Voorbeelden van subsidieaanvragen bij ZonMw
Hierna zijn twee voorbeelden geplaatst van subsidieaanvragen die voldoen aan de criteria zoals beschreven in hoofdstuk 5 van Handleiding medisch-wetenschappelijk onderzoek. Het gaat om onderzoeksvoorstellen die bij ZonMw zijn ingediend en waarvoor daadwerkelijk subsidie is verkregen. De aanvragen zijn met toestemming van de aanvrager overgenomen, geanonimiseerd en op onderdelen enigszins aangepast. Hoewel in verband met de beoordeling door buitenlandse referenten aanvragen bij ZonMw doorgaans in het Engels dienen te worden opgesteld, wordt van het eerste voorbeeld de Nederlandse vertaling gegeven. Het eerste voorbeeld over preventie van ziekenhuisinfecties heeft een klinischepidemiologisch karakter, het tweede voorbeeld heeft betrekking op een meer mechanistische vraagstelling uit de medisch-biologische wetenschappen.

Dergelijke infecties komen bij ca. 45% van de ICUpatiënten voor en zijn geassocieerd met een verhoogde sterftekans, een langer verblijf op de ICU en een toename van de kosten voor de gezondheidszorg. Men veronderstelt dat veel van deze infecties voortkomen uit kolonisatie van potentieel pathogene micro-organismen in de mondkeelholte of in het darmstelsel. Selectieve decontaminatie van het darmstelsel (SDD) is een manier om ziekenhuisinfecties op een ICU te voorkomen. Met SDD hoopt men, door lokale of systemische toepassing van antibiotica, potentieel pathogene micro-organismen te elimineren zonder de anaërobe darmflora te verstoren en daarmee uiteindelijk het aantal ziekenhuisinfecties te verminderen. Het nadeel van SDD is echter dat antibioticaresistentie kan optreden en infecties met resistente micro-organismen zijn doorgaans fataal. Bovendien leiden resistente micro-organismen op populatieniveau tot een verhoging van de morbiditeit en mortaliteit. Ideaal zou zijn wanneer men kon beschikken over een methode die wel de voordelen van SDD kent, maar niet het nadeel van de antibiotica-resistentie. Bij patiënten met buikchirurgie, levertransplantatie of necrotische pancreatitis, bleek het gebruik van een combinatie van Lactobacilli met vezels het aantal ziekenhuisinfecties te verminderen, maar bij patiënten die chirurgisch of internistisch behandeld worden op een ICU is deze methode nog niet toegepast. Het grote voordeel van deze methode is dat er geen selectie van antibiotica-resistente micro-organismen plaatsvindt. Dit onderzoek beoogt, in een unit-gerandomiseerde klinische trial, bij ICU patiënten van een Universitair Medisch Centrum, preventieve behandeling met SDD te vergelijken met een combinatie van Lactobacilli en vezels. Patiënten worden gerandomiseerd en behandeld gedurende de hele verblijfsduur in het ICU met hetzij SDD, hetzij Lactobacilli met vezel. Demografische gegevens, indicatoren voor de ernst van de ziekte en follow-up gegevens worden op gestandaardiseerde wijze verkregen. Regelmatig worden kweken ingezet van sputum, keeluitstrijken en rectaaluitstrijken. De incidentie van op de ICU opgelopen

infecties vormt het primaire eindpunt in dit onderzoek. Secundaire eindpunten zijn de sterftecijfers gedurende het verblijf in de ICU c.q. het ziekenhuis, het optreden van antibiotica-resistentie en het additionele gebruik van antibiotica. Antibiotica-resistentie wordt gevolgd tot aan moment van overlijden dan wel 2 weken na ontslag van de ICU. Sterfte wordt gevolgd tot het moment van ziekenhuisontslag. Uit een power-analyse blijkt dat per groep 250 patiënten geïncludeerd dienen te worden om equivalentie tussen beide behandelingsregimes te kunnen aantonen. De resultaten van het onderzoek zullen via internationale medisch-wetenschappelijke tijdschriften en congressen gepresenteerd worden aan collegae onderzoekers en behandelaars. In Nederland zal een survey over het gebruik van SDD door ICU-artsen worden uitgevoerd. Daarna zal deze groep zowel het resultaat van de survey als het resultaat van het voorgestelde onderzoek toegestuurd worden. 3 Inhoud Probleemstelling Op intensive care units (ICU’s) zorgen nosocomiale (in het ziekenhuis opgelopen) infecties voor een aanzienlijk deel van de morbiditeit en mortaliteit onder de opgenomen patiënten. Van de meeste van deze infecties veronderstelt men dat er een orofaryngeale of intestinale kolonisatie van potentieel pathogene micro-organismen aan vooraf gaat. Selectieve decontaminatie van het darmstelsel (SDD) gaat als preventieve maatregel uit van het idee dat de eigen darmflora beschermt tegen Gramnegatieve bacteria. Het doel van SDD is dan ook om de Gram-negatieve bacteria en schimmels uit het darmstelsel te verwijderen zonder de anaërobe flora te verstoren. Klassieke SDD bestaat uit 4 elementen: 1) selectieve eradicatie van potentieel pathogene micro-organismen in de mondholte en decontaminatie van het darmstelsel door toediening van niet-absorbeerbare antibiotica, 2) systemische profylaxe tegen infecties van commensale luchtweg micro-organismen die zouden kunnen optreden tijdens de eerste dagen met beademingsintubatie, 3) regelmatige kweken van keeluitstrijken en feces teneinde de effectiviteit van SDD te monitoren, 4) optimale hygiëne ter preventie van kruisinfecties. SDD is de best onderzochte preventieve maatregel tegen infectie van ICU patiënten. In een review van 27 prospectieve gerandomiseerde trials en 6 meta-analyses bleek dat toch nog niet onomstotelijk is vastgesteld dat SDD een gunstig effect heeft (1). Pas in een recente Nederlandse studie (2) heeft men kunnen aantonen dat SDD leidt tot een significante daling van de ziekenhuismortaliteit. Het probleem in deze studie was echter dat geen sprake was van een unit-cross-over opzet, waardoor de resultaten mogelijk vertekend zijn.

