Onderzoek doen is een mooie evenwichtsoefening. Aan de ene kant ligt onze primaire focus natuurlijk bij de directe patiëntenzorg. Aan de andere kant maken we zoveel mogelijk tijd vrij voor onderzoek, een essentieel onderdeel waarmee onze organisaties inmiddels in een rijke traditie staan. Onze organisaties hebben veel te bieden, zeker op het gebied van toegepast onderzoek. Inzet hierbij is altijd dat meer patiënten kunnen profiteren van nieuwe inzichten en nieuwe behandelingen. Daarbij is het goed dat we steeds meer samenwerken, niet alleen met elkaar maar ook met onderzoeksinstituten uit de regio en op landelijk en internationaal niveau. Die focus helpt om state-of-the-art te blijven en om goede medisch-specialisten op te leiden én om ze uit te kunnen dagen om onze zorg nog beter te maken. Het doet mij deugd u dit overzichtsboekje aan te mogen bieden. Het biedt een overzicht van al het onderzoek dat in 2010 heeft plaatsgevonden in het Deventer Ziekenhuis, het Radiotherapeutisch Instituut Stedendriehoek en Omstreken en Dimence. Het gaat om publicatie van wetenschappelijke artikelen in nationale en internationale (wetenschappelijke) tijdschriften. Maar ook om boekbijdragen, voordrachten, posters en om promoties. Altijd is er een heldere band met de patiëntenzorg waar het Deventer Ziekenhuis, Dimence, Zorggroep Solis en het RISO voor staan en waarmee ze zich onderscheiden. Dit boekje laat als geen andere uitgave zien dat wij ook het voorbije jaar ervoor hebben gezorgd dat we naast het verlenen van gastvrije, veilige en effectieve zorg ook een mooie bijdrage hebben kunnen leveren aan wetenschap en innovatie. Lenient versus strict rate control in patients with atrial fibrillation. N Engl J Med. 2010 Apr 15;362(15):1363-73. Epub 2010 Mar 15. Abstract BACKGROUND: Rate control is often the therapy of choice for atrial fibrillation. Guidelines recommend strict rate control, but this is not based on clinical evidence. We hypothesized that lenient rate control is not inferior to strict rate control for preventing cardiovascular morbidity and mortality in patients with permanent atrial fibrillation. METHODS: We randomly assigned 614 patients with permanent atrial fibrillation to undergo a lenient rate-control strategy (resting heart rate <110 beats per minute) or a strict rate-control strategy (resting heart rate <80 beats per minute and heart rate during moderate exercise <110 beats per minute). The primary outcome was a composite of death from cardiovascular causes, hospitalization for heart failure, and stroke, systemic embolism, bleeding, and life-threatening arrhythmic events. The duration of follow-up was at least 2 years, with a maximum of 3 years. RESULTS: The estimated cumulative incidence of the primary outcome at 3 years was 12.9% in the lenient-control group and 14.9% in the strict-control group, with an absolute difference with respect to the lenient-control group of -2.0 percentage points (90% confidence interval, -7.6 to 3.5; P<0.001 for the prespecified noninferiority margin). The frequencies of the components of the primary outcome were similar in the two groups. More patients in the lenientcontrol group met the heart-rate target or targets (304 [97.7%], vs. 203 [67.0%] in the strict-control group; P<0.001) with fewer total visits (75 [median, 0], vs. 684 [median, 2]; P<0.001). The frequencies of symptoms and adverse events were similar in the two groups. CONCLUSIONS: In patients with permanent atrial fibrillation, lenient rate control is as effective as strict rate control and is easier to achieve. (ClinicalTrials.gov number, NCT00392613.)

Prognostic value of galectin-3, a novel marker of fibrosis, in patients with chronic heart failure: data from the DEAL-HF study. Abstract AIMS: Biomarkers are increasingly being used in the management of patients with chronic heart failure (HF). Galectin-3 is a recently developed biomarker associated with fibrosis and inflammation, and it may play a role in cardiac remodeling in HF. We determined its prognostic value in patients with chronic HF. METHODS AND RESULTS: Patients with chronic HF (New York Heart Association functional class III or IV) who participated in the Deventer-Alkmaar heart failure study were studied. Galectin-3 levels were determined at baseline using a novel optimized enzyme-linked immunosorbent assay. Univariate and multivariate analyses were used to determine the prognostic value of this biomarker. We studied 232 patients; their mean age was 71 +/- 10 years, 72% were male, and 96% were in NYHA class III. During a follow-up period of 6.5 years, 98 patients died. Galectin-3 was a significant predictor of mortality risk after adjustment for age and sex, and severity of HF and renal dysfunction, as assessed by NT-proBNP and estimated glomerular filtration rate, respectively (hazard ratio per standard deviation 1.24, 95% CI 1.03-1.50, P = 0.026). CONCLUSION: Plasma galectin-3 is a novel prognostic marker in patients with chronic HF. Its prognostic value is independent of severity of HF, as assessed by NT-proBNP levels, and it may potentially be used in the management of such patients.
RESULTS: We describe a 76-year-old female, who was analyzed for dyspnea on exertion (DOE) and chronic fatigue, with known myelodysplastic syndrome and an aneurysmal circumflex coronary artery-coronary sinus fistulous connection associated with severe mitral regurgitation. Mitral valve replacement using a bioprosthesis was performed as well as ligation of the fistula. The postoperative course was complicated with cardiac tamponade, which was successfully drained. CONCLUSION: Our patient presented with chronic fatigue and DOE and was found to have a coronary artery fistula and severe mitral regurgitation associated with known myelodysplasia. Conventional coronary angiography failed to demonstrate the entire fistula characteristics (origin, pathway, and outflow). Multidetector computed tomography was complementary to demonstrate the complex anatomy of the fistula. The fistula was surgically ligated in combination with mitral valve replacement. She remains well.

They occur in 0.1-0.2% of patients undergoing coronary arteriography. The origin of the fistulas is the right coronary artery followed by the left anterior descending and lastly by the circumflex artery (17%). Termination into the right heart side occurs in 90% of cases. Termination into the coronary sinus is rare in 3% of cases. Circumflex artery-coronary sinus fistulas are even rarer. DESIGN: A single case report and literature review between 1993 and 2007.
Management of chronic heart failure guided by individual N-terminal pro-B-type natriuretic peptide targets: results of the PRIMA (Can PRo-brainnatriuretic peptide guided therapy of chronic heart failure IMprove heart fAilure morbidity and mortality?) study. Abstract OBJECTIVES: The purpose of this study was to assess whether management of heart failure (HF) guided by an individualized N-terminal pro-B-type natriuretic peptide (NT-proBNP) target would lead to improved outcome compared with HF management guided by clinical assessment alone. BACKGROUND: Natriuretic peptides may be attractive biomarkers to guide management of heart failure (HF) and help select patients in need of more aggressive therapy. The PRIMA (Can PRo-brain-natriuretic peptide guided therapy of chronic heart failure IMprove heart fAilure morbidity and mortality?) study is, to our knowledge, the first large, prospective randomized study to address whether management of HF guided by an individualized target NT-proBNP level improves outcome. METHODS: A total of 345 patients hospitalized for decompensated, symptomatic HF with elevated NT-proBNP levels at admission were included. After discharge, patients were randomized to either clinically-guided outpatient management (n = 171), or management guided by an individually set NT-proBNP (n = 174) defined by the lowest level at discharge or 2 weeks thereafter. The primary end point was defined as number of days alive outside

Dermatologie the hospital after index admission. RESULTS: HF management guided by this individualized NT-proBNP target increased the use of HF medication (p = 0.006), and 64% of HF-related events were preceded by an increase in NT-proBNP. Nevertheless, HF management guided by this individualized NT-proBNP target did not significantly improve the primary end point (685 vs. 664 days, p = 0.49), nor did it significantly improve any of the secondary end points. In the NT-proBNP-guided group mortality was lower, as 46 patients died (26.5%) versus 57 (33.3%) in the clinically-guided group, but this was not statistically significant (p = 0.206). CONCLUSIONS: Serial NT-proBNP measurement and targeting to an individual NT-proBNP value did result in advanced detection of HF-related events and importantly influenced HF-therapy, but failed to provide significant clinical improvement in terms of mortality and morbidity. Publicaties Resorcinol peels as a possible self-treatment of painful nodules in hidradenitis suppurativa. Boer J, Jemec GBE Clin Exp Dermatol. 2010 Jan; 35(1): 36-40 Epub 2009 Jun 22 Abstract BACKGROUND: Hidradenitis suppurativa (HS) is a chronic, inflammatory skin disease characterized by abscess formation localized to apocrine sweat gland-bearing skin. The most important factor in patients’ overall assessment of disease severity is pain. The duration of abscesses takes days to weeks and are always painful. AIM: To assess the efficacy of self-treatment with topical 15% resorcinol in an open study. METHODS: The case notes of 12 women with stage 1 or 2 HS treated with topical resorcinol and followed up for at least 1 year were reviewed. The patients rated the efficacy of treatment on global maximum pain of nodules and abscesses on a visual analogue scale (VAS) and by self-report of the mean duration (days) of a painful lesion. RESULTS: All patients experienced a significant decrease in pain as assessed by VAS and reported a reduction in mean duration of the painful abscesses. CONCLUSIONS: Topical treatment with 15% resorcinol reduced pain from painful nodules in all patients with HS. Further trials are warranted to confirm these results.

Deroofing: A tissue-saving surgical technique for the treatment of mild to moderate hidradenitis suppurativa lesions. Zee van der HH, Prens EP, Boer J J Am Acad Dermatol. 2010 Sep;63(3):475-80 Abstract BACKGROUND: Hidradenitis suppurativa (HS) is a chronic inflammatory skin disease, often refractory to treatment. Patients with HS and dermatologists are in need of an effective, fast surgical intervention technique. Deroofing is a tissue-saving technique, whereby the “roof” of an abscess, cyst, or sinus tract is electrosurgically removed. The use of a probe is mandatory to explore the full extent of a lesion. 11
OBJECTIVE: We sought to evaluate the efficacy and patient satisfaction of the deroofing technique for recurrent Hurley I (mild) or II (moderate) graded HS lesions at fixed locations. METHODS: An open study consisted of 88 deroofed lesions in 44 consecutive patients with HS, treated by a single clinician with a follow-up time of up to 5 years. RESULTS: Fifteen of 88 (17%) treated lesions showed a recurrence after a median of 4.6 months. In all, 73 treated lesions (83%) did not show a recurrence after a median follow-up of 34 months. The median patient satisfaction with the procedure rated 8 on a scale from 0 to 10. Of the treated patients, 90% would recommend the deroofing technique to other patients with HS. One side effect occurred in the form of postoperative bleeding. LIMITATIONS: Some patients were lost to follow-up. CONCLUSIONS: The deroofing technique is an effective, simple, minimally invasive, tissue-saving surgical intervention for the treatment of mild to moderate HS lesions at fixed locations and it is suitable as an office procedure.

Samenvatting De nattende navel bij volwassenen berust vaak op een intertrigineus eczeem, psoriasis inversa of dermatomycose en is relatief eenvoudig te behandelen. In een zeer klein percentage van de gevallen is deze huidafwijking een voorbode van ernstige pathologie. In deze klinische les presenteren wij een casus van een nattende navel die bleek te berusten op een metastase van een endometriumcarcinoom. Het eponiem van deze aandoening leidt terug naar een oplettende Amerikaanse verpleegkundige uit de vorige eeuw: de Sister Mary Joseph’s nodule.

Begeleiders: J. Boer, G. Folkerts Abstract Hidradenitis suppurativa (HS) is a distressing chronic skin disorder which affects body areas rich in apocrine glands such as groins and axillae. It is a common disease with an estimated prevalence of 1% in the unselected population and 4.1% among younger adults. The aetiology is unknown, but several factors seem to play a role such as genetics, smoking and obesity. Women are affected more often than men and are younger than the general population. The diagnosis is mainly based on the patients’ history of recurrent inflammatory lesions in the inverse areas. In addition, deep seated nodules which develop from non-inflamed nodules to inflamed nodules, sinus formations and scarring with intermittent suppuration are key disease hallmarks of HS It was thought to be a disease of the apocrine glands, but recent histological findings have specified that HS is a disease of the hair follicles. More specifically, follicular dilatation and occlusion in combination with hyperkeratinization have been shown to be the early events occurring in HS. HS is related to acne due to the element of poral occlusion and visible infrainfundibular inflammation, but is different from acne in epidemiology, sebaceous gland involvement and therapeutical response. Thus, the term ‘acne inversa’ should be utilized more cautiously. It is contemplated that the pathogenic mechanisms such as the disruption of abnormal hair follicles in the inverse areas are triggered by mechanical factors. Moreover, continuous skin friction of already predisposed individuals may lead to disruption of the hair follicles and subsequent lesion formation. The clinical management of HS is tremendously challenging because no standard and effective therapy is currently at hand. However, HS has been treated with combinations of topical and systemic antibiotics, retinoids, immunosuppressives and more recently the biologics. Surgical interventions such as deroofing is still the method of choice when drugs fail to achieve the desired results. The effectiveness of a certain therapeutical option strongly depends on the patients disease severity. Frequently, HS patients are treated with a combination of the above options. Among the retinoids, isotretinoin, etretinate and acitretin have been used in the treatment of HS. Isotretinoin has been shown to be of extremely high value in the treatment of acne. However, no positive results have been noted when used in the treatment of HS. On the other hand, successful cases of HS treatment with etretinate and acitretin have been reported. Recently, Dr. Jurr Boer has treated 12 HS patients with acitretin at Deventer Hospital, The Netherlands. An overall improvement was noticed in all the patients. 13
Fysiotherapie Furthermore, the level of pain, as was assessed by VAS, was significantly reduced in all the patients. No serious side-effects were noticed. Thus, acitretin may become part of the medical arsenal of dermatologists in the therapy of HS. This should be further confirmed by conducting randomized clinical trials.

Are patient characteristics associated with the accuracy of hysterosalpingography in diagnosing tubal pathology? An individual patient data meta-analysis. Epub 2010 Dec 8 Abstract BACKGROUND: Conventional meta-analysis has estimated the sensitivity and specificity of hysterosalpingography (HSG) to be 65% and 83%. The impact of patient characteristics on the accuracy of HSG is unknown. The aim of this study was to assess by individual patient data meta-analysis whether the accuracy of HSG is associated with different patient characteristics. METHODS: We approached authors of primary studies reporting on the accuracy of HSG using findings at laparoscopy as the reference. We assessed whether patient characteristics such as female age, duration of subfertility and a clinical history without risk factors for tubal pathology were associated with the accuracy of HSG, using a random intercept logistic regression model. RESULTS: We acquired data of seven primary studies containing data of 4521 women. Pooled sensitivity and specificity of HSG were 53% and 87% for any tubal pathology and 46% and 95% for bilateral tubal pathology. In women without risk factors, the sensitivity of HSG was 38% for any tubal pathology, compared with 61% in women with risk factors (P = 0.005). For bilateral tubal pathology, these rates were 13% versus 47% (P = 0.01). For bilateral tubal pathology, the sensitivity of HSG decreased with age [factor 0.93 per year (P = 0.05)]. The specificity of HSG was very stable across all subgroups. CONCLUSIONS: The accuracy of HSG in detecting tubal pathology was similar in all subgroups, except for women without risk factors in whom sensitivity was lower, possibly due to false-positive results at laparoscopy. HSG is a useful tubal patency screening test for all infertile couples.

