Summary

The aim of this thesis was to assess the effectiveness in the Netherlands of collaborative care for Major Depressive Disorder (MDD) with all the elements of the IMPACT-model and a patient tracking system. Secondary aims were the evaluation of cost-effectiveness and the influence of concomitant physical symptoms on the outcome of MDD treated according to the principles of collaborative care. The main findings of the thesis are described below. 1. Summary (main findings): Effectiveness of collaborative care for MDD in the Netherlands In Chapter 3 the effectiveness of collaborative care for MDD in the Netherlands was assessed. The model tested in the CC:DIP-trial proved to be more effective than care as usual after three and nine months. It also appeared to be more effective after six and twelve months, but the results were not statistically significant. This shows that there is still room for improvement, but the results after three and nine months are promising. The odds ratio (OR) for this effect was around 5 after both three and nine months, which means that the odds on reaching a favourable response to treatment (a reduction of at least 50% on the PHQ9 compared to baseline) were 5 times higher for patients in the collaborative care group than for patients who received care as usual. The number needed to treat (NNT) varied between 2 after three months and 3 after nine months. This implies that a general practitioner (GP) should provide collaborative care treatment for 23 patients to expect response to treatment in one extra patient (compared to care as usual). This is a good result: for instance, studies evaluating the effectiveness of interventions for hypertension with the goal of reducing cardiovascular morbidity and mortality generally report much higher NNT.204;205 Although this comparison might not be completely fair §§ , the low NNT does show that treatment for depression can be just as successful as treatment for some chronic somatic conditions. This promising result may, in part, be due to the fact that the percentages of patients who responded to treatment in the care as usual group were low. Only 10.5% of the patients in the care as usual group responded to treatment after three months. In the collaborative care group as a whole this percentage was 45.9%. In comparison, in the care as usual group in the IMPACT-study the percentage that responded to treatment after three months was 14.8%.12 These low response percentages in the care as usual groups might reflect on the availability of guideline based care for patients receiving care as usual. Indeed Prins and colleagues concluded that patients with depressive and anxiety symptoms requesting treatment in the primary care setting in the Netherlands receive guideline based care in less than half of cases.119 This may be due to certain characteristics of primary care such as the fact that consultations generally last no longer than 10 minutes (which leaves little time for psychosocial problems) and the issue of competing demands (somatic and psychiatric diseases 'compete' for attention, for instance when a patient suffers from diabetes and depression).38

  Our study shows that collaborative care appears to deal successfully with some of these problems enhancing the effectiveness of primary care for MDD. Elements of care that are propagated by the guidelines for depression in the Netherlands11;18, are incorporated in the collaborative care model: e.g. monitoring of the course at least every two weeks, and encouraging pleasurable activities and achievable goals (addressed in the collaborative care model during PST and in the self-help manual). Adherence to the guideline is also facilitated when applying collaborative care because certain time consuming elements of effective treatment are delegated to a care manager, and because specialist advice (by a psychiatrist) is easily available. The collaborative care model may therefore be an important tool to achieve one of the goals of the Depression initiative, of which this project was part: implementation of the Multidisciplinary guideline for depression in the Netherlands.17 These are promising developments, but particularly in times of cutbacks on public spending implementation will still be be very difficult if collaborative care proves to be effective but expensive. This topic is explored below. cost-effectiveness of collaborative care for MDD in the Netherlands In Chapter 4 the cost-effectiveness of collaborative care was assessed. Both the direct medical costs and the indirect costs *** of collaborative care were lower than those for care as usual.The lower indirect costs were mainly due to the decrease of long-term absence in the collaborative care group (this particularly made a difference in the screened collaborative care group). As collaborative care was only slightly more effective in terms of Quality Adjusted Life Years (QALY, this roughly refers to 1 life year in good health that is gained), there is room for improvement in terms of effectiveness when quality of life is the outcome. Nevertheless from the societal perspective (when productivity costs were also assessed), the model became dominant over care as usual. The implications of these findings will be discussed further on in this discussion, but not without noting one more finding regarding cost-effectiveness. When the collaborative care patients who were identified by their GP were added to the analyses the model was also dominant from a direct medical perspective. This implies that GPs might be capable of selecting patients who are particularly likely to benefit from the intervention. More research regarding the success of direct referral by the GP is necessary as was discussed in Chapter 3 of this thesis. The influence of medically unexplained physical symptoms 70% of the patients who go to their GP with health complaints that later turn out to be at least partly associated with MDD, only present physical symptoms at first.25 In other words: the association of physical symptoms with MDD should not be ignored by researchers and clinicians as it is very common. In Chapter 7 the results are presented from a pre planned secondary data analysis on the CC:DIP trial that took concomitant physical symptoms into account. These symptoms were clearly associated with a less favourable response to treatment in both groups in the trial. When the results all patients in the trial were analysed as belonging to one group (collaborative care + care as usual) the odds on a favourable response for patients not suffering from clinically significant concomitant physical symptoms were up to 7 times higher than for patients who did suffer from a clinically significant amount of such symptoms. So, in both groups (care as usual and collaborative care alike) concomitant

physical symptoms were associated with much lower odds on response to treatment. The issue of co-occurrence of physical symptoms and depression thus may shed some light on the question why collaborative care induces a favourable response in 61% of the patients at best (after six months for the collaborative care total group, see Table 2 in Chapter 3 of this thesis) ††† , and why care as usual if only effective for 26% of the patients at best (after nine months). A supplementary finding in Chapter 7, was the fact that the distinction between physical symptoms that were 'explained' by chronic medical disorders and the physical symptoms that were 'unexplained' did not prove to be relevant in the light of the outcome measure: a favourable response to treatment for depression. The severity of physical symptoms (scoring above or below the cut-off on the Physical Symptoms Questionnaire, PSQ, that was given in Chapter 7) turned out to be the factor interfering with effective treatment for MDD, and not the nature of the symptoms (explained by for instance diabetes or COPD, or 'unexplained', for instance unexplained abdominal pain, nausea, or headaches). This finding may be highly relevant, because researchers have pointed out that many studies that assess the epidemiology of medically 'unexplained' symptoms fail to correct for a possible medical explanation (in these cases symptoms are measured on a scale, such as the PSQ, but whether or not these symptoms are actually unexplained is not clear since a medical assessment is lacking). Crombez and colleagues have called this issue 'the unbearable lightness of somatisation'.35 We were able to circumvent this problem to a certain extent in Chapter 7 because we asked the participants in our study to list the chronic medical disorders they were suffering from at baseline. It is advisable that future studies also provide an assessment by a medical doctor at baseline, but at least our method is a step in the right direction. Importantly, the findings from Chapter 7 are in line with those in Chapter 5 and 6 (although we were not able to address the issue of medically unexplained versus medically 'explained' symptoms in Chapter 5 and 6). In Chapter 5 the results from a systematic review were presented. It was concluded that the scientific literature points to a trend for a negative association of concomitant physical symptoms with the course of MDD. The effect might be considerable, but the studies that are available at present do not allow for statistical pooling. This could change in the future if more studies were to use the same outcome measures (for instance by defining a favourable response to depression treatment as a reduction of at least 50% on a validated questionnaire for depression severity). Finally, in Chapter 6 we found that concomitant physical symptoms were a were a strong predictor of a poor outcome in both treatment groups in a trial that assessed the effectiveness of Problem Solving Treatment (PST) in primary care.180 This is an important finding because we also applied PST in our collaborative care algorithm in the CC:DIPtrial. This gives us reason to believe that it might be worthwhile to think about other types of treatment for patients with MDD who are suffering from concomitant physical symptoms. Moreover, given the fact that concomitant physical symptoms interfere with both the effectiveness of PST and collaborative care, PST could be replaced by a form of treatment that is effective for (medically unexplained) physical symptoms if these are also present (for instance if a patient scores above the cut-off for clinically significant physical symptoms on the Physical Symptoms Questionnaire, PSQ, as applied in Chapter 7). A more elaborate discussion of this topic will be provided below.

General discussion

This section provides a more in-depth look into the factors that influence the effectiveness of collaborative care in primary care and discusses future directions for research and possibilities for implementation of the model in the Netherlands. The results with regard to the effectiveness of collaborative care (Chapter 3) are promising, particularly the low Number Needed to Treat (NNT). This means that it would be relatively easy to gain good results if collaborative care were to be implemented successfully. Furthermore, the results with regard to cost-effectiveness (Chapter 4) show that at least from a societal perspective (taking productivity costs into account) collaborative care is dominant over care as usual. Moreover, there is a good chance that cost-effectiveness can be further enhanced if GPs identify the patients eligible for collaborative care directly instead of by screening (screening is not common practice anyway). If the GP identified patients are included in the analyses collaborative care is also dominant from a direct medical perspective. More research into the benefits of GP identification instead of screening is necessary (for instance in a patient randomized trial in which all patients are identified by the GP), but the outlook is promising. Whether or not (cost)effectiveness could also be enhanced by adapting the model for patients suffering from concomitant physical symptoms is also an intriguing question. So, should collaborative care be welcomed as if it were the holy grail of mental health care? We do not claim that it is, but the results indeed give reason to implement the model in the Netherlands. The demands stated in the sieve of Dunning (see introduction) are met: treatment for MDD is necessary in primary care (the prevalence of MDD is too high to be met exclusively in secondary care), collaborative care is more effective than care as usual, and the costs are reasonable. Not all problems are solved however. And as those of you who have knowledge of Problem Solving Treatment will agree, problems are a normal part of life. So, which issues do still remain? I. Complete remission is still rare. Concomitant physical symptoms and personality disorders provide leads for future improvements Firstly, collaborative care induces a relative improvement: it is more effective than care as usual, but this does not mean that all patients benefit. As mentioned before, the maximum percentage of patients with a favourable response to treatment is 61%. Furthermore, if we apply more strict criteria and look at complete remission of depressive symptoms (PHQ9<5), the maximum percentage goes down to 37.7% (the total collaborative care group after 9 months of treatment)! Put differently: 62.3% of the patients never experience a complete remission of depression during the trial! The conclusion should be that MDD can be treated more successfully applying collaborative care, but that more research is needed to increase the efficacy of treatment for MDD in general. Increasing the remission percentages is of paramount importance, particularly because complete remission is associated with a decreased probability of relapse.193 Given that it has been stated that depression causes a decrease in quality of life of 50% compared to the general population, which is in the same range as after a severe stroke193, preventing relapse and a chronic course should be the aim of treatment. Sobocki and colleagues observed that for patients who achieved remission both health care costs and quality of life increased with more than 40% as compared to non remitting patients.193

