this disease by getting information from the genetic 'make-up' of normal host cells. Our hypothesis is that polymorphisms in genes encoding enzymes that metabolize chemotherapeutic agents are associated with treatment outcome. In addition, we hypothesize that these polymorphisms may indicate which children will experience severe toxicity of the therapy. To investigate whether this hypotheses is accurate, a thorough analysis of a uniformly treated patient cohort is needed. 2. Doel / vraagstelling van het onderzoek In the proposed investigation, we will study the relevance of polymorphisms in genes encoding drugmetabolizing enzymes as well as transporters and targets of drugs on treatment outcome of patients with Ewing sarcoma. 2: Do the identified genetic variants correlate with the clinical outcome (both efficacy and toxicity) in a well defined group of children and adolescents with Ewing sarcoma. The aim of our study is to define a patients genetic profile predicting the response to chemotherapy. The ultimate goal of this study is to use this genetic profile to develop, in future studies, more individualized, better therapy for patients with Ewing sarcoma based on pharmacogenetic profiles. In addition, we hope that this profile can be used to identify patients with an increased risk of developing treatment-related toxicity, leading to adjustment of therapy. 3. De relevantie van het onderzoek voor patiënten met Ewing sarcoma A pharmacogenetic approach can be used to identify individual determinants of response and outcome in order to maximise the benefits of treatment and keep side effects to a minimum. Ultimate goal of this study is the development of a genetic profile for drug response of an individual patient with Ewing sarcoma based on our results, leading to improved efficacy of treatment and reducing drug-induced toxicity. 4. Eventuele preliminaire resultaten The study described in this research proposal is well embedded in the UMCN and will be performed in collaboration with the Department of Human Genetics. This department has a long and successful history in studies in genetics of monogenic and complex diseases. The department of Human Genetics runs its own microarray-, genotyping-, linkage- and sequence facilities and has a leading position in the expertise regarding genomic DNA microarrays in the Netherlands. There has been a strong collaboration with the department of Human Genetics and the department of Paediatric Oncology for several years now, e.g. a collaboration on array CGH analysis in paediatric ALL. The department has an active research line in pharmacogenomics and statistical support and expertise are present to ensure correct execution and data interpretation of the present study. Pharmacogenetic studies considering patients with osteosarcoma are already being performed at our deparmatment (KiKa 2007-UMCN04 (Pharmacogenomics in patients with osteosarcoma). 5. Een (korte) beschrijving van de te gebruiken onderzoekmethoden DNA isolated of patients with Ewing sarcoma will be used for the analysis of SNPs of genes involved in the response to chemotherapy. Several genes for each treatment component will be included to define a polygenic determinant of drug response. For SNP selection in these experiments, literature and genetic databases such as dbSNP (http://www.ncbi.nlm.nih.gov/entrez/query.fcgi) and the Cancer Genome Anatomy Project SNP500Cancer Database (http://snp500cancer.nci.nih.gov/home) will be used, concentrating on (presumed) functional variants. DNA material of the patients will be prepared from blood and/or paraffin embedded normal bone material. Subsequently DNA concentration measurement (using Picogreen), for the SNP analysis on the Illumina Beadarray platform will be performed. The Illumina Beadarray platform array is fully operational since 2005 and with this technique hundreds of SNPs per sample (>99% call-accuracy) can be successfully analyzed. After DNA of patients is isolated, the DNA is activated for binding to paramagnetic particles using activated biotin. In the following step, oligonucleotides, hybridization buffer and paramagnetic particles are combined for binding to the DNA. After a thorough wash step allele-specific extension and ligation are performed. The extended and ligated products form a synthetic template that is transferred to a PCR reaction, the strand containing the fluorescent signal in the PCR products is isolated and hybridized to the Sentrix® Universal Array for detection. After the hybridization, the Sentrix® Array is washed and imaged on the BeadArray Reader. The internal illumiCode that is specific for each locus binds preferentially to its complementary bead. Quality control removes unreliable genotypes. Subsequently, clinical data of patients will be analyzed and statistical (multivariate) analysis will be performed to evaluate the role of SNPs in genes that play a role of chemotherapeutics used for the treatment of Ewing sarcoma. The treatment regimen of these patients consists of vincristine, ifosfamide, doxorubicin and etoposide. As after preoperative chemotherapy, surgical resection of the tumor is performed the response of therapy is established in these patients. The patients are qualified as good responders (less than 10% vital tumor present) or poor responders. To evaluate the significance of SNPs in genes involved in the metabolic pathway of chemotherapeutics used in the treatment of these patients with Ewing sarcoma, analysis of SNP pattern in relationship to the histological response after preoperative chemotherapy will be performed. In addition, analysis will be

performed with disease-free survival as end point to evaluate whether there is an association with outcome of the disease. Subsequently, with the results obtained, a genetic signature of the patients will be developed in relationship to treatment outcome. 6.