Patients’ attitude towards residents’ participation during gynaecological surgery. Versluis MA, Van der Linden PJ. Eur J Obstet Gynecol Reprod Biol. 2010 Dec;153(2):203-6. Epub 2010 Aug 21 Abstract OBJECTIVE: To evaluate patients’ attitude towards participation of residents in training in gynaecological surgery and patients understanding of resident position. STUDY DESIGN: An observational survey study was performed between July and February 2007-2008 in an outpatient clinic of a department of obstetrics and gynaecology of a teaching hospital. Patients planned for gynaecological surgery were concerned eligible. 247 questionnaires were handed out of which 204 were returned (response rate 82.6%). Primary outcome measure was level of confidence in a resident performing hysterectomy in various operative settings. Secondary, patients’ understanding of the medical education system was tested. RESULTS: There was no difference between confidence in a specialist performing a hysterectomy compared to an experienced resident considered competent and under supervision of a specialist. Respondents have significantly less confidence in less experienced residents operating under supervision and in all residents operating without supervision (P<0.001). From the respondents, 85.6% believes a resident in training has received less education than a resident not in training. 38.6% believes a resident received less training than a medical student. Statistical analysis was done in SPSS 16.0 (SSPS Inc., Chicago, IL, USA) using a Wilcoxon signed-rank test. CONCLUSIONS: Respondents have confidence in a resident in training performing an operation under supervision when it concerns an experienced resident that is considered competent. A majority of respondents does not understand the position of a resident in training. Patient education on resident participation should be improved and the certainty that an operating resident is competent should be emphasized.

Abstract BACKGROUND: Primary postpartum haemorrhage is an obstetrical emergency often causing acute anaemia that may require immediate red blood cell (RBC) transfusion. This anaemia results in symptoms such as fatigue, which may have major impact on the health-related quality of life. RBC transfusion is generally thought to alleviate these undesirable effects although it may cause transfusion reactions. Moreover, the postpartum haemoglobin level seems to influence fatigue only for a short period of time. At present, there are no strict transfusion criteria for this specific indication, resulting in a wide variation in postpartum policy of RBC transfusion in the Netherlands. METHODS/DESIGN: The WOMB trial is a multicentre randomised non-inferiority trial. Women with acute anaemia due to postpartum haemorrhage, 12-24 hours after delivery and not initially treated with RBC transfusion, are eligible for randomisation. Patients with severe physical complaints are excluded. Patients are randomised for either RBC transfusion or expectant management. Health related quality of life (HRQoL) will be assessed at inclusion, at three days and one, three and six weeks postpartum with three validated measures (Multi-dimensional Fatigue Inventory, ShortForm-36, EuroQol-5D). Primary outcome of the study is physical fatigue three days postpartum. Secondary outcome measures are general and mental fatigue scores and generic health related quality of life scores, the number of RBC transfusions, length of hospital stay, complications and health-care costs. The primary analysis will be by intention-to-treat. The various longitudinal scores will be evaluated using Repeated Measurements ANOVA. A costs benefit analysis will also be performed. The power calculation is based on the exclusion of a difference in means of 1.3 points or greater in favour of RBC transfusion arm regarding physical fatigue subscale. With missing data not exceeding 20%, 250 patients per arm have to be randomised (one-sided alpha = 0.025, power = 80%). DISCUSSION: This study will provide evidence for a guideline regarding RBC transfusion in the postpartum patient suffering from acute anaemia. Equivalence in fatigue score, remaining HRQoL scores and physical complications between both groups is assumed, in which case an expectant management would be preferred to minimise transfusion reactions and costs.

Abstract OBJECTIVE: To assess the economic consequences of labour induction compared with expectant monitoring in women with gestational hypertension or pre-eclampsia at term. DESIGN: An economic analysis alongside the Hypertension and Pre-eclampsia Intervention Trial At Term (HYPITAT). SETTING: Obstetric departments of six university and 32 teaching and district hospitals in the Netherlands. POPULATION: Women diagnosed with gestational hypertension or preeclampsia between 36(+0) and 41(+0) weeks of gestation, randomly allocated to either induction of labour or expectant monitoring. METHODS: A trial-based cost-effectiveness analysis was performed from a societal perspective during a 1-year time horizon. MAIN OUTCOME MEASURES: One-year costs were estimated and health outcomes were expressed as the prevalence of poor maternal outcome defined as either maternal complications or progression to severe disease. RESULTS: The average costs of induction of labour (n = 377) were 7077 versus 7908 for expectant monitoring (n = 379), with an average difference of -831 (95% CI -1561 to -144). This 11% difference predominantly originated from the antepartum period: per woman costs were 1259 for induction versus 2700 for expectant monitoring. During delivery, more costs were generated following induction (2190) compared with expectant monitoring (1210). No substantial differences were found in the postpartum, follow-up and for non-medical costs. CONCLUSION: In women with gestational hypertension or mild pre-eclampsia at term, induction of labour is less costly than expectant monitoring because of differences in resource use in the antepartum period. As the trial already demonstrated that induction of labour results in less progression to severe disease without resulting in a higher caesarean section rate, both clinical and economic consequences are in favour of induction of labour in these women. The prognostic capacity of these tests in the predicton of spontaneous pregnancy has been investigated previously by Mol et al. in 1999. This study was based on aggregated data, restricting the estimation of the prognostic value to the complete group of patients. Individual patient data (IPD) meta-analysis provides the possibility to analyse prognostic values at the individual patient level. This will result in a more individualized prognosis for subfertility patients. The aim of this study is to assess the prognostic capacity of HSG in patients with tubal pathology using IPD meta-analysis. MATERIAL AND METHODS: We approached authors of primary studies on hysterosalpingography and diagnostic laparoscopy for tubal pathology and requested their original data. The individual patient data on HSG and laparoscopy were linked with follow-up data on pregnancy outcome. The association between spontaneous pregnancy, defined as a viable pregnancy at a gestational age of 12 weeks and HSG findings was estimated by Chi square statistic. Since time to event data were not yet available, a logistic regression model was used to predict spontaneous pregnancy. Patient characteristics that influence pregnancy rates, such as age, duration and type of subfertility,
history of pelvic inflammatory disease and anovulation, were assessed and included into the model. We used a random intercept term to account for the heterogeneity between studies. RESULTS: We obtained data of seven studies reporting on 7,220 subfertile women. For 5,210 women data on ongoing pregnancy were available, for 3,640 of them HSG findings were available. 2837 (78%) patients showed no tubal pathology on HSG, 447 (12%) had a unilateral tubal pathology, and 356 (10%) had a bilateral tubal pathology. The overall ongoing pregnancy rate was 24 %. There was a non significant association between any tubal pathology as detected by HSG and ongoing pregnancy, as estimated by Chi-square statistic. The model showed a non significant prognostic value of unilateral HSG findings (OR 0.94, 95% CI 0.65-1.38) and a significant prognostic value of bilateral HSG findings (OR 0.20, 95% CI 0.06-0.63). CONCLUSION: Unilateral tubal pathology detected on HSG has limited prognostic significance for spontaneous pregnancy, whereas bilateral tubal pathology detected on HSG reduces fertility prospects considerably.

Methods: In a partly retrospective, partly prospective observational study, 70 women received remifentanil intravenously given as patient-controlled analgesia (PCA). Remifentanil was available in constant boluses (bolus doses 15-25μg depending on the women’s weight before the pregnancy) and a background infusion starting at 0,025 μg/kg/min. A lock-out time of 4 minutes was used. There was a possibility of increasing the background infusion to a maximum of 0,05 μg/kg/min when a lower dose could not relief the pain satisfactorily. Pain severity was assessed on a scale 0-3 (0=no pain, 3=severe pain) in the retrospective part of de study. In the prospective part a visual analog scale was used to asses the severity of the pain. Heelkunde of the administration were assessed by women after termination of the labour by means of a survey. Secondary goals were to investigate maternal and neonatal side effects and possible effects on the duration and termination of the labour. Results: There was a significant reduction in pain scores from baseline scores (P<0,05) in the first and second hour following the start of the remifentanil. After the first two hours it seemed like pain scores returned towards baseline, but there were too little data to make this certain. 91% of the women was satisfied with the analgesic effects given by remifentanil. More than 60% of the women had problems with timing of the bolus doses. There were mild maternal side effects. The most common side effect mentioned by women was drowsiness (in 60%). There was no evidence of side effects in the new-borns, de median apgarscore after 1 minute was 9 and after 5 minutes was 10. Conclusions: Remifentanil PCA with background infusion may especially be adequate for women who are just a few hours away from delivery. The pain relief given by remifentanil PCA may not be as good as epidural analgesia but it gives women a satisfactorily relief of pain, without severe side effects for mother and child. More investigation needs to be done to find out the most effective and safe balance between background infusion and boluses.

Abstract Cell-based therapy has been proposed as a novel strategy for patients with severe peripheral arterial disease by stimulating vascular growth. In clinical studies of this therapy, the emphasis has been on demonstrating recovery of clinical parameters, rather than on evaluation of blood flow recovery. Angiography is still the gold standard for the assessment of lower leg arteries. Therefore, we studied the usefulness of angiography in the evaluation of cell-based therapy. Sixteen patients with critical leg ischemia (ischemic rest pain or ulcers), or persistent (>12 months) profound disabling claudication were unilaterally treated with autologous bone marrow-derived mononuclear cells. Pre- and 6 months post-treatment digital subtraction angiographies (DSA) were assessed and compared in a blinded fashion twice by a panel of seven vascular surgeons and interventional radiologists. Inter- and intraobserver variability on qualitative (poor/moderate/rich) and semiquantitative (increase/no difference/decrease) assessment of collateral circulation were evaluated. Agreement was expressed inter- and intraclass correlation coefficients (CC). Inter- and intraobserver agreement was moderate for the qualitative grading of collateral extent (CC = 0.46 and 0.60, respectively). Agreement was moderate (inter-CC = 0.60) to good (intra-CC = 0.73) for comparing pre- and post-treatment DSA. Clinical response was based on limb salvage, pain-free walking distance, ankle-brachial pressure index and pain scores. No difference was observed in the extent of collateral circulation between pre- and post treatment DSA after separate analysis of clinical responding and non-responding patients (P = 0.92). DSA is not a suited modality for the evaluation of therapeutic angiogenesis.

resection is more difficult for diverticular disease as compared with malignancy. For this reason, starting laparoscopic surgeons might avoid diverticulitis, making the implementation phase unnecessary long. The aim of this study was to determine whether laparoscopic resection for diverticular disease should be included during the implementation phase. METHODS: All consecutive patients who underwent an elective laparoscopic recto-sigmoid resection in our hospital for diverticulitis or cancer from 2003 to 2007 were analysed. RESULTS: A total of 256 consecutive patients were included in this prospective cohort study. One hundred and fifty-one patients were operated on for diverticulitis and 105 for cancer. There was no significant difference in operation time (168 vs. 172 min), blood loss (189 vs. 208 ml), conversion rates (9.9% vs. 11.4%), hospital stay (8 vs. 8 days), total number of peroperative (2.3% vs. 1.6%) or postoperative complications (21.9% vs. 26.9%). The occurrence of anastomotic leakages was associated with higher American Society of Anesthesiologists (ASA) classification, which differed between the groups (86.8% vs. 64.8% ASA I-II, p < 0.001). CONCLUSION: Since there are no differences in operation time, blood loss, conversion rate and total complications, there is no need to avoid laparoscopic recto-sigmoid resection for diverticular disease early in the learning curve.

PATIENTS AND METHODS: Internal fixation with the DLBP was performed in 25 consecutive patients with an intracapsular hip fracture within 24 h from admission. Failure of fixation, due to non-union, avascular necrosis, implant failure or secondary displacement of the fracture, was the primary outcome measurer. Functional outcome was assessed by the Harris Hip Score. RESULTS: Following internal fixation of intracapsular hip fractures with the DLBP, a failure rate of 2 out of 25 patients and excellent functional results were seen after a follow-up of more than 2 years. CONCLUSION: The initial clinical results of the DLBP are promising and justify the start of a randomised controlled trial.

Abstract AIM: To evaluate the development of diagnostic tools, indications for surgery and treatment modalities concerning diverticular disease (DD) in The Netherlands. METHODS: Data were collected from 100 patients who underwent surgery for DD in three Dutch hospitals. All hospitals used the same standardized database. The collected data included patient demographics, patient history, type of surgery and complications. Patients were divided into two groups, one undergoing elective surgery (elective group) and the other undergoing acute surgery (acute group). RESULTS: Two hundred and ninety-nine patients were admitted between 2000 and 2007. One hundred and seventy-eight patients underwent acute surgery and 121 patients received elective operations. The median age of the 121 patients was 69 years (range: 28-94 years), significantly higher in acute patients (P = 0.010). Laparoscopic resection was performed in 31% of elective patients. In the acute setting, 61% underwent a Hartmann procedure. The overall morbidity and mortality were 51% and 10%, and 60% and 16% in the acute group, which were significantly higher than in the elective group (36% and 1%). Only 35% of the temporary ostomies were restored. CONCLUSION: This study gives a picture of current surgical practice for DD in The Netherlands. New developments are implemented in daily practice, resulting in acceptable morbidity and mortality rates

Primary hemiarthroplasty versus conservative treatment for comminuted fractures of the proximal humerus in the elderly (ProCon): a multicenter randomized controlled trial. Abstract BACKGROUND: Fractures of the proximal humerus are associated with a profound temporary and sometimes permanent, impairment of function and quality of life. The treatment of comminuted fractures of the proximal humerus like selected three-or four-part fractures and split fractures of the humeral head is a demanding and unresolved problem, especially in the elderly. Locking plates appear to offer improved fixation; however, screw cut-out rates ranges due to fracture collapse are high. As this may lead to higher rates of revision surgery, it may be preferable to treat comminuted fractures in the elderly primarily with a prosthesis or non-operatively. Results from case series and a small-sample randomized controlled trial (RCT) suggest improved function and less pain after primary hemiarthroplasty (HA); however these studies had some limitations and a RCT is needed. The primary aim of this study is to compare the Constant scores (reflecting functional outcome and pain) at one year after primary HA versus non-operative treatment in elderly patients who sustained a comminuted proximal humeral fracture. Secondary aims include effects on functional outcome, pain, complications, quality of life, and cost-effectiveness. METHODS/DESIGN: A prospective, multi-center RCT will be conducted in nine centers in the Netherlands and Belgium. Eighty patients over 65 years of age, who have sustained a three-or four part, or split head proximal humeral fracture will be randomized between primary hemiarthroplasty and conservative treatment. The primary outcome is the Constant score, which indicates pain and function. Secondary outcomes include the Disability of the Arm and Shoulder (DASH) score, Visual Analogue Scale (VAS) for pain, radiographic healing, health-related quality of life (Short-form-36, EuroQol-5D) and healthcare consumption. Cost-effectiveness ratios will be determined for both trial arms. Outcome will be monitored at regular intervals over the subsequent 24 months (1, 3 and 6 weeks, and 3, 6, 12, 18, and 24 months). Data will be analyzed on an intention to treat basis, using univariate and multivariable analyses. DISCUSSION: This trial will provide level-1 evidence on the effectiveness of the 26

A comparison of the Doppler-derived maximal systolic acceleration versus the ankle-brachial pressure index or detecting and quantifying peripheral arterial occlusive disease in diabetic patients. Abstract AIM: The aim of this study was to assess the diagnostic accuracy of the Doppler derived maximal systolic acceleration (ACCmax) as a novel technique for evaluating peripheral arterial occlusive disease (PAOD) in patients with diabetes mellitus, who are known for a falsely elevated ankle-brachial index (ABI). METHODS: In this retrospective analysis ACCmax was measured at ankle level in a series of 163 consecutive patients referred to the vascular laboratory for initial assessment of PAOD. Patients were classified according to the presence or absence of diabetes. In the non-diabetic patients PAOD was defined as ABI < or =0.90. This group was used to establish the association between ACCmax and ABI in a linear regression model. The result was then used to predict the presence or absence of PAOD in the diabetic patients. RESULTS: The authors examined 301 lower limbs. The study group consisted of 166 limbs of patients without diabetes and 135 limbs of patients with diabetes. PAOD was present in 52% of limbs in the nondiabetic group versus 59% of limbs in the diabetic group (ABI < or =0.90, or in case of non-compliant vessels toe-brachial index (TBI) < or =0.70). An ACCmax cut-off value of >10 m/s2 was found to be highly predictive for the exclusion of PAOD (negative predictive value 95%). In addition, the ACCmax cut-off value of <6.5 m/s2 was highly predictive for the detection of PAOD (positive predictive value 99%). A strong quadratic association was found between ACCmax and ABI in the non-diabetic group (R2=0.85). In the diabetic patients R2 values were 0.81 and 0.79 after ABI and TBI measurement respectively. CONCLUSION: DUS-derived ACCmax is an accurate marker that could offer significant benefits for the diagnosis of PAOD, especially in diabetic patients. 27
Results of 244 consecutive patients with hemorrhoids treated with Doppler-guided hemorrhoidal artery ligation. Abstract AIM: This study was designed to determine the effect of treating hemorrhoids with the Doppler-guided hemorrhoidal artery ligation (DG-HAL) procedure. METHODS: From June 2005 to March 2008, 244 consecutive hemorrhoidal patients underwent hemorroidal artery ligation performed with the DG-HAL system from AMI. All patients were evaluated postoperatively with a proctologic examination and interview. Further follow-up was performed by telephone with a standardized questionnaire. When indicated, patients revisited the clinic for further examination and treatment. RESULTS: 244 patients were treated with DG-HAL. The mean follow-up time was 18.4 months (range 1.4-37.2). Sixty-seven percent of the patients had an improvement of symptoms after one treatment. Fifty-three patients (22%) underwent a second procedure because of persisting symptoms. Thirteen patients (25%) underwent a second DG-HAL and 40 (75%) underwent rubber band ligation. In total, 69% of the patients had a good response using the DG-HAL technique. Multivariate logistic regression analysis revealed prolapse to be an independent risk factor for persistent symptoms (OR = 2.38, 95% CI 1.10-5.15). Patients with grades 3 and 4 hemorrhoids had a higher risk of developing recurrent disease (OR = 4.94, 95% CI 0.67-36.42). CONCLUSION: DG-HAL seems to be an effective procedure for treating low-grade hemorrhoids. A resection procedure should be the treatment for patients with recurrent disease.

