Summary Ken Kersemans Development of new radiofluorinated methylphenylalanine analogues for tumour imaging with PET During the past decade, tumour detection using nuclear oncology methods such as PET and SPECT has been improved dramatically as they are powerful tools for the measurement of biological processes and pathologies at the molecular and cellular level. The most widespread and routinely used radiopharmaceutical for PET is [ 18F]FDG. Despite its numerous clinical applications, its shortcomings such as its low specificity ([18F]FDG is also taken up by inflammatory lesions) and high background in the brain have become a concern for oncologists. In fact, reports that document these issues or describe methodologies to minimise them are legio. Amongst the new tracers that have been developed to overcome the drawbacks of [ 18F]FDG, radiolabelled amino acids are in the centre of interest because they show greater potential for more specific diagnosis of tumours than [18F]FDG. Indeed, in contrast to [18F]FDG, radiolabelled amino acids are not taken up significantly in inflammatory lesions. Moreover, amino acid uptake in the normal brain is low allowing for easier lesion detection. Although 18 F-labelled amino acids have been around for many years now, it is only recently that they have been synthesised with high radiochemical yields, using procedures which offer practical advantages for routine clinical practice. At present, the best established fluorinated amino acid analogue for brain tumour imaging with PET is O-(2-[18F]fluoroethyl)-L-tyrosine ([18F]FET). Albeit being used world wide, its success was limited to brain and head and neck carcinomas as [18F]FET did not seem to accumulate significantly in other peripheral tumours. This means that other more potent tumour-specific radiofluorinated amino acid analogues that can be synthesized for routine clinical use are still required. In earlier publications, our research group demonstrated that [123I]-2-I-L-phenylalanine could diagnose preclinically a wide array of tumour types. This specific tumour uptake is governed by the LAT1 transporter system, which is over-expressed in the majority of tumours. Consequently, our strategy for the development of new potent amino acid analogues for PET was based on the replacement of the iodine by a fluoroalkyl side chain. Because the increase of the initial molecular volume by introduction fluoromethyl or fluoroethyl side chain is comparable to that by an iodine atom, it was expected that these compounds would show similar tumour targeting properties of the successful SPECT tracer [123I]-2-I-L-phenylalanine. We showed that it was possible to synthetise the precursor molecules and reference compounds of the fluoromethylated and fluoroethylated phenylalanine analogues studied in this work by means of two strategies. On the one hand, radical bromination of methyl side chains provided easy and direct access to the methylated compounds while the „Stille‟
reaction allowed for a longer but relatively straightforward synthesis of the ethylated derivatives. All references could be synthetised with a high purity, suitable for direct nucleophilic radiofluorination. Overcoming of different pitfalls in the manual radiosynthesis of no carrier added 2[18F]FMLP (based on a direct aliphatic nucleophilic radiofluoride-brome exchange), such as the stability of the benzylic carbon - fluorine bond, resulted in the development of a method that for a limited amount of radioactivity allowed for high yields and high radiochemical purities of radiofluorinated compound. For routine clinical applications an automated “in hotcell” radiosynthesis was required. Automatization of the radiosynthesis was successfully accomplished using units of a Scintomics HotBoxThree module. The strategy applied in this method also allowed an easy adaption of the system for the radiosynthesis of a series of 18F labeled fluoroalkyl – phenylalanine analogues, mimetic molecules as well as of [18F]FET in reproducible high yields and high radiochemical purities (>99 %). Additionally, the versatility of the method and the automatic system makes the new compounds readily accessible for extensive structure-activity studies in both research and clinical practice. Prior to proceed with structure-activity studies, the