Řešení mnohočetných agenezípomocídentálních implantátů Dental implants as a finál solution of multiple agenesis

U mnohočetných agenezí ve frontální i v laterální oblasti reprezentují významnou terapeutickou možnost. Implantáty mohou být solitární nebo tvoří pilíře fixních můstků. V případech mnohočetné ageneze bývají zbylé zuby v anomálním postavení, protože jim chybí podpora sousedů. Ortodontista vytváří dostatečný prostor pro implantát a brání relapsu anomálie. Musí také spolupracovat s protetikem. Umístění pilíře a korunky či můstku je výsledkem multidisciplinárního přístupu. (Ortodoncie 2001,10, č.1, s. 17-23) Summary Dental implants are commonly ušed to replace single teeth orgroups ofteeth missing due to various reasons. They represent a meaningful solution in serious cases of multiple agenesis both in the fronta! and in the iaterai regions. Dental implants can be either as sólo implants or as an abutment of a bridgework. In many cases of multiple agenesis the remaining teeth are in malposition because of the lack of adjacent support. The orthodontist must gain sufficient space in the implant area and retain the orthodontically moved teeth to avoid relapse; the cooperation with a prosthodontist is also necessary. The position of the abutment, the crown or the bridge is the result ofa multidisciplinary effort. meziálně posouváme tak, aby uzavřely mezeru po chybějícím řezáku. Kompozitní dostavba špičáků vylepší estetický dojem a první premoláry budou na místě špičáků [8]). 2. Proteticky, tedy můstky [4,6]. 3. Pomocí dentálních implantátů [8]. V případě neléčené mnohočetné ageneze jsou přítomné stálé zuby téměř vždy v malpozici. Vychází to z jejich přirozené tendence zaplnit co nejvíce místa v zubní řadě. Dočasné zuby s více či méně resorbovanými kořeny velmi často perzistují. Je nutné počítat s rotacemi, sklony, supraokluzemi a posuny stálých zubů, se zkříženým skusem, nonokluzí a ve vážnějších případech i s úplným kolapsem výšky skusu [8]. Agenezi zubů je možné diagnostikovat relativně brzy. Prvotní léčení pomocí mezerníků je estetickým řešením, profylaktickým opatřením proti zmíněným změnám poloh sousedních zubů a v případě mnohočetných agenezi může sloužit ke zlepšení výslovnosti [5]. K definitivnímu zaplnění mezer se přistupuje až po ukončení růstu. Implantáty je možné umístit do čelistí u dívek nejdříve v šestnácti, u chlapců až v osmnácti letech. V tomto období je již konec pubertálního růstového spurtu, ale i následný zbytkový růst může mít rušivé vlivy [12]. Zhotovení, vyhodnocení a překrytí dvou kefalometrických snímků s půlročním odstupem zpomalení růstu ozřejmí. Pro implantologa je významné, že alveol v místě ageneze zubu většinou není dostatečně vyvinutý. Implantace pak vyžaduje augmentační zákrok, nejčastěji přenos autogenního kostního štěpu z brady [10]. Implantovat v krajině horních molárů není téměř nikdy možné bez předchozí elevace spodiny čelistní dutiny (sinus lift) [10]. V horním frontálním úseku jsou neobyčejně vysoké požadavky estetické [9]. Obtížné je zejména vytvořit slizniční lem vkrčkových partiích (tzv. "červená estetika"). V molárové krajině dolníčelisti zase vyvstává nebezpečí přetížení implantátu [11]. Kasuistika 16-letá dívka byla odeslána ošetřujícím stomatologem ke konziliárnímu vyšetření eventuálně k převzetí do péče s předběžnou diagnózou mnohočetné ageneze zubů (obr. 1). Toho času nosila mezerník. Po zhotovení ortopantomogramu, telerentgenových snímků, modelů a po klinickém vyšetření jsme stanovili ortodontickou diagnózu: skeletální I. třída, neutrální rotace, retruzní postavení horních i dolních řezáků, skeletální věk - Ru, pacientka je po pubertálním růstovém spurtu. Dentálnídiagnostika: Angle II. třída oboustranně v molárech i ve špičácích, incizální schůdek 3,7 mm, hloubka skusu 3,9 mm, střed horního zubního oblouku je posunut o 2 mm doprava vzhledem ke 18

