Chronic inflammation H87-98. Inflammatio chronica 87. What is the definition of the chronic inflammation? Characteristics of the chronic inflammations: - long term process, - coexistance of active inflammation, - tissue destruction and - secondary, connective tissue healing. - It implies that the chronic inflammation heals almost exclusively by secondary intention. 88. Which are the major subtypes of the chronic inflammation according to the clinicopathological hallmarks? 1. Chronic inflammation arising on the ground of repeated inflammation. 2. Chronic inflammation with autoimmune mechanism. 3. Chronic inflammation induced by inanimate, non-degradable, endogenic materials (urate deposition /tophus/, amyloid, lipoidok), by endogenic materials which are considered foreign ones (released cryptic proteins (testis, corpus vitreum, tumor cells) or exogenic materials (real foreign bodies: talcum power, surgical stitches, implants, silicon, etc.). The SiO2 induced silicosis does not belong into this group. 4. Cronic inflammation mediated by type IV HRS induced by microbial agents of low or no toxicity. Tuberculosis induced by the Mycobacterium tuberculosis is the prototype of this form. Type 3 and 4 inflammations are also called granulomatous inflammation. In the inflammatory infiltrate, frequently arranged in granules, lymphocytes, plasmocytes, macrophages and their modified cells, the epithelioid cells participate. The appearance of epithelioid cells is pathognomonic for type IV HRS. Despite of it, type 3. chronic inflammation is frequently called non-immun granuloma, the type 4. chronic inflammation, however, immun-granuloma. It is because the immune mechanism is obvious in the latter one, whereas less obvious in the previous one, or there is other way for the the epithelioid cells to develop. Fusion of macrophages / epithelioid cells results in multinucleated giant cells of Touton-type or Langhans-type. 89. Which are the typical cellular elements of the chronic inflammatory infiltrate? Granulomatosus gyulladásban ez a repertoár epithelioid sejtekkel és óriássejtekkel egészül ki. 90. Which are the clinicopathological examples of the 1st type of chronic inflammation? Pyelonephritis chronica. All the conditions causing chronic stasis of urine, most commonly urolithiasis, as well as diabetes mellitus are the major predisposing factors for chronic pyelonephritis. The most common cause in children is the vesicoureteral reflux disease, in adulthood in men the bilateral prostate hyperplasia (BPH). On the basis of macroscopy it can be differentiated from the chr. glomerulonephritis, the benign and malignant nephrosclerosis as well as from the multiplex obsolete renal infarctions. Cholecystitis chronica. The major predisposing factor is the cholelithiasis. It is called porcelain gallbladder. It represents a precancerous condition. Osteomyelitis chronica. It may arise following open bone fractures as well as by haematogenic spread. Due to the latter one, bacterial colonization takes place usually in the metaphase with sluggish blood supply of the tubular bones. Reactive bone formation, bony encasement occurs around the early acute inflammatory focus, the inflammation persist in this ’sequester’. Complication of it are the fistule formation (dermatitis, Celsus cancer), AA type of amyloidosis and sepsis / septicopyemia. 91. What are the clinicopathological features of the Rheumatoid Arthritis representing one of the prototype of the autoimmun type of chronic inflammation? Chronicus, szisztémás gyulladásos betegség, mely elsősorban súlyos chr. sysnovitist okoz a kéz – láb kisizületekben (interphalangealis és metacarpophalangealis) valamint a csukló, boka, könyök és térdizületeket is érintheti, következményes ankylosis-sal, de érfal (PN tipusú fibrinoid necrosis), bőr, sziv, tüdő, vázizom, ideg (vasa vasorum) valamint szem manifesztáció is előfordul. A digitalis arteriák érintettsége miatt bőrfekélyek valamint körömágy infactus alakulhat ki. Glomerulonephritis nincs. A populáció 1%-a érintett, a betegség manifesztációja széles skálán mozog. A nő / férfi arány 3-5 / 1, egy 20 év körüli valamint egy sokkal nagyobb 40 év körüli incidencia halmozódás jellemző. A II. hisztokompatibilitási antigének közül a HLADRB1*0401 ill. *0404 haplotipus predisponál a