Het wezenlijke probleem van SDD is dat antibioticagebruik kan leiden tot selectie en overgroei van antibiotica-resistente micro-organismen. Beschikbare klinische gegevens laten zien dat SDD de bacteriële flora van de gastheer wijzigt en daarmee inderdaad predisponeert tot de ontwikkeling van antibiotica-resistente pathogenen (3). Hoewel in de studie van P. Puk e.a. (2) geen toename van antibiotica-resistentie kon worden aangetoond, hebben anderen wel een toename van antibioticum-resistente bacteriën gevonden. In Oostenrijk bijvoorbeeld leidde het gebruik van SDD tot een toename van het aantal methiciline-resistente Staphylococcus Aureus (MSRA) van 17% naar 81% en van ciporfloxacine-resistente S. aureus van 33% naar 80% (3). In Duitsland verdween MRSA toen SDD gestaakt werd (4). In een Spaanse studie bleek de mate van kolonisatie met methiline-resistente S. aureus, met coagulase negatieve staphylococci en met entereococci significant hoger in patiënten die behandeld waren met SDD, vergeleken met controle patiënten (5). Ook in een Belgisch onderzoek was het gebruik van SDD geassocieerd met een toename van MRSA en van Gram-negatieve bacteria die resistent waren voor tobramycine en ofloxacine (6). Het aantal infecties veroorzaakt door multiresistente pathogenen, zoals Acinetobacter spp. lijkt ook toe te nemen bij SDD. Idealiter zou men willen beschikken over een maatregel die wel de gunstige effecten van SDD heeft, maar niet het risico verhoogt op antibioticum-resistente micro-organismen. Een ontwikkeling van de laatste jaren is dat men de darmbacterieflora en de kwaliteit van het darmslijmvlies tracht te beïnvloeden met behulp van probiotica. Het doel van deze toepassing van probiotica is de darmflora zodanig te beïnvloeden dat pathogenen zich niet kunnen vermenigvuldigen doordat de darmafweer wordt gestimuleerd. Lactobacillus spp. en Bifidobacterium spp. zijn veelvuldig gebruikte probiotica. Het is aangetoond dat inname van melkzuurbacteriën met fermenteerbare vezel translocatie van de bacteriën tegengaat, de afweer stimuleert, de infectierate doet afnemen, en het omvang van celschade en weefselnecrose beperkt (7). Toepassing van levende Lactobacilli met vezel bleek in vergelijking tot een behandeling met dode Lactobacilli te leiden tot een significante daling van het aantal sepsisgevallen en tot verbeterde klinische uitkomsten bij patiënten die bovenbuikchirurgie of een levertransplantatie hadden ondergaan (8,9). Vergeleken met SDD, bleek het gebruik van Lactobacilli in combinatie met tarwe tot een significante daling te leiden van de incidentie van postoperatieve infecties en complicaties (48% resp. 14%) bij patiënten die een levertransplantatie ondergingen (9). Behandeling met een combinatie van Lactobacilli plantarum 299V met vezel bleek eveneens effectief in het verminderen van pancreatitis bij patiënten met pancreasnecrose (10). Slechts één van de 22 patiënten die levende L. plantarum kregen, ontwikkelde een geïnfecteerde pancreasnecrose tegen 7 van de 23 patiënten die behandeld waren met dode L. plantarum. In het onderhavige onderzoek willen we nagaan of levende

L actobacilli in combinatie met vezels net zo effectief zijn als SDD in het bestrijden van nosocomiale infecties en sterfte onder gelijktijdige afname van de kans op de ontwikkeling van antibioticum-resistente organismen. Relevantie Nosocomiale infecties vormen een belangrijk complicatie risico bij de behandeling van patiënten in ICU’s, met percentages variërend van 5 tot 85. Infecties bij ICU-patiënten gaan samen met een verhoogde sterfterisico, een langere verblijfsduur op de ICU, een toename van het gebruik van antibiotica en daardoor meer kosten voor de gezondheidszorg (1,5). Een ander probleem van het toegenomen antibioticumgebruik in ICU’s is de ontwikkeling van antibioticum-resistente bacteria. Vooral bij ICU-patiënten, doorgaans immunogecompromitteerd en met multipele medische problematiek, kan een infectie met antibioticum-resistente micro-organismen desastreus zijn. Preventie van nosocomiale infecties heeft om die reden de hoogste prioriteit. Van alle infecties die ontstaan op een ICU, schat men dat 50% primair endogeen is (d.w.z. veroorzaakt door een pathogeen micro-organisme dat bij opname al in de flora van de patiënt aanwezig was), 30% secundair endogeen (d.w.z. veroorzaakt door een potentieel pathogeen microorganisme tijdens het ICU-verblijf opgelopen) en dat slechts 20% een exogene oorzaak heeft (11). Dit betekent dat in theorie het grootste deel van de infecties in een ICU voorkomen kunnen worden door de pathogenen die de patiënt bij zich draagt te elimineren. Een methode om dat te bereiken is selectieve decontaminatie van de darm, voor het eerst beschreven door van Rijn e.a. in 1971 (12). SDD is als profylactisch regime voor infecties in 1984 geïntroduceerd in de intensive care geneeskunde (15) en wordt sinds die tijd in diverse ziekenhuizen in Nederland toegepast. Nederlandse behandelaars concluderen echter, op basis van de beschikbare literatuur, dat SDD niet routinematig toegepast moet worden bij patiënten die mechanisch beademd worden (1), ondanks het feit dat SDD tot een significante reductie van beademing gerelateerde pneumonieën leidde. Uit twee meta-analyses bleek zelfs een sterftedaling van 20% bij volledige toepassing van SDD. Ook recent was in een gerandomiseerde studie, waarin SDD werd vergeleken met placebo (2), sprake van 35% daling in ICU-sterfte en 23% daling in ziekenhuissterfte. Anderen twijfelen echter nog steeds aan de effecten van SDD op de sterfte en op de ontwikkeling van antibioticum-resistentie (16). Al deze gegevens bij elkaar suggereren, in tegenstelling tot wat uit bovengenoemde meta-analyse werd geconcludeerd, een gunstig preventief effect van SDD op de ontwikkeling van Gram-negatieve bacteriën in het darmstelsel. Het probleem echter is dat alle antibiotica bij therapeutische doses leiden tot de ontwikkeling van resistentie (13). Strikt genomen bestaat selectieve darmdecontaminatie dus niet. Er worden dan ook nog steeds twijfels geuit over de effecten van SDD op de sterfte en op de ontwikkeling van antibioticum-resistentie (16).

