SKIN The skin is the largest organ of the body, accounting for about 15% of the total adult body weight. It performs many vital functions, including protection against external physical, chemical, and biologic assailants, as well as prevention of excess water loss from the body and a role in thermoregulation. The skin is continuous, with the mucous membranes lining the body’s surface (Kanitakis, 2002).
SKIN The skin is composed of three layers: the epidermis, the dermis, and subcutaneous tissue (Kanitakis, 2002). The outermost level, the epidermis, consists of a specific constellation of cells known as keratinocytes, which function to synthesize keratin, a long, threadlike protein with a protective role. The epidermis provides a protective waterproof barrier that also keeps pathogens at bay and regulates body temperature. The middle layer, the dermis, is fundamentally made up of the fibrillar structural protein known as collagen.
The dermis lies on the subcutaneous tissue, or panniculus, which contains small lobes of fat cells known as lipocytes. The thickness of these layers varies considerably, depending on the geographic location on the anatomy of the body.
EPIDERMIS The epidermis is a stratified, squamous epithelium layer that is composed primarily of two types of cells: -keratinocytes -dendritic cells. The epidermis harbors a number of other cell populations, such as melanocytes, Langerhans cells, and Merkel cells, but the keratinocyte cell type comprises the majority of the cells by far.

EPIDERMIS The epidermis commonly is divided into four layers according to keratinocyte morphology and position as they differentiate into horny cells, including: the basal cell layer (stratum germinativum), the squamous cell layer (stratum spinosum), the granular cell layer (stratum granulosum), a the cornified or horny cell layer (stratum corneum)
Keratinocytes At least 80% of cells in the epidermis are the ectodermally derived keratinocytes. The differentiation process that occurs as the cells migrate from the basal layer to the surface of the skin results in keratinization, a process in which the keratinocyte first passes through a synthetic and then a degradative phase. The predominant cell type in the epidermis, the outermost layer of the skin, constituting 95% of the cells found there. Those keratinocytes found in the basal layer (stratum germinativum) of the skin are sometimes referred to as basal cells or basal keratinocytes.
Basal Layer
Squamous Cell Layer
The basal layer (the stratum germinativum), contains column-shaped keratinocytes that attach to the basement membrane zone with their long axis perpendicular to the dermis. These basal cells form a single layer and adhere to one another as well as to more superficial squamous cells through desmosomal junctions (Murphy, 1997). Other distinguishing features of the basal cells are their dark-staining oval or elongated nuclei and the presence of melanin pigment transferred from adjoining melanocytes The basal layer—sometimes referred to as stratum basale—is the deepest of the five layers of the epidermis.
Overlying the basal cell layer is a layer of the epidermis that is 5–10 cells thick and known as the squamous cell layer or stratum spinosum The layer contain lamellar granules Lamellar granules are membrane-bound organelles containing glycoproteins, glycolipids, phospholipids, free sterols, and a number of acid hydrolases, including lipases, proteases, acid phosphatases, and glycosidases.

The most superficial layer of the epidermis containing living cells, the granular layer or stratum granulosum, is composed of flattened cells holding abundant keratohyaline granules in their cytoplasm. These cells are responsible for further synthesis and modification of proteins involved in keratinization
Horny cells (corneocytes) of the cornified layer provide mechanical protection to the underlying epidermis and a barrier to prevent water loss and invasion by foreign substances The corneocytes, which are rich in protein and low in lipid content, are surrounded by a continuous extracellular lipid matrix

The Regulation of Epidermal Proliferation and Differentiation
Kulit tersusun dari tiga lapis, yaitu :
1.Lapisan epidermis - terdiri dari lapisan sel yg telah mati (lapisan tanduk/ keratinocyte). - Ketebalannya 0,006 - 0,800 mm, pH 4,2 – 6,5; kadar air 10-25%. - Lapisan epidermis tidak ada pembuluh darah dan merup.perintang utama terhadap absorpsi obat.
- Epidermis sebagai sawar dasar dari kulit terhadap kehilangan air, elektrolit dan nutrisi dari badan dan sawar dasar terhadap penetrasi air dan substansi asing dari luar badan. - Epidermis juga mencegah atau menghambat kehilangan air dari badan, hingga semua jaringan yang lain menjaga keseimbangan dinamis dengan lingkungan dalam.
Lapisan berbutir-butir (stratum granulosum) lapisan keratohialin yang terdiri dari dua atau tiga lapis sel-sel gepeng dengan sitoplasma berbutir kasar, diantaranya terdapat inti Lapisan Malphigi (stratum spinosum) terdiri dari beberapa lapis sel yang bentuknya polygonal dan mengandung banyak glikogen, diantara sel-selnya terdapat intercellular bridges dan terdapat sel Langerhans
Lapisan epidermis dari bagian terluar hingga ke dalam yaitu : Lapisan Tanduk (stratum corneum) terdiri dari beberapa lapis sel gepeng yang telah mati, tidak berinti, protoplasma telah menjadi keratin. Jenis protein ini yang tidak larut dalam air, dan sangat resisten terhadap bahan-bahan kimia. Lapisan “ barrier “ (stratum lucidum) terdiri dari lapisan sel-sel gepeng tanpa inti, protoplasma berubah menjadi oleidin
Lapisan Basal (stratum germinativum) Lapisan basal adalah lapisan terbawah epidermis. Di dalam stratum germinativum juga terdapat sel-sel melanosit, yaitu sel-sel yang tidak mengalami keratinisasi dan fungsinya hanya membentuk pigmen melanin dan memberikannya kepada sel-sel keratinosit melalui dendrit-dendritnya . Melanosit adalah sel dendritik yang membentuk melanin. Melanin berfungsi melindungi kulit terhadap sinar matahari.

