SUMMARY
SUMMARY In this doctoral thesis the linguistic tests applied before, during and after awake surgery were reviewed; and the development of a well-balanced linguistic protocol (DuLIP) for intraoperative language testing was described. Cortical and subcortical stimulation by means of DuLIP, multimodal tests (Quick Mixed Test) and spontaneous speech were illustrated with case reports. In addition, cognitive outcome and risk factors for cognitive decline following awake surgery were investigated in 40 brain tumour patients by means of a longitudinal study (preoperation, 6 weeks, 6 months postsurgery). Finally, Quality of Life and patients' experience with the awake procedure were examined with a newly developed questionnaire (n = 30, case report). In Chapter 1, awake brain surgery, corticosubcortical direct electrical stimulation (DES) and intraoperative language and non-language testing are introduced. In addition, literature findings about cognitive outcome and Quality of Life (QoL) after awake surgery are described. Finally, the aims for this doctoral thesis are presented. Chapter 2 focuses on the intraoperative language mapping techniques and language tests used in the context of awake surgery. This literature review reveals that intraoperative DES is the gold standard for language mapping, but that no guidelines or well-balanced linguistic protocols are available to reliably monitor language functions during DES. In Chapter 3, the development of a Dutch Linguistic Intraoperative Protocol (DuLIP) to assess language production and perception at the phonological, semantic and syntactic level is described. The results of standardisation in a control population (normative data) are provided as well as structure-to-function models for clinical use in a brain tumour population. A tailored selection of DuLIP tasks taking into account tumour location and preoperative language level enhances the reliability of identifying language functions during DES in order to avoid postoperative permanent language deficits. Chapter 4 describes the subcortical mapping procedure and some clinical recommendations are made to improve subcortical language and non-language testing. Case descriptions showed the advantage of multimodal tests such as the Quick Mixed Test and spontaneous speech over single tasks to identify critical subcortical brain areas and pathways in the language dominant hemisphere.

SUMMARY
Chapter 5 reports on the unexpected anatomoclinical findings in a right-handed patient with a tumour in the left frontal lobe. The tumour near Broca’s area was safely removed since no language positive sites were found during intraoperative DES. As pre- and postoperative language examinations were normal as well, atypical language dominance was suspected. Postoperative fMRI and DTI confirmed right cerebral language dominance as well as a crossed cerebro-cerebellar functional link with the left cerebellar hemisphere. Preoperative brain plasticity mechanisms inducing a shift of language functions to the right hemisphere and right hemisphere language dominance as a maturational variant were discussed as potential explanations for the crossed language dominance. In Chapter 6, the short-term and long-term effects of awake brain surgery on cognitive functions are described. 29 patients with left hemisphere tumours and 11 patients with right hemisphere tumours were administered with language, memory, attentional, executive and visuospatial tasks in the preoperative phase, at 6 weeks postsurgery and at 6 months postsurgery. Tumour resection near or within a critical language area (identified by DES) was a risk factor for cognitive decline at short-term follow-up for naming (measured by object/action naming), fluency (measured by phonological/semantic fluency), auditory comprehension (measured by token test) and attention/reading rate (measured by Stroop I); and at long-term follow-up for semantic fluency and attention/reading rate. Language, attentional and executive test scores declined at 6 weeks postsurgery. At 6 months postsurgery, performance on all language tasks (except semantic fluency) recovered to normal or preoperative level whereas performance on most attentional and executive tasks (TMTA, TMTB, Stroop I, Stroop II) was still significantly worse. Memory and visuospatial test scores did not significantly change after surgery. No effect on cognitive change was found for the tumour and treatment related variables, i.e. tumour location (left/right), hemispheric dominance, extent of resection, tumour grade or adjuvant therapy. Chapter 7 demonstrates the usefulness of a subjective measurement to study different aspects of QoL and patients' experience with the awake procedure. A newly developed questionnaire (ASPRO) and a cognitive test battery were administered in 30 brain tumour patients (10 patients at 6 weeks; 10 patients at 6 months; 10 patients at 1 year postsurgery). Overall, patients tolerated the procedure well and predominantly cognitive and somatic complaints were reported. Cognitive impairment negatively influenced QoL and especially the cognitive subscale of ASPRO. In addition, patients' experience with the awake procedure was positively associated with general well-being. In Chapter 8, we present a case with non-organic language deficits following awake brain surgery in the left anterior temporal area. Although eloquent sites were preserved and functionally intact at the end of the awake procedure, a range of atypical language symptoms (e.g. fluctuation over time and context) consistent with a diagnosis of non-organic language disorder were observed. An acute stress disorder due to the awake condition was put forward and compared with other explanations (e.g. conversion disorder, unintended imitation of witnessed aphasia, decompensation) to clarify the psychogenic deficits. Chapter 9 discusses the main findings and clinical implications of these studies as well as limitations and future directions.

