LEGIONELLA PNEUMOPHILA: CURRENT DIAGNOSTIC AND THERAPEUTIC ASPECTS Dr. G. Ieven UZA

Summary Legionella pneumophila is increasingly recognized as a cause of both sporadic and epidemic communityacquired pneumonia requiring hospitalization, causing 2-16% of all cases. However, the true incidence of Legionnaires’ disease is difficult to establish. Although most developed countries conduct surveillance for infection, underdiagnosis remains a common problem owing to the lack of highly specific clinical definition and because of underuse of specific microbiological diagnostic tests. Multiplex real time formats are available that detect up to 20 different agents. Some loss of sensitivity in multiplex reactions as compared to monoreactions can be avoided by careful selection of the PCR primers. The role of PCR testing on other sample types, such as urine, serum and leucocyte samples is less clear. Definitive diagnosis of legionellosis is often based on culture of the microorganism. Legionellae can be isolated from a variety of respiratory sample types although lower respiratory tract secretions (e.g. sputum and bronchoscopy samples) are the samples of choice. The major limitation of sputum culture is however that fewer than half of the patients with Legionnaires’ disease produce sputum. Estimated sensitivities of sputum culture range from < 10 to 80% and vary according to different comparison standards and by individual laboratories. Serologic comparison of acute and convalescent serum specimens demonstrating a fourfold rise in IgG titer is very specific but paired samples, collected at an interval of 2 to 3 weeks, are required. In practice however often only one serum sample, from the acute-phase of the illness is available or the two samples are collected within a too short time interval to detect a titre rise. Since IgM antibodies usually appear earlier than IgG antibodies, the detection of IgM in serum is a widely used approach for the early serologic diagnosis of many acute infections. It should be realized that IgM antibodies are often not produced or may also appear too late in cases of Legionnaires’ disease. Solitary high IgG titers have no diagnostic meaning for an acute infection since the moment of the seroconversion is unknown and necessarily took place some time before the illness under observation started. Single high titres, for which a cutoff value has to be determined by a local evaluation, are useful only in prevalence studies among population groups. Therefore, serological testing is mainly useful as an epidemiological tool. With the advent of recently developed rapid techniques such as the immunochromatographic tests, the urinary antigen tests and particularly the nucleic acid amplification tests (NAATs) that produce results within 30 minutes or 4-5 hours, microbiologic information is becoming clinically useful. Urinary antigen test is currently the most helpful rapid test for the diagnosis of legionella infections. Several test formats have been developed, the enzyme immunoassay (EIA) format being more suited to test a larger number of specimens and taking a few hours to complete. The immunochromatographic format is better suited for single specimens, and produces a result within minutes. The major limitation of urinary antigen tests is that currently available tests are intended to detect Legionella pneumophila serogroup 1 antigen, which is the most common cause of legionella infection. The other serogroups of L. pneumophila, however, or the other species of Legionella are not reliably detected by this test although cross reactions with these species also do occur. These tests are particularly useful since culture of Legionella spp. is slow and takes 3-4 days. Legionella urinary antigen detection is frequently the first positive laboratory test in this infection. The sensitivity of the tests is 63.7 to 66.6% in unconcentrated urine and 86.6 to 88.8% after concentration of the specimen. The assay may be negative in some patients during the first 5 days of the disease and remain positive for between 6 and 14 day. For patients with mild Legionnaires' disease, test sensitivities range from only 40 to 53%, whereas for patients with severe Legionnaires' disease who needed immediate special medical care, the sensitivities reach 88 to 100%.

Nucleic acid amplification (NAAT) tests are highly sensitive and have shown to be promising for the rapid diagnosis of Legionella infections. They can be applied on respiratory samples as individual tests in a traditional format or as real time formats For the diagnosis of CAP they can be incorporated in a multiplex PCR. Multiplex real time formats are available that detect up to 20 different agents. Some loss of sensitivity in multiplex reactions as compared to monoreactions can be avoided by careful selection of the PCR primers. The role of PCR testing on other sample types, such as urine, serum and leucocyte samples is less clear. The rapid initiation of appropriate antimicrobial treatment has been shown to be a crucial factor in order to ensure treatment success. Therefore, any delay should be avoided. Initial treatment has to be empiric in all instances. Traditional approaches for the guidence of antimicrobial treatment such as the syndromatologic approach “typical” versus “atypical” pneumonia have been shown to be invalid. Instead, an approach according to the individual risk is advocated. Due to the epidemiology and clinical picture of legionellosis no randomised trials comparing the clinical efficacy of new macrolides and new quinolones have ever been performed. Only a few observational studies have compared the clinical efficacy of macrolides (not including azithromycin) and quinolones (mainly levofloxacin). Some results suggested that quinolones may produce a superior clinical response compared with the macrolides with regard to defervescence, complications, and length of hospital stay. However none of these studies were randomised trials, so biases or limitations could easily be identified. Addition of rifanpicin to macrolides during an introductory phase may be beneficial. Especially in immunocompromised patienst with severe infections who are critically ill, mortality rates are notably higher (> 20%). In these cases, a more aggressive therapeutic approach might be prudent so as to maximise outcome.

Summary Streptococcus pneumoniae is a major cause of morbidity and mortality world-wide. It is the most common cause of milder respiratory tract infections such as otitis media and sinusitis as well as to community-acquired pneumonia. However, it may also cause more severe diseases such as septicaemia and meningitis, with a risk for mortality in 20% and 30% of cases respectively. Even though these bacteria are devastating pathogens they may also be found in the nasopharynx of healthy children attending day-care canters in up to 60-70%. Still it is not fully understood how come that these bacteria sometimes cause just carriage and otherwise a severe disease with even a lethal outcome. During the last decades we have witnessed an emerging increase of antibiotic resistance in pneumococci. The frequency of resistance differs with geographic area but may exceed 50% to penicillin in some areas. Multi resistance (resistance to more than two drugs) is also emerging. Hence, new therapeutic drugs are needed and also we need preventive and prophylactic strategies that will help eradicating these pathogens. Since a long time we have used a 23-valent polysaccharide vaccine, based on 23 out of the at least 90 capsular serotypes described so far. Due to lack of efficiency especially in the risk groups, the small children, the elderly and in immunodeficient individuals, novel vaccines have been created where the polysaccharide has been coupled to a protein thereby increasing the immune response and efficacy. Several studies have shown a tremendous reduction of invasive pneumococcal disease due to the introduction of this vaccine for example in the USA. However, the number of serotypes included is limited and the serotype distribution has been shown to differ with geographic area and time point studied. Also, there have been discussions on serotype replacement issues due to a selective pressure of the vaccine. The vaccine is now been implemented in many countries in the childhood vaccination program and interestingly a herd immunity effect has been shown in the elderly after vaccination of children. Several efforts are today being made to increase the number of capsular types included in the conjugated vaccine and also novel approaches such as protein based vaccines are being developed.

Le vaccin conjugué heptavalent (PCV7) a été introduit sur le marché belge en octobre 2004 et est recommandé pour les enfants de 0 à 23 mois. Depuis le mois de janvier 2007, il est inclus dans le calendrier vaccinal et administré gratuitement. En 2006, alors qu’il n’était que partiellement remboursé par certaines mutualités, la couverture vaccinale parmi les enfants de 18 à 23 mois a été estimée à 26% à Bruxelles et à 30% en Wallonie, où 65% de ces enfants ont reçu au moins 1 dose. En octobre 2005, une surveillance postvaccinale a été mise en place pour suivre les changements épidémiologiques et l’impact du vaccin, y compris la détection de remplacement des sérotypes.
Het heptavalente geconjugeerde pneumokokkenvaccin (PCV7) werd in oktober 2004 op de Belgische markt gebracht en is aanbevolen voor kinderen van 0 tot 23 maanden. Het vaccin wordt echter pas terugbetaald sinds januari 2007. Voor 2006, toen het vaccin slechts deels door enkele ziekenfondsen werd terugbetaald, wordt de vaccinatiegraad bij kinderen van 18 tot 23 maanden geschat op 26% in Brussel en 30% in Wallonië; 65% van de Waalse kinderen kreeg ten minste 1 dosis toegediend. In oktober 2005 werd een postvaccinale surveillance opgestart om epidemiologische veranderingen en de impact van het vaccin op te volgen, waaronder de mogelijke vervanging van serotypes.

En 2002-2003, une étude d'une durée d’un an a permis d’estimer l'épidémiologie pré-vaccinale parmi des enfants de moins de 5 ans, sur base des cas rapportés par les services pédiatriques des hôpitaux belges et de leur sérotypage par le Laboratoire de Référence (KUL). La surveillance post-vaccinale est basée sur le rapportage par ces mêmes services pédiatriques et par le typage au laboratoire de référence. Les résultats de la première année (calendrier) de la surveillance (janvier-décembre 2006) sont comparés aux résultats pré-vaccinaux.
In 2002-2003 schatte een eenjarige studie de prevaccinale epidemiologie bij kinderen jonger dan 5 jaar op basis van de gevallen gerapporteerd door Belgische pediatrische ziekenhuisafdelingen en geserotypeerd in het referentielaboratorium (KUL Leuven). De postvaccinale surveillance is gebaseerd op rapporten van dezelfde pediatrische ziekenhuisafdelingen en op de serotypering door het referentielaboratorium. Gegevens van het eerste surveillancejaar (van januari tot december 2006) werden vergeleken met de prevaccinale resultaten.