compounds were tested for their stability in physiological conditions. As opposed to the fluoroethylated compounds and 4[18F]Fluoromethyl-L-phenylalanine, 2-[18F]Fluoromethyl-L-phenylalanine was found to be extremely sensitive to radiodefluorination in aqueous formulations. Therefore, the factors that influence the stability of the [18F]-fluoro-benzyl group were studied. Switching the fluoromethyl side chain on the ring of phenylalanine from the 2-position to the 4 position resulted in a stable compound, namely 4-[18F]FMLP. Therefore it was proposed that the ammonium function was responsible for the instability of 2-[18F]Fluoromethyl-Lphenylalanine due to a intramolecular interaction. This unfavourable interaction could be inhibited by adding calcium ions (0.04 M) to the final radiopharmaceutical formulation, ensuring a shelf-life of at least 6 h without significant radiodefluorination. The observed difference in reaction rate for the hydrolysis of the 2- and 4- isomers of nocarrier added [18F]FMLP lead to a further understanding of the mechanism of the radiodefluorination, which is governed by two distinct intramolecular interactions. First, the interaction between the amine and the benzylic fluorine weakens the carbon–fluorine bond. Secondly, the formation of an additional hydrogen bridge between the carboxyl group and one of the benzylic hydrogen atoms renders the fluorine atom even more susceptible to hydrolysis. The stability studies provided a better insight in the factors governing the stability of the [18F]benzylfluoride group, allowing for a better design of future radiofluorinated compounds that employ this motif. To verify the aforementioned model and to provide a theoretical basis for the postulations put forward on the basis of these stability studies, a series of quantum chemical calculations were performed. The quantum chemical study on the underlying motives for the difference in hydrolysis kinetics between no-carrier added 2- and 4-[18F]-fluoromethyl-L-phenylalanine not
only lead to a deeper understanding of the aspects involved in the stability of the [ 18F]benzylfluoride group, but also to identify and quantify the intramolecular hydrogen bonds in no-carrier added 2-[18F]-fluoromethyl-L-phenylalanine. Within the amino acid neighbouring groups that can form hydrogen bonds with the negatively charged fluorine on the 2- position, the interaction of the ammonium group was found to be the most decisive factor. Unexpected was the weak hydrogen bond between the fluorine and an hydrogen atom of the β carbon atom. The theoretical study showed that today quantum chemical calculations can be efficient and powerful tools to predict intramolecular interactions affecting the potential of hydrolysis of [18F]-benzylfluoride group containing radiopharmaceuticals. Thus, time consuming synthesis and stability experiments can be avoided in the future. Finally, an in vitro structure-activity study was performed on all radiofluorinated phenylalanine analogues. Within this series, 4-[18F]FMLP proved to be the most promising candidate based on its affinity and efflux properties with regard to the LAT1 transporter system. Moreover, it was demonstrated that this compound was highly selective towards the targeted transport system as opposed to the widely accepted brain tumour tracer [18F]FET. Finally, a clinical case report study using 4-[18F]FMLP was performed on a patient with a Oligodendroglioma Grade II brain tumour. Increased tracer uptake was demonstrated in the tumour region which was identified by MRI. Although extensive preclinical and clinical studies of 4-[18F]FMLP, using [18F]FET as a reference, will have to be performed to verify these findings, the promising first results show that 4-[18F]FMLP can improve the existing imaging protocols to diagnose both brain and extra-cranial tumours.
