Vroegkinderlijke traumatisering Psychoanalyse For almost 45 years, the experience of Jewish children who were hidden during World War II was considered to be of little importance, particularly with respect to what had taken place in the concentration camps. Their very history was ignored in the many accounts of the Holocaust. It was only at the end of the 1980s that their experience began to be thought of as potentially traumatic. In this paper, the authors report on their psychoanalytical research project concerning the psychological outcomes of those experiences that had remained concealed for such an extraordinarily long latency period. The results are based on the analysis of 60 accounts and on psychoanalytically-oriented group work. The authors show that the trauma experienced by those hidden children was triggered by the retroactive effect of a deferred action Velde, W. op den War stress and late-life mortality in World War II male civilian resistance veterans / W. op den Velde [et al.] In: Psychological reports = ISSN 0033 2941: vol. 108 (2011), issue 2, page 437-448. The veterans' data were compared with a randomly selected population-based sample of same-aged males. In 2005, 70% of the war stress veterans had died, and only 35% of the comparison group. The baseline quality of life was significantly poorer in the war stress veterans than in the comparison group. Baseline variables explained 42% of the increased risk of mortality among war stress veterans. Smoking was the largest single contributor to mortality. Onderzoek Background: There is growing evidence of the impact of post-migration factors on the mental health of refugees. To date, few UK studies have been conducted. Aims: The study investigated the relationship between trauma, post-migration problems, social support and the mental health of refugees and asylum seekers. Methods: Refugees and asylum seekers (n = 47) were recruited mainly from clinical settings. Self-report measures of post-migration problems, mental health problems and social support were completed in an interview. Results: Bivariate associations were identified between increased symptoms and number of traumas, adaptation difficulties, loss of culture and support and confidant support. In multivariate analyses post-migration problems were significantly associated with post-traumatic stress disorder symptoms and emotional distress. There was no significant association of symptoms and number of traumas or social support. Conclusions: The results suggest that clinical services should provide holistic interventions within a phased approach when working with refugees and asylum seekers. At a policy level, the results suggest the need for asylum policies that reduce post-migration problems and provide support for refugees and asylum seekers. This article analyses how cultural-political narratives may frame individual traumatic experiences and channel them into collective causes among Tamil refugees in Norway. Data derives from participant observation within Tamil NGOs in Norway between September 2006 and February 2010. Cultural-political narratives and symbolism related to heroes and martyrs who literally sacrifice themselves for the Tamil cause often prompt guilt among exiled Tamils who live comfortable lives in the Norwegian welfare society. The way out of it entails a reconceptualization of what sacrifice means and the purposes it serves in exile. Heide, F. PTSD Asielzoekers Vluchtelingen Therapieën Stabilisatie EMDR Marteling Transculturele psychotherapie Effectiviteit Onderzoek Background: Traumatised asylum seekers and refugees are clinically considered a complex population. Discussion exists on whether with this population treatment guidelines for post-traumatic stress disorder (PTSD) should be followed and TraumaFocused Cognitive-Behavioural Therapy (TF-CBT) or Eye Movement Desensitisation and Reprocessing (EMDR) should be applied, or whether a phased model starting with stabilisation is preferable. Some clinicians fear that trauma-focused interventions may lead to unmanageable distress or may be ineffective. While cognitive-behavioural interventions have been found to be effective with traumatised refugees, no studies concerning the efficacy of EMDR with this population have been conducted as yet. Objective: In preparation for a randomised trial comparing EMDR and stabilisation with traumatised refugees, a pilot study with 20 participants was conducted. The objective was to examine feasibility of participation in a randomised trial for this complex population and to examine acceptability and preliminary efficacy of EMDR. Design: Participants were randomly allocated to 11 sessions of either EMDR or stabilisation. Symptoms of PTSD (SCID-I, HTQ), depression and anxiety (HSCL-25), and quality of life (WHOQOL-BREF) were assessed at pre- and post-treatment and 3-month follow-up. Results: Participation of traumatised refugees in the study was found feasible, although issues associated with complex traumatisation led to a high pre-treatment attrition and challenges in assessments. Acceptability of EMDR was found equal to that of stabilisation with a high drop-out for both conditions. No participants dropped out of the EMDR condition because of unmanageable distress. While improvement for EMDR participants was small, EMDR was found to be no less