En 2006, un total de 406 infections invasives à pneumocoque (IPD) a été diagnostiqué, dont 336 chez des enfants de moins de 5 ans, en comparaison aux 342 cas dans l'étude pré-vaccinale. Parmi les souches sérotypées (91% des cas), les types vaccinaux représentaient 41% des cas de moins de 2 ans (groupe cible du vaccin) contre 72% dans l'étude prévaccinale. Cependant, trois types non vaccinaux ont augmenté de manière significative, représentant 40%

L'introduction du PCV7 en Belgique a diminué de manière significative la proportion de types vaccinaux chez les enfants, malgré la couverture vaccinale basse, mais elle n'a pas diminué de manière significative l'incidence totale des IPD. L’hypothèse d’un remplacement des types vaccinaux par des types non vaccinaux doit être investigué. Cependant, le sérotype 1 a augmenté en Belgique avant l'introduction du vaccin et des fluctuations des sérotypes ont été décrites dans des pays sans introduction du vaccin. La poursuite de cette surveillance renforcée est essentielle. Les nouveaux vaccins devraient couvrir plus de sérotypes pour avoir un impact sur l'incidence des IPD en Belgique.
Door de invoering van PCV7 in België nam het aandeel van vaccintypes bij kinderen ondanks de lage vaccinatiegraad significant af maar de totale IPDincidentie daalde niet. De mogelijke vervanging van de vaccintypes door niet-vaccintypes moet worden onderzocht. Serotype 1 steeg echter al in België vóór de invoering van het vaccin en variaties van serotypes zijn beschreven zonder invoering van het vaccin. Verdere surveillance is dan ook noodzakelijk. Nieuwe vaccins dienen meer serotypes te bevatten om een impact te hebben op de IPD-incidentie in België.
Background Invasive pneumococcal disease (IPD) is a major cause of meningitis and pneumonia in children under 5 years. A Belgian study, based on the participation of pediatric wards and of the KUL Reference Laboratory in March 2002 - March 2003, revealed high IPD incidence rates in children under 5 years of age: 59.5 cases per 100 000 (1). According to a recent European review, Belgium ranked second in overall IPD incidence under 5 years, and showed the highest reported rates for pneumococcal meningitis in those aged <2 years (16.1/100 000, 2). In October 2004, the 7-valent pneumococcal conjugate vaccine (PCV7) Prevenar® has been introduced on the Belgian market and recommended by the Health Council for all children under 2 years of age and for children 24 years at risk (basic schedule with 3 doses + 1 booster or 3+1). It was not reimbursed till January 2007, but most mutualities have offered a partial reimbursement to their members since mid-2005. A survey conducted in Wallonia in 2006 estimated a vaccine coverage in the 18-24 months at 30% for a complete schedule, and 65% of children received at least 1 dose (3). In Brussels, a similar survey in 2006 found that 26% of children aged 1823 months were correctly immunized and 38% had received at least one dose (4). Since January 2007, the vaccine is included in the routine childhood immunisation with a 2+1 schedule. New vaccines with higher serotype valence have been developed and European licensing are expected in 2009-2010 (Table 2). Post-licensing surveillance of IPD is considered essential to monitor vaccine impact and identify vaccine failures. A laboratory surveillance of IPD is being carried out in Belgium since 1983 through the national Reference Laboratory (KUL-Leuven) and the sentinel laboratory network (5). The coverage of each of these systems at national level is considered as high (estimated at around 70%). However, data on clinical syndromes and vaccine status were often missing and not standardized, and typing was limited to the determination of serogroup of referred strains. Since these surveillance systems were not sufficient to estimate the impact of vaccination, a reinforced surveillance of pediatric IPD has been launched in October 2005, with the following objectives: to monitor epidemiological trends in Belgium, to estimate the impact of the conjugate 7-valent vaccine at national level (including the detection of serotype replacements), to detect possible vaccine failures and to evaluate the need for future pediatric pneumococcal vaccines.
Method This surveillance is run by the Scientific Institute of Public Health (Epidemiology Section), with the collaboration of the KUL Reference Laboratory and the Belgian pediatrician groups: the Groupement Belge des Pédiatres de langue Française (GBPF), the Vlaamse Vereniging voor Kindergeneeskunde (VVK), and the Belgian Society of Pediatrics (SBP/BVK).

Clinical categories for diagnosis were defined (Annexe 1) and kept as close as possible to those of the prevaccine study to allow for comparability of results. Data collection This active surveillance targets all children presenting with an IPD and admitted in one of the participating hospitals. Data are collected through the existing pediatrician network “Pedisurv” and from the KUL Reference Laboratory, applying as much as possible the methods carried out in the pre-vaccine study. However, no incentive has been provided to participants - except for regular feed back - and staff resources were limited. 1. Clinical data are collected by the network of pediatricians. This network is already collecting data on measles, mumps and acute flaccid paralysis (AFP) pediatric cases, and added IPD to its list of diseases in September 2005. It is based on the reporting by Belgian pediatricians, covering 40% of pediatricians at national level, and general practitioners from the region of Brussels. To improve the coverage of the network in hospitals, all pediatricians who participated in the 2002-2003 study have been contacted, proposed to join Pedisurv and asked to centralize IPD data collection for their hospital. They cover 108 pediatric wards on the 116 identified wards in Belgium. In addition, the three societies of pediatrics also stimulated their members to participate and made advertisement for the surveillance. IPD questionnaires were sent in advance to each contact pediatrician and they were given the possibility to report clinical data directly by Internet on a protected website. A reminder is sent each month to pediatricians. When IPD cases are notified but no clinical form is transmitted, the questionnaire is sent to the reporting pediatrician. When cases reported by the Reference Laboratory have not been reported by pediatricians or clinical data are missing, the pediatrician is contacted to fill in missing data. When data on the vaccine status have not been recorded by the pediatricians, parents of the case have also been contacted whenever possible, after approval of the doctor. 2. Pediatricians and microbiologists of all participating hospitals have been asked to ensure that every invasive strain is sent to the KUL Reference Laboratory. This national Reference Laboratory is receiving strains from around 100-115 Belgian laboratories. It confirms identification, determines serogroup/serotype and antibiotic sensitivity to penicillin G, tetracycline, ofloxacine, erythromycine and cefotaxime. Annual reports, information for peripheral laboratories on material required, tests performed and delays is available on the IPH website at http://www.iph.fgov.be/epidemio/labo/. The Reference Laboratory is sending recent IPD data on a regular basis to the IPH. Since mid 2006, it directly determines the serotype of pediatric cases and transmits results to the referring laboratory. Pediatricians are transmitting data on patient, laboratory testing, clinical syndrome (using standardized case definitions, Annexe 1), vaccine status, risk factors and outcome. The Reference Laboratory is transmitting data on serogroup, serotype and sensitivity testing. The IPH is centralizing data collection, checking data for relevance, completeness and coherence, matching case reports, carrying out de-duplication, analyzing data and providing feed back to pediatricians.

This study has received the approval of the Ethical Committee of the CERVA/ ISP/ IPB in February 2006. An “IPD Technical Committee” has been constituted to provide technical guidance to the IPD surveillance and validate its results (see experts in Annexe 2). It meets twice a year. Data from the first calendar year of surveillance (Jan – Dec 2006) are presented and compared with the results of the 2002-2003 pre-vaccine study.
Results From January till December 2006, a total of 406 cases of IPD have been reported in children 0-15 years. In children below 5 years of age, 336 cases were reported resulting in an estimated incidence rate of 59.2 per 100 000 children. The overall incidence did not show a significant decrease compared to the pre-vaccine study: 342 children below 5 years were recorded in 2002-2003, resulting in an incidence of 59.5 per 100 000. Age distribution Age is known for 100% of the cases. The highest incidence was among children under 2 years, as classically observed. Incidence per age group (Table 1) did not show a significant decrease compared to pre-vaccine values.

Serotypes Serotyping by the Reference Laboratory is available for 91% of the reported cases (92% < 2 years, 89% in 2-4 years and 90% in 5-15 years). Overall, the most frequent serotypes were serotype 1 (17%), 19A (14%), 14 (12%) and 7F (8%), which were responsible for 51% of typed cases (Figure 1).

The serotype distribution per age group is presented in Figure 2 under the corresponding vaccine valences. In the youngest cases (< 2 years), serotypes 19A (24%), 14 (16%), 6B (11%) en 7F (10%) are the most frequently found. Serotype 1 is the leading serotype in the older children, representing 30% of the 2-4 years and 52% of the 5-15 years.

The proportion of serotypes contained in the 7-valent vaccine is presented in Figure 2 and compared with the results of the pre-vaccine study. In the age group targeted by the vaccine (< 2 years), the proportion of vaccine types significantly decreased from 72% in 2002-2003 to 41% in 2006 (p < 0.001), suggesting an impact of the vaccine. The distributions of serotypes contained in future vaccines (10-valent and 13-valent) are also compared. An important rise of serotype 1 (included in the 10-valent) is observed in older children, while 7F (included in the 10-valent) and 19A (included in the 13-valent) increased in younger children.

Clinical syndromes The clinical diagnosis is reported for 74% of the cases (Table 3). In children below 2 years, the most frequent diagnosis is bacteremia without focus. Meningitis is also frequent (22%) and did not decrease compared to prevaccine era (15% in 2002-2003 with same case definition). In the older age groups, a higher proportion of pneumonia is observed: 72% of cases in the 2-4 years and 71% in the 5-15 years. A high proportion of pneumonia was already reported in the pre-vaccine study (45% in the 2-4 years, similar case definition).