Kwantificering van het gevraagde materiaal/gegevens.
For analysis of the presence of SNPs in germline cells of patients with Ewing sarcoma, 10 ml of whole EDTA blood is needed to isolate DNA. Subsequently, 10 ml of heparin blood is needed for RNA isolation. The best time point to collect blood for DNA isolation and RNA isolation is before start of the chemotherapy treatment, however, it can be taken also at a later time point if white blood cell counts are above 2.0 x 10 9/L. A sum-score of the number of polymorphisms will be derived for each patient, by simply summing up the number of variants present, taking into account the (presumed) function of the polymorphism. Logistic regression analysis will then be performed to evaluate the importance of the profile in (A) survival and (B) disease-free survival. In an exploratory second step, the effects of individual genotypes will be assessed (single marker analysis), comparing survival or tumor response in groups with and without a specific genetic polymorphism. From this analysis, all polymorphisms associated with survival or tumor response at p≤0.1 will be selected and included in the sum-score analysis in an equally sized replication sample. Power estimation for an analysis with 60% survival assuming an odds ratio of 5 for the highest sum-score of variants and a frequency of this highest sum-score of 5% shows that 164 (we will include 200 patients for in case of technical failures) patients have to be studied to reach a power of 80% power (alpha =0.01). We expect to include at least 50 patients a year. For the calculation of power the „Genetic Power Calculator‟ (Purcell S, Bioinformatics, 19(1):149-150) was used . 7. Curriculum vitae van de aanvrager Dr M te Loo will be the principle investigator of the project, supervising on a daily basis the appointed PhD student. Dr B. Franke will be responsible for the SNP analyses and Prof. Dr P. Hoogerbrugge will take care for the overall responsibility of the entire project. Dr Maroeska te Loo (1973) is a pediatric hemato-oncologist working at the department of pediatric oncology, UMCN since 2006. From 2002 to 2006 she was trained in pediatrics at the UMCN after earnig her PhD in 2002 and MD cum laude in 1997. She was awarded for her achievements as investigator by the Dutch Society of Trombosis and Hemostasis (January 2001), and the Dutch Society of Nephrology (March 2001). In November 2002 she received the prestigious price of the Dutch Society of Pediatrics (Price Best Young Investigator). She is a committee member of the ethical review board at the UMCN and is a committee member of the Dutch Society for Pharmacology for Pediatrics (NKFK) and in this consortium responsible for initiatives on pharmacogenomics. Dr Barbara Franke (1969) is assistant professor at the dept of Human Genetics of the UMCN and heads the Researchlab for Multifactorial Disorders within the division of DNA-diagnostics of this dept since 2001. She works at the dept of Human Genetics since 1999, since 2003 she also has an appointment at the dept of Psychiatry. Dr Franke received her PhD from the Laboratory for Physiological Chemistry and the dept of Haematology at Utrecht University for work on the regulation of the small G-protein Rap1 in platelet signaling, after having studied Biology at the Justus Liebig University in Giessen (Germany) and Utrecht University. One of her areas of expertise is pharmacogenetics research. Due to her affiliation with the DNAdiagnostics division, her primary goal in this research is the implementation of pharmacogenetic testing into clinical practice. She is (co-)author of 38 peer reviewed publications and is/was a principal investigator on 11 research grants (8 from national, 3 from local foundations) since 2000. Dr. Franke is a member of the national Pharmacogenetics working group and is an aspiring principal investigator in the Nijmegen Centre for Molecular Life Sciences (NCMLS), the Nijmegen Centre for Evidence-Based Practice (NCEBP) and the Neuroinstitute. Prof. Dr Peter Hoogerbrugge (1957) is head of the dept of pediatric oncology (from 1999 onwards) at the UMCN. From 1997 to 1999 he was clinical director (associated professor) at the dept. of pediatric hematology/BMT at the Univ. of Leiden, director of clinical studies at the IntroGene gene therapy company (Leiden, 1995-1997) and pediatric hemato-oncologist at the Sophia Children‟s Hospital (1995-1997). He received his PhD cum laude on BMT in metabolic diseases in 1988 (Leiden). His areas of research include pediatric oncology, particularly ALL and bone tumors, stem cell transplantation, immunotherapy and gene therapy. He is co-author of 91 peer reviewed publications (june 2007), and was/is principle investigator in 13 research grants (8 from national foundations, 5 from local foundations). Peter Hoogerbrugge is chairman of the DCOG (Dutch Childhood Oncology Group) committees “bone tumors” and “relapsed ALL-2005”, is board member of DCOG, Dutch Soc. for Oncology and Dutch Society for Hematology and treasurer of the hematology division of the Dutch Soc. of Pediatrics.