Preventie van translocatie van Gram-negatieve bacteria en schimmels door middel van niet-antibiotische methoden kent deze nadelen van SDD niet. Zo bleek behandeling met een combinatie van L. plantarum 299V en vezel effectief in het reduceren van de kans op sepsis bij 45 patiënten met pancreasnecrose. Slechts één van de 22 patiënten die levende L. plantarum kregen, ontwikkelde een infectie, vergeleken met 7 van de 23 patiënten die met een gedode L. plantarum waren behandeld (10). L. Plantarum 299V is een veelvoorkomende lactobacillus spp, geïsoleerd uit gezond humaan mucosa en vaak gebruikt bij de fermentatie van voedsel. Deze bacterie overleeft de passage door het maagdarmkanaal. In vitro laat L. plantarum een sterke adhesie zien aan mannose bevattende glycoproteïnen, daarmee verhinderend dat E. coli en andere enterobacterieën aan de darmmucosa hechten. L. plantarum is voorts in staat arginine te kataboliseren en stikstofoxide te genereren (17). In een rapport van een Europese conferentie over de veiligheid van melkzuurbacteriën die als probiotica worden gebruikt, werd geconcludeerd dat er geen aanwijzingen zijn dat het slikken van melkzuurbacteriën enig serieus gezondheidsrisico met zich meebrengt. Het infectierisico is erg laag en in vrijwel alle gevallen waarin het toch optreedt, blijkt de bron van de infectie niet het toegediende probioticum te zijn geweest, maar de commensale flora van de patiënten zelf. De lange ervaring met menselijke consumptie, de beschikbare epidemiologische gegevens, diverse klinische trials en acute toxiciteitstudies laten allemaal zien dat melkzuurbacteriën, zowel in gefermenteerd voedsel, als in de vorm van probiotica, veilig gebruikt kunnen worden (18). Tijdens de consumptie van L. plantarum 299V, neemt het aantal Lactobacilli binnen een week significant toe van 10e4-5 tot 10e8 CFU/g feces, voornamelijk bestaande uit de L. plantarum 299V die is geslikt. Op grond hiervan veronderstellen wij dat toepassing van levende melkzuurbacteriën bij ICU-patiënten voorkomt dat Gram-negatieve bacteria en schimmels alsnog gaan koloniseren in de darm, zonder risico op resistentieontwikkeling. Over het effect van L. plantarum op de ontwikkeling van resistentie zijn overigens geen gegevens bekend. Voor zover bekend, zijn of worden vergelijkbare studies nog niet uitgevoerd. De ontwikkeling van resistente micro-organismen is onderwerp van onderzoek in veel projecten die worden uitgevoerd in het Preventie Programma van ZonMw. De onderhavige studie past daarom goed in dit programma, zonder met andere studies te overlappen. Wanneer melkzuurbacteriën een effectieve manier zouden blijken te zijn om infecties te voorkomen, zou dit niet alleen van belang zijn voor de Nederlandse situatie. De methode zou ook toegepast kunnen worden in andere landen met een hoog risico op antibioticum-resistentie (zoals de Verenigde Staten, ZuidEuropa). Eerder is al gewezen op de nadelen van SDD in die landen doordat het bijdraagt aan het risico op resistentieontwikkeling (14).

Relevantie prioriteiten en prioriteitsdoelgroepen Dit onderzoek draagt zowel op individueel als op populatieniveau bij aan de bestrijding van infectieziekten: 1) Van preventie van ICU-infecties is inmiddels aangetoond dat de individuele patiënt er voordeel van ondervindt (1,2,5). 2) Deze studie is bedoeld om te laten zien dat infectiepreventie met niet antibiotische middelen (L. plantarum in combinatie met vezels) even effectief is als infectiepreventie met antibiotische profylaxe. Gebruik van antibiotica gaat echter, i.t.t. de niet-antibiotica profylaxe gepaard met selectieve ontwikkeling van antibioticum-resistente micro-organismen, een groot volksgezondheidsprobleem. Vermindering van deze resistentieontwikkeling zal uiteindelijk voordelig zijn voor de algemene bevolking. Omdat niet kan worden voorspeld welke typen personen en patiënten op een ICU worden opgenomen en welke daarvan een ziekenhuisinfectie oplopen, zal een positief resultaat van het onderhavige onderzoek voor alle potentiële ICU-patiënten (dus de hele algemene bevolking) van belang kunnen zijn. Kennisoverdracht, implementatie, bestendiging Het voorgestelde onderzoek is een directe evaluatie van twee verschillende medische strategieën. De leden van het projectteam hebben verschillende specialismen en achtergronden, waarmee niet alleen verschillende benaderingen van het klinische probleem ontstaan, maar ook een groter bereik van medische specialismen, waarmee de resultaten van het onderzoek uitgewisseld kunnen worden. Als de trial voltooid is, zullen we drie verschillende doelgroepen op drie manieren benaderen. 1 Internationaal zullen onderzoekers en artsen worden geïnformeerd over de onderzoeksresultaten via presentaties op (internationale) congressen en via artikelen in internationale medische tijdschriften. Voor artsen zal benadrukt worden dat de effectiviteit van SDD tot nog toe alleen is aangetoond in landen met lage prevalenties voor methicilline-resistente stafylokokken of vancomycinen-resistente enterokokken. Er zijn al openlijk twijfels geuit over het gebruik van SDD in gebieden met een hoge prevalentie van dergelijke bacteriën, waaronder de Verenigde Staten en Zuid-Europa (14). Als Lactobacilli in combinatie met vezels even effectief zou zijn als SDD zonder selectie van antibioticumresistente micro-organismen, zou deze methode, i.t.t. SDD, in alle landen gebruikt kunnen worden, zelfs in landen met een hoge incidentie van resistente bacteriën (14). Voor onderzoekers is het van belang vast te stellen dat het mechanisme waarmee levende Lactobacilli tot vermindering van het aantal infecties leidt, nog steeds niet bekend is. Daarom zal dit onderzoek (mits positief over de effecten van Lactobacilli) het fundamentele onderzoek kunnen bevorderen naar de interactie tussen Lactobacilli, darmflora en de menselijke gastheer.