New surgical techniques and effective neoadjuvant treatment regimens have significantly improved colorectal cancer outcome. Nevertheless, there seem to 28
be substantial differences in quality of care between European countries, hospitals and doctors. To reduce hospital variation, most initiatives aim on selective referral, encouraging patients to seek care in high-volume hospitals, where cancer care is concentrated to site-specialist multidisciplinary teams. As an alternative to volume-based referral, hospitals and surgeons can also improve their results by learning from their own outcome statistics and those from colleagues treating a similar patient group. European national audit registries in surgical oncology have led to improvements with a greater impact on survival than any of the adjuvant therapies currently under study. Moreover, they offer the possibility to perform research on patient groups that are usually excluded from clinical trials. Nevertheless, between European countries remain differences in outcome and treatment schedules that cannot be easily explained. The European CanCer Organisation (ECCO) has recognised these importances and created the ‘European Registration of Cancer Care’ (EURECCA) framework to develop a European colorectal audit structure. EURECCA will advance future treatment improvements and spread these to all European cancer patients. It provides opportunities to treat elderly and comorbid patients evidence based while it offers an unique insight in socialeconomical healthcare matters such as the consequences of commercialisation, treatment availability and screening initiatives. As such, ECCO has established the basis for a strong, multidisciplinary audit structure with the commitment to improve cancer care for every European cancer patient.

Simple dislocations are characterized by the absence of fractures, while complex dislocations are associated with fractures. After reduction of a simple dislocation, treatment options include immobilization in a static plaster for different periods of time or so-called functional treatment. Functional treatment is characterized by early active motion within the limits of pain with or without the use of a sling or hinged brace. Theoretically, functional treatment should prevent stiffness without introducing increased joint instability. The primary aim of this randomized controlled trial is to compare early functional treatment versus plaster immobilization following simple dislocations of the elbow. METHODS/DESIGN: The design of the study will be a multicenter randomized controlled trial of 100 patients who have sustained a simple elbow dislocation. After reduction of the dislocation, patients are randomized between a pressure bandage for 5-7 days and early functional treatment or a plaster in 90 degrees flexion, neutral position for pro-supination for a period of three weeks. In the functional group, treatment is started with early active motion within the limits of pain. Function, pain, and radiographic recovery will be evaluated at regular intervals over the subsequent 12 months. The primary outcome measure is the Quick Disabilities of the Arm, Shoulder, and Hand score. The secondary outcome measures are the Mayo Elbow Performance Index, Oxford elbow score, pain level at both sides, range of motion of the elbow joint at both sides, rate of secondary interventions and complication rates in both groups (secondary dislocation, instability, relaxation), healthrelated quality of life (Short-Form 36 and EuroQol-5D), radiographic appearance of the elbow joint (degenerative changes and heterotopic ossifications), costs, and cost-effectiveness. DISCUSSION: The successful completion of this trial will provide evidence on the effectiveness of a functional treatment for the management of simple elbow dislocations. TRIAL REGISTRATION: The trial is registered at the Netherlands Trial Register (NTR2025).

Veel voorkomende klachten zijn bloedverlies, jeuk, hygiëneproblemen en soiling. Een vezelrijk dieet, eventueel met vezelsuppletie, verhelpt vaak de klachten. Bij onvoldoende resultaat is de volgende stap rubberbandligering: een veilige en eenvoudige behandeling. Op lange termijn is het resultaat van rubberbandligering vaak onvoldoende. Vroeger was dan hemorroïdectomie het enige alternatief. De hemorroïdectomie kan gepaard gaan met ernstige complicaties, zoals invaliderende pijn en incontinentie. Hemorroïdectomie werd daarom in de CBO-consensus van 1994 als obsoleet beschouwd. Tegenwoordig zijn nieuwe operatietechnieken beschikbaar: dopplergeleide ligering van hemorroïdale arteriën (DG-HAL) en geniete anopexie. Beide technieken zijn veilig en de resultaten zijn goed. Bovendien veroorzaken deze technieken weinig postoperatieve pijn. DG-HAL en geniete anopexie bieden een alternatief aan patiënten die niet uitkomen met vezelsuppletie en rubberbandligering.

Tollenaar Datum uitgave: april 2010 Samenvatting Er stierven vorig jaar driehonderd patiënten na een darmkankeroperatie. Dat zijn vierhonderd overlijdensgevallen minder dan er voorheen geregistreerd werden. Dit is één van de opvallende uitkomsten van de DSCA Jaarrapportage 2009 van de Dutch Surgical Colorectal Audit (DSCA). Aan dit onderzoek deden alle 84 Nederlandse ziekenhuizen mee. DSCA De DSCA is een uniek registratiesysteem dat vorig jaar is opgezet door
darmkankerchirurgen, samen met de betrokken chirurgische beroepsverenigingen. Het heeft als doel informatie te vergaren die aanknopingspunten geeft om de kwaliteit van de zorg te verbeteren. Behalve de nieuwe gegevens over overlijdensgevallen na operaties, kwam naar voren dat darmkankerpatiënten in ons land gemiddeld twaalf dagen in het ziekenhuis liggen. Dat is meer dan artsen dachten. Deze meetmethode kan in alle andere zorgdomeinen worden toegepast, opdat ook daar een scherper beeld van de prestaties kan ontstaan. Operatierisico Na darmkankeroperaties treden regelmatig complicaties op die soms leiden tot het overlijden van de patiënt. Uit de audit-resultaten is nog niet te concluderen of die complicaties deels te vermijden zijn. De sterfte na darmkankeroperaties in Nederland is circa 4 procent. Op basis van de huidige gegevens is er geen enkel statistisch bewijs dat er ziekenhuizen zijn met een overmatig sterfterisico. Wel blijkt dat oudere patiënten, een groep die de komende jaren zal toenemen, een sterk verhoogd risico op overlijden hebben ten opzichte van jongere patiënten. Er is wellicht ruimte voor verbetering in de zorg voor deze kwetsbare groep patiënten. Leeftijd Van de acuut geopereerde darmkankerpatiënten die ouder zijn dan tachtig jaar overlijdt in Nederland 20 procent na de operatie. Alle patiënten met een colorectaal carcinoom die acuut geopereerd moeten worden, lopen een hoger risico op complicaties, op het niet volledig verwijderen van de tumor en op langere ziekenhuisopname. Een deel van deze patiënten was al onder behandeling voordat zij acuut moesten worden geopereerd. Zij zouden baat kunnen hebben bij een versneld preoperatief traject, om te voorkomen dat zij in een acute situatie terecht komen. Dit zou kunnen leiden tot een vermindering van het aantal acute operaties en dus tot minder sterfte. Ook het identificeren van ziekenhuizen die goed scoren in de opvang van acute patiënten met een colorectaal carcinoom, kan leiden tot een lager risico voor alle patiënten die in deze situatie terecht komen. Kijkoperaties In Nederland worden vaker dan in andere landen darmtumoren via een kijkoperatie (laparoscopisch) verwijderd. De patiënten die laparoscopisch geopereerd zijn overlijden minder vaak na de operatie, hebben minder vaak complicaties en moeten minder vaak opnieuw geopereerd worden. In het kader van de recente discussie over de veiligheid van kijkoperaties in Nederland, is dit een zeer positieve bevinding. Toch zijn er ook voor deze groep patiënten nog verbeteringen te bewerkstelligen, vooral wanneer een kijkoperatie alsnog moet worden geconverteerd naar een open.
Toekomst DSCA De systematiek waarmee de DSCA gegevens verzamelt, opslaat en analyseert is generiek van opzet, waardoor deze vorm van medical auditing eenvoudig toepasbaar is op andere zorgdomeinen. Dat betekent dat ook andere audits gebruik zouden kunnen maken van de door de DSCA ontwikkelde en beheerde ICT-infrastructuur. De methode van de DSCA biedt niet alleen een analyse op jaarbasis aan de deelnemende ziekenhuizen, maar ook tussentijds een wekelijkse terugkoppeling van hun eigen resultaten in combinatie met de actuele landelijke cijfers. Kwaliteitsverbetering De metingen en terugkoppelingen maken regelmatig benchmarken mogelijk, hetgeen aangetoond een sterk kwaliteitsbevorderend instrument is. Bij uitbreiding van deze audit-structuur naar andere zorgdomeinen zal de focus moeten liggen op aandoeningen die vaak voorkomen, waar verbeteringen te behalen zijn en waarvoor wetenschappelijk onderbouwde richtlijnen beschikbaar zijn. Zulke door de beroepsgroep gedragen richtlijnen zijn noodzakelijk om een zinvolle set van te verzamelen gegevens (gevalideerde dataset) te kunnen bepalen. Een bijkomend voordeel is dat door middel van de audit de richtlijn verder getoetst kan worden.

Promotie Experimental strategies in restenosis and critical limb ischemia R.B.M. van Tongeren Leiden, Rijksuniversiteit Leiden, 22 april 2010 Samenvatting Radiotherapie bleek een hype, voor stamcellen is er hoop De studies in het eerste deel concentreren zich op de mogelijkheid om door middel van radiotherapie (re)stenose vorming, veroorzaakt door intima hyperplasie en ‘remodeling’, tegen te gaan. Dit werd onderzocht na PTA in het femoropopliteale traject bij patiënten met perifeer vaatlijden en bij patiënten bij wie een kunststof arterio-veneuze (AV) fistel ten behoeve van hemodialyse werd aangelegd. Het tweede deel van dit proefschrift focust op de experimentele behandeling met “stamcellen” (celtherapie) van patiënten met ernstig perifeer vaatlijden bij wie een reguliere chirurgische of endovasculaire behandeloptie niet mogelijk is. Straling ter preventie van restenose: de verdwijning van de vasculaire radiotherapie In een studie bij 60 patiënten kon na randomisatie geen verschil worden aangetoond tussen PTA plus aanvullende endovasculaire brachytherapie (EBT) of door middel van alleen PTA. Rekening houdend met het type laesie resulteerde een dosis van 14 Gray op de vaatwand gegeven (Iridium192) in een absolute risico reductie van slechts 9% bij patiënten met een stenose, maar in geval van occlusie bedroeg de reductie 32%. Een substantieel aantal patiënten met laag-complexe, de-novo laesies lijkt verklarend voor het beperkte effect. Inmiddels is gebleken dat radiotherapie met name effectief is bij recidief stenosen en langere, complexe afwijkingen. Voorts werd het effect van uitwendige bestraling (EBI) op de doorgankelijk heid van kunststof dialyse fistels onderzocht (AVF trial). De veneuze anastomose werd bij 50 patiënten door middel van loting al dan niet aanvullend bestraald (18 Gy in 2 fracties). Hoewel er geen stralingsgerelateer de bijwerkingen werden waargenomen na 12 maanden follow up, resulteerde de bestraling niet in minder stenosen of reïnterventies. In het veld van de vasculaire radiotherapie heeft een significante ommezwaai plaatsgevonden. Na ongeveer een decade van gestage groei, met name 35
binnen de interventie-cardioologie, volgde een nogal abrupt verval rond 2005. Dit werd veroorzaakt door de klaarblijkelijke beperkingen van EBT zoals hierboven genoemd (alleen effectief bij recidief stenosen) en de zeer bewerkelijke aard van de procedure. De gebleken bescheiden lange-termijn resultaten in follow-up studies vormden de genadeslag. Binnen de interventiecadiologie werd EBT in een kort tijdsbestek werd het veld overgenomen door de drug-eluting stents. Celtherapie bij perifeer vaatlijden Het concept van celtherapie kwam voort uit de gedachte dat het toedienen van een cocktail van cytokines, groeifactoren en cellen aanleiding geeft tot een meer fysiologische en effectieve methode om tot vaatgroei te komen in ischemisch weefsel. In de meeste studies is gebruik gemaakt van mononucleaire cellen uit het beenmerg. Bij 30 patiënten bij wie reguliere behandeling door middel van bypass chirurgie of endovasculaire interventie was uitgesloten, werd een 40 ml concentraat autologe beenmergcellen (BMCs) toegediend. Negen van hen ondergingen binnen 3 maanden amputatie van het been vanwege aanhoudende ischemie. In de overige patiënten resulteerde de behandeling in een significante en aanhoudende (>12 maanden) verbetering. Na 6 maanden was de pijnvrije loopafstand verdrievoudigd. De gemiddelde enkel-arm index steeg 23% en pijnscores lieten een reductie van 50% zien. Hoewel deze benadering technisch goed mogelijk is gebleken in dierexperimentele- en enkele klinische studies, weten we nog relatief weinig van het werkingsmechanisme en de klinische effectiviteit. Veel is nog gebaseerd op hypothesen. De huidige topics zijn als volgt:
Het ideale celtype Samenvattend zou de ideale celpopulatie om vaatgroei te stimuleren: - moeten kunnen uitgroeien tot rijpe stadia van verschillende weefseltypen en regenereren zonder hun potentie tot differentiatie te verliezen. - bij voorkeur collateralen met een sufficiënte diameter en daardoor een lage weerstand kunnen formeren. Capillairen daar en tegen hebben een hoge weerstand en zijn niet geschikt om de doorbloeding significant te verbeteren. - van autologe origine zijn en in grote aantallen eenvoudig beschikbaar zijn. Ideale toedieningsweg Op dit moment bestaat er geen optimale strategie voor de toediening van de cellen; intramusculair, rechtstreeks in de arterie, of een combinatie van beide? Een belangrijk aspect is het dirigeren van de cellen naar het juiste weefsel (“homing”). Onderzoek naar de verdeling van de geïnjecteerde beenmergcellen in de verschillende weefsels toonde dat maar een paar
procent getraceerd kon worden in het ischemische weefsel. Bij diermodellen werd het overgrote deel van de cellenteruggevonden in de lever en de milt. Toekomstige research zal zich ook moeten richten op strategieën om tot een verbeterde homing te komen. Ideale celfunctie Patiënt factoren als leeftijd, diabetes en hypercholesterolemie zijn geassocieerd met een verstoorde respons van het lichaam op ischemie en met een verminderd aantal circulerende stamcellen. Verschillende benaderingen van therapeutische angiogenese zullen wellicht gecombineerd worden. Genetische modulatie van stamcellen zal overexpressie van groeifactoren kunnen geven en daarmee de signaalactiviteit bevorderen, of de levensduur van de getransplanteerde cellen verbeteren. Mechanisme Gezien de delings- en regeneratiemogelijkheden van beenmergcellen is het verleidelijk te suggereren dat celtherapie neovascularisatie stimuleert door directe incorporatie in de vaatwand. Echter, de data over transdifferentiatie van deze cellen zijn controversieel. Anderen trekken deze theorie in twijfel en leveren aannemelijk bewijs voor de hypothese dat beenmergcellen niet of nauwelijks in de vaatwand incorporeren. Daar veel celpopulaties uit het beenmerg een bron zijn van groeifactoren, cytokines en chemokines is een complementaire hypothese dat deze cellen een meer ondersteunende rol spelen. Momenteel is de algemene gedachte dat het potentiële gunstige effect meer is gerelateerd aan een paracrien effect dan aan transdifferentiatie. Mogelijke ongewenste effecten Stamcellen theoretisch tumor vascularisatie stimuleren. Op dezelfde wijze kan retinopathie en bestaande atherosclerose bevorderd worden. Aan de andere kant bestaat er een groeiende hoeveelheid bewijs dat circulerende stamcellen een belangrijke rol spelen bij de regeneratie van endotheel en het verminderen van de atherosclerotische plaque, meer dan bij het bevorderen van atherosclerose. Het uiteindelijke succes van celtherapie zal echter afhangen van het aantonen van klinische effectiviteit, veel meer dan van het volledig ontrafelen van een complex mechanisme. Uiteraard is identificatie van de werkzame (cel)componenten van belang, evenals de vraag of het effect direct wordt bewerkstelligd door de getransplanteerde cellen of indirect door de betrokkenheid van genoemde factoren. Zo kan doelgerichte toediening van essentiële componenten mogelijk worden. Dit is waarschijnlijk een kritieke stap in de verdere realisatie van deze experimentele benadering. Uiteraard veelbelovend, maar de nog vele onbeantwoorde vragen maken dat daadwerkelijke klinische toepassing van celtherapie wellicht nog even op zich laat wachten.

