SAMENVATTING
Hoofdstuk I . Voor de organisatie van de bestrijding van een ziekte is het nodig de aard en het verloop ervan te kennen en te weten in welke mate zij onder de bevolking voorkomt. Hoofdstuk II. Uit de bekende gegevens over de ziekten, die in het algemeen worden aangeduid met de namen 'astma bronchiale', 'astmatische (eosinofiele) bronchitis', 'chronische bronchitis', 'emfyseem' en 'bronchiectasie' wordt geen duideìijk beeld van de presentie van deze chronische aspecifieke respiratoire aandoeningen verkregen. Dit komt a. doordat de beschreven onderzoekingen plaatsvonden bij nietvergelijkbare en vaak niet-representatieve groepen van personen; (in die gevallen waar 'samples' van de gehele populatie werden onderzocht, was het onderzoek zo summier dat de resultaten tot onderschatting van de presentie leidden); b. door verschil in terminologie. Geconcludeerd wordt dat het gewenst is, dat in de vorm van een 'planned field-survey' een onderzoek wordt ingesteld naar het voorkomen van deze aandoeningen door een huisarts, die gedurende een reeks van jaren in dezelfde praktijk heeft gepraktiseerd en de beelden van deze ziekten in alle stadia heeft gezien. Hoofdstuk I I I . Het in dit werk beschreven onderzoek vond plaats in 1958 te Stadskanaal, een overwegend agrarisch dorp van omstreeks 15.000 inwoners in het noord-oosten van Nederland. Hoewel daaromtrent geen exacte cijfers voorhanden zijn, mag men aannemen dat luchtvervuiling er niet of vrijwel niet voorkwam. Enkele geologische, etnografische, hygiënische en sociale bijzonderheden over de plaats, waar de praktijk is gevestigd, worden meegedeeld. Het onderzoek vond plaats bij 2979 personen, van wie een verdeling naar geslacht en leeftijdsgroepen wordt gegeven (zie bl. 23). Daar om verschillende redenen ook een enquête werd gehouden betreffende enkele andere aandoeningen (dauwworm, urticaria, ulcus ventriculi s. duodeni, hypertensie, eczeem, migraine, longontsteking en overgevoeligheid voor voedings- en geneesmiddelen)
werden de patidnten fysisch onderzocht en werd hun een aantal vragen gesteld (zie bl. 22). De gegevens werden uiteraard getoetst' aan de ervaring die in 25 jaar in de praktijk was verkregen en konden zo nodig op grond van die ervaring gecorrigeerd worden. Sociale en economische aspecten en rookgewoonten werden niet in het onderzoek betrokken.
Hoofdstuk IV. Een summiere beschrijving wordt gegeven van de betekenisontwjkkeling van het woord 'astma'. Gezien de misverstanden en de verwarring die uit de historische belasting van het woord voortvloeien, is het noodzakelijk een nieuwe nomenclatuur t e geven. Gebruikt zal worden de term 'astma' (Cara). Chronische Aspecifieke Respiratoire Aandoeningen. Onder deze term worden de vijf onder hoofdstuk I1 genoemde ziekten begrepen. Hoofdstuk V . Beschreven worden de klinische symptomen, de patho-fysiologische gegevens, de pathologisch-anatomische feiten, de constitutionele momenten en de etiologische factoren van 'astma' (Cara). Mede op grond van de literatuur worden die patiënten geacht aan 'astma'(Cara) te lijden, bij wie worden gevonden: a. als klinische symptomen: kortademigheid (zowel in aanvallen als chronisch), hoest en sputumproduktie; b. een 'allergische' resp. 'hyperreactieve' constitutie, zich uitend in een persoonlijke anamnese van dauwworm, hooikoorts, rhinitis vasomotoria; c. een familieanamnese van 'astma' (Cara), dauwworm, hooikoorts, rhinitis vasomotoria; d. bloed- en sputumeosinofilie; e. huidreacties van het immediate type; f . overgevoeligheid voor acetylcholine en histamine; g. als provocerende oorzakelijke factoren: allergenen, prikkelende stoffen, infecties en emoties. Hoofdstuk V I . Beschrijving van de resultaten van het onderzoek. a. onder de 2979 onderzochten werden 224 patiënten gevonden, die gedurende minstens 3 maanden van 2 opeenvolgende jaren geleden hadden aan chronische resp. recidiverende kortademigheid, hoesten en opgeven.