efficacious than stabilisation. Different symptom courses between the two conditions, with EMDR showing some improvement and stabilisation showing some deterioration between pre-treatment and post-treatment, justify the conduct of a full trial. Conclusion: With some adaptations in study design, inclusion of a greater sample is justifiable to determine which treatment is more suitable for this complex population. Traumatische ervaringen Psychotrauma PTSD Migranten Vluchtelingen Cultuur Hulpzoekgedrag Hulpverlening Behandeling Interventies Collectieve verwerking Gezinnen In this paper, the theme of the special issue on Culture and Trauma is introduced. In both empirically oriented articles as well as theoretical and contemplative contributions, the role of culture and migration context in the development of psychological problems following trauma, culture-specific or generic coping with the consequences, help-seeking behavior, and the need for culturally sensitive treatment is explored. First, the influence of the migration context is considered with regard to the possible sickening role of the asylum procedure, the burden for immigrant women who have undergone female genital mutilation and now live in a society that seriously condemns it, and the dilemma of Islamic migrants who experience homosexual feelings while living in a secularized society. Following are some contributions on cultural diversity in the expression of trauma symptoms, like somatization among refugees and the articulation of rage and embitterment among labor migrants. The impact of trauma in combination with migration on families and how children can be affected by the traumatization of their parents is the
12
topic of two contributions in which the role of the (extended) family is explored and a contextual approach to working with families with trauma-related symptoms (a mentalization-based multifamily therapy) is described. The factors that impede therapyseeking behaviour and drop-out are discussed in a study on Moroccan or Turkish girls who were raped. Finally, the applicability and efficacy of evidence-based interventions for treating ethnic minority patients with posttraumatic stress disorder is illustrated by a study on Eye Movement Desensitization and Reprocessing with refugees and an article on specific elements of intercultural trauma therapy that appear to be culturally sensitive. It shows how the kinship and solidarity found in female networks, in a matrilineal society, has helped them survive the conflict. In Batticaloa, female-headed households bear the main burden for caring for the traumatised, and sometimes injured, returning female, former soldiers. This is done in the absence of social welfare services or specific medical or psychosocial care. Disabled female ex-combatants find it especially difficult to build a future within the community. Although Sri Lanka's National Action Plan for the Re-Integration of Ex-Combatants does include disabled fighters, in reality, disabled female ex-combatants receive hardly any support. The author
13
concludes that money is spent on programmes that are not aimed at restoring trust between the Tamil population and the Sri Lankan state, but at reconciling ex-combatants with local communities. This is unnecessary, as communities already accept and help them, especially in the female-headed households. Households that have extra mouths to feed, because they provide care to returning female soldiers, should at least receive economic support. Asielzoekers Traumatische ervaringen PTSD Stress Risicofactoren Asielprocedure Politiek Behandeling The problems of asylum seekers are multiple and complex. Next to having experienced a lot of adversities in their country of origin, when they arrive in a host country they face a (usually long) asylum procedure and a multitude of post-migration living problems. Literature shows high prevalence rates of mental disorders and low quality of life. The length of the asylum procedure is an even more important risk factor for psychopathology than the adverse life events in the country of origin. Both qualitative and quantitative research has shown the relevant types of post-migration living problems, such as uncertainty about the future, the fear to be sent home, missing one’s family, and lack of work. In order to improve the mental health status of this very vulnerable group, the government should reconsider their policy regarding length of the asylum procedure, living conditions in the asylum seeking centres, work, and family reunification, and should facilitate support by voluntary agencies. Mental health workers should realize the importance of the asylum procedure and the (often related) post-migration living problems and consider to focus their treatment on (coping with) these problems instead of only focusing on traumas from the past. A resilience-oriented approach is recommended. Loncar, M. Self-assessment of well-being as an indicator of quality of life of former war prisoners - a Croatian study / M. Because of the specific war experience of this population it is important to better understand the
14