Vaccination status was known for 218 children (54%), and 55 of those (25%) had received at least one dose of the 7-valent vaccine. Among the vaccinated cases, the vaccine schedule was complete for 64% (according to age and the 3+1 scheme in place at the study time), incomplete for 29% and unknown for 7%. Among the vaccinated cases, serotype is known in 50 cases. Three cases had a vaccine serotype (14 and 6B). Two of them had received an incomplete schedule, and vaccine status is still assessed for one case. We thus have not detected vaccine failure so far but it cannot excluded, due to missing vaccination status data. The most frequent serotypes in the vaccinated cases were the non-vaccine types 19A (28%), 7F (22%) and 1 (16%). Among children under 5 years, the proportion of serotypes 1 (14%) in vaccinated cases did not show significant differences with the proportion in non vaccinated cases, but the proportions of 19A and 7F among vaccinated cases under 5 years were significantly higher than those observed in non vaccinated cases of the same age group Evolution The evolution of IPD is known for 243 children (60%). Eleven fatal cases (4.5%) have been reported, with age ranging from 4 months to 2 years. It concerns mainly severe infections: meningitis (6 cases) and shock with or without meningitis. Seven of these fatal cases were infected by a vaccine type (6B, 9V en 23F): 5 were unvaccinated and the vaccine status was unknown for 2. Non vaccine types were found in 3 fatal cases (1, 19A, 7F). Serotype was unknown for one case.
Discussion Despite the introduction of the 7-valent vaccine in October 2004, the 2006 results did not show a decrease in overall incidence compared to the pre-vaccine period (2002-2003), suggesting that no impact of the new vaccine can be seen on the IPD epidemiology. This high incidence was not expected as the pre-vaccine study was believed to reach a higher comprehensiveness of cases due to the use of incentives and high resources to track every case. A capture-recapture study of both systems is currently undertaken by the IPH to estimate missing cases and provide better estimates of incidence rates. However, several factors can explain this finding: 1. The low vaccine coverage: only 26-30% of children in Brussels and Wallonia had received a complete schedule. 2. The vaccine had a real impact as shown by the significant decrease of the 7 vaccine types in the age group targeted by the vaccine (< 2 years): it fell from 141 cases in 2002-2003 (72%) to 85 cases (41%) in 2006 (p<0.001). But this fall could not impact on the overall incidence as non vaccine types have increased in the same period, especially serotypes 1, 7F and 19A: in children under 5 years, these 3 serotypes represented together 49 cases (18%) in 2002-2003 vs 125 cases (40%) in 2006 (P<0.001). This rise has counterbalanced the positive effect of the vaccine. 3. Serotype replacement has been suggested by studies in Alaska and Spain as a factor for the emergence of non vaccine types (6,7). However, it is too early to suggest that such mechanism is responsible for the observed rise in non vaccine types, moreover since the vaccination was not universal in 2006. Furthermore, natural fluctuations of serotypes have been shown in several long term surveillances, even if countries without vaccination programme. For instance, the Reference Laboratory has observed a serotype 1 increase in Belgium since 2000 in the 2-4 years; this has also been reported in several European countries. Nevertheless, the rise of 7F and 19A should be further monitored and investigated in our country as it has been relatively concomitant with the vaccine introduction, affects the vaccine target age group and these serotypes are more frequent among vaccinated cases than among the non vaccinated. The absence of detected vaccine failures, based on available vaccine details, confirms that the vaccine had an impact on vaccine types. However, this is based on only 54% of cases, and serotyping has not always been requested in vaccinated cases. This implies that vaccine failures cannot be discarded. Further surveillance of IPD is essential to follow-up the impact of PCV7 on the epidemiology of invasive pneumococcal disease and to assess whether pneumococcal conjugate vaccines with higher serotype coverage (10 or 13-valent) should be recommended once available in Belgium. Therefore, it is essential that clinicians invest time to describe the vaccination status of every IPD case and systematically request determination of

serotype at the Reference Laboratory. This is even more crucial since the vaccine schedule has been changed in 2007 into a shorter scheme (2 doses + 1 booster at 12 months). [email protected] [email protected] In juli/augustus 2007 vond de tweejaarlijkse ISSTDR-IUSTI-conferentie plaats in Seattle, USA (International Society for STD Research & International Union against STI). Deze presentatie brengt gegevens van de meeting. (Abstracts beschikbaar op www.isstdr.org). Prof King Holmes is één van invloedrijkste soa-experts wereldwijd. De conferentie vond plaats in zijn thuishaven, Seattle. Zijn zeventigste verjaardag werd gevierd om zijn ‘life time achievements’ te eren. We hadden het voorrecht om King Holmes te interviewen voor de Lancet Infectious Diseases. Het interview en uitspraken van King Holmes lopen als een rode draad doorheen de presentatie (King Holmes interviewed by Marc Vandenbruaene. Lancet Infectious Diseases Augustus 2007).
Hoe zullen we gonorroe behandelen binnen 20 jaar? Multiresistentie van Neisseria gonorrhoea is een majeur probleem in de soa-bestrijding. King Holmes beschrijft hoe hij het soa-domein in de jaren ’60 binnentrad gedurende de oorlog in Vietnam. Zijn eerste research voerde hij immers uit aan boord van het vliegdekschip de USS Enterprise. King Holmes:‘Ik trad het soa-domein binnen op het moment dat penicillinedoses moesten verhoogd worden door de gedaalde gevoeligheid van Neisseria gonorrhoea-stammen. Nadien zag ik een zeer snelle resistentieontwikkeling kort na de introductie van nieuwe antibioticaklassen. Momenteel hebben we fluoroquinolones, ciproxine, van de lijst genomen in de Verenigde Staten. Mocht er resistentie ontstaan tegen derdegeneratie cefalosporines, dat zou een ‘major threat’ zijn voor de toekomst van de gonorroebehandeling.’ De Wereldgezondheidsorganisatie pleit voor het behandelen van gonorroe op basis van de regionale resistentiegegevens en dus niet op basis van het antibiogram van een individuele patiënt. Een behandeling instellen gebeurt immers op klinische en anamnestisch verdenking. Dus vooraleer resistentiegegevens beschikbaar zijn. De WGO stelt dat een antibioticum niet meer bruikbaar is voor gonorroebehandeling als meer dan 5% van de stammen in een regio resistentie vertonen tegen het geneesmiddel. Ook in België is ciproxine niet meer aangewezen voor de gonorroebehandeling. In 2006 vertoonden 61% van de stammen onderzocht in het referentielaboratorium van het Instituut voor Tropische Geneeskunde in Antwerpen immers resistentie tegen ciproxine. (Noot: een referral bias is mogelijk. Slecht 50% van de gonorroestammen uit extramurale labs worden doorgestuurd naar het referentielab in Antwerpen. Mochten alle niet doorgestuurde stammen gevoelig zijn voor ciproxine, dan bedraagt de prevalentie van resistente stammen nog 30%. Dus boven de 5%-drempel van de WGO). (Zie slides: 1 & 2). Het Neisseria gonorrhoea-referentielab in Antwerpen benadrukt het blijvende belang van de kweekmethode als diagnostisch instrument. Mochten alle labs overstappen op moleculaire amplificatietechnologie, wordt gevoeligheidsbepaling en de epidemiologische opvolging van resistentieontwikkeling onmogelijk. Daarbij hebben de amplificatietests een lagere specificiteit en geven dus meer vals-positieve resultaten. Clinici kunnen in de war raken, omdat gonorroe-behandelingsrichtlijnen in België niet eenduidig zijn. Een aantal raden het gebruik van ciproxine nog steeds aan. In Seattle werd een casus gepresenteerd van een patiënt met faryngeale gonorroe met een stam die een verlaagde gevoeligheid vertoonde voor derdegeneratie cefalosporines (www.isstdr.org Robert et al. Abstract O094). De auteurs concluderen dat de casus de allereerste keer is dat verlaagde gevoeligheid tegen deze klasse van antibiotica werd gedocumenteerd. Het kan een marker zijn van beginnende resistentievorming tegen derdegeneratie cefalosporines. In de Verenigde Staten loopt er een discussie of combinatietherapie nodig is om de resistentieontwikkeling te vertragen.

Antibiotica research pipeline: eerder beperkt Zullen we binnen enkele decennia spreken over een post-antibiotica-tijdperk voor gonorroe? Resistentievorming is natuurlijk een veel ruimer probleem dan gonorroe-resistentie alleen. In het voorjaar 2007 lanceerde de Amerikaanse Centers for Disease Control and Prevention een persbericht waarin ze inderdaad stellen dat de optielijst voor gonorroebehandeling verkleint. ‘We are running out of options to treat this disease,’ klinkt de noodkreet van Dr John Douglas Jr., directeur van de CDC’s afdeling voor soa-preventie. ‘Gonorroe staat nu op de lijst van ‘superbugs’ waarvoor het aantal therapieopties gevaarlijk klein wordt,’ voegt Dr Henry Masur, president van de Infectious Disease Society of America toe. ‘Wat het probleem nog verergert is dat de research naar nieuwe antibiotica voor deze infecties afneemt.’ Tijdens de presentatie staan we kort stil bij de antibiotica-research-pipeline. Syfilisresistentie voor azitromycine: tot 88% in Ierland Penicilline is nog steeds de standaardbehandeling voor syfilis. Tijdens het interview verbaasde King Holmes er zich over dat er nog steeds geen resistentieontwikkeling is vastgesteld tegen penicilline. Een orale monodosis azitromycine is succesvol gebruikt om primaire en secundaire syfilis te behandelen (N Engl J Med 2005;353:1236-44). Snelle resistentievorming tegen azitromycine is echter gedocumenteerd. Sheila Lukehart vond met haar researchgroep een Treponema pallidum-mutatie die codeerde voor azitromycineresistentie: A2058G mutatie in het 23S rRNA gen. (Slide 3. N Engl J Med 2004;351:154-8). King Holmes: ‘We merkten dat patiënten met syfilis terugkwamen naar de soa-poli nadat ze behandeld waren met een monodosis azitromycine. Klachten en klinische tekens persisteerden en herbesmetting lag niet voor de hand als eerste oorzaak. We konden opnieuw Treponema pallidum preleveren uit de letsels. De microörganismen kweekten we dan in het konijnenmodel. Treponema pallidum kan je immers alleen kweken in konijnentestikels. Zo vonden we een mutatie die codeerde voor azitromycineresistentie. Een aantal groepen screenden dan klinische monsters op de aanwezigheid van de mutatie.’ Een groep in San Francisco vond de mutatie in 4% van de stammen in de 1999-2002. In 2003 hadden reeds 37% van de stammen de resistentiemutatie. Prevalentie van de mutatie in andere steden: Seattle 2001-2003: 13%; Dublin 2002: 88%. Op basis van deze gegevens wordt azitromycine niet meer aanbevolen voor de behandeling van syfilis in de Verenigde Staten en Europa. Er zijn grote regionale verschillen. In een klinische trial in Madagascar naar het effect van een monodosis azitromycine versus penicilline voor de behandeling van vroege syfilis, vonden de researchers de azitromycinemutatie bij geen enkele Treponema pallidum-stam: 0/103 stalen. De onderzoekers hopen dat de orale monodosis azitromycine toch bruikbaar blijft voor de behandeling van syfilis in Afrika. (Behets et al www.isstdr.org Abstract O-095).
Nieuwe variant van chlamydia trachomatis In 2006 rapporteerden gezondheidsautoriteiten in Zweden een nieuwe Chlamydia trachomatis-variant. De stam werd opgespoord na een onverwachte stijging met 25% van het aantal gerapporteerde chlamydiagevallen in de Halland regio in Zweden. De variant werd niet opgepikt door een aantal amplificatietests (Cobas Amplicor assay[t1], Cobas Taqman48 en Abbott m2000). Patiënten kregen dus een vals-negatief resultaat. Andere tests pikken de variant wel op (ProbeTec Becton Dickinson, Aptima Combo 2 Genprobe, RealArt CT Kit Qiagen). Tien tot 65% van de chlamydiagevallen in Zweden waren besmet met de nieuwe stam. De variant werd ook aangetroffen in Ierland, Noorwegen en Denemarken. Hier ging het om om een zeer laag aantal gevallen. Het ESSTI-netwerk en ECDC onderzochten het fenomeen en concluderen dat er geen evidentie is van een uitgebreide verspreiding van de nieuwe Chlamydia trachomatis-stam in Europa.

