Summary This thesis provides insights into important aspects of allergic contact dermatitis to p-phenylenediamine (PPD): penetration, sensitization and elicitation. The review in chapter 2 describes and summarizes studies regarding PPD penetration through skin(analogues) and studies on the amino acids that are targeted by PPD. Additionally, the auto-oxidation and N-acetylation steps inseparably linked to PPD metabolism were described. PPD itself is intrinsically unstable and under air exposure or in solution it is susceptible to autooxidation and conjugation reactions. The highly reactive, primary oxidation product of PPD, p-benzoquinonediimine (PBQD), is proposed to undergo further oxido-conjugation reactions, to form dimers, the trimer Brandowski’s base (BB) or even pentamers. In addition to these oxido-conjugation reactions, in vitro and ex vivo studies showed that PPD is metabolised within skin into monoacetyl-PPD (MAPPD) and diacetyl-PPD (DAPPD). The review showed that under hair dyeing conditions only a small amount of PPD, i.e. less than 1% of the amount of PPD applied, penetrates the skin. More than 80% of the PPD that penetrates, will be converted into the detoxification products MAPPD and DAPPD, by the N-acetyltransferase 1 (NAT1) enzymes. Consequently, the small amount of PPD that does not become N-acetylated is susceptible to the auto-oxidation reactions, yielding protein-reactive PPD derivatives. These derivatives may bind to specific amino acids and some of these adduct might be the complexes responsible for sensitization. However, true in vivo evidence is lacking and there may be many unknown (oxidative) PPD derivatives, the impact of which is unpredictable. Thus, further experiments to define the specific antigenic adducts responsible for naïve T cell activation are needed to improve our understanding of the mechanism of PPD sensitization. Chapter 3 further elaborates on the penetration of PPD. Although the penetration, autooxidation and N-acetylation pathways have already been studied extensively in in vitro 124
and ex vivo experiments, a clear in vivo picture on PPD penetration characteristics and on

real-time penetration of a topically applied chemical in vivo, non-invasive Confocal Raman
where and when PPD-oxidation and -acetylation products are formed is lacking. To study the Microspectroscopy (CRM) is the method of choice. To get insight into the in vivo penetration and clearance of PPD in the human skin and to determine the presence of PPD-oxidation or -acetylation products, patch test chambers containing 1% PPD in petrolatum were applied to the volar forearm of 2 healthy human subjects, using increasing application times and several follow-up measurements. It could be demonstrated that directly after removal of the PPD patch applied for 30 minutes, significant amounts of PPD (70 mg PPD/g keratin at 4 µm depth) were detectable up to 20 µm below the skin. PPD contents were highest after repeated application of 3 x 1 hour (350 mg PPD/g keratin), followed by single applications for 2 hours 40 minutes, 2 hours and lowest after a single application of 23 hours. For all the application
times studied, the PPD amounts decreased with increasing depth. Concerning follow-up measurements, it could be shown that the 3 x 1 hour patch resulted in 2-3 times more PPD in the skin as compared to the 2 hours patch. PPD contents in the skin decreased with time, most clearly seen between 0-10 µm depth and approached the baseline reading at 20-24 µm below the skin surface. The PPD half-time was 3 hours. No spectral contributions of BB, MAPPD or DAPPD could be detected. In summary, we have gained insight into the in vivo penetration of PPD in human skin by non-invasive Raman spectroscopy. Penetration into the skin was fast and detected PPD concentrations in the stratum corneum were high. PPD was detected in the stratum corneum as well as in the viable epidermis. Oxidized or acetylated PPD derivatives could not be detected. In chapter 4 we investigated the role of glutathione S-transferase (GST) gene polymorphisms in sensitization to PPD and other xenobiotics, by performing an association study as well as a meta-analysis. We found that common polymorphisms in genes encoding the detoxifying phase II GST enzymes do not play a major role in sensitization to PPD and other xenobiotics. In fact, even when considering the combined ‘high risk’ genotype GST(theta)T1*0/GST(mu) M1*0, to study a possible synergistic effect, no significant association with sensitization to PPD was found. This pointed to the conclusion that polymorphisms in GST genes have disease-modifying, rather than disease-causing effect and that they only have a weak biologic impact. As described previously, the identification of susceptibility loci in complex, multifactorial diseases such as allergic contact dermatitis is