possible contribution of key socio-demographic variables, war traumatization and acute disturbances in mental health to their subjective assessment of their own well-being, which represents a psychological category and is based on a subjective assessment. The starting point is a theoretical precept according to which individual characteristics, together with war experience, can have repercussions on mental health, and eventually on the general well-being of an individual and their quality of life. The study comprised 184participants who had given their informed consent for participation and filled out complete questionnaires. The participants were a convenience sample of male persons who had survived war captivity in the Homeland War in the period from 1991 to 1995. The study was conducted as part of the physical examinations at the University Hospital "Fran Mihaljevi?" in Zagreb. The data was collected using several self-evaluation measuring instruments one of which served to collect socio-demographic data, two to collect data on the participants' mental health, one for the data on the participants' combat and war experiences and one to assess the participants' well-being. The data obtained suggest that only avoidance and arousal symptoms and psychosomatic difficulties are predictors of the well-being of persons who have experienced war captivity. In this paper, the mentalization-based multifamily therapy is described. It concerns a contextual approach to working with families with trauma-related symptoms and dynamics. Individual wellbeing as well as parenting capacities are being negatively impacted by the sequence of dramatic life experiences that migrant families often are confronted with, including the stay in asylum-seeker centers and legal procedures or getting settled in the Netherlands. The multifamily therapy is adopted from years of experience at the Marlborough Centre in London, working with families dealing with a multitude of problems with various cultural backgrounds. Principles of the methodology, examples of interventions, and experiences of working with this methodology are described. As it is directed at optimal autonomy of families and its members, incorporating mentalizationenhancing strategies, it is less focused on rules and ‘standard’ norms and values, therefore enabling culturally sensitive services. METHODS The psychiatric expert opinions on asylum seekers, furnished in an 8-year-period at Hannover Medical School, were analysed retrospectively for qualitative and quantitative characteristics. RESULTS 62 psychiatric expert opinions on asylum seekers were included in this study. The asylum seekers originated from 18 different countries, mainly from Turkey and former Yugoslavia. Most
16
expert opinions were given in secondary asylum procedures, i. e. after the initial asylum request had been rejected. The asylum seekers reported on traumatisation in 82.3 %. The most frequently reported forms of traumatisation were rape in female, and torture in male persons. According to ICD-10 or DSM-IV-R criteria posttraumatic stress disorder (PTSD) was the most frequent diagnosis (74.1 %) in this study. The second most common diagnoses were depressive disorders (ICD-10: F32.x in 33.9 % and ICD-10: F33.x in 25.9 %). Suicidal tendency was found in 56.5 % of the asylum seekers. CONCLUSIONS Cultural differences, language barriers, a heavy burden by psychological symptoms, and clinical severity are difficulties in the process of psychiatric assessment of refugees in legal asylum procedures. At Foundation House counselling and complementary therapies are practised collaboratively. The paper is based on two independent qualitative research projects: one project examined refugee women's experiences of complementary therapies and the second investigated counsellors' reasons for referral to complementary therapies. We formulate the areas of commonality in our findings under three 'modes of action': relationship, cultural familiarity and somatic presentations. Our combined findings extend current notions of holistic refugee health care to include culturally familiar health care practices in the form of complementary therapies. Depressie Angst Onderzoek Following resettlement in Western countries, unaccompanied refugee minors (URM) are at risk of developing posttraumatic stress disorder (PTSD). It is unclear to what extent PTSD in this group may become manifest at later stages following resettlement and which factors are associated with late onset. We examined data from URM collected 1 (T1) and 2 years (T2) following resettlement for differences between groups with no PTSD, PTSD at T1, and late-onset PTSD (at T2 only) using multinomial regression and path analysis. Of the children and adolescents (ages 12-18) completing both assessments (N=554), 223 (40%) met criteria for PTSD at T1, and 88 (16%) endorsed late-onset PTSD. Late-onset PTSD was associated with traumatic event exposure, older age, and low education. In the late-onset PTSD group, the predictive effects of traumatic event exposure on symptom severity at T2 were fully mediated by depression and anxiety symptoms at T1. These results suggest that late-onset PTSD is a clinically relevant problem among URM that may be heralded by early depression and anxiety symptoms. Method: Systematic searches of Medline, PsycInfo and Embase were performed in October 2010. MBSR, MBCT and Mindfulness Meditation were key words. Only randomized controlled trials (RCT) using the standard MBSR/MBCT programme with a minimum of 33 participants were included. Results: The search produced 72 articles, of which 21 were included. MBSR improved mental health in 11 studies compared to wait list control or treatment as usual (TAU) and was as efficacious as active control group in three studies. MBCT reduced the risk of depressive relapse in two studies compared to TAU and was equally efficacious to TAU or an active control group in two studies. Overall, studies showed medium effect sizes. Among other limitations are lack of active control group and long-term follow-up in several studies. Conclusion: Evidence supports that MBSR improves mental health and MBCT prevents depressive relapse. Future RCTs should