Fortunately we are not completely in the dark on the way forward. The CC:DIP study provides two important leads that can be followed in the future. The first lead is connected to physical comorbidity. Chapter 7 showed that both care as usual and collaborative care are less effective for patients suffering from MDD and concomitant physical symptoms. The results from Chapter 6 point out that this might be connected to the fact that physical symptoms also limit the effectiveness of Problem Solving Treatment (PST), one of the prime ingredients of our collaborative care model. So, could the effectiveness of collaborative care be enhanced by adapting the model to make it more suitable for patients suffering from concomitant physical symptoms? There is reason to assume that this may be the case. A study assessing the effectiveness of a collaborative care model for patients suffering from medically unexplained symptoms (and not necessarily depression) showed good results. In this trial all patients were seen by the GP and a consultant psychiatrist.14 In the CC:DIP-trial patients were only seen by the consultant psychiatrist in case of stagnation. One advice could therefore be that all patients who score above the cut-off on the Physical Symptoms Questionnaire (PSQ) are seen by the GP, the caremanager and the consultant psychiatrist before collaborative care treatment is initiated in primary care (and not just by the GP and the caremanager). Another option (not necessarily excluding the previous one) would be to replace PST by a form of treatment that specifically targets medically unexplained physical symptoms if patients score above the cut-off for clinically significant physical symptoms on the PSQ. Referring these patients to a so called psychosomatic physiotherapist could be an option, as these physiotherapists have been trained to recognize and influence the complex interplay between motor function, mental function, and psychosocial context.203 Unfortunately at present, to my knowledge, no randomised controlled trial has evaluated the effectiveness of psychosomatic physiotherapy. A collaborative care trial that incorporates this form of treatment as described above, comparing collaborative care without psychosomatic physiotherapy to collaborative care with psychosomatic physiotherapy, would be an excellent way to prove added value. Another option might be to replace PST by brief psychodynamic interpersonal psychotherapy, because this was found to be effective for patients suffering from multisomatoform disorder. In that particular trial though, depression did not improve (the main improvement was a decreased physical component score on the SF36). But this could possibly change if brief psychodynamic interpersonal psychotherapy was incorporated in a collaborative care model that also focuses on depression.206 Interestingly, during the exit-interviews after the CC:DIP-trial had finished, GPs did not mention concomitant physical symptoms as one of the factors that may interfere with the effectiveness of collaborative care. Instead they did mention comorbid personality disorders and history of traumatic experiences.207 Severe somatisation has been linked to personality disorders and traumatic experiences in early childhood.208 Perhaps GPs are indirectly linking severe medically unexplained physical symptoms (the cut-off on the PSQ applied in the CC:DIP trial points to severe concomitant physical symptoms) to personality disorders pointing to a group of patients that is hard to treat in primary care. This is an intriguing thought that could explain why GPs did not mention concomitant physical symptoms, but we did not ask them whether this was indeed what they meant. In any case expanding the role of the consultant psychiatrist appears to be necessary for both personality disorders and medically unexplained physical symptoms. Referral to secondary care, to a primary care psychologist (for instance for brief interpersonal psychodynamic psychotherapy), or to a psychosomatic physiotherapist should be options that the consultant psychiatrist keeps in mind in these cases depending on the specific clinical picture.

  II. Will collaborative care help to decrease the prevalence and societal burden of Major Depressive Disorder? Another issue altogether is that of the societal impact of collaborative care. In Section 1 of the general introduction of this thesis the case was made that MDD can be seen as a huge burden for society. If the burden is huge, then the benefits of effective treatment are also possibly large, even if there is room for improvement when looking at the remission percentages. Given the results regarding cost-effectiveness in primary care, there is reason to be hopeful. Nevertheless, two questions arise with respect to this topic: 1. Is it possible to implement collaborative care successfully? Proving effectiveness in a trial is one thing. Implementation in everyday care is something else. 2. If collaborative care is implemented, will it reach a sufficient amount of possible patients to have an impact on prevalence and societal burden? With regard to the first question, reimbursement will of course be of vital importance for successful implementation, but other (in part more practical) issues connected to everyday care should not be ignored either. Given the fact that collaborative care appears to be cost-effective, the outlook for reimbursement seems promising. This section will therefore focus on practical issues that may influence implementation. One of the factors that has been associated with better depression outcomes after a collaborative care intervention was regular and planned supervision of care managers by a psychiatrist (quite probably this could also be a psychologist, for instance with respect to Problem Solving Treatment).209 This is relatively easy to organize for a trial, but might be a lot harder in every day care, especially if care managers are not employed by a mental health care institution. Another practical issue is connected to the webbased patient tracking system that operated during the trial. GPs reported that they would be more inclined to use the system if this could be connected to the systems they already use to keep their electronic patient files. We were not able to achieve this during the trial (the systems operated next to each other). It should of course be possible to solve these technical problems, but this is somewhat more problematic than one would think at first glance. Several systems are used in primary care in the Netherlands (Medicom and Omnihis in our study) and nationwide implementation would imply connecting the webbased patient tracking system to all different types of electronic patient files. This is one example of how one of the characteristics of primary care in the Netherlands could complicate implementation of collaborative care (and probably of many disease management models): in the Netherlands, every primary care practice is a small business unit with its own culture and rules (and electronic patients system). The number of GPs in one practice usually ranges from 1-5, 2 GPs on average.124;125 Although this appears to be changing rapidly many GPs are not co-located with other health care professionals such as psychiatric nurses, social workers, psychologists, or physiotherapists.16 Furthermore, the reimbursement system in the Netherlands demands regular negotiations between each health care provider and the various health care insurance companies.16 The UK, the other European country where the IMPACT collaborative care model was found to be effective, has a nationalized health service and a larger average number of GPs per primary care practice.121 This could make implementation of disease management models relatively easier. On the other hand,

  given the fact that GP-practices in the Netherlands are in essence small business units, collaborative care could be attractive to GPs if health care insurers (or other stakeholders) are willing to reward implementation of the model. Implementation could also be influenced should the personal contribution to mental health care that patients had to pay in the Netherlands in 2012 return. This personal contribution is not part of the preliminary plans for 2013, but it might return in the light of the discussion on the ever increasing expenditure on health care (in any case, it will be interesting to compare 2012 to 2013 with regard to mental health care service use). The personal contribution in 2012 for mental health care in primary care was 20 Euros per session, which corresponded to 120 Euros for 6 sessions with a caremanager (for instance a primary care psychologist). An exception was made for the mental health care-practice nurse (MHC-PN, or Praktijkondersteuner GGZ, POH-GGZ in Dutch) who works directly for the GP. It seems possible that patients would refuse primary mental health care that includes care management, should the personal contribution return and should the exception for MHC-PN disappear. This could lead to an increase of the prescription of antidepressant medication (without monitoring), an issue that is discussed below. The second question that was posed above was whether or not collaborative care could reach a sufficient amount of possible patients to have an impact on prevalence and societal burden of MDD, if it were to be implemented. The answer might be yes if one looks at the prevalence of MDD in primary care, which is approximately 10%.5 Whether this is also the case for the general population remains to be seen, mainly because it is not always easy to convince patients who might have a depression to accept treatment for this condition. In her thesis Wittkampf proposed that there may be a relatively large group of patients that is not motivated for treatment. Some of the patients in her study connected their symptoms to transient problems in their lives, such as financial troubles, worries about housing, or relationship problems. They claimed that their symptoms would improve when the problems were over.210 Comparable to Wittkampf's findings it also proved remarkably hard to find a sufficient amount of patients to participate in the CC:DIP-study: only 25% of the patients returned the screening questionnaire (the PHQ9). A screener had to be send out to almost 27.000 patients to include 93 screened patients ‡‡‡ in the trial! Indeed, quite a few patients were not motivated for treatment. It thus appears to be hard to target 'new' patients who are depressed according to the criteria for MDD, but who are not in care at the moment. This problem is not solved by collaborative care and screening for depression might not solve it either. Therefore, even though collaborative care is an effective intervention, its influence (if implemented on a wide scale) on the prevalence of depression might be modest. As mentioned before, one of the reasons why patients often do not receive treatment for MDD may be that not everyone accepts 'the depression label'.210 Apparently some patients who suffer from MDD according to the DSM IV criteria do not feel that they have this disorder (or they associate their symptoms with something else). The finding by Wittkampf210 that at least some of them connect feelings of depression to transient problems and not to a medical disorder is reminiscent of the claim made by Horwitz and Wakefield (see general introduction Section 1) that the criteria for MDD may be overinclusive.4 In the future we might see a debate on whether or not the DSM-criteria

for classifying MDD should be adapted. §§§ Feelings of depression may well be a normal reaction to for instance loss of a long lasting love relationship (and not just to a loss experience connected to bereavement) or to severe financial problems. In these instances coaching or internet interventions for specific problems (such as financial troubles) instead of a medical diagnosis could be less stigmatizing. Problem Solving Treatment could, for instance, easily be applied outside the context of MDD. The same goes for interventions that are focused on increasing happiness instead of curing sadness211, and for instance for running therapy81 and other interventions focused on life style changes. If a person still has a depressed mood after the problems have passed, fails to address the problems adequately after help has been offered (for instance through an internet intervention for work related stress or by a financial coach), or still feels very sad months after a love relationship has ended, a diagnosis of depression may have more face validity. Having said this, I do not agree that the symptoms that are currently used to diagnose MDD should be ignored. As noted in section one of the general introduction, the societal burden of these symptoms is very high.6;7 Symptoms such as concentration problems and insomnia could also be a bigger problem than they were some decades ago. For the majority of the workforce in the Western world, the nature of work has shifted from manual labor to knowledge-work. This is type of work is very difficult if you are, for instance, having concentration problems. Because of this it seems unwise not to apply the MDD-classification in its current form in health care settings, before a new paradigm for dealing with these problems has been established. Many effective interventions are based on the current MDD-criteria. These interventions may decrease the economic burden of MDD and, more importantly will decrease suffering for many patients. This may also be the case for collaborative care. The question of whether or not we should actively try to persuade the general population to visit their GP if they are feeling sad or blue (for instance through mass screening, and not targeting screening on high risk groups, such as patients suffering from diabetes, COPD, or somatoform disorders) is another question. The answer may well be no.212 Collaborative care could also play a part in dealing with one of the problems that is commonly associated with the medicalization of MDD, namely the fact that according to estimations approximately 1 million people in the Netherlands are using antidepressant medication.213 This may have something to do with the time constrained nature of primary care. A General Practitioner who has to deal with a patients' complex psychosocial problems within in a ten minute consultation will generally not be very effective at doing this. If the possibility to delegate such 'time consuming' elements of care (to a caremanager) exists, a GP may less inclined to choose 'a quick solution' such a prescribing antidepressants. If care management and brief psychological treatment (such as Problem Solving Treatment) are available in the health care centre where a patient also consults his or her GP, this might also make it less difficult for him or her to accept treatment based on 'talking instead of medication' (or talking and medication instead of medication as standalone treatment).   called 'Rita down the hall effect', named after one the caremanagers who participated in the project. Viewed this way collaborative care could be one of the facilitators that could help GPs to implements new guidelines for treatment of depression that are due in the Netherlands. Recently newspapers in the Netherlands reported that the Dutch College of General Practitioners is moving towards regulations that state that GPs should only prescribe antidepressant medication for relatively severe depressive symptoms because this type of medication does appear to be less effective for mild symptoms.214 This could well be the outcome of the before mentioned debate on the criteria for depression in the future: 'all or nothing' criteria (see box 1 in the general introduction) might not suffice. Diagnosing depression on a continuum could be the way forward (for instance by measuring depressive symptoms on a scale such as the PHQ9). Low intensity interventions (such as behavioral activation, motivational interviewing, PST, and perhaps running therapy) could be targeted at mild symptoms (for instance a PHQ9-score between 5 and 10), whereas antidepressant medication could be reserved for relatively severe symptoms (for instance a PHQ9-score of at least 10). Such an approach based on 'staging and profiling'215 seems promising. More research into this is necessary. 3. Conclusion Summing up the main findings from this thesis, a concluding remark could be that treating depression should be a team effort in which the GP, the psychiatrist or clinical psychologist, a caremanager, and of course the patient collaborate. As was stated during the training the IMPACT-group provided at the start of the CC:DIP-project: "None is as smart as all of us." Another important finding is that it may be possible to enhance the effectiveness of collaborative care by adding elements to the model that increase its impact on concomitant physical symptoms. Ten years ago the Netherlands commission on mental health care published the report 'A care of many' (in Dutch: zorg van velen)216 on the reasons for the increasing demand for mental health care in the general population. One of the recommendations was that specialized mental health care should be integrated with other health care settings and other organizations that promote wellbeing in the general population. Stepped care with specialized mental health care as the last step was noted as one of the methods to achieve such integration. Collaborative care incorporates stepped care and also facilitates cooperation between primary and specialized care. These are important reasons to proceed with the implementation of the model (as well as the results from CC:DIP regarding cost-effectiveness). Another recommendation in the report was that mental wellbeing should indeed be 'a care of many'216 and that the community should participate in this, perhaps against the societal trend of individualization. Feeling lonely is probably associated with depression. Here lies an important task for society as a whole.