Voor België hebben we geen volledige cijfers. Tot nu toe werd de Chlamydia-trachomatis-variant niet opgemerkt in stalen getest op het Instituut voor Tropische Geneeskunde (ITG). (In routine gebeurt de ProbeTec van Becton Dickinson. Initieel positieve resultaten worden dan getest met de Roche Amplicor. Een ProbeTec positief resultaat gevolgd door een Amplicor negatieve uitslag zou kunnen wijzen op een infectie met de nieuwe Chlamydia trachomatis-variant. Het werd echter nog niet vastgesteld op het ITG). Lymfogranuloma venereum-uitbraak sinds 2003: chirurgische LGV-complicaties in de toekomst? Lymfogranuloma venereum, LGV, is een belangrijk volksgezondheidsprobleem bij MSM (men who have sex with men) in de westerse wereld. Half december 2003 rapporteerden clinici in Rotterdam een cluster van LGVgevallen aan de gezondheidsdiensten. Bijna alle patiënten waren gekende hiv-positieven. Er was een link met de MSM-horeca in Antwerpen. Sindsdien zijn er enkele honderden gevallen gerapporteerd in de Europese Unie (EuroSurveillance 2 juni 2005. http://www.eurosurveillance.org/ew/2005/050602.asp zie slide 4-6). 70-80% van de LGV-patiënten zijn gekende hiv-positieven. Het gaat om mensen met hoog risicogedrag (bezoek van darkroom; seks parties; uitwisselen van faeces via enema’s; …). LGV is een stam van Chlamydia trachomatis. Het L2-genotype veroorzaakt de huidige uitbraak. LGV spreidt via het lymfeweefsel en geeft aanleiding tot uitgebreide pathologie: genitale ulceraties, proctitis, colitis, inguinale adenitis. De zekerheidsdiagnose van LGV kan gebeuren met een chlamydia-amplificatietest gecombineerd met genotypering. Therapie: doxycycline 200 mg per dag gedurende 20 dagen. LGV kan bloedende anale letsels geven en maakt hiv-positieven tijdelijk tot ‘supertransmitters’ van hiv. In Nederland bleken 12% van de LGVpatiënten een seroconvertie van hepatitis-C door te maken. In deze populatie kan men hepatitis-C als soa aanzien (gebruik van dildo’s; bloedcontact via bloedende anale letsels; …). (Data: RIVM, Nederland. RijksInstituut voor Volksgezondheid en Milieu). Tijdens de Seattle-conferentie toonde Dr John Richens van de University College Londen, gegevens van een LGV-cohorte uit Jamaica in de periode 1952- 1958. Het ging om 104 vrouwen met rectale stricturen na het doormaken van een LGV-infectie. Dr Richens sprak de vrees uit dat in het kielzog van de huidige LGV-epidemie een ‘uitbraak’ van chirurgische complicaties kan optreden (Diseases of Colon & Rectum 1961;4:17-26; ISSTDR st Symposium, Tuesday July 31 : LGV a re-emerging infection). King Holmes: ‘Niemand heeft een pasklaar antwoord over hoe men de preventie bij MSM opnieuw moet aanzwengelen. Ik geloof dat serosorting één van de belangrijkste redenen is waarom we nu geconfronteerd worden met soa-epidemies zoals syfilis, gonorroe en LGV bij MSM. Samenvatting
L’activité humaine modifie le système climatique ainsi que la structure et le fonctionnement des écosystèmes terrestres. Ces modifications peuvent avoir de multiples impacts sur la santé humaine, via par exemple des vagues de chaleur, la pollution de l’air (notamment par l’ozone en été), une contamination accrue de l’eau et de la nourriture par des bactéries ou protozaires sensibles à la température ainsi qu’une modification de l’écologie des maladies infectieuses.
De menselijke activiteit wijzigt het klimatologische systeem evenals de structuur en de werking van de landecosystemen. Deze wijzigingen kunnen veelvuldige effecten op de gezondheid van de mens hebben, door bijvoorbeeld hittegolven, luchtverontreiniging (met name door ozon in de zomer), toegenomen bezoedeling van water en voedsel door temperatuurgevoelige bacteriën of protozoën en veranderingen in de ecologie van infectieuze aandoeningen.
Particulièrement complexe est l’impact des changements climatiques sur le risque de transmission des maladies vectorielles et des zoonoses - ces maladies pour lesquelles un agent pathogène est transmis à l’homme via un vecteur animal de l’embranchement des arthropodes (moustique, tique, mouche, …) ou par un hôte mammifère (rongeurs, …).
Bijzonder ingewikkeld is het effect van klimatologische veranderingen op het risico van overdracht van vectorziekten en zoönosen, waarbij een ziekteverwekker op de mens wordt overgedragen via een dierlijke vector van de tak van de geleedpotigen (mug, teek, vlieg, enz.) of door een gastheer onder de zoogdieren (knaagdieren, enz.).
L’émergence ou la réémergence de maladies est presque toujours liée à une combinaison de plusieurs facteurs, tels que des modifications de l’environnement physique (y compris le climat), les migrations humaines, le commerce d’espèces animales, l’intensification de l’agriculture, des comportements humains qui augmentent la promiscuité accrue avec des animaux domestiques et sauvages, et d’autres facteurs socio-économiques. Une approche multidisciplinaire de ces questions sera illustrée par quelques exemples.
Het ontstaan of het opnieuw opduiken van ziekten is bijna altijd het gevolg van een combinatie van verschillende factoren, zoals de veranderingen in het fysische milieu (het klimaat inbegrepen), de menselijke migraties, de handel in diersoorten, de intensivering van de landbouw, het menselijke gedrag waardoor de nabijheid van tamme en wilde dieren toeneemt en andere sociaal-economische factoren. Een multidisciplinaire benadering van deze kwestie zal aan de hand van enkele voorbeelden worden toegelicht.
Introduction Au cours du dernier siècle, l’humanité a transformé la planète Terre au point de menacer la production, par la Nature, de biens et services dont elle dépend. Ces changements environnementaux se sont considérablement accélérés au cours des 50 dernières années. L’activité humaine a poussé le système terrestre au-delà des frontières de son comportement naturel. Les réponses futures de l’environnement naturel aux pressions exercées par l’homme deviennent dès lors imprévisibles. Étant donné la complexité du système terrestre, des changements abrupts ne sont pas à exclure une fois certains seuils critiques dépassés, en particulier si, par exemple, des variations rapides du climat se produisaient de manière simultanée à des changements rapides dans les sphères politique, économique ou sociale. Certaines régions du monde sont plus vulnérables que d’autres.