2 In Nederland zullen artsen betrokken bij ICU-patiënten op verschillende manieren worden benaderd. Tegen het einde van het onderhavige onderzoek zullen alle ICU-artsen (chirurgen, internisten, anesthesiologen) een korte vragenlijst toegestuurd krijgen met vragen over het aantal en type patiënten in hun ICU (ongeval, internistisch, chirurgisch) en hun kennis over het mogelijke gebruik van SDD. De resultaten van deze survey zullen, samen met de resultaten van het onderhavige onderzoek, naar alle ICU-artsen worden gestuurd met de mogelijkheid om over de resultaten te discussiëren. Aanvullend zullen alle leden van de Nederlandse Vereniging voor Intensive Care (NIC), de Vereniging van Infectieziekten (VIZ), de Nederlandse Internisten Vereniging (NIV) en de Nederlandse Vereniging voor Medische Microbiologie (NVMM) worden geïnformeerd over de onderzoeksresultaten om de kennis verkregen met dit onderzoek over te brengen op de artsen en onderzoekers voor wie het onderwerp van belang is. 3 Het is niet zinvol specifieke patiëntengroepen te informeren, omdat niet kan worden voorspeld welke patiënten op een ICU worden opgenomen. Om die reden worden populair-wetenschappelijke tijdschriften (bijv. Volkskrant Gezond) geïnformeerd over de onderzoeksresultaten, teneinde een groot deel van de bevolking hiervan op de hoogte te stellen. Doelstelling Het doel van dit onderzoek is in een cross-over gerandomiseerde klinische trial de effectiviteit van niet-antibiotische profylaxe (Lactobacillus met vezels) ter preventie van nosocomiale infecties bij ICU-patiënten te vergelijken met de effectiviteit van antibiotische profylaxe (SDD, d.w.z. lokale en systemische antibiotica). Specifieke doelstellingen in dit project zijn: 1 vaststellen of gebruik van levende Lactobacilli in combinatie met vezels net zo effectief is als SDD bij de preventie van nosocomiale ICU-infecties; 2 evalueren van de ICU- en ziekenhuissterfte bij patiënten die levende Lactobacilli met vezels gebruiken in vergelijking tot patiënten die SDD (waarvan bekend is dat het de sterfte reduceert) gebruiken; 3 vaststellen of de ontwikkeling van antibioticum-resistente micro-organismen lager is bij patiënten die lactobacillus met vezel gebruiken dan bij degenen die SDD gebruiken. 4 Analyseren van het extra gebruik van antibiotica in beide groepen. Plan van aanpak In dit project worden de effecten van het profylactische gebruik van SDD en het gebruik van levende L. plantarum 299V met vezel vergeleken t.a.v. de incidentie van infecties, de ziekenhuissterfte en de ontwikkeling van antibioticumresistente micro-organismen.

Het onderzoek is ontworpen als een gerandomiseerd klinisch experiment met cross-over van behandelingsunits en een interventieperiode van 32 maanden. Patiënten en degenen die de bacteriekweken uitvoeren zijn geblindeerd voor de interventie. Voor de behandelend artsen is blindering niet mogelijk omdat de kweekresultaten onthullen in welke unit SDD wordt gebruikt en in welke unit niet. Het is evenmin mogelijk de SDD-kweekresultaten achter te houden omdat additioneel antibioticumbeleid dat niet is gebaseerd op kweekresultaten schadelijk zou kunnen zijn voor de patiënt en dus ethisch onverantwoord. Het onderzoek wordt uitgevoerd op de afdeling Intensieve Zorg van het Universitair Medisch Centrum Zoekdam (UMCZ). In deze algemene ICU worden per jaar 350 volwassen patiënten opgenomen die voldoen aan de inclusiecriteria voor dit onderzoek. De onderzoekspopulatie bestaat uit alle opeenvolgende patiënten die op de ICU van het UMCZ worden opgenomen, mits ouder dan 18 jaar, en met een verwachte beademingsduur van minimaal 48 uur, een verwachte ICU-verblijfsduur van 72 uur, of beide. Patiënten met een eerdere ICU-opname in afgelopen 3 maanden, patiënten die antibiotica gebruiken op het moment van opname, patiënten die overgevoelig zijn voor de te onderzoeken medicatie, zwangeren, patiënten met een contraindicatie voor maagsondevoedsel, patiënten die naar verwachting zeer spoedig zullen overlijden en patiënten die al aan een ander wetenschappelijk onderzoek deelnemen, worden uitgesloten van deelname aan het onderzoek. De ICU bestaat uit twee gescheiden units met een vergelijkbare mix van internistische, chirurgische, neurologische en traumapatiënten. Beide units bieden een vergelijkbaar pakket aan standaardzorg, uitgevoerd door dezelfde verpleegkundigen en artsen. Verpleegkundigen zijn op een van beide units actief, terwijl artsen behandeling geven in beide units. Over de periode 2004-2006 was de sterfte op beide units vergelijkbaar (driejaarsgemiddelde 19,9 versus 18,8%, p=0.8). Een van beide units zal worden aangewezen als SDD-unit en de andere als Lactobacilli-unit om te voorkomen dat er besmetting vanuit de omgeving en kruisinfecties van patiënten kunnen optreden. Om vertekening te voorkomen doordat een verschillende omgeving en personeel, zullen de units tijdens de interventieperiode veranderen van interventiestatus. Na een overgangsperiode (wash-out) wordt de SDD-unit aangemerkt als Lactobacilli-unit en omgekeerd. De patiënten worden aan een van beide units toegewezen volgens een door de computer gegenereerd random nummer. Van elke patiënt of verwante zal een geschreven informed consent worden verkregen. Wanneer het consent niet verkregen kan worden of wordt geweigerd, zal de patiënt op de unit geplaatst

worden waarop de meeste bedden beschikbaar zijn en daar standaard zorg ontvangen. Deelnemende patiënten in de SDD-units worden 4 keer per dag behandeld met een orale pasta met 2% polymyxine E, 2% tobramycine en 2% amfotericine B. De pasta wordt aangebracht in de mondholte. Via de maag-darmsonde wordt 100 mg polymyxine E, 80 mg tobramycine en 500 mg amfotericine B toegediend. Gedurende de eerste 4 dagen wordt 4 keer per dag Cefotaxime 100 mg toegediend. Patiënten met een tracheostoom worden bovendien vier keer per dag op de huid rond de tracheostoom behandeld met polymyxine E, tobramycine en amfotericine B-pasta. Patiënten met blind eindigende darmlissen krijgen aanvullend op de orale pasta 2-4 keer per dag zetpillen met 42 mg amfotericine B, 42 mg polymixine E en 64 mg tobramycine. SDD-medicatie wordt gemaakt door de apotheek van het UMCZ. Patiënten in de Lactobacilli-unit worden 2 keer per dag behandeld met 10e9 levende Lactobacilli plantarum 299V met 10 gram tarwevezels via een maagdarmsonde. L. plantarum 299V met vezel wordt geproduceerd door een fabrikant (AB Probi, Lund, Sweden). Behandeling met hetzij SDD, hetzij Lactobacilli wordt voortgezet tot ontslag van de ICU. Bij opname op de ICU en vervolgens 2 keer per week worden standaard kweken van longsputum en mondslijmvlies verkregen en hiermee wordt doorgegaan tot 2 weken na ontslag van de ICU. De kweken worden kwantitatief geanalyseerd op de aanwezigheid van (antibioticum-resistente) Gram-negatieve bacteriën en gisten. Hierbij worden selectieve en antibioticum-neutraliserende media gebruikt om fout-negatieve resultaten te vermijden door antibiotische activiteit van de lokaal toegediende antibiotica in de kweek. Voor de identificatie van micro-organismen worden standaard microbiologische methoden gebruikt, waaronder het Phoenix-systeem. De antibiotische gevoeligheid van de geïsoleerde bacteriën zal met hetzelfde systeem worden vastgesteld. De gevoeligheid van gisten voor de verschillende antimyotica zal met commercieel verkrijgbare microtiterplaten (MCS-diagnostics, Swalmen) worden bepaald. Alle geïsoleerde micro-organismen worden bij -70 oC opgeslagen voor eventueel toekomstige nadere analyse (bijvoorbeeld voor het vaststellen van bacteriële verwantschap). Kweken en kwantificeren van L. plantarum 299V zal eenmaal per week worden uitgevoerd volgens een methode elders beschreven (19). Aanvullende cultures (bijvoorbeeld bronchoalveolaire lavage (BOL), bloed, urine) worden ingezet wanneer klinische infectie wordt vermoed. Bij opname op de ICU worden op gestandaardiseerde wijze demografische gegevens (o.a. geslacht, leeftijd, behandelend specialist, preëxistente ziekten,

