Hello. I would like to provide you with information about a disease called tuberculosis. This disease affects millions of people worldwide each year. There are certain variations between different countries as to how the disease is managed. This is due to the fact that knowledge about tuberculosis varies from country to country but also because tuberculosis is more common in some countries than in others. Some of the information shown in this video may therefore not be relevant or true for your personal situation. However, all tuberculosis patients have one thing in common: the disease is a pivotal experience and changes the life of nearly everyone who is affected by it and falls ill. This video thus aims to help you to understand what causes tuberculosis, how the disease is diagnosed and what treatment is needed. Hallo. In deze video wil ik u graag informeren over tuberculose, ook wel TBC genoemd. Deze ziekte treft jaarlijks miljoenen mensen. Hoe tuberculose wordt bestreden verschilt van land tot land, afhanklijk van hoeveel tuberculose er ergens voorkomt en wat met ervan weet. Sommige informatie in deze video zal daarom niet toepasbaar zijn op uw persoonlijke situatie. Een ding hebben alle tuberculose patienten overeenkomstig: De ziekte is een infrukwekkende ervaring en verandert het leven van iedereen die het krijgt. Het doel van deze video is om goed uit te leggen van de oorzaak van tuberculose is, hoe men tot de diagnose TBC komt en hoe dit behandeld wordt. X-Ray

An X-Ray needs to be taken to establish whether your lungs are affected.You will be asked to stand in front of an X-Ray machine. You will have to take a deep breath and then hold your breath for a few seconds. The X-Ray image can provide information about any damage to the lungs, for example about socalled 'cavities' or 'infiltration'. These can be a sign of an infectious disease such as tuberculosis but can also be caused by other diseases.
Een röntgenfoto wordt gemaakt om te kijken of uw longen aangedaan zijn. U zal gevraagd worden om voor een röntgenapparaat te gaan staan. U moet goed inademen en dan uw adem een paar seconde vasthouden. De röntgenfoto kan informatie geven over schade aan de longen, bijvoorbeeld over zogenoemde "ontstekings holtes" of "ontstekings velden". Dit zou een teken kunnen zijn van een infectie zoals tuberculose, maar dit zou ook het gevolg kunnen zijn van een andere ziekte in de long.
Computed tomography (CT scan)
CT scan
To obtain further information about the changes in your lungs, a CT scan is required. You will lie in the round opening of a CT machine for a series of special X-Rays to be taken. It is important not to move during the scan and to hold your breath when asked to. The CT gives detailed information about any changes or destruction of your lungs. Even small alterations that are not visible on a plain X-Ray can be detected. These can point towards an infectious disease like tuberculosis.
Om uw longen nauwkeriger te onderzoeken, kunnen we een CT-scan maken. U ligt daarvoor op een bewegende tafel die door de ronde opening van het apparaat gaat. Er worden een aantal speciale röntgenfoto's maakt. Het is belangrijk dat u niet beweegt tijdends de scan, en dat u uw adem even inhoudt als dat aan u gevraagd wordt. Een CT scan geeft gedetailleerde informatie over veranderingen of schade aan de longen, die soms op een röntgen foto niet zichbaar zijn. Hiermee zouden kleine veranderingen gezien kunnen worden die kunnen passen bij tuberculose.

Sputum analysis
Sputum onderzoek
To gain more information about a possible infection with tuberculosis, the phlegm that you cough up needs to be examined. Tuberculosis bacteria in your 'sputum' - which is the medical word for 'phlegm' - can be dyed with a special dying method called Ziehl-Neelsen and then looked at under a microscope. This is a method used to establish whether or not you are infectious when you cough. Only when bacteria are visible on microscopy you are infectious to others. During the course of treatment, the number of bacteria being coughed up declines. Therefore it is very important that the sputum sample is coughed up from the deep parts of the lung. It should be spat in the tube supplied.
Om aan te tonen of u aan TBC lijdt wordt u gevraagd slijm, ook wel sputum genoemd, op te hoesten voor onderzoek. De tuberculose bacteriën in het sputum kunnen gekleurd worden met een speciale kleurstof, ook wel ZielNeelsen genoemd, en dan worden gezien onder een microscoop. Dit is een methode die aantoond of u besmettelijk bent als u hoest. Alleen wanneer bacteriën in het sputum worden gezien bent u besmettelijk voor anderen. Tijdens de behandeling zal het aantal bacteriën in het sputum verminderen. Het is daarom belangrijk dat het sputum diep uit de longen wordt opgehoest en in een bakje wordt gespuugd.