apply optimal design including active treatment for comparison, properly trained instructors and at least one-year follow-up. Posttraumatic stress disorder (PTSD) is a well-characterized anxiety disorder that may occur after exposure to a traumatic event. Research is ongoing to document neuroanatomic, neuroendocrine, and genetic correlates to the behavioral phenotype. PTSD occurs in 20% to 51% of patients with musculoskeletal injury. Orthopaedic outcomes, including return to work, activities of daily living, patient perception of physical recovery, and objective physical parameters, suffer considerably in patients with PTSD. There are several ways by which treating orthopaedic surgeons can recognize at-risk patients and help prevent the development of PTSD. Should prevention prove unsuccessful, effective treatment strategies exist, as well. Research is needed to investigate whether a correlation exists between successful management of the psychiatric manifestations of PTSD and improved physical outcomes. Debiec, J. Noradrenergic enhancement of reconsolidation in the amygdala impairs extinction of conditioned fear in rats-a possible mechanism for the persistence of traumatic memories in PTSD/ J. Debiec, D.E. PTSD Psychotrauma Neuropsychologie Angst Herinneringen Experimenten Onderzoek Background: Posttraumatic stress disorder (PTSD) is associated with enhanced noradrenergic activity. Animal and human studies demonstrate that noradrenergic stimulation augments consolidation of fear learning. Retrieval of well-established memories by presenting a learned fear cue triggers reconsolidation processes during which memories may be updated, weakened, or strengthened. We previously reported that noradrenergic blockade in the rat amygdala impairs reconsolidation of fear memories. Here we investigated the effects of noradrenergic enhancement on reconsolidation of learned fear. Methods: Using auditory fear conditioning in rats, we tested the effects of postretrieval intraamygdala infusion of the β-adrenergic receptor agonist isoproterenol or the antagonist propranolol on conditioned fear in the amygdala. Results: A single intraamygdala infusion of isoproterenol following a retrieval of a well-consolidated memory enhanced fear memory elicited by the learned fear stimulus and impaired extinction of this memory 48 hr later. Intraamygdala infusion of the β -adrenergic receptor antagonist propranolol following a consecutive retrieval trial blocked the enhancing effects of isoproterenol on fear memory. Conclusions: Postretrieval β -adrenergic stimulation in the amygdala enhances reconsolidation of fear memories, making them resistant to extinction. Noradrenergic augmentation during retrieval of fear memories may thus contribute to persistence and severity of traumatic memories. Reconsolidation may be a useful tool in understanding the pathology of PTSD and may thus help in developing new and in modifying existing treatments of traumatic memories. There is, however, less knowledge about what psychological interventions may be helpful in reducing these symptoms. Evidence from the literature, to date, suggests that of the seven studies that have addressed this issue only four were randomized controlled trials (RCTs). However, all these studies included less than 100 patients with the vast majority reporting positive results. Overall, it seems that although cognitive-behavioural treatment (CBT)-based psychological interventions appear to be efficacious in the treatment of PPTS, the studies are too small to draw any firm conclusions and should be subjected to larger good-quality RCTs. Further research will also need to establish the role of mediating variables such as shame and depression in the treatment of PPTS. Loiselle, K.A. Onderzoek Posttraumatic growth (PTG) involves personal psychological growth in response to a traumatic or very stressful event. Using theoretical guidance from Tedeschi and Calhoun's cognitive model, this study evaluated the relationship between specific individual, distress, and stress-processing factors and PTG among young adults who experienced an illnessrelated trauma earlier in life through a relative's serious illness. Sixty individuals with a relative with a serious illness completed measures of PTG, posttraumatic stress symptoms (PTSS), anxiety, and coping. PTG was positively associated with trait anxiety, PTSS, and the use of active, problem-focused coping strategies. Factors associated with PTG development in individuals who have a relative with a chronic illness are similar to that of individuals who had a serious illness themselves. The relationship between PTSS and PTG is moderated by whether the relative's current illness status is resolved