SUMMARY Dissociation in epilepsy as a psycho-organic process is understood analogically as organic dissociation according to 10. ICD (F 06.5). On the mental level dissociation is often defined as inability to integrate some psychic contents, for example feelings or thoughts, into the consciousness. Dissociative states may be elicited by organic damage, action of drugs or psychic trauma. We examined dissociation in 21 epileptic patients by means of the Czech version of the questionnaire Dissociative Experiences Scale (DES). We found the median DES score 12,32, which is significantly higher than in normal population (median DES score 4,38). Our results are comparable with other DES testings of epileptic patients and suggest important role of psychic traumas in epileptic etiology. Key words: epilepsy, dissociation, Dissociative Experiences Scale SOUHRN V článku je chápána disociace v případech epileptických pacientů jako psycho-organický proces. Na psychické úrovni bývá disociace často definována jako neschopnost integrovat některé psychické obsahy, například některé city nebo myšlenky do vědomí. Disociované stavy mohou být vyvolány organickým poškozením, působením drog nebo psychickým traumatem. Zkoumali jsme disociaci u 21 epileptických pacientů prostřednictvím české verze dotazníku Dissociative Experiences Scale (DES). Náš výsledek, který představuje medián DES skóre, je 12,32 a představuje tak signifikantně vyšší hodnotu, než se vyskytuje u normální populace, která má medián DES skóre 4,38. Naše výsledky jsou srovnatelné s výsledky dosaženými v jiných klinických testováních epileptických pacientů a upozorňují spolu s dalšími pracemi na neopominutelnou roli psychické traumatizace v epileptické etiologii. Introduction Dissociation is a process of splitting psyche into its parts. We distinguish physiological and pathological dissociation as a consequence of innate dispositions to certain types of reactions, which were called in ethology as patterns of behaviour. In psychological point of view these reactions mean different states of the psyche in its relationship to some stimulus from the external world. These states are distinguished by characteristic emotions and thinking activity. Consciousness represents a tendency to unifying many different parts of the psyche into the whole, for example psyche of a child may be split or dissociated in pathological sense by adversity of its parents, which are objects of its identification and attachment. Unity of child’s parents is thus a necessity for unity of its mind. In general, intra-psychic conflict is a cause of splitting. Consciousness has an integrative role and dissociation leads to lowering mental level („abaissement du niveau mental“) and superiority of automatism (Janet, 1890). The concept of dissociation was first fully described by Pierre Janet (Janet, 1890; Ellenberger, 1970; van der Hart and Friedman, 1989; Havens, 1966) but he had many famous precedors and followers as is shown in references (Ellenberger, 1970; Hilgard, 1974; Meares, 1999; Bob, 2000). The term dissociation originates from disassociation, which means disconnecting of associated psychic contents, for example any contents which are connected with emotionally painfull events. During free associations in the verbal association test these events are connected to longer reaction on stimulus word (Jung, 1973). Investigation of dissociation originates from the study of hypnosis and it is connected with the phenomenon of multiple personality which can be grasped as a model of extremely dissociated personality. There are many connections between multiple personality and epilepsy, above all in cases of temporal lobe epilepsy seizures, which are often connected with changing personality (Mesulam, 1981; Benson et al., 1986; Schenk and Bear, 1981; Devinsky et al., 1989). For example, patient during the temporal lobe seizure bought the ticket on the train and went to some place. After this poriomanic state, which is an equivalent of temporal lobe epilepsy, generalization of the epileptic process in the form of
grand mal followed. After the grand mal seizure dissociated state disappearred and the patient did not know where he was (Faber, 1998). In etiology of epileptic seizures psychic traumas may be often important (Stevens, 1959; Faber and Bob, 2000). Patient B.I.L, for example, had the grand mal seizure usually connected with the dream from her child age when she was punished by denying of favourite meal (Faber, 1995) and seizures occurred more often after the quarrels with her husband. Some clinical findings thus points to the dissociation as an important factor of epileptic etiology. Dissociative symptoms occur also in interictal stages and show that differential diagnostics between psychogenic and typically epileptic seizures in many cases is not possible and that the influence of psychic traumas in epileptic etiology should be considered. A quantitative measurement of dissociation was firstly enabled by the questionnaire Dissociative Experiences Scale (DES) (Bernstein and Putnam, 1986), validity of which was repeatedly confirmed. In the present, there are also other questionnaires for the measurement of dissociation (Frankel, 1996; Riley, 1988; Pribor et al, 1993; Warshaw et al., 1993; Bowman and Coons, 1992; Ross and Joshi, 1992; Nijenhuis et al., 1996; Nijenhuis et al., 1997). Dissociative Experiences Scale Dissociative Experiences Scale (DES) is a questionnaire with 28 items, which mapps dissociative experiences occurring in general population as well as in psychopathological cases. DES is aimed to measure main characteristics of dissociation, i.e., depersonalization, derealization, amnesia, gaps in awareness, absorption and imaginative involvement (Alper et al., 1997). 