Samenvatting Collaborative care voor depressie in de eerstelijnszorg en de invloed van bijkomende lichamelijke klachten. Een proefschrift uit het Depressie Initiatief.

Hoewel veel mensen bij het woord depressie misschien denken aan een bezoek aan de psycholoog of psychiater, komen veruit de meeste mensen bij wie later een depressie wordt vastgesteld eerst bij de huisarts. In de meeste gevallen is de hoofdklacht ook niet somberheid of nergens meer plezier aan kunnen beleven, maar een lichamelijke klacht zoals hoofdpijn, buikpijn, duizeligheid of vermoeidheid.25 Hiermee wordt meteen duidelijk dat de behandeling van depressie voor de huisarts geen gemakkelijke taak is: depressie komt vaak voor (naar schatting ongeveer 10% van de patiënten die de huisarts ziet)5, 'verschuilt' zich vaak achter lichamelijke klachten, en blijkt ook nog eens vaak samen te gaan met lichamelijke ziekten als diabetes, COPD en hart- en vaatziekten.26 Omdat depressies ook geassocieerd zijn met psychosociale problemen (denk aan financiële problemen, relatiecrises, ruzie op het werk)39;176, is het van belang dat hier ook ruimte voor is. Ga er maar aan staan in 10 minuten (of 20 minuten bij een dubbel consult)! De oplossing lijkt echter niet dat al deze patiënten worden doorverwezen naar de tweede lijn (gespecialiseerde instellingen op het gebied van de Geestelijke Gezondheidszorg, GGZ). Daar zijn al wachtlijsten en bovendien is de prevalentie van depressie te hoog: op elk moment voldoet naar schatting ongeveer 5% van de Nederlandse bevolking aan de criteria voor een depressie: ongeveer 800.000 mensen. Hoewel lang niet al deze mensen zich in het 'depressie-label' zullen herkennen210 en er enige wetenschappelijke discussie is over de criteria voor een depressieve stoornis4, is het duidelijk dat voldoen aan de hulpvraag van patiënten bij wie bij de huisarts een depressie wordt vastgesteld een uitdaging is. In dit proefschrift is de effectiviteit van collaborative care onderzocht, een in de Verenigde Staten ontwikkeld samenwerkingsmodel dat erop gericht is verschillende hulpverleners in de eerstelijns gezondheidszorg optimaal samen te laten werken.59 Daarbij is een belangrijke rol weggelegd voor de zogeheten caremanager die kortdurende gespreksbehandeling aanbiedt en de organisatie van zorg (bijvoorbeeld monitoring van klachten en overleg met een psychiater) voor een deel overneemt van de huisarts. Een ander belangrijk element is dat de tweedelijn de zorg niet overneemt, maar dat de expertise (in de vorm van overleg of consultatie) wel beschikbaar is. De huisarts blijft hoofdbehandelaar en bepaalt samen met de patiënt, de caremanager en een consulent psychiater welke stappen gezet moeten worden. Op deze manier beoogt het collaborative care model een aantal van de in de vorige alinea's geschetste knelpunten op te lossen. Hieronder worden de resultaten van het CC:DIP-onderzoek samengevat. CC:DIP (Collaborative Care: Depression Initiative in Primary care) was onderdeel van het depressie-initiatief17, een landelijk programma dat liep tussen 2006 en 2011 waarin gestreefd werd de depressiezorg in Nederland te verbeteren. Hoofduitvoerder was het Trimbos-instituut en het onderzoek werd gefinancierd door het Innovatiefonds zorgverzekeraars. CC:DIP was een (cluster)gerandomiseerde trial (experiment) waarbij huisartspraktijken willekeurig werden ingedeeld in een groep die met collaborative care

  ging werken en een groep die gebruikelijke zorg bleef geven. 150 patiënten werden een jaar lang gevolgd in 18 huisartsenpraktijken. De ernst van de depressie werd gemeten met de PHQ9, waarbij de belangrijkste uitkomstmaat 'response' was: een minimaal 50% lagere score ten opzichte van de beginmeting. Twee andere belangrijke aspecten van het onderzoek waren de kosteneffectiviteit (wordt een mogelijk behandeleffect ook tegen maatschappelijk aanvaardbare kosten bereikt) en de invloed van bijkomende lichamelijke klachten (aangezien deze zo vaak samengaan met een depressie). In de appendix (na de referenties) staat het protocol dat t.a.v. collaborative care tijdens CC:DIP is gehanteerd, voor wie geïnteresseerd is in hoe het behandelmodel in de praktijk werd toegepast (daar staat ook een versie van de PHQ9). Hieronder worden de wetenschappelijke uitkomsten van het proefschrift besproken. Na de bespreking van de resultaten volgt een algemene conclusie.

1. De effectiviteit en kosteneffectiviteit van collaborative care in de eerstelijn in Nederland (hoofdstuk 3 en 4): Zoals hierboven beschreven was de belangrijkste uitkomstmaat een afname van de PHQ9-score met tenminste 50% t.o.v. de beginscore (response). Daarnaast werd gekeken naar een strengere uitkomstmaat: remissie. Dat wil zeggen dat het klachtenniveau niet meer klinisch relevant is. Kroenke heeft uitgezocht dat dit overeenkomt met een PHQ9-score tussen 0 en 4.1 De grootste behandeleffecten t.a.v. response worden gevonden na 3 en 9 maanden. Na 3 maanden bereikt 45,9% van de patiënten in de collaborative care groep response tegenover 10,5% in de gebruikelijke zorggroep. Na 9 maanden bereikt 61,0% van de collaborative care groep response tegenover 25,8% in de gebruikelijke zorggroep. Ook na 6 en 12 maanden zijn de verschillen in het voordeel van de collaborative care groep, maar de resultaten zijn dan niet statistisch significant. Ook de resultaten t.a.v. remissie pakken steeds in het voordeel van de collaborative care groep uit. Na 6 maanden is er een statistisch significant effect op remissie voor de patiënten die collaborative care kregen. Bij 23,8% van de patiënten in die groep is de depressie op dat moment in remissie vergeleken met 10,3% in de gebruikelijke zorggroep. In tabel 2 in hoofdstuk 3 staan alle resultaten t.a.v. response en remissie weergegeven. Op basis van de resultaten voor response valt ook een voor de huisarts nuttige uitkomstmaat te berekenen: de number needed to treat (NNT). Dit staat voor het aantal mensen dat een huisarts samen met de caremanager en de consulent psychiater moet behandelen om response te bereiken voor 1 extra patiënt (vergeleken met gebruikelijke zorg). Dit getal was 2 na 3 maanden en 3 na 9 maanden. Dat de NNT zo laag is, zou het aantrekkelijk kunnen maken voor huisartsen om volgens de principes van collaborative care te gaan werken. Het implementeren van een nieuwe methode gaat niet gemakkelijk, maar het levert potentieel veel op: de kans dat een patiënt er baat bij zal hebben is aanzienlijk. Voordat we verder gaan dient een aantal punten genoemd te worden, waar we in dit onderzoek tegen aan liepen. Het grootste probleem was dat er erg veel patiënten (26.580) gescreend moesten worden om uiteindelijk 93 patiënten middels screening te includeren. Daarbij waren we ook wel streng: patiënten moesten minimaal 10 scoren op de PHQ9 en ze moesten op een diagnostisch interview, het MINI neuropsychiatrisch interview, aan de diagnostische criteria voor een depressie voldoen (zie figuur 1 in hoofdstuk 3 voor een overzicht van de instroom van patiënten in het project). Wellicht speelt mee dat uit onderzoek naar voren is gekomen dat niet alle mensen met klachten