demanding, especially in the view of the relatively low frequency in the population, the presence of genes with minor effects and ethnic differences. This has also been addressed in chapter 5, in our response to a comment of Westphal et al. in which we explain the differences found in the meta-analysis by the fact that GSTs have different modes of action and that detoxification of the studied xenobiotics cannot be solely attributed to the examined GST genes. Consequently, as each individual has many single nucleotide polymorphisms (SNPs) that together create a unique DNA sequence, SNPbased genetic linkage analysis can be used to map disease loci, and hence determine disease susceptibility genes. The advances in SNP genotyping array technologies have made wholegenome association studies (WGAS) within reach. In chapter 6 we analyzed the elicitation response of an interesting and well-described subgroup of individuals that had a weak response to a diagnostic PPD patch test, but who failed to respond to a previous conducted hair dye model test. By using varying exposure conditions, with increasing exposure times and hence increasing dose, we aimed to get detailed insight into the dose-dependency of the elicitation responses of these subjects. Furthermore, we related these findings to the measured exposure level (MEL), which refers to the actual dose becoming available for elicitation under simulated hair dye exposure conditions, rather than to the dose applied. (applied dose 3000 µg PPD cm-2, MEL 6.8 µg PPD cm-2) was more than 10-fold lower than under patch diagnostic patch test conditions (applied dose 400 µg PPD cm-2, MEL 205 µg PPD cm-2) . Six subjects were available for this follow-up elicitation study. One of six subjects responded with a + response to the hair dye model applied for 60 min. Four of the five remaining subjects elicited a + response to the PPD TRUE test® applied subsequently, while one of five responded doubtfully. From these results it could be concluded that the elicitation dose needed to elicit allergic reactions in truly, but weakly PPD sensitized individuals, is between the dose becoming available by diagnostic patch testing and the dose becoming available by our model hair dye test exposure and that this dose was different for each individual. Importantly, these differences in MEL only seemed to be of importance in patients who belong to the lower end of the patch test reactivity spectrum, given the fact that all previously tested ++ and +++ responders (in diagnostic patch testing) reacted to the 2%PPD hair dye test model. Thus, for this particular group of subjects, the + diagnostic PPD patch test seemed not to have a direct relevance for reactivity to a PPD concentration of 2% under simulated hair dye conditions. In these cases, the PPD diagnostic patch test only indicates sensitization and does not predict whether a contact allergic reaction will occur during daily life PPD exposure, i.e. hair dyeing. However, since only a limited number of subjects were available for this study, a larger number of subjects would be needed to substantiate the results found thus far. Furthermore, we confirmed that consideration of the MEL is a useful tool to better characterize thresholds of elicitation than consideration of the applied dose alone. In chapter 7 we investigated the elicitation response to 2-methoxymethyl-PPD (ME-PPD) under simulated hair dye conditions in 30 PPD-allergic individuals with diagnostic patch test grades +, ++ or +++ according to the ICDRG classification. An amount of 100 µl of the 2%MEPPD containing hair dye test product was applied to a van der Bend® patch test chamber and placed on the volar forearm of the subjects. A dye-free test product was used as a control. 126

Cross-reactivity to the ME-PPD containing hair dye test product was elicited in 9/30 (30%) subjects, while 70% did not elicited a reaction. Cross-reactivity was elicited in 2 of 4 cases with grade +++, 3 of 10 with grade ++, and 4 of 16 with grade +. No reactions were observed after application of the control patch. Under identical conditions, PPD was previously found to elicit 21/27 of these PPD-allergic individuals. In 18 of these 21 individuals, either the strength of the cross-elicitation response to ME-PPD was decreased or no response occurred. In summary, our data indicate that under simulated hair colouring conditions a significantly lower degree of cross-elicitation to ME-PPD (30%) was observed than previously reported for PPD (84%). Furthermore, the observed decreased strength of the elicitation response to ME-PPD across all three patch test grades may reflect the reduced skin sensitization properties, possibly including limited T cell recognition. However, careful dermatological evaluation remains necessary to assess cross-reactivity to ME-PPD in hair dye-allergic patients.