SAMENVATTING In dit proefschrift werden de taaltesten gebruikt vóór, tijdens en na wakkere chirurgie besproken alsook de ontwikkeling van een talige testbatterij (DuLIP) om taalfuncties intra-operatief te testen. Aan de hand van een aantal casussen werd corticale en subcorticale stimulatie met behulp van DuLIP testen, multimodale testen (QMT) en spontane taal geïllustreerd. In een longitudinaal onderzoek bij 40 hersentumorpatiënten werden de cognitieve functies en risicofactoren voor cognitieve stoornissen na hersenchirurgie onderzocht door middel van een uitgebreid neuropsychologisch testprotocol. Daarnaast werd de kwaliteit van leven en de algehele ervaring na een wakkere operatie bevraagd en geanalyseerd bij 30 hersentumorpatiënten. In hoofdstuk 1 worden de termen wakkere hersenoperatie, corticosubcorticale stimulatie en intra-operatieve taaltesten geïntroduceerd. Daarnaast worden de resultaten van studies gericht op cognitie en kwaliteit van leven na wakkere hersenchirurgie beschreven. Hoofdstuk 2 overloopt en bediscussieert de gebruikte intra-operatieve 'taalmapping' technieken en taaltesten in de context van wakkere chirurgie. Deze review toont aan dat intra-operatieve DES (Directe Electrische Stimulatie) de gouden standaard is voor het in kaart brengen van de taalfuncties, maar dat er geen richtlijnen noch een wetenschappelijk onderbouwde taaltestbatterij beschikbaar zijn om taalfuncties betrouwbaar te bewaken tijdens een wakkere operatie. Hoofdstuk 3 beschrijft de ontwikkeling van de Nederlandse taaltestbatterij (DuLIP). Deze batterij bestaat uit fonologische, semantische en syntactische taalproductie en perceptietaken die intra-operatief kunnen worden gebruikt tijdens corticosubcorticale stimulatie. De resultaten van de standaardisatie in een controle populatie (normen) worden aangeleverd. Daarenboven worden anatomoklinische modellen gepresenteerd die als basis dienen voor de selectie van DuLIP taken bij tumorpatiënten. Er werd per patiënt een op maat gemaakte testbatterij (selectie van DuLIP taken) samengesteld rekening houdend met de tumor locatie en het preoperatieve taalniveau van de patiënt. De casusbeschrijvingen in dit hoofdstuk tonen aan dat deze manier van werken toelaat om op een betrouwbare manier taalfuncties intra-operatief te identificeren en zo postoperatieve permanente taalproblemen te vermijden.

SUMMARY
In hoofdstuk 4 wordt de subcorticale mapping procedure beschreven; en worden een aantal richtlijnen met betrekking tot intraoperatieve subcorticale testen geformuleerd. Aan de hand van een aantal casussen werd aangetoond dat het gebruik van multimodale testen en spontane taal tijdens subcorticale stimulatie zinvol is. In hoofdstuk 5 geeft de resultaten weer van een anatomoklinische studie uitgevoerd bij een rechtshandige patiënt met een tumor in de linker frontale kwab, dicht bij het gebied van Broca. Tijdens de resectie van deze tumor konden geen kritische taalgebieden gevonden worden met DES in en rond Broca. Bijgevolg kon de tumor veilig verwijderd worden en werden geen postoperatieve taalstoornissen gedetecteerd. Gezien ook de preoperatieve taaltesten normaal bleken te zijn, werd atypische taaldominantie als verklaring aangebracht. Postoperatieve fMRI en DTI bevestigden deze atypische rechter cerebrale taaldominantie, evenals een gekruiste cerebro-cerebellaire connectie met de linker cerebellaire hemisfeer. Twee mogelijke hypothesen werden bediscussieerd om deze atypische rechter dominantie te verklaren: 1) Preoperatieve plasticiteit mechanismen induceerden een translocatie van taalfuncties naar de rechter hersenhelft; 2) De rechter taal dominantie was hier een zeldzame maturationele variant. In hoofdstuk 6 wordt het effect van een wakkere hersenoperatie en tumorvariabelen op de cognitieve domeinen taal, geheugen, aandacht, executieve en visuospatiële functies gerapporteerd. 29 patiënten met een linker hersentumor en 11 patiënten met een rechter hersentumor werden getest voor de operatie, 6 weken na de operatie en 6 maanden na de operatie. Een tumorresectie in een taalkritische regio (bepaald via DES) was een risicofactor voor postoperatieve achteruitgang op korte termijn voor alle gemeten taalfuncties (benoemen van objecten/acties, semantische/fonologische woordvlotheid, taalbegrip) en aandacht/leessnelheid; en op lange termijn voor semantische woordvlotheid en aandacht/leessnelheid. Alle taal-, aandacht en executieve testen verslechterden op 6 weken postoperatief. Op 6 maanden na de operatie werd een vooruitgang gevonden op alle taaltaken, en een vooruitgang op 2 aandachtstaken (TMTA, Stroop I). De geheugen en visuospatiële test scores wijzigden niet significant na tumorresectie. Tumor en behandelingseigenschappen, zoals tumorlocatie (links/rechts), taaldominantie, tumorgraad, mate van resectie en adjuvante therapie waren geen bijkomende risicofactoren voor cognitieve verandering.

SUMMARY
In hoofdstuk 7 wordt een nieuw samengestelde subjectieve vragenlijst voorgesteld (ASPRO). Deze werd afgenomen samen met een cognitieve testbatterij bij 30 tumorpatiënten na de wakkere operatie om verschillende aspecten van kwaliteit van leven en ervaring met de wakkere procedure te bestuderen. Over het algemeen bleken de patiënten de wakkere operatie goed te verdragen en waren er voornamelijk cognitieve en somatische klachten. Cognitieve stoornissen bleken de kwaliteit van leven en het cognitieve domein van de vragenlijst negatief te beïnvloeden. Daarnaast werd er een positieve correlatie gevonden tussen het algemeen welbevinden van de patiënten en de algehele ervaring met de wakkere procedure. In hoofdstuk 8 presenteren we een patiënt die psychogene taalproblemen ontwikkelde na een wakkere hersenoperatie in het linker anterieure temporale gebied. Hoewel de kritische taalgebieden (gevonden tijdens DES) werden bewaard en dus functioneel intact waren aan het einde van de wakkere procedure, werd er een reeks atypische taal symptomen (bijv. fluctuering over tijd en context) geobserveerd die niet-organisch bleken te zijn. Om de psychogene taalproblemen te verklaren werden volgende hypotheses opgesteld: een acute stress stoornis ten gevolge van de wakkere procedure; een conversie stoornis; onbedoelde imitatie van een taalstoornis (afasie) bij een familielid; decompensatie. Hoofdstuk 9 bediscussieert de belangrijkste bevindingen en klinische implicaties van deze studies. Daarenboven worden beperkingen en suggesties voor toekomstige studies beschreven.
