Nepoměr velikosti horních a dolních frontálních zubů měřený malým Boltonovým indexem Tooth size discrepancv of upper and lower frontal teeth measured with small Bolton index

Bylo také zjišťováno ujaké části ortodontických pacientů je větší masa zubů v dolním nebo naopak v horním frontálním úseku chrupu. Procento ortodontických pacientů s hodnotou malého Boltonova indexu mimo rozmezí dvou směrodatných odchylek od průměru je klinicky významné a má v našem souboru 500 pacientů hodnotu 27,4 %. V praxi to znamená, že přibližně u každého čtvrtého ortodontického pacienta lze očekávat výrazný nepoměr ve velikosti frontálních zubů. Z této skupiny pacientů mají dvě třetiny větší masu zubů v dolním frontálním úseku a jedna třetina větší masu zubů v horním frontálním úseku. Z toho vyplývá, že na konci ortodontické léčby je možnost výskytu neestetických mezer ve frontálním úseku chrupu v důsledku signifikantního nepoměru šířek zubů přibližně dvakrát častější v horním zubním Oblouku (Ortodoncie 2006,15, č. 5, s. 22-29).
Abstract The aim of the study was to find the percentage of orthodontic patients whose discrepancy in the width of the upper and lower frontal segment evaluated according to Bolton small index may affect the plan of orthodontic treatment as well as the treatment result. We also strove to find out in what part of orthodontic patients there is the bigger tooth mass in the upper or in the lower frontal segment of dentition. The portion of orthodontic patients with Bolton small index beyond two standard deviations from the average is clinically significant, in our sample of 500 patients it makes up for 27.4%. This means that approximately every fourth orthodontic patient may manifest expressive discrepancy of the size of frontal teeth. In this group of patients, two thirds have bigger tooth mass in the lower frontal segment, and one third in the upper frontal segment. This fact suggests that at the end of orthodontic treatment there may be spaces in frontal dental segment due to the significant discrepancy of the width of teeth. The spaces occur twice as often in the upper dental arch (Ortodoncie 2006,15, No. 5, p. 22-29).

Úvod Důležitými cíly komplexní ortodontické léčby je dosažení optimální konečné okluze, hloubky skusu a incizálního schůdku. Je mnoho faktorů, které budou ovlivňovat reálnou dosažitelnost těchto cílů. Jedním z nich je velikost zubů. Problém se týká jak délky, tak šířky zubů. Nesprávná velikost korunek zubů vytváří estetický a funkční problém v zubním oblouku, pokud chceme dosáhnout správné okluze. K posouzení velikostních vztahů korunek zubů se s výhodou používají Boltonovy indexy. Velký Boltonův index nás informuje o celkové diskrepanci v zubním oblouku, zatímco malý Boltonův index se týká frontálního úseku chrupu. Signifikantní odchylky hodnot těchto indexů by měly být v rámci léčebného plánu vyhodnoceny a řešení nepoměru šířekzubů potom včas naplánováno. V úvahu přicházejí fotokompozitní dostavby, protetické rekonstrukce, stripping nebo recontouring korunek zubů [1]. Vztah masy horních a dolních frontálních zubů je ovlivňován hloubkou skusu [2], angulací, torzí a šířkou zubů frontálního úseku chrupu. Angulace řezáků a špičáků částečně ovlivňuje velikost prostoru, který vyplňují frontální zuby. Andrews ve své práci o šesti klíčích okluze upozornil na skutečnost, že prostor, který je vyplněn korunkami, jež jsou meziodistálně skloněné, je větší, než když jsou napřímené. Tento efekt je větší u korunek obdélníkovitých než u soudkovitých (ovoidních) [1]. Na prostor vyplněný horními a dolními řezáky má také vliv torze. Obecně platí, že při větší palatinálnítorzi kořene vyplní jejich korunky více místa. Nejčastější problém nesouhry šířek zubů v klinické praxi je, že v určitém úseku mezi horními frontálnízuby zůstávají tremata. Pokud nepoměr mezi šířkami zubů horního a dolního frontálního úseku není výrazný, lze ho v některých případech vyrovnat rovněž palatinální torzí kořenů horních řezáků případně v kombinaci s labiální torzí kořenů dolních řezáků [1]. Obecně lze říci, že přidání 5° palatinální torze na kořeny horních řezáků způsobí zvětšení prostoru vyplně-