The first case describes a patient who suffered fatal bleeding after percutaneous dilatational tracheostomy due to a tracheoinnominate fistula. In this patient the percutaneous dilatational tracheostomy was sited too low down. The second case describes a patient who died following venous bleeding after percutaneous dilatational tracheostomy. This patient had had previous neck surgery. We use these two cases to focus attention on two pitfalls of percutaneous dilatational tracheostomy placement in ICU patients, namely: low tracheostomy placement may lead to tracheoinnominate fistula, and patients with previous neck surgery may have aberrant vasculature.

Abstract BACKGROUND: Guidelines have set goals for risk factor management in chronic kidney disease (CKD) patients. These goals are often not met. In this analysis, we set out to assess the quality of risk factor management in CKD and to identify factors that determine the quality of care (QoC). For that purpose, baseline data of the MASTERPLAN (Multifactorial Approach and Superior Treatment Efficacy in Renal Patients with the Aid of Nurse practitioners) study have been used. MASTERPLAN is a multicentre study which evaluates the effect of a multifactorial intervention in prevalent CKD patients on cardiovascular (CV) events and progression of kidney failure. METHODS: QoC was quantified using a score based on the number of 11 defined treatment goals on target. The maximum score per patient was 11. RESULTS: The average (±SD) QoC score was 6.7 (±1.5). The average score per centre ranged from 5.9 to 6.9. In a multivariable analysis, centre proved to be a significant, independent determinant of QoC with a difference up to 0.7 between centres. This difference remained when adjustments were made for those risk factors primarily treated by pharmacotherapy. Other factors that were significantly related to the QoC were estimated glomerular filtration rate, 39
Caucasian race, diabetes mellitus, diabetic nephropathy as cause of kidney disease and previous kidney transplantation. CONCLUSIONS: In CKD patients, risk factors for progression of kidney failure and CV events were inadequately controlled. Treatment centre proved to be an important determinant of QoC. This data may point towards the physician’s interest and preference as important determinants of QoC. VALIDERING TCPO2 METING Validering van de transcutane zuurstofmeting bij patiënten met diabetes en een voetulcus: is het betrouwbaar? Naam: A.H. Bloemendal Begeleider: M.N. Gerding Disciplinegroep: Interne geneeskunde, Deventer Ziekenhuis Periode: 5 oktober 2009 - 19 februari 2010 Achtergrond: Bij de ontwikkeling van een diabetisch voetulcus spelen verschillende mechanismen een rol, waaronder neuropathie, microangiopathie en macroangiopathie (perifeer arterieel vaatlijden). Perifeer arterieel vaatlijden (PAV) is tevens een belangrijke voorspeller van de genezingkans van een ulcus. Het opsporen van PAV wordt op de diabetische voetulcera polikliniek van het Deventer Ziekenhuis onder andere gedaan met
behulp van de transcutane zuurstofmeting (TcpO2 meting), zoals wordt aanbevolen in de huidige NIV richtlijn. De betrouwbaarheid - intra- en interobserver variabiliteit - van deze meting is echter nauwelijks onderzocht. Studiedoel: Het hoofddoel van deze studie was het vaststellen van de betrouwbaarheid - intra- en interobserver variabiliteit - van de TcpO2 meting bij patiënten met een diabetisch voetulcus. Het eerste nevendoel van de studie was het genereren van referentiewaarden in een groep patiënten met diabetes mellitus zonder een voetulcus naar leeftijd en geslacht. Het tweede nevendoel was het vaststellen hoe vaak een TcpO2 < 50 mmHg respectievelijk < 30 mmHg voorkomt in een groep hemodialysepatiënten van het Deventer Ziekenhuis. Methode: Bij 25 patiënten (17 mannen en 8 vrouwen, mediane leeftijd 74 jaar (range 50-85)) met een diabetisch voetulcus werd driemaal de TcpO2 waarde gemeten; één meting werd uitgevoerd door een podotherapeut en twee metingen werden uigevoerd door een geneeskundestudent. Intra- en interobserver variabiliteit werden onderzocht door het berekenen van de intraclass correlation coefficient (ICC) en werden grafisch weergegeven met behulp van Bland-Altman plots. Voor het genereren van referentiewaarden werden 43 patiënten met diabetes mellitus onder behandeling van een pedicure geïncludeerd en geclassificeerd in zes subgroepen, gestratificeerd per decade naar geslacht en leeftijd. De TcpO2 waarden in de subgroepen werden met elkaar vergeleken met behulp van een Kruskal-Wallis toets. In een representatieve steekproef van 30 consecutieve hemodialysepatiënten (20 mannen en 10 vrouwen, mediane leeftijd 65 jaar (range 22-84)) werd aan beide voeten de TcpO2 gemeten. Resultaten: Analyses van de intraobserver variabiliteit lieten een ICC van 0.90 (95% betrouwbaarheidsinterval 0.79-0.85) zien. Analyses van de interobserver variabiliteit werden twee keer uitgevoerd en lieten een ICC van respectievelijk 0.81 (95% betrouwbaarheidsinterval 0.62-0.91) en 0.88 (95% betrouwbaarheidsinterval 0.74-0.94) zien. Bland-Altman plots lieten zien dat de observer variabiliteit gelijk verdeeld was. Het 95% betrouwbaarheidsinterval van het intraobserver verschil was -3,5-13,7 en het 95% betrouwbaarheidsinterval van het interobserver verschil was respectievelijk -4,6-19,1 en 2,3-15,3. De KruskalWallis analyse liet geen significant verschil zien tussen de verschillende groepen diabetespatiënten zonder een voetulcus (Chi-Square = 3.18, p = 0.673). Bij 57% van de hemodialysepatiënten werd een TcpO2 kleiner dan 50 mmHg gemeten en bij 20% eenTcpO2 kleiner dan 30 mmHg. Conclusie: Zowel de intraobserver overeenstemming als de interobserver overeenstemming is erg goed. Bland-Altman plots laten daarentegen een spreiding van 18 mmHg zien voor het intraobserver verschil en een spreiding van bijna 24 mmHg voor het interobserver verschil. Hier moet rekening mee

Bij een aanzienlijk deel van de hemodialysepatiënten werd een lage TcpO2 gevonden, waardoor het mogelijk zinvol is in een asymptomatisch stadium reeds preventieve maatregelen bij deze patiënten te treffen, om zo amputaties in de toekomst te voorkomen. Verder onderzoek hiernaar is noodzakelijk.

An accurate method for assessment of tumor response and unambiguous guidelines are needed to improve management of patients undergoing neoadjuvant chemotherapy. Aim of this study was to determine the method of response measurement that is best predicting pathological outcome, second to analyse predictive value of preoperative MRI for presence and absence of tumor at microscopy, and finally to determine clinical value of automatic analysis by CADstream. Materials and methods. In 35 patients with locally advanced breast cancer, dynamic contrast-enhanced MRI was performed before chemotherapy, after half of cycles chemotherapy and before surgery. Tumorsize is measured using RECIST, tumor cross-section area, tumor volume (at initial and late enhancement) and by RECIST on Gadolineum-tumor washout. Response was defined as percentage decrease of tumor size of second MRI in comparison with first MRI. Influences of age, baseline tumor size and histology for pathological outcome were determined. Characteristics of measurable tumor at third MRI were correlated with those at pathology. Finally, manually measured volumes of tumors were correlated with those measured by CADstream. Results. RECIST at initial enhancement (p=0,014) and washout (p=0,006) are best methods for predicting pathological outcome. Pathological outcome is not influenced by tumor size, age and tumor histology. In our study population, positive predictive value of washout is 100% for residual tumor at pathology, equal to negative predictive value of late enhancement. Finally, only a weak association was shown between volume measurements by CADstream and those performed manually. Conclusion. RECIST at initial enhancement or washout should be the standard 42

Abstract BACKGROUND: This study aimed to evaluate a single institute’s experience with resection of metachronous pulmonary malignancy in patients treated for squamous cell carcinoma of the head and neck. METHODS: Sixty-three consecutive patients treated curatively for head and neck squamous cell carcinoma underwent surgical resection of malignant lung lesions. Survival was estimated and potential prognostic factors investigated. RESULTS: The median overall survival for the total group was 22.2 months. Fifty-one patients (81 per cent) had one lung lesion, while the remainder had multiple lesions (range, two to seven). In the 63 patients, 35 lobectomies, 4 pneumonectomies and 24 wedge resections were performed. For patients with pulmonary squamous cell carcinoma (n = 52), the three-year survival rate was 35 per cent (95 per cent confidence interval, 22-48); for patients with resected adenocarcinoma (n = 10), it was 50 per cent (95 per cent confidence interval, 18-75). The overall five-year survival rate was 30 per cent (95 per cent confidence interval, 19-42). CONCLUSION: In patients treated curatively for head and neck squamous cell carcinoma, resection of secondary pulmonary cancer is associated with favourable long term overall survival, especially for patients with adenocarcinoma lesions.
longcarcinoom of longmetastase. Van alle tweede primaire carcinomen bij deze patientgroep komt 23% tot 30% voor in de long. Longmetastasen omvatten 66% van alle metastasen op afstand van HNSCC met een incidentie variërend van 1,6% tot 25% afhankelijk van het tumorstadium. Voor patiënten met een larynxcarcinoom in de voorgeschiedenis is het risico op een tweede primaire longkanker het hoogst. Lange tijd werden conventionele thoraxfoto’s gebruikt als standaardprocedure om tweede longcarcinomen op te sporen, maar de sensitiviteit van deze beeldvorming blijft laag (35%). Hoewel CTscanonderzoek sensitiever is (80%), heeft deze methode een lage specificiteit, door detectie van veel niet maligne laesies. 18F-FDG PET/CT is nog sensitiever, maar de specificiteit is wederom relatief laag. Ondanks de technische vooruitgang in beeldvormingstechnieken, is de meerwaarde van screening op secundaire longkanker bij HNSCC patiënten nog steeds niet wetenschappelijk aangetoond. Een belangrijk onderdeel van de discussie om wel of niet te screenen, naast het vraagstuk van de meerwaarde van vroegdetectie voor overleving, is het risico op overdiagnosticering. Ook een betrouwbare differentiatie tussen tweede primaire longkanker en metastase lijkt van belang. Daarnaast wordt vaak verondersteld dat radiologische screening voor patiënten te belastend zou zijn.

Geurts Amsterdam, Universiteit van Amsterdam, 2010 Samenvatting Patiënten die een plaveiselcelcarcinoom in het hoofd-halsgebied (HNSCC) overleven, hebben een hoog risico op het ontwikkelen van een tweede primair
Het proefschrift probeert antwoord te vinden op de voorgaande vragen. Uit ons onderzoek is gebleken dat patiënten tijdens de follow-up geen psychologische problemen ondervinden van de radiologische screening. Daarnaast is na bestudering van de literatuur en verwerking van Nederlandse epidemiologische gegevens in dit proefschrift vastgesteld dat een prospectieve gerandomiseerde klinische trial om de meerwaarde van screening wetenschappelijk aan te tonen met de huidige beschikbare beeldvormingstechnieken als nagenoeg onhaalbaar moet worden ingeschat. Tevens lijkt het van groot belang om een betrouwbaar onderscheid te maken tussen een tweede primaire longkanker of metastase. Dit maakt de ontwikkeling van specifiekere diagnostische methoden noodzakelijk, die idealiter preoperatief worden uitgevoerd met minimaal invasieve procedures. Het proefschrift beschrijft verder de ontwikkeling van een moleculaire methode, waarbij gebruik wordt gemaakt van LOH analyse (Loss of Heterozygosity) en TP53 mutatieanalyse, om longmetastasen en tweede primaire longcarcinomen van elkaar te onderscheiden. Uitkomsten van dit onderzoek zijn in een algoritme verwerkt, waarmee een betrouwbaar onderscheid gemaakt kan worden tussen tweede primaire longkanker en metastase. Hierdoor is komen vast te staan dat er in tegenstelling tot eerdere klinische interpretaties, in ca 50% van de gevallen sprake is van een tweede primaire longtumor in plaats van een veronderstelde metastase. Kindergeneeskunde was dat de overlevingspercentages van moleculair gedefinieerde longmetastasen identiek waren aan die van tweede primaire longtumoren. Verder bleek dat in geselecteerde patiënten, die eerder curatief behandeld zijn voor HNSCC, resectie van secundaire longkanker geassocieerd is met gunstige overleving (5-jaars overleving 30% (95%; betrouwbaarheidsinterval 19-42)), in het bijzonder bij adenocarcinomen.

