SUMMARY Type 1 diabetes is an autoimmune disease with an increasing incidence in the entire world. The disease is characterized by a deficiency in the production of insulin by pancreatic islets. Nowadays the most common treatment is insulin injections. These insulin injections cannot prevent frequent episodes of hyper- or hypoglycemia. This has a high impact on the quality of life of patients and may result in severe side effects on the long term. The only way to prevent hyper- and hypoglycemia is by providing the patients with an insulin source that regulates the glucose levels from a minute-to-minute level. This can be achieved by transplanting either the full pancreas or the insulin producing pancreatic islets. Transplantation of the insulin producing islets is having the preference as it is in contract to transplantation of the full pancreas not associated with major surgery. Unfortunately, transplantation is limited by the availability of donors and the fact that it requires immunosuppression to prevent rejection. Especially the application of immunosuppression limits application of transplantation in diabetes patients as immunosuppression not only reduces the immune responses against the graft but also against pathogens and cancer cells. For this reason immunosuppression will never be an acceptable alternative for insulin therapy. Islets encapsulated in immunoprotective microcapsules are possibly an approach that can overcome the obstacle of immunosuppression. In this technology, islets are enveloped in semi-permeable but immunoprotective membranes that protect the islets against the effects of the immune system. In Chapter 1 we describe the details of the technology and its biocompatibility issues as well as the versatility of the system. Many materials for producing microcapsules have been proposed, either from natural sources as alginate, agarose, chitosan, cellulose, collagen and xanthan, or from synthetic sources like poly-ethylene glycol, polyvinyl alcohol, polyurethane, poly-ether sulfone, polypropylene, sodium polystyrene sulfate or polyacrylate. However, up to now, only alginate qualifies as ready for clinical application. This is the reason why we focus in this thesis on alginate. Many different alginate-based capsule systems do exist and can be produced from alginates. Examples of this are the calcium and barium beads, characterized by their high mechanical stability and easy production. Other common systems are the polyaminoacidalginate capsules, which allow the adjustment of permeability and stability of the microcapsule combining an alginate bead core with an outer layer of polyaminoacids. A pitfall in the field of encapsulation is that these aforementioned systems are applied without a targeted strategy with varying degrees of success after transplantation as a consequence. In Chapter 1, we discuss these different alginate-based systems in view of future application in humans. The use of allogeneic and xenogeneic islet sources is discussed since immunoprotection requirements expectedly different approaches for alloand xenotransplantation. All encapsulating systems must prevent the diffusion of isletderived molecules after cellular death, such as the danger associated molecular patterns (DAMPs) since they are able to induce an immune reaction. A common overlooked property of the encapsulation systems is that alginates for medical application should have an extreme high degree of purity and lack pathogen-associated molecular patterns (PAMPs) to avoid activation of the recipient’s immune system. Differences between relevant innate and adaptive immune responses in humans and other experimental species that are used to study and predict efficacy of encapsulated islets in humans are reviewed. Especially, the extreme differences between the immune system of non-human primates and humans is 115
discussed from the perspective of specific polymorphisms or lack of some genes. Those differences result in diverse mortality rates in juvenile and adult ages, making non-human primates a not-suitable model to proof efficacy in transplantation. Therefore a multistep model combining in vitro, ex vivo and in vivo tests should be developed to predict the efficacy of encapsulated grafts in humans. Current insight is that although the immunisolation-technology is versatile, major research efforts are required for identifying the mechanical, immunological, and physico-chemical requirements that alginate-based capsules should meet for successful human application. Our first approach to reproducibly achieve success in the use of capsules led to define a standardized and reproducible platform to assess the quality, purity and immune reactivity of alginate. This technology platform, described in Chapter 2, assesses the immunostimulatory capacity of alginate or its contaminants. It describes where in the purification process PAMPs are removed, and which Toll-like receptors (TLRs) and ligands are involved. A THP1 cell-line expressing pattern recognition receptors (PRRs) and