Introduction Amongst the important objectives of orthodontic treatment are optimal occlusion, overbite and overjet. There are many factors that can influence the availability of these objectives. Tooth size is one of them. It includes length as well as width of teeth. Incorrect size of crowns is the esthetic and functional problem in the appearance of the dental arch in case we aim at correct occlusion. To assess the size relations of tooth crowns we use Bolton indexes. The great Bolton index informs about the overall discrepancy within the dental arch, while Bolton small index refers to the frontal segment of dentition. Significant deviations of the values of these indexes should be evaluated during the phase of treatment planning and therefore the solution of tooth width discrepancy should be planned well ahead. We may consider photo composite build-ups, prosthetic reconstructions, stripping or recontouring of tooth crowns [1]. The relation of mass of the upper and lower frontal teeth is influenced by the overbite [2], angulation, torque, and the width of frontal teeth. Angulation of incisors and canines are partially affected by the space which is filled in with frontal teeth. In his study about the six keys to normal occlusion, Andrews points to the fact that the space filled in with crowns that incline mesiodistally, is larger than in case the crowns are upright. This effect is more profound in case of rectangular crowns than ovoid crowns [1]. The space filled in with upper and lower incisors is also affected by torque. In general, in case of greater palatal torque of the root, the crowns fill in more space. In clinical practice, the most common problem in tooth width discrepancy is that the spaces remain between upper frontal teeth. If the discrepancy between the width of the upper and lower frontal teeth is not significant, it is possible to solve it with palatal torque of maxillary incisors, sometimes combined with labial torque of the roots of lower incisors [1].

Jak již uvedeno, důležitým faktorem je také tvar korunek řezáků [1]. Šířka zubů ve frontálním úseku chrupu, respektive vzájemný poměr masy horních a dolních frontálních zubů, hraje důležitou roli při vytváření optimální okluze a při dosahování co nejestetičtějšího výsledku v rámci ortodontické terapie [3]. Vzájemný poměr masy horních a dolních frontálních zubů nám určuje malý Boltonův index. Jeho průměrná hodnota je 77,2 % se směrodatnou odchylkou 1,65%, takže rozmezí odpovídající jedné směrodatné odchylce je od 75,6 % do 78,8 %. Každá hodnota mimo toto rozmezí poukazuje na nepoměr mezi masou zubů horního a dolního frontálního úseku při vytváření pravidelné okluze. Prakticky to znamená, že pacienti s hodnotou malého Boltonova indexu v rozmezí jedné směrodatné odchylky by neměli mít po dosažení pravidelné okluze mezery ve frontálním úseku chrupu (Obr. 1). Pacienti s hodnotou malého Boltonova indexu vyšší než 78,8 % mají po dosažení pravidelné okluze mezery v horním frontálním úseku chrupu (Obr. 2) a pacienti s hodnotou malého Boltonova indexu nižší než 75,6 % mají po dosažení pravidelné okluze mezery v dolním frontálním úseku (Obr. 3) [2].