duur van ziekenhuisopname voor ICU) verkregen. Tevens worden de Acute Physiology and Chronic Health Evaluation (Apache) II scores (20) verzameld. Het aantal dagen op de ICU en aan de beademing, chirurgische ingrepen, infectieparameters (temperatuur, leukocytentelling plus differentiatie, bevindingen van borströntgenfoto’s, resultaten van de kweken) en het gebruik van antibiotica wordt genoteerd. Routinekweken van sputum, keeluitstrijken en rectaaluitstrijken worden afgenomen bij opname en 2 keer per week gedurende het verblijf op de ICU, tot aan het moment van overlijden dan wel 2 weken na ontslag van de ICU. Bij alle patiënten worden aanvullende kweken ingezet wanneer een infectie wordt vermoed. Besmetting van de ICU zelf wordt vastgesteld met behulp van kweken van veegmonsters op diverse plaatsen op de ICU’s (bedrand, keyboard van de bedgebonden computer, wastafel). De primaire uitkomst in dit onderzoek is de incidentie van infecties in de periode vanaf 48 uur na opname op de ICU tot aan het moment van overlijden dan wel 2 weken na ontslag van de ICU. Secundaire uitkomsten zijn de sterfte tijdens verblijf op de ICU, respectievelijk het ziekenhuis, de prevalentie van antibioticum-resistente micro-organismen, en het extra respectievelijk totale gebruik van antibiotica. De prevalentie van op de ICU opgelopen infectie loopt in de gerapporteerde studies uiteen van 5-85% (1) met een gemiddelde van ca. 45%. Gebruik van SDD zal resulteren in een verwachte infectieprevalentie van 25% (1,5). Het onderzoek richt zich op het aantonen van equivalente effectiviteit tussen L. plantarum 299V met tarwe en SDD in het voorkomen van infecties. Bij een acceptabel verschil van 10%, een alfafout van 0,05 en een bèta van 0,20 (type IIfout), zijn 250 bruikbare patiënten per groep nodig. Om dat te bereiken worden per groep 300 patiënten geïncludeerd. Per jaar worden ca. 350 patiënten opgenomen op de ICU, die in aanmerking komen voor dit onderzoek. In een vorig onderzoek op de ICU van het UMCZ bleek 8,5% van de gescreende patiënten niet geschikt of niet te willen meewerken (22). In een ander ziekenhuis bleek 14% van de patiënten niet deel te kunnen nemen. Uitgaande van een non-participatie van 14%, dienen 684 patiënten te worden geïncludeerd voor dit onderzoek, ofwel 21 per maand. In een inclusieperiode van 28 maanden kunnen in totaal 800 patiënten gescreend worden, ofwel 28 per maand. Dit lijkt ruim voldoende om de benodigde aantallen patiënten voor dit onderzoek te kunnen halen. Gegevensverwerking en analyse: Alle gegevens worden dubbel ingevoerd om foutieve codering te vermijden. Het resulterende databestand zal allereerst univariaat geanalyseerd worden om van alle variabelen verdelingen te verkrijgen

en om potentiële uitschieters te identificeren. Voor de analyse van de behandelingseffecten worden gestratificeerde analyses uitgevoerd en regressiemodellen ontworpen. De primaire uitkomstgegevens worden volgens het intention-to-treat principe geanalyseerd. Het verschil in incidentie van ICU-infecties wordt geschat met een bijbehorend betrouwbaarheidsinterval door middel van univariate en gecorrigeerde multivariate logistische regressie. De secundaire uitkomstgegevens worden op gelijke wijze geanalyseerd. Het aanvullende gebruik van antibiotica tussen beide groepen wordt vergeleken met behulp van de Wilcoxon-Mann-Whitney toets. In het onderzoek worden de volgende definities en afkappunten gehanteerd: Van een kolonisatie is sprake wanneer micro-organismen (bacteriestam, gist, enzovoort) in 2 of meer opeenvolgende monsters van dezelfde lichaamsplek worden geïsoleerd, zonder dat er sprake is van een infectie. Een kolonisatie bij opname is dan een kolonisatie aangetoond binnen 48 uur na opname op de ICU. Een kolonisatie wordt geacht verdwenen te zijn indien in 2 of meer opeenvolgende monsters van dezelfde lichaamsplek waarop bij opname sprake was van kolonisatie, geen micro-organismen meer worden geïsoleerd. Van een verworven kolonisatie is sprake wanneer na 48 uur na ICU-opname bij patiënten vrij van kolonisatie bij opname, alsnog een kolonisatie wordt gevonden. Van een verdenking op een beademingsgerelateerde pneumonie (VAP) is sprake bij een nieuw, persisterend of progressief infiltraat op een röntgenfoto van de borst én 3 of meer van de volgende 4 criteria: (a) rectaaltemperatuur > 38 ºC of <35,5 ºC; (b) leukocyten > 10e9/L en/of sterke daling of leukopenie (leukocyten < 3e9/L); (c) positieve kweek uit luchtwegslijm; (d) 10 leukocyten per gezichtsveld in de Gram-kleuring van de bronchoalveolaire lavage vloeistof (BALF). Van een bewezen VAP is sprake bij een positieve kwantitatieve kweek van de BALF (afkappunt > 10e4 kolonievormende units (=CFU) per ml) en/of aanwezigheid van > 2% geïnfecteerde cellen bij microscopisch onderzoek van het BALF-monster, of een positieve bloedkweek die niet aan een andere infectiehaard gekoppeld is, of een positieve kweek uit pleuravloeistof bij afwezigheid van eerder pleuraonderzoek. In de laatste twee gevallen dient het gekweekte micro-organisme identiek te zijn aan die gekweekt uit het luchtwegslijm. Een ICU-verworven pneumonie is een pneumonie gediagnosticeerd vanaf 48 uur na ICU-opname. Deze kan vroeg zijn, d.w.z. diagnose < 4 dagen na het begin van de beademing, of laat, d.w.z. diagnose > 4 dagen na het begin van de beademing. Nosocomiale infecties anders dan VAP zullen worden gediagnosticeerd volgens de definities van het Center of Disease Control (21). Het gebruik van antibiotica wordt uitgedrukt als ‘defined daily doses’ per 1000 patiëntendagen.