1) Depersonalization and derealization is covered by following items: 11. The experience of looking in a mirror and not recognizing themselves. 12. The experience of feeling that other people, objects, and the surrounding world are not real. 13. The experience of feeling that the body does not belong to them. 16. The experience of being in a familiar place, but finding it strange and unfamiliar. 22. Some people feel and act as if they were two different people. 27. Hearing voices inside the head ordering to do things or commenting person’s activity. 28. Feeling as if looking at the world through a fog so that people and objects appear far away or unclear. 2) Amnesia/inattention: 2. Listening to someone’s talk without realizing that it has been heard to. 3. The experience of finding themselves somewhere and having no idea how they got there. 20. Staring off into space, thinking of nothing, and being not aware of the passage of time. 24. Inability to realize its own activity (for example, some people do not know whether they have just mailed a letter or have only thought about mailing it). 25. The evidence is found about having done things, which are not remembered. 26. Finding writings, drawings, or notes which must have been done but are not remembered. 3) Absorption-imaginative involvement: 10. The experience of being accused of lying, but being not aware of a lie. 14. The experience of remembering a past event so vividly that it is considered to be reexperienced. 15. The experience of not being sure whether remembered things really happened or are only dreamed. 17. Watching TV or a movie so absorbed in the story that it leads to the loss of contact with other events happening around. 18. Being so involved in a fantasy or daydream that it is perceived as a reality. 21. Soliloquy. 4) Amnesia: 4. Some people have the experience of finding themselves dressed in clothes that they don’t remember putting on. 5. Some people have the experience of finding new things among their belongings that they do not remember buying. 6. Some people sometimes find that they are approached by people that they do not know who call them by another name or insist that they have met them before. 7. The experience of being out of own body, watching it from outside as a different person. 8. Not recognizing friends or family members. 9. Forgetting important life - events (for example, a wedding or graduation). 5) Reality distortion: 23. Transient ability to do things smoothly and with amazing and unusual spontaneity (for example, sports, work, social situations, etc.). 6) Other items: 1. Some people have the experience of traveling by car or bus or subway and suddenly realizing that they don’t remember what has happened during the trip. 19. Some people find that they are sometimes able to ignore pain. Each item includes the instruction: ‘Mark the line to show what percentage of time this happens to you’; each answer is marked on one-hundred millimeter line. Arithmetic average from all items represent resulting score of the questionnaire. As was supposed, dissociative experiences occur in general population and markedly distinguish cases of psychiatric disorders (see table 1; Bernstein and Putnam, 1986). Results We measured the dissociation using DES in 21 epileptic patients (8 males and 13 females), which showed predominantly complex symptoms, in some cases with secondary generalization. Our results support previous findings that dissociation may play an important role in etiology of epilepsy. Above all in pharmacoresistant patients showing important dissociative psychopathology. Median DES score in our patients was 12,32 (c = 6,19), which is more than two times higher than in normal subjects. Discussion Our results are comparable to other works (Alper et al., 1997; Devinsky et al., 1989). In the study Dissociation in Epilepsy and Conversion Nonepileptic Seizures (Alper et al., 1997) two groups of patients were compared. Median of the DES score was 12.7 for epilepsy and 15.1 for cases of conversion nonepileptic seizures. In other work (Devinsky et al., 1989) dealing with dissociation and epilepsy, median DES score was lower (8.75). Nevertheless, this is 2 times higher than in normal population (table 1). If we consider organic dissociation according to 10. International Classification of Diseases (in the Czech version MKN10) (F 06.5), we can grasp dissociation as a psycho-organic process of mutual conversion from organic damages into psychopathology and from a psychic pain into somatic symptoms.
