Total E&P Indonésie, hereinafter referred to as TI, is continually improving a safe and healthy place for employees, contractors, and neighbors. Only those contractors who have demonstrated management leadership and system resulting in good HSE performance are added to the approved contractors list. Please complete this pre-qualification form with the respective document (hardcopy and/or softcopy is preferable). Please provide the up to date (latest) document with clear linked evidences between this form and attachment. TI melakukan perbaikan terus menerus terhadap keselamatan dan kesehatan tempat kerjanya untuk kepentingan para karyawan, kontraktor, dan penduduk sekitar. Hanya kontraktor-kontraktor yang memiliki kepemimpinan dan sistem manajemen yang menghasilkan kinerja keselamatan, kesehatan kerja dan lindungan lingkungan (K3LL) yang baik yang akan diikutsertakan ke dalam daftar kontraktor yang disetujui. Harap formulir pra-kualifikasi ini dilengkapi dengan dokumen terkait (hardcopy and/or softcopy lebih dianjurkan). Mohon dokumen yang dikirimkan merupakan dokumen yang terkini dilengkapi dengan sistem penandaan yang jelas pada form ini dan lampirannya. To be considered as a contractor qualified to perform the specified work for TI, you must meet the minimum acceptance criteria established at the discretion of TI. Untuk dipertimbangkan sebagai kontraktor yang memenuhi syarat untuk melaksanakan pekerjaan tertentu bagi TI, Anda harus memenuhi kriteria penerimaan minimum yang ditetapkan oleh TI. TI expects that you :  Have a documented HSE program / plan that meet TI standard requirements applicable to the work.  Have a program to comply with TI HSE Notice to Contractors (SOP 07-701 HSE NC).  Agree that any utilized sub-contractor will meet the requirements listed above.