the cosignalling molecules CD14 and MD2 was used to assess immune activation of alginates during the different steps of purification of alginate. To determine if this activation was mediated by TLRs, a THP1-defMyD88 cell-line was applied. This cell-line possesses a non-functional MyD88 coupling protein, necessary for activating NF-kB via TLRs. To identify the specific TLRs being activated by the PAMPs, we used different human embryonic kidney (HEK) cell-lines that express only one specific TLR. Finally, specific enzyme-linked immunosorbent assays (ELISAs) were applied to identify the specific PAMP. By applying this three-step procedure, we can screen alginate in a manner, which is both labour and cost efficient. The efficacy of the platform was evaluated with an alginate that did not pass our quality control. We demonstrate that this alginate was immunostimulatory, even after purification due to reintroduction of new contaminants. In addition, we tested two commercially available purified alginates that contained peptidoglycan, lipoteichoic acid, flagellin, and even lipopolysaccharides (LPS). The platform presented here can be used to evaluate the efficacy of purification procedures in removing PAMPs from alginates in a cost-efficient manner. The effectiveness of our platform was tested in Chapter 3, where we show that commercial, crude alginates contain pathogen-associated molecular patterns (PAMPs), which are recognized by the sensors of the innate immune system. Known sensors are Tolllike receptors (TLRs), NOD receptors, and C-type lectins. Using a THP1 cell-line with a non-functional MyD88, we were able to show that contaminants of alginates signal mainly via MyD88. This was found for alginates applied in encapsulation systems with a low-, intermediate-, and high- ratio of guluronic acid, i.e. calcium-beads, barium-beads as well as in alginate–PLL–alginate capsules. These alginates did stimulate TLRs 2, 5, 8, and 9 but not TLR4 (LPS receptor). Upon implantation in rats these alginates provoked a strong inflammatory response resulting in fibrosis of the capsules. Analysis demonstrated that commercial alginates contain the PAMPs peptidoglycan, lipoteichoic acid, and flagellin. By applying purification procedures, these PAMPs were largely removed. This removal was associated with deletion of the inflammatory tissue responses as confirmed by an implantation experiment in rats. Our data also show that alginate itself does not provoke TLR mediated responses. In between the period of transplantation and revascularization (in case of free islets) or when islets are encapsulated, the islets are exposed to hypoxic and low-nutrient conditions. In Chapter 4 we mimicked those harsh circumstances in vitro to study the 116
involvement of different cell death processes, release of danger-associated molecular patterns (DAMP), and associated in vitro immune activation. Under hypoxic and low nutrient conditions, not only apoptosis but also autophagy and necroptosis occur in human islets. Necroptosis and necrosis are responsible for DAMP-release such as dsDNA, uric acid, and HMGB1. The sensors of the innate immune system are able to recognize these DAMPs. Known sensors are Toll-like receptors (TLRs), NOD receptors, and C-type lectins. By using THP1 cell-lines with a non-functional MyD88 coupling molecule, we were able to show that the islet-derived DAMPs signal mainly via TLR. Immunoisolation in immunoprotective membranes reduced DAMP release and immune activation via retention of the relatively large DAMPs in the capsules. Another strategy that was effective was suppressing necroptosis using the inhibitor nec-1. Although the effect on cell-survival was minor, nec-1 was able to reduce the release of HMGB1 and its associated MyD88 dependent NF-κB activation in THP1 cells. Our data demonstrate that in the immediate post-transplant period DAMPs contribute to enhanced innate immune responses in vitro which can be reduced by immunoisolation or intervention with nec-1. To study the impact of DAMPs on graft survival duration and function in vivo we used, in Chapter 5, a MyD88 knockout mice model with non-functional TLRs to compare the graft survival with that of a wild type mice. After streptozotocin-induced diabetes, both MyD88 knockout mice and wild type animals received xenografts of 1000 albino-oxford (AO) islets. The MyD88 knockout recipients became normoglycemic in 4 days while in the wild types it took 11 days. The normoglycemia was maintained at least for 7 weeks in both, MyD88 KO BL6 recipients. A glucose tolerance test was performed after, at least, 4 weeks of normoglycemia in MyD88 KO and BL6 mice, and demonstrated a higher islet activity in MyD88 KO. After graft failure capsules were retrieved and studied the number of capsules affected by cellular overgrowth and the composition of the overgrowth by immunocytochemistry. Finally, Chapter 6 contains the general discussion and future perspectives on the development and application of immunoisolated islets for treatment of diabetes.