Deze 224 patiënten werden verdeeld in: I. 138 personen, bij wie we zelf typische astmatische benauwdheidsaanvallen hadden gezien; 11.41 personen die deze benauwdheidsaanvallen in hun persoonlijke anamnese hadden, of wier bovengenoemde klachten we niet konden toeschrijven aan afwijkingen van longen of luchtwegen van bekende oorzaak. 17 mannen vrouwen Het bleek niet mogelijk deze 179 patiënten op bexrredigende wijze bij de bekende ziekte-eenheden ('astma bronchiale', 'astmatische bronchitis' en 'chronische bronchitis') in te delen. 111. 45 patiënten, bij wie we voor de genoemde klachten een 'oorzaak' vonden. Deze 45 patiënten werden niet tot de 'astma' (Cara)-lijders gerekend, hoewel mag worden aangenomen dat verscheidene van deze personen, behalve de gevonden 'oorzaak', ook een 'astmatische' constitutie hadden. b. onder de 892 personen van 0-15 jaar werden 26 patiënten (20 jongens en G meisjes) genoteerd die leden aan dauwworm. Van deze 26 patiënten hebben 10 jongens en 4 meisjes 'astma' (Cara). Bij de 2087 personen b o v e n l 5 jaar gaven slechts 5 personen aan dat ze aan dauwworm hadden geleden. 127
c. van de 892 jongens en meisjes van 0-15 jaar hadden 283 (137 jongens en 146 meisjes) geleden aan urticana. 16 van deze 137 jongens en 7 van de 146 meisjes hadden 'astmal(Cara). Er bestond geen significant verschil tussen de 'astma'(Cara)groep en de niet-'astma'(Cara)-lijders. d. van de 1853 personen (917 mannen en 936 vrouwen) boven 20 jaar leden 75 mannen d.i. 8,2 % en 10 vrouwen d.i. 1,l aan ulcus duodeni s. ventnculi. Hoewel geconstateerd werd, dat het percentage 'astma'(Cara)-lijders bij de ulcuszieken iets hoger was dan bij de niet-ulcuslijders bleek dit verschil niet significant. e. uit de opgenomen bloeddrukwaarden van de 1853 personen boven 20 jaar is geen duidelijk verschil te vinden tussen de 'astmaJ(Cara)-groep en de niet-lijders. f . onder al de onderzochten werden 3 patiënten met hooikoorts aangetroffen. g. uit de overige verkregen gegevens konden we aangaande de in hoofdstuk I11 ook nog genoemde aandoeningen geen conclusies trekken.
Hoofdstzck VII. Uit de 179 gevonden lijders werden aselect, uit de leeftijdsgroepen 7-15 jaar, 15-25 jaar en boven 50 jaar telkens 17 personen getrokken; voor elk van deze personen werd een onderzoekspartner gezocht van hetzelfde geslacht en dezelfde leeftijd, niet lijdende aan 'astma' (Cara). Deze 3 X 17 X 2 personen werden in een onderzoek, in samenwerking met een deskundig team van de interne universiteitskliniek van Groningen vergeleken: a. op het aantal eosinofiele granulocyten van het perifere bloed onder gelijke-basale omstandigheden; b. op de gevoeligheid van de huid voor huisstof, schimmels, branden en roesten, onkruiden, boompollen, voorjaarspollen, graspollen, veren en haren; c. op de volgende longfuncties: de vitale capaciteit (VC) de Cén-secondewaarde (CUS) het effect na intraveneuze injectie van histamine en acetylcholine op VC en CUS het effect na subcutane resp. intramusculaire injectie van adrenaline en thiazinamium (multergan) op de VC en CUS.
De thans gebruikelijke, betere, methode van TIFFENEAU (met de modificatie-DE VRIES) kon in 1.958 nog niet worden toegepast. Een sputumonderzoek kon helaas niet in het onderzoek worden betrokken. Het zou echter, gezien de korte duur van het onderzoek, een zeer fragmentarisch karakter hebben gehad. In de onderstaande tabel geven we de uitkomsten van de verschillende bepalingen bij de beide groepen. De significanties tussen de groepen werden - behalve voor de allergie, waarbij de X2test ( 2 x 2 velden) werd gebruikt - bepaald met de toets van WILCOXON.

Men mag uit de cijfers van deze tabel concluderen dat de in de literatuur bekende criteria voor een 'astmatische' constitutie (resp. manifest 'astmatisch' lijden) ook significant verschillend voorkomen in de beide groepen. Niet bekend is echter, in hoeverre deze kenmerken werkelijk discriminerend zijn en of dit discriminerend vermogen wordt beinvloed door geslacht en leeftijd van de betrokkenen. Ook zijn de kritische waarden niet goed bekend. De resultaten van het in deze richting ingestelde onderzoek worden in de volgende tabel gegeven. (De kritische waarden werden in tabel op bl. 110 vermeld).

