Een belangrijke activiteit in het licht van deze missie is het in kaart brengen van de omvang en verspreiding van beroepsziekten in Nederland, alsmede het signaleren van trends. De gegevens die de registraties van het NCvB opleveren en de interpretatie daarvan, worden beschreven in het Signaleringsrapport Beroepsziekten. Dit rapport wordt jaarlijks opgesteld in opdracht van het Ministerie van Sociale Zaken en Werkgelegenheid. The data obtained from the registrations of the NCvB and their interpretation are described in the Alert Report on Occupational Diseases. This report is drawn up every year by order of the Ministry for Social Affairs & Employment. This data can contribute to a policy of prevention by the government, social partners and Occupational Health Services.
Het NCvB participeert samen met de Kenniscentra, Medwerk en STECR (Stichting Expertisecentrum Reïntegratie) in het kennisnetwerk arbeidsrelevante aandoeningen. In het verslagjaar was het Medwerkconsortium in opbouw. Een samenwerkingsproject van het kennisnetwerk is de helpdesk voor arbeid en gezondheid. Het NCvB geeft inmiddels invulling aan de gezamenlijke frontoffice. Hiervoor is subsidie verleend door ZonMw (Zorgonderzoek Nederland-Medische wetenschappen). Het NCvB heeft een spilfunctie in het netwerk van deskundigen op het terrein van de klinische arbeidsgeneeskunde. Het Solvent Team project voor de diagnostiek en begeleiding van patiënten is een voorbeeld van een multidisciplinaire benadering van arbeidsgeneeskundige problematiek in een klinische setting.
Together with the Knowledge Centers, Medwerk and STECR (Expert Reintegration Studies Center), NCvB participates in the national knowledge network for work-related diseases. The Medwerk consortium was in preparation during the reporting year. The helpdesk for work and health is a collaborative project of the knowledge network. The NCvB has already given substance to the joint front office. ZonMw (Council for Medical and Health Research) granted subsidy for this. The NCvB plays a pivotal role in the network of experts in the area of clinical occupational medicine. The Solvent Team project for the diagnosis and rehabilitation of patients is an example of a multidisciplinary approach to occupational medicine in a clinical setting. The NCvB had three areas of activity in 2001: the

Melding en registratie Een centrale activiteit van het NCvB is de organisatie van de landelijke melding en registratie van beroepsziekten door Arbo-diensten. Het NCvB is hiertoe aangewezen door het Ministerie van Sociale Zaken en Werkgelegenheid. Doelstelling van de registratie is het verkrijgen van inzicht in het vóórkomen en de verspreiding van beroepsziekten binnen bedrijfstakken en beroepsgroepen en verschuivingen en trends hierin. Ook het genereren van signalen (‘alerts’) over beroepsziekten of over knelpunten in de preventie of zorg is een belangrijke functie van het registratiesysteem. Een groep meldingen of zelfs een enkele melding kan aanleiding zijn een verdiepingsproject te starten. Verdiepingsprojecten leveren extra kennis op over een specifieke beroepsziekte.
Notification and registration One of the NCvB’s main activities is the organisation of notification and registration of occupational diseases in the Netherlands by Occupational Health Services. The NCvB was appointed to do so by the Ministry for Social Affairs & Employment. The purpose of registration is to obtain insight into the occurrence and spread of occupational diseases within industrial sectors and occupational groupings, and the shifts and trends therein. Another important function of the registration system is the generation of warning signs (alerts) on occupational diseases or on bottlenecks in prevention or care. A group of notifications or even a single notification can be reason to launch an in-depth project. In –depth projects provide additional knowledge on a specific occupational disease.
Het NCvB heeft registratierichtlijnen opgesteld als ondersteuning voor bedrijfsartsen die beroepsziekten melden. Deze registratierichtlijnen staan vermeld op de website van het NCvB. Beroepsziekten kunnen gemeld worden middels een formulier, via de website of via een meldingssysteem van de Arbo-dienst zelf. Naast het centrale registratiesysteem door Arbodiensten heeft het NCvB een aantal peilstations opgezet of is voornemens deze op te zetten. Deze registraties geven aanvullende informatie over het vóórkomen en de verspreiding van beroepsziekten en verschuivingen en trends hierin.
The NCvB has drawn up registration guidelines to support the occupational physicians who notify occupational diseases. These registration guidelines can be found on the NCvB’s website. Occupational diseases can be notified by means of a form, the website or through a notification system of the Occupational Health Service itself. In addition to the central registration system through Occupational Health Services, the NCvB has established or has plans to establish a number of monitoring projects. These registrations provide additional information on the occurrence and spread of occupational diseases and the shifts and trends therein.