Samenvatting Dit proefschrift verschaft inzicht in belangrijke aspecten van allergisch contact eczeem door p-fenyleendiamine (PPD): penetratie, sensibilisatie en elicitatie. De review in hoofdstuk 2 beschrijft en resumeert de literatuur over PPD penetratie door huid(analogen) en studies over de door PPD gebonden aminozuren. Daarnaast worden de onlosmakelijk met PPD verbonden auto-oxidatie en N-acetylatie processen beschreven. PPD is intrinsiek instabiel wanneer het wordt blootgesteld aan de lucht of zich in oplossing bevindt en is gevoelig voor auto-oxidatie en conjugatie reacties. Er wordt verondersteld dat het zeer reactieve primaire oxidatie product van PPD, p-benzoquinonediimine (PBQD), verdere oxidoconjugatie reacties ondergaat om zodanig dimeren, het trimeer Brandowski’s base (BB) of zelfs pentameren te vormen. Naast deze oxido-conjugatiereacties hebben in vitro en ex vivo studies laten zien dat PPD in de huid wordt gemetaboliseerd tot monoacetyl-PPD (MAPPD) en diacetyl-PPD (DAPPD). De review liet zien dat onder haarverfcondities een kleine hoeveelheid PPD, d.w.z. minder dan 1% van de aangebrachte dosis, de huid penetreert. Meer dan 80% van de PPD die penetreert, zal worden omgezet in de detoxificatieproducten MAPPD en DAPPD door het N-acetyltransferase 1 (NAT1) enzym. Zodoende is de kleine hoeveelheid PPD die niet wordt ge-N-acetyleerd, beschikbaar voor auto-oxidatie reacties waardoor er eiwitreactieve PPD derivaten ontstaan. Deze derivaten kunnen binden aan specifieke aminozuren en een aantal van deze gevormde complexen is mogelijk verantwoordelijk voor sensibilisatie. Echter, daadwerkelijk in vivo bewijs ontbreekt en er zijn mogelijk vele onbekende (oxidatieve) PPD derivaten waarvan de impact niet te voorspellen is. Er zijn dus vervolgstudies nodig die specifieke antigene adducten die verantwoordelijk zijn voor naïeve T cel activatie kunnen definiëren, om zo ons inzicht het PPD sensibilisatiemechanisme te verbeteren. In hoofdstuk 3 wordt verder ingegaan op de penetratie van PPD. Hoewel de penetratie, auto128
oxidatie en N-acetylatie processen uitgebreid bestudeerd zijn in in vitro en ex vivo studies,

waarin PPD-oxidatie en -acetylatieproducten worden gevormd. Voor het real-time vervolgen
ontbreekt in vivo inzicht in PPD penetratiekarakteristieken en de locatie van en tijdsspanne van penetratie van een topicaal aangebrachte chemische stof is de non-invasieve Confocale Raman Microspectroscopie (CRM) de methode bij uitstek. Om inzicht te verkrijgen in de in vivo penetratie en klaring van PPD in de humane huid en de aanwezigheid van PPD-oxidatie of -acetylatieproducten te bepalen, werden plakkers met 1% PPD in petrolatum aangebracht op de volaire zijde van de onderarm van 2 gezonde proefpersonen. Hiertoe werden toenemende applicatieduren gebruikt en werden meerdere follow-up metingen verricht. Direct na het verwijderen van een 30 minuten aangebrachte PPD plakker, konden significante hoeveelheden PPD (70mg PPD/g keratine op 4 µm diepte) worden aangetoond tot op een diepte van 20 µm. De PPD hoeveelheden waren het hoogst na herhaalde applicatie van 3 x 1 uur (350 mg