Ces changements ne risquent-ils pas d’avoir des impacts négatifs sur la santé humaine au cours des décennies à venir ? La facture de notre modification en profondeur de l’environnement terrestre va-t-elle bientôt nous être présentée sous la forme d’une dégradation de notre bien-être et en particulier de notre santé? Selon un sondage récent, 76% des Belges estiment que la pollution de l’environnement constitue la principale menace pour leur santé.
Maladies non infectieuses La pollution de l’air a des conséquences négatives sur la santé humaine. Cette pollution est causée par des gaz ou particules ayant des propriétés irritantes pour l’appareil respiratoire, en particulier pour les asthmatiques : l’ozone, le dioxyde d’azote, le dioxyde de soufre, les particules fines en suspension d’un diamètre inférieur à 10 µm (PM10) et dont certaines sont cancérogènes, le benzène qui est également cancérogène et le monoxyde de carbone qui est mortel à forte dose. L’Organisation Mondiale de la Santé (OMS) estime que 3 millions de décès par an (soit 5 % des décès annuels dans le monde) peuvent être attribués à l'effet de la pollution atmosphérique. Ce chiffre est de 310.000 décès par an dans l’Union Européenne. En Grande-Bretagne, les décès imputables à la pollution atmosphérique sont neuf fois plus nombreux que les décès causés par les accidents de la route. Les concentrations élevées en ozone lors des canicules ont provoqué 919 morts en Flandre en 2003. L’environnement urbain a un impact sur la santé par ce qu’on appelle l’îlot thermique urbain. En moyenne annuelle, les températures sont de 0,5 à 1,2°C plus élevées dans les villes par rapport aux zones rurales voisines. Cet écart de température peut atteindre de 5 à 12°C en écarts maximaux dans certaines grandes villes. Cet effet est lié aux facteurs suivants : (i) une diminution de l’effet refroidissant de l’évapotranspiration étant donné la diminution du couvert végétal ; (ii) une diminution de l’albédo de surface et donc une augmentation de la chaleur absorbée par la surface ; (iii) une diminution de la vitesse du vent ; et (iv) la production de chaleur par l’activité humaine, notamment par le chauffage central et le conditionnement d’air des bâtiments. En Europe, la probabilité de connaître des vagues de chaleur pendant l’été a doublé au cours des dernières ème décennies suite à l’influence humaine sur le climat. Au cours de la seconde moitié du 21 siècle, ces vagues de chaleur deviendront plus intenses, plus fréquentes et plus longues en Europe. La canicule qui a sévi en Europe au cours de la première quinzaine d’août 2003 a provoqué de 22.000 à 45.000 décès supplémentaires, dont 15.000 rien qu’en France. Il est toutefois intéressant de noter que la canicule de juillet 2006, qui a certes été un peu moins sévère, n’a provoqué que 112 décès en France. Cette différence s’explique par une série de mesures préventives qui ont été prises par les autorités publiques. Les relations entre les changements environnementaux et la santé doivent donc être analysées en termes de vulnérabilité. La vulnérabilité est le risque qu’une société particulière subisse des effets défavorables liés à une série de perturbations extérieures. Elle dépend de caractéristiques de personnes ou de groupes de personnes qui leur permettent d’anticiper, faire face, résister à, et récupérer après une perturbation. Cette approche élargit donc l’analyse environnementale des changements physiques, biologiques ou chimiques d’un environnement naturel aux stratégies économiques, sociales ou politiques d’adaptation face à ces changements et à la capacité qu’a une société de répondre à l’augmentation d’un risque environnemental. La vulnérabilité face à une vague de chaleur dépend de : (i) l’exposition à ce risque : un quartier résidentiel sur une hauteur, à l’ombre d’une forêt ou le long d’un cours d’eau est moins exposé qu’un quartier urbain densément peuplé, dans une vallée où la circulation de l’air est faible ; (ii) la sensibilité à ce risque : les personnes jeunes en pleine santé sont moins sensibles à une vague de chaleur que les personnes âgées ou celles dont le système immunitaire est déjà affaibli ; et (iii) la capacité de réponse de la société: alors qu’à un certain niveau de revenu, installer un système d’air conditionné et aller à la campagne ou à la piscine les jours les plus chauds permet de diminuer le stress physiologique lié aux températures élevées, les personnes isolées, sans moyen de transport, sont sans échappatoire.
Maladies infectieuses Près de 70% des maladies humaines émergentes ou réémergentes sont d’origine animale. La surveillance des populations animales est indispensable pour une identification précoce, et donc un contrôle, de l’émergence de zoonoses. Cette surveillance est particulièrement difficile à mettre en œuvre lorsque l’infection de l’hôte animal réservoir est non symptomatique. En particulier, les changements et perturbations dans les écosystèmes qui offrent un « continuum » entre les animaux sauvages, les animaux domestiques et les populations humaines signalent une augmentation du risque d’émergence de zoonoses. Ces modifications peuvent être en partie liées au changement climatique, bien que leur cause principale est l’impact de l’activité humaine sur les milieux naturels. Une étude récente a estimé que, entre les années 2000 et 2005, 50 millions de personnes ont été affectées par les zoonoses, qui ont causé quelque 78.000 décès. L’émergence ou la réémergence de ces maladies s’explique

par l’intensification des contacts entre les hommes et les animaux domestiques ou sauvages. Ces contacts augmentent la probabilité qu’un agent pathogène parvienne à “passer” d’un organisme animal à l’homme et que, grâce à des mutations, puisse ensuite se transmettre d’homme à homme. Beaucoup de ces maladies nous viennent d’Asie et en particulier de Chine, Thaïlande et Indonésie. Ce fut le cas par exemple de la grippe aviaire et du SRAS. En Asie, la production animale a été multipliée par huit en moins de 30 ans, essentiellement pour l’exportation. Cette production est concentrée dans de grandes fermes industrielles en périphérie des villes, ce qui crée les conditions idéales pour l’émergence et la propagation de souches pathogènes sur des animaux immunodéprimés par leurs conditions d’élevage. Les hautes densités de population humaine à proximité, avec parfois de mauvaises conditions d’hygiène, favorise la transmission de ces agents infectieux à l’homme. Ces agents pathogènes se diffusent ensuite à l’échelle mondiale par les circuits commerciaux transnationaux. Dans le monde, la population de volaille est passée de 2 à 10 milliards d’individus en 30 ans.
Maladies à transmission vectorielle Alors que les maladies infectieuses sont responsables de 11 millions de morts par an (soit 19% du total des décès), les maladies à transmission vectorielle causent 1.43 millions de décès chaque année, selon l’OMS. Les maladies transmises par des moustiques et en particulier la malaria (ou paludisme) forment l’essentiel de ce total, soit 1.30 millions de victimes. La morbidité associée à ces maladies est également très importante. Dans les maladies à transmission vectorielle, l’agent infectieux est transmis d’un individu infecté à un autre individu par un hôte invertébré. Il y a donc quatre agents : (i) l’agent infectieux (virus, protozoaire, bactérie ou ver); (ii) le vecteur invertébré (mouche, moustique, tique, escargot…); (iii) l’hôte intermédiaire (animaux sauvages ou domestiques qui sont un réservoir pour l’agent infectieux) et (iv) l’hôte humain. Ces maladies sont particulièrement sensibles aux changements environnementaux car les conditions de l’environnement influencent le cycle de vie du vecteur et des hôtes. Les vecteurs animaux de l’embranchement des arthropodes n’ont pas de mécanisme physiologique de régulation de leur température corporelle. Leur survie et leur capacité à transmettre un agent pathogène dépend donc fortement de la température ambiante et de ses variations saisonnières, de l’humidité de l’environnement, et de la présence d’habitats adaptés à chaque vecteur (végétation, sol, topographie…).
Conclusion L’émergence de nouvelles maladies infectieuses est liée aux changements environnementaux suivants : (i) les changements dans l’environnement physique, aussi bien liés au climat qu’à la végétation et à l’affectation des terres; (ii) l’urbanisation qui crée une masse critique d’hôtes susceptibles ; et (iii) l’agriculture intensive, qui augmente les contacts et la promiscuité entre les hommes et les animaux domestiques. Un quatrième facteur, la mobilité croissante des populations humaines et animales, favorise la diffusion mondiale de ces maladies émergentes. Un équilibre délicat doit être trouvé entre les objectifs du développement économique, de la préservation de l’environnement naturel et de l’amélioration de la santé humaine. Les impacts de la mondialisation sur les risques d’émergence de nouvelles maladies risquent de devoir être pris en compte de manière croissante. Diminuer la vulnérabilité des sociétés humaines face aux maladies émergentes nécessite aussi de diminuer la pauvreté, qui est souvent liée aux conditions favorables aux épidémies, tout en augmentant notre capacité de ème réponse face à des épidémies par des programmes de surveillance et de vaccination. Au début de ce 21 siècle, force est de reconnaître que le développement a un coût écologique et donc sanitaire qui doit devenir un paramètre important dans les décisions tant politiques qu’individuelles.

Lyme-borreliose is een multisysteeminfectie veroorzaakt door spirocheten van de Borrelia burgdorferi sensu lato-genogroep die overgebracht worden naar de mens door teken. De laboratoriumdiagnostiek van Lymeborreliose berust vooral op serologische tests.
La borréliose de Lyme est une infection à multisystèmes causée par des spirochètes du génogroupe Borrelia burgdorferi sensu lato, transmis à l'homme par des tiques. Le diagnostic en laboratoire de la borréliose de Lyme repose surtout dans des tests sérologiques.
Vooraleer laboratoriumtests aan te vragen dient, gebaseerd op anamnese en klinisch onderzoek, een zorgvuldige schatting gemaakt te worden van de waarschijnlijkheid dat de patiënt een Borrelia-infectie heeft. De proportie fout-positieve resultaten bij patiënten met een lage pretestwaarschijnlijkheid (< 20%) voor de aandoening is onaanvaardbaar hoog.
Avant de faire la demande d’un test en laboratoire basé sur l’anamnèse et la recherche clinique, il faut réaliser une estimation précise de la probabilité que le patient ait une infection à Borrelia. Il est inacceptable que la proportion de résultats faussement positifs chez les patients avec une faible probabilité au pré-test (< 20%) pour l'affection soit aussi élevée.
Het testen van personen met een doorgemaakte tekenbeet in afwezigheid van ziektetekens is dan ook niet zinvol. Ook het bevestigen van de diagnose bij een patiënt met een erythema-chronicummigransletsel is weinig zinvol aangezien geen enkele diagnostische test een aanvaardbare sensitiviteit haalt. Bij de andere cutane, neurologische, cardiale en articulaire vormen wordt de diagnose gesteld op basis van een mogelijke blootstelling, compatibele klinische symptomen en compatibele laboratoriumdiagnostiek. Bij patiënten met neurologische of articulaire klachten bij wie twijfel blijft over de etiologie, kan een PCR die positief is voor Borrelia bijdragen tot een zekerheidsdiagnose. Een negatieve PCR sluit de diagnose echter niet uit.
Tester les personnes ayant vécu une morsure de tique en absence de signes de la maladie n’a pas de sens non plus. De plus, confirmer le diagnostic chez un patient avec une lésion d’érythème migrant chronique a peu de sens, étant donné qu'aucun test diagnostique n'atteint une sensibilité acceptable. Chez les autres formes cutanées, neurologiques, cardiales et articulaires, le diagnostic est posé sur base d'une exposition possible, de symptômes cliniques compatibles et d’un diagnostic en laboratoire compatible. Chez les patients avec des plaintes articulaires ou neurologiques et un doute quant à l'étiologie, une PCR positive pour Borrelia peut contribuer à un diagnostic de sécurité. Une PCR négative n'exclut toutefois pas le diagnostic.
Belangrijk is te benadrukken dat de diagnose van Lyme-borreliose vooral klinisch is. Positieve laboratoriumtests kunnen enkel dienen ter ondersteuning van de klinische bevindingen.
Il est important de souligner que le diagnostic de la borréliose de Lyme est surtout clinique. Les tests positifs en laboratoire peuvent uniquement servir de support aux constatations cliniques.