NEDERLANDSE SAMENVATTING Type 1 diabetes is een auto-immuunziekte die wereldwijd steeds vaker voorkomt. Het belangrijkste kenmerk van de ziekte is dat de eilandjes van Langerhans in de alvleesklier geen insuline kunnen produceren. Op dit moment zijn injecties met insuline de meest voorkomende vorm van behandeling. Deze injecties kunnen echter niet voorkomen dat er regelmatig een te hoge of te lage suikerspiegel in het bloed is. Dit heeft een negatieve invloed op de kwaliteit van leven van diabetespatiënten en kan de gezondheid negatief beïnvloeden op de langere termijn. De enige manier om een te hoge of lage suikerspiegel te voorkomen is om patiënten een bron van insuline te geven waarmee de hoeveelheid glucose in het bloed van minuut tot minuut wordt gereguleerd. Dit kan bereikt worden met transplantatie van de volledige alvleesklier, of de insuline producerende eilandjes van Langerhans. Transplantatie van alleen de eilandjes geniet de voorkeur, omdat het een minder ingrijpende operatie is dan transplantatie van de hele alvleesklier. Nadelen van transplantatie zijn dat er een geschikte donor voorhanden moet zijn en het immuunsysteem moet onderdrukt worden met medicijnen om afstoting te voorkomen. Voornamelijk het onderdrukken van het immuunsysteem is problematisch, aangezien niet alleen de immuunreactie tegen het getransplanteerde weefsel wordt onderdrukt, maar ook de immuunreactie tegen bijvoorbeeld pathogenen en kankercellen. Om deze reden zal transplantatie nooit een acceptabel alternatief zijn voor injecties met insuline. Het inkapselen van de eilandjes in microcapsules om bescherming te bieden tegen het immuunsysteem is mogelijk een manier om het onderdrukken van het immuunsysteem met medicijnen overbodig te maken. Met deze technologie worden de eilandjes ingekapseld in semi-permeabele membranen die de eilandjes beschermen tegen de schadelijke effecten van het immuunsysteem. In Hoofdstuk 1 beschrijven we de details van de techniek. Daarnaast bespreken we problemen met biocompatibiliteit en de verschillende mogelijke toepassingen van deze techniek. Veel verschillende materialen zijn voorgesteld voor de productie van microcapsules. Dit betreft natuurlijke materialen of synthetische materialen. Echter tot nu toe is alleen alginaat, afkomstig uit zeewier, klaar voor klinische toepassing. Daarom focussen we ons in dit proefschrift op alginaat. Er zijn veel verschillende op alginaat gebaseerde capsule systemen en die kunnen geproduceerd worden van alginaat. Voorbeelden zijn calcium en barium beads, die bekend staan om hun hoge mechanische stabiliteit en die makkelijk te produceren zijn. Andere veel gebruikte systemen zijn de polyaminozuur-alginaat capsules. Hiermee kan de permeabiliteit en de stabiliteit van de microcapsule aangepast worden door een kern van alginaat te combineren met een buitenlaag van poly-aminozuren. Een valkuil in het vakgebied van inkapsulatie is dat de eerder genoemde systemen worden gebruikt zonder een duidelijke strategie, waardoor de resultaten na transplantatie wisselend zijn. In Hoofdstuk 1, behandelen we de verschillende systemen gebaseerd op alginaat met het oog op de toepassing ervan in de toekomst bij mensen. Het gebruik van eilandjes van humane donoren of dieren wordt bediscussieerd, aangezien de benodigdheden voor bescherming tegen het immuunsysteem zullen verschillen tussen transplantatie met humaan of dierlijk materiaal. Ongeacht voor welk inkapsulatie systeem wordt gekozen, ze moeten in ieder geval ervoor zorgen dat stoffen die worden uitgescheiden door stervende cellen, de zogenaamde danger associated molecular patterns (DAMPs), niet in staat zijn om het immuunsysteem te waarschuwen. Tevens is het belangrijk dat het alginaat erg puur is en niet verontreinigd is met materiaal afkomstig van micro-organismen. Bepaalde 118
