Abstract Many mothers are inappropriately advised to discontinue breastfeeding or avoid taking essential medications because of fears of adverse effects on their infants. This cautious approach may be unnecessary in many cases, because only a small proportion of medications are contraindicated in breastfeeding mothers or associated with adverse effects on their infants. Information to inform physicians about the extent of excretion for a particular drug into human milk is needed but may not be available. Previous statements on this topic from the American Academy of Pediatrics provided physicians with data concerning the known excretion of specific medications into breast milk. • Eiwit transporters – IgA, IgM, IgG, prolactin, IgF-1 “If none present in maternal plasma, none in milk”
Risico voor zuigeling • Afhankelijk van 3 grote factoren: – Soort medicatie – Leeftijd van de zuigeling • Prematuren: zeker risico • Oudere zuigelingen: minimaal risico

Post-partum depressie • Onbehandelde maternele depressie leidt tot majeure sequelae in BV en FV zuigelingen. • Vroegere aarzeling om antidepressiva te gebruiken neemt af na studies die duidelijk aantonen dat PPD interfereert met optimaal moederschap/binding en met de neurologische en gedragsmatige ontwikkeling op leeftijd 1 jaar
Post-partum depressie • In moeders met depressie gedurende de eerste zes maanden, vertoonden de zuigelingen een depressieve interactiestijl, en lagere Bayley scores op leeftijd 1 jaar.
Gebruik antidepressiva in BV moeders? • De meeste antidepressiva zijn veilig voor BV zuigelingen. • Geen argumenten om aan te nemen dat antidepressiva negatieve laattijdige gevolgen hebben op neuromotorische/gedragsmatige outcome bij zuigelingen
Enkele gegevens ivm neuroleptica • Afkickreacties bij SSRIs* zijn normaal en vinden plaats in 10-30% van de zuigelingen postnataal. • Geen gegevens dat de neurologische en gedragsmatige ontwikkeling van de zuigeling aangetast is na blootstelling tijdens zwangerschap of borstvoeding. • Na zwangerschap, gedeeltelijke contradictorische data: – – – –
Lichte stijging in pulmonale hypertensie (but fleeting) Lichte stijging in prematuriteit (but same in untreated) Lichte stijging in lager geboortegewicht Doch, risico van onbehandelde depressie and psychose is MAJEUR, zodat het gebrek aan behandeling gevaarlijker is dan de behandeling zelf * SSRI = Selective serotonin reuptake inhibitor

Candida albicans: is it associated with nipple pain in lactating women? Amir LH, Garland SM, Dennerstein L, Farish SJ.
Author information Abstract Persistent nipple pain in lactating women, burning in nature, and associated with radiating breast pain, has been claimed by some authorities to be due to 'thrush' (candida) infection. Yet, scientific proof has been lacking. This study compared microbiological assessment of 61 women with nipple pain, 64 women without nipple pain, and 31 non-lactating women. Swabs of the nipple and baby's mouth, and expressed breast milk were collected for culture. Growth of Candida albicans (nipple and milk) was found more often in the women with nipple pain (19%) than in the control group (3%, p < 0.01). In addition, Staphylococcus aureus was also associated with nipple pain (p < 0.0001), and independently associated with nipple fissures (p < 0.0001). Neither C. albicans nor S. aureus was found on the nipples of the non-lactating group.