De participatie van Arbo-diensten en bedrijfsartsen aan het registratieproject is nog verre van volledig. Het NCvB heeft een onderzoek uitgevoerd naar de belemmeringen voor het melden van beroepsziekten door bedrijfsartsen. De belangrijkste belemmeringen die uit dit onderzoek naar voren kwamen zijn samengevat in tabel 1.
Notification and registration through Occupational Health Services Participation of Occupational Health Services The participation of Occupational Health Services and occupational physicians in the registration project is still far from complete. The NCvB has conducted research into the obstacles in the notification of occupational diseases by occupational physicians. The most important obstacles illustrated by this research are summarised in Table 1.

Het onderwijs voert de ranglijst aan van sectoren waarin werkgebonden psychische aandoeningen worden gemeld. De werkdruk en de emotionele belasting zijn er hoog. Infectieziekten en stemproblemen zijn andere voorbeelden van beroepsgerelateerde gezondheidsproblemen in deze sector. The education sector tops the list of sectors in which work-related psychological problems are notified. The pressure of work and the emotional stress are high in this sector. Other examples of work-related health problems are infectious diseases and voice problems.

Om de participatie te bevorderen heeft het NCvB een aantal activiteiten ondernomen: 1. Intensieve instructie en voorlichting aan bedrijfsartsen Er is een nieuwe brochure en registratiemap voor bedrijfsartsen gemaakt. Deze wordt uitgereikt tijdens voorlichtingsbijeenkomsten. In 2001 zijn 44 voorlichtingsbijeenkomsten georganiseerd. Ook genoemd dient te worden dat drie nascholingsmodules samen met de NSOH (Netherlands School of Occupational Health) zijn ontwikkeld en dat er een stevige verankering is in het basiscurriculum van de NSOH en de SGBO (Sociaal Geneeskundig Bedrijfsartsen opleiding) Nijmegen.
The NCvB has undertaken a number of activities to encourage participation: 1. In 2001 zijn 5593 meldingen verricht door 981 artsen. Het aantal meldingen is dus iets teruggelopen vergeleken bij 2000. Tabel 2 geeft een overzicht van het aantal meldingen, het aantal meldende bedrijfsartsen en het uitvalpercentage in 1999, 2000 en 2001.

Het peilstation wordt uitgevoerd in nauwe samenwerking met het Kenniscentrum voor arbeidsrelevante huidaandoeningen Nederlands Kenniscentrum ArbeidsDermatosenNECOD. Contact met de deelnemende dermatologen wordt onderhouden via een nieuwsbrief en een jaarlijkse bijeenkomst. Tabel 5 geeft een overzicht van de meldingen in 2001.

ArbeidsDermatosen Surveillance-ADS (Occupational Dermatoses Surveillance) The measurement station for occupational skin diseases (ADS) was launched in 2000. A select group of dermatologists send a card to the NCvB stating the workrelated skin diseases they have identified in the previous month. The measurement station is operated in close collaboration with NECOD, the Netherlands Expert Center for Occupational Dermatoses. Contact with the participating dermatologists is maintained by means of a newsletter and an annual meeting. Table 5 shows a summary of the notifications in 2001.