Klinisch redeneren in de praktijk: Een jongen met koorts, hoofdpijn, braken en verwardheid. Sybrich Tiemersma, Jeroen Mourmans, Hans van der Deure Praktische pediatrie 2010;4(2):126-31 Samenvatting In dit artikel wordt een jongen van 12 jaar met koorts, hoofdpijn, braken en afasie gepresenteerd. Liquoronderzoek toonde een verhoogd leukocyten- en erytrocytengetal, een verhoogde totaal eiwitconcentratie en een normaal glucose. MRI-onderzoek liet frontotemporaal typische afwijkingen zien. Met een positieve PCR op herpes simplex virus type 1 in de liquor werd de diagnose herpes simplex encefalitis gesteld. De aanknopingspunten om deze relatief zeldzame diagnose te stellen worden beschreven. Het is van belang zo snel mogelijk de behandeling te starten met aciclovir.
Wetenschappelijk stages Incontinentie bij jonge kinderen (3 t/m 6 jaar). Een retrospectieve evaluatie van diagnosen en behandeling. Naam: N. Sijtema Begeleider: J. van der Deure Locatie: Deventer Ziekenhuis, afdeling Kindergeneeskunde Periode: april 2010 - augustus 2010 Inleiding: Incontinentie voor urine is een veel voorkomend probleem in de algemene kindergeneeskunde en (kinder)urologie, wat ook regelmatig bij kinderen van 6 jaar en jonger voorkomt. Er is echter weinig onderzoek verricht naar het effect van behandeling in deze jongere patiëntencategorie. In het Deventer Ziekenhuis (DZ) was de afgelopen jaren een toename van het aantal jonge patiënten, bij wie de ervaringen met behandeling positief waren. Het doel van dit onderzoek is te evalueren wat de resultaten zijn van de behandeling van incontinentie in het DZ in deze jongere leeftijdscategorie. Materiaal en methoden: Er werd een retrospectieve cohortstudie uitgevoerd van kinderen van 3 tot en met 6 jaar die in de periode 2004-2010 op de incontinentiepolikliniek van afdeling kindergeneeskunde van het Deventer 46

Ziekenhuis werden behandeld voor incontinentie. De medische dossiers van deze patiënten werden retrospectief geanalyseerd. Eerdere behandeling, symptomen, mictieparameters, co-morbiditeit, diagnose en behandeling werden onderzocht. Behandelresultaten werden vastgesteld door vergelijking van symptoomfrequenties voor en na behandeling. Voor het vaststellen van lange termijn resultaten werd telefonische follow-up uitgevoerd. Statistische analyse werd uitgevoerd met de software van PASW Statistics, release 18. Het significantieniveau werd vastgesteld op p<0,05. De gebruikte statistische toetsen waren de Pearson Chi-kwadraatttoets, Fisher’s Exacte toets, Student’s t-toets, gepaarde t-toets en ANOVA, Mann-Whitney U toets en Kruskal-Wallis toets. Resultaten: De respons van de follow-up was 94,6%. De statistische analyse werd uitgevoerd met 143 patiënten van wie de korte termijn resultaten bekend waren. De korte termijn resultaten volgens de ICCS definities waren als volgt: nonresponse 29,4%, response 16,8%, partial response 18,9% en full response 35,0%. Er waren geen significante verschillen tussen de geslachten, diagnosegroepen of leeftijdsgroepen. In de gehele populatie was een significante daling van symptoomfrequentie na de behandeling. Deze daling was ook significant voor de afzonderlijke leeftijdsgroepen van 4 tot en met 6 jaar. Na 2 jaar voldeed 44,2% van de kinderen met initieel goed resultaat, aan de ICCS definitie voor complete success (geen relapse en minder dan 1 symptoom per maand). De mediane leeftijd van starten van zindelijkheidstraining en zindelijk worden waren respectievelijk 24 en 33 maanden voor meisjes, en 30 en 36 maanden voor jongens, deze verschillen waren significant. Psychosociale stressfactoren of het starten met de basisschool waren een luxerende factor in 7,7% van de populatie. Conclusie: De multidisciplinaire behandeling van kinderen van 3 tot en met 6 jaar met incontinentie in het Deventer Ziekenhuis is effectief. De resultaten zijn vergelijkbaar met resultaten bij oudere kinderen. Om deze reden wordt poliklinische evaluatie en behandeling van incontinentie bij deze jonge kinderen aanbevolen. De wereldwijde trend van latere zindelijkheidstraining en bereiken van zindelijkheid wordt in deze studie bevestigd. Een relatie tussen deze trend en incontinentie kon niet worden onderzocht, hiervoor is verder onderzoek met controlepopulaties nodig. Starten met de basisschool en psychosociale stressfactoren leken, in tegenstelling tot de gedachte in de algemene bevolking, geen belangrijke luxerende factoren te zijn.
Vervolgstudie op ‘INDIAAN’. Invloed van BCG vaccinatie bij pasgeboren op de prevalentie van astma en allergische rinitis op latere leeftijd. Naam: Groenestein D. Locatie: Wilhelmina Kinderziekenhuis & Deventer ziekenhuis Periode: september 2009 - februari 2010 Disciplinegroep: Kinderallergologie Achtergrond: Preventie van allergie moet zeer waarschijnlijk plaatsvinden voor de eerste verjaardag van een kind. De gedachte is dat allergie/ atopie een Th2-gemedieerde reactie is, waarbij het stimuleren van het immuunsysteem richting Th1-gemedieerde reacties vroeg in het leven allergie- ontwikkeling zou kunnen voorkomen. Een sterke Th1 stimulans is een BCG vaccinatie. Verschillende epidemiologische onderzoeken hebben een effect van het BCG vaccin op atopische aandoeningen aangetoond In een voorgaande studie (INDIAAN) is de hypothese gesteld dat vroege BCG vaccinatie de ontwikkeling van allergische symptomen zou verminderen bij hoog risico groepen. Deze studie betrof een follow- up van 4 jaar. Zij toonde onder ander aan dat er een inverse relatie bestond tussen een BCG vaccinatie en het gebruik van medicatie voor van eczeem, maar er is tot op heden geen statistisch significant effect aangetoond voor de ontwikkeling van astma en allergische rinitis. De diagnose astma kan het best na de leeftijd van zes jaar gemaakt worden en de prevalentie van allergische rinitis stijgt vanaf de leeftijd van 4 jaar, vandaar dat er een follow- up studie wordt gestart om te onderzoeken of BCG vaccinatie invloed heeft op de prevalentie van deze twee atopische aandoeningen. Het doel van deze follow-up studie is om te bepalen of de toediending van een BCG vaccinatie op een leeftijd van 6 weken, bij hoog risico kinderen, effect heeft op de prevalentie en ernst van astma en/of allergische rinitis. Methode: De 116 deelnemers die de INDIAAN studie hebben afgemaakt zijn benaderd voor een eenmalige controle. Hierbij is anamnese, lichamelijk onderzoek en bloed afgenomen. Tevens zijn er vragenlijsten afgenomen. Er wordt gekeken naar de invloed die de neonatale BCG vaccinatie heeft op de prevalentie van astma en allergische rinitis bij kinderen met de leeftijd tussen de 7 en 10 jaar. Resultaten: Er is een trend te zien die aan geeft dat een BCG vaccinatie op een leeftijd van 6 weken een verminderd voorkomen van astma bewerkstelligd op de leeftijd van 7 tot 10 jaar (p = 053). Conclusie: Dit onderzoek lijkt eerder uitgevoerd epidemiologisch onderzoek te ondersteunen wat betreft de bevinding dat een BCG vaccinatie op jonge leeftijd de prevalentie van astma doet verminderen

Gert van Dijk Clin Auton Res. 2010 Feb;20(1):Epub 2009 Oct 15 Abstract BACKGROUND: In autonomic failure (AF), blood pressure (BP) falls upon standing which is commonly ascribed to defective vasoconstriction and excessive pooling. Observations on the amount of pooling in AF are contradictory. METHODS: We evaluated pooling using strain-gauge plethysmography (SGP) during head-up tilt (HUT) with a parachute harness fixed to the tilt table to avoid muscle tension in the lower limbs and thus to maximise pooling. 23 healthy subjects and 12 patients with AF were tilted for 5 min. BP and calf volume changes, as measured by SGP, were measured continuously. Multiple regression analysis was used to examine the effect of AF on orthostatic fluid shifts after adjustment for potential confounders. RESULTS: Patients did not differ from controls with respect to the increase of calf volume after 5 min HUT. The acute (0-1 min) and the prolonged (1-5 min) phases of calf volume responses to HUT were also similar between patients and controls. No correlation was found between the degree of orthostatic hypotension and the orthostatic calf volume change in AF. In one patient an additional measurement was made before rising from bed in the early morning demonstrating a greater albeit small increase of calf volume upon HUT. CONCLUSION: Orthostatic fluid shifts at the level of the calf in AF are not augmented during the course of the day despite marked hypotension. However, a small increase of pooling may be expected when the patient first gets out of bed in the morning probably due to the absence of oedema.

Samenvatting Verdamping van plasma en kwaliteitscontroleserum heeft een nadelig effect op de resultaten van klinisch-chemische testen. In dit onderzoek beschrijven we, afhankelijk van het klimaat op het laboratorium, een toename in plasmaconcentratie van 6 - 21% gedurende 10 uur van alle componenten, met uitzondering van CK, bilirubine en bicarbonaat. Gebruik van NoVapo® ‘liquid closure’, een vloeibare afsluiting direct op plasma aangebracht in de vorm van een hydrofobe vloeistof met lage dichtheid, voorkomt verdamping volledig gedurende een periode van 24 uur, met uitzondering van bovengenoemde drie parameters. NoVapo® wordt niet herkend door de pipetteernaalden van drie uitgeteste klinisch-chemische analysers, waardoor kwaliteitscontroleserum en plasma langdurig op de analyser aanwezig kan blijven zonder verandering van concentratie door indamping. Er is geen interferentie aantoonbaar met componenten uit plasma of kwaliteitscontroleserum.

Abstract BACKGROUND: The OLGA (operative link on gastritis assessment) staging system is based on severity of atrophic gastritis (AG). AG remains a difficult histopathologic diagnosis with low interobserver agreement, whereas intestinal metaplasia (IM) is associated with high interobserver agreement. OBJECTIVE: The aim of this study was to evaluate whether a staging system based on IM is preferable to estimate gastric cancer risk. DESIGN AND SETTING: Prospective multicenter study. PATIENTS: A total of 125 patients previously diagnosed with gastric IM or dysplasia. INTERVENTIONS: Surveillance endoscopy with extensive biopsy sampling. MAIN OUTCOME MEASUREMENTS: Three pathologists graded biopsy specimens according to the Sydney classification. Interobserver agreement was analyzed by kappa statistics. In the OLGA, AG was replaced by IM, creating the OLGIM. RESULTS: Interobserver agreement was fair for dysplasia (kappa = 0.4), substantial for AG (kappa = 0.6), almost perfect for IM (kappa = 0.9), and improved for all stages of OLGIM compared with OLGA. Overall, 84 (67%) and 79 (63%) patients were classified as stage I-IV according to OLGA and OLGIM, respectively. Of the dysplasia patients, 5 (71%) and 6 (86%) clustered in stage III-IV of OLGA and OLGIM, respectively. LIMITATION: Prospective studies should confirm the correlation between gastric cancer risk and OLGIM stages. CONCLUSION: Replacement of AG by IM in the staging of gastritis considerably increases interobserver agreement. The correlation with the severity of gastritis remains at least as strong. Therefore, the OLGIM may be preferred over the OLGA for the prediction of gastric cancer risk in patients with premalignant lesions.

Abstract BACKGROUND: Endoscopic surveillance of pre-malignant gastric lesions may add to gastric cancer prevention. However, the appropriate biopsy regimen for optimal detection of the most advanced lesions remains to be determined. Therefore, we evaluated the yield of endoscopic surveillance by standardized and targeted biopsy protocols. MATERIALS AND METHODS: In a prospective, multi-center study, patients with intestinal metaplasia (IM) or dysplasia (DYS) underwent a surveillance gastroscopy. Both targeted biopsies from macroscopic lesions and 12 nontargeted biopsies according to a standardized protocol (antrum, angulus, corpus, cardia) were obtained. Appropriate biopsy locations and the yield of targeted versus non-targeted biopsies were evaluated. RESULTS: In total, 112 patients with IM (n = 101), or low-grade (n = 5) and high-grade DYS (n = 6) were included. Diagnosis at surveillance endoscopy was atrophic gastritis (AG) in one, IM in 77, low-grade DYS in two, high-grade DYS in three, and gastric cancer in one patient. The angulus (40%), antrum (35%) and lesser curvature of the corpus (33%) showed the highest prevalence of pre-malignant conditions. Non-targeted biopsies from the lesser curvature had a significantly higher yield as compared to the greater curvature of the corpus in diagnosing AG and IM (p = .05 and p = .03). Patients with extensive intragastric IM, which was also present at the cardia were at high risk of a concurrent diagnosis of dysplasia or gastric cancer. High-grade DYS was detected in targeted biopsies only. CONCLUSIONS: At surveillance endoscopies, both targeted and non-targeted biopsies are required for an appropriate diagnosis of (pre-)malignant gastric lesions. Non-targeted biopsies should be obtained in particular from the antrum, angulus and lesser curvature of the corpus.

Antiviral activity of narlaprevir combined with ritonavir and pegylated interferon in chronic hepatitis C patients. Abstract Narlaprevir (SCH 900518) is a potent inhibitor of the hepatitis C virus (HCV) nonstructural protein 3 serine protease that is primarily metabolized by the cytochrome P450-3A4 system. In order to explore the use of ritonavir-based pharmacokinetic enhancement of an HCV protease inhibitor, this study investigated the safety, tolerability, pharmacokinetics, and antiviral activity of narlaprevir (with or without ritonavir) administered as monotherapy and as combination therapy with pegylated interferon-α-2b (PEG-IFN-α-2b) to HCV genotype 1-infected patients. This was a randomized, placebo-controlled, two-period, blinded study in 40 HCV genotype 1-infected patients (naïve and treatment-experienced). In period 1, narlaprevir was administered for 7 days as 800 mg three times daily without ritonavir or 400 mg twice daily with 200 mg ritonavir twice daily. In period 2, after a 4-week washout, the same dose and regimen of narlaprevir was administered in combination with PEG-IFN-α-2b for 14 days. Upon completion of period 2, all patients initiated PEG-IFN-α-2b and ribavirin treatment. A rapid and persistent decline in plasma HCV-RNA was observed in both treatment-experienced and treatment-naïve patients during period 1, with a mean viral load decline of at least 4 log in all treatment groups. A high percentage of both treatment-experienced (50%) and treatment-naïve (≥ 60%) patients had undetectable HCV-RNA (< 25 IU/mL) after period 2. Standard of care resulted in sustained virological response (SVR) rates of 38% and 81% in treatment-experienced and treatment-naïve patients, respectively. Narlaprevir (with or without ritonavir) alone or in combination with PEG-IFN-α-2b was safe and well tolerated. CONCLUSION: Narlaprevir administration resulted in a robust HCV-RNA decline and high SVR rates when followed by standard of care in both treatment-experienced and treatmentnaïve HCV genotype 1-infected patients.
Safety and antiviral activity of JTK-652: a novel HCV infection inhibitor. Abstract BACKGROUND: Standard treatment of chronic hepatitis C with pegylated interferon and ribavirin is associated with suboptimal virological response rates and substantial side effects. This study describes the in vitro and in vivo development of JTK-652, a novel pyrrolopyridazin-derived HCV infection inhibitor. METHODS: JTK-652 was evaluated in multiple cell lines using an in vitro HCV infection model consisting of HCV pseudotype vesicular stomatitis virus bearing HCV E1/E2 envelope proteins. Safety, tolerability, pharmacokinetics and efficacy of JTK-652 were tested in a randomized double-blind and placebo-controlled study in healthy male volunteers (n=36) and chronic hepatitis C patients. A total of 10 HCV genotype-1-infected patients (treatment-naive [n=2] and treatment-experienced [n=8]) with HCV RNA>1x10(5) IU/ml received an oral dose of 100 mg JTK-652 three times daily or placebo (8:2 ratio) for 4 weeks. RESULTS: JTK-652 showed potent inhibitory activity against HCV genotype 1a and 1b pseudotype viruses bearing HCV E1/E2 envelope proteins in HepG2 cells and in human primary hepatocytes. No significant clinical laboratory, vital sign, ECG or physical examination abnormalities were observed during the Phase I trial. JTK-652 was found to be well tolerated. No significant changes in HCV RNA levels compared with baseline were observed at the end of treatment. CONCLUSIONS: Although results from the preclinical studies indicated that JTK-652 has well-established antiviral properties and a Phase I clinical trial has showed that JTK-652 was safe and well tolerated at a 100 mg three times daily dose level, plasma HCV RNA levels in chronically HCV-infected patients did not decrease during 28 days of dosing at a 100 mg three times daily dose level.