In general, the application of 5° torque on the roots of maxillary incisors results in the increase of the space filled in with maxillary incisors by 1 mm. The application of palatal torque to the roots of small teeth will be less effective than the same torque applied to large teeth. Another important factor is the shape of crowns [1]. The tooth width in frontal segment of dentition, the relation of the mass of the upper and lower frontal teeth respectively, plays the important role in creating optimum occlusion and in striving to achieve the best possible esthetic result within the orthodontic treatment [3]. The relation of the mass of upper and lower frontal teeth is given by Bolton small index. Its mean value is 77.2%, standard deviation (SD) 1.65%, so that the range corresponding to one standard deviation is between 75.6% and 78.8%. Any value beyond this range suggests discrepancy between the mass of the teeth of upper and lower front segment during creation of regular occlusion. This means that patients with Bolton small index value within one standard deviation should not have any spaces in the frontal segment of dentition afterthe regular occlusion is achieved (Fig. 1). Patients with Bolton small index value over 78.8% have spaces in upper frontal teeth (Fig. 2), and patients with Bolton small index below 75.6% have spaces in the lower frontal segment (Fig. 3) [2]. nými stálými zuby od špičáku po špičák v horním i dolním zubním oblouku. Analýza velikosti korunek zubů ve frontálním úseku chrupu byla prováděna ve dvou etapách. Nejprve byly změřeny maximální meziodistální šířky klinických korunek zubů 13, 12, 11, 21, 22, 23, 33, 32, 31, 41, 42, 43 na studijních modelech posuvným měřidlem s ostrými hroty s přesností odečítání na 0,1 mm. Měření prováděl 1 člověk po dobu 14 dnů. Pro analýzu chyby měření bylo s odstupem 1 měsíce znovu proměřeno 10 náhodně vybraných vzorků souboru a byl vypočítán koeficient spolehlivosti. Koeficient spolehlivosti má hodnotu 0,99. To znamená, že chyba měření je zanedbatelná. Pak byly provedeny výpočty malého Boltonova indexu pro celý soubor. Vzorec pro výpočet malého Boltonova indexu je (3,2,1 -1,2,3 / 3,2,1 + 1,2,3) x 100. Data byla zpracována na osobním počítači v programu Microsoft Office Excel plug-in Statistická analýza. Byla vypočtena průměrná hodnota malého Boltonova indexu, medián, rozmezí hodnot, směrodatná odchylka (SD) a chyba průměru pro celý soubor a pro obě pohlaví samostatně. Získané hodnoty malého Boltonova indexu byly dále vyhodnocovány a zjišťovalo se jejich procentuální zastoupení v rozsahu jedné a dvou směrodatných odchylek nebo mimo rozmezí dvou směrodatných odchylek od Boltonem stanovených hodnot. Průměr Boltonova souboru je 77,2 % se směrodatnou odchylkou 1,65 %. Hodnoty malého Boltonova indexu mimo rozmezídvou směrodatných odchylek byly brány jako klinicky významná odchylka nepoměru šířek zubů, která vyžaduje úpravu. To po přepočtu přibližně odpovídá diskrepanci 2-3 mm. Na základě této úvahy bylo zjištěno procento jedinců, u kterých nepoměr v šířce zubů horního a dolního frontálního úseku pravděpodobně ovlivní plán ortodontické léčby nebo jejívýsledek. Tyto výpočty byly samostatně provedeny i pro obě pohlaví. Rozdíl mezi zastoupením mužů a žen s hodnotou malého Boltonova indexu mimo rozmezídvou směrodatných odchylek byl vyhodnocen pomocíchí-kvadrát testu na hladině významnosti 0,05. Vybrané hodnoty byly nakonec porovnány s jinými studiemi.

gment of dentition. Fully erupted permanent teeth from canine to canine in both the upper and lower dental arches were necessary. The analysis of the tooth size in frontal segment was performed in two phases. First, the maximum mesiodistal width of clinical crowns of teeth 13,12,11, 21,22, 23, 33, 32, 31,41, 42, 43 was measured on the casts with the caliper rule with sharp tips, with the accuracy of 0.1 mm. Measurements were performed by one person during a fortnight. To examine the measurement error, 10 randomly chosen models were measured again after one month, and the reliability coefficient was determined. The reliability coefficient is 0.99, which means the measurement error is negligible. Then Bolton small index was calculated for the whole sample. The formula: (3, 2, 1 - 1, 2, 3/3, 2, 1 + 1,2, 3) x 100. The data obtained were processed in the programme Microsoft Office Excel plug-in Statistic analysis. The following data were calculated - mean value of Bolton small index, median, range of values, standard deviation, the error of the mean for the whole sample and for each sex separately. The obtained values of Bolton small index were further evaluated, their percentage within the range of one or two SD or beyond the range of two standard deviations from Bolton values were appraised. The mean value in Bolton sample is 77.2 %, standard deviation of 1.65%. The values of Bolton small index beyond the range of two standard deviations were considered as the clinically significant deviation of tooth width discrepancy that must be adjusted. This corresponds to the discrepancy of 2-3 mm. Then the proportion of individuals in which the tooth size discrepancy in frontal teeth will impact the plan or treatment result. The calculations were carried out also for each sex separately. The difference between the number of men and women with the value of Bolton small index beyond the range of two standard deviations was evaluated by means of Chi square test with the level of significance 0.05. Selected values were then compared with the data from other studies.