It is planned that you should undergo an investigation of your lungs called a bronchoscopy. This is necessary to obtain mucus, fluid and possibly small tissue samples from your lower airways. It is possible to diagnose tuberculosis from these samples using several different methods. It is possible that several weeks will pass before a positive or negative result is available. You will receive sedative medication to ensure that you sleep during the bronchoscopy. You will receive oxygen through a small tube to the nose and you will be monitored continuously
Een kijkonderzoek van de longen om eventuele TBC aan te tonen heet een bronchoscopie. Dit is nodig om het slijm, vocht en soms wat weefsel diep uit de longen te verkrijgen. Dit wordt dan onderzocht op tuberculose door middel van verschillende methoden. Once they have been inhaled, tuberculosis bacteria are able to multiply in the immune cells of the lungs. Parts of the lung may be destroyed by the human immune system in an attempt to eliminate the infected cells. This leads to holes in the lung tissue that are called 'cavities' and these are often visible on X- Ray. In the cavities, the bacteria are no longer accessible to the immune system. Furthermore, bacteria can be expelled from the cavities into the air when coughing, making a tuberculosis patient contagious to others.
TBC wordt door het hoesten van kleine druppeltjes met bacterien overgedragen van persoon tot persoon. Eenmaal ingeademd zijn de bacteriën in staat zich te vermenigvuldigen in de afweercellen van de long. Prevention of transmission
Voorkomen van besmetting
As long as bacteria are coughed up, one may be considered to be contagious to others. In this situation, you should wear protective face masks covering the mouth and nose when meeting with others face to face. It is also important to air your room frequently as the small bacteria can float in the air for many hours and infect others. For this reason, people in contact with you have to wear protective masks, too. These should be specially licensed masks fulfilling the standard FFP2 or N95 criteria. These masks offer good protection and allow friends and relatives to visit you.
Zolang als u bacteriën ophoest bent u besmettelijk voor andere mensen. Daarom moet u een masker dragen dat over uw mond en neus gaat als u met andere mensen contact heeft. Ook is het belangrijk dat uw kamer regelmatig gelucht wordt, omdat de kleine druppetjes met bacteriën uren in de lucht kunnen zweven en andere mensen kunnen besmetten. Daarom moeten ook andere mensen die contact hebben met TBC patienten een masker dragen over neus en mond. Deze masker moeten het kwaliteitskenmerk FFP2 of N95 hebben. Pathogenesis

Tuberculosis or TB is caused by bacteria. These are small and very resilient organisms which can be inhaled unnoticed. This way the bacteria travel to the lungs where they enter into immune cells. The immune cells are able to eliminate most bacteria. But the bacteria that cause tuberculosis - called Mycobacterium tuberculosis in medical terms - can survive inside the immune cells and even multiply there. The human body recognizes these infected cells and tries to eliminate them which causes an inflammatory response. Some people are able to control the tuberculosis infection for many years or even decades without any medical intervention. They do not develop symptoms and sometimes even recover completely. Unfortunately, the immune system often fails to stop the multiplication of the bacteria which results in an inflammatory response and then slow progression of the disease, followed by destruction of the lungs and other organs.