Een vrouw met pijn op de borst. Ter Borg MJ, Ledeboer M Ned Tijdschr Geneeskd. 2010;154:A1174 Casus Een 62-jarige Turkse vrouw met in de medische voorgeschiedenis hypertensie presenteerde zich op de afdeling Eerste Harthulp met plots ontstane pijn op de borst. Patiënte was hemodynamisch stabiel. Een hartinfarct werd door middel van laboratoriumonderzoek en ecg uitgesloten. Onder verdenking van een aortadissectie werd een CT-scan met contrast verricht, welke een aspecifiek verdikte slokdarmwand toonde (figuur a). Bij gastroscopie werd op 22 cm van de tandenrij een groot hematoom in de slokdarmwand gezien, doorlopend naar distaal (zie figuur c). Patiënte werd conservatief behandeld met een niets-per-os-dieet en werd later via een maagsonde gevoed. Het verdere beloop was ongecompliceerd en patiënte werd uiteindelijk na 10 dagen ontslagen. Bij controlegastroscopie 2 maanden later werden geen afwijkingen meer gezien. Een slokdarmhematoom is een zeldzame aandoening die zich meestal presenteert met pijn op de borst, dysfagie en soms bloedbraken. Het kan iatrogeen ontstaan, bijvoorbeeld na een transoesofageale echocardiografie of na het plaatsen van een maagsonde. Differentiaaldiagnostisch kan worden gedacht aan een hartinfarct, een aortadissectie, een oesofagusperforatie, een ulcus of het syndroom van Mallory-Weiss. Bij gastroscopie wordt een zwelling van de slokdarmwand gezien met meestal een blauwe verkleuring, die verward kan worden met slokdarmvarices en tumoren. De behandeling is conservatief, met tijdelijk een niets-per-os-dieet.

Bartelink Locatie: Maag-, darm-, leverziekten, Deventer Ziekenhuis Periode: 26 oktober 2009 - 12 maart 2010 Achtergrond en studiedoel: Leverbiopsie draagt bij aan de diagnostiek bij patiënten met verhoogde leverenzymwaarden waarbij geen diagnose gesteld kan worden na laboratoriumonderzoek en beeldvormend onderzoek. Leverbiopsie is echter een invasieve ingreep met kans op complicaties. Bovendien wordt er naar aanleiding van een leverbiopsie lang niet altijd een diagnose gesteld of een beleidsverandering doorgevoerd. Het doel van deze studie is om de baten van leverbiopsie in deze groep af te wegen tegen de complicaties, zodat er meer duidelijkheid gegeven kan worden over de plaats van deze procedure in het diagnostische proces. Methode: Alle histologieverslagen van leverbiopten verricht in het Deventer Ziekenhuis in de periode 1999-2009 werden retrospectief bestudeerd en aangevuld met bijbehorende elektronische patiëntendossiers. De diagnostische bijdrage werd beoordeeld bij biopten welke om diagnostische redenen werden verricht. Complicaties werden onderzocht in alle verrichtte 59
leverbiopsiën, ongeacht of ze werden uitgevoerd om diagnostische redenen. Verbanden tussen enerzijds verschillende variabelen en anderzijds complicaties, diagnostische bijdrage en beleidsverandering werden geanalyseerd met de Chi-kwadraattest en de Fisher exacttest. Resultaten: Er werden 198 leverbiopsieverslagen bestudeerd waarvan 105 (53,0%) om diagnostische redenen werden verricht. Bij 6,6% (n=13) van alle leverbiopsiën traden complicaties op. Van alle diagnostische leverbiopsiën trad er bij 10,5% een complicatie op, dit is significant vaker (p=0,017) vergeleken met andere indicaties (4,3%). Bij 60,0% van de gevallen droeg leverbiopsie bij aan de diagnostiek; in 18,1% van deze gevallen werd een verandering in medicatie doorgevoerd. Bij 31,4% werden leefstijladviezen gegeven of werd de controlefrequentie veranderd, bij 29,5% bleef het beleid ongewijzigd. Er werd geen verband gevonden tussen een diagnostische bijdrage van leverbiopsie en het veranderen van het beleid (p=0,256). Conclusie: Leverbiopsie heeft een zeer beperkte meerwaarde voor patiënten met verhoogde leverenzymwaarden van onbekende origine na laboratorium onderzoek en beeldvormend onderzoek. Er wordt in dit onderzoek geen verband aangetoond tussen een diagnostisch bijdragend leverbiopt en verandering in beleid. Bij deze groep patiënten is er een significant verhoogde kans op complicaties; dit onderstreept de geboden terughoudendheid bij het verrichten van leverbiopsie. Het uitvoeren van een leverbiopsie lijkt in deze studie niet effectiever dan het pragmatisch behandelen van deze patiënten na laboratoriumonderzoek en beeldvormend onderzoek.
was used to identify all subjects that underwent a resection for colonic adenocarcinoma with preceding endoscopic biopsy in the years 2004 and 2005. All (anonymous) pathology records were evaluated manually to score the above-mentioned variables. RESULTS: 3,472 cases were included (50.3% men). Median age was 71.0 years (23-97). Median TUR was 28 days (1-440). Univariate analysis unexpectedly showed that advanced disease (TNM stages III and IV) was less often present when TUR increased; the same was true for infiltration depth, tumour size and the number of affected lymph nodes. These associations persisted after correction for age, sex, histological grade and tumour localization (p=0.000), as well as after excluding urgent resections (TUR less than one week) or TUR of more than 2 months. Histological grade and young age were independently associated with advanced tumour stage at operation. Women had significantly shorter TUR than men; the same was true for young versus old patients. CONCLUSION: Increased waiting time is associated with less advanced tumour at operation, even after exclusion of urgent resection and waiting times more than 2 months. Poor histological differentiation may be a reason to give patients priority on the waiting list. Differences in TUR found between sexes and younger and older age groups may be due to comorbidity or psychological factors. Additional prospective research to explain these findings is urgently needed.

Evaluation of the Sysmex UF-1000i® Urine Flow Cytometer in the Diagnostic Work-Up of Suspected Urinary Tract Infection in a Dutch General Hospital. Abstract BACKGROUND: Automation and standardization of sediment analysis of urine samples by flow cytometry might serve as an alternative to labor-intensive laboratory methods, such as microscopic examination and culture. The Sysmex UF-1000i is a urine flow cytometer that uses two separate channels for counting blood cells and bacteria. METHODS: In this study, 358 urine samples were analyzed with the Sysmex UF-1000i in parallel with manual microscopy, Gram stain and bacterial culture, the latter considered the gold standard. RESULTS: Reproducibility for detection of white and red blood cells and bacteria was good, while detection of yeast proved unreliable. Depending on the definition of urinary tract infection (UTI) used, the negative predictive value and the percentage of false-negative results were 100% and 0% [UTI ≥ 10(5) colony forming units (CFU)/mL] and 99% and 1.3%, (UTI ≥ 10(4) CFU/mL), respectively. Pre-screening with the Sysmex UF-1000i would have resulted in a reduction of bacterial culture by 42%. Carry over of bacteria between consecutive samples due to the use of fixed sample needle was observed, but did not result in false-positive interpretation of Sysmex UF-1000i results. Because of the occurrence of carry over, samples that have been analyzed with the Sysmex UF-1000i cannot be used for subsequent urine culture. CONCLUSIONS: In conclusion, the Sysmex UF-1000i offers the possibility for screening high numbers of urine samples in a fast and standardized way, resulting in a reduction in workload and speeding the diagnostic process. It is not recommended for use in complicated patient populations, such as neutropenic patients and patients in whom yeast infection is suspected.

Abstract The purpose of this retrospective study was to assess the prevalence of papilledema in patients with isolated craniosynostosis. Second, we wanted to assess if the presence of ventricular dilatation on computed tomography (CT) scan is a predictive factor for the occurrence of papilledema. We included 205 consecutive children with an isolated single-suture craniosynostosis, who had at least 1 fundus examination. Preoperative CT scans of the brain were examined for the presence of ventricular dilatation.Papilledema developed in 14 of 205 patients: 10 developed papilledema before surgery and 4 during the follow-up period. Ten of the patients with papilledema had a synostosis of the sagittal suture, and 4 of the metopic suture. Prevalence of papilledema in scaphocephaly was 9.7%, and in trigonocephaly, 5.6%. Based on evaluation of all CT scans, ventricular dilatation seemed not to be a predictive factor for papilledema in children with isolated craniosynostosis.The incidence of papilledema in almost 10% of scaphocephaly patients is remarkably higher than expected. Therefore, we recommend routine preoperative screening, especially for patients with scaphocephaly, but also for patients with trigonocephaly. Postoperative screening is recommended in all patients when there is any uncertainty.

Long-term functional outcome in 167 patients with syndromic craniosynostosis; defining a syndrome-specific risk profile. The objective of this study was to define the prevalence per syndrome of elevated ICP, OSA, impaired sight and impaired hearing. METHODS: A retrospective study was undertaken on 167 consecutive patients diagnosed with Apert, Crouzon, Pfeiffer, Muenke or Saethre-Chotzen syndrome, aged 1-25 years and treated between 1983 and 2008. The mean age at time of referral and review was 1 years and 2 months and 10 years and 3 months, respectively RESULTS: Patients with Apert and Crouzon/Pfeiffer syndromes had the highest prevalence of elevated ICP (33% and 53%, respectively) and OSA (31% and 27%, respectively), while Saethre-Chotzen syndrome was also associated with a fair risk for elevated ICP (21%). The prevalence of impaired sight (61%) and hearing (56%) was high in all syndromes. CONCLUSION: Based on these data, a syndrome-specific risk profile with suggestions for screening and treatment is presented.

Abstract BACKGROUND: The bone mineral density (BMD) of the distal femur decreases by 16-36% within one year after total knee arthroplasty (TKA) because of the femoral component’s stress-shielding effect. The aim of this prospective study was to determine the quantitative change from the baseline BMD in the distal femur 1 year after patellofemoral arthroplasty using dual-energy X-ray absorptiometry (DXA). METHODS: Between December 2007 and December 2008, 14 patients had patellofemoral arthroplasty for isolated patellofemoral osteoarthritis. Distal femoral BMD was assessed using DXA in 2 regions of interest (ROI) on the lateral view 2 weeks before and 12 months after patellofemoral arthroplasty. The contra-lateral knee was used as a control, with BMD measurements performed in identical ROIs. RESULTS: The mean change from baseline BMD in the operated knees after 1 year was -0.169 g/cm2 (95% CI: -0.293 to -0.046 g/cm2) behind the anterior flange (-15%), and -0.076 g/cm2 (95% CI: -0.177 to 0.024 g/cm2) in the supracondylar area 1 cm above the prosthesis (-8%) (p = 0.01 and p = 0.13, respectively). The mean change from baseline BMD in the non-operated knees after 1 year was 0.016 g/cm2 (95% CI: -0.152 to 0.185 g/cm2) behind the anterior flange (2%), and 0.023 g/cm2 (95% CI: -0.135 to 0.180 g/cm2) in the supracondylar area 1 cm above the prosthesis (2%) (p = 0.83, and p = 0.76, respectively). CONCLUSIONS: Our findings suggest that patellofemoral arthroplasty results in a statistically significant decrease in BMD behind the anterior flange.
the highest level of available evidence on the nonoperative and operative treatment of isolated patellofemoral osteoarthritis to develop an evidencedbased discussion of treatment options. METHODS: A systematic computerized database search (Cochrane Database of Systematic Reviews, Cochrane Central Register of Controlled Trials, MEDLINE (PubMed), and EMBASE) was performed in March 2009. The quality of the studies was assessed independently by two authors using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach. RESULTS: We extracted data from 44 articles. The best available evidence for treatment of isolated patellofemoral osteoarthritis is sparse and of generally low methodological quality. Nonoperative treatment using physiotherapy (GRADE: high quality, weak recommendation for use), taping (GRADE: moderate quality, weak recommendation for use), or injection therapy (GRADE: very low quality, weak recommendation for use) may result in short-term relief. Joint-preserving surgical treatment may result in insufficient, unpredictable, or only short-term improvement (GRADE: low quality, weak recommendation against use). Total knee replacement with patellar resurfacing results in predictable and good, durable results (GRADE: low quality, weak recommendation for use). Outcome after patellofemoral arthroplasty in selected patients is good to excellent (GRADE: low quality, weak recommendation for use). INTERPRETATION: Methodologically good quality comparative studies, preferably using a patient-relevant outcome instrument, are needed to establish the optimal treatment strategy for patients with isolated patellofemoral osteoarthritis.

Some orthopedic surgeons use circumpatellar electrocautery (diathermy) to reduce the prevalence of postsurgical anterior knee pain; however, the extent of its use is unknown. MATERIALS AND METHODOLOGY: In April 2009, a postal questionnaire was sent to all 98 departments of orthopedic surgery in The Netherlands. The

questions focused on the frequency of total knee arthroplasties, patellar resurfacing, and the use of circumpatellar electrocautery. RESULTS: The response rate was 92%. A total of 18,876 TKAs, 2,096 unicompartmental knee arthroplasties, and 215 patellofemoral arthroplasties are performed yearly in The Netherlands by the responding orthopedic surgeons. Of the orthopedic surgeons performing TKA, 13% always use patellar resurfacing in total knee arthroplasty for osteoarthritis, 49% use selective patellar resurfacing, and 38% never use it. Fifty-six percent of orthopedic surgeons use circumpatellar electrocautery when not resurfacing the patella, and 32% use electrocautery when resurfacing the patella. CONCLUSION: There is no consensus among Dutch orthopedic surgeons on the use of patellar resurfacing or circumpatellar electrocautery in total knee replacement performed for osteoarthritis. A prospective clinical trial is currently underway to fully evaluate the effect of circumpatellar electrocautery on the prevalence of anterior knee pain following total knee arthroplasty.

Abstract The purpose of this study was to correlate the long-term survival of patellofemoral arthroplasty with primary diagnosis, age, sex, and body mass index. One hundred eighty-five consecutive Richards type II patellofemoral arthroplasties were performed in 161 patients with isolated patellofemoral osteoarthritis. Diagnoses included primary patellofemoral osteoarthritis, posttraumatic patellofemoral osteoarthritis, and patellofemoral osteoarthritis with a previous realignment procedure for patellar subluxation or trochlear dysplasia. Median time to follow-up was 13.3 (range, 2.0-30.6) years. Patellofemoral arthroplasty survival was 84% at 10 years and 69% at 20 years. Primary diagnosis, sex, or age at patellofemoral arthroplasty did not significantly affect the rate of revision (P=.35, P=.24, and P=.65, respectively). The rate of revision in obese patients (body mass index>30 kg/m2) was higher than that in nonobese patients (P=.02).