Samenvatting Gedurende het laatste decennium zijn de detectie methoden van de nucleaire oncologie, zoals PET en SPECT, aanzienlijk verbeterd en zijn het krachtige hulpmiddelen geworden voor het bestuderen van biologische processen en pathologieën op moleculair en cellulair niveau. Het meest wijdverspreide radiofarmacon dat routinematig gebruikt wordt is [18F]FDG. Ondanks de vele klinische toepassingen vertoont het vele tekortkomingen die de oncologen zorgen baren, zoals een lage specificiteit ([18F]FDG wordt ook opgenomen in inflammatoire letsels) en een hoge achtergrond in de hersenen. Meer nog, het aantal wetenschappelijke publicaties dat deze tekortkomingen documenteert of waarin methoden worden aangereikt om deze te minimaliseren is groot. Te midden de nieuwe tracers, die ontwikkeld werden om de tekortkomingen van [18F]FDG te overwinnen, genieten radiogemerkte aminozuren speciale aandacht aangezien ze een groter potentieel vertonen voor specifiekere tumor diagnose in vergelijking met [18F]FDG. In tegenstelling tot [18F]FDG worden deze niet significant opgenomen in inflammatoire letsels. De opname van aminozuren in de hersenen is tevens laag, wat een makkelijkere detectie van laesies toelaat. Hoewel [18F]-gemerkte aminozuren reeds gedurende vele jaren in gebruik zijn, worden ze nog maar sinds kort gesynthetiseerd met hoge radiochemische opbrengsten en met behulp van procedures die praktische voordelen bieden voor routinematig gebruik in de kliniek. Op dit ogenblik is O-(2-[18F]fluoroethyl)-L-tyrosine ([18F]FET) het meest ingeburgerde radiogefluoreerde aminozuuranaloog voor de beeldvorming van hersentumoren. Echter, hoewel dit radiofarmacon overal ter wereld wordt aangewend, beperkt zijn succes zich tot hersentumoren en hoofd- en nektumoren gezien [18F]FET geen significante accumulatie vertoont in andere perifere tumoren. Dit betekent dat er nog steeds een vraag is naar betere tumorspecifieke aminozuuranalogen die toegepast kunnen worden in klinische routine. Onze onderzoeksgroep toonde in eerdere publicaties reeds aan dat, in preklinische studies, [123I]-2-I-L-phenylalanine een breed gamma aan tumortypes kon opsporen. Deze tumorspecifieke opname wordt toegeschreven aan de opname van het radiogemerkte aminozuur analoog via het LAT1 transport systeem, dat tot overexpressie wordt gebracht in het merendeel van de gekende tumortypes. Daarom werd in dit onderzoek ervoor gekozen om de ontwikkeling van een nieuw aminozuuranaloog voor PET te baseren op het vervangen van het volumineuze jood atoom door een fluoroalkyl zijketen. Gezien de toename van het moleculair volume ten gevolge van het invoegen van deze zijketen vergelijkbaar is met het invoegen van een jood atoom op phenylalanine, werd ervan uitgegaan dat deze fluoroalkyl verbindingen gelijkaardige eigenschappen zouden vertonen met de succesvolle SPECT tracer [123I]-2-I-L-phenylalanine met betrekking tot beeldvorming van tumoren.
We toonden aan dat het mogelijk was om de precursormoleculen en de referentiemoleculen te synthetiseren voor de in deze studie geselecteerde fluoromethyl en fluoroethyl analogen van phenylalanine. Radicalaire bromering van de methyl zijketen werd een geschikte methode bevonden voor de synthese van de phenylalanine analogen met een fluoromethyl zijketen terwijl de "Stille" reactie een langere doch vrij directe route bleek te zijn voor de synthese van de fluoroethyl gederivatiseerde phenylalanines. Alle precursoren werden aangemaakt met een hoge zuiverheid, wat toeliet om via een rechtstreekse nucleofiele substitutie de beoogde radiogefluoreerde analogen te bekomen. Het overkomen van een aantal moeilijkheden gedurende de manuele synthese van geen-drager toegevoegd 2-[18F]FMLP (gebaseerd op een rechtstreekse nucleofiele radiofluoride-broom uitwisseling) resulteerde in de ontwikkeling van een methode die toeliet kleine hoeveelheden radiogemerkt aminozuur aan te maken met een hoog rendement en een hoge radiochemische zuiverheid. De voornoemde moeilijkheden konden toegeschreven worden aan effecten die een invloed hadden op de stabiliteit van de koolstof - fluor binding. Voor klinische toepassingen was een automatische radiosynthese in een "hot cell" vereist. Deze automatisatie werd succesvol voltooid door gebruik te maken van de basisonderdelen van een Scintomics HotBoxThree module. De strategie die toegepast werd voor de synthese van 2-[18F]FMLP kon makkelijk aangepast worden om de synthese van een reeks [18F]-gemerkte fluoroalkyl-phenylalanine analogen, simulatiemoleculen alsook [18F]FET mogelijk te maken met hoge radiochemische rendementen en radiochemische zuiverheden (> 99%). Meer nog, de flexibiliteit van de methode en het automatische systeem maken dat de nieuwe verbindingen toegankelijk zijn voor uitgebreide