als somberheid zich herkennen in het depressie label.210 Zij koppelen hun klachten eerder aan problemen van voorbijgaande aard (zoals relatieproblemen of financiële problemen) en zijn misschien minder snel geneigd om hiervoor hulp te zoeken via hun huisarts. Een andere onder epidemiologen bekend fenomeen kan het onderzoek ook getroffen hebben: het blijkt altijd erg lastig om het aantal patiënten te includeren dat je op basis van epidemiologische voorspellingen zou verwachten. Dit staat ook wel bekend als 'de wet van Lasagna', een Amerikaanse onderzoeker die dit probleem beschreef.217 Een van de factoren die een rol blijkt te spelen bij lage inclusie is een gevoel van weerstand bij patiënten tegen het onderwerp of de interventie.217 Dit zou natuurlijk voor depressie kunnen gelden gezien het stigma dat erop rust en het 'depressie label' dat niet door iedereen wordt onderschreven.  Overigens onderstreept dit het belang van de mogelijkheid van aanmelden die de huisartsen ook tot hun beschikking hadden: deze patiënten zochten zelf actief hulp voor hun depressie. De uitkomsten voor deze groep (meegenomen in hoofdstuk 3 in de totale collaborative care groep) zijn mogelijk representatiever voor de alledaagse praktijk. Screenen voor depressie maakt daar immers doorgaans geen onderdeel van uit. Wel is er dan nog een methodologisch probleem. Bij cluster randomisatie (het willekeurig indelen van de huisartsenpraktijken in een controlegroep en een interventiegroep i.p.v. het willekeurig indelen van de patiënten in twee groepen) is het namelijk ingewikkeld om patiënten op een andere manier dan via screening te werven. Het belangrijkste probleem is dat het heel moeilijk is om huisartsen blind te houden voor de groep waar ze in zitten (zeker bij een complexe interventie als collaborative care waar ze ook voor getraind moeten worden). Huisartsen in de collaborative care groep zullen naar verwachting enthousiast zijn over de nieuwe methode die ze in de aanbieding hebben. Dit in tegenstelling tot huisartsen in de gebruikelijke zorggroep (de controlegroep). Daarbij komt dat de huisartsen in de collaborative care groep weten dat alle patiënten die zij aanmelden de nieuwe interventie zullen krijgen, terwijl de huisartsen in de gebruikelijke zorggroep 'niets nieuws in de aanbieding hebben'. Dit kan voor onevenwichtigheden zorgen in het onderzoek en dat kan leiden tot wat epidemiologen 'selectie bias' noemen: groepen die niet goed vergelijkbaar zijn waardoor je het echte effect van de interventie niet meer kunt bepalen. Bij de door de huisarts aangemelde patiënten in de totale collaborative care groep is dan ook mogelijk enige vertekening van de resultaten opgetreden (selectie bias). Hiervoor kan echter met zgn. propensity scores redelijk goed worden gecorrigeerd (zie de methodesectie van hoofdstuk 3 voor uitleg hierover). Om dit probleem volledig te ondervangen zou een nieuw onderzoek met patiëntrandomisatie moeten worden uitgevoerd (waarbij huisartsen alle patiënten aanmelden). De vraag is echter of dit echt nog nodig is. Deze vorm van randomiseren kent ook nadelen **** . Bovendien werd bij de IMPACT-studie al gebruik gemaakt van patiëntrandomisatie en de resultaten waren hier ook duidelijk in het voordeel van collaborative care.59 De Engelse collaborative care studie van Richards en collega's kende tevens een elegante oplossing: een cluster gerandomiseerde controlegroep en patiënt gerandomiseerde controle en collaborative care groepen.22 Naast de hierboven genoemde punten die wellicht nog nader onderzoek in de toekomst vergen, is ten aanzien van de implementatie natuurlijk ook van belang dat collaborative care ook vergoed wordt. In sommige gevallen zal de huisartsenpraktijk er zelfs iemand

voor aan moeten nemen (de caremanager). Hiervoor zijn vooral de resultaten met betrekking tot de kosteneffectiviteit van belang. In de jaren '90 van de vorige eeuw werd aan de hand van de bekende Trechter van Dunning23 beschreven aan welke voorwaarden een interventie zou moeten voldoen om te worden geïmplementeerd in de gezondheidszorg: I. is deze zorg noodzakelijk? II. Is het ook effectief (effectiever dan de gebruikelijke zorg)? III. Kan dat effect worden bereikt met een aanvaardbare hoeveelheid inzet van middelen en personeel? Dat het ondersteunen van de huisarts bij het geven van goede zorg voor depressie noodzakelijk is, is hopelijk duidelijk geworden in het eerste deel van deze samenvatting (denk bijvoorbeeld aan de prevalentie en de complexiteit van de problemen die in het korte tijdsbestek van een consult aan bod zouden moeten komen). Dat collaborative care effectiever is dan gebruikelijke zorg kwam naar voren uit hoofdstuk 3 van dit proefschrift. Hoe zit het dan met de kosten (als equivalent voor de inzet van middelen en personeel)? Hiertoe werd in hoofdstuk 4 een maat geïntroduceerd die vaak wordt gehanteerd in gezondheidszorgeconomie: kosten per QALY. QALY staat voor 'Quality Adjusted Lifeyear' †††† , oftewel hoeveel winnen patiënten die zijn behandeld volgens de principes van collaborative care aan gezonde levenstijd en wat kost dat dan? Uit de kostenanalyse komt naar voren dat de gescreende collaborative care patiënten gemiddeld genomen 0,01 QALY extra winnen ten opzicht van de patiënten in de gebruikelijke zorg groep (als ook de door de huisarts aangemelde collaborative care patiënten in de analyse worden meegenomen gaat het om 0,04 QALY). Dat lijkt natuurlijk niet veel, maar dit blijkt wel tegen aanvaardbare kosten te gebeuren (en daarbij geldt ook: patiënten in de collaborative care groep winnen niet alleen 0,01 QALY, maar natuurlijk ook PHQ9-punten tegen deze kosten). Sterker nog collaborative care blijkt zelfs goedkoper dan de gebruikelijke zorg (€ 1.540 zijn de gemiddelde kosten voor de gescreende collaborative care groep tegen €2.232 voor de gescreende gebruikelijke zorggroep). Als ook de indirecte kosten (bijv. voor productieverlies door ziekte) in ogenschouw worden genomen, wordt collaborative care ook 'dominant', wat wil zeggen dat de interventie duidelijk goedkoper en effectiever is dan de gebruikelijke zorg (zie ook figuur 2 in hoofdstuk 4). Vanuit een maatschappelijk perspectief is het dus verstandig om collaborative care in te voeren. Wel lijkt er dus nog veel winst mogelijk in de zin van QALYs (1 QALY is immers een gezond extra levensjaar, terwijl de opbrengst nu 0,01-0,04 QALY is). Hiernaar kan in de toekomst zeker nog gekeken worden. Zorgverzekeraars hebben gezien de lagere direct medische kosten echter genoeg reden om de implementatie van collaborative care aan te moedigen. Omdat de winst (met name in de gescreende collaborative care groep) voor een deel op het gebied van indirecte kosten ligt, zouden mogelijk ook andere partijen kunnen bijdragen. Werkgevers hebben bijvoorbeeld baat bij het lagere ziekteverzuim dat collaborative care met zich mee lijkt te brengen.

In onze studie werd hiervoor de EQ5D gebruikt, zoals beschreven in hoofdstuk 2 en hoofdstuk 4. Wie wil weten uit welke vragen de EQ5D is opgebouwd, kan een voorbeeld in het Engels downloaden via www.euroqol.org. Het voordeel van de EQ5D is dat het instrument niet 'ziekte specifiek' is. Aan een steekproef uit de bevolking is gevraagd hoe erg zij een bepaald ziekteprofiel vonden, los van welke ziekte dit veroorzaakte. Hoe erg vindt de gemiddelde Nederlander het bijvoorbeeld dat een ziekte problemen met lopen veroorzaakt? Je kunt je voorstellen dat juist voor psychische problemen waar vaak een stigma op rust, zo een waardevrij instrument heel nuttig kan zijn.

Overigens heeft de identificatie van patiënten door de huisarts wellicht een gunstige uitwerking op de kosteneffectiviteit: deze neemt namelijk nog duidelijk toe als de door de huisarts aangemelde patiënten in de analyse worden meegenomen. Kennelijk waren de huisartsen in staat om een groep te identificeren die relatief veel baat had bij de interventie. Hiernaar is nog wel meer onderzoek nodig, bijv. in een patiënt gerandomiseerd onderzoek waarin de huisarts alle patiënten aanmeldt. Bij de algemene conclusie (hieronder) komt dit punt ook nog eens aan bod. Ten aanzien van de implementatie kan al wel worden gezegd dat het van belang lijkt om het collaborative care model te integreren in de huisartsenzorg voor andere chronische lichamelijke aandoeningen (denk daarbij bijv. aan COPD, Diabetes en de ziekte van Parkinson die vaak samen voorkomen met depressie). 'Diseasemanagement modellen' waartoe ook collaborative care gerekend kan worden zouden uiteindelijk niet beperkt moeten worden tot één aandoening. Het patiënt trackingsysteem dat voor het CC:DIPonderzoek werd ontwikkeld zou vanuit deze optiek geïntegreerd moeten worden met het EPD (en niet als aparte module los van het EPD) naast andere modules voor andere chronische aandoeningen. Hierbij kan tevens gedacht worden aan een elektronisch dossier dat deels ook zelf door de patiënt beheerd wordt. Rechtstreekse toegang tot hun dossier faciliteert patiënten in het krijgen van geïntegreerde zorg (wat goed aansluit bij de collaborative care gedachte), en als patienten op hun beurt zorgverleners en eventuele anderen toegang geven tot hun dossier ontstaat een geïntegreerde omgeving. Wettelijk is de patient eigenaar van diens dossier zodat geen problemen met privacywetgeving te verwachten zijn. Innovatieve huisartsengroepen, transmurale GGZ instellingen of ICT ontwikkelaars kunnen hiermee de klant van de toekomst beter gaan bedienen (denk hierbij ook aan e-health). Dit zou een interessante ontwikkeling kunnen zijn, waarnaar nog wel enig onderzoek nodig lijkt.