canines may be mesially moved to close the space after the missing incisor. The remodelling of canines (by means of composite plastic) improves the esthetic effect and first premolars will occupy the pláce of canines [8]). 2) Purely prosthetic approach (i.e. by means of bridgeworks [4, 6]). 3) Dental implants placement [8]. In čase of non-treated multiple agenesis the present permanent teeth are almost always in malposition due to their natural tendency to fill as much space as possible in the dental arch. Věry often deciduous teeth (with more or less resorbed roots) persist. It is necessary to consider rotations, inclinations, supraocclusions and malpositions of permanent teeth, lateral crossbite, nonocclusion, and in more serious cases also a total collapse of the overbite [8]. Dental agenesis is possible to diagnose rather early. The primary treatment by use of space maintainers is not only an esthetic solution but also a profylactic measure against the abovementioned alterations in the position of the adjacent teeth. In čase of multiple agenesis it may serve to improve the speech [5]. The finál space closure is made only after the growth is completed. Implants may be placed in thejaws nosoonerthan at the age of 16 in girls and 18 in boys. The pubertal growth spurt is finished at this age, however, the subsequent residual growth may intervene as well [12]. Taking, evaluation and superimposition of two cephalometric X-ray pictures within six months will spell out the slowing down of the growth. It is an important fact for an implantologist to know that the alveolar bone in the pláce of a tooth agenesis is usually not sufficiently developed. It is necessary then to perform bone augmentation, most often by means of the transfer of an autogenous bone graft from the chin [10]. It is hardly ever possible to apply an implant in the pláce of upper molars without prior elevation of the base of a maxillary sinus (sinus lift) [10]. The esthetic requirements are exceptionally demanding in the upperfrontal region [9]. It is especially difficultto create a mucous border surrounding the crown neck (the socalled 'red esthetics'). In the molar region of the mandible there is the danger of implant overload [11]. Čase Report A 16-year-old girl was sent to our department by her dentist for a consultation or a treatment respectively with a preliminary diagnosis of multiple agenesis (Fig.1). She wore a space maintainer at that time. On completion of orthopantomographic radiograph, cephalometric X-ray pictures, modelsand after a clinical examination the orthodontic diagnosis was arrived at: Skeletal Class I, neutral rotation, retrusion of both upper and lower incisors, skeletal age - Ru, the pubertal growth spurt is finished. Occlusal diagnosis: Angle Class II bilateral - both in molars and canines, overjet 3.7 mm, overbite 3.9 mm,

střední čáře obličeje, bukální nonokluze 26, meziální sklon a inklinace 13 a 23, ageneze 12, 14, 15, 18, 22, 24, 28, 35, 37, 38, 45, 47 a 48, perzistence 55, 64 a 75 (obr. 2,4,6). Předběžný léčebný plán: Solitérní dentální implantáty v lokalizaci 12 a 22. Implantáty určené k náhradě chybějících premolárů budou zavedeny do méně kvalitní kosti a z obavy před přetížením budou spojeny s prvním molárem. Metalokeramické suprakonstrukce. Ortodontické předléčení: Extrakce 64, 55, konzervativní ošetření 75. Cíl: Angle II oboustranně v molárech, Angle I oboustranně ve špičácích, derotace a transverzální zúžení vzdálenosti mezi 16 a 26. Plný horní fixní aparát, bez léčení v dolním zubním oblouku. Distální rotace a inklinace 13 a 23, mírná vestibulární inklinace 11 a 21, uzávěr diastematu tak, aby střed horního zubního oblouku byl ve střední čáře obličeje (obr. 3,5,7). Po dobu předléčení budou mezery v zubním oblouku vyplněny čtyřmi umělými zuby se zámky. Aktivní ortodontická terapie trvala 15 měsíců, od února 97 do května 98. Po zúžení mezimolárové vzdálenosti bihelixem (obr. 8) jsme nasadili palatinální oblouk dle Goshgariana. Dostatečnou mezeru (5 mm) pro implantáty 12 a 22 jsme vytvořili pomocítechniky straight wire. Dosáhlo se uspokojivých vztahů ve špičácích a molárech. Po celou dobu ortodontického předléčení měla pacientka čtyři umělé zuby, na nichž byly připevněné zámky se slotem 0,018" (obr. 9 až 11) a byly přivázané na drátěný oblouk. Fixtury jsme implantovali v září 1998 (obr. 12), získanou vzdálenost mezi sousedními zuby jsme zajistili po dobu kostního hojení mezerníkem. Suprakonstrukce byla zhotovena v květnu 1999. Pacientkaje nynív dispenzární péči Implantologického centra Stomatologické kliniky FN v Hradci Králové (obr. 13 až 16). V dolním pravém kvadrantu zhotovíme fixní můstek.

