Pijlers van behandeling • Zuurstof • Frequent – continu bronchusverwijding per inhalatie • Systemisch corticosteroïden
Therapie Bronchusverwijding • Vernevelen (continu): 2.5 - 5 mg salbutamol aanvullen tot 4 ml met NaCl 0.9%. Iedere 10 min herbeoordelen en spraykamer opnieuw vullen. • Eerste 3x ipratropiumbromide 0.25-0.5 mg toevoegen, daarna a 3 uur
Therapie • Prednisolon po of iv: 1-2 mg/kg/dg in 2x, maximaal 40-60 mg • Magnesiumsulfaat 10% iv: 25 mg/kg (0.25 ml/kg) in 10 minuten, max. 2 gram
Therapie • Salbutamol iv – bolus 4 mcg/kg in 10 min iv – onderhoud 0.1 - 0.3 mcg/kg/min op geleide klinisch effect en HF (niet > 200/min)
• Bijwerkingen: – hypokaliemie, lactaat acidose – hypotensie, ischemie, aritmie

Vroeg starten met steroïden? • Use of corticosteroids within 1 hour of presentation to an ED significantly reduces the need for hospital admission in patients with acute asthma. Benefits appear greatest in patients with more severe asthma, and those not currently receiving steroids. Children appear to respond well to oral steroids.

Steroïden tijdens opname bij kinderen • Systemic corticosteroids produce some improvements for children admitted to hospital with acute asthma. The benefits may include earlier discharge and fewer relapses. Inhaled or nebulised corticosteroids cannot be recommended as equivalent to systemic steroids at this time. Further studies examining differing doses and routes of administration for corticosteroids will clarify the optimal therapy.

However there is no apparent reduction in symptoms, number of nebulised treatment and length of hospital stay. There is insufficient evidence to assess the impact on oxygenation, PICU admission and mechanical ventilation. Aminophylline is associated with a significant increased risk of vomiting

Plain language summary Using magnesium sulfate as well as standard drugs when hospitalised for a severe asthma attack may provide extra relief, especially when standard treatments are not working well.

Antibiotica / macrolide geven? Authors' conclusions The role of antibiotics in the treatment of acute asthma is difficult to assess from the current literature. Recommendations regarding antibiotic use in acute asthma will remain consensus driven until more research is conducted which includes larger numbers of patients.