2. Depressie en lichamelijke klachten (hoofdstuk 5 en 6) en het belang hiervan voor collaborative care (hoofdstuk 7): Zoals aan het begin van deze samenvatting al werd genoemd komen lichamelijke klachten en depressie erg vaak samen voor. Het is zelfs zo dat een ruime meerderheid van de patiënten bij het eerste consult voor wat later een depressie blijkt te zijn in eerste instantie alleen lichamelijke klachten rapporteert.25 Het lijkt daarom voor de hand liggend om uit te zoeken of lichamelijke klachten ook het beloop van een depressie beïnvloeden (Het beloop is een epidemiologische term voor hoe een ziekte verloopt. Gaat iemand bijvoorbeeld alleen maar achteruit of gaat het juist beter met de tijd?). In hoofdstuk 5 van dit proefschrift wordt de wetenschappelijke literatuur over dit onderwerp onder de loep genomen. Omdat we benieuwd waren naar de invloed van lichamelijke klachten op de effectiviteit van collaborative care voor depressie, hebben we voor het artikel waar hoofdstuk 7 op gebaseerd is een analyse gedaan op de CC:DIP-data waarin lichamelijke klachten op baseline (bij het begin van de studie, de 0-meting) werden meegenomen. Voor hoofdstuk 6 deden we een secundaire data-analyse naar de invloed van lichamelijke klachten op de uitkomsten van een ander gerandomiseerd gecontroleerd onderzoek dat over de effectiviteit van Problem Solving Treatment (PST)187 handelde. PST is de kortdurende gespreksbehandeling die ook tijdens CC:DIP door de care managers werd toegepast. Door naar de uitkomsten van een studie die over PST op zich ging te kijken, kunnen we ook iets zeggen over de invloed van lichamelijke klachten op een van de onderdelen van collaborative care. Dat is een nuttige aanvulling, want in de CC:DIP-studie kunnen we alleen naar de effectiviteit van het hele pakket kijken en niet naar de elementen op zich. Op basis van de literatuurstudie in hoofdstuk 5 kwamen we tot een aantal inzichten: 1. Er zijn relatief weinig studies naar het beloop van depressie (of naar de uitkomsten van interventies) waarin lichamelijke klachten bij aanvang worden meegenomen. We vonden er 9. Dat is niet veel als je je bedenkt hoe vaak depressie en lichamelijke klachten samen voorkomen. 2. Omdat de studies nogal verschillende uitkomstmaten hanteren, is het niet verstandig om die uitkomsten samen te namen (te poolen) om 1 overkoepelende uitkomst te bepalen. 3. Ondanks dat we de gegevens niet statistisch kunnen poolen, komt uit een beschrijvende analyse naar voren dat er sterke aanwijzingen zijn dat bijkomende lichamelijke klachten het beloop van een depressie negatief beïnvloeden. 4. Het lijkt daarom van belang om bestaande en nieuwe behandeling voor depressie tegen het licht te houden omdat zij minder goed zouden kunnen werken als patiënte ook lichamelijke klachten hebben. Dit zou in ieder geval kunnen gelden voor behandeling met antidepressiva. Daarvoor zijn in het literatuuronderzoek in hoofdstuk 5 al aanwijzingen gevonden. Hoe zit dit dan bij collaborative care? Een hypothese zou kunnen zijn dat de effectiviteit van collaborative care verhoudingsgewijs minder beïnvloed zal worden door bijkomende lichamelijke klachten. Farmacotherapie is er immers maar een onderdeel van. Bovendien bleek in een eerdere studie dat de interventie effectief was voor lichamelijk onverklaarde klachten.14

In hoofdstuk 7 werd aangetoond dat collaborative care voor depressie inderdaad effectiever is dan gebruikelijke zorg voor patiënten met bijkomende lichamelijke klachten. Er hoort echter wel een grote 'maar' bij: als je binnen de collaborative care groep de patiënten met bijkomende lichamelijke klachten vergelijkt met hen die deze klachten niet (of in minder ernstige mate) hebben, dan blijken de patiënten met lichamelijke klachten een veel lagere kans op de response te hebben. Eenvoudiger gezegd: een huisarts kan een patiënt die lijdt aan een depressie nog steeds beter collaborative care aanbieden als deze ook lichamelijke klachten heeft, maar de kans dat de interventie ook het beoogde effect (response) zal hebben, is wel veel minder. De odds ratio daarbij ligt tussen de 6 en de 7, afhankelijk van het moment waarop werd gekeken. Odds ratio zegt iets over hoe groepen zich tot elkaar verhouden. In dit geval zegt het getal 6 dat de verhouding tussen het aantal patiënten met lichamelijke klachten met response en patiënten zonder lichamelijke klachten met response ongeveer 1:6 is, in het nadeel van de groep met lichamelijke klachten. In het algemeen kan worden gesteld dat collaborative care is niet de oplossing is voor het probleem uit hoofdstuk 5, maar er is ook geen reden om patiënten met een depressie en ernstige lichamelijke klachten de interventie te onthouden. Net als de hiervoor genoemde behandelvormen (m.n. farmacotherapie, collaborative care en gebruikelijke huisartsenzorg) wordt ook PST, de kortdurende gespreksbehandeling die ook onderdeel uitmaakt van het collaborative care pakket zoals toegepast in CC:DIP, negatief beïnvloed door lichamelijke klachten. Dat blijkt uit de analyses die worden beschreven in hoofdstuk 6. Daarom kan worden overwogen om PST als onderdeel van het collaborative care pakket in de toekomst te vervangen door een andere vorm van behandeling voor patiënten die lijden aan bijkomende lichamelijke klachten. Hierbij kan bijvoorbeeld worden gedacht aan psychosomatische fysiotherapie. Fysiotherapeuten met deze specialisatie zijn specifiek opgeleid om lichamelijke klachten in een psychosociale context te behandelen.203 Een collaborative care model waarin dit concept wordt toegepast (PST als er geen ernstige lichamelijke klachten zijn en psychosomatische fysiotherapie als dit wel zo) lijkt zeer veel belovend. Een gerandomiseerde gecontroleerde studie met die groepen –gebruikelijke zorg, collaborative care met PST en collaborative care met PST of psychosomatische fysiotherapie- zou een erg interessante vervolgstap op dit proefschrift kunnen zijn. Een nuttig instrument in dit verband zou de Lichamelijke Klachten Vragenlijst (LKV, in hoofdstuk 7 afgekort als PSQ, Physical Symptoms Questionnaire) kunnen zijn die in hoofdstuk 7 werd gebruikt. Deze vragenlijst die is ontwikkeld door Van Hemert en collega's.33 Patiënten kunnen voor 51 lichamelijke klachten (op alle tractussen) aangeven hoe vaak zij hier last van hebben: niet, soms, geregeld, of vaak. Als iemand 'geregeld' of 'vaak invult' wordt een symptoom geteld. In de 'normale' bevolking geldt een score van 5 voor mannen en 7 voor vrouwen als hoog. Bij patiënten die lijden aan angst of depressie geldt 15 voor mannen en 17 voor vrouwen als hoog. Dit werd als afkapwaarde genomen in hoofdstuk 7 en zoals hierboven al beschreven bleken patiënten die hoog scoorden op lichamelijke klachten minder baat te hebben bij behandeling (zowel met gebruikelijke zorg als met collaborative care) voor hun depressie. Overigens lijkt het daarbij niet uit te maken of de klachten verklaard zijn (bijv. door een chronische lichamelijke ziekte als diabetes of COPD) of dat er geen bekende oorzaak voor is. In dat laatste geval wordt gesproken over Somatisch Onvoldoende verklaarde Lichamelijke Klachten (SOLK). Om iets te kunnen zeggen over het belang van een verklaring voor de klachten of niet, hebben we in het statistische regressiemodel dat we gebruikten te toetsen de variabele 'aantal lichamelijke ziekten' toegevoegd. Patiënten

hadden namelijk bij de 0-meting (voorafgaand aan de start van het onderzoek) aangegeven welke lichamelijke ziekten zij hadden. Het toevoegen van deze variabele aan het model maakte niet dat onze uitkomsten veranderden: de LKV-score bleef bepalend. Het lijkt daarmee minder van belang voor het beloop van een depressie (of voor het behandelresultaat van een interventie gericht op het verminderen van depressieve klachten) of lichamelijke klachten samenhangen met een chronische lichamelijke ziekte (waarvoor duidelijke onderliggende somatische pathologie gevonden is). Dat iemand lichamelijke klachten ervaart (ongeacht de oorzaak) lijkt van belang. Naar dit onderscheid was nog weinig onderzoek gedaan133 en het lijkt een nuttige toevoeging aan de kennis over de samenhang tussen stemmingsklachten en lichamelijke klachten. De LKV-score is van belang ongeacht de oorzaak van de lichamelijke klachten en het kan zeker nuttig zijn om deze af te nemen voorafgaand aan een behandeling voor depressie. Zeker als een patiënt hoog scoort (voor depressieve patiënten 15 voor mannen en 17 voor vrouwen) is aanvullende aandacht voor lichamelijke klachten geïndiceerd.

3. Conclusie: De belangrijkste conclusie van het CC:DIP-onderzoek is dat collaborative care ook in Nederland (kosten)effectiever lijkt te zijn dan gebruikelijke huisartsenzorg voor depressie. Een huisarts moet naar verwachting 2 tot 3 patiënten deze interventie aanbieden om voor 1 extra patiënt een goed behandelresultaat (in dit geval een reductie van 50% op de PHQ9, een korte vragenlijst voor de ernst van depressie) te bereiken. De meerkosten hiervoor lijken ook binnen maatschappelijk aanvaardbare marges te liggen, wat implementatie een stap dichterbij brengt. Een andere interessante bevinding tijdens het onderzoek, was dat het zeer moeilijk bleek om patiënten middels screening te werven. Als dit zich laat generaliseren naar de algemene praktijk (dus als patiënten ook in het dagelijks leven niet geneigd zijn een behandeling voor depressie aan te gaan op basis van screening), dan lijkt het niet zinvol om breed in te zetten op screening als toegeleiding naar deze veelbelovende interventie in de eerste lijn. Screening zou zich dan kunnen richten op hoog risico groepen (bijv. patiënten met diabetes, patiënten met veel lichamelijke klachten, of patiënten die in het verleden al eens depressief zijn geweest) en collaborative care zou vooral kunnen worden ingezet voor patiënten die langs deze meer toegespitste weg geïdentificeerd worden of voor patiënten die zich bij de huisarts melden met somberheidsklachten (of klachten die kunnen wijzen op depressie zoals slapeloosheid of lichamelijke klachten). De resultaten voor de patiënten in de door de huisarts aangemelde collaborative care groep doen vermoeden dat huisartsen prima in staat zijn om een groep patiënten te identificeren die relatief veel baat heeft bij de interventie. Mogelijk zou dit nog iets kunnen verbeteren als vragenlijsten als de PHQ9 standaard worden afgenomen bij een vermoeden van depressie. Van de 100 patiënten die de huisartsen aanmeldden (zie de flowchart in hoofdstuk 3) hadden er 84 een score op de PHQ9 van 10 of hoger. Voor patiënten die lager dan 10 scoren is het collaborative care pakket wellicht wel erg intensief als eerste stap. Dit sluit ook aan bij de nieuwe richtlijn voor de huisartsen218 waarin wordt aanbevolen om bij patiënten met milde klachten niet meteen een antidepressivum of psychotherapie in te zetten, maar te starten met dagstructurering, activiteitenplanning en kortdurende psychologische behandeling (bijv. begeleide zelfhulp). Collaborative care zou dan eerder kunnen worden toegepast in het kader van 'disease management' als ook echt een depressieve stoornis is vastgesteld.