midline discrepancy (maxillary dental midline moved to the right by 2 mm in relation to facial midline), buccal nonocclusion 26, mesial inclination 13 and 23, agenesis 12,14,15,18, 22, 24, 28, 35, 37, 38, 45, 47 and 48, persistence 55, 64 and 75 (Fig. 2,4, 6). Preliminary treatment pian: Sólo dental implants in locations 12 and 22. Implants replacing the missing premolars will be introduced into a bone of inferior quality. To avoid overload the implants will be joined to the first molar. Metaloceramic supraconstructions. Orthodontic pre-treatment: Extraction 64, 55, restorative treatment 75. The objective: Angle Class II - bilateral in molars, Angle Class I - bilateral in canines, derotation and transversal narrowing of a distance between 16 and 26. Full upper fixed appliance, no treatment in the lower dental arch. Distal rotation and inclination 13 and 23, moderate vestibular inclination 11 and 21, closure of the diastema so that the upper dental arch midline meets the facial midline (Fig. 3, 5, 7). During the pretreatment the diastemas in the dental arch will be filled with four artificial teeth with brackets. The active orthodontic treatment took 15 months between February 1997 and May 1998. After the distance between molars was narrowed with bihelix (Fig.8), the Goshgarian-type palatal arch was applied. The sufficient space (i.e. 5 mm) for the implants 12 and 22 was secured by use of a straight-wire appliance. The relations in canines and molars were satisfactory. During the orthodontic pre-treatment the patient had four artificial teeth with brackets of slot size 0.018" (Fig. 9-11) ligated to an archwire. The fixtures were implanted in September 1998 (Fig. 12), the gained space between the adjacent teeth was secured with a space maintainer in the course of bone healing. The supraconstruction was made in May 1999. The patient is currently in the observation of the Implantology Centre, Faculty Hospital in Hradec Králové (Fig. 13 -16). In the lower right quadrant the fixed bridge will be applied.

Diskuze V plánování ortodontické léčby a následné interdisciplinárníterapieu pacienta s mnohočetnou agenezí zubů je nutné si uvědomit, že implantáty se mohou zavést do čelisti až po ukončení růstu. Někteří dokonce nedoporučují implantovat dřív než ve věku 20 - 22 let. Thilanderová [12] implantovala u pacientů ve věku od 14 do 19 let (v průměru 15 let a 4 měsíce) a došlo k infraokluzi implantátů ve frontální oblasti průměrně o 0,1 mm za rok. Relativní zanořování implantátů do alveolu činilo v prvních pěti letech po implantaci celkem 0,7 mm, ačkoliv již žádné růstové změny v kraniofaciální oblasti v tomto období nebyly patrné. Celková infraokluze dosáhla 1,5 mm během osmiletého pozorování. To již mělo vážné estetické důsledky. Implantát se tedy choval jako ankylotický zub. Okolní struktury pokračovaly ve vývoji (a okolní zuby v erupci) i po ukončení tělesného růstu. Ortodontické předléčení by mělo vytvořit dostatek místa pro implantát se suprakonstrukcí a mělo by intrudovat zuby co nejméně, a to proto, že relaps intruzních pohybů může zvýraznit pozdější infraokluzi implantátu [12]. Ideální je, když implantační fáze navazuje na fázi ortodontickou tak, že se vhojovací období fixtury překrývá s posledními měsíci ortodontické léčby. Často se stane, že vedle dočasného špičáku prořezává do mezery po chybějícím laterálním řezáku stálý špičák. Pak je možné dočasný špičák ponechat až do konce růstu [7] anebo alespoň do doby, než můžeme začít ortodontické předléčení (15 - 16 let). Přítomné kořeny špičáků jsou zárukou pro dostatečnou šířku a výšku alveolárního výběžku. To může platit i pro ostatní dočasné zuby, které nemají příliš resorbovaný kořen a destruovanou korunku [7]. Discussion In planning of the orthodontic treatment and subsequent interdisciplinary treatment in a patient suffering from multiple agenesis it is necessary to rememberthatthe implants may be introduced into ajaw only after the growth is completed. Some experts suggestto considerthe implantation not until the age of 20 - 22. Thilander [12] performed implantation in patients of the age between 14 and 19 (i.e. the average age was 15 years and 4 months). The infraposition of the implants in thefrontal region equaled 0.1 mm peryear. The relative intrusion of the implants into alveolus made 0.7 mm in the five years following the implantation, though there were no evident growth changes in the craniofacial area during that period. During the eight yearšfollow up the infraposition made 1.5 mm and posed serious esthetic consequences. The implant behaved as an ankylotic tooth. Adjacent structures continued their development (and adjacent teeth their eruption) even after the growth had been finished. The orthodontic pre-treatment should result in opening sufficient space for an implant and supraconstruction. The intrusion should be as little as possible because the relapse of the intrusive tooth movement may strengthen the subsequent infraposition of the implant [12]. It is suitable when the implantation phase follows the orthodontic pre-treatment so that the fixture healing period overlaps the last months of the orthodontic treatment. It often happens that alongside a deciduous canine a permanent canine is erupting in the space after a missing lateral incisor. It is possible to retain the deciduous canine till the growth is finished [7] or at least till the orthodontic pre-treatment is due to start (i.e. till the age of 15 - 16). The present roots of canines guarantee that the width and height of the alveolar ridge is adequate. This may be valid also for the other deciduous teeth without marked root resorption and with the crown that is not destroyed [7]. For instance in