structuren afkomstig van micro-organismen, pathogen-associated molecular patterns (PAMPs) genoemd, kunnen namelijk ook een immuunreactie opwekken. Tevens worden verschillen tussen het immuunsysteem van mensen en diermodellen besproken die gebruikt worden om de ingekapselde eilandjes te testen. Hieruit blijkt met name dat niet-menselijke primaten een ongeschikt diermodel zijn om het succes van transplantatie te testen. Daarom is een model nodig dat in vitro, ex vivo, en in vivo technieken combineert, om goed te kunnen voorspellen hoe succesvol de transplantatie van ingekapselde eilandjes naar de mens zal verlopen. De stand van zaken op dit moment is dat de technologie breed toegepast zou kunnen worden, maar er is nog veel onderzoek nodig voordat capsules gebaseerd op alginaat succesvol gebruikt kunnen worden bij mensen. Onze eerste aanpak om succesvol gebruik te maken van de capsules was om gestandaardiseerde en reproduceerbare methoden te ontwikkelen om de kwaliteit, puurheid, en immuunreactiviteit van alginaat te bepalen. Met dit technologie platform, beschreven in Hoofdstuk 2, wordt gekeken in hoeverre alginaat of eventuele verontreinigingen het immuunsysteem kunnen activeren. Hier wordt beschreven waar de PAMPs worden verwijderd in het purificatie-proces. Tevens wordt er ingegaan op de structuren en de bijbehorende receptoren van een bekende groep van PAMPs die hierbij betrokken zijn, de zogenaamde Toll-like receptoren (TLRs). De THP-1 humane monocyten cellijn heeft veel van die receptoren en is daarom gebruikt om te kijken of alginaat afkomstig van verschillende stappen in het purificatieproces in staat was om een immuunreactie teweeg te brengen. Om te onderzoeken of de immuunreactie inderdaad was veroorzaakt door TLRs, is een cellijn gebruikt die deficiënt is in het eiwit MyD88, wat TLRs nodig hebben om de immuunreactie op gang te brengen. Om te bestuderen welke TLR precies betrokken was bij de immuunreactie, hebben we cellijnen gebruikt die elk maar één TLR tot uitdrukking brengen. Daarnaast hebben we ELISA gebruikt om de specifieke PAMP te identificeren. Door gebruik te maken van deze drie procedures, kunnen we de eigenschappen van alginaat beoordelen op een goedkope en efficiënte manier. De bruikbaarheid van het platform hebben we geëvalueerd met alginaat dat niet door de kwaliteitscontrole heen kwam. We laten zien dat dit alginaat het immuunsysteem activeert, zelfs na zuivering. Daarnaast ontdekten we dat twee commercieel verkrijgbare gezuiverde alginaten verontreinigd waren met meerdere PAMPs. De effectiviteit van ons platform hebben we getest in Hoofdstuk 3, waar we laten zien dat commercieel verkrijgbare ongezuiverde alginaten PAMPs bevatten, die herkend worden door receptoren van het aangeboren immuunsysteem. Bekende receptoren zijn de TLRs, NOD receptoren, en C-type lectins. Met behulp van een THP-1 cellijn die deficiënt is in het eiwit MyD88 laten we zien dat de verontreinigingen in alginaat vooral MyD88 activeren. Dit resultaat was hetzelfde voor alle