structuur-activiteits studies voor zowel onderzoek als voor de klinische praktijk. Vooraleer over te gaan tot de structuur-activiteits studies, werden de verbindingen getest op hun stabiliteit in fysiologische omstandigheden. In tegenstelling tot de fluoroethyl verbindingen en 4-[18F]Fluoromethyl-L-phenylalanine, bleek dat 2-[18F]Fluoromethyl-Lphenylalanine uiterst gevoelig was ten overstaan van radiodefluorering in waterig midden. Omwille van deze reden werd onderzocht welke factoren een invloed hebben op de stabiliteit van de the [18F]-benzylfluoride groep. Omwisselen van de positie van de fluoromethyl zijketen van de 2- naar de 4- positie op de aromatische ring van phenylalanine resulteerde in een stabiele verbinding, namelijk 4[18F]FMLP. Daarom werd verondersteld dat de ammonium entiteit verantwoordelijk was voor de destabilisering van 2-[18F]Fluoromethyl-L-phenylalanine, ten gevolge van een intramoleculaire interactie. Deze nadelige interactie kon verhinderd worden door toevoegen van calcium ionen (0,04 M) aan de radiofarmaceutische formulering, wat de houdbaarheidsperiode tot op 6 uur bracht, zonder dat er significante radiodefluorering kon worden vastgesteld. De studie op het verschil in reactiesnelheid voor de hydrolyse van de 2- en 4-isomeren van niet-drager toegevoegd [18F]FMLP verschaftte een dieper inzicht in het mechanisme van de radiodefluorering, dat gedomineerd wordt door twee afzonderlijke intramoleculaire
interacties. Ten eerste verzwakt de interactie tussen het amine en het benzylische fluor de koolstof-fluor binding. Ten tweede wordt het fluor atoom nog gevoeliger voor radiodefluorering door vorming van een tweede waterstofbrug, namelijk deze tussen the carboxyl groep en één van de benzylische waterstof atomen. Het diepere inzicht in de factoren die de stabiliteit van de [18F]-benzylfluoride groep beïnvloeden laten toe om op een efficiëntere manier nieuwe radiogefluoreerde verbindingen te ontwikkelen die gebruik maken van deze functionaliteit. Om het voornoemde model te verifiëren en om de gemaakte postulaten, gemaakt op basis van de stabiliteitsstudies, te voorzien van een theoretische basis werd een reeks quantum chemische berekeningen uitgevoerd. Deze quantum chemische studie op de onderliggende redenen voor het verschil in hydrolyse kinetiek tussen geen-drager toegevoegd 2- and 4-[18F]fluoromethyl-L-phenylalanine bevestigde niet alleen voornoemde postulaten, maar liet ook toe om de intramoleculaire waterstofbruggen, aanwezig in geen-drager toegevoegd 2-[18F]fluoromethyl-L-phenylalanine, te kwantificeren. Van alle groepen die mogelijks een waterstofbrug kunnen vormen met het negatief geladen fluor atoom op de 2-positie bleek de ammonium interactie de sterkste te zijn. Onverwacht was de vaststelling dat er een zwakke waterstof brug teruggevonden werd tussen het fluor atoom en het waterstof atoom van het β koolstof atoom. Deze theoretische studie toonde aan dat quantum chemische berekeningen efficiënte en krachtige werktuigen kunnen zijn om intramoleculaire interacties te voorspellen die een invloed kunnen hebben op het hydrolyse potentieel van [18F]-benzylfluoride groep bevattende radiofarmaca. In de toekomst kunnen tijdrovende syntheses en stabiliteitsexperimenten dan ook vermeden worden. Tenslotte werd een in vitro structuur-activiteits studie uitgevoerd op alle radiogefluoreerde phenylalanine analogen. Binnen deze reeks bleek, afgaande op zijn affiniteit en efflux eigenschappen ten aanzien van het LAT1 transport systeem, 4-[18F]FMLP de meest beloftevolle kandidaat te zijn. Meer nog, er kon aangetoond worden dat deze verbinding hoogst selectief was voor het beoogde transport systeem, in tegenstelling tot de wijdverspreide hersentumor tracer [18F]FET. Tot slot werd een klinische pilootstudie uitgevoerd, gebruik makende van 4-[18F]FMLP, op een patiënt met een Oligodendroglioma Graad II hersentumor. Een verhoogde opname van de tracer werd aangetoond in een tumor gebied dat geïdentificeerd was met behulp van MRI. Hoewel uitgebreide preklinische en klinische studies met 4-[18F]FMLP (met [18F]FET als referentie) nodig zijn om deze beloftevolle resultaten te bevestigen, kan men stellen dat het gebruik van 4-[18F]FMLP kan leiden tot een verbetering van de bestaande protocols voor de beeldvorming van hersen- en extra-craniële tumoren.