6. Bijlage Trialbureau All data, except safety information, will be collected by remote data entry. For this purpose national and group offices and institutions will be linked through the Internet (https). Data managers and investigators of the centres have to follow a MARVIN training, given by trial managers of the DCOG. All safety information, i.e. SAE and SUSAR reports, must be entered on paper forms and submitted by fax. The data is checked visually for consistency and completeness when entered in the database. Thereafter, validation checks will be performed. The following rules for completing paper CRFs have to be observed: CRFs are to be filled in with a blue/black ballpoint pen in a clear handwriting. Mistakes are to be cancelled by a simple horizontal line and correction is to be written above or next to it. The correction has to be signed and dated. Data fields which cannot be completed due to missing information have to be marked and commented. Every first page of the CRF starts with the patient unique registration number and date of birth. Every last page of the CRF ends with the date of completion and signature of the local data manager. All requested data fields should be answered completely; even if there is no major change from a previous examination. At all times the local investigator is responsible for the accuracy and authenticity of all clinical and laboratory data entered. Time points for submitting CRFs e-CRF about diagnosis: 1 month after initial diagnosis e-CRFs for treatment: 1 month after treatment or course. e-CRFs about the follow-up: 2 months after visit. Paper CRFs concerning a SAE, immediately after occurrence of SAE. See Chapter 16 Safety . Please send them by fax to the DCOG office. The DCOG office will submit the SAE reports to the international data centre. The archiving of all study relevant documents at the local centres and at the DCOG office will be handled according to national law. Each patient receives an unique patient number (UPN). All study relevant data will be stored electronically and handled confidentially. The investigators and all members of a trial centre or other persons involved in the trial are obliged to keep study data and information confidential and to grant access only to individuals who are involved in the study. Annual reports are made for the Study Committee and for the Data Safety Monitoring Board. Registration and Randomisation The registration of new patients and randomisation of eligible patients will be done by remote data entry in the center.

SAE Serious Adverse Event The definition of a Serious Adverse Event (SAE) is an adverse event occurring at any dose and resulting in one of the following outcomes: (a) death, (b) is life threatening, (c) an unexpected admission to hospital or unexpected prolongation of existing hospitalization, (d) a persistent or significant disability or incapacity to conduct normal life‟s functions, (e) a congenital anomaly or birth defect in the offspring. Every SAE can be classified as expected or unexpected. Expected SAEs are toxic reactions described in the drug information. An unexpected SAE is all toxicity that does not meet the description for expected SAE or the nature/ severity of the event is not consistent with the applicable drug information. Protocol specific exceptions of SAE Reporting The following does not require reporting on the SAE form: (a) hospitalization due to signs and symptoms of disease progression, (b) death due to disease progression, (c) expected hospitalization for procedures such as blood transfusion or unexpected hospitalization for treatment of grade 1-3 toxicities, (d) pre-existing toxicities before entering the study which meet the criteria of a SAE. Serious Adverse Event Reporting During protocol treatment all deaths, all SAEs that are life-threatening and any unexpected SAE must be reported to the DCOG Trial Office within 48 hours of the initial observation of the event. Where possible, a diagnosis rather than a list of symptoms should be given. It is important not to wait for full details before making the initial report. All SAE Reports must be dated and signed by the responsible investigator or one of his/her authorized staff members. This form, with a signature of the treating physician should be sent by fax to the DCOG Trial Office. All SAEs that occur during treatment until 3 months after end of protocol treatment should be reported. In case of initial SAE-reporting in the Netherlands, please use the fax: 0031 - 70 - 359 8718 (DCOG Trial Office) The trial manager from the Trial Office will forward the SAE- report to the study coordinator within 24 hours of receipt. The study coordinator decides (if time allows with consultation of the study committee) what action(s) is (are) necessary for the safety of the included patients. All participating centres are informed if relevant. The Trial Offices will be responsible for reporting of SAEs to regulatory authorities, as appropriate. Every SAE must be followed up until the condition resolves or stabilizes. As soon as possible, but at least within one month after the condition resolved or stabilized, the SAE follow-up has to be send to the DCOG Trial Office.