Uitgaande van de klinische gegevens kan men dus vaststellen dat door de aanwezigheid van 2 positieve objectieve kenmerken een goede scheiding tot stand komt tussen 'astma'(Cara)-patiënten en niet-lijders, terwijl bij het aanwezig zijn van 3 kenmerken deze scheiding zeer goed is. De mogelijke oorzaken van het feit dat de bovenbeschreven kenmerken eerst in combinatie voldoende discriminerend zijn, worden besproken. Het blijkt: 1. dat het aantal eosinofiele ceilen in het perifere bloed zowel bij de 'astmaJ(Cara)- als bij de controlegroep met het ouder worden afneemt. Wel behoudt de eosinofielenwaarde een belangrijke discriminerende waarde; 2. dat de allergie (zoals bekend is) in de regel op jeugdige leeftijd optreedt en daarna geleidelijk verdwijnt. Bij de oudere groep (en) speelt de allergie dan ook een minder grote rol; 3. dat de methode, die bij dit onderzoek werd gebruikt, om de hyperreactiviteit te bepalen, vele nadelen heeft. Met name in de jongste groep kon op deze wijze geen verschil in hyperreactiviteit worden geconstateerd; 4. dat de reactie op medicamenten in de jongste leeftijdsgroep lager is dan in de oudste groep, terwijl in de middelste groep geen reactie werd gevonden; 5. dat bij het ouder worden duidelijk een toenemende vermindering van de longfuncties valt waar te nemen. Eerst treedt een verlaging van de Cén-secondewaarde op en later ook een verlaging van de vitale capaciteit.

Chapter I. I n planning an effective fight against a disease it is necessary to understand its nature and natura1 history and to know to what extent it is prevalent among the population. Chapter 11. From the facts known about the diseases which are generally designated by the names 'bronchial asthma', 'asthmatic (eosinophilic) bronchitis,' 'chronic bronchitis', 'emphysema' and 'bronchiectasis', no true picture of the prevalence of these chronic non-specific respiratory diseases can be obtained. The reasons for this are: a. that the inquiries described (in the Netherlands and in many other countries) were made with groups of persons which were not comparable and were frequently not representative. (In those cases in which samples of the whole population were examined, the inquiry was mostly co superficial that the results caused prevalence to be underestimated); b. that the terminology used is confusing. It is concluded that it is desirable that in the form of a 'planned field-surveyJ an inquiry should be made into the prevalence of these diseases by a genera1 practitioner, who during a period of years has worked in the Same practice and has seen the symptoms of these diseases in al1 stages. Chapter III. The inquiry described in the present work was made a t Stadskanaal in 1958. This is a predominantly agricultural village of about 15,000inhabitants in the north-eastern part of the Netherlands. Although no exact figures are available, it may be assumed that there is little or no air-pollution in this village. Some geological, ethnographic, hygienic and sociological particulars about the village where the practice is established, are given. 2,979 persons, of which a division as to sex and age groups is given, were exarnined (see page 23). As, for various reasons, an inquiry was also made int0 some other diseases (infantile eczema, urticaria, ulcus ventriculi s. duodeni, hypertension, eczema, migraine, pneumonia and hypersensitivity
to foodstuffs and drugs) the patients were physically examined and they were asked a number of questions (see page 22). The clata were naturally tested by the expenence obtained in a 25 years' practice, and, if necessary, they could be corrected on the strength of this experienee.
Social and economic features and smoking habits were not included in the inquiry.
Clcafiter IV. A brief description is given of the development of the meaning of the word 'asthma'. In view of the misunderstandings and the confusion inherent in the word it is necessary to give a new nomenclature. The term 'asthma'(Cara)-chronic non-specific respiratory diseases - wil1 be used. In this term the five diseases mentioned sub Chapter I1 are comprehended. Chapter V . The clinical syrnptoms, the patho-physiological data, the pathological-anatomical facts, the constitutional moments and 'etiological' factors of 'asthma' (Cara) are described. In conformity to the literature dealing with the subject, those patients are considered to suffer from 'asthma'(Cara), with whom are found: a. as clinica1 symptoms: breathlessness (in attacks as wel1 as chronic), cough and expectoration of sputum; b. an 'allergical' or 'hyperreactive' constitution, showing itself in a personal anamnesis of infantile eczema, hay-fever, rhinitis vasomotor; c. a family anamnesis of 'asthma' (Cara),infantile eczema, hay-fever or vasomotor rhinitis; d. eosinophilia of blood and sputum; e. skin reactions of the immediate type; f . hyperreactivity to acetylcholine and histamine; g. as provocative causal factors: allergens, irritants, infections and emotions. Chapter V I . Description of the results of the inquiry. a. among the 2,979 persons examined 224 patients were found with the clinica1 pattern, who for a t least three months of two successive years suffered from chronic or recurrent breathlessness, cough and expectoration.
