Casus • 33 jaar, G2P1 • Medische indicatie: PIH + sectio in VG • 16+3 AD
Patiënte: “Ik zie die bevalling echt niet zitten, ik heb er slapeloze nachten van. Ik wil een keizersnede!”
U vraagt verder…. • Algemene VG: blanco • Obstetrische VG: • 1e zwangerschap (2005): vaak hoofdpijn en slaapproblemen • Inleiding bij 37+4 ivm PIH (met klachten) • Trage ontsluiting, bijstimulatie, epiduraal analgesie • Bij VO: deceleratief CTG en ongunstig MBO • Vacuum: mislukt Æ Spoed sectio • Kind met goede start (Apgar 8/9)
Over de bevalling… “Die eerste bevalling was een blauwe plek, een steen om mijn nek. Tijdens de bevalling voelde ik me in de steek gelaten door de artsen en verpleging. Ik drukte op de bel maar die was kapot, en er kwam maar niemand. Ik wist niet wat er gebeurde en niemand vertelde mij wat. Ik was heel bang dat mijn kindje het niet ging redden toen we ineens naar de operatiekamer moesten rennen”

Nu…. “Ik denk nog steeds heel vaak aan de bevalling terug, soms heb ik er nare dromen over. Als ik langs het ziekenhuis fiets dan begin ik te zweten en moet ik huilen. Ik ben ook nog steeds boos over hoe het toen gegaan is. Als iemand anders vertelt over haar bevalling dan word ik emotioneel. Zelf wil ik er niet over praten, en de foto’s van de geboorte van mijn zoontje wil ik ook niet zien. Ik heb heel lang getwijfeld of ik een tweede kindje wel aandurfde. Voor de bevalling ben ik doodsbang, ik zie het echt niet zitten. ´s Nachts kan ik niet slapen omdat ik denk aan wat er allemaal fout kan gaan”

Posttraumatische stress stoornis (PTSS) na de bevalling