verschillende inkapselsystemen die we getest hebben. Deze alginaten activeerden TLRs 2, 5, 8, en 9, maar niet TLR4 (LPS receptor). Na implantatie in ratten veroorzaakten deze alginaten een sterke immuunreactie, wat uiteindelijk leidde tot fibrose van de capsules. Na analyse van commercieel verkrijgbare alginaten konden we specifieke PAMPs identificeren. Deze PAMPs werden grotendeels verwijderd na zuivering. Als gevolg daarvan activeerden de alginaten na zuivering geen immuunreactie meer, wat we bevestigd hebben met een implantatie experiment in ratten. Onze data laten ook zien dat alginaat zelf geen TLRs activeert. In de periode van transplantatie en herstel van de bloedvoorziening, of wanneer de eilandjes worden ingekapseld, moeten de eilandjes een tijd overleven met weinig zuurstof en voedingsstoffen. In Hoofdstuk 4 hebben we zulke barre omstandigheden nagebootst in 119
vitro om de betrokkenheid van verschillende celdodings processen te onderzoeken. Tevens hebben we gekeken naar de productie van DAMPs en de daarmee geassocieerde immuunreactie. Bij gebrek aan zuurstof en voedingsstoffen gaan de cellen in de eilandjes niet alleen op een nette manier dood (apoptose), maar er is ook sprake van autofagie en necrose. De laatste twee processen zorgen voor de uitscheiding van bepaalde DAMPs, die herkend kunnen worden door receptoren van het aangeboren immuunsysteem. Met behulp van een THP-1 cellijn die deficiënt is in het eiwit MyD88 laten we zien dat het DAMP signaal afkomstig van de eilandjes vooral wordt herkend door TLRs. Immuun-isolatie in immuun-beschermende membranen zorgde voor minder uitscheiding van DAMPs en immuunactivatie doordat de grotere DAMPs achterbleven in de capsules. Een andere veelbelovende strategie was het onderdrukken van een bepaalde vorm van necrose (necroptosis) met de remmer nec-1. Het effect op het aantal cellen die doodgingen was minimaal, maar nec-1 zorgde er wel voor dat een bepaalde DAMP, HMGB1, minder werd uitgescheiden. Als gevolg daarvan was er ook een lagere immuunreactie. Onze data demonstreren dat vlak na transplantatie DAMPs bijdragen aan de immuunreactie en dat deze reactie onderdrukt kan worden door immuun-isolatie, of interventie met de remmer nec-1. In Hoofdstuk 5 hebben we de impact van DAMPs op de overleving van de eilandjes van Langerhans na transplantatie onderzocht in een diermodel. Hiervoor gebruikten we muizen deficiënt in het voor TLRs essentiële eiwit MyD88. Overleving van de eilandjes na transplantatie werd in deze muizen vergeleken met normale muizen. Eerst werden de muizen diabeet gemaakt door toediening van de stof streptozotocin. Daarna kregen beide groepen muizen 1000 eilandjes afkomstig van ratten. De MyD88-deficiënte muizen hadden weer normale hoeveelheden glucose in het bloed na 4 dagen, terwijl het bij de gewone muizen 11 dagen duurde. De glucosewaarde in het bloed bleef minimaal 7 weken normaal in beide groepen muizen. Na minimaal 4 weken van normale glucosewaardes hebben we een glucose tolerantie test gedaan in beide groepen. Hieruit bleek dat de eilandjes in de MyD88-deficiënte muizen actiever waren. Samenvattend, laten onze resultaten inderdaad zien dat TLRs een belangrijke rol spelen in een diermodel van transplantatie. In Hoofdstuk 6 worden de resultaten bediscussieerd en ook behandelen we de toekomstperspectieven voor de ontwikkeling en toepassing van ingekapselde eilandjes voor de bestrijding van diabetes.
