Dankwoord
Chapter 3: A highly sensitive flow cytometric method for functional analysis of the apoptosis pathways in isolated lymphoma cells and related hematopoietic cells
Figure 2: Validation of the 7AAD-beads method for detection of apoptosis (E) May-Grünwald Giemsa staining of cytospin preparations of Nalm6 cells treated with 500 nM etoposide for 16 hours. Levels of apoptosis were comparable with the obtained results using the 7AAD-beads method.
Chapter 4: Inhibition of the intrinsic apoptosis pathway downstream of caspase 9 activation is a cause for chemotherapy resistance in diffuse large B-cell lymphoma
Figure 1: High expression of pro- and anti-apoptosis regulating genes in a subset of DLBCL samples and cell lines (A) RT-MLPA analysis of apoptosis-regulating gene expression in isolated DLBCL cells. The results following unsupervised cluster analysis are shown. Two groups of DLBCL could be identified: one with relatively low levels of both pro- and anti-apoptotic gene expression, and one with high levels of apoptosisregulating gene expression. The expression levels of BCL-G, Harakiri, cIAP, and BIRC7 were too low or not detectable. Cluster analysis was restricted to those genes that were expressed in at least 80% of the samples. Omitted genes were BNIP3 and Bik. (B) RT-MLPA analysis of apoptosis-regulating gene expression in cell lines. Unsupervised cluster analysis was performed. Similarly to the DLBCL samples, two groups could be identified: one group with high levels of apoptosis regulating gene expression, including AIF, BAD, BAK, BIM, MCL1-long and survivin, and one group with low apoptosis-regulating gene expression. Expression levels of BCL-G, Harakiri and BIRC7 were too low or not detectable. (C) Immunohistochemical detection of Bim, XIAP, and MCL1 in DLBCL samples with low or high expression of pro and anti-apoptotic genes. Original magnification: 400X. (D) Caspase 9 activity was detected using a luminescent assay in the isolated lymphoma cells of eighteen DLBCL samples. Experiments were done in triplicate. DLBCL samples with relatively high expression of pro-apoptotic genes show high expression of caspase 9 activity (p=0.008). Peripheral blood B-cells and GCB cells demonstrate lower levels of caspase 9 activity. 138

Chapter 6: Human soluble TRAIL/Apo2L induces apoptosis in a subpopulation of chemotherapy refractory nodal diffuse large B-cell lymphomas, determined by a highly sensitive in vitro apoptosis assay
Figure 4: hsTRAIL/Apo2L induces apoptosis in Etoposide resistant B-cell lymphoma/ leukaemia derived cell lines. (A) Caspase dependent cell death in B-cell lines. B-cell lines were incubated for 4, 8, 16 and 24 hours with 25 ÌM Etoposide and 100 ng/ml hsTRAIL/Apo2L respectively. The percentage cell death was quantified using FACS analysis. Indicated is the percentage cell death at 24 hours incubation. Samples are sorted from left to right according to sensitivity to Etoposide. Similar to DLBCL samples hsTRAIL/Apo2L also induced apoptosis in Etoposide resistant cell lines. (B) Caspase 3/7 activity in relation to induction of apoptosis in an Etoposide (SMS-SB) and hsTRAIL/Apo2L (8226) sensitive cell line respectively. Caspase 3/7 activity was determined with a fluorimetric assay at 4, 8, 16 and 24 hrs of incubation. Bars represent percentage cell death. Line represents levels of caspase 3/7 activity (Log scale). Caspase 3/7 activity precedes Etoposide and HsTRAIL/Apo2L induced apoptosis by 4 to 12 hours. (C) Immunocytochemical detection of caspase 3 activation following 4 hours incubation with Etoposide in SMS-SB (left panel) and hsTRAIL/Apo2L in 8226 (right panel), confirming caspase 3 activity in these cell lines. 140
Chapter 7: Chemotherapy-refractory diffuse large B-cell lymphomas (DLBCL) are effectively killed by CD20 antibody-induced complement-mediated cytotoxicity
Figure 3: CD20 expression levels do not correlate with sensitivity to CDC-mediated cell death by ofatumumab or rituximab in DLBCL (A) CD20 expression was detected in ten DLBCL samples using immunohistochemistry. Expression of CD20 in a DLBCL sample (L13, top) with a high LD50 value for ofatumumab and a case (L24, bottom) with a low LD50 value are represented. (B) Flow cytometric analysis for CD20 expression in DLBCL case L13 with low CD20 expression (top) and DLBCL case L24 with high CD20 expression (bottom). Black peaks depict isotype control staining and white peaks correspond to staining with CD20 antibody. (C) Percentage positive CD20 stained cells in relation to % cell death of ten DLBCL samples tested, incubated with 0.1 g/ml ofatumumab ( ) or rituximab (), using flow cytometric analysis. (D) Expression of CD20 in relation to % cell death of ten DLBCL samples tested, incubated with 0.1 g/ml ofatumumab ( ) or rituximab (), using flow cytometric analysis. The Relative Fluorescence Intensity (RFI) was calculated as Mean Fluorescence Intensity (MFI) of CD20 /MFI IgG1 isotype control.
141
Chapter 7: Chemotherapy-refractory diffuse large B-cell lymphomas (DLBCL) are effectively killed by CD20 antibody-induced complement-mediated cytotoxicity
Figure 4: CD59 expression levels correlate with sensitivity to CDC-mediated cell death by ofatumumab or rituximab in DLBCL (A) CD59 expression was detected in 10 DLBCL samples using immunohistochemistry. Expression of CD59 in a DLBCL sample (L13, left) with a high LD50 value for ofatumumab and a case (L24, right) with a low LD50 value for ofatumumab are represented. (B) Patient samples were divided into two groups based on their CD59 expression (negative or positive). The LD50 values for ofatumumab (left) or rituximab (right) were plotted at the Y-axis. Horizontal lines represent median CD59 expression levels for each group. (C) DLBCL case L7 was lysed with 10 g/ml ofatumumab ( , left) or rituximab ( , right) and NHS in the presence or absence of 10 g/ml blocking CD46 mAb, CD55 mAb and/or CD59 mAb. Cell death was measured using the 7AAD-beads FACS method. The blocking mAb did not induce CDC by themselves and isotype controls showed no increase in CDC-mediated lysis in both lymphomas (data not shown). 142
Chapter 8: Discussion and future perspectives
Figure 2: Figure 2. The intrinsic apoptosis pathway is constitutively activated in a subset of chemotherapy DLBCL with downstream disruption of this pathway. Previous RT-MLPA analysis revealed two groups of DLBCL: one with relatively low expression levels of both pro- and antiapoptotic genes (low apoptosis profile) and one group of DLBCL with relatively high expression levels of pro-apoptotic genes in combination with high expression levels of anti-apoptotic genes (high apoptosis profile). DLBCL with a high apoptosis profile frequently appeared to be chemotherapy-refractory and were characterized by spontaneous caspase 9 activation without induction of caspase 3 activation or apoptosis (A). Chemotherapy failed to induce apoptosis in DLBCL with a high apoptosis profile, due to inhibition of caspase 3 by XIAP protein expression (B). Low apoptosis profile cells showed no spontaneous caspase activation or apoptosis, only after induction with chemotherapy, induction of apoptosis was observed. (red= activated; green= inactivated)