Dit heeft geleid tot meer dan 18 peer-reviewed artikelen over dit onderwerp (zie publicatielijst 1-10), inclusief meta-analyses en gerandomiseerde klinische trials. Een van de deelnemende medische microbiologen (B. Geleider) is gespecialiseerd in het stellen van betrouwbare diagnoses van beademinggerelateerde pneumonie (publicatielijst ref. 5, 16). Er is verder veel ervaring in de diagnostiek van kolonisatie en kruisinfecties in het ziekenhuis (publicatielijst ref. 3-5, 9 en 10). Een van de hoofdthema’s voor onderzoek in de afdeling Medische Microbiologie is antibioticum-resistentie: o.a. verspreiding, prevalentie en mechanismen voor het ontstaan van antibioticum-resistentie binnen en buiten het ziekenhuis (publicatielijst ref. 11, 12, 15, 17-19). Verschillende studies naar het effect van L. plantarum 299V met vezel zijn reeds uitgevoerd, zowel bij humane vrijwilligers als bij patiënten (publicatielijst ref. 13 en 14). Dr. I.N. Ternist is staflid geweest op de ICU en is thans waarnemend hoofd van de afdeling. Hij is betrokken bij de opleiding van ICU-artsen op nationaal niveau en heeft ervaring met onderzoek naar de rol van darmflora en probiotica (incl. L. plantarum 299V) op gezondheid en ziekte. Prof.dr. P. Romotor is hoofd van de afdeling Medische Microbiologie van het UMCZ en promotor van meer dan 10 promotieonderzoeken over ICU-infecties, antibioticumrichtlijnen en antibioticum-resistentie. Diverse studies over dit onderwerp zijn gefinancierd door ZonMw. Dr. W.G. Leider is in 1996 gepromoveerd in Weetdam op een onderzoek naar longinfecties. Thans werkt zij als staflid internist en infectioloog op de afdeling Medische Microbiologie. Hoewel niet gespecialiseerd in infectiecontrole op ICU’s, heeft zij gepubliceerd over infectieziektenuitbraken (o.a. ref. 20 en 22) en over een sepsismodel (ref. 21 en 23). In totaal heeft zij meer dan 40 publicaties op haar naam staan en was zij twee keer copromotor.

Historie subsidieaanvraag Dit is een eerste indiening Toelichting op de begroting Onder supervisie van twee leden van de projectgroep (WL en BG) zal een artspromovendus het onderzoek opzetten en uitvoeren. Hij/zij zal de gegevens analyseren onder supervisie van een statisticus-epidemioloog (AV). Een promovendus is nodig omdat het onderzoek fulltime aandacht vraagt van een toegewijde arts-onderzoeker. Voor supervisie worden geen kosten in rekening gebracht bij de subsidiegever. De kosten voor de antibiotica zijn voor rekening van de zorgverzekeraars, L. plantarum 299V met vezel wordt gratis ter beschikking gesteld door de producent (AB Probi, Lund, Zweden). Van de producent wordt verder geen financiële steun verkregen en op geen enkele manier zal AB Probi invloed kunnen uitoefenen op het verloop van het onderzoek. Standaard kweken vallen onder de reguliere patiëntenzorg in het ICU en zullen door de zorgverzekeraars vergoed worden. Dat geldt niet voor de kweken van monsters van de omgeving en voor het kwantificeren van L. plantarum 299V. Omdat dit specifiek voor het onderzoek wordt uitgevoerd, zijn de kosten in de begroting opgenomen.

Project information Summary Inflammatory diseases of the skin are highly prevalent in the general population. Genetic studies on monogenic and complex skin diseases have indicated that the innate immune system and barrier function of the skin contribute to their etiology and pathogenesis. Recent advances in the cell biology of inflammation, such as the discovery of Toll-like receptors (TLRs), NOD-like receptors (NLR) and the inflammasome, have provided arguments to consider a role for innate immune mechanisms in a number of skin disorders. Previous work of our groups has focused on the epidermal innate immune system in common skin diseases and clinical studies on the role of inflammasome-mediated signaling in Schnitzler syndrome, a rare auto-inflammatory disease. The present project aims to study auto-inflammatory processes in human epidermis. Schnitzler syndrome will be used as a model because of the clear role of interleukin-1 (IL-1) in its pathogenesis. Results from experiments with Schnitzler syndrome will then prompt experiments in other, more common skin diseases. Specific aims are to: 1 Make an inventory of expressed genes of the TLR and NLR pathways in vivo, in normal and inflamed epidermis. 2 Investigate the inflammasome in cultured normal human keratinocytes. 3 Investigate inflammasome and IL-1beta signaling in primary keratinocytes, monocytes and macrophages from Schnitzler syndrome patients, compared to controls and patients with other inflammatory skin diseases. 4 Extend the clinical studies on Schnitzler syndrome. To this end we will take the following steps: 1 A comprehensive gene expression analysis will be performed in purified epidermal cells from controls and patients with inflammatory skin diseases (Schnitzler’s syndrome, psoriasis and atopic dermatitis). 2 In vitro, expression of these genes will be studied in normal cultured keratinocytes under various conditions of stimulation, such as TLR/NLR ligands and inflammatory cytokines. Analysis will be performed by qPCR, ELISA and fluorimetric bead analysis. IL-1beta gene induction, protein processing and secretion will be studied in detail. 3 Primary keratinocyte cultures of Schnitzler syndrome patients will be established for comparison with keratinocytes from controls and other inflammatory diseases (cell banks are already available). Keratinocytes, monocytes and monocyte-derived macrophages will be stimulated in vitro and analyzed as mentioned under part 2. This will allow us to study

cell-autonomous differences in inflammasome function between patients and controls. 4 We will investigate the histopathology of skin from patients with Schnitzler syndrome, and the relation to similar diseases such as chronic idiopathic urticaria and cryopyrin-associated periodic syndromes. Germline-encoded pattern recognition receptors (PRRs), and their recognition of highly conserved pathogen-associated molecular patterns (PAMPs), allow the innate immune system to discriminate between different types of pathogens and also to generate an appropriate (adaptive) immune response. These PRRs include the Toll-like receptors (TLRs), NODlike receptors (NLRs), C-type lectin receptors (CLRs), and several other receptors that contribute to immune activation in response to diverse stimuli, including infection or tissue injury (3-5). Very similar to the other innate immune receptor molecules like the TLRs, the NLR proteins are involved in sensing the presence of pathogens via PAMPs; however, they also can detect endogenous danger or stress signals. This is very well illustrated by the discovery of mutations or functional alterations in NLR-related pathways that are associated with chronic noninfectious inflammatory diseases such as Blau syndrome and Crohn’s disease (6), Muckle–Wells syndrome and Familial cold urticaria syndrome (7). For this reason, these diseases are collectively referred to as auto-inflammatory diseases (8).