versus not resolved. Lu, W. PTSD Psychotrauma Traumatische ervaringen Psychosen Interviews Onderzoek Psychotic symptoms, coercive interventions, and other negative treatment experiences have been found to lead to posttraumatic stress disorder (PTSD) symptoms. However, prior research has not examined the importance of the DSM-IV A1 (perception of threat) and A2 (negative emotion at time of event) criteria for a traumatic event due to a psychotic episode. To address this question, 50 clients with a history of multiple episodes of psychosis were interviewed to identify distressing experiences related to past episodes, with PTSD assessed (including A1/A2 criteria) related to those events, and other psychiatric symptoms, psychosocial functioning, and coping style. Participants reported intense distress related to psychotic symptoms (66%), treatment experiences (25%), and their combination (8%), with 69% meeting symptom criteria for PTSD (excluding A1/A2 criteria), and 31% meeting full diagnostic criteria for PTSD (including A1/A2 criteria). Clients meeting symptom criteria for PTSD, as well as those meeting full diagnostic criteria for PTSD were similar, with both groups reporting more severe symptoms and distress, and more problems in daily functioning, than clients with fewer or no PTSD symptoms. The results are similar to a previous study of PTSD in persons with recent onset of psychosis (Mueser et al., 2010), and suggest that individuals with PTSD symptoms related to psychosis and coercive treatment may benefit from interventions designed to help them integrate their experiences into their lives and reduce PTSD symptoms, regardless of whether the triggering event(s) meet DSM-IV A1/A2 criteria for a traumatic event. In this study, the authors used a dataset on bereaved spouses and bereaved parents at 4- and 18-months postloss to examine the mediating effects of self-worth and worldviews (benevolence and meaningfulness beliefs). Persons bereaved by violent causes had significantly more posttraumatic stress disorder (PTSD), grief, and depression symptoms at 4- and 18months postloss than persons bereaved by natural causes. Moreover, self-worth but not worldviews mediated the effects of violent loss on PTSD and depression symptoms cross sectionally and PTSD symptoms longitudinally. Findings underscore that self-views are a critical component of problematic reactions to violent loss, but fail to support the role of "shattered" worldviews. Scholte, Willem F. PTSD Melancholie Manie Diagnostiek Cultuur Historians and psychiatrists have repeatedly looked to both real and imagined individuals of the past, like Achilles and Samuel Pepys, and found evidence that they were suffering from symptoms of trauma and posttraumatic stress disorder. The assumptions that allow such historical "diagnoses" have, however, recently been called into question by philosophers such as lan Hacking, anthropologists like Allan Young and psychiatrists such as Patrick Bracken. These scholars have all suggested in various ways that experiences of trauma could not have occurred until the diagnosis of trauma and its symptoms had been formalized and the language of trauma had been developed in the late 19th century. This article attempts to resolve this bifurcation of opinion on the universality of the mind and historical experiences of trauma in two ways. First, it argues for the necessity of applying modern categories of analysis to further present understandings of the past. Second, it considers discussions of "melancholia" and "mania" in premodern medical literature and argues that there are enough similarities between the causes and symptoms of these premodern disorders and modern trauma to suggest that experiences of trauma may not be wholly culturally bound to the modern world, as the above scholars have suggested. While melancholy or mania cannot simply be understood as premodern names for trauma, and it is not always correct to "diagnose" a premodern person who exhibits symptoms of these illnesses with trauma, such an assumption is not always ahistorical or incorrect either. The sample consisted of 123 daily smokers (62% women: M(age)=29.7, SD=11.9). Compared to the PA and no psychopathology groups, the PTSD group reported significantly higher levels of anxiety sensitivity, discomfort intolerance, negative affectivity, anxious arousal, and anhedonic depression; and, the PTSD group reported significantly lower levels of perceived control over anxiety-related events than the PA group. The PD group, compared to those in the PA and no psychopathology groups, reported significantly higher levels of anxiety sensitivity, negative affectivity, and anxious arousal; and significantly lower levels of perceived control over anxiety-related events. No significant differences

Answers were subjected to thematic content analysis. Five major and not mutually exclusive themes were identified: "need no help" (57 participants), "personal ways of coping" (72 participants), "negative attitude towards psychiatric treatment" (91 participants), "comparative insignificance" (24 participants), and "external barriers" (65 participants). While most participants, for different reasons, did not want to seek psychiatric treatment, a significant number, particularly in Western European countries, felt prevented from receiving treatment.