Measurement Station for Work-related Lung Diseases
In analogie met het ADS is in juni 2001 het peilstation voor arbeidsgebonden longaandoeningen van start gegaan. Het peilstation wordt uitgevoerd in nauwe samenwerking met het Nederlands Kenniscentrum Arbeid en Longaandoeningen-NKAL. Ondanks een aantal aanloopproblemen is inmiddels wel een netwerk van longartsen opgezet met interesse voor arbeidsgebonden luchtwegproblematiek.
Analogous to the ADS, the measurement station for workrelated lung diseases was launched in June 2001. The measurement station is operated in close collaboration with the Knowledge Center for Work-related Lung Diseases (NKAL). In spite of a few initial problems, a network of lung specialists with an interest in work-related respiratory problems has been established.
European cooperation
Europese samenwerking Het NCvB heeft intensieve contacten met buitenlandse instituten en internationale verbanden die zich bezig houden met beroepsziekten. Het NCvB participeert het Eurostatproject ‘European Occupational Diseases Statistics’ (EODS), dat tot doel heeft te komen tot betere afspraken over criteria en codering van beroepsziekten in Europese landen. Dit moet uiteindelijk beter vergelijkbare beroepsziektestatistieken opleveren. Het NCvB levert hiervoor gegevens aan. Ook participeert het NCvB in een werkgroep van het ‘European Forum of insurances against accidents at work and occupational diseases’. Deze werkgroep heeft als taak vergelijking van beleid ten aanzien van signalering en preventie van beroepsziekten tussen de verschillende Europese landen.
The NCvB has intensive contacts with foreign institutions and international associations involved with occupational diseases. The NCvB participates in the Eurostat project ‘European Occupational Diseases Statistics’ (EODS), which aims to realise better agreements with regard to criteria and coding of occupational diseases in European countries. This should ultimately result in occupational disease statistics which can be compared more easily. The NCvB provides data for this. The NCvB also participates in a working group of the European Forum of insurances against accidents at work and occupational diseases’. The task of this working group is to compare differences in policy with regard to identification and prevention of occupational diseases between the different European countries.

De werkbelasting in de horeca is vaak hoog met lange en onregelmatige werkdagen. Stress en klachten van het bewegingsapparaat doen zich vaak voor in deze sector. Ook huidklachten door veelvuldig contact van de handen met water en voedingsmiddelen zijn een bekend probleem in deze sector. Het NCvB participeert samen met de Kenniscentra, Medwerk en STECR in het kennisnetwerk arbeidsrelevante aandoeningen. In het verslagjaar was het Medwerk-consortium in opbouw. Een samenwerkingsproject van het kennisnetwerk is de helpdesk voor arbeid en gezondheid. Het NCvB geeft inmiddels sinds 1 april 2002 invulling aan de gezamenlijke frontoffice. Hiervoor is subsidie verleend door ZonMw.

The helpdesk for work and health is a collaborative project of the knowledge network. Since 1 April 2002, the NCvB has already given substance to the joint front office. Klinische arbeidsgeneeskunde Het NCvB wil een actieve rol spelen in de ontwikkeling van een infrastructuur voor de klinische arbeidsgeneeskunde in Nederland. Klinische arbeidsgeneeskunde kan gestalte krijgen in de vorm van multidisciplinaire teams voor diagnostiek en behandeling van specifieke beroepsziekten en in de vorm van arbeidsgeneeskundige consultatie in de curatieve zorg. Als voorbeeld van een multidisciplinaire benadering in de klinische arbeidsgeneeskunde functioneert binnen het NCvB het Solvent Team project, gericht op de diagnostiek en begeleiding van patiënten met Chronische Toxische Encephalopathie (CTE) ten gevolge van oplosmiddelen. In het AMC bestaat een klinisch arbeidsgeneeskundige consultatiefunctie bij de afdeling longziekten. Met de afdeling klinische audiologie van het AMC is het expertisecentrum ‘Gehoor en Arbeid’ opgericht. Het NCvB is eveneens initiatiefnemer voor de werkgroep ‘allergische beroepsziekten’, die tweemaal per jaar bijeenkomt, en participeert in de werkgroep ‘bedrijfsarts in de kliniek’ van de Nederlandse Vereniging voor Arbeids- en Bedrijfsgeneeskunde (NVAB).
Clinical Occupational Medicine The NCvB wants to play an active role in the development of an infrastructure for clinical occupational medicine in the Netherlands. Clinical occupational medicine can take shape in the form of multidisciplinary teams for the diagnosis and treatment of specific occupational diseases and in the form of occupational-medical consultation in curative healthcare. An example of a multidisciplinary approach in clinical occupational medicine is the Solvent Team project within the NCvB, which is aimed at the diagnosis and supervision of patients with Chronic Toxic Encephalopathy (CTE) caused by exposure to solvents. The AMC (Academic Medical Center in Amsterdam) has a clinical occupational medical consultation function in the department of pulmonary diseases. The expertise center for Hearing and Work has been established with the AMC’s department of clinical audiology. The NCvB is also taking the initiative in the working party on ‘allergic occupational diseases’ which meets twice a year, and participates in the working party on ‘the occupational physician in the clinic’ of the Nederlandse Vereniging voor Arbeids- en Bedrijfsgeneeskunde (the Dutch Society for