Výsledky Průměrná hodnota malého Boltonova indexu je u celého našeho souboru ortodontických pacientů 77,80% se směrodatnou odchylkou 3,14%, u mužů 78,20% se směrodatnou odchylkou 3,00% a u žen 77,40% se směrodatnou odchylkou 3,24%. Rozmezí hodnot je pro celý soubor od 66,70% do 86,70%, pro muže od 68,60% do 86,70% a pro ženy od 66,70% do 85,70% (Tab. 1). Procentuální zastoupení indexu v rozsahu jedné a dvou směrodatných odchylek nebo mimo rozmezí dvou směrodatných odchylek vzhledem k Boltonem stanoveným hodnotám je zobrazeno v grafech jak pro celý soubor (Obr. 4), tak zvlášť pro muže (Obr. 5) a ženy (Obr. 6). V celém souboru má 41,2 % pacientů hodnotu malého Boltonova v rozmezí jedné směrodatné odchylky, 31,4 % v rozmezí mezi jednou a dvěma směrodatnými odchylkami a 27,4 % mimo rozmezí dvou směrodatných odchylek (Obr. 7). Právě hodnota 27,4 % nám udává procentuální zastoupení ortodontických pacientů, u kterých nepoměr v šířce zubů horního a dolního frontálního úseku pravděpodobně ovlivní plán léčby nebo její výsledek. U mužů je to 28,4 % a u žen 26,4 % (Obr. 8, 9). Jejich Boltonův malý index je větší než 80,5 % nebo menší 26

Results In the whole sample of orthodontic patients the mean value of Bolton small index was 77.8%, SD 3.14%; for males the value was 78.2%, SD 3.00%, for females 77.4%, SD 3.24%. The range of values for the whole sample between 66.7% and 86.7%; in males 68.6%-86.7%, in females 66.7%-85.7% (Table 1). The proportion of the values within the range of one and two standard deviations or beyond this range according to Bolton is given in the diagrams forthe whole sample (Fig. 4), for males only (Fig. 5) and females only (Fig. 6). Within the sample, 41.2% of patients have the value of Bolton small index within the range of one standard deviation, 31.4% within one and two standard deviations, and 27.4% beyond the range of two standard deviations (Fig. 7). The number 27.4% gives the proportion of orthodontic patients in which the tooth size discrepancy in upper and lower frontal teeth most probably affects the treatment plan or the treatment result. In males the number is 28.4%, in females 26.4% (Fig. 8,9). Their Bolton small index is over 80.5% or less than 73.9%. než 73,9 %. Hodnotu malého Boltonova indexu větší než 2 směrodatné odchylky má 18,4 % (ti mají větší masu zubů v dolním frontálním úseku), hodnotu menší než 2 směrodatné odchylky má 9,0 % pacientů (ti mají větší masu zubů v horním frontálním úseku) (Obr. 4). Při porovnávání počtu mužů a žen s hodnotou malého Boltonova indexu mimo rozmezí dvou směrodatných odchylek pomocí chí-kvadrát testu na hladině významnosti 0,05 nebyl zjištěn statisticky signifikantní rozdíl mezi těmito soubory. Vybrané hodnoty byly porovnány s podobnými studiemi (Tab. 2 a 3). Jednalo se o průměrnou hodnotu malého Boltonova indexu celého souboru (Šrytr 77,8 % ,Bolton 77,2 %, Stifter 78,2 %, Crosby a Alexander 77,5 %, Freeman77,8%, Santorová78,1 %), o směrodatnou odchylku, o rozmezí hodnot malého Boltonova indexu a o procentuální zastoupení ortodontických pacientů s hodnotou malého Boltonova indexu mimo rozmezí dvou směrodatných odchylek (Šrytr 27,4 %, Crosby aAlexander30,6 %, Freeman22,9 %, Santorová28,0 %). Při porovnávání průměrné hodnoty malého Boltonova indexu našeho souboru se soubory dalších autorů lze konstatovat, že si přibližně odpovídají. Směrodatná odchylka malého Boltonova indexu našeho souboru jev porovnáníse studiemi Boltona a Stiftera větší, taktéž rozmezí hodnot malého Boltonova indexu je v porovnání s Boltonovou studií větší.