the mid 1990's STS research has increased in scope and volume, yet, little is known about how mental health practitioners experience Secondary Traumatic Stress. This dissertation differentiates itself from previous studies in two ways. First, the sample group covers a range of different providers rather than focusing on one provider type. Second, it uses a hybrid inductive-deductive thematic analysis to ascertain provider experience with STS research. Third, this is the first study to look at Christian counselors. This exploratory, qualitative research explores the ways in which Christian counselors define secondary traumatic stress, how they experience it, and what client, practitioner, and system characteristics influence STS occurrence. The primary goals of this study were to: (1) synthesize the substantive and theoretical literature on STS; (2) identify the system characteristics that promote or attenuate STS occurrence; and (3) identify the context within which STS occurs. This study drew heavily upon three theoretical frameworks. First, it used systems theory principals to explain the elements of the mental health system as they pertain to STS and its component parts. Second, it used synthetic thinking precepts to reveal how the mental health system functions regarding trauma therapists, trauma clients, and trauma therapy. Finally it drew upon chaos theory to identify the "patterns within the system noise" (Patton, 2007: 128). This qualitative study included twenty-five (25) in-depth interviews with Christian mental health practitioners as well as a survey. Both the interviews and the surveys included questions relating the conceptualization of STS, secondary traumatic stress experience, barriers to STS knowledge, and STS education. Results from the analysis show eight of 42 practitioners in this study evidenced high to severe secondary traumatic stress; the majority of providers were in the lowest quartile when measured using the Secondary Traumatic Stress Scale (Bride 2006) and were in the middle to low quartiles on the Professional Quality of Life Scale (Stamm 2009). All practitioners cited a need for educational changes not only around STS but around trauma as well. Practitioners acknowledged a multitude of barriers to STS education including in two-tiered unspoken difference between mental health practitioners and disaster mental health practitioners. Participants identified the need for and the types of supervision needed to prevent STS occurrence and the attendant consequences such as practitioner attrition, poor service quality, and victim retraumatization. These findings contribute to a more comprehensive analysis of the promoters of STS in mental health providers and provides a basis for making policy recommendations to identify effective educational policies. In addition, this work has implications for public policy. Having one in five practitioners showing high or severe levels of STS warrants significant changes to education policy surrounding secondary traumatic stress. Trainingen This article discusses the training of mental health workers whose basic job is with clients that have been seriously affected by armed conflict and/or natural disasters by using 'helping through talking', and who have had little education that is relevant to this work. It sums up the characteristics required of the workers, their learning needs, the messages that the training needs to convey, and the characteristics and potential contents of a tailor made, participants-oriented programme. This approach is illustrated with a few key points from such a training programme.

Natuurrampen Adolescenten Enquêtes PTSD Psychotrauma Onderzoek China BACKGROUND: It is important to understand which factors increase the risk of posttraumatic stress disorder (PTSD) in adolescents. Previous studies have shown that the most important risk factors for PTSD include the type, severity, and duration of exposure to the traumatic events.METHODS: A cross-sectional survey was used to investigate the psychological symptoms associated with the aftermath of a snowstorm disaster in the Hunan province of China in January 2008. Students living in Hunan were surveyed at a three-month follow-up after the disaster. The questionnaire battery included the Impact of Event Scale-Revised (IES-R, trauma and symptoms associated with PTSD), the Chinese version of the Life Orientation Test-Revised (LOT-R, optimism and pessimism), the Chinese version of the Eysenck Personality Questionnaire (EPQ, neuroticism and extraversion), the Chinese Trait Coping Style Questionnaire (TCSQ, positive and negative coping styles), and a range of questions addressing social demographic characteristics and factors relating to the snowstorm. 968 students completed and returned the questionnaires.RESULTS: The results showed that 14.5% of the students had a total IES-R score ? 20. Students with greater school-to-home distances showed higher levels of posttraumatic stress symptoms than students who lived shorter distances from school. Students with emotional support from their teachers reported higher levels of posttraumatic stress symptoms (21.20%) than students without a teacher's emotional support (11.07%). The IES-R total and subscale scores correlated with all variables except extraversion. The binary logistic regression analysis results showed that the teacher's emotional support were risk factors that predicted PTSD frequency and severity.CONCLUSIONS: The risk factors that significantly impacted the onset of posttraumatic stress reactions in students living in Hunan, China following a snowstorm disaster were the school-to-home distance, negative coping, neuroticism, and teacher's emotional support.