Solvent Team project The aim of the Solvent Team is to provide effective assessment and rehabilitation of patients with possible chronic toxic encephalopathy (CTE). Research and evaluation takes place as part of the patient care function. The project is carried out in collaboration with the Medisch Spectrum Twente in Enschede (led by Dr. G. Hageman, neurologist). The faculty of neurology and psychiatry of the AMC and the IRAS Institute (Dr. Ir. H. Kromhout) of the University of Utrecht are also closely involved in the research projects. There is an observable trend of occupational physicians more frequently being the primary referrer. The referral indication involved exposure to solvents in 96% of the cases.

Beschouwing en een blik vooruit De afgelopen jaren is door het NCvB veel energie geïnvesteerd in de opbouw van het centrum, het ‘upgraden’ van het registratiesysteem en het eigen kennisnetwerk. De komende jaren zullen deze investeringen ten goede moeten komen van de doelgroepen. Dit betekent als het ware een kanteling naar meer gerichtheid op de doelgroepen. Tevens zal het NCvB zich de komende jaren richten op verbetering van de kwaliteit van registraties, de analyse van de gegevens en de presentatie daarvan aan de doelgroepen. Hieronder worden per activiteitsgebied in grote lijnen de plannen van het NCvB voor de komende jaren aangegeven.
Consideration and prospects Over the last few years the NCvB has invested a great deal in building up the center, upgrading the registration center and its own knowledge network. In the future these investments will have to benefit the target groups. This means a shift, as it were, towards a greater bias towards the target groups. In addition, in the next few years, the NCvB will focus on improving the quality of registrations, the analysis and the presentation of data to the target groups. The NCvB’s plans for the next few years are outlined by activity area below.
Notification and registration
Melding en registratie Het NCvB onderhoudt een aantal databases over beroepsziekten die in omvang snel toenemen. Voor de registratie van beroepsziekten die gemeld worden door Arbo-diensten is een apart systeem gebouwd. De verwachting is dat de hoeveelheid informatie die het NCvB te verwerken krijgt zal toenemen in de toekomst. Om beleidsrelevante informatie te kunnen genereren dient de analyse, interpretatie en presentatie van gegevens verder verbeterd te worden. Hiervoor is goed ‘datamanagement’ nodig. De wensen van de doelgroepen zijn bepalend voor welke output geleverd wordt. De intentie is dat de databases, uiteraard voor zover deze geen privacygevoelige gegevens bevatten, zoveel mogelijk via internet beschikbaar zullen zijn. Peilstations leveren additionele informatie op over beroepsziekten. In 2002 zal het NCvB starten met het peilstation intensief melden (PIM). Dit peilstation zal eveneens gegevens verzamelen over het beloop, de sociale gevolgen en kosten van beroepsziekten.
The NCvB maintains a number of rapidly growing databases on occupational diseases. A separate system has been constructed for the registration of occupational diseases notified by the Occupational Health Services. The amount of information which NCvB receives for processing is expected to increase in the future. In order to be able to generate policy-relevant information, the analysis, interpretation and presentation of data must be improved further. This requires good data management . The wishes of the target groups are determining factors in which output is provided. The intention is that the databases will be available on the Internet as much as possible, of course only insofar as they do not contain privacy-sensitive information. Measurement stations provide additional information on occupational diseases. In 2002 the NCvB will launch the measurement station for intensive notification (PIM). This measurement station will also collect data on the course, the social consequences and the costs of occupational diseases.
Knowledge center
Kenniscentrum Met de vier kenniscentra voor arbeidsrelevante aandoeningen, STECR en Medwerk heeft het NCvB een plaats in de kennisinfrastructuur voor arbeid en gezondheid in ons land. Het NCvB heeft met beroepsziekten wel een eigen aandachtsgebied. Het beleid voor kennisverspreiding van het NCvB richt zich op: – Verbeteren van de communicatie met doelgroepen over welke behoeften er leven aan informatie over beroepsziekten. – Het beter omschrijven van de producten en diensten van het NCvB en evaluatie van deze ‘services’. – Investeren in de website en afstemming met andere websites, portals en organisaties. – Intensievere communicatie met de overheid en

Het NCvB neemt deel aan en coördineert deels landelijke activiteiten op dit terrein en ondersteunt initiatieven. Tevens onderzoekt het NCvB de mogelijkheden voor uitbreiding van klinische arbeidsgeneeskunde (naast het Solvent Team project) op derdelijns niveau in het AMC, in samenwerking met klinische afdelingen van het AMC en de VU.