The role of innate immunity in autoinflammatory syndromes is now fairly well recognized, and we are just beginning to understand the molecular mechanisms that are involved in the pathogenesis of these rare diseases, and the potential role in inflammation and infection more generally. A central concept in the biology of auto-inflammatory processes is the inflammasome (9), a molecular platform for the activation of inflammatory caspases leading to caspase-1 mediated cleavage of IL-1beta or IL-18 (10). An intriguing disorder in which there is a strong indication of a central role for dysfunction of the inflammasome is Schnitzler syndrome. This disease is characterized by chronic recurrent urticaria, fever, bone pains and gammopathy (11). Its pathophysiology is unclear at present, but an increased secretion of IL-1beta by monocytes has been demonstrated (12;13). Also, inhibition of IL-1 is a very successful treatment option, which results in a complete resolution of the urticaria and fever. Most of our knowledge on NLR and TLR biology is derived from model systems that use monocytes or macrophages (5;10;14) whereas the inflammasome signaling has largely been performed with macrophage cell lines only. The question whether inflammasome signaling is active and relevant in epithelia is largely unanswered. In vitro studies have indicated that keratinocytes express TLRs and can respond to selected TLR ligands (15;16). Some of the components of the inflammasome were recently found to be present in human epidermal keratinocytes, and it was shown that exogenous stimuli like skin irritants and contact sensitizers could activate IL-1beta processing and secretion (17). The relevance of NLR and TLR signaling in keratinocytes for normal host defense (infection and inflammation) or for common inflammatory diseases like psoriasis and atopic dermatitis is hitherto unexplored. The main questions of the current application deal with (a) activity and function of the inflammasome in keratinocytes relative to monocytes and macrophages, and (b) the relevance of these pathways for Schnitzler syndrome and other selected skin diseases. The current application is based on our earlier studies on Schnitzler syndrome, the expertise on infection and innate immunity of the department of Internal Medicine, and the expertise of the department of Dermatology on host defense and inflammation of human epidermis (see the list of publications). We have previously shown that patients with Schnitzler syndrome, a disease whose symptoms include intermittent fever and chronic urticaria, respond favorably to IL-1 blockade. We initiated several clinical studies and established international collaborations to review all cases published. With respect to keratinocyte biology we have focused on aspects of cutaneous host defense and discovered

several key molecules in these processes. Recently we identified beta-defensins as a novel risk factor in psoriasis. Our unpublished data on human skin and cultured keratinocyte models, suggest that TLR and NLR are important in stress signaling in epidermis, and that IL-1beta transcription, processing and secretion is under control of these pathways. Collectively, our clinical work and laboratory findings provide a solid base to investigate inflammasome biology and IL-1beta signaling in human epidermis, and the pathogenetic role in Schnitzler syndrome as a model disease. Relevance Inflammatory skin diseases such as psoriasis, atopic dermatitis, chronic urticaria and (occupational) contact dermatoses are highly prevalent, and pose a serious problem to patients and society. Although the etiology and pathogenesis of many of these diseases was formerly attributed largely to the adaptive immune system, it is now understood that epithelium-expressed genes involved in barrier function and innate immunity are equally important. The recent advances in our knowledge of the innate immune system such as the TLRs, antimicrobial proteins and the inflammasome call for further research regarding the role of the innate immune system in inflammatory diseases. Although the disease that we will study in detail (Schnitzler syndrome) is rare, we know from previous studies that uncommon disorders are exquisitely suited to provide insights in more common conditions. We expect that the results of this project will be broadly applicable to the biology of inflammatory dermatoses. More specifically we expect to define the possible role of inflammasome signaling and processing of cytokines in specific diseases. This will provide a scientific basis of rational, novel therapies that are mechanism-based. Again, Schnitzler syndrome will set a nice example, as we have demonstrated the efficacy of IL-1 blockade by a recombinant IL-1 receptor antagonist. Ultimately, we aim to elucidate the mechanism by which interference with IL-1 signaling in the skin, suppresses inflammatory changes such as urticaria. Various levels of crosstalk between TLR and NLR have been described and it appears that the inflammasome requires input from both pathways. Molecular dissection of these interactions is an area of intense research and will undoubtedly improve our understanding of the mechanisms by which these protein platforms are activated and fulfill an essential role during the inflammatory cytokine response. So far most of the data on pattern recognition have been obtained in cellular system involving monocytes or macrophages, whereas inflammasome studies mainly have used cell lines. Recently our