betegségre. Pathogenesisében pontosan nem identifikált arthritogen faktor, valószinüleg mikrobiális ágens szerepel ( EBV, parvovirus, Borrelia, Mycoplasma, Mycobacterium egyaránt gyanúsitott, de nem bizonyitott), melyek egyes antigénjei homológiát mutatnak a 2. tipusú kollagénnel, a porc GP-39 molekulával és kölcsönösen reagálnak a fenti HLA-DRβ-lánc antigén kötő részével. A kezdeti antigén stimulus tiltott CD4+ klónok reaktivációját, autoimmun tipusú gyulladás kialakulását, az aktivált helper sejtekből származó IL-1 és TNFα ktsz. és porc destrukciót okoz. Az izületekben villonodularis tenosynovitis alakul ki, exuberáns synoviával (pannus), következményes porc destrukcióval, osteoblast aktiválódás révén osteophyta képződéssel, izületi tok hegesedéssel, ankylosissal. 71
TÁMOP-4.1.1.C-13/1/KONV-2014-0001 A T4 reaktiváció autoreaktiv B-sejt képződéshez, IgG elleni autoantitest (IgM) termeléshez és rheumatoid faktor-nak nevezett IgG-IgM immunkomplex képződéshez vezet. Igy szekunder módon a humorális rendszer is részt vesz a betegség pathogenesisében. A kóros immunkomplexek a synoviában ill. bármely érintett szervben rheumatoid nodulusokat ( palizáló histiocytákkal körülvett, fibrinnel átitatott necrotikus gócok), az erekben pedig fibrinoid necrosissal jellemzett vasculitist idéznek elő. A klinikai prezentáció széles spektrumot mutat, 10% súlyos lefolyást mutat, a hosszantartó gyógyszerelés (aspirin, steroid) GI vérzést, infekciókat eredményezhet, hosszútávon AA tipusú amyloidosis fenyeget. Az anti TNFα terápia (bioterápia) eredményes lehet. A Felty szindróma klinikai variáns: RA + splenomegalia + neutropenia. 92. Which are the typical examples of the 3rd type of chronic, granulomatous inflammation? The foreign body granulomas induced by the causative agents listed under Q88/3. Lipogranuloma mammae. Trauma induced ruptured adopocytes in the breast followed by the release of lipoid substances induces this palpable nodular lesion. Fibrous attachment to the skin of the breast followed by obligatory shrinkage may evoke orange peel syndrome (Janisevsky sign) or mamilla retraction by which the benign lesion mimics the desmoplastic malignant tumor, carcinoma scirrhosum (scirrhus) of the breast. Sperm granuloma (Orchitis granulomatosa). The released sperm cells due trauma induced rupture of the seminiferous tubules evoke this reaction. Palpable lesion, it needs to be differentiated from tumor. Uveitis granulomatosa. Trauma induced injury of the corpus vitreum my evoke such a reaction. It may also occure as part of sarcoidosis. Sarcoidosis. Localized or disseminated granulomatous inflammatory disease. Etiologia: unknown. Histology: non-caseating epithelioid cell granulomas, ’hard granulomas’ characterize the disease. The Langhans or Touton types of granulomas may exhibit i.c. Ca++ containing bodies (Schauman-bodies) or stellate-like structures of unknown origin (’asteroid bodies’) in vacules. For the diagnosis of sarcoidosis exclusion of all other causes, which may induce granulomatous inflammation, is needed. Sarcoid-like reaction / lesion may develop around tumor cells or in their regional lymph nodes. Clinicopathological features: the bilateral hilar lymphadenopathia – BHL) and the peribronchial pulmonary involvement are present in 90% of cases. Dissemination with involvement of the spleen, liver, bone marrow, skin, upper airways as well as iris / uvea - salivary gland – parotis together (Heerfordt syndrome – febris uveoparotidica) may also occur. Laboratory finding: The CD4+ T-helper cells accumulate in the granulomas, as a consequence, they are depleted in the blood. Hypercalcamia, increased level of 72

Outcome: It responds well to steroid in70% of cases, in another 20% permanent disease exists, in the remaining 10 % fatal outcome follows due to disseminated forms or restrictive pulmonary disease. 93. Which might be the etiological factors in the background of the 4th subtype of chronic granulomatous inflammation (immun granuloma formation)? Mycobacterium tuberculosis ( tuberculosis) Mycobacterium leprae ( leprosy) Treponema pallidum ( syphilis) Afipia felis (Bartonella henselae) (cat scratch disease) – bacterial infections Shistosoma (shistosomiasis) Tenia solium / saginata (Cysticercosis) Dirofilaria repens (filariasis) – protozoonal infections Cryptococcus (Cryptococcosis) – mycotic infections. etc. (The list is not a complete one). 