PPD/g keratin), gevolgd door de enkele applicaties van 2 uur en 40 minuten, 2 uur en waren het laagst voor de enkele applicatie van 23 uur. Voor alle bestudeerde applicatietijden werd een afnemende hoeveelheid PPD gezien bij een toenemende diepte. De follow-up metingen lieten zien dat na de 3 x 1 uur plakker 2-3 keer zoveel PPD de huid penetreerde als na de 2 uur plakker. De hoeveelheid PPD in de huid nam af in de tijd - meest duidelijk zichtbaar op een diepte van tussen de 0 en 10 µm - en naderde de uitgangswaarde op 20-24 µm onder het huidoppervlak. De halfwaardetijd was 3 uur. Er werden geen spectrale bijdragen van BB, MAPPD of DAPPD gedetecteerd. Samenvattend hebben we inzicht verkregen in de in vivo penetratie van PPD in de menselijke huid, door middel van non-invasieve Raman spectroscopie. Penetratie in de huid was snel en de gedetecteerde PPD concentratie in het stratum corneum was hoog. PPD werd zowel in het stratum corneum als de levende epidermis gedetecteerd. Er konden geen geoxideerde of geacetyleerde PPD derivaten worden aangetoond. In hoofdstuk 4 werd de rol van glutathion S-transferase (GST) polymorfismen in sensibilisatie voor PPD en andere xenobiotica onderzocht door middel van een associatie- en een metaanalyse. Wij concludeerden dat de prevalente polymorfismen in de genen coderend voor de detoxificerende fase II GST enzymen, geen grote rol spelen in de sensibilisatie voor PPD en andere xenobiotica. Zelfs bij het gecombineerde ‘hoog risico’ genotype GST(theta)T1*0/ GST(mu)M1*0, waarbij mogelijk een synergistisch effect speelt, werd geen significante associatie met sensibilisatie voor PPD gevonden. Dit leidde tot de conclusie dat polymorfismen in GST genen eerder een ziekte-modificerend dan een ziekte-veroorzakend effect hebben met maar een zwak biologische impact. Zoals eerder beschreven, is de identificatie van susceptibiliteitsloci in complexe multifactoriële ziekten zoals allergisch contacteczeem een uitdaging, zeker in het licht van de relatief lage frequentie in de populatie, genen met geringe invloed en etnische verschillen. Deze aspecten werden ook aangehaald in hoofdstuk 5, in onze respons op een commentaar van Westphal et al., waarin we de verschillen die gevonden werden in de meta-analyse verklaren met het feit dat de GST verschillende werkingsmechanismes kent en dat detoxificatie van de bestudeerde xenobiotica niet alleen kan worden toegekend aan de onderzochte GST genen. Aangezien elk individu vele verschillende enkel-nucleotide polymorfismen (SNPs) heeft welke samen een unieke DNA-sequentie vormen, kan SNP-gebaseerde genetische linkage-analyse gebruikt worden om ziekte-loci in kaart te brengen en zo ziekte-susceptibiliteitsgenen te bepalen. De ontwikkelingen op het gebied van SNP genotyping array technologie heeft associaties studies betreffende het gehele genoom (WGAS) binnen bereik gebracht. In hoofdstuk 6 werd de elicitatiereactie onderzocht van een interessante en goed gekarakteriseerde subgroep van personen die een zwakke reactie hadden op de diagnostische PPD plakproef, maar niet reageerden op de eerder uitgevoerde haarverfplakproef. Door verschillende blootstellingscondities te gebruiken met toenemende blootstellingsduur en dus toenemende dosis, wilden we gedetailleerd inzicht verkrijgen in de dosis-afhankelijkheid

van de elicitatiereactie van deze personen. Bovendien relateerden we onze bevindingen aan de measured exposure level (MEL), die verwijst naar de dosis die daadwerkelijk in de huid beschikbaar komt voor elicitatie onder gesimuleerde haarverfcondities, in plaats van de aangebrachte dosis. Onze groep liet eerder zien, door ex vivo varkenshuid te gebruiken, dat de daadwerkelijke dosis die voor elicitatie beschikbaar komt onder gesimuleerde haarverfcondities (aangebrachte dosis 3000 µg PPD cm-2, MEL 6.8 µg PPD cm-2) meer dan 10 keer zo laag was als onder plakproefcondities (aangebrachte dosis 400 µg PPD cm-2, MEL 205 µg PPD