Uit hoofdstuk 5, 6 en 7 komt naar voren dat extra aandacht nodig is voor een groep patiënten die naast hun depressie ook veel lichamelijke klachten rapporteren (al dan niet medisch verklaard). Zij profiteren in het algemeen minder van behandeling voor depressie. Het collaborative care model hoeft niet persé te worden aangepast voor deze groep (ook voor patiënten met lichamelijke klachten is collaborative care effectiever dan gebruikelijke zorg), maar het is toch iets om rekening mee te houden. Het lijkt erop dat bij alle interventies (of het nu antidepressieve medicatie, Problem Solving Treatment, gebruikelijke zorg of collaborative care is) voor depressie in de eerste lijn aanvullende aandacht nodig is voor bijkomende lichamelijke klachten. De Lichamelijke Klachtenvragenlijst (LKV) kan hierbij een nuttig hulpmiddel zijn. Voor mannen met een score van 15 en vrouwen met een score van 17 op de LKV moet behandeling voor depressie wellicht worden aangepast. Tot slot is het nog goed om op te merken dat collaborative care ook voor angststoornissen effectief is219 en dat het de moeite waard zou zijn om te onderzoeken of een gecombineerd model voor angst, depressie en lichamelijke klachten kan worden opgezet.

Korte inleiding

In het onderstaande document is de collaborative care behandeling zo exact mogelijk uitgewerkt. Het dient als back-up voor het trackingsysteem. Van week tot week is in dit document uitgewerkt hoe de behandeling in elkaar steekt. In paragraaf 8 vindt u een checklist waarin u snel kunt zien waar de interventie van week tot week uit bestaat. Als u vragen heeft, neem dan gerust contact op met de onderzoekers of de consulent psychiater. In de bijlagen bij dit document vindt u alle relevante formulieren voor dit onderzoek. Het advies algoritme voor antidepressiva staat in bijlage 6.

§1: (Voorafgaand aan start interventie): Afspraak maken met patiënt

1. De onderzoeker van het Trimbos-instituut geeft na afname van het MINI-interview (diagnostisch interview waarmee de depressie wordt vastgesteld en exclusiecriteria worden uitgevraagd) aan de assistente door dat er een afspraak van een uur gemaakt moet worden met de patiënt bij de care manager en de huisarts (de huisarts is laatste 10 minuten van het eerste gesprek aanwezig). 2. Het secretariaat geeft, ter bevestiging, aan de onderzoeker per mail door wanneer de eerste afspraak met de patiënt ingepland staat.

§2: 1e afspraak ( N.B. dit is week 0 in de checklist!):

1. De care manager legt uit dat de patiënt is doorverwezen, omdat bij het diagnostisch interview bleek dat hij/zij last had van klachten die op een depressie kunnen wijzen. Vervolgens wordt er psycho-educatie gegeven over depressie. Bij de uitleg over de klachten die nodig zijn om van een depressie te mogen spreken, wordt de PHQ-9 langsgelopen en wordt de totaalscore bepaald. (De patiënt kan de PHQ-9 eventueel al in de wachtkamer invullen). Omdat de PHQ-9 niet naar pijnklachten vraagt, is het van belang om hier ook naar te vragen. De care manager legt uit dat de symptomen die de patiënt heeft, wijzen op een depressie (leg zo nodig uit dat een depressie ook lichamelijk kan aanvoelen, bijvoorbeeld als de patiënt pijnklachten heeft). Vervolgens wordt uitgelegd dat een depressie kan worden veroorzaakt of erger worden gemaakt door alledaagse problemen. Bespreek aan het einde van de psycho-educatie de behandelmogelijkheden bij een depressie. N.B. Een hulpmiddel bij deze psycho-educatie kan de checklist zijn uit de Handleiding PST (zie bijlage 1 van de handleiding PST, met name punt 1 t/m. 3 zijn hier van belang). 2. De care manager stelt een risicoprofiel op (zie bijlage 4). 3. De care manager informeert de patiënt over de duur, het doel en de inhoud van de interventies. Er wordt naar gestreefd om de behandeling binnen 18 weken af te ronden. De eerste 6 sessies zijn wekelijks. Daarna kunnen de sessies ook om de week plaatsvinden. De eerste PST-sessie duurt een uur, de daaropvolgende vijf sessies duren een half uur. De behandeling zal bestaan uit:  het volgen van de klachten met de PHQ-9  PST  het zelfhulpwerkboek  mogelijk antidepressiva

mogelijk doorverwijzen (als de care manager na overleg met de consulent psychiater tot de conclusie komt dat een patiënt in de eerste lijn niet adequaat behandeld kan worden).

Zie paragraaf 2.1 voor een extra toelichting bij de hierboven besproken punten.

4. De huisarts neemt deel aan de laatste tien minuten van het gesprek en stelt samen met de care manager en de patiënt een behandelplan op met behulp van het voorgeschreven format (zie bijlage 2). Indien nodig zal de huisarts antidepressieve medicatie voorschrijven (zie bijlage 6). N.B. Het is in ieder geval van belang dat de huisarts kennis neemt van het behandelplan. 5. De care manager geeft de patiënt het volgende mee:  het zelfhulpwerkboek (hier zit ook een folder in voor familieleden)  een kopie van het behandelplan  de DVD over pijn en depressieve klachten (dit alleen als de patiënt ook pijnklachten heeft).  een snelhechter waarin de patiënt de ontvangen documenten kan bewaren. 6. Vraag de patiënt om voorafgaand aan de volgende afspraak de inleiding en hoofdstuk 1 van het zelfhulpwerkboek door te nemen. 7. De care manager legt een dossier aan met daarin het behandelplan, het risicoprofiel en de PHQ9.

§2.1: Uitleg over de verschillende onderdelen van het collaborative care pakket:

Tijdens het eerste gesprek wordt de patiënt geïnformeerd over de volgende onderdelen:

De PHQ-9: leg uit dat de klachten en de voortgang van de behandeling om de twee weken, met behulp van de score op de PHQ-9, worden geëvalueerd (zie voor de PHQ9 bijlage 3). Leg ook uit welke stappen mogelijk genomen kunnen worden als er onvoldoende voortgang in de behandeling is. Mogelijke stappen zijn: het aanbieden van zes extra sessies PST, het aanbieden van antidepressieve medicatie, het aanpassen van de dosis van antidepressieve medicatie, of het switchen naar een ander antidepressivum (de huisarts beslist hierover op basis van het algoritme voor antidepressieve medicatie en eventueel na overleg met de consulent psychiater).

Het zelfhulpwerkboek: de patiënt werkt elke week zelfstandig een hoofdstuk door. Elke sessie zal de care manager vragen of het gelukt is om het hoofdstuk te lezen en de daarbij behorende opdrachten te maken. Enkel bij problemen of onduidelijkheden wordt een hoofdstuk uit het zelfhulpwerkboek uitgebreider

  besproken. (Het probleem kan eventueel nader worden uitgewerkt als probleemstelling tijdens de aansluitende PST-sessie). Als de patiënt problemen heeft op het gebied van assertiviteit, een gezonde leefstijl of slapen of als de patiënt een chronische ziekte heeft, kan de care manager wijzen op de bijbehorende facultatieve hoofdstukken van het zelfhulpwerkboek. N.B. zie voor verdere informatie de handleiding zelfhulpwerkboek.

Problem Solving Treatment (PST): leg uit dat het een korte gestructureerde training is. Benadruk dat het in deze training om het hier-en-nu gaat, en dat het doel is om de probleemoplossende vaardigheden van de patiënt te versterken. Tijdens de volgende afspraak zal uitgebreid worden besproken wat PST precies inhoudt.

Antidepressieve medicatie: leg uit dat de patiënt zelf de keuze heeft om antidepressiva wel of niet in het behandelplan op te nemen. Geef informatie over antidepressiva. Benadruk dat de patiënt ook later in de interventie kan kiezen voor antidepressiva. Leg wel uit dat uit onderzoek gebleken is dat bij ernstige en/of recidiverende depressies een combinatiebehandeling inclusief antidepressiva effectiever is dan een enkele interventie zoals psychotherapie. Bij patiënten met een lichte tot matige depressie is niet gebleken dat een combinatiebehandeling van praten én pillen effectiever is dan één van beiden. De huisarts schrijft antidepressieve medicatie voor wanneer deze in het behandelplan worden opgenomen.

§3: De interventie van week tot week.

2e afspraak (week 1):

1. De care manager vraagt de patiënt naar het hoofdstuk van het zelfhulpwerkboek dat hij/zij heeft doorgenomen in de afgelopen week. Enkel bij problemen wordt er kort op ingegaan (Als de patiënt op problemen stuit bij het doorwerken van het zelfhulpwerkboek, kunnen deze tijdens de PST-sessie behandeld worden). 2. Indien de patiënt antidepressieve medicatie heeft, bespreekt de care manager kort hoe dit gaat. Slikt de patiënt deze consequent? Ervaart de patiënt vervelende bijwerkingen? Benadruk bij patiënten met antidepressieve medicatie dat bijwerkingen in de regel direct optreden, terwijl het werkzame effect vaak pas na enkele weken optreedt. Bespreek zo nodig met de huisarts. Bij problemen (bijvoorbeeld als de patiënt gestopt is met de medicatie vanwege de bijwerkingen) neemt de huisarts contact op met de consulent psychiater (informeer de huisarts dat hij/zij dit zou moeten doen).

3. De care manager start met de eerste sessie PST, zoals beschreven staat in de handleiding PST (zie voor een nadere toelichting over deze eerste PST-sessie hoofdstuk 3 van de handleiding PST). De checklist in bijlage 1 van de Handleiding PST is hierbij een nuttig hulpmiddel. Als punten 1 t/m. 3 van deze bijlage uit de PST-handleiding vorige week aan bod zijn geweest hoeven ze hier niet te worden herhaald. 4. Geef de patiënt de PHQ-9 mee, met als instructie die net voorafgaand aan de volgende sessie in te vullen (het is ook mogelijk dat de patiënt de PHQ-9 van de assistente krijgt als hij/zij komt voor de volgende afspraak). 5. Geef aan dat van de patiënt verwacht wordt dat deze voor de volgende afspraak aan de slag gaat met wat er in het actieplan (PST) staat beschreven en het volgende hoofdstuk uit het zelfhulpwerkboek doorwerkt. 6. Bij problemen wordt de consulent psychiater geraadpleegd (zie hiervoor paragraaf 6 van dit draaiboek: contact met de onderzoekers en de psychiater).