94. Which are the epidemiological hallmarks as well as the causative agents of the tuberculosis? Epidemiology: 1/3 of the population of the globe gets infected with the microbial agent(s) (it does not equal with active disease). Three (3) millio peoples die yearly from tuberculosis – an infection disease with the highest mortality (if those AIDS patients dying from tuberculosis are also included). In healthy individuals both natural immunity by subclinical infection or vaccine induced immunity may exist. In spite of it, active tuberculosis may develop in low social-economic conditions, with decreasing immune surveillance (poverty, starvation, devastaging diseases, etc.). This is also proved that there is no relationship between incidence of tuberculosis disease of various geographical areas with or without administration of the BCG vaccination. There is no BCG vaccination in the USA, in GB vaccination is given at age of 14 provided that no natural immuniy has developed by that age. In Hungary, similar to many other European countries, vaccination starts at the first week after birth. The incidence was 8, 9 and 103 %000 in 2000 in the USA, England and Honkong, respectively, this data correspond in Hungary 96 (1970), 34 (1990), 42 (1996) and 18 %000 (2010). The incidence of tuberculosis is permanently increasing from 1980 due to AIDS disease as well as the multidrug resistant tuberculosis strains. Etiology: The disease is caused by the Mycobacterium strains. Mycobacterium tuberculosis (most frequent, usually airborn infection)

Veterinary control of the dairy herds as well as pasturization of the milk eliminate this source of infection. Mycobacterium avium and Mycobacterium intracellulare cause infection in the immunsuppressed individuals. DR (drog resistant) or XDR (extreme drog resistant) strains (being resistant for one or more than one chemotherapeutical agents) cause the largest problems. In Middle Asia these strains cause 80 % of the tuberculosis infections, in Hungary this ratio is 5 %. 95. What is the pathomechanism of the inflammatory change characteristic for tuberculosis? A Mycobacteriumnak nincs exotoxinja, endotoxinja vagy histolyticus enzimje.Viszont több, a pathogenesisben is lényeges molekulával bir. - Cord-faktor (cord= kötél, zsineg, zsinór). Csak a virulens törzseknél előforduló felszini glikolipid, mely szerepet játszik abban, hogy a kórokozó in vitro jellegzetesen szerpentin szerűen rendezett vonalak mentén nő. Egészseg egyénben történő injektálása granuloma képződést indukál. - Mycosid. Szulfatált felszini glikolipid, mely megakadályozza a phagosoma – lysosoma fúziót. - Lipoarabinomannan – LAM (major heteropoliszacharid – MHP), mely fokozza a macrophágok IFN-γ általli aktiválódását, Gram- bakteriumok endotoxinjához részlegesen hasonló hatású, a macrophágokban TNF-α termelést indukál, mely citokin lázat, szövetkárosodást és súlyvesztés okoz. - Mycobacterium 65 kD-os hő-sokk fehérjéje, mely rendkivül immunogen, homológiát mutat a humán megfelelővel és III. tipusú HRS reakció révén autoimmun jellegő mechanizmussal hozzájárulhat a szövetdestrukcióhoz. Pathogenetikai képességét annak köszönheti, hogy - bár a phagocyta sejtek képesek az opszonizált LAM receptoron keresztül internalizálni a Mycobacteriumot - a phagosoma és a lysosoma fúzió gátlásának képessége révén el tudja kerülni a granulocyták ill. macrophágok általi elölést, a Mycobacterium endogen urease aktivitása révén nem alakul ki savi milliő. A legkoraibb fázisban a bakteriális behatolás helyén ebben az esetben is granulocytás beáramlást látunk (exudativ fázis), de a granulocyták elhalnak. A második vonalban macrophagok is csak phagocytálni tudják a kórokozót, de elpusztitani nem. Mégis sikeresebbek: i.c. bakterium proliferáció mellett részleges degradáció, Mycobacterium specifikus antigének MHC-II-n keresztüli prezentációja és TH sejtek aktiválódása, a macrophagok IL-12 termelése által a TH0 sejtek TH1 sejtekké történő differenciálódása megy végbe. A folyamatos MHC-II – TCR stimuláció eredményeként IL-2 / IL-2R vezérelt autokrin TH1 stimuláció, IFN-γ szekréció, macrophág stimuláció, effektivebb NOtermelésre képes epithelioid sejt irányú transzformáció ill. sejtfúzió révén többmagvú óriássejt (Langhans-sejt) képződés történik. Ez a macrophágokból, epithelioid 74