Als gevolg hiervan ontstaat er schade aan de longen en andere organen.
Symptoms
Klachten
The inflammatory response in the body leads to the typical symptoms of tuberculosis. The disease progresses very slowly. Therefore, symptoms can persist for many months before you decide to see a doctor. Typical symptoms are a longstanding cough, fever, night sweats and weight loss. Some people have to change their sheets several times per night because they sweat so heavily. Many people notice weight loss of up to several kilograms.
De afweerreactie van uw lichaam veroorzaakt de typische klachten van tuberculose. De ziekte wordt langzaam erger. Daarom kunnen de klachten maanden bestaan voorday u besluit ermee naar de dokter te gaan. Typisch klachten van tuberculose zijn langdurig hoesten, koorts, gewichtsverlies en nachtelijk zweten. Sommige mensen moeten hun beddegoed een aantal malen per nacht wisselen omdat ze zo heftig zweten. Veel mensen hebben gewichtsverlies, dat meerdere kilo's kan zijn.

The typical complaints pointing towards tuberculosis are cough for several weeks, fever, night sweats and unintentional weight loss.
De typische klachten die kunnen duiden op tuberculose zijn hoesten sinds een paar weken, koorts, nachtelijk zweten en onbedoeld gewichtsverlies.
HIV Test
HIV/AIDS test
The world health organisation WHO recommends that every person infected with tuberculosis should be tested for HIV or AIDS. The reason is that people infected with HIV have a higher risk of developing tuberculosis because their immune system is suppressed. Infection with HIV can nowadays be treated with drugs that are well tolerated. This antiretroviral therapy is also extremely important in the treatment of tuberculosis in someone with HIV.
De Wereld Gezondheids Organisatie WHO raadt aan om bij iedereen die tuberculose heeft een HIV/AIDS test te doen. De reden hiervoor is dat mensen die HIV/AIDS hebben een groter risico hebben op tuberculose omdat hun afweersysteem onderdrukt is. HIV/AIDS can tegenwoordig behandeld worden met medicatie die goed verdragen wordt. Deze antiretrovirale therapie is ook erg belangrijk voor de behandeling van tuberculose, als deze patient ook HIV/AIDS heeft.
Isolation

As long as you are infectious to others, you will be asked to follow certain rules in order to prevent the infection from being passed on to others. You may not be allowed to leave your hospital room for some time. Discharge from hospital is not permitted in many countries as long as you are coughing up bacteria. You should be supplied with all your necessities in your hospital room. All visitors should wear protective face masks to avoid the risk of infection. If you have to leave your room - for instance for an investigation - you should wear such a mask, too. The doctors or nursing staff can give you instructions as to how to use the masks.
Zo lang u besmettelijk bent, wordt u gevraagd een aantal regels te volgen om te voorkomen dat u andere mensen besmet. Zo kan het zijn dat u tijdelijk niet uw ziekenhuiskamer mag verlaten. In veel landen moet u in het ziekenhuis blijven zolang u nog bacteriën ophoest. U zult alles op uw kamer krijgen wat u nodig heeft. Alle bezoekers moeten beschermende maskers dragen om besmetting te voorkomen. Als u uw kamer moet verlaten, bijvoorbeeld voor een onderzoek, moet u ook een masker dragen. De artsen of de verpleegkundigen kunnen u instructies geven over hoe u het masker moet gebruiken.
Testing of contacts

The local health authority will contact all patients with tuberculosis. You will be asked about your contact with others and about the duration and intensity of these contacts. This can include family members, room- or house- mates, colleagues or friends. The health authority will then assess these contacts to determine whether they are also affected by tuberculosis. A blood test or an X-Ray may be necessary. These investigations are necessary to rule out transmission of tuberculosis to your family or friends. Possibly, the contagious source of a tuberculosis infection can be tracked down but unfortunately this is rarely possible. Contact persons may be monitored by the health authorities for several months if necessary.
De gemeentelijke gezondheidsdienst neemt contact op met iedereen die tuberculose heeft. Zij zullen u vragen met wie u een langere tijd of intensief contact gehad hebt. Dat kunnen onder andere familieleden, medebewoners, collega's of vrienden zijn. De gemeentelijke gezondheidsdienst zal deze contactpersonen onderzoeken om te kijken of zij tuberculose hebben. Mogelijk zal er een bloedtest gedaan worden of een röntgenfoto gemaakt worden. Deze onderzoeken zijn nodig om eventuele besmetting van je vrienden fof familie op te sporen. Soms komt men er op deze manier achter wie u aangestoken heeft, helaas is dit niet altijd mogelijk. Contactpersonen zullen soms een aantal maanden vervolgd worden door de gemeenselijke gezondheidszorg.

