SIZE AND COURSE What’s the problem?
BELOOP •
Acute (<3 weken) op grond van 15 prospectieve onderzoeken (zowel eerste als tweedelijnsonderzoeken; follow-up ten minste 3 maanden) tot de conclusie dat men in de meeste gevallen snel verbetering van pijn en hinder ervaart en dat van degenen bij wie sprake is van werkverzuim ruim 80% na een maand weer aan het werk is. Tot 3 maanden na het begin van de aandoening is er nog sprake van verdere verbetering, daarna nauwelijks meer. Na 1 jaar heeft bijna driekwart ten minste 1 recidief doorgemaakt [Pengel 2003].
•
In een andere systematische review (36 merendeels prospectieve zowel eerste als tweedelijnsonderzoeken, follow-up ten minste 1 jaar) kwam men tot de volgende schattingen: na een half jaar is er bij 16% (spreiding 3-40%) weer of nog sprake van werkverzuim vanwege aspecifieke lagerugpijn, en na 1 jaar heeft 62% (spreiding 42-75%) weer of nog klachten [Hestbaek 2003].
•
De kans op aspecifieke lagerugpijn is bij iemand die al één of meer aspecifieke lagerugpijnepisodes heeft gehad tweemaal zo groot als bij iemand zonder eerdere klachten [Hestbaek 2003].
BELOOP 60% plotseling begin. •
Prognose is in een open populatie gunstig. De LBP verdwijnt bij 75-90% binnen 4-6 weken 1.
•
In patiëntenpopulaties is de prognose echter minder gunstig. Gemiddelds 62% van de patiënten heeft na 12 maanden nog steeds klachten. 16% van de patiënten die aan het begin van de episode het werk verzuimden vanwege de rugpijn doet dit nog na 6 maanden 2.
•
Recidief hoog; 66%. Eenmaal rugpijn gehad, 2x zoveel kans.
•
Studies in acute populaties laten goede prognose zien, studies in chronische populaties slechte.

Recidiverende rugklachten (al jaren soms last, gaat dan soms naar therapie, maar kent oefeningen inmiddels wel). Nu veel last, want heeft nieuwe medicatie voor hartritme stoornis en heeft daardoor obstipatie gekregen. Waarschijnlijk daardoor nu weer meer last van de rug. Staan geeft grootste probleem.

PAIN EDUCATION •
An individual oral educational intervention of 2.5 hours is useful to speed up return-towork in workers with acute or subacute LBP. Simple patient education sessions of shorter duration or written information do not seem to be effective as a single treatment. However, as they may be considered harmless if they are evidence-based and up-to-date, there is no reason for not using oral and written education to support treatment.

Orthopedisch chirurg, een pijnspecialist en klinische psycholoog een aantal fysiotherapeuten en ze nam anti depressiva sterke analgetica en spierverslappers.

2 dagen per week licht werk.Provocatie door flexiehoudingen en bewegingen zoals onderuitgezakt zitten, bukken aankleden en tillen. Extensie gerelateerde bewegingen zoals staan en lopen waren pijn vrij.

Hoge niveau’s van anxiety irt haar pijn en disability. Ze maakte zich constant zorgen over haar rugpijn en geloofde dat ze niet beter kon worden omdat ze een discus letsel had dat niet genas. Ze ging met haar klachten om door de pijn te vermijden en haar activiteiten in flexierichting te verminderen. Haar pijn intensiteit was 8/10, Oswestry 40% en ze had een hoog niveau van kinesiofobie (Tampa).

JOINT RECEPTORS VS MUSCLE SPINDLES •
The view on the role of joint receptors and muscle spindles changed when studies of afferent discharges originating from knee receptors in cats turned afterwards out to come from muscle spindles in the popliteus muscle whose afferents had taken an aberrant course in the posterior articular nerve (McIntyre, Proske, & Tracey, 1978).

Studies in subjects with joint replacement studies, in which all capsular tissue was removed without a loss of proprioception (Grigg, Finerman, & Riley, 1973; Zati, Degli Esposti, Spagnoletti, Martucci, & Bilotta, 1997)

Muscle receptors were believed to play an initiating role in the mono-synaptic reflex, and muscle vibration was used to stimulate spindle afference and elicit a tonic vibration reflex

De signalen uit het lichaam worden verwerkt op 3 niveau’s, een stretch reflex, in het cerebellum en corticaal.

In cerebellum en/of corticaal is een interne representatie te vinden van het lichaam waarlangs de binnenkomende informatie wordt vergeleken en gewogen met signalen uit andere regio’s.

Wanneer signalen minder betrouwbaar zijn worden deze minder zwaar gewogen en zal de respons op een prikkel kleiner worden

Signalen zijn minder betrouwbaar wanneer men op foam staat, maar ook wanneer men rugpijn heeft, daarom heeft vibratie daar een kleiner effect, en treedt een verschuiving van weging op naar andere regio’s.

LAGE RUGPIJN •
Vrouw, 21 jaar

In 2012 in flexie/rotatie door de rug gegaan. Sindsdien ervaart ze pijnklachten in de lage rug en rechter been. Geen tintelingen/dofheid/ krachtsverlies. Hoesten/niezen/persen gaven alleen pijn in de lage rug.
•
Geen natuurlijk herstel. Is toen bij de pijnpoli geweest, dit gaf geen verbetering.
•
Daarna een pijnrevalidatie traject in gegaan, waar ze voornamelijk heeft geleerd hoe ze met de pijn om moest gaan. De fysiotherapeut heeft haar alleen een oefenprogramma geadviseerd.
•
Nu komt ze weer bij de fysiotherapeut. Is gestopt met haar stage. Kan niet lang staan of zitten en tillen is naar eigen zeggen bijna onmogelijk door de pijn. Er is op dit moment alleen nog pijn in de rug, geen beenpijn meer aanwezig.
•
Werk/stage: horeca (bediening) 40 uur per week.
•
sport: sport op dit moment niet ivm drukte op haar stage.
•
Maakt zich zorgen of ze in de toekomst wel in de horeca kan blijven werken ivm haar rug.
