Empirische antibioticatherapie
Doel antibiotische behandeling • • • •
genezing van de patiënt eradicatie oorzakelijk pathogeen vermijden van transmissie naar anderen vermijden van resistentieontwikkeling
• empirische antibiotische behandeling = antibiotische behandeling die gestart wordt vóór het causale micro-organisme geïdentificeerd is
Goede antibiotische therapie: 4 D’s • Drug: juiste empirische keuze • Dose: correcte dosis aangepast aan de patiënt • De-escalation = aanpassing volgens antibiogram
• Duration: niet té vroeg maar wel tijdig stoppen

Basisvragen • Heeft deze patiënt een infectie? Is het een bacteriële infectie? • Is behandeling met antibiotica (nu) nodig voor deze bacteriële infectie? • Wat is de focus (en dus meest waarschijnlijke kiemen?) • Wat is het aangewezen antibioticum in deze patiënt?
Heeft deze patiënt een bacteriële infectie? • Koorts/inflammatie ≠ infectie antibiotica! overweeg telkens niet-infectieuze oorzaken van koorts of inflammatie!

Bedenking bij afname van kweken • een relevant staal is niet altijd beschikbaar (geen sputa, anurie,…) • patiënten werden soms voorbehandeld met antibiotica • sowieso maar in +/-50% van mensen met een infectie positief.. zo mogelijk wel, zo goed mogelijk, zo uitgebreid mogelijk
