Grip op de wereld: de evolutie van de hand; Dr. Roy H.J. Erkens; Lecturer Evolutionary biology Maastricht Science Programme Maastricht University De hand met opponeerbare duim is ontzettend belangrijk voor mensen in het dagelijks leven. Het is een zo vanzelfsprekend onderdeel van onze anatomie, dat we ons niet vaak de lange evolutionaire geschiedenis ervan realiseren. Door terug te kijken naar deze geschiedenis, is het echter mogelijk een beter begrip te krijgen van de mogelijkheden en onmogelijkheden van de bouw van de hand. Ook bepaalde aandoeningen zijn beter te begrijpen met deze achtergrond in gedachte. Deze lezing laat zien hoe vanuit vissenvinnen de moderne menselijke hand is ontstaan.

Thumb carpometacarpal osteoarthritis results in pain and loss of function and is often treated using orthoses. The objective of this study was to analyse differences in pain reduction, hand function, patient satisfaction and patient preference between Push Ortho Thumb Brace and a custom-made orthosis in patients with primary osteoarthritis of the carpometacarpal-1 joint. A multicentre crossover randomised controlled trial was conducted that included 63 patients with primary thumb carpometacarpal osteoarthritis; 59 of them completed the study; mean age (SD) was 60.1 (8.2) years. Patients used both orthoses for two weeks with a two-week washout period inbetween. Pain was measured on a 10-cm visual analogue scale. Hand function was assessed using the Jebsen Taylor Hand Function test, Nine Hole Peg Test, key grip, pinch grip and FIHOA. Patient preference was assessed using D-Quest. Both orthoses reduced pain minimally, without significant difference between orthoses. Push Ortho Thumb Brace interfered less with the key grip (p<0.001) and the Nine Hole Peg Test (p<0.001) than the custom-made orthosis. Push Ortho Thumb Brace resulted in higher satisfaction (p<0.001). Most patients preferred this orthosis for future use. Therefore, when prescribing an orthosis for thumb carpometacarpal osteoarthritis this small orthosis seems to be a good choice.

Handtherapie staat niet stil, staat niet los van innovatie in andere sectoren en wensen van patiënten. Voor patiënten zijn comfort en snel herstel, of zo min mogelijk pijn, de belangrijkste wensen. Een handtherapeut helpt de patiënt om dat comfort en dat snelle herstel te realiseren. Technische innovatie helpt in de gezondheidszorg steeds meer en kan ook binnen handtherapie steeds meer van waarde zijn. Aan de andere kant: een handtherapeut wil geen technicus of softwareontwikkelaar worden. Deze presentatie laat een ontwikkeling zien die technische innovatie dichter bij de handtherapeut brengt. De vraag die in deze presentatie wordt beantwoord is: “Hoe maak ik als handtherapeut een maatwerk-orthese voor vinger of duim die een aantal bestaande problemen als vocht, matige pasvorm en reproduceerbaarheid oplost, en wanneer kan ik die werkwijze toepassen?” De gepresenteerde werkwijze is in ontwikkeling, het gaat wel snel. Een jaar geleden werd door Brenda Groenendijk een visie gepresenteerd over het maken van orthesen met behulp van 3D-printen. Nu is die visie concreet. Een maatwerkorthese wordt gemaakt met speciaal daarvoor ontwikkelde hulpmiddelen en werkwijze, waaronder het genoemde 3D-printen. Enkele voorbeelden laten zien hoe de handtherapeut vanuit het patiëntprobleem tot een concrete orthese voor duim of vinger komt zonder een uitgebreide technische opleiding te hoeven doen.
Casuïstiek Crushletsel van de duim Katinka Bastiaansen; ergotherapeut/handtherapeut; werkzaam in Rode Kruis ziekenhuis Beverwijk Op 30/04/2015 wordt Klaus binnengebracht op de Spoedeisende hulp van het Rode Kruis Ziekenhuis in Beverwijk. Bij zijn werk als onderhoudsmonteur voor vrachtwagens is zijn hand beklemd geraakt tussen een luchtbalk en het onderstel van een vrachtwagen. Het gevolg is een groot crushletsel van dig. 1 en 3 waarbij het letsel van dig. 1 gepaard gaat met fors weke delenletsel. De geamputeerde top van dig 1 is meegekomen maar niet meer geschikt voor replantatie. Ter behandeling van de acuut bedreigde duim wordt gekozen voor een lieslapreconstructie. Dig. 3 wordt conservatief behandeld. Tijdens de handtherapie behandeling blijkt al gauw dat het niet de duim is die de grootste uitdaging vormt.
Casuïstiek: Waar een wil is..... Titia van Dijken fysiotherapeut/handtherapeute CHT-NL; werkzaam in st. Antonius ziekenhuis Nieuwegein/ Utrecht. In deze casus wordt het bijna 2 jarig durend revalidatietraject van een 53 jarige man besproken. Een revalidatietraject dat begon na een val op de hand. Gedurende deze revalidatie zijn er vele momenten geweest waarop er keuzes gemaakt moesten worden. Keuzes t.a.v. welke hand therapeutische behandeling gaan we starten? Maar ook op operatief gebied. Wat voor soort operatie, welke operatie eerst en wat is het goede moment om te opereren? Dit alles in samenspraak met de patiënt die tot het uiterste wilde gaan, die zeer gemotiveerd was en een enorm doorzettingsvermogen had. Terugkijkend op de casus komt de vraag: wat als de patiënt niet zoveel doorzettingsvermogen en motivatie had? Waren al deze stappen doorlopen? Hadden wij de patiënt zo kunnen motiveren?