Inleiding Lyme borreliose is een infectie die verloopt over meerdere stadia, waarbij verschillende organen kunnen aangetast worden. De ziekte wordt veroorzaakt door spirocheten van de Borrelia burgdorferi sensu latogenogroep, namelijk Borrelia burgdorferi sensu stricto (Noord-Amerika en West-Europa), Borrelia afzelii (Westen Centraal-Europa en Rusland) en Borrelia garinii (Europa, Rusland en Noord-Azië). De overdracht van de spirocheten op de mens gebeurt door teken. De diagnose van Lyme borreliose wordt gesteld aan de hand van een zorgvuldig uitgevoerd anamnestisch en klinisch onderzoek van de patiënt (I). Laboratoriumtests kunnen aanvullende informatie geven. Voor de interpretatie van deze testresultaten moet echter steeds rekening gehouden worden met de klinische context. Een foutieve diagnose van Lyme borreliose is de meest voorkomende oorzaak van therapiefalen (2). De laboratoriumdiagnostiek van Lyme borreliose is voornamelijk gebaseerd op de serologische detectie van antistoffen tegen Borrelia burgdorferi sensu lato. Daarnaast kunnen ook moleculaire diagnose of kweek worden toegepast. In dit artikel worden eerst de belangrijkste diagnostische methoden beknopt besproken, waarna de waarde van de verschillende tests in functie van de klinische manifestaties wordt toegelicht.
Serologie De antigenen gebruikt voor serologisch onderzoek zijn volledige bacteriële extracten of recombinante antigenen. De talrijke antigenen aanwezig in ‘enzyme linked immunosorbent assay’ ELISA-tests gebaseerd op volledige bacteriën kan resulteren in een kruisreactie met antistoffen gericht tegen andere micro-organismen of weefselcomponenten. Verschillende aandoeningen kunnen aanleiding geven tot fout positieve anti-Borrelia-IgMen/of -IgG-bepalingen waaronder auto-immune aandoeningen, EpsteinBarr-virusinfectie, bacteriële endocarditis, syfilis, infecties met andere spirocheten en Helicobacter pylori-infectie (1). In tegenstelling tot de serologische reagentia gebruikt voor HIV-screening, zijn de beschikbare commerciële kits ter diagnose van de ziekte van Lyme weinig gestandaardiseerd. Het toenemend gebruik van recombinante antigenen in plaats van volledige celextracten zal de standaardisatie zeker ten goede komen. Alhoewel de drie genospecies van B. burgdorferi sensu lato gelijkaardige antigenen exprimeren, zijn er toch significante verschillen. Hierdoor moet steeds worden nagekeken of een kit wel geschikt is voor gebruik in Europa, waar antistoffen tegen de drie genospecies moeten opgespoord worden, in tegenstelling tot kits gebruikt in Amerika, waar alleen B. burgdorferi sensu stricto als verwekker van Lyme borreliose voorkomt. De immuunrespons ten opzichte van een B. burgdorferi sensu lato-infectie begint met het verschijnen van specifieke IgM-antistoffen, meestal binnen de eerste weken na de blootstelling. De aanwezigheid van specifieke IgM-antistoffen kan echter niet gebruikt worden als enig criterium voor de diagnose van een recente infectie aangezien de IgM-respons gedurende verschillende maanden of jaren kan persisteren ondanks succesvolle behandeling (3). Een maand na de infectie ontwikkelen bijna alle patiënten IgG-antistoffen. Ook deze antistoffen kunnen gedurende vele jaren persisteren. Zowel de IgG- als de IgM-antistoffen kunnen sterk onderdrukt worden of afwezig zijn bij patiënten die in het begin van de ziekte antimicrobiële therapie krijgen. Er is geen rol voor het gebruik van serologische tests om de respons op de therapie op te volgen. Ondanks de toenemende standaardisatie van de seologische tests blijft de specificieit van deze bepalingen een zwak punt. Zoals voor de meeste diagnostische tests daalt de voorspellende waarde van de test indien de probabiliteit voor de aandoening op basis van epidemiologische of klinsche gronden, laag is. Hierop steunt de aanbeveling van de American College of Physicians om serologische tests ter diagnose van de ziekte van Lyme enkel uit te voeren indien de waarschijnlijkheid voor infectie met B. burgdorferi, gebaseerd op zorgvuldige klinische evaluatie, de 20% overschrijdt (4). Patiënten met vage subjectieve klachten zoals hoofdpijn, vermoeidheid en myalgie hebben een lage pretestprobabiliteit. Een positief ELISA-resultaat in deze setting is dikwijls een fout positief resultaat en kan leiden tot foutieve diagnose en onnodig gebruik van antibiotica. De Centers for Disease Control and Prevention (CDC) raden een tweestapsteststrategie aan op alle serumstalen ter diagnose van Lyme borreliose (5). In een eerste stap wordt een eerder sensitieve test zoals een ELISA of een (IFA) „indirect fluorescent assay" uitgevoerd. Serums met een onduidelijk of een positief resultaat worden vervolgens onderworpen aan een meer specifieke test, namelijk immunoblotting. Het belangrijkste nadeel van ELISA en IFA-tests blijft een gebrek aan sensitiviteit in het beginstadium van de ziekte. Immunoblotting laat toe om antistoffen op te sporen ten opzichte van individuele antigenen van B. burgdorferi sensu lato. Zowel IgM- als IgG-immunoblot kits zijn beschikbaar, maar IgM-immunoblotting is minder specifiek dan IgG-immunoblotting. Een positieve IgM-bepaling in de afwezigheid van IgG-antistoffen, bij een patiënt met symptomen die reeds meer dan een maand aanwezig zijn, kan niet gebruikt worden om de diagnose van Lymeborreliose te ondersteunen.

Moleculaire detectietechnieken Moleculaire tests voor de diagnose van Lyme borreliose zijn niet in de handel verkrijgbaar en worden in een beperkt aantal centra uitgevoerd, waaronder de Cliniques Universitaires Saint-Luc in Brussel. Er zijn verschillende rapporten over het succesvol gebruik van de polymerasekettingreactieproef (PCR) op stalen zoals huidbiopten, bloed, cerebrospinaal en synoviaal vocht. Vanwege het verschil in de gebruikte technieken, verschillende genomische targets en beperkte klinische validatie is het gebruik van moleculaire tests voor deze pathologie omstreden. Een belangrijk probleem is de gevoeligheid van de PCR (net zoals voor de kweek) aangezien de spirocheten dikwijls slechts voorbijgaand en in gering aantal aanwezig zijn in weefsels en lichaamsvochten. Gegevens van een meta-analyse suggereren dat de klinische sensitiviteit en specificiteit van de verschillende PCR-tests gelijkwaardig zijn voor een bepaald staaltype, ongeacht de gebruikte methode of target (6). Een negatief PCR-resultaat sluit in geen enkele situatie de diagnose van Lyme-borreliose uit. Het persisteren van Borrelia-nucleïnezuren gedurende een lange periode in afwezigheid van levende spirocheten pleit tegen het gebruik van PCR voor het opvolgen van de effectiviteit van therapie.
Kweek Kweek van Borrelia-species uit een klinisch staal blijft het standaardonderzoek voor de bevestiging van Lymeborreliose. Deze techniek is nog steeds de meest specifieke bepaling ter bevestiging van een infectie. Het kweken van de spirocheten vereist echter een speciaal kweekmedium (Barbour-Stoenner-Kelly-medium of een modificatie) alsook donkerveldmicroscopie of fluorescentiemicroscopie ter opsporing van de karakteristieke spirocheten. Daarenboven heeft een kweek slechts een aanvaardbare sensitiviteit gedurende de acute fase van de infectie (eerste weken) maar niet gedurende de late fase van de ziekte. Kweek van synoviaal en cerebrospinaal vocht is niet zinvol vanwege de geringe en transiënte aanwezigheid van spirocheten in deze lichaamsvochten. De kweek wordt voornamelijk toegepast op huidbiopten van atypische erythemamigransletsels (gevoeligheid 50% voor primaire en 90% voor secundaire erythema-migranslaesies). Een andere bron voor de kweek is bloed (vereist volume: meer dan 9 ml) afgenomen gedurende de eerste twee à drie weken van infectie (vroege disseminatie, gevoeligheid 48%) (1). De kweek wordt enkel in gespecialiseerde laboratoria uitgevoerd. In België zijn ons geen laboratoria bekend waar de Borrelia kweek nog wordt uitgevoerd.
Waarde van de laboratoriumdiagnose in functie van de klinische manifestatie Asymptomatische infectie of aspecifiek ziektebeeld Het gaat hier om serologische resultaten compatibel met een doorgemaakte infectie in afwezigheid van ziektesymptomen. De diagnose is dus zuiver serologisch en heeft geen therapeutische implicaties: behandeling is niet vereist (7). Gezien de lage pretestprobabiliteit en het hieraan verbonden hoge risico van fout positieve tests is het dan ook niet zinvol asymptomatische patiënten serologisch te testen of te screenen. Ook het testen van personen met een doorgemaakte tekenbeet is in afwezigheid van ziektetekens niet zinvol. Bij vroegtijdig testen na een tekenbeet is de kans op een fout negatieve test bij een reële infectie hoog, zowel in afwezigheid als in aanwezigheid van ziektetekens. Als de test laattijdig wordt uitgevoerd in afwezigheid van compatibele ziektetekens, dan is de kans op een foutpositieve test hoog. Erythema migrans Erythema migrans is een zich over een verloop van dagen tot weken uitbreidend, meestal pijnloos, jeukloos en vlak huidletsel met centrale opklaring, dat na 3 tot 30 dagen op de plaats van de beet verschijnt. Hoewel het letsel klein begint, groeit het 2 à 3 cm per dag en is het bij diagnose bijna steeds meer dan 5 cm groot, met een mediane diameter van 16 cm. De centrale opklaring is in de meest typische letsels aanwezig, maar enkel bij minder dan 40"/a van de letsels (8), zodat centrale opklaring zeker geen strikte vereiste vormt voor diagnose. Het huidletsel is pathognomonisch voor Lyme borreliose, hoewel men de bovenvermelde variabiliteit van de letsels in acht moet houden. Soms ontstaan secundaire letsels met eenzelfde aspect. Het huidletsel kan gepaard gaan met veralgemeende symptomen (vermoeidheid, spier- en gewrichtspijnen, hoofdpijn, koorts) en regionale lymfeklierzwelling. Huidletsels die binnen enkele uren na een tekenbeet optreden, zijn overgevoeligheidsreacties en mogen niet als erythema migrans gediagnosticeerd worden.