Newly diagnosed susceptible tuberculosis

The clinical and microbiological investigations have shown that you are infected with normal tuberculosis bacteria that are susceptible to the standard drugs usually used for the treatment of tuberculosis. The results that are available so far show that the bacteria are not resistant to any drugs. However, more specific testing may take several weeks.
De testen hebben aangetoond dat u een tuberculose bacterie bij u draagt die goed reageert op gewone TBC medicatie en dus een "gewone" TBC bacterie is. De tuberculose bacterie die bij u gevonden is is niet resistent tegen medicijnen die gebruikt worden om tuberculose te behandelen. The clinical and microbiological investigations of have shown that you are infected with tuberculosis bacteria that are resistant to some drugs. According to these tests, not all of the drugs usually prescribed for the treatment of tuberculosis will work in your case. You will therefore receive several different tablets and possibly intravenous treatment (through a drip) to kill the bacteria. Unfortunately, bacterial resistance requires a longer duration of treatment.
De testen hebben aangetoond dat u een tuberculose bacterie bij u draagt die ongevoelig resistent- is voor sommige TBC medicatie. Volgens deze testen kunnen bij u niet alle antibiotica die normaal gebruikt worden voor de behandeling van TBC gebruikt worden. U zal daarom verschillende tabletten en mogelijk intraveneuze medicatie moeten krijgen om de bacterie te doden. Deze resistente bacterie zal helaas ook een langere behandeling nodig hebben.
General information about therapy
Algemene informatie over behandeling
Tuberculosis can be treated with antibiotic drugs. Unfortunately treatment takes many months and requires a lot of stamina. You will have to take a lot of tablets for many months. It is very important that you take your tablets regularly. If you do not take the tablets regularly, the treatment may not be successful. The bacteria will start to multiply again and can become resistant to the antibiotics. This leads to an even more complicated and prolonged treatment. In the worst case, the disease can become untreatable and fatal.
Tuberculose kan met antibiotica behandeld worden. Helaas duurt de behandeling vele maanden en vraagt veel geduld. U zult vele maanden verschillende tabletten moeten slikken. Het is erg belangrijk dat de tabletten altijd ingenomen worden. Als u de tabletten niet altijd neemt kan het zijn dat de behandeling onvoldoende is. De bacteriën zullen zich weer gaan vermenigvuldigen en worden ongevoelig voor de antibiotica. Dit maakt de behandeling weer langer en moeilijker. In het ergste geval is de ziekte niet meer te behandelen en kunt u eraan overlijden.
Standard treatment
Standaard behandeling
Usually, tuberculosis bacteria are killed by standard medication and the treatment takes six months. You will be given four different drugs, the active ingredients of these are called Isoniazid, Rifampicin (or Rifampin), Pyrazinamide and Ethambutol. Once the first two months of therapy have been completed, you will continue treatment with Isoniazid and Rifampicin for at least four further months. The success of your treatment is monitored through analyses of your phlegm and X-Rays.
Gewoonlijk worden tuberculose bacteriën gedood door standaard medicatie die gedurende 6 maanden moet worden genomen. U zult vier verschillende medicijnen krijgen, die Isoniazide, Rifampicine, Pyrazinamide en Ethambuthal genoemd worden. Zodra de eerste twee maande van de behandeling voorbij zijn, zal u door moeten gaan met de isoniazide en rifampicine voor tenminste vier maanden. De slagingskans van de behandeling wordt in de gaten gehouden door het testen van uw sputum en röntgenfoto's.