group has succeeded in demonstrating inflammasome activation in primary monocytes and monocyte derived macrophages. Interestingly, monocytes have an intrinsically activated inflammasome whereas macrophages do not: like the macrophage cell lines the inflammasome needs to be activated to convert procaspase-1 into caspase-1 and thereby enable the activation of IL-1beta. Here we aim to study these phenomena in human skin, which is an anatomical location where inflammation is common and easily observed. Our project combines clinical expertise and availability of a well-defined patient group with state-ofthe-art molecular cell biology of skin inflammation. The role of the inflammasome in inflammatory skin disease has largely remained unexplored, and we are confident that this project will generate novel insights both at the basic level and in relation to human disease. The clinical studies that are scheduled in the current application will be done in close collaboration with the department of AIG. The experimental studies will be performed in the laboratory of skin biology and experimental dermatology that focuses on the role of epidermal innate immunity in inflammatory skin diseases. As such, this application is complementary to ongoing research on genetics and molecular mechanisms of these conditions. Knowledge transfer, Implementation, Consolidation The results of our studies will be published in peer-reviewed scientific journals and communicated on international meetings. In addition, we will make this knowledge easily accessible to patients and physicians via the Schnitzler syndrome website (www.schnitzlersyndrome.com). Objective The overall, more distant aim is to define the role of inflammasome-mediated processes in inflammatory skin diseases. Specific aims and questions of the current application are: 1 To learn which of the known TLR, NLR and CLR signaling and effector molecules are expressed in epidermis, in vivo 2 To elucidate (in vitro) whether these genes are expressed constitutively or whether keratinocytes require activation to express these genes; if so, how are distal effector proteins (caspase-1, IL-1beta) activated to exert their function? We will use monocytes and monocyte-derived macrophages as a reference cell type in these experiments. 3 To use these results in combination with data from our previous studies to learn whether cell-autonomous differences in inflammasome signaling exist between different diseases. from healthy controls (large cell bank available), Schnitzler syndrome patients (to be established in this project), and other inflammatory skin diseases (large cell bank available). We will investigate if keratinocyte-derived factors can activate target cells to secrete vasoactive mediators that are involved in urticarial reactions. In parallel, we will study these processes in monocytes and macrophages from the same patients. 4 To increase our diagnostic, pathophysiologic and possibly therapeutic knowledge on Schnitzler syndrome, we will perform clinical studies in patients with the disease. What are the similarities and differences between Schnitzler syndrome and common diseases such as chronic idiopathic urticaria, or cold urticaria? And how does Schnitzler syndrome compare with the rare genetic cryopyrin-associated periodic syndromes, where the pathogenetics is much better defined? Strategy We plan to do four types of studies, each with a focus on one of the four specific objectives. 1 In vivo studies on molecular components of TLR, NLR and CLR signaling and effector pathways We have previously performed a microarray analysis of gene expression in vivo in lesional skin of psoriasis and atopic dermatitis patients [ref ]. This yielded a comprehensive, although not complete, list of expressed genes that encode TLRs, NLRs, CLRs, antimicrobial proteins, cytokines, chemokines and proteases involved in processing. These data were validated using quantitative real-time PCR (qPCR). We have > 200 validated qPCR assays for genes involved in skin biology and inflammation that can be used with greater accuracy and flexibility than alternative methods as microarrays or microfluidics qPCR cards. These assays will be used to analyze profiles of expressed genes in purified epidermal cells. Briefly, we isolate human epidermis from 4 mm punch biopsies as described previously, at 4oC in a 4 h protocol to avoid induction and breakdown of mRNA [ref ]. This precludes admixture of dermal cells, adnexal structures and inflammatory cells, and will yield >90% pure keratinocyte populations for mRNA extraction. Tissues to be analyzed are derived from normal skin and inflammatory skin conditions from which we have lesional and non-lesional skin. These include psoriasis, atopic dermatitis, allergic contact dermatitis, irritant contact dermatitis, and superficial skin injury. These mRNA samples are already available in our skin bio-bank (biopsies, serum, cultured cells, DNA, mRNA, clinical data) that we have collected over the past two years. In addition we will collect lesional and non-lesional epidermis from patients

with Schnitzler syndrome and chronic idiopathic urticaria. Schnitzler syndrome patients will be recruited via our national and international network. Chronic urticaria patients will be recruited via our out-patient department. Analyses will be largely done by qPCR because of its accuracy and throughput. When we consider a mean difference of 2 PCR cycles between groups to be biologically relevant, power calculations show that 10-12 individuals will be sufficient to give a false negative rate of 0.2 which is acceptable. Large scale qPCR analysis (30-40 genes of the aforementioned pathways) on these samples will provide a comprehensive overview of expressed genes. Data will be analyzed by multivariate techniques (factorial ANOVA), using the appropriate corrections for family-wise error, as we have recently published [ref ]. 2 In vitro analysis of inflammasome components and activity Normal human keratinocytes will be cultured as previously described [ref ] and will be stimulated by cytokine mixtures associated with Th1 cells (gammainterferon, TNF-alpha and IL-1), Th2 cells (IL-4 and IL-13) and Th17 cells. We have previously found that keratinocytes are responsive to these stimuli and show distinct expression of antimicrobial proteins and other markers [refs]. In addition we will test TLR/NLR/CLR ligands such as double stranded RNA analogs, muramyl dipeptide (MDP), flagellin, LPS and imiquimod. The project will greatly benefit from the expertise of the lab of General Internal Medicine that has extensive experience with these ligands in systems using monocytes and macrophages. We will run these experiments on the inflammasome and TLR/NLRs in keratinocytes in parallel with similar studies in monocytes and macrophages that serve as positive controls. Some of these ligands have been investigated by us in pilot experiments with keratinocytes and it was found that these cells are responsive to some of these ligands, as measured by induction of chemokine and IL-1beta expression. Selection of genes to be measured as readout is based on literature findings, our previous microarrays, and the outcome of part 1A of the project. The induction of TLR/NLR/CLR genes will be measured by qPCR. Secretion of IL-1beta will be analyzed by luminex bead assay or an ELISA that is specific for the processed 17 kDa form. Previous experiments by us and others have indicated that although IL-1beta and IL-18 are highly expressed (mRNA and intracellular protein), very little is actually processed and secreted using cytokine stimulation. For this reason we will also examine additional stress factors as co-stimulators to investigate synergistic effects on IL-1beta secretion. These factors include ATP, serum, contact allergens and skin irritants. In addition we will examine the effect of cell culture under different temperatures. Normally, keratinocytes are cultured at 37oC, but this is not the normal temperature to which cells are exposed in vivo

(the actual skin temperature is a few degrees lower). These experiments will address the question if a temperature-dependence exists. We will also examine higher temperatures (up to 41oC) to mimic conditions during fever. To analyze IL-1beta processing, we will extract the cells and collect the cell supernatant. Pro-IL-1beta (31 kDa) and processed (17 kDa) will be measured with ELISAs that specifically recognize and quantify either form. Western blot analysis will be performed as an additional control. The dependence of this process on caspase-1 activation will be investigated in two ways. First using western blots caspase-1 activation will be measured (this assay has been recently developed in the AIG laboratory for use with primary cells). Secondly addition of the specific caspase-1 inhibitor YVAD-FMK will allow to further assess the degree of caspase activation. Measurement of processing and secretion of IL-18 will corroborate the activation of caspase-1, since this is also a caspase-1 activated cytokine. We have recently found that keratinocytes also produce large amounts of the new IL-1 family members IL-1F8 and IL-1F9. Very little is known about processing and secretion of the cognate proteins. An optional experiment will be to analyze these proteins in our cellular system (collaboration with prof. C. Borat, UMCX). 3 Cell-autonomous processes Previous studies from our lab have shown that cell-autonomous differences exist between cultured keratinocytes from psoriasis and atopic dermatitis patients, with respect to expression of innate immunity genes following activation by cytokines [ref ]. Here we want to address if similar differences can be demonstrated between cells from Schnitzler syndrome patients, healthy controls and patients with other inflammatory diseases, with respect to TLR/ NLR/CLR signaling. Cell banks from healthy controls and psoriasis and atopic dermatitis patients are available. Keratinocytes from Schnitzler syndrome patients will be obtained from 4 mm punch biopsies. The epidermal cells are separated enzymatically from the dermis and plated on irradiated 3T3 feeder cells and cultured to confluency. First passage cells are stored in liquid nitrogen and used for experiments. We aim at establishing primary cell cultures from at least 7 patients. Because Schnitzler syndrome is quite rare we will, if necessary, use our international collaborations to collect sufficient material. Keratinocytes, monocