1/ Vroegtijdige stadia: “Curatief” – Stadium IA (T1a-bN0) en Stadium IB (T2aN0): • Heelkunde • Geen adjuvante chemotherapie • Zo medisch inoperabel: Radicale radiotherapie.
– Stadium IIA (T2bN0, T1a-N1, T2aN1) en Stadium IIB (T2bN1 en T3N0); Stadium IIIA (T3N1): • Heelkunde • Adjuvante chemotherapie bij fitte patiënten • Adjuvante radiotherapie indien sectievlak wijst op incomplete resectie (eerst chemotherapie, dan radiotherapie). • Zo medisch inoperabel: Radicale radiotherapie.

1/ Vroegtijdige stadia: – Stadium IIIA (T1-3 unforeseenN2, i.e. N2 na negatieve PET-CT en/of mediastinoscopie, dus tijdens thoracotomie gevonden) • Heelkunde. • Adjuvante chemotherapie bij fitte patiënten. • Adjuvante radiotherapie indien het sectievlak wijst op incomplete resectie (de sequens is dan eerst chemotherapie, erna radiotherapie).

2/ Lokaal gevorderde stadia: Multimodale therapie
“in opzet curatief”
– remissie en/of genezing, wordt mee bepaald door "fitheid" van de patiënt. – Richtinggevend hierin zijn: • WHO performance status <2, of Karnofsky >=80%. • Voldoende cardiopulmonale functie. • Labo bevindingen die chemotherapie toelaten (hematologie, leverfunctie, nierfunctie). • Gewichtsverlies <10% in de laatste 3 maanden.

2/ Lokaal gevorderde stadia: – Potentieel resecabel: • Stadium IIIA- N2 – Inductiechemotherapie, bij downstaging, heelkunde – Bij progressie onder chemo: RT/ 2de lijn chemo/ BSC – Bij positieve N2 of positief sectievlak na resectie: RT
• Sulcus superior cT3-4 N0-1 – Inductie chemoradiotherapie – heelkunde - consolidatie chemotherapie

2/ Lokaal gevorderde stadia: – Potentieel resecabel – Irresecabel: Stadium IIIA – bulky N2 en stadium IIIB: • Concurrente chemoradiotherapie = standaard • Sequentiële chemoradiotherapie: bij minder fitte patiënt of te groot tumorvolume (max 4 weken interval) • Bij nog minder fitte patiënt of progressie onder chemo: evt radiotherapie voor lokale tumor/symptoomcontrole

3/ Gevorderde stadia:
“Palliatief”
– Best supportive care – Differentiatie tussen spinocellulair carcinoom en nietspinocellulaire carcinoom (EGFR mutatie-analyse/ IHC) – Eerste, tweede, derde lijnsbehandeling met chemotherapie/ EGFR-TKI

1/ Vroegtijdige stadia: heelkunde + evt adjuvante chemo “Curatief” 2/ Lokaal gevorderde stadia potentieel resecabel: inductie chemo + heelkunde irresecabel: concurrente chemo-radiotherapie “In opzet curatief” 3/ Gevorderde stadia: Chemo / TKI / BSC “Palliatief”

1/ Limited disease: aantasting beperkt tot één thoraxhelft met inbegrip van de hilaire, ipsilaterale en contralaterale mediastinale, of ipsilaterale en contralaterale supraclaviculaire lymfeknopen, evenals patiënten met ipsilaterale pleurale effusie.
2/ Extensive disease: elke verdere uitbreiding.

1/ Limited disease:
“In opzet curatief”
– concurrente of sequentiële chemoradiotherapie (afhankelijk van fitheid van de patiënt (performantiescore, co-morbiditeit, leeftijd)).
– Steeds gevolgd door PCI – Heelkunde: • ‘very limited’ (maximaal cT1-3N0-1cM0, of stadium I-II, na negatieve mediastinoscopie). • Vaak diagnose pas gesteld tijdens operatie van nodulus met preoperatief ongekende histologie. • Steeds mediastinale klieruitruiming. • Steeds gevolgd door adjuvante chemotherapie (4 tot 6 cycli). • Gevolgd door mediastinale radiotherapie zo klieraantasting (≥ pN1). • Steeds gevolgd door hersenbestraling.

– Chemotherapie – PCI bij respons op chemotherapie – Radiotherapie heeft enkel een plaats als lokale palliatieve behandeling