Comparison of males and females with the value of Bolton small indexoutsidethe two standard deviations by means of Chi square test with the level of significance 0.05, there was not determined statistically significant difference between males and females. The data were compared with similar studies (Table 2,3). The following data were compared: the mean value of Bolton small index for the whole sample (Srytr 77.8%, Bolton 77.2%, Stifter 78.2%, Crosby et Alexander 77.5%, Freeman 77.8%, Santorova 78.1%), standard deviation, range of the values of Bolton small index, proportion of patients with the value of Bolton small index beyond the range of two standard deviations (Srytr 27.4%, Crosby and Alexander 30.6%, Freeman 22.9%, Santorova 28.0%). We may conclude that the mean values of Bolton small index given by other authors correspond to the values given in our study. Standard deviation of Bolton small index of our set is bigger, compared with the numbers given by Bolton and Stifter, the range of the values of Bolton small index is also larger in comparison with Bolton's work.

Diskuse Zatímco zjištěná průměrná hodnota malého Boltonova indexu v naší práci je blízká té, kterou zjistili Bolton a Stifter, směrodatná odchylka a rozmezí hodnot indexu jsou větší. To může být vysvětleno rozdílným typem zkoumaného souboru. Bolton použil soubor 55 pacientů s ideální okluzí. U žádného s pacientů nebyl primárně zřetelný nepoměr v šířce frontálních zubů charakterizovaný mezerami nebo stěsnáním. Proto jsou směrodatná odchylka a rozmezí hodnot malého Boltonova indexu výrazně menší. Zjišťované procento ortodontických pacientů s hodnotou malého Boltonova indexu mimo rozmezídvou směrodatných odchylek je klinicky signifikantní a má hodnotu 27,4 % (Crosby a Alexander 30,6 %, Freeman 22,9 %, Santorová28,0 %). V praxi to znamená, že přibližně u každého čtvrtého pacienta můžeme očekávat výrazný nepoměr ve velikosti frontálních zubů. Dále bylo zjištěno, že 18,4 % (Crosby a Alexander 13,8 %, Freeman 19,7 %) těchto pacientů má větší masu zubů v dolním frontálním úseku a 9,0 % (Crosby a Alexander 9,2 %, Freeman 10,8 %) z nich má větší masu zubů v horním frontálním úseku. Z uvedeného vyplývá, že ve shodě s Freemanovou studií by měla být na konci ortodontické léčby možnost výskytu neestetických mezer ve frontálním úseku v důsledku signifikantního nepoměru šířek zubů přibližně dvakrát častější v horním zubním oblouku. Při porovnávání počtu mužů a žen s hodnotou malého Boltonova indexu mimo rozmezídvou směrodatných odchylek nebyl zjištěn statisticky signifikantní rozdíl. Z toho vyplývá, že není rozdíl mezi pohlavími 28

Discussion The mean value of Bolton small index found in our work is similar to that identified by Bolton and Stifter, however, standard deviation and the range of index values are bigger. This may be explained by the different type of the sample. Bolton worked with the sample of 55 patients with the ideal occlusion. In no patient there was expressed discrepancy in the width of frontal teeth characterized by spaces or crowding. Therefore, the standard deviation and the range of values of Bolton small index are significantly smaller. Proportion of orthodontic patients with the value of Bolton small index beyond the range of two standard deviations is clinically significant, i.e. 27.4%. In practice this means that we can expect significant discrepancy in size of frontal teeth in approximately every fourth patient. Further we found out that 18.4% (Crosby and Alexander 13.8%, Freeman 19.7%) of the patients have greater mass of dentition in the lower frontal segment, and 9.0% (Crosby and Alexander 9.2%, Freeman 10.8%) have greater mass of dentition in the upper frontal segment. From the above said it follows that - in agreement with the study by Freeman - at the end of the orthodontic therapy there is possibility of non-esthetic spaces in frontal segment approximately twice as frequent in the upper dental