3e afspraak (week 2):

1. Neem de PHQ9 door met de patiënt en noteer de PHQ-9 score in het dossier (zie §5 voor uitleg over de scores). 2. Overleg zo nodig met de patiënt of deze nog heeft nagedacht over de mogelijkheid om antidepressieve medicatie te gaan gebruiken. Plan een afspraak met de huisarts als de patiënt voor medicatie kiest en vul een gewijzigd behandelplan in. Geef een kopie van deze gewijzigde behandelovereenkomst mee aan de patiënt. 3. Indien de patiënt antidepressieve medicatie heeft, bespreekt de care manager kort hoe dit gaat. Bespreek zo nodig met de huisarts. Bij problemen (bijvoorbeeld als de patiënt gestopt is met de medicatie vanwege de bijwerkingen) neemt de huisarts contact op met de consulent psychiater. 4. De care manager vraagt de patiënt naar het hoofdstuk van het zelfhulpwerkboek dat hij/zij heeft doorgenomen in de afgelopen week. Enkel bij problemen wordt er kort op ingegaan. 5. Tweede sessie PST. 6. Eindig sessie met aangeven dat van de patiënt verwacht wordt dat deze voor de volgende afspraak aan de slag gaat met wat er in het actieplan staat beschreven en het volgende hoofdstuk uit het zelfhulpwerkboek doorwerkt.

4e afspraak (week 3):

1. De care manager vraagt de patiënt naar het hoofdstuk van het zelfhulpwerkboek dat hij/zij heeft doorgenomen in de afgelopen week. Enkel bij problemen wordt er kort op ingegaan. 2. Als de patiënt antidepressiva gebruikt, bespreek dan kort hoe dit gaat. 3. Derde sessie PST. 4. Geef de patiënt de PHQ-9 mee, met als instructie die net voorafgaand aan de volgende sessie in te vullen.

5. Eindig sessie met aangeven dat van de patiënt verwacht wordt dat deze voor de volgende afspraak aan de slag gaat met wat er in het actieplan staat beschreven en het volgende hoofdstuk uit het zelfhulpwerkboek doorwerkt.

5e afspraak (week 4):

1. Neem de PHQ9 door met de patiënt en noteer de PHQ-9 score in het dossier (zie voor uitleg over de scores). N.B. In principe is het dossier dus het §5 trackingsysteem. 2. Overleg zo nodig met de patiënt of deze nog heeft nagedacht over de mogelijkheid om antidepressieve medicatie te gaan gebruiken. Plan een afspraak met de huisarts als de patiënt voor medicatie kiest en vul een gewijzigd behandelplan in. Geef een kopie van de gewijzigde behandelovereenkomst mee aan de patiënt. 3. Indien de patiënt antidepressieve medicatie heeft, bespreekt de care manager kort hoe dit gaat. Bespreek zo nodig met de huisarts. Bij problemen (bijvoorbeeld als de patiënt gestopt is met de medicatie vanwege de bijwerkingen) neemt de huisarts contact op met de consulent psychiater. 4. De care manager vraagt de patiënt naar het hoofdstuk van het zelfhulpwerkboek dat hij/zij heeft doorgenomen in de afgelopen week. Enkel bij problemen wordt er kort op ingegaan. 5. Vierde sessie PST. 6. Eindig sessie met aangeven dat van de patiënt verwacht wordt dat deze voor de volgende afspraak aan de slag gaat met wat er in het actieplan staat beschreven en het volgende hoofdstuk uit het zelfhulpwerkboek doorwerkt.

6e afspraak (week 5)

1. De care manager vraagt de patiënt naar het hoofdstuk van het zelfhulpwerkboek dat hij/zij heeft doorgenomen in de afgelopen week. 2. Als de patiënt antidepressiva gebruikt, bespreek dan kort hoe dit gaat. 3. Vijfde sessie PST. 4. Aangezien in deze week in principe de voorlaatste PST-sessie plaatsvindt, is het een goed moment om met de patiënt te bespreken of aanvullende PST-sessies zinvol zijn (aankondigen dat dit volgende week besproken gaat worden, is hier nog voldoende). N.B. Uitbreiding van het aantal sessies is alleen onder bepaalde voorwaarden zinvol: zie hiervoor hoofdstuk 4 van de Handleiding PST. 5. Geef de PHQ-9 mee 6. Als de patiënt overduidelijk voldoende hersteld is, kan hij/zij met behulp van hoofdstuk 7 van het zelfhulpwerkboek een terugvalpreventieplan maken (evalueer wel aan de hand van de score op de PHQ-9 in week zes of de patiënt ook echt hersteld is alvorens de behandeling te beëindigen).

  7e afspraak (week 6)

1. Neem de PHQ-9 door met de patiënt. 2. In principe is vandaag de laatste PST-sessie (zie hoofdstuk 4 van de PSThandleiding). 3. Bespreek eventuele uitbreiding van het aantal PST-sessies, eventuele overstap naar antidepressieve medicatie of eventuele doorverwijzing. 4. Als de behandeling in deze week stopt, maak dan een afspraak voor de eerstvolgende keer dat de PHQ-9 wordt afgenomen (dat is in dit geval week 8), om het terugvalpreventieplan te bespreken. Geef aan de patiënt door dat het de bedoeling is dat hij voor die afspraak het hoofdstuk over terugvalpreventie doorneemt.

Week 7 tot week 18

1. De PHQ9 wordt afgenomen in de weken: 8,10,12,14,16 en 18 (als nog geen remissie is bereikt). De patiënt kan hiervoor naar de praktijk komen, maar het kan ook telefonisch als de patiënt de PHQ9 zelfstandig in kan vullen. Het is in beide gevallen van belang om de scores op te nemen in het dossier. Bij doorverwijzing hoeft de PHQ-9 niet meer te worden afgenomen. 2. Als de patiënt remissie heeft bereikt, d.w.z. een score lager dan 5 op de PHQ-9 (zie ook paragraaf 5), maak dan een afspraak voor de eerstvolgende keer dat de PHQ-9 wordt afgenomen om het terugvalpreventieplan te bespreken (als remissie is bereikt in week 8 is dat bijvoorbeeld week 10; als remissie is bereikt in week 10 is volgende afname van de PHQ-9 in week 12). Geef aan de patiënt door dat het de bedoeling is dat hij voor die afspraak het hoofdstuk over terugvalpreventie uit het zelfhulpwerkboek doorneemt en de opdrachten invult. Meer uitleg over het terugvalpreventieplan volgt in paragraaf 4. 3. De eventuele resterende PST-sessies vinden in week 7 tot en met 18 plaats. Bedenk hierbij dat de interventie in principe na 18 weken afgerond zou moeten zijn. Als er na week 18 nog enkele PST-sessies plaats zouden moeten vinden dan is dit mogelijk. Dit zou bijvoorbeeld het geval kunnen zijn als de patiënt een aantal weken met vakantie is geweest of ziek is geweest. 4. Zie voor suggesties over het beëindigen van de interventie § 4.

§4: Beëindiging interventie

1. De interventie duurt in principe maximaal 18 weken. Als de patiënt echter pas in week 12 is begonnen met medicatie, kan het voorkomen dat de interventie 24 weken duurt. De care manager bereidt de patiënt voor op het einde van de interventie door vier weken voor het einde te herhalen dat de maximale duur van de interventie 18 (of bij een late start met medicatie 24) weken is.

  2. De care manager bespreekt voor het beëindigen van de interventie het hoofdstuk ‘Winst behouden’ (= het terugvalpreventieplan) van het zelfhulpwerkboek (hoofdstuk 7). Bespreek met de patiënt signalen van een (naderende) depressie en actieplannen voor wanneer de patiënt dergelijke signalen bij zichzelf herkent. 3. De care manager blikt samen met de patiënt terug op de behandeling. 4. De care manager bespreekt met de patiënt de huidige score op de PHQ-9. Indien deze score een indicatie is voor een doorverwijzing naar andere zorg, bespreekt deze dit met de patiënt. Wanneer de patiënt hiermee akkoord gaat, wordt doorverwezen volgens het doorverwijsprotocol (zie paragraaf 7 van dit draaiboek). 5. De care manager koppelt de beëindiging van de behandeling terug naar de huisarts, de consulent psychiater en de onderzoekers. Hierbij is naast de PHQ-9 score de ‘checklist per patiënt’ van belang. Deze vindt u in bijlage 5 van dit draaiboek. 6. De care manager neemt in de 12 maanden die volgen op het eind van de interventie elke maand telefonisch contact op met de patiënt om een vinger aan de pols te houden.

§5: De behandeling volgen en bijsturen aan de hand van de PHQ9:

De totaalscores van de PHQ-9 worden als volgt geïnterpreteerd: 0-4:

geen depressie (of als er een depressie was is deze nu in remissie).

5-10: milde depressieve klachten 11-15: depressieve klachten van matige ernst 16-20: matig tot ernstige depressieve klachten 21-27: ernstige depressieve klachten

Bij de tweewekelijkse afname van de PHQ9 (en zeker in de 6e, 12e en 18e week) zijn de volgende ijkpunten van belang:

1. Remissie: Hiervan is sprake als de PHQ9 is gedaald tot onder de 5 punten (score van 0 tot 4). Er zijn dan geen aanwijzingen meer voor de aanwezigheid van een depressie. Het is van belang om hierbij te kijken of dit ook klopt met het klinische beeld van de huisarts en de care manager. Als er sprake is van remissie kan de PST worden afgebouwd en kan m.b.v. hoofdstuk 7 van het zelfhulpwerkboek een terugvalpreventieplan worden gemaakt. Als de patiënt antidepressiva gebruikt worden afspraken gemaakt over het continueren van de medicatie. 2. Respons: Van respons is sprake als de PHQ9 met 5 punten of meer afneemt in week 6 (ten opzichte van de beginmeting). In dit geval kan de behandeling op dezelfde wijze worden voortgezet. Als de totaalscore op de PHQ9 nog steeds erg

  hoog is (bijvoorbeeld een score van 17, ondanks de 5 punten afname) is het raadzaam om de onderzoekers of de consulent psychiater om advies te vragen. N.B. Respons is alleen van belang in week 6. Week 12 is het volgende ijkpunt. In deze week dient sprake te zijn van remissie. Als in week 12 geen sprake is van remissie wordt de behandeling geïntensiveerd. 3. Geen respons: Als de PHQ9 niet afneemt of met minder dan 5 punten afneemt, wordt de behandeling geïntensiveerd. Dit kan door de patiënt te wijzen op de mogelijkheden voor antidepressiva en zo nodig een consult hiervoor bij de huisarts te boeken. Als patiënt al langer dan 6 weken antidepressiva gebruikt, wordt een afspraak met de huisarts gemaakt zodat deze een nieuwe stap kan zetten in het algoritme voor het voorschrijven van antidepressiva. N.B. Boek hiervoor een consult in bij de huisarts. Bij vragen over het voorschrijven van medicatie kan de huisarts contact opnemen met de consulent psychiater. Gebruik als medicatie wordt voorgeschreven het formulier ‘gewijzigde behandelovereenkomst’ (bijlage 2).