This will determine the date when your treatment can stop. It is not necessary to stay in hospital for the entire duration of treatment. But it is possible that a hospital stay of several weeks or even months is required.
Dit zal bepalen wanneer u mag stoppen met de medicijnen. Het is niet nodig dat u gedurende de gehele behandeling in het ziekenhuis opgenomen bent. Maar het is mogelijk dat u enige weken of zelfs maanden opgenomen moet zijn.
Treatment of resistant tuberculosis
Behandeling van resistente tuberculose
In resistant tuberculosis, the drugs that are usually prescribed do not work anymore. This type of tuberculosis is called MDR tuberculosis, which is an abbreviation for 'multi drug resistant tuberculosis', or XDR tuberculosis which stands for 'extremely drug resistant tuberculosis'. These conditions are treated with several different drugs for many months. The recommended duration of treatment is currently at least twenty months, that is nearly two years. You may have to stay in hospital for a very long time to prevent the spread of the disease. Additionally, you will receive intravenous treatment (through a drip) to treat the infection. This may be required for the entire time of treatment
In het geval van resistente TBC werken sommige medicijnen die normaal voorgeschreven worden voor TBC niet meer. Dit type tuberculose wordt MDR tuberculose, wat staat voor multi drug resistant tuberculose, of XDR tuberculose genoemd, wat staat voor extremely drug resistant tuberculose. Deze vorm van tuberculose wordt met verschillende medicijnen gedurende vele maanden behandeld. Op dit moment wordt aangeraden 20 maanden te behandelen. Dat is bijna 2 jaar. Het kan zijn dat u langere tijd opgenomen moet zijn in het ziekenhuis om verdere verspreiding te voorkomen. Ook kan het zijn dat u medicatie via een infuus nodig heeft om de infectie goed te behandelen, mogelijk de gehele behandelduur van 20 maanden.

If you have an infection with the human immunodeficiency virus, HIV, this should be treated correctly. You will receive at least three different drugs, some of which may be combined in a single tablet. Because the HIV virus suppresses the immune system, its treatment is very important to help you recover from tuberculosis.
Als u het human immunodeficiency virus, HIV, bij u draagt of AIDS heeft moet u daarvoor behandeld worden. U zal hiervoor tenminste drie verschillende medicijnen krijgen, waarvan enkele gecombineerd kunnen worden in een tablet. Omdat het HIV virus het immuunsysteem onderdrukt is behandeling hiervan erg belangrijk om beter te worden van tuberculose.
Anti-HIV treatment prevents multiplication of the virus in the body leading to a recovery of socalled 'helper cells' that belong to the immune system. These helper cells fight against organisms such as viruses, bacteria and fungi attempting to invade the human body.
Anti-HIV medicatie voorkomt vermenigvuldiging van het virus in het lichaam wat leidt tot een herstel van de zogenaamde "helper-cellen" die onderdeel zijn van uw afweersysteem. Deze helper-cellen vechten tegen organismen zoals virussen, bacteriën en schimmels die het lichaam aanvallen. Het is mogelijk dat in de eerste weken van de HIV behandeling deze nieuwe helper-cellenerg druk zijn met het opruimen van organismen die zich in uw lichaam bevinden. Hierdoor is het mogelijk dat u zich zieker gaat voelen. Dit wordt het immuun reconstitutie syndroom genoemd en is een teken dat uw afweer sterker wordt. Geeft u het bij de arts aan als u zich slechter voelt na het starten van de HIV behandeling.
It is possible that in the first few weeks of antiHIV treatment, these new helper cells are heavily engaged in the elimination of foreign organisms from a your body. This can lead to a deterioration of the your well-being. This is called immune reconstitution syndrome and is a sign that your immune system is recovering. You should inform the doctor if you start feeling unwell after the start of anti-HIV treatment.