cii druhých premolárů je vhodné ponechat druhý dočasný molár. U mnohočetných agenezí by se předléčení mělo provádět tak, aby ve špičácích byla I. třída dle Anglea [5], plná třída v molárech, dále je nutné zajistit dobrou výšku skusu, vyhovující incizální schůdek a léčit zkřížený skus. Velmi často pacient žádá estetické řešení již v období ortodontického předléčení. Spíše než snímatelný mezerník se osvědčily umělé zuby s nalepenými zámky se slotem .018", které jsou volně přivázané k drátěnému oblouku. Již na začátku ortodontického předléčení musí být jasné, kolik implantátů bude indikováno, kam budou umístěny a zda je nutná augmentace kosti, prodlužující chirurgickou fázi léčby. Posouzení těchto souvislostí je na domluvě ortodontisty s implantologem.

hypodontia of second premolars it is advisable to maintain the second deciduous molar. In multiple agenesis the pre-treatment should result in Class I in canines [5], the full class in molars. It is also necessary to ensure an appropriate overbite, satisfactory overjet, and to correct the lateral crossbite. Quite often patients ask for esthetic solution even during the orthodontic pre-treatment. Artificial teeth with bonded brackets the slot size of .018" attached to an archwire seem to be more suitable than a removable space-maintainer. As early as at the very beginning of the orthodontic pretreatment the numberof implants indicated must be clearly stated, their position as well as whether the bone augmentation resulting in the prolongation of the surgical phase of treatment is necessary. These questions must be the subject of discussion between an orthodontist and an implantologist.
Terapie oligodoncie je téměř vždy multidisciplinární. Na ortodontickou fázi navazuje buď implantace a pak zhotovení suprakonstrukce, anebo přímo protetické řešení. Čistě ortodontickymi způsoby lze obvykle uzavřít mezery pouze v případě nezaložených maximálně dvou zubů v jedné čelisti (kromě třetích molárů). K definitivnímu řešení situace lze přistoupit až po ukončení růstu. Nestačí konec růstového spurtu, zbytkový růst v adolescentním věku může vést k výrazným změnám v postavení implantátů vůči okolí. Ideální období k započetí ortodontického předléčení mnohočetných agenezí je u dívek ve věku 14 -15 let, u chlapců o1 -1,5 roku později. Do té doby je nutné udržet dokonalou ústní hygienu a eventuálně zhotovit mezerník z důvodů estetických, jako profylaxi posunů
Conclusion Oligodontia treatment is almost always multidisciplinary. After the orthodontic phase the implantation and supraconstruction follows or prosthetic solution is applied directly. The purely orthodontic approach is ušed to solve the space in čase of the maximum of two teeth agenesis in one jaw (apart from third molars). The finál solution is possible only after the growth is completed - not only the pubertal growth spurt as the residual growth in the adolescents may cause marked changes in the position of the implants towards adjacent teeth. The best age to start the orthodontic pre-treatment of multiple agenesis is 14 -15 in girls, a year or 1.5 years later in boys. Until then it is necessary to maintain a perfect oral hygieně. Sometimes a space maintainer is applied as an esthetic solution but also as a profylaxis of the movements and rotations of the already erupted teeth. The space maintainer may also contribute to a better speech articulation.