§6: Doorverwijsprotocol: Indien er geen verbetering optreedt op de PHQ9 wordt de patiënt, na maximaal 18 weken en na overleg tussen patiënt, care manager, huisarts en consulent psychiater, doorverwezen. NB In verband met lange wachttijden is het waarschijnlijk verstandig om al na 12 weken contact op te nemen met de GGZ, als de prognose er niet goed uitziet (geen respons, of een PHQ9 score hoger dan 5). Overleg in elk geval met de consulent psychiater. U kunt gebruik maken van de concept verwijsbrief (op pagina 10). Deze kunt u aanvullen met specifieke informatie omtrent de patiënt.

§6.1: Overige redenen verwijzing naar de 2de lijn: Hieronder staan een aantal situaties die los van de score op de PHQ-9 redenen kunnen zijn voor verwijzing naar de 2de lijn. Neem in deze situaties wel contact op met de consulent psychiater om te bepalen of verwijzing naar de 2de lijn nodig is. Als wordt besloten om de patiënt door te verwijzen, kan gebruik worden gemaakt van de concept doorverwijsbrief. 1. Bij het opstellen van het risicoprofiel (zie bijlage 4) wordt het algemene risico als ernstig ingeschat. 2. Tijdens de behandeling blijkt de patiënt een verhoogd suïciderisico te hebben. 3. Tijdens de behandeling blijkt de patiënt afhankelijk te zijn van alcohol of andere middelen, een bipolaire stoornis te hebben of psychotisch te zijn. Deze situatie zal zich waarschijnlijk niet vaak voordoen, want tijdens het diagnostisch interview dat afgenomen wordt voorafgaand aan de start van de behandeling, wordt ook gevraagd naar de aanwezigheid van deze stoornissen en als er één aanwezig blijkt te zijn, wordt de patiënt uitgesloten van het onderzoek. Overleg in dit geval met de consulent psychiater. 4. Tijdens de behandeling lijkt de patiënt een persoonlijkheidsstoornis te hebben die de behandeling van de depressie in de eerste lijn ernstig belemmert. 5. Tijdens de behandeling blijkt dat de patiënt waarschijnlijk meer baat heeft bij een andere therapie dan PST (bijvoorbeeld relatietherapie, hulp bij rouwverwerking, cognitieve gedragstherapie).

Bijlage 6: Advies algoritme antidepressiva Antidepressiva in het CC:DIP-onderzoek Dr. C.M. van der Feltz-Cornelis (psychiater-epidemioloog) Dr. H.W.J. van Marwijk (huisarts) Drs. Beekman (psychiater) In het kader van het project CC:DIP is een algoritme ontwikkeld om stapsgewijs de behandeling van depressieve patiënten door de huisarts en caremanager te monitoren aan de hand van de PHQ-9, een korte screeningsvragenlijst voor depressieve klachten. In dat kader past ook een advies over hoe de antidepressieve medicatie voorgeschreven kan worden. Het advies voorziet in twee stappen van elk 6 weken. Uitgangspunt is dat de huisarts met de patiënt een keuze maakt voor een antidepressivum binnen het kader van de NHG Standaard Depressie. Goede monitoring, evaluatie en dosering zijn daarbij belangrijk; vandaar dat wij voor een aantal antidepressiva een voorstel voor dosering doen, te gebruiken in de eerste en tweede fase. De huisarts en patiënt kunnen dan kiezen welke ze zullen nemen. We hebben ons daarbij laten leiden door een aantal overwegingen. De keuze gaat uit naar een middel: 1. 2. 3. 4.

dat in principe generiek wordt voorgeschreven, waar geen patent meer op is; dat vaak wordt voorgeschreven in de huisartspraktijk; dat redelijk makkelijk te starten en te stoppen is, zonder veel voorkomende problematiek daarbij; 5. dat goed verdragen wordt door de patiëntengroep die aan dit onderzoek meedoet, waarin ouderen met somatische co-morbiditeit een substantieel deel zullen vormen. Ook de volgende overwegingen speelden een rol: 6. Voor de eerste stap hebben we een serotonerg werkend middel gekozen dat met name bij ouderen en bij somatische cardiovasculaire comorbiditeit goed wordt verdragen. 7. Voor de tweede stap geven we als optie een breed werkend middel, dus zowel serotonerg als noradrenerg werkend; het alternatief is een tricyclisch antidepressivum dat door ouderen met somatische comorbiditeit redelijk goed verdragen wordt. Dit alles binnen het kader van de NHG standaard. We geven dan de volgende keuzeopties: STAP 1. Citalopram, 1e week 20 mg, Tweede week 40 mg daags

STAP 2 Indien na 6 weken de PHQ-9 onvoldoende is gedaald, dat wil zeggen minder dan 5 punten, op de eerste medicatie, dan wordt geswitcht naar een van de volgende middelen: Venlafaxine, 1e week 75 mg, tweede week 150 mg, 4e week evt. nog op te hogen tot 225 mg daags, indien de PHQ-9 geen verbetering geeft. Ofwel Nortriptyline, 1e week 50 mg 2e week 100 mg 3e week 150 mg daags De PHQ-9 wordt door de huisarts en caremanager gemonitord en de consulent psychiater is beschikbaar voor overleg elke 6 weken of tussentijds bij vragen met betrekking tot de te volgen stappen.

Dankwoord Op de eerste plaats wil ik alle huisartsen, caremanagers en patiënten die aan het CC:DIP-project hebben meegewerkt bedanken. Collaborative care gaat over samenwerking en als jullie die niet met mij waren aangegaan was dit project er niet geweest! Het project was er ook niet geweest zonder het Innovatiefonds Zorgverzekeraars (toen nog RVVZ) dat het Depressie-initiatief mogelijk maakte waar dit proefschrift een onderdeel van is. Dit proefschrift was er ook niet geweest als niet ooit mijn interesse voor wetenschap was gewekt. Goed, ik ben dan geen sterrenkundige geworden, maar de Jongerenwerkgroep voor Sterrenkunde (JWG), heeft zonder meer een grote rol gespeeld: het aantal proefschriften van mede zomerkampleiders begint inmiddels aardig wat ruimte in de boekenkast in te nemen. Ik hoop jullie allemaal nog vaak te zien, in de Stichtse, achter een telescoop of ergens anders! Van de JWG had ik weer nooit gehoord zonder een van meest bevlogen natuurkundedocenten die ik ooit heb ontmoet: Walter Wamsteker van het Norbertuscollege. Op deze bijzondere middelbare school in Roosendaal krijgen leerlingen veel ruimte en mogelijkheden om hun interesses te ontplooien, onder leiding van betrokken docenten. Zelfs het Engels van oud leerlingen wordt nog gecontroleerd door betrokken docenten als Harry Mulders, waarvoor mijn hartelijke dank! Een aantal van mijn beste vrienden heb ik leren kennen op het Norbertus. Pieter Baaijens, Pieter Westerbos, Guido en Roel, ik ben blij dat we na al die jaren nog steeds bevriend zijn. Wie weet komt het er nog eens van, maar ik ben in ieder geval heel blij dat je mij ook op deze laatste meters als paranimf terzijde staat. Het was nou ook weer niet goed voor mij geweest als ik me alleen maar met wetenschap was gaan bezighouden. Op de basisschool had ik al als bijnaam ‘de verstrooide professor’. Vandaar mijn wens om ook in de praktijk als psycholoog te gaan werken. Ik wil Altrecht psychosomatiek (de Eikeboom) en mijn huidige werkgever, de Polikliniek Psychosomatiek van GGZinGeest graag bedanken voor de kans die zij mij hebben gegeven om de stap van wetenschap naar praktijk te zetten. Of ik trouwens minder verstrooid ben geworden weet ik niet, maar na de gerichte feedback van mijn kamergenotes bij de Eikeboom geloof ik toch dat ik de laatste tijd wat minder tegen mijn computer praat. Naast aan de JWG en het Norbertus, heb ik ook veel goede vrienden overgehouden aan de Studentengroep Sociale wetenschappen (SGS). Ik kan helaas niet iedereen noemen, maar ik wil jullie allemaal bedanken voor jullie interesse de afgelopen 6,5 jaar en ik hoop dat jullie allemaal dit feest met mij mee willen vieren! Jaap en Hilde, ik vraag me af of ik veel beter ga klaverjassen als gepromoveerde, maar ik weet zeker dat ik nog evenveel plezier aan onze avondjes zal beleven. Maarten, ik ken niemand met wie ik zoveel kan discussiëren ondanks dat we het eigenlijk altijd eens zijn. Ik hoop dat we dat nog jaren zullen doen. Ruud en Andrea (en de familie Van Leeuwen), bedankt voor alle schrijfweekendjes en de gezelligheid daar omheen. Andrea, we hebben elkaar er vaak door gesleept bij alle proefschriftfrustraties. Ik ben blij dat we dat nog een keer kunnen doen, jij als paranimf bij mij en ik bij jou! Arie en Gonnie, bedankt dat ik altijd welkom ben aan de Van Marnixlaan en voor veel liefdevolle coaching. Dirk en Debby, dit boekje is niet echt ideaal voorleesmateriaal, maar ik ben blij dat het nu af is, want ik kom mijn nichtje Fenne natuurlijk graag zo vaak mogelijk bewonderen. Pa en ma, jullie hebben mijn nieuwsgierigheid steeds aangemoedigd en mij vrij gelaten in mijn keuzes. Dankzij jullie kan ik de wereld verkennen en weet ik mij altijd gesteund. Amor vincit omnia. Lieve Gemma, ook deze lange reis hebben we samen volbracht. Gelukkig hebben we dit boek nu uit, maar met jou raak ik nooit uitgepraat op deze reis van het leven!

Curriculum Vitae Klaas Huijbregts was born on February 22nd 1980 in Roosendaal. He received his secondary education at the Norbertuscollege. Subsequently he went to Australia and New Zealand for a year of working and travelling. After this year, despite being an expert in the field of fruit picking, he decided to start studying Psychology at Utrecht University (1999). Klaas specialised in Clinical and Health Care Psychology. His masters thesis was on the association between stress physiology, mood and physical symptoms. He graduated in 2004 after a clinical internship at the Sinai Centrum in Amersfoort where he worked for another year researching the possibilities of testimony therapy for children of parents who were traumatized during Word War II. In 2006, Klaas started his PhD research at the Netherlands Institute of Mental Health and Addiction (Trimbos Instituut) and the Department of General Practice at the VUmc in Amsterdam. In 2010, when all data was gathered and the process of writing the thesis was underway, he started working as a psychologist again at the department of Psychosomatics of Altrecht mental health care in Zeist and Den Dolder. Since September 2012, Klaas works at the department of Pschosomatics of GGZinGeest in Amsterdam. He is also studying to become a Certified Health Care Psychologist (GZ-psycholoog) and expects to graduate in September 2014. Klaas lives together with his partner Gemma in Utrecht. He is an astronomy enthusiast that likes to run and travel as much as possible.