Verlies van functie van een duim is bij kinderen meestal veroorzaakt door een aanlegstoornis van de duim en bij volwassenen ligt een trauma, degeneratieve afwijkingen, rheuma, zenuwletsel of spasticiteit hieraan ten grondslag. 10% van de aangeboren afwijkingen bij kinderen wordt gevormd door afwijkingen aan bovenste extremiteit, 11 % van de afwijkingen aan de hand wordt gevormd door duimafwijkingen en het grootste deel daarvan wordt gevormd door hypoplasie tot aplasie van de duim. De aandoening kan enkel-of dubbelzijdig zijn en deel uitmaken van een syndroom zoals Holt-Oram, Trombopenie Absent Radiussyndroom (TAR) , Vacterl associatie en Fanconi anemie. De oorzaak lijkt multifactorieel met een combinatie van omgevingsfactoren en genetische invloeden. De genetische classificaties voorspellen niets over de functie noch over het te volgen behandelplan. Duimhypoplasie: De bekendste classificatie is die van Blauth type 1 t/m 5 waarbij 1 minimale hypoplasie is en type 5 volledige afwezigheid van de duim betekent. Interventies kunnen bestaan uit peestransfers, reconstructie van het ulnaire collaterale ligament, transfer van een gewricht en type 4 en 5 worden behandeld met pollicisatie. Het principe van pollicisatie werd door ons ook gebruikt bij een volwassene die zijn duim kwijtraakte door een vuurwerkletsel. Met vereende krachten is hij inmiddels tevreden weer aan de slag.

Casuïstiek Pollicisatie: Een 'nieuwe duim' en dan ... Elianne Eijffinger hand-ergotherapeut CHT-NL MSc; afdeling Revalidatie Academisch Medisch Centrum in Amsterdam Wat is de impact van een pollicisatie, een ‘neoduim’, van de linker niet dominante hand op het uitvoeren van de dagelijks activiteiten en participatie van een 25 jarige man? De volgende aspecten worden besproken; voorlichting & evaluatie preoperatief, coping & motivatie van de patiënt, onderzoek en behandeling. Tevens komen voor- en nadelen van de pollicisatie zoals ervaren door de patiënt aan bod.