Communicatie is een centrale functie voor het NCvB. Via de helpdesk vindt communicatie plaats met arboprofessionals en artsen. Met de overheid en maatschappelijke partijen vindt communicatie plaats over beroepsziektestatistieken, signalen en trends. Het is van belang voor het NCvB een communicatiebeleid te ontwikkelen waarin vermeld staat wat de communicatiedoelstellingen zijn en voor welke doelgroepen, met welke organisaties kan worden samengewerkt en welke communicatiekanalen benut kunnen worden. De Kenniscentra en Medwerk hebben reeds initiatieven genomen om te komen tot een communicatiebeleid. Het ligt in de rede dat het NCvB tracht aansluiting te vinden bij deze organisaties. Tevens is de samenwerking met de bedrijfsartsenopleidingen en het Richtlijnenbureau van de NVAB van groot belang voor de verspreiding van kennis en informatie. In het communicatiebeleid zal de website een belangrijke functie vervullen. Statistieken en andere gegevens over beroepsziekten zullen gemakkelijk toegankelijk moeten zijn via de website van het NCvB. Het NCvB streeft ernaar dat het melden van beroepsziekten in de toekomst voor een groot deel via internet zal gaan.
achieving the objectives mentioned. On a national level it is very important to have a good position in the complex network of knowledge organisations in the area of Work and Health. The NCvB aims to enter into collaborative partnerships and to carry out joint projects with relevant knowledge organisations. On an international level the NCvB strives to maintain an extensive network and to participate in projects of organisation such as EODS, Euroforum, WHO (global burden of disease) and ICOH. Communication is a key function for the NCvB. Communication with occupational health professionals and physicians takes place through the helpdesk. Communication on occupational disease statistics, warning signs and trends takes place with the government and social parties. It is important for the NCvB to develop a communication strategy stating what the communication objectives and the target groups are, with which organisations it can collaborate and which communication channels can be used. The Knowledge Centers and Medwerk have already taken initiatives to achieve a communication policy. Naturally, the NCvB seeks to affiliate itself with these organisations. In addition, the collaboration with occupational physician training courses and the Dutch Society for occupational medicine (NVAB) Guideline Bureau is extremely important for the dissemination of knowledge and information. The website will fulfil an important role in the communication policy. Statistics and other data on occupational diseases will have to be easily accessible through the NCvB’s website.

Beroepsziekten in de agrarische sector blijven vaak onderbelicht omdat in deze sector veel zelfstandigen en hun gezinsleden werkzaam zijn die niet vallen onder de arbozorg. Toch kent deze sector een hoog beroepsziekterisico. Aandoeningen van het bewegingsapparaat, klachten van huid- en luchtwegen en beroepsinfectieziekten komen o.a. voor. Occupational diseases in the agricultural sector are often underreported because many self-employed persons and their families work in this sector and are not covered by Occupational Healthcare. Nevertheless, this sector is at high risk of occupational diseases. These diseases include musculoskeletal problems, skin and respiratory problems and occupational infectious diseases.

De missie van het NCvB is: “Het bevorderen van de kwaliteit van preventie, (vroeg)diagnostiek, behandeling en begeleiding van beroepsziekten en arbeidsgebonden aandoeningen”. Hiertoe ontplooit het NCvB activiteiten op drie gebieden: registratie van beroepsziekten, kennisverspreiding en de klinische arbeidsgeneeskunde. Het NCvB is een netwerkorganisatie en participeert samen met de Kenniscentra, Medwerk en STECR in het kennisnetwerk arbeidsrelevante aandoeningen. The mission of the NCvB is: “To improve the quality in the prevention, (early) diagnosis, treatment and supervision of occupational diseases and work-related illnesses”. To achieve this goal, the NCvB develops activities in three areas: registration of occupational diseases, dissemination of knowledge and clinical occupational medicine. The NCvB is a network organisation and collaborates with a wide range of organisations within the Netherlands and international.