Abstract BACKGROUND: The indication for total ankle replacement (TAR) as an alternative to ankle fusion continues to be a much-debated topic. The reported survival of TAR at midterm followup is approximately 90%. The aim of this study was to compare functional outcome and survival of TAR in low volume centers versus high volume centers. MATERIALS AND METHODS: A retrospective cohort study was carried out in four low volume centers. Sixty-four Salto TARs were performed between 2003 and 2007 in 60 patients. Fifty-five (59 TAR) patients were eligible for followup with 28 men. Standardized American Orthopaedic Foot and Ankle Society (AOFAS) scores, patient satisfaction, and range of motion (ROM) were measured. Standardized and dynamic radiographs were used for evaluation of radiolucencies, ROM and component alignment. RESULTS: Seven of the 59 ankle prostheses had to be revised: five for loosening and two for deep infection. Three of the five revised for loosening went on to fusion, and in two a revision of one of the components was performed. Both infected ankles were fused. Five patients declined to participate this study, among these two were TAR failures. Survival with revision as the endpoint was 86% at final followup. The average AOFAS score was 75 (SD ± 15). On dynamic radiographs the ROM was 22 degrees (SD ± 8) in the tibiotalar joint. CONCLUSION: This study demonstrated that functional results of total ankle replacement in low volume centers were comparable to most high volume centers but survival was lower especially when we consider our shorter followup than most comparable series.
Total knee arthroplasty in younger patients: a 13-year follow-up study. Bisschop R, Brouwer RW, Van Raay JJ. Orthopedics. 2010 Dec 1;33(12):876 Abstract Total knee arthroplasty (TKA) is a well-established treatment at the end stage of a degenerated knee joint. This operative treatment generally relieves pain, improves physical function, and has a high level of patient satisfaction, especially in the elderly. Younger patients, however, are demanding and have a higher level of physical activity compared to elderly patients. One could 69
therefore expect more mechanical problems such as prosthetic loosening and polyethylene wear after long-term follow-up. The goal of this retrospective cohort study was to determine the survival and long-term results of TKA in young patients. Patients who received a TKA at age 60 years or younger for any reason were included. Minimum follow-up was 10 years. Thirty-nine TKAs (Anatomic Graduate Components; Biomet, Warsaw, Indiana) in 31 patients were included. Average patient age was 52.6 years. There were 3 revisions because of infection; in 1 knee the patella was revised because of aseptic loosening. After an average 13-year follow-up, the survival rate was 89.7% and function scores showed a reasonably functioning TKA. There was no difference in survival rate and function scores between patients with rheumatoid arthritis and those with primary or secondary (posttraumatic) osteoarthritis. Our experience with TKA in a younger patient population has been encouraging. The risk of loosening and wear of the implant in our study is low, and this type of TKA also seems to be an effective and safe treatment for younger patients.

Non-operative treatment modalities, such as physical therapy, taping, or injection therapy, may result in short-term relief. Partial lateral facetectomy, distal realignment procedures, lateral retinacular release, and other joint-preserving surgical treatments may lead to insufficient, unpredictable, or only short-term improvement. Though total knee replacement with patellar resurfacing results in good predictable and durable results, it is probably too aggressive of a treatment for what is, in effect, a disease confined to one compartment. In patellofemoral arthroplasty, the femorotibial compartments, with cruciate 70
ligaments and menisci, are spared, which may allow the preservation of the physiological biomechanics of the femorotibial joint. The reported clinical results are related to patient selection, prosthetic design properties, and surgical technique, and show good to excellent medium- and long-term results. This review considers the role of patellofemoral joint replacement and other treatment modalities in isolated patellofemoral osteoarthritis.

Sialorrhea is often treated with anticholinergics, but central side effects limit their usefulness. Glycopyrrolate (glycopyrronium bromide) is an anticholinergic drug with a quaternary ammonium structure not able to cross the blood-brain barrier in considerable amounts. Therefore, glycopyrrolate exhibits minimal central side effects, which may be an advantage in patients with PD, of whom a significant portion already experience cognitive deficits. OBJECTIVE: To determine the efficacy and safety of glycopyrrolate in the treatment of sialorrhea in patients with PD. METHODS: We conducted a 4-week, randomized, double-blind, placebocontrolled, crossover trial with oral glycopyrrolate 1 mg 3 times daily in 23 patients with PD. The severity of the sialorrhea was scored on a daily basis by the patients or a caregiver with a sialorrhea scoring scale ranging from 1 (no sialorrhea) to 9 (profuse sialorrhea. RESULTS: The mean (SD) sialorrhea score improved from 4.6 (1.7) with placebo to 3.8 (1.6) with glycopyrrolate (p = 0.011). Nine patients (39.1%) with glycopyrrolate had a clinically relevant improvement of at least 30% vs 1 patient (4.3%) with placebo (p = 0.021). There were no significant differences in adverse events between glycopyrrolate and placebo treatment. CONCLUSIONS: Oral glycopyrrolate 1 mg 3 times daily is an effective and safe therapy for sialorrhea in Parkinson disease. Classification of evidence: This study provides Class I evidence that glycopyrrolate 1 mg 3 times daily is more effective than placebo in reducing sialorrhea in patients with Parkinson disease during a 4-week study.

OBJECTIVE: To investigate the effects on blood pressure and renal function of the short-term combined use of NSAIDs with diuretics and/or RAAS inhibitors. DESIGN: Prospective, observational cohort study. METHODS Patients admitted for elective knee or hip replacement, receiving treatment with a diuretic and/or an RAAS inhibitor, and without impaired renal function, were included. Based on post-operative NSAID therapy patients with combined use were compared to patients without combined use. Primary outcome measures were blood pressure and renal function, measured on first and fourth day post-operative. RESULTS: In total 153 patients were included; 97 in the NSAID group and 53 in the control group. Because of missing data 3 patients were excluded in the outcome analysis. Both groups were comparable regarding demographic criteria, with an exception for the combined use of diuretics with RAAS inhibitors, which was lower in the group receiving post-operative NSAID treatment. No differences in blood pressure and renal function between the two groups were observed. CONCLUSION: Short-term NSAID treatment in the post-operative period can be combined with diuretics and/or RAAS inhibitors.

Promotie Assessment of drug therapie in Parkinson’s disease. Maurits Erwin Leo Arbouw Utrecht, Universiteit Utrecht, 29 juni 2010 Samenvatting DE ZIEKTE VAN PARKINSON Bij de ziekte van Parkinson vindt een afbraakproces (degeneratie) plaats van zenuwcellen in een bepaald gebied in de hersenen: de ‘zwarte kernen’ (substantia nigra). Door het verdwijnen van zenuwcellen in deze kernen wordt er niet voldoende dopamine aangemaakt. Dopamine is een neurotransmitter, een stof die nodig is om zenuwimpulsen van de ene zenuwcel op de andere over te brengen. Door een tekort aan dopamine gaan de hersenkernen, die betrokken zijn bij het soepel laten verlopen van lichaamsbewegingen, slechter functioneren. De ziekte van Parkinson wordt onder andere gekenmerkt door de volgende bewegingsstoornissen (motorische symptomen): rust tremor (trillen bij rust, eerst aan één hand, arm of been, later aan beide), rigiditeit (stijfheid van de ledematen), bradykinesie (bewegingsarmoede), en posturele instabiliteit (gestoorde, soms voorovergebogen, houding en gestoorde houdingsreflexen). Er wordt nu algemeen aangenomen dat de ziekte van Parkinson een complexe ziekte is met diverse uitingsvormen, waaronder neuropsychiatrische en andere niet-motorische symptomen naast de genoemde motorische symptomen. Naar schatting waren er in 2007 tussen de 33.000 en 61.000 mensen in Nederland die de ziekte van Parkinson hadden. Elk jaar wordt in Nederland bij ongeveer 8.000 mensen de diagnose ziekte van Parkinson gesteld. Leven met de ziekte van Parkinson Patiënten met de ziekte van Parkinson hebben een levensverwachting die maar enkele jaren korter is dan mensen zonder deze ziekte. Echter, de ziekte van Parkinson is een progressieve ziekte. In de meer gevorderde stadia ervaren patiënten zowel motorische als niet-motorische symptomen die zeer lastig te behandelen zijn. Uit een Nederlands onderzoek blijkt dat de ziekte van Parkinson een grote invloed op de kwaliteit van leven heeft: de ziekte van Parkinson (gevorderde stadia) werd als derde gerangschikt na dementie en slokdarmkanker, met een verlies van kwaliteit van leven van 68%. Behandeling van de ziekte van Parkinson De ziekte van Parkinson is niet te genezen en de behandeling is dan ook gericht op het verminderen en verlichten van de klachten en symptomen. De medicamenteuze behandeling van de ziekte van Parkinson bestaat vooral uit levodopa en dopamine agonisten (dopaminerge geneesmiddelen). Deze geneesmiddelen vullen het tekort van dopamine aan of stimuleren de dopamine aangrijpingspunten (receptoren).
Naast het beoogde effect kunnen deze geneesmiddelen ook bijwerkingen geven. Een tweetal mogelijke bijwerkingen van dopaminerge geneesmiddelen is in dit proefschrift onderzocht. Variabiliteit van geneesmiddelrespons Het is onbekend waarom sommige patiënten bijwerkingen krijgen en anderen niet, en waarom sommige patiënten veel last hebben van fluctuaties in bewegingssymptomen en anderen minder of niet. Diverse niet-genetische factoren, zoals de ziekteduur of gelijktijdig gebruik van andere geneesmiddelen, zouden hierin een rol kunnen spelen. Ook wordt er aangenomen dat variatie in genen een belangrijke rol speelt in de interindividuele variabiliteit in de respons op een geneesmiddel. Het betreffen genen die coderen voor enzymen die geneesmiddelen omzetten, voor geneesmiddelenaangrijpingspunten, en voor signaal eiwitten. Farmacogenetica is het onderzoeksgebied dat zich richt op de vraag of, en in welke mate, genetische variatie de respons op geneesmiddelen kan verklaren en voorspellen bij individuele patiënten. DIT PROEFSCHRIFT In dit proefschrift komen diverse aspecten aan de orde die te maken hebben met de behandeling met geneesmiddelen van de ziekte van Parkinson. Het doel van dit proefschrift was om inzicht te krijgen in: - de incidentie (hoe vaak het voorkomt) en determinanten (genetische en niet-genetische) van stoppen van de behandeling met niet-ergoline dopamine agonisten; - het ontstaan van bepaalde complicaties bij patiënten die dopaminerge geneesmiddelen gebruiken; - de resultaten van behandeling van niet-motorische symptomen; - het aandeel van farmacogenetica in het voorspellen en verklaren van variabiliteit van geneesmiddelrespons. Diverse farmaco-epidemiologische principes zijn in dit proefschrift toegepast. In farmaco-epidemiologisch onderzoek staat het bestuderen van effecten en het gebruik van geneesmiddelen in patiëntenpopulaties centraal. Hoofdstuk 1 is een algemene inleiding waar wordt ingegaan op de ziekte van Parkinson en de behandeling ervan. In Hoofdstuk 2 wordt een overzicht gegeven wat de invloed is van genetische variaties op resultaten van behandeling met geneesmiddelen (farmacogenetica). Het is bekend dat er een grote variabiliteit is in respons op de anti-Parkinson geneesmiddelen, zowel ten aanzien van effectiviteit als ten aanzien van toxiciteit/bijwerkingen. Er wordt aangenomen dat genetische variaties in genen belangrijke factoren zijn in de verklaring van deze variabiliteit van geneesmiddelrespons. In de ideale situatie kan kennis over genetische variaties (polymorfismen) helpen om het effect van behandeling met geneesmiddelen te voorspellen.

De rol van farmacogenetica in de behandeling van de ziekte van Parkinson is maar beperkt onderzocht. Daarom was het doel om al de beschikbare literatuur in kaart te brengen, en om mogelijk interessante genetische variaties voor de toekomst te beschrijven. Het beperkte aantal onderzoeken bracht een aantal interessante associaties aan het licht, maar sommige resultaten waren helaas tegenstrijdig. Er waren associaties gevonden tussen 1) levodopa-geïnduceerde dyskinesieën (overmatige bewegelijkheid) of motorische fluctuaties en polymorfismen in het dopamine D2 receptor gen; 2) het optreden van hallucinaties en polymorfismen in het dopamine transporter gen, het cholecystokinine gen en het apolipoproteine E gen; en 3) het optreden van slaapaanvallen zonder voortekenen en polymorfismen in het dopamine receptor 2 en dopamine receptor 4 gen. Het is waarschijnlijk dat het aantal geïncludeerde patiënten in veel onderzoeken te klein was om associaties aan te tonen. De conclusie van het literatuuronderzoek was dat aanvullend onderzoek nodig is met grotere patiënten aantallen waarbij variaties in meerdere genen tegelijk moeten worden onderzocht. In Hoofdstuk 3 staan de nieuwere anti-Parkinson geneesmiddelen, de niet-ergoline dopamine agonisten, centraal. Deze geneesmiddelen zijn vanaf 1997 geïntroduceerd en bestaan uit ropinirol, pramipexol en, meer recent, rotigotine. Het is bekend dat geneesmiddelen in onderzoeken slechts gedurende een beperkte tijd worden onderzocht, onder strenge condities en in een homogene, zorgvuldig geselecteerde patiëntengroep. Hierdoor is het moeilijk om resultaten uit dergelijke onderzoeken te vertalen naar de dagelijkse praktijk. Daarom hebben we in Hoofdstuk 3.1 gekeken naar de verschillen tussen patiënten uit de dagelijkse praktijk en patiënten in onderzoeken die een niet-ergoline dopamine agonist gebruikten. We hebben de patiëntenkarakteristieken en het aantal patiënten die de therapie stopten vergeleken. De groep patiënten uit de dagelijkse praktijk omvatte een retrospectief cohort van patiënten van het Medisch Spectrum Twente in Enschede, die voor het eerst ropinirol (45 patiënten) of pramipexol (59 patiënten) gebruikten. Indien er meer dan 180 dagen tussen twee ophaalmomenten bij de apotheek zat, werden deze patiënten geclassificeerd als ‘stoppers’. Het bleek dat binnen drie jaar 51% van de ropinirol gebruikers en 60% van de pramipexol gebruikers waren gestopt. In de literatuur vonden we tien onderzoeken met ropinirol en 12 met pramipexol. De patiëntenkarakteristieken verschilden niet tussen de patiënten in de onderzoeken en de groep uit de dagelijkse praktijk. We concludeerden dat het percentage patiënten die de therapie in de onderzoeken stopten tot één jaar vergelijkbaar was met de dagelijkse praktijk. Het lijkt er op dat voor de langere termijn het percentage stoppers in de dagelijkse praktijk hoger is dan in de onderzoeken.
In Hoofdstuk 3.2 hebben we niet-genetische determinanten voor het stoppen met niet-ergoline dopamine agonisten in kaart gebracht. In een deelonderzoek hebben we gekeken naar determinanten in genen die coderen voor de dopamine 2 en dopamine 3 receptor. In het niet-genetische deel hebben we 90 patiënten van het Medisch Spectrum Twente in Enschede geïncludeerd. Het bleek dat het gebruik van het anti-Parkinson geneesmiddel apomorfine geassocieerd was met stoppen van een niet-ergoline dopamine agonist (hazard ratio [HR] 6,26; 95% betrouwbaarheid interval [95%BI] 1,85-21,2). Het aantal gebruikers van apomorfine was echter maar beperkt. Ook levodopa doseringen tussen 500 en 1000 mg waren geassocieerd met stoppen van een niet-ergoline dopamine agonist (HR 2,31; 95%BI 1,08-4,93). In het farmacogenetische deelonderzoek werden 38 patiënten geïncludeerd. Afwezigheid van de 15x CA herhaling in het dopamine receptor 2 gen bleek geassocieerd te zijn met minder stoppen van niet-ergoline dopamine agonist therapie (HR 0,23; 95%BI 0,07-0,81). Een polymorfisme in het dopamine receptor 3 gen liet een niet-significant effect zien, maar het risico op stoppen nam wel per allel toe (allel-dosis effect). Dit ondersteunt het bestaan van een dergelijke relatie. We concludeerden dat onze bevindingen in een grotere groep patiënten dienen te worden onderzocht. In Hoofdstuk 4 hebben we gekeken naar twee mogelijke complicaties door het gebruik van dopaminergica bij patiënten met de ziekte van Parkinson. In Hoofdstuk 4.1 hebben we onderzocht of er een associatie bestaat tussen het gebruik van dopamine agonisten en opname in het ziekenhuis met een ischemische oorzaak (onvoldoende zuurstof door onvoldoende doorbloeding). Er wordt namelijk gesuggereerd dat dopamine agonisten, in het bijzonder de ergoline dopamine agonisten, ischemische complicaties kunnen geven. Het is bekend dat ergotamine, een ergot alkaloïd die bij de behandeling van migraine wordt gebruikt, een verhoogd risico geeft op dergelijke ischemische complicaties. Het bestaan van een associatie is onderzocht door middel van een case-controle onderzoek binnen de PHARMO database. Dit is een Nederlandse onderzoeksdatabank waarin momenteel van meer dan twee miljoen personen (geanonimiseerde) zorggegevens kunnen worden gekoppeld ten behoeve van farmaco-epidemiologisch onderzoek. Alle patiënten met minimaal één voorschrift voor levodopa na het 55ste levensjaar in de periode 1994 tot 2006 zijn geïdentificeerd. Cases waren gedefinieerd als patiënten die na november 1997 waren opgenomen voor een ischemische complicatie (= index datum). Bij iedere case werden maximaal vier controle patiënten gezocht zonder een ischemische complicatie. In het onderzoek werden uiteindelijk 542 case patiënten en 2155 controle patiënten geïncludeerd. Blootstelling aan dopamine agonisten en ook mogelijke verstorende