arch. Comparison of male and female patients with the value of Bolton small index beyond the range of two standard deviations did not prove any statistically relevant difference. ve výskytu malého Boltonova indexu nad dvě směrodatné odchylky a výskyt výrazného nepoměru v šířce korunek zubů ve frontálním úseku je u mužů a žen přibližně stejný. Co stojí za pozornost, je rozložení hodnot malého Boltonova indexu mimo rozmezí dvou směrodatných odchylek v souboru mužů a žen (Obr. 5 a 6). Zatímco u žen je rozložení hodnot malého Boltonova indexu nad 80,5 % a pod 73,9 % přibližně stejné, u mužů výrazně převládají hodnoty malého Boltonova indexu nad 80,5 % (větší masa zubů v dolním frontálním úseku). Když je hodnota malého Boltonova indexu větší než 78,8 %, je malý jeden nebo více horních frontálních zubů, nebo jsou větší dolní frontální zuby. Pokud jsou velikost a proporce horních frontálních zubů správné, řešeníspočívá ve stripování dolních řezáků, případně i špičáků. Naopak, pokud má jeden nebo více horních frontálních zubů menší velikost, je třeba horní zuby doplnit do správného rozměru a ortodonticky pro to vytvořit podmínky. Když je hodnota malého Boltonova indexu menší než 75,6 %, jsou buď horní frontální zuby příliš veliké, nebo naopak dolní frontální zuby příliš malé. Řešení pak spočívá ve stripování zubů horního frontálního úseku, nebo v dostavbě dolních řezáků. Vždy přitom ale musíme zohledňovat tvary a proporce zubů.
Noticeable is the distribution of the values of Bolton small index beyond the range of two standard deviations in males and females (Fig. 5 and 6). In women the distribution of the values of Bolton small index above 80.5% and below 73.9% is approximately the same, in males the values above 80.5% prevail significantly (i.e. greater mass of dentition in the lower dental arch).
Závěr Výsledky ukazují, že u poměrně velké části ortodonticky léčených pacientů nacházíme nepoměr mezi velikostmi horních a dolních frontálních zubů, který ovlivňuje estetický výsledek léčby. V souboru ortodontických pacientů, který jsme zpracovali, se jednalo o 27,40% jedinců. V praxi to znamená, že u více než čtvrtiny ortodontických pacientů bude pravděpodobně třeba velikost zubů upravovat. U menších odchylek v poměru velikostí zubů často vyhovuje použití stripovací techniky, u větších odchylek jsou vhodné estetické fotokompozitní dostavby, fasety nebo korunky. Vždy je třeba typ řešení zvažovat individuálně s ohledem na velikost, tvar, proporce, symetrii a biologický faktor zubů.
The results suggest that the tooth size discrepancy between upper and lower frontal teeth is found in relatively high proportion of orthodontic patients. The discrepancy affects the esthetic result of the treatment. In our sample of orthodontic patients the proportion was as high as 27.4%. It means that the size of teeth should be adjusted probably in more than one fourth of orthodontic patients.
If the value of Bolton small index is over 78.8%, one or more upper frontal teeth are small, or the lower frontal teeth are bigger. In case the size and proportion of the upper frontal teeth are correct, the problem can be solved by stripping of the lower incisors, and sometimes canines, as well. On the contrary, if one or more upper teeth are of smaller size, it is necessary to buildup the upper teeth to obtain the correct dimension. If the value of Bolton small index is below 75.6%, then the upper frontal teeth are either too big or the lower teeth too small. We can solve the problem by stripping the upperteeth or by building-up of lower incisors. We have to consider the shape and proportion of teeth. Conclusion
In case of minor discrepancies we may apply stripping, in case of greater discrepancies the esthetic photo composite build-ups, veneers or crowns are suitable. The type of solution must be considered individually for each patient with regard to the size, shape, proportion, symmetry and biological factor of teeth.