These 224 patients were divided into: I. 138 persons in whom we ourselves had observed typically asthmatic attacks; 11.41 persons who had these attacks in their personal anamnesis or whose abovementioned complaints could not be attributed to deviations of the lungs or bronchia, which had a 'specific cause'. l 3females I t was clearly impossible to divide these patients in a satisfactory way int0 the familiar disease-units 'bronchial asthma', 'asthmatic bronchitis' and 'chronic bronchitis'. 111. 45 patients with whom we found a 'cause' for the abovementioned complaints. These 45 patients were not reckoned as 'asthmaJ(Cara)-patients, although it may be assumed that several of these persons, except for the 'cause found', had an 'asthmatic' constitution as well. b. among the 892 persons, aged G 1 5 , 26 patients (20 boys and 6 girls) were noted down as suffering from infantile eczema. Of these 26 patients 10 boys and 4 girls had 'asthmal(Cara).
Among the 2,087 persons, over 15 years of age, only 5 persons stated that they had suffered from infantile eczema. Apparently infantile eczema is practically always forgotten when the patient grows up. c. of the 892 boys and girls, aged &15,283 (137 boys and 146 girls) had suffered from urticaria, 16 of these 137 boys and 7 of the 146 girls suffered from 'asthmaJ(Cara).
There was not a significant difference between the group of 'asthmatics' (Cara) and the group of non-'asthmatics' (Cara). d . of the 1,853 persons (917 men and 936 women) of over 20 years of age 75 men i.e. 8,2 percent and 10 women i.e. 1,l percent suffered from ulcus ventriculi s. duodeni. Although of these ulcus-patients a somewhat higher percentage suffered from 'asthma' (Cara) then of the non ulcus-patients, this difference did not prove to be significant. e. from the recorded values of blood-pressure of the 1,853 persons of over 20 years of age there is not a marked difference to be found between the group of 'asthmaticsJ(Cara) and the group of non-'asthmatics' (Cara). f. among the persons examined 3 patients were found to suffer
from hay-fever.
g. from the rest of the data obtained we could not draw any conclusions as to the other diseases mentioned in Chapter 11. Chapter VII. From the 179 patients found, 17 persons were drawn (as a random sample) from each of the age-groups 7-15 years, 15-25 years and over 50 years; for each of these persons a controlperson of the Same sex and the Same age not suffering from 'asthma' (Cara), was selected. In collaboration with an expert team of the medical clinic of the Groningen University, these 3 X 17 X 2 persons were compared in an inquiry into: a. the number of eosiriophile granulocytes of the peripheral blood under similar basic-circumstances;
b. the sensitivity of the skin to house dust, moulds, smuts and rusts, weeds, tree pollens, spring pollens, p a s s pollens, feathers and hairs;
c. the following lung functions: the vital capacity (V.C.) the F.E.V,. the effect after intraveneus injection of histamine and acetylcholine on the V.C. and F.E.V,. the effect after subcutaneous or intramuscular injection of adrenalin and thiazinamium (multergan) on the V.C. and F.E.V. The better method of TIFFENEAU (with the DE VRIES modification), used nowadays, could not yet be applied in 1968. It is to be regretted that examination of the sputum could not be included in the inquiry either. However, in view of the relatively short duration of the inquiry, the results would have been of a very fragmentary character. I n the table below the results of our research are given. The significances between the groups, except for the allergy, for which the X2 tests (2 X 2 fields) was used determined with the WILCOXON test.

From the figures of this table it may be concluded - as rnight be expected - that the familiar criteria for an 'asthmatic' constitution
or manifest .'asthatic' suffering alco occur in both these groups with a significant difference. It is not known, however, to what extent these characteristics are reaily discriminating, and whether this discriminating capacity is influenced by the sex and the age of the persons involved. Likewise, we are not certain of the critica1 values. The results of this examination are given in the following table. (The critical values were mentioned in the table on page 110). Co, starting from the clinica1 data it can be stated that on the basis of two positive objective characteristics an effective divisim can be established between 'astha'(Cara) patients and non 'asthma'(Cara), while this division is aii the more obvious when three characteristics are found. The possible causes for the fact that the abovementioned characteristics are only sufficiently discriminating in combination, are discussed. I t appears : 1. that in the case of the 'asthmaJ(Cara) group as well as of the
control group the number of eosinophile celis in the peripheric blood decreases, as people grow older. The eosinophile value, however, retains a considerable discriminating value. 2. that, as a rule, the aiiergy - as is well-known - shows itself at an early age, increases with age until about the third decade of life and then gradually disappears. Consequently, in case of the
older age-group(s) the allergy plays a less important part. 3. that the method used in this inquiry, of determining the hyperreactivity has many disadvantages. Thus especially in the younger age-group no difference in hyperreactivity could be ascertained. 4. that in the younger age-group the reaction on drugs is lower than in the older age-group, while no reaction was found in the middle age-group. The reason of this phenomenon is discussed. 5. that, as people grow older a decrease of the lung functions clearly manifests itself. First a decrease of the F.E.V,. sets in and it is only afterwards that it is accompanied by a decrease of the vital capacity.