TÁMOP-4.1.1.C-13/1/KONV-2014-0001 sejtekből, Langhans sejtekből és lymphocytákból álló többgócú sejtgyülem, mely az expozició után kb 2-3 héttel alakul ki, még nem tartalmaz necrosist., ezért proliferativ (Schüpfel) gümő-nek hivjuk. A centrális necrosis (caseatio) a 3-4. hétre alakul ki azáltal, hogy a T8 sejtek a Mycobacteriumokat tartalmazó, antigénjeiket expresszáló macrophágokat előlik, savi pH alakul ki, melyben a Mycobacteriumok nem szaporodnak, de életben maradhatnak. Igy alakul ki a necrotizáló / casealo gümő, mely centrális sajtos necrotikus masszából és körülötte koszorúban elhelyezkedő epithelioid sejtekből, kevés macrophágból, Langhans sejtből és lymphocytából áll. A mikroszkópos méretű csomókba, granulomákba rendezett szöveti válasz nagyobb csomókba (tuberculum) olvadhat össze, innen ered a betegség elnevezése. 96. Which are the clinicopathological entities of tuberculosis? Primary tuberculosis. It develops in individuals who have not been exposed with Mycobacterium and do not have an immunity. I.A. Primary complex (complexus primarius) formation. The change develops at the site of the primary infection. The complex is composed from the tuberculotic lesion of the tissue as well as that in the regional lymph node. Ranke-Ghon complex. By far the most common form. Following airborn infection, a subpleular pulmonary lesion (usually in the middle or lower lobe) as well as a hilar lymph node involvement develops. The potential outcomes are the followings. Secondary healing – fibrosis - dystrophic calcification follow. In the otherwise healthy individuals who have not been vaccinated by BCG, such subclinical infection results in the development of natural immunity. Living Mycobacteria may remain even in the fibrotic and calcified nodules. Epituberculosis. It means an atelectasia due to the compression of a broncus via the hilar lymphadenomegaly. Primary caverna. Larger subpleular lesion with cheesy – necrotic centre. Source of early generalization type of disorders. Tabes mesaraica. Primary complex formation in the Peyer’s plaques and the mesenteric regional lymph node following GI infection by M. bovis. Scrophulosis. Primary complex formation in the tissue of the pharynx / Waldeyer’s ring as well as in one of the cervical lymph node. I.B. Early generalisation disorders. They develop in the background of a still active primary complex due to endogenic, lymphohaematogenic re-infection. They are frequently fatal. Tuberculosis miliaris pulmonum. The tuberculotic infection via lymphatic spreading from the primary infection through d. thoracicus (colonization on the Weigert’s intima plaque), right side of the heart and pulmonary arteries causes a bilateral, disseminated, multifocal, miliary tuberculosis (miliary = of millet size). If the pulmonary tissue lesions are larger, the conditions are called

TÁMOP-4.1.1.C-13/1/KONV-2014-0001 bilateral, disseminated small / large nodular pulmonary tuberculosis, in the latter one with frequent secondary caverna formation. Meningitis basilaris tuberculosa. The infection spread from the primary complex via the pulmonary veins and the left side of the heart to the systemic circulation. Colonisation occurs typically in the subarachnoideal space in the fossa interpeduncularis with a risk for arrosional type of haemorrhage. It might be accompanied with miliary tuberculosis of the parenchymal organs (Tuberculosis miliaris lienis, hepatis, renis et medullae rubrae ossium. Pneumonia alba. This change develops due to the bronchoalveolar spread of the infection from the primary complex followed by the deposition of the caseous - necrotic mass in the small airways and airspaces. This imparts the whitish colour of the lesional tissue. Landouzy typhobacillosis. In immunosuppressed patients no granuloma formation takes place in response to the Mycobacterium infection due to the insufficient type IV HSR. Instead, lymphohaematogenic dissemination without granuloma formation, but with tissue necrosis – essentially – Mycobacterium sepsis – develops. I.C. Late generalization disorders. They represent isolated organ tuberculoses arising usually years after the primary infection in the background of a ’healed’ primary complex. Pathogenetically, it may arise by low copy number spread from the primary complex still active – with late manifestation, or spread of the Mycobacterium still alive in the ’healed´ primary complex. These disorders are consistent with endogenic reinfection by lymphohaematogenic spread. Tuberculosis fibrosa densa pulmonis (auscultation of the Krönig’s space) Tuberculosis fibrosa diffusa pulmonis Peritonitis tuberculosa Tuberculosis peritonei Pericarditis tuberculosa Salpyngoophoritis tuberculosa Orchidoepididymitis tuberculosa Tuberculosis renis / suprarenalis Spondylitis tuberculosa – abscessus frigidus Any of the isolated, late organ tuberculotic forms might be sorce of the frequently fatal, early generalisation disorders (see above)! Secondary tuberculosis It develops in individuals with previous exposition with Mycobacterium, thus with immunity, in the background of declining immunsurveillance and exogenous re-infection by a virulent Mycobacterium via the airways. Isolated pulmonary tuberculosis arises starting with a subclavicular pulmonary infiltrate (Asman-Redecker’s infiltrate), spreading in an apico-caudal direction and by ulcero-cavernous way (tertiary caverna), leading to a full devastation of the lung tissue (Phthisis pulmonis). Typical fatal complication is the arrosion bleeding of the vessels bridging the cavernas. There is a mutual exclusion for the co-existance of the secondary tuberculosis as well as that of the various forms of the late generalizations of the primary tuberculosis. 76

97. What are the diagnostic procedures to detect Mycobacterium? How the immunity against Mycobacterium can be raised and clinically investigated? What is the significance of the immunity against Mycobacterium in the course of subsequent exposition to Mycobacterium? In cytological preparations or in formol-paraffin slides, Mycobacterium can be detected by Ziehl-Neelsen reaction. First, the preparation is stained with basic fuchsin (triphenyl-methane) containing carbolic acid. After differentiation, done in ethanol with 10% HCl, only the Mycobacteria retain the stain (deep red – purple coloured curved rods), which explain the name: ’acid resistant rods’. This histochemical stain has a very high specificity, but a low sensitivity (reliable at the bacterial density of 104 – 106 / ml ). Microscopically the fluorescent Auramin labeling is more sensitive.. A tenyésztés nehézkes és hosszú időt (heteket) vesz igénybe , de szenzitivebb (102 csira/ml), a tesztmintával történő állatoltást (10 csira/ml - tengerimalac) ma már nem használják. A Mycobacterium modern laboratóriumi diagnosztikája PCR alapú, mely 1 csira/ ml érzékenységgel bir. Immunitás természetes úton, szubklinikai fertőzés (primér komplex képződés) révén vagy attenuált Mycobacteriummal (Bacillus Calmette-Guerin) történő vakcinációval nyerhető ( történekem: lübecki katasztrófa – 1941). Az immunitás fennálta a Mantoux próbával vizsgálható ( alkar börébe fecskendezett tisztitott Mycobacterium fehérje – PPD /purified protein derivate/ által okozott reakció). A Mantoux + állapot immunitást és nem aktiv betegséget jelent, a Mantoux- állapot anergiát jelez, revakcinációt indikálhat és fogékonyságot jelent Mycobacterium fertőzésre. A BCG vaccináció jelentősége állandó és globális vita tárgya (l. 94. pont). A vakcinációt a postpartum időszakban alkalmazó országokban azzal érvelnek, hogy ez a gyermekkori, általában fatális korai generalizációs kórképek kalakulásának valószinűségét csökkenti - , mások ezzel nem értenek egyet. 98. What the ’ex juvantibus’ treatment implies? A fentiek alapján nyilvánvaló, hogy a Tuberculosis rendkivül színes kórképegyüttes, sokféle szervet érinthet, ezen klinikai körülmények valamint a diagnosztikus nehézségek miatt is gyakran nagyon nehéz az egyértelmű kóroki klinikai diagnozis. Mindezek miatt, ha Tuberculosisra van gyanú, de nincs kóroki mikrobiológiai – pathologiai diagnosis, viszont más kórkép kizárható, a nagyobb baj kerülése érdekében jogos ex juvantibus antituberculotikus kezelést alkalmazni és a klinikai kép változásának függvényében azt folytatni ( juvare (l) = segit, juvans = segitő, juvantibus: többes, részeseset, ex = ból, ből, vminek az alapján; ex juvantibus: ’segitségnyujtás (mint jogosultság) alapján’ végzett orvosi beavatkozás).