cm-2). Er waren 6 proefpersonen beschikbaar voor deze vervolgstudie. Een van de zes personen reageerde met een + reactie op de haarverfplakproef welke gedurende 60 minuten was aangebracht. Vier van de vijf resterende proefpersonen hadden een elicitatiereactie op de vervolgens aangebrachte PPD TRUE test® en een van de vijf reageerde twijfelachtig. Op basis van deze resultaten konden we concluderen dat de dosis die nodig is om een allergische reactie te eliciteren in daadwerkelijke, maar voor PPD zwak gesensibiliseerde personen, ligt tussen de dosis die beschikbaar komt door diagnostische plakproeven en de dosis die beschikbaar komt na het aanbrengen van onze haarverf plakproef en dat deze dosis verschillend is voor elk individu. Het is belangrijk te vermelden dat deze verschillen in MEL alleen van belang lijken te zijn in patiënten die tot de onderkant van het plakproef reactiviteitsspectrum behoren, aangezien alle eerdere personen met ++ en +++ reacties (bij diagnostische plakproeven) wel reageerden op de 2%PPD haarverf plakproef. Concluderend kan worden gesteld dat voor deze specifieke groep van personen, de diagnostische PPD plakproef niet direct relevant lijkt te zijn voor reactiviteit tegen een PPD concentratie van 2% onder gesimuleerde haarverfcondities. In deze gevallen wijst een positieve diagnostische PPD plakproef dus alleen op sensibilisatie en voorspelt deze niet of er een contact allergische reactie zal optreden tijdens blootstelling aan PPD in het dagelijks leven, d.w.z. tijdens haarverven. Echter, gezien het beperkte aantal beschikbare proefpersonen voor deze studie, zou een groter aantal proefpersonen nodig zijn 130

om de resultaten die tot nu toe gevonden zijn te onderbouwen. Voorts bevestigden we met deze studie dat de MEL een bruikbaar en beter middel is om de drempels voor elicitatie te bepalen, dan de aangebrachte dosis alleen. In hoofdstuk 7 onderzochten we de elicitatiereactie op 2-methoxymethyl-PPD (ME-PPD) onder gesimuleerde haarverfcondities in 30 PPD-allergische proefpersonen met een +, ++ of +++ diagnostische plakproefreactie volgens de ICDRG classificatie. 100 µl van 2%ME-PPD bevattende haarverf werd aangebracht op een van der Bend® plakproefkamer en deze werd op de volaire zijde van de onderarm van de proefpersonen geplaatst. Een kleurvrij testproduct werd gebruikt als controle. Kruisreactiviteit tegen de ME-PPD bevattende haarverf werd opgewekt in 9/30 (30%) van de proefpersonen, terwijl bij 70% geen reactie werd opgewekt. Deze kruisreactiviteit werd gezien bij 2 van de 4 patiënten met een +++ reactie, 3 van de 10 met een ++ reactie en 4 van de 16 patiënten met een + reactie. Het controle testproduct wekte geen reacties op. In een eerdere studie werd er onder verder identieke condities, door een
PPD bevattende haarverf in 21 van de 27 PPD-allergische patiënten een reactie opgewekt. In 18 van deze 21 individuen was ofwel de sterkte van de kruisreactie op ME-PPD verlaagd of er trad geen reactie op. Samenvattend laten onze resultaten zien dat er onder gesimuleerde haarverfcondities, een significant lagere mate van kruisreactiviteit tegen ME-PPD plaatsvond (30%) dan eerder werd gerapporteerd voor PPD (84%). Mogelijk reflecteert de waargenomen afname in reactiesterkte binnen alle drie de plakproefgraderingen het verminderde sensibilisatiepotentieel van ME-PPD; mogelijk inclusief beperkte T cel herkenning. Zorgvuldige dermatologische evaluatie blijft echter nodig om kruisreactiviteit tegen ME-PPD in haarverf allergische patiënten te beoordelen.