Faced with pervasive loss, life threat, and moral conflict in the field of battle, the human body and brain adapt to extraordinary circumstances in extraordinary ways. These adaptations come at a high price, and many men and women returning from Iraq and Afghanistan are paying that price every day. For the clinician who seeks to help, an understanding of the physiology of war-zone stress and resilience is an essential foundation, both for recovery from post-traumatic stress disorder and other conditions, and for addressing the stigma and shame that keep many service members and veterans from seeking and accepting the help and support they need. This article explores ways in which the body and brain adapt to war-zone stress, resulting challenges, and implications for clinical services and ongoing recovery. KOC Gade, D.M. Gevechtsreacties Veteranen PTSD Psychorauma Geestelijke gezondheid Vietnam-veteranen Golfoorlog-veteranen Onderzoek Using a random sample of more than 4000 veterans, we test the effects of combat exposure on mental health. We focus on two cohorts of veterans: those who served in Vietnam (1964-1975) and the Gulf War (1990-1991). Combat exposure differed between these groups in intensity, duration and elapsed time since exposure. We find that combat exposure generally, and exposure to dead, dying, or wounded people, specifically, is a significant predictor of mental health declines as measured by an individual's Mental Component Summary score. Under our general specifications, the negative effects of combat on mental health were larger for Gulf war veterans than for Vietnam veterans as of 2001. These effects persist after controlling for demographic characteristics, insurance coverage, income and assets. Using discrete factor, nonparametric maximum likelihood (DFML) estimation we controlled for unobserved heterogeneity as well as the factors above. In the DFML specifications we find a negative impact of exposure to dead, wounded or dying people for both Gulf and Vietnam veterans, but find no statistically significant effect for combat exposure overall for Vietnam veterans as of 2001. Based on our Gulf war parameters, we estimate that the costs of mental health declines to be between $87 and $318 per year for each soldier with combat service and exposure to dead, dying and wounded people. Veteranen PTSD Psychotrauma Depressie Hoop Behandeling Cognitieve therapie Onderzoek Research exploring nonspecific mechanisms of change is necessary to understand processes that contribute to psychotherapy outcomes. Many veterans diagnosed with posttraumatic stress disorder (PTSD) report a profound sense of hopelessness prior to receiving treatment, and thus one possible nontargeted change mechanism is the enhancement of hope. In this study, 164 veterans diagnosed with PTSD and admitted to a Veteran's Administration residential treatment program received 6 weeks of cognitive processing therapy. Measures of PTSD symptoms, depression and hope were gathered across the course of treatment. Results revealed that higher levels of hope midway through treatment affected reductions in PTSD and depression from mid- to posttreatment (and not the other way around), thus supporting that hope is one nonspecific change mechanism toward symptom reduction. Implications of these findings as they extend to cognitive-behavior therapies as a whole, and to the understanding of how psychological strengths serve as nonspecific change mechanisms in the treatment of PTSD, conclude the article. Long-term outcomes of disability benefits in US veterans with posttraumatic stress disorder / Maureen Murdoch, Nina A. Sayer, Michele R. Spoont en meerdere auteurs[...et al.] In: Archives of General Psychiatry = ISSN 0003 990x: vol. 68 (2011), issue 10, page 1072-1080. Met lit.opg.. Veteranen PTSD Lange-termijn effecten Invaliditeit Uitkeringen Onderzoek Verenigde Staten CONTEXT: Most studies examining the clinical impact of disability benefits have compared aid recipients with people who never applied for benefits. Such practices may bias findings against recipients because disability applicants tend to be much sicker than never-applicants. Furthermore, these studies ignore the outcomes of denied claimants. OBJECTIVE: To examine long-term outcomes associated with receiving or not receiving Department of Veterans Affairs (VA) disability benefits for posttraumatic stress disorder (PTSD), the most common mental disorder for which veterans seek such benefits. DESIGN: Comparison of outcomes between successful and unsuccessful applicants for VA disability payments. Because we could not randomize the receipt of benefits, we used exact matching by propensity scores to control for potential baseline differences. We examined clinical outcomes approximately 10 years later. Setting and PARTICIPANTS: Stratified, nationally representative cohort of 3337 veterans who applied for VA PTSD disability benefits between January 1, 1994, and December 31, 1998. MAIN OUTCOME MEASURES: Assessment on validated survey measures of PTSD; work, role, social, and physical functioning; employment; and poverty. We compared outcomes with earlier scores. Homelessness and mortality were assessed using administrative data. RESULTS: Of still-living cohort members, 