Aangezien het huidletsel al drie dagen na de beet kan optreden, is de kans reëel dat de serologie nog negatief is bij het verschijnen van het huidletsel en ook bij vroegtijdige behandeling negatief blijft, zoals hoger werd uitgelegd. In een Franse cohorte vertoonde bij erythema migrans 76% van de patiënten IgM-antistoffen en 64% IgG-antistoffen (9). In een ander artikel werd bij een Amerikaanse groep patiënten met erythema migrans de waarde van de verschillende diagnostische tests vergeleken: de sensitiviteit bedroeg voor huidkweek 51%, voor bloedkweken 45%, voor serologie 40%, voor serologie met reconvalescent staal 66% en voor PCR op huidbiopt 64% tot 81% naargelang de gebruikte techniek (10). Een meta-analyse van PCR-technieken voor erythema migrans gaf een mediane sensitiviteit van 76% (range 59-92%) en een specificiteit van 100% (6). Histopathologisch onderzoek van het huidletsel kan compatibel blijken met erythema migrans, maar is niet diagnostisch voor de aandoening: de bevindingen zijn een perivasculair lymfocytair infiltraat en mild cutaan oedeem (1 l). Geen enkele van de diagnostische tests haalt dus een aanvaardbare sensitiviteit en het lijkt ons dan ook zinloos bij een huidletsel suggestief voor erythema migrans de diagnose met laboratoriumtests te pogen te bevestigen. Opvolging van de serologie na behandeling is eveneens zinloos, aangezien bij vroegtijdig behandelde Lymeborreliose de serologie negatief kan blijven en aangezien de antistoffen, zowel IgM als IgG, ook na behandeling langdurig positief kunnen blijven zonder verband met het ziekteverloop (12). Borrelia-lymfocytoom Een Borrelia-lymfocytoom is een solitaire, pijnloze, blauwrode nodulus of plaque op de plaats van de tekenbeet, die soms jaren kan persisteren. Dit letsel wordt gewoonlijk aangetroffen op de oorlel, de oorschelp, de tepel of het scrotum. Een compatibele serologie (met aanwezigheid van titerwijziging op opeenvolgende stalen) is vereist voor de diagnose. Histopathologisch onderzoek van het letsel toont een intens B-lymfocytair infiltraat. Kweek en/of PCR van het huidletsel kunnen positief zijn. Acrodermatitis chronica atrophicans Acrodermatitis chronica atrophicans is een chronische aandoening van de huid, meestal aan de strekzijde van de ledematen, met aanvankelijk een blauwrode verkleuring en soms een deegachtige zwelling, later schrompeling en atrofie. Vaak gaat dit gepaard met neuropathie. Een compatibele serologie (met aanwezigheid van hoge IgG-titers) is vereist voor de diagnose. In de eerder vermelde Franse cohorte vertoonde 30% IgM-antistoffen en 100% IgG-antistoffen in serum. Kweek van het huidletsel is positief in 60% van de gevallen (13). De meta-analyse van PCR-technieken gaf een mediane sensitiviteit van 84% (6). Neuroborreliose Neuroborreliose omvat craniale neuropathie, radiculoneuritis en meningo-encefalitis en kan zich manifesteren vanaf enkele weken tot maanden na de besmetting.
Craniale neuropathie Antistoffen in serum en intrathecale antistofproductie kunnen aanwezig zijn, maar niet steeds. In een Franse studie vertoonde bij vroegtijdige neuroborreliose 100% IgM-antistoffen en 94% IgG-antistoffen op serum, en 77% IgM en 100% IgG op cerebrospinaal vocht, maar de gevalsdefinities vormen in deze cohorte een selectievertekening (9). De gegevens uit deze cohorte illustreren wel mooi dat de diagnose niet steeds eenvoudig is.
Zo kan men bij een patiënt met een facialisparese met een positief serum-IgM maar negatief IgG, de diagnose neuroborreliose niet uitsluiten: de klinische context zal in dit geval de doorslag geven voor het al dan niet stellen van de diagnose. Lymfocytose op cerebrospinaal vocht is soms maar niet steeds aanwezig (14).
Radiculoneuritis Antistoffen zijn positief in serum en bij meer dan 80% van de patiënten ook in cerebrospinaal vocht. Lymfocytaire pleocytose in cerebrospinaal vocht is aanwezig bij meer dan 80% van de patiënten (14).

Meningo-encefalitis Meer dan 80% van de patiënten vertoont lymfocytaire pleocytose in het cerebrospinaal vocht bij meningitis, maar minder bij encefalitis. Antistoffen zijn positief in serum en meestal ook in cerebrospinaal vocht. Intrathecale productie van antistoffen wordt aangetoond door een verhouding van de antistoffen tot de immuunglobulinen die tweemaal hoger is in cerebrospinaal vocht dan in serum. Kweek van cerebrospinaal vocht is slechts uitzonderlijk positief, zodat dit niet zinvol is. De PCR voor Borrelia op cerebrospinaal vocht is slechts positief bij minder dan 50% van de patiënten (14) maar kan bij een verdacht beeld met niet-diagnostische serologie helpen bij de diagnose. Lyme-carditis Carditis manifesteert zich enkele weken tot maanden na de tekenbeet, meestal onder de vorm van een wisselend atrioventriculair blok en soms als prominente perimyocarditis. De antistoffen in serum zijn positief. Histopathologisch onderzoek van een endomyocardbiopt toont lymfocytaire interstitiële infiltratie met een variabele hoeveelheid necrose van myocyten, fibrose en oedeem. Zelden kan een spirocheet microscopisch worden aangetoond. Uitzonderlijk werd het organisme ook gekweekt uit biopten (15). Lyme-artritis Lyme-artritis is een mono- of oligoarticulaire artritis (de knie is het vaakst aangetaste gewricht) en evolueert bij een deel van de patiënten naar chronische artritis. Geobjectiveerde synovitis is een vereiste voor de diagnose. Er zijn bijna steeds hoge antistoftiters. In een Franse cohorte vertoonde 44% IgM-antistoffen en 92% IgGantistoffen in serum (9). De kweek van Borrelia uit gewrichtsvocht of -weefsel is uiterst zelden succesvol. De meta-analyse van PCR-technieken gaf een mediane sensitiviteit van 65% (range 23-100%), zodat ook een negatief PCR-resultaat de diagnose niet uitsluit (6). In twijfelgevallen kan een positief PCR-resultaat wel bijdragen tot een zekerheidsdiagnose.

Deze gevalsdefinities, gebaseerd op een consensus van Europese experts, zijn ook gepubliceerd (16).
Besluit De diagnose van de verschillende vormen van Lyme borreliose blijft in hoofdzaak klinisch. Dit is zeker het geval voor erythema migrans, waar serologie niet aangewezen is en zelden iets bijbrengt. Bij de zeldzame huidletsels waar twijfel over de etiologie blijft, kan PCR van het huidletsel worden uitgevoerd. Bij de neurologische, cardiale en articulaire vormen wordt de diagnose gesteld op basis van de combinatie van mogelijke blootstelling, compatibele klinische manifestatie en compatibele laboratoriumdiagnose, voornamelijk de aanwezigheid van IgG-antistoffen. Het aanvragen van serologische tests bij aspecifieke ziektebeelden of in het kader van screening is tegenaangewezen, aangezien de proportie fout-positieve resultaten in deze populatie onaanvaardbaar hoog is en er geen therapeutische implicaties zijn.

[email protected] Extended-spectrum ß-lactamases (ESBLs) are plasmid-mediated bacterial enzymes that confer resistance to a broad range of ß-lactams. While the first ESBLs described in the early 80’s were originating by genetic mutations from native ß-lactamases found in gram-negative bacteria (mainly Klebsiella spp) there are nowadays reported worldwide in a wide array of bacterial species virtually including all Enterobacteriaceae as well as several non-fermenters species. Several new families of ESBLs (the most common being CTX-M) have emerged independently and spread worldwide mainly in the community (rather than in hospitals) and in commensal organisms (E. coli). Because ESBL-producing strains are resistant to most beta-lactams and a wide variety of commonly used antimicrobials, their proliferation poses a serious global health concern that has complicated treatment strategies not only for hospitalized patients but also for the management of communityacquired infections. Furthermore, some ESBL producers can cause nosocomial outbreaks and can spread rapidly among patients and across units or hospitals when necessary control measures are not applied. Accurate identification of ESBL-producing organisms is one challenging issue of utmost importance for clinical laboratories. This presentation aims to review the different recommandations specifically addressing laboratory detection (screening, confirmation, reporting to clinicians). All the statements were issued from a working group of the Belgian Infection Control Society (BICS) and were presented in a preliminary form at a symposium in June 2007 (BICS symposium ”Les entérobactéries BLSE+ vont-elles prendre la place des MRSA”, UCL MontGodinne 19/06/2007). It is expected that a complete set of practical and comprehensive guidelines aiming to address the different challenging issues ESBLs for clinical microbiologists and infection control specialists will be made available by the BICS under a question/answer format (about 70 different queries and answers) by the end of this year.