2.Lapisan dermis
merupakan lapisan di bawah epidermis, memiliki ketebalan 3-5 mm. berupa anyaman serabut kolagen dan elastin. Dermis mengandung pembuluh darah, pembuluh limfe, folikel rambut, kelenjar lemak (sebasea), kelenjar keringat, serabut saraf dan korpus pacini. Lapisan dermis dibagi menjadi dua bagian, yaitu: 1. Pars papilare, yaitu bagian yang menonjol ke epidermis, berisi ujung serabut saraf dan pembuluh darah. 2. Pars retikulare, yaitu bagian bawah dermis yang menonjol ke arah subkutan, dan terdiri serabut kolagen, elastin, dan retikulin
Fungsi kulit 3.Lapisan hipodermis/ sub kutis merupakan kelanjutan dermis yang terdiri dari jaringan ikat longgar berisi sel-sel lemak di dalamnya. Lapisan sel lemak disebut penikulus adiposus, berfungsi sebagai cadangan makanan. Di lapisan ini terdapat ujung-ujung saraf tepi, pembuluh darah, dan saluran getah bening .
Kulit mempunyai fungsi bermacam-macam untuk menyesuaikan dengan lingkungan. Adapun fungsi utama kulit adalah (Djuanda,2007): 1. Fungsi proteksi Kulit menjaga bagian dalam tubuh terhadap gangguan fisik atau mekanik (tarikan, gesekan, dan tekanan), gangguan kimia ( zatzat kimia yang iritan), dan gagguan bersifat panas (radiasi, sinar ultraviolet), dan gangguan infeksi luar.
3. Fungsi ekskresi
2. Fungsi absorpsi Kulit yang sehat tidak mudah menyerap air, larutan dan benda padat tetapi cairan yang mudah menguap lebih mudah diserap, begitupun yang larut lemak. Permeabilitas kulit terhadap O2, CO2 dan uap air memungkinkan kulit ikut mengambil bagian pada fungsi respirasi. Kemampuan absorpsi kulit dipengaruhi oleh tebal tipisnya kulit, hidrasi, kelembaban, metabolisme dan jenis vehikulum.
Kelenjar kulit mengeluarkan zat-zat yang tidak berguna lagi atau sisa metabolisme dalam tubuh berupa NaCl, urea, asam urat, dan amonia. 4. Fungsi persepsi Kulit mengandung ujung-ujung saraf sensorik di dermis dan subkutis sehingga kulit mampu mengenali rangsangan yang diberikan. Rangsangan panas diperankan oleh badan ruffini di dermis dan subkutis, rangsangan dingin diperankan oleh badan krause yang terletak di dermis, rangsangan rabaan diperankan oleh badan meissner yang terletak di papila dermis, dan rangsangan tekanan diperankan oleh badan paccini di epidermis.

5.Fungsi pengaturan suhu tubuh (termoregulasi) Kulit melakukan fungsi ini dengan cara mengekskresikan keringat dan mengerutkan (otot berkontraksi) pembuluh darah kulit. Di waktu suhu dingin, peredaran darah di kulit berkurang guna mempertahankan suhu badan. Pada waktu suhu panas, peredaran darah di kulit meningkat dan terjadi penguapan keringat dari kelenjar keringat sehingga suhu tubuh dapat dijaga tidak terlalu panas.
6.Fungsi pembentukan pigmen Sel pembentuk pigmen (melanosit) terletak di lapisan basal dan sel ini berasal dari rigi saraf. Jumlah melanosit dan besarnya butiran pigmen (melanosomes) menentukan warna kulit ras maupun individu. 7. Fungsi kreatinisasi Fungsi ini memberi perlindungan kulit terhadap infeksi secara mekanis fisiologik. 8. Fungsi pembentukan/sintesis vitamin D