85.1% returned usable surveys. Symptoms of PTSD were elevated in both groups. After adjustment, awardees had more severe PTSD symptoms than denied claimants but were nonetheless more likely to have had a meaningful symptom reduction since their last assessment (-6.1 vs -4.4; SE, 0.1; P = .01). Both groups had meaningful improvements of similar magnitude in work, role, and social functioning (-0.15 vs -0.19; SE, 0.01; P = .94), but functioning remained poor nonetheless. Comparing awardees with denied claimants after adjustment, 13.2% vs 19.0% were employed (P = .11); 15.2% vs 44.8% reported poverty (P < .001); 12.0% vs 20.0% had been homeless (P = .02); and 10.4% vs 9.7% had died (P = .66). CONCLUSIONS: Regardless of claim outcome, veterans who apply for PTSD disability benefits are highly impaired. However, receiving PTSD benefits was associated with clinically meaningful reductions in PTSD symptoms and less poverty and homelessness. KOC Pitman, Roger K. Psychotrauma Geheugen Psychofarmaca Onderzoek Many articles about memory reconsolidation conclude with its therapeutic implications for posttraumatic stress disorder (PTSD). A core feature of PTSD is the memory of a traumatic event that is characterized by excessive strength, immalleability, and persistence. We found that Korean and World War II veterans with PTSD showed elevated physiological responses during mental imagery of their personal combat events as long as 40 years later. Recent animal research has challenged the permanence of consolidated memory traces by suggesting that reactivation (retrieval) of a memory can return it to an unstable state from which it must be "re-consolidated" if it is to persist. There is no reason to assume that if the therapeutic PTSD bomb can eventually beconstructed, its ultimate ingredient will be propranolol. In unpublished research with rats, we have found that the glucocorticoid receptor antagonist mifepristone, in addition to blocking reconsolidation of inhibitory avoidance learning (Taubenfeld et al., 2009), has substantially stronger cue-induced-fear reconsolidation-blocking properties than propranolol. This drug has yet to be tested in human reconsolidation experiments. Other drugs may exist that are stronger still. However, for any drug to be clinically useful, it must be approved for human use and capable of systemic administration. Moreover, the efficacy of any drug has yet to be compared with the efficacy of behavioral memory updating techniques. All these questions and more will need to be addressed within a large research and development Manhattan project for PTSD. The translation from preclinical work to clinical application may prove long and difficult, and even unsuccessful. However, given the importance of PTSD as a public mental health problem, it is worth pursuing. Psychotrauma Depressie Prikkelbare darm syndroom Psychosomatiek Comorbiditeit Onderzoek BACKGROUND: Over 1.8 million women in the U.S. are veterans of the armed services. They are at increased risk of occupational traumas, including military sexual trauma. AIM: To evaluate the association between major traumas and irritable bowel syndrome among women veterans accessing Veteran Affairs (VA) healthcare. METHODS: We administered questionnaires to assess trauma history as well as IBS, post-traumatic stress disorder (PTSD) and depression symptoms to 337 women veterans seen for primary care at VA Women's Clinic between 2006 and 2007. Logistic regression was used to evaluate the association between individual traumas and IBS risk after adjustment for age, ethnicity, PTSD and depression. RESULTS: Irritable bowel syndrome prevalence was 33.5%. The most frequently reported trauma was sexual assault (38.9%). Seventeen of eighteen traumas were associated with increased IBS risk after adjusting for age, ethnicity, PTSD and depression, with six statistically significant . Depression and PTSD were significantly more common in IBS cases than controls, but neither substantially explained the association between trauma and increased IBS risk. CONCLUSIONS: Women veterans report high frequency of physical and sexual traumas. A lifetime history of a broad range of traumas is independently associated with an elevated risk of the irritable bowel syndrome. Met lit.opg.. Gevechtsreacties Vietnam-veteranen PTSD Psychotrauma Onderzoek Active participation in combat trauma increased reports of posttraumatic stress disorder (PTSD) symptoms over passive witnessing of trauma. Using archival data from 376 U.S. soldiers who took part in the family interview component of the 1988 National Vietnam Veteran Readjustment Study (NVVRS), findings are that even after statistically accounting for witnessing combat trauma, U.S. soldiers who likely killed enemy soldiers in combat reported elevated levels of PTSD symptoms. Both inference and direct self-reports were used to measure killing in combat, and both measures accounted equally well for variance in PTSD symptoms. The likelihood of a soldier killing enemy combatants was also weakly related to his spouse's report of physical domestic violence in the past year. Diagnosing the mental health symptoms of combat soldiers should specifically assess whether they actively participated in wounding or killing the enemy. KOC