Patient's contribution to recovery
Zorg door de patient voor herstel
You can contribute to your recovery with simple measures. It is important not to smoke. Recovery takes longer in smokers than in nonsmokers. Also, fresh air and occasional sunlight exposure appear to be helpful. The most important factor is a regular intake of your prescribed medication. This ensures that recovery from tuberculosis can be achieved as fast as possible.
Iedereen die TBC heeft kan er met eenvoudige maatregelen bijdragen aan zijn eigen herstel. Het is belangrijk om niet te roken. Het herstel gaat langzamer in rokers dan in niet-rokers. Ook frisse lucht en regelmatig zonlicht vangen lijken bij te dragen aan herstel. Het belangrijkst is dat u de medicatie altijd en regelmatig inneemt. Zo kunt u ervoor zorgen dat het herstel van tuberculose zo spoedig mogelijk is.
Isoniazid
Isoniazide
Isoniazid is one of the most important drugs for the treatment of tuberculosis. In many cases, this drug will be given thoughout the whole course of your treatment. You will receive a dose that is adjusted to your body weight. Occasionally, some people experience an allergic reaction with rash and itching. This drug can affect your liver and regular blood tests are required to monitor your liver function. To protect your liver, you should abstain from alcohol during your treatment period. Furthermore, Isoniazid may affect your nerves, leading to numbness in your hands and feet. This can be prevented by giving vitamin B6 supplements. Please inform your doctor about any side effects that you observe. Your doctor may be able to adjust your treatment regimen accordingly.
Isoniazide is een van de twee belangrijkste medicijnen tegen tuberculose. In veel gevallen zal dit medicijn gedurende de gehele behandeling genomen worden. De dosis die u moet nemen wordt berekend op basis van uw gewicht. Sommige mensen ontwikkelen een allergie tegen het medicijn, wat zich uit in een huiduitslag en jeuk. Dit medicijn kan leverschade veroorzaken en daarom is regelmatige bloedtesten nodig om dit in de gaten te houden. Om uw lever te beschermen wordt geadviseerd geen alcohol te drinken tijdens de behandeling. Een andere bijwerking van Isoniazide is schade aan zenuwen, waardoor u een doof gevoel kunt krijgen in handen en voeten. Dit kan voorkomen worden door extra vitamine B6 te slikken. Rifampicin or Rifampine acts on both actively multiplying and resting bacteria. Therefore, it is one of the most important drugs of your standard tuberculosis therapy and it should be taken througout the whole course of your therapy. You will receive a dose that is adjusted to your body weight. Rifampicin causes orange colouring of your urine and stool. Occasionally, some people experience an allergic reaction, with rash and itching. The drug can affect your liver and regular blood tests are required to monitor your liver function.
Rifampicine werkt op zowel de vermenigvuldigende als op de slapende bacteriën. Het is daarom een van de belangrijkste medicijnen van de standaard tuberculose therapie en moet gedurende de gehele behandeling genomen worden. De dosis wordt berekend op basis van uw gewicht. Rifampicine veroorzaakt een oranje kleur van de urine en ontlasting. Soms treedt er een allergische reactie op bij sommig mensen, wat zich uit in huiduitslag en jeuk. Dit medicijn kan leverschade veroorzaken en daarom is regelmatige bloedtesten nodig om dit in de gaten te houden. Om uw lever te beschermen wordt geadviseerd geen alcohol te drinken tijdens de behandeling. The intensive treatment phase involves giving a large number of different drugs simultaneously in order to control the tuberculosis infection as quickly and as effectively as possible. You will receive a dose that is adjusted to your body weight. Ethambutol is not usually given throughout the whole treatment period, but this may be required in cases of drug resistant tuberculosis. Please ask your physician how long you will receive Ethambutol for. During your treatment with Ethambutol, your eyes need to be examined regularly. The drug can cause an inflammation of your eyes that can leads to a deterioration of your vision. An early alarming side effect may be a change in your colour vision. Another occasional side effects is joint aches. Please inform your doctor about any side effects that you observe. Your doctor may be able to adjust your treatment regimen accordingly