Summary Methicillin-resistant Staphylococcus aureus (MRSA) is a major pathogen in hospitals and in the community. Recently, a novel MRSA genotype has been reported with unexpected high prevalence among livestock animals, farmers and veterinarians in the Netherlands and neighboring countries. By molecular typing, those strains are resistant to digestion with endonuclease SmaI and belong to sequence type (ST) 398 by MLST. The objectives of this study were to determine the prevalence and the risk factors of MRSA carriage in pig farmers and household contacts in Belgium. Farmers and household contacts from 49 pig farms were screened for the presence of MRSA by culturing nasal swabs onto selective agars. MRSA identification were confirmed by PCR and genotyped by pulsed field gel electrophoresis (PFGE) after SmaI macrorestriction, staphylococcal cassette chromosome mec (SCCmec) typing, spa sequence typing and MLST. Susceptibility to 18 antimicrobials was determined by disk diffusion method. Demographic data, animal exposure and medical history were reviewed to determine risk factors of MRSA acquisition in this population. Of 127 farmers screened, 48 (38%) were positive for MRSA. MRSA carriers were found in 25 farms from seven provinces of Flanders and Wallonia. The prevalence was highly associated with porcine MRSA carriage and the frequency of contact with animals. By molecular typing, all isolates were resistant to SmaI digestion and belonged to ST398 with spa types t011 or t034 with SCCmec type IV or type V. MRSA strains were resistant to tetracycline (100%), trimethoprim (100%), MLS (>50%), aminoglycosides (>40%) and ciprofloxacin (32%). Conclusions: The prevalence of MRSA carriers was high (38%) in pig farmers and contacts. The risk factors of MRSA colonisation were contact with pig MRSA carriers and frequent contact with other animals. All MRSA strains belonged to ST398 “animal MRSA clone” which is reported in various livestock animals, farmers and veterinarians in Europe. .
Introduction Staphylococcus aureus est un des principaux pathogènes humains, responsable d’un large spectre d’infections telles que des infections de la peau et des tissus mous, des bactériémies, des pneumonies et des toxémies. La première souche de S. aureus résistant à la méticilline (MRSA) a été décrite en 1962 en Grande-Bretagne peu après l’introduction de la méticilline en usage clinique. Au cours des dernières décennies, des souches épidémiques de MRSA se sont largement disséminées dans les hôpitaux puis dans les institutions de soins chroniques. La plupart des souches de MRSA d’origine hospitalière (HA-MRSA) appartiennent à un nombre limité de clones ayant un bagage génétique particulièrement bien adapté à l’environnement hospitalier. Fin des années nonante, des infections à MRSA ont été décrites chez des enfants et des adultes jeunes n’ayant aucun contact avec des structures de soins et sans les facteurs habituels de risque d’acquisition de MRSA. Ces souches de MRSA d’origine communautaire (CA-MRSA) appartiennent à des clones différents de ceux d’origine nosocomiale et produisent souvent une exotoxine appelée leucocidine de Panton-Valentine (PVL).
Des cas d’infections ou de colonisations à MRSA ont aussi été décrits chez différentes espèces d’animaux dont les vaches, les chevaux, les chiens et les chats. Des cas de transmission de MRSA entre humains et animaux ont été rapportés aux USA et en Europe. Par typage moléculaire, la plupart de ces souches appartenaient à des clones épidémiques de MRSA d’origine hospitalière comme « UK EMRSA-15 » ou communautaire comme ST80-SCCmec IV. Récemment, une prévalence anormalement élevée de MRSA (760 fois supérieures à la

normale) a été observée au Pays-Bas chez les porcs, les éleveurs de porcs et les vétérinaires en contact avec les porcs. Par génotypage, ces souches de MRSA étaient résistantes à la digestion par l’enzyme de restriction SmaI utilisé pour l’analyse moléculaire en champs pulsé (PFGE) et appartenaient au génotype ST398 par multilocus sequence typing (MLST). Ces souches ont été isolées chez d’autres espèces animales, dont les chevaux, les veaux et la volaille. Chez l’homme, des cas d’infections des tissus mous et cutanées et des pneumonies ont été décrits principalement chez des personnes en contact avec des animaux. A ce jour, des souches appartenant à ce génotype ont été rapportées aux Pays-Bas, au Danemark, en Allemagne et en France. En Belgique, 8 cas d’infections humaines ont été enregistrés par le laboratoire de Référence des Staphylocoques MRSA depuis 2003, principalement en Flandre Occidentale dans la région du pays à la plus haute densité en élevage de porc.
Au vu de ces éléments, une étude a été lancée sous l’égide de la Belgian Antibiotic Policy Coordination Committee (BAPCOC) en collaboration avec l’Institut Scientifique de la Santé Publique (ISP), le Centre d’Etude et de Recherches Vétérinaires et Agronomiques – Centrum voor Onderzoek in Dierengeneeskunde en Agrochemie (CODA-CERVA), la Plate-forme fédérale d’hygiène hospitalière et le laboratoire de référence des staphylocoques – MRSA. Les objectifs de cette enquête étaient 1) de déterminer la prévalence chez les porcs et les éleveurs de porcs en Belgique ; 2) de déterminer les facteurs de risques de portage de MRSA chez les éleveurs ; 3) de caractériser les souches de MRSA isolées chez de porcs et les éleveurs de porc en Belgique.
Matériel et Méthode Sélection des fermes Cinquante élevages de porc répartis en Flandre et en Wallonie ont été sélectionnés au hasard à partir de la base
de données « Varken Sanitel ». Dans chaque ferme, un échantillon de porcs et toutes les personnes présentes au moment de l’enquête ont été dépistés pour la présence MRSA. Les frottis de dépistage réalisés chez les animaux ont été envoyés au Coda-Cerva, tandis que les échantillons d’origine humaine ont été analysés par le laboratoire de référence des Staphylocoques-MRSA. Des données démographiques et des facteurs de risque d’acquisition de MRSA ont été enregistrés par les médecins et infirmières épidémiologistes de l’ISP lors de l’enquête à l’aide d’un questionnaire standardisé. Culture et identification des souches de MRSA Les mêmes techniques microbiologiques phénotypiques et génotypiques ont été utilisées dans les 2 laboratoires pour la recherche et la caractérisation des souches de MRSA. Après 24 heures d’incubation, les bouillons d’enrichissement ont été repiqués sur des géloses sélectives. L’identification des colonies suspectes de MRSA ont été confirmées par des tests phénotypiques et par PCR multiplex mecA, nuc, 16SrDNA. Caractérisation des souches de MRSA La sensibilité à 18 antibiotiques a été déterminée par la méthode de diffusion par disque. Les gènes de codant pour la résistance aux antibiotiques et les facteurs de virulence codant pour les exotoxines TSST-1, PVL ont été recherchés par PCR. Les souches de MRSA ont été génotypées par électrophorèse en champs pulsé (PFGE) après restriction avec SmaI, par spa typing, MLST typing et détermination du type de cassette chromosomique mec (SCCmec).
Résultats Sur les 127 personnes dépistées dans 49 fermes, 48 (37.8%, intervalle de confiance à 95% 25.6%-50.0%) étaient porteurs de MRSA. Parmi les porteurs de MRSA, un seul (2%) présentait une infection cutanée à MRSA. Des porteurs de MRSA (écart de 1 à 6 par ferme) ont été retrouvés dans 25 (51%) fermes dans 7 provinces en Flandre et en Wallonie. Le portage de MRSA chez les fermiers était indépendamment associé 1) à des contacts avec des porcs porteurs de MRSA, 2) à la fréquence des contacts avec des porcs , des chiens et des chevaux, 3) avec des niveaux d’hygiène et de protection personnel paradoxalement plus élevés.

Toutes les souches de MRSA d’origine humaine étaient résistantes aux tétracyclines et triméthoprime. Elles étaient fréquemment résistantes aux MLS, aux aminoglycosides et à la ciprofloxacine. Toutes les souches étaient sensibles à l’acide fusidique, à la mupirocine, au linézolide et aux glycopeptides. Par typage moléculaire, les souches étaient résistantes à la digestion par SmaI, et appartenaient au spa type t011 (94%), t034 (4%) et t567 (2%). Ces marqueurs moléculaires sont caractéristiques du génotype ST398 par MLST correspondant au clone de MRSA dit « animal ». Les souches de MRSA portaient deux types de SCCmec IV (53%) ou V (43%). La plupart des souches pouvaient être classées dans deux clones spa type t011 – SCCmec IV et SCCmec V. Ces deux génotypes avaient des profils de résistance particuliers.
Les souches appartenaient à un seul clone dans la plupart des élevages dont plus d’un fermier étaient porteurs de MRSA. Les éleveurs étaient colonisés par des souches de MRSA appartenant aux mêmes génotypes que celles isolées chez les porcs. En Belgique, ces génotypes sont rarement isolés (<1%) chez les personnes hospitalisés ou vivant dans des maisons, de repos et de soins et non jamais été associés à des cas d’infections communautaires. Elles appartiennent au même génotype ST398 précédemment décrit chez des animaux, fermiers vétérinaires dans de nombreux pays voisins.
En conclusion, on observe une prévalence élevée (38%) de MRSA chez les éleveurs de porcs dans 7 provinces de Flandre et de Wallonie. Les principaux facteurs de risque d’acquisition de MRSA sont les contacts avec des porcs porteurs de MRSA et la fréquence de contacts avec des porcs ou d’autres animaux. Par typage moléculaire, les souches appartenaient au clone ST398 de MRSA dit « animal » qui a été rapporté chez des animaux d’élevage et les fermiers dans les pays voisins mais est rarement observé dans les autres populations humaines. Cette étude a permis d’identifier une nouvelle population à risque de portage de MRSA, pouvant avoir des conséquences sur la politique de prévention de MRSA dans les hôpitaux et au niveau des soins de santé primaires.