Revant Revalidatiecentrum Breda Een vreemde gewaarwording om als handtherapeut een handletsel te ondergaan. Bij een val met de fiets op 6 januari luxeerde ik mijn IP gewricht in hyperextensie. Er was een open verwonding met verdenking van fractuur. Na repositie en hechten kon ik in de gips naar huis. Na 2 weken gips kreeg ik een beschermend spalkje en startte de handtherapie. Maar ondanks veel oefenen bleef de mobiliteit beperkt. Een echo wees uit dat er een botfragmentje van ongeveer 3x2 mm aan de volaire zijde lag met daar omheen een forse synovitis wellicht verantwoordelijk voor de flexiebeperking in het gewricht. 23 maart heeft er een exploratie van het gewricht plaats gevonden: de FPL was verkleefd met de A2 pulley en losgemaakt, om het botfragmentje te bereiken moest een doorsnijding van de volaire plaat plaats vinden. Het kraakbeen van het IP gewricht aan de volaire zijde bleek aangetast en deels afwezig waarschijnlijk als gevolg van de avulsie. Bij testen was er een flexie mogelijk van 60°. Postoperatief werd meteen de revalidatie weer opgestart.
De ski duim: patiëntenervaringen over de huidige postoperatieve spalk behandeling en reflecties ten aanzien van mogelijke veranderingen Marjolein Wind; ergotherapeut / handtherapeut CHT-NL; 4hands, praktijk voor handtherapie en Hand & Polscentrum Amsterdam General context Currently different post-operative treatment protocols are used after ulnar collateral ligament repair without substantiation for length of immobilization and splint design. Few research studies showed that modified splint designs, in which early movement of the metacarpophalangeal joint is allowed, resulted in less stiffness, earlier return to work and less therapy sessions. Specific research problem In the hand therapy practice patients demonstrate dissatisfaction with current splint provision and disappointing postoperative outcomes like stiffness and pain. Most research studies towards postoperative management of skiers’ thumb injuries focuses on quantitative outcome measurers; no study from patient’s perspective towards the topic was found. Purpose of this study is to explore patients’ experiences, perceptions and recommendations regarding the design of a splint used after surgical repair of a complete tear of the ulnar collateral ligament of the thumb. Research methodology To answer the research question semi-structural in depth interviews were conducted with six participants. The aim was to explore experiences towards immobilization after surgery and provision of removable splints, recommendations towards the design of splints, ideas towards modified splints and final outcomes after rehabilitation resulting in recommendations towards postoperative treatment. Findings One participant received a removable splint immediately after surgery and was the only participant being completely satisfied towards the whole postoperative treatment. Besides recommendations towards sharp edges, there are no complaints after three months. Removable, adjustable thermoplastic splints should be provided immediately after surgery instead of 6 weeks immobilization in casts. All participants are positive towards possible changes to the splint design that allows early mobilization in the splint. Conclusions and recommendations Findings suggest that a removable, adjustable, thermoplastic splint direct after surgery resulted in better functional outcomes and improved patient satisfaction. Early referral to hand therapy is recommended to fabricate an adequate splint and to instruct exercises in an early stage.

Chronisch compartimentsyndroom van de thenarmusculatuur Drs. T.D. van Brederode, afdeling plastische chirurgie, Isala Zwolle Het compartimentsyndroom wordt meestal met een acuut probleem geassocieerd. Er bestaat echter ook een chronische vorm, waarbij bij herhaalde belasting van weefsels er een verhoogde druk in een fascie-compartiment ontstaat. Hierdoor ontstaan pijnklachten en soms neurologische uitval. Het chronisch compartimentsyndroom wordt het meest beschreven in de onderste extremiteit bij atleten. Het komt veel minder vaak voor in de bovenste extremiteit, en zit dan meestal in de spieren van de onderarm. Zeer zelden wordt geschreven over chronisch compartimentsyndroom van de thenarmusculatuur. Deze diagnose wordt in de praktijk nogal eens gemist, mede doordat de klachten kunnen lijken op carpometocarpale 1 artrose of een carpaal tunnelsyndroom. De hand bevat 10 afzonderlijke fascie-compartimenten, waarvan de thenarmusculatuur er één is. Patiënten presenteren zich met een pijnlijke, soms opgezette duimmuis bij herhaalde inspanning van de duim. Daarnaast kunnen er krachtsverlies, tintelingen en een doof gevoel optreden in de duim. De klachten nemen in rust weer langzaam af. De diagnose wordt gesteld op basis van de kliniek en dynamische drukmeting binnenin het fascie-compartiment. De definitieve oplossing is een electieve fasciotomie van het thenar compartiment onder lokale verdoving. Literatuurverwijzing: 1.