variabelen werden in de periode van één jaar voor de index datum vastgesteld. Het gebruik van dopamine agonisten bleek niet te zijn geassocieerd met een hoger risico op opname in het ziekenhuis voor een ischemische oorzaak (odds ratio [OR] 1.19; 95%BI 0,95-1,49). In Hoofdstuk 4.2 hebben we gekeken naar het effect van dopaminerge geneesmiddelen en gelijktijdig gebruik van psychotrope geneesmiddelen op het risico van heup- of dijbeen fracturen. Dopaminerge geneesmiddelen hebben verscheidene effecten die het valrisico en fractuurrisico kunnen verhogen. Aan de andere kant kunnen dopaminerge geneesmiddelen ook zorgen voor een verlaagd fractuur risico door een betere mobilisatie en door een mogelijke verbetering van de botdichtheid. We hebben een case-controle onderzoek uitgevoerd binnen de PHARMO database met patiëntenregistraties over de periode 1991 tot en met 2002. Cases waren patiënten ouder dan 18 jaar met een eerste heup- of dijbeen fractuur (= index datum). Bij iedere case werden maximaal vier controle patiënten gezocht zonder een fractuur maar die met de cases overeenkwamen wat betreft geboortejaar, geslacht en geografische regio. In het onderzoek werden uiteindelijk 6.763 case patiënten en 26.341 controle patiënten geïncludeerd. Huidig gebruik van dopaminerge geneesmiddelen (1-30 dagen voor de index datum) was in vergelijking met geen gebruik geassocieerd met een verhoogd risico op heup- of dijbeen fracturen (OR 1,76; 95%BI 1,39-3,22), maar dit verhoogde risico verdween wanneer de dopaminerge behandeling meer dan een jaar werd gestopt. Er was geen verschil in fractuur risico tussen de typen dopaminerge geneesmiddelen. Gelijktijdig gebruik van antidepressiva verhoogde het risico op heup- of dijbeen fracturen nog verder (OR 3,5; 95%BI 2,10-5,87), terwijl er geen additioneel verhoogd risico was bij gelijktijdig gebruik met andere psychotrope geneesmiddelen. Het is mogelijk dat de gevonden associatie niet causaal is, maar toe te schrijven is aan verschillen in de ernst van de ziekte van Parkinson. We meenden toch te kunnen concluderen dat een beoordeling op fractuurrisico overwogen moet worden bij oudere gebruikers van dopaminerge geneesmiddelen. In Hoofdstuk 5 hebben we de behandeling van twee niet-motorische symptomen nader belicht. In Hoofdstuk 5.1 hebben we gekeken naar de associatie tussen initieel gebruik van serotonerge antidepressiva en veranderingen in het gebruik van anti-Parkinson geneesmiddelen. Dit is onderzocht in een retrospectief cohort binnen de PHARMO database met patiëntenregistraties over de periode 1994 tot en met 2004. In totaal werden 221 patiënten geïncludeerd die ouder dan 40 jaar waren, en die voor het eerst een antidepressivum kregen en die levodopa gebruikten in periode van minimaal 180 dagen voorafgaand aan het antidepressivum. De patiënten werden gedurende maximaal 180 dagen gevolgd. Het eindpunt was
de eerste wijziging in de anti-Parkinson behandeling, gedefinieerd als een toename van de dagelijkse dosering, de start van een nieuwe behandeling, of een wijziging van de doseringsvorm. We classificeerden de antidepressiva op twee manieren: op basis van de geneesmiddelklasse [selectieve serotonine heropnameremmers (SSRIs), tricyclisch antidepressiva (TCAs) en overige antidepressiva] of op basis van de mate van serotonine heropnameremming (hoog, middel, laag). De gecorrigeerde HR op een wijziging in de antiParkinson behandeling was 0,7 (95%BI 0,3-1,5) met SSRIs ten opzichte van TCAs. Dit was 0,9 (95%BI 0,4-2,1) met overige antidepressiva ten opzichte van TCAs. De gecorrigeerde HR op een wijziging in de anti-Parkinson behandeling voor middelen met een hoge mate ten opzichte van een lage mate van serotonine heropnameremming was 0,6 (95%BI 0,3-1,4). Dit was 0,7 (95%BI 0,3-1,4) voor middelen met een middel mate ten opzichte van een lage mate van serotonine heropnameremming. Hieruit concludeerden we dat er geen bewijs is om met SSRIs of serotonerge antidepressiva voorzichtiger te zijn ten opzichte van andere antidepressiva. In Hoofdstuk 5.2 hebben we de effectiviteit en veiligheid van behandeling met glycopyrroniumbromide onderzocht in Parkinson patiënten met overmatig speekselverlies. Overmatig speekselverlies treft tot 75% van de Parkinson patiënten. Overmatig speekselverlies wordt vaak behandeld met zogenaamde anticholinergica, maar centrale bijwerkingen, zoals verwardheid, beperken het gebruik. Glycopyrroniumbromide is ook een anticholinergicum, maar het heeft een quaternaire ammonium chemiestructuur, waardoor dit middel de bloed-hersenbarrière niet in hoge mate zal kunnen passeren. Dit verkleint de kans op centrale bijwerkingen, hetgeen een voordeel is bij patiënten met de ziekte van Parkinson, waarbij cognitieve problemen veelvuldig voorkomen. We hebben in het Medisch Spectrum Twente in Enschede een vier weken durend, gerandomiseerd, dubbelblind, placebogecontroleerd, cross-over onderzoek uitgevoerd bij 23 patiënten met de ziekte van Parkinson. De gebruikte dosering was drie maal daags 1 mg oraal glycopyrroniumbromide. De ernst van het speekselverlies werd vastgesteld met een speekselverlies scoreschaal van 1 (geen speekselverlies) tot 9 (zeer overvloedig speekselverlies). De gemiddelde (standaard deviatie) score verbeterde van 4,6 (1,7) met placebo tot 3,8 (1,6) met glycopyrroniumbromide (p=0,011). Negen patiënten (39,1%) hadden met glycopyrroniumbromide een klinisch relevante verbetering van minstens 30% tegen één patiënt (4,3%) met placebo (p=0,021). Er waren geen significante verschillen in bijwerkingen tussen glycopyrroniumbromide en placebo behandeling. We concludeerden dat drie maal daags 1 mg oraal glycopyrroniumbromide een effectieve en veilige behandeling is van overmatig speekselverlies bij patiënten met de ziekte van Parkinson. In toekomstig onderzoek moet worden overwogen om

Opleidingsinstituut de effectiviteit en bijwerkingen van hogere doseringen te onderzoeken. In Hoofdstuk 6, de algemene discussie, worden drie onderwerpen in een bredere context bediscussieerd: 1) individualiseren van behandeling met geneesmiddelen; 2) waarde van niet-experimentele versus experimentele onderzoeken; en 3) methodologie. We doen aanbevelingen voor de klinische praktijk en voor toekomstig onderzoek. Concluderend, de ziekte van Parkinson heeft een aanzienlijke invloed op het dagelijkse functioneren. In dit proefschrift hebben we diverse aspecten van de farmacologische behandeling van de ziekte van Parkinson bestudeerd, en hebben we aanbevelingen voor interventies gedaan om de uitkomsten van behandeling met geneesmiddelen te verbeteren.

Publicaties Vaginal brachytherapy versus pelvic external beam radiotherapy for patients with endometrial cancer of high-intermediate risk (PORTEC-2): an open-label, non-inferiority, randomised trial. Lancet. 2010 Mar 6;375(9717):816-23. Abstract BACKGROUND: After surgery for intermediate-risk endometrial carcinoma, the vagina is the most frequent site of recurrence. This study established whether vaginal brachytherapy (VBT) is as effective as pelvic external beam radiotherapy (EBRT) in prevention of vaginal recurrence, with fewer adverse effects and improved quality of life. METHODS: In this open-label, non-inferiority, randomised trial undertaken in 19 Dutch radiation oncology centres, 427 patients with stage I or IIA endometrial carcinoma with features of high-intermediate risk were randomly assigned by a computer-generated, biased coin minimisation procedure to pelvic EBRT (46 Gy in 23 fractions; n=214) or VBT (21 Gy high-dose rate in three fractions, or 30 Gy low-dose rate; n=213). All investigators were masked to the assignment of treatment group. The primary endpoint was vaginal recurrence. The predefined non-inferiority margin was an absolute difference of 6% in vaginal recurrence. Analysis was by intention to treat, with competing risk methods. The study is registered, number ISRCTN16228756. FINDINGS: At median follow-up of 45 months (range 18-78), three vaginal recurrences had been diagnosed after VBT and four after EBRT. Estimated 5-year rates of vaginal recurrence were 1.8% (95% CI 0.6-5.9) for VBT and 1.6% (0.5-4.9) for EBRT (hazard ratio [HR] 0.78, 95% CI 0.17-3.49; p=0.74). 5-year rates of locoregional relapse (vaginal or pelvic recurrence, or both) were 5.1% (2.8-9.6) for VBT and 2.1% (0.8-5.8) for EBRT (HR 2.08, 0.71-6.09; p=0.17). 1.5% (0.5-4.5) versus 0.5% (0.1-3.4) of patients presented with isolated pelvic recurrence (HR 3.10, 0.32-29.9; p=0.30), and rates of distant metastases were similar (8.3% [5.1-13.4] vs 5.7% [3.3-9.9]; HR 1.32, 0.63-2.74; p=0.46). We recorded no differences in overall (84.8% [95% CI 79.3-90.3] vs 79.6% [71.288.0]; HR 1.17, 0.69-1.98; p=0.57) or disease-free survival (82.7% [76.9-88.6] vs 78.1% [69.7-86.5]; HR 1.09, 0.66-1.78; p=0.74). Rates of acute grade 1-2 gastrointestinal toxicity were significantly lower in the VBT group than in the EBRT group at completion of radiotherapy (12.6% [27/215] vs 53.8% [112/208]). INTERPRETATION: VBT is effective in ensuring vaginal control, with fewer gastrointestinal toxic effects than with EBRT. VBT should be the adjuvant 90

Multiinstitutional study on target volume delineation variation in breast radiotherapy in the presence of guidelines. Radiother Oncol. 2010 Mar;94(3):286-91 Abstract PURPOSE: This study aims to determine magnitude, causes and consequences of post-operative breast tumour target volume delineation variation among radiation oncologists in the presence of guidelines. MATERIALS AND METHODS: Excision cavities, CTVs and PTVs of eight breast cancer patients were delineated on CT scans by 13 Dutch radiation oncologists (observers) from 12 Dutch institutes participating in the international Young Boost Trial. Delineated volumes and conformity indices were determined. CTV delineation variation (SD) was determined for anatomically relevant regions. Nonparametric statistics were performed to establish effects of observers, patient characteristics and regions on delineation variation. RESULTS: Even in the presence of delineation guidelines considerable delineation variation is present (0.24<SD<1.22 cm). Presence of clips or seroma reduced interobserver variation (0.24<SD<0.62 cm). Region-specific analysis showed distinct regions of higher variability per patient. This could not always be ascribed to anatomical features, suggesting interobserver variation is not solely due to lack of image quality. CONCLUSIONS: In this study, interobserver delineation variation in breast tumour target volume delineation is larger than, e.g. setup inaccuracies and results from limited reliable visual guidance as well as interpretation differences between observers, despite guidelines. Reduction of delineation variation is essential in view of current developments in planning techniques, particularly for External Partial Breast Irradiation.

The number of metastatic sites for stage IIIA endometrial carcinoma, endometrioid cell type, is a strong negative prognostic factor. Gynecol Oncol. 2010 Apr;117(1):32-6 Abstract The aim of this study was to look at the impact of the number of sites with tumour involvement on outcome for patients with stage IIIA endometrioidtype endometrial carcinoma. PATIENTS AND METHODS: 141 patients stage IIIA were included. A central histopathological review was performed. Patients staged solely on the presence of a positive peritoneal washing were excluded. Follow-up ranged from 2 to 217 months with a median of 43 months. Endpoints of the study were locoregional recurrence rates, distant metastasis-free survival (DMFS), disease-free survival (DFS) and disease-specific survival (DSS). RESULTS: In multivariate analyses the number of involved sites showed to be the only independent significant variable for DMFS, DFS, and DSS with a Hazard Ratio of 2.1, 2.2, and 2.2, respectively. The DSS was significantly related to the number of involved sites, with a 5-year DSS of 70.4% for one site, 42.8% for two sites, and 43.9% for three sites, respectively (p=0.001). CONCLUSION: The number of involved sites outside the corpus uterine for stage IIIA seems to be a strong negative prognostic factor for stage IIIA endometrial carcinoma.

Abstract Different planning and treatment systems for intracranial stereotactic radiosurgery available in the Netherlands are compared. The systems for intracranial radiosurgery include: Gamma Knife, Cyberknife, Novalis, and Tomotherapy. Electronic data of 5 patients was transferred to all participating centres and treatment plans were generated according to 2 different prescription protocols. For this study, plans were also generated for a conventional linac. Even systems with a high resolution (Gammaknife and Novalis) have conformity indices in violation with RTOG guidelines (CI > 2.5) when target volumes of <0.5 cc are treated. For medium sized targets (0.5-1 cc) all systems performed reasonably well, but for the different systems a large 92
range of conformity indices was seen (1.1 to 3.7). The differences are partly system dependent but depend also on specific planning choices made. For larger target volumes (> 1 cc), all systems perform well. The workload of the different techniques was comparable although the treatment times were usually longer for Gamma Knife radiosurgery. We conclude that small targets should be treated by dedicated systems, larger volumes (> 0.5-1 cc) can also be treated using conventional treatment systems equipped with a MLC.

Publicaties Fear of falling after hip fracture: a systematic review of measurement instruments, prevalence, interventions, and related factors. Visschedijk J, Achterberg W, Van Balen R, Hertogh C. J Am Geriatr Soc. 2010 Sep;58(9):1739-48 Abstract The objective of this review was to systematically describe and analyze fear of falling (FoF) in patients after a hip fracture, focusing on measurement instruments for FoF, the prevalence of FoF, factors associated with FoF, and interventions that may reduce FoF. Fifteen relevant studies were found through a systematic literature review, in which the PubMed, Embase, PsychINFO, and CINAHL databases were searched. Some of these studies indicated that 50% or more of patients with a hip fracture suffer from FoF, although adequate instruments still have to be validated for this specific group. FoF was associated with several negative rehabilitation outcomes, such as loss of mobility, institutionalization, and mortality. FoF was also related to less time spent on exercise and an increase in falls, although knowledge about risk factors, the prevalence over a longer time period, and the exact causal relations with important health outcomes is limited. Most studies suffer from selection bias by excluding patients with physical and cognitive disorders. Hence, more research is required, including in patients who are frail and have comorbidities. Only when knowledge such as this becomes available can interventions be implemented to address FoF and improve rehabilitation outcomes after a hip fracture.

Wetenschappelijk onderzoek naar COPD-revalidatie binnen het verpleeghuis is wenselijk. Welke groep heeft baat bij een revalidatietraject, welke behandeling is zinvol, met welk effect en wat is het beloop op de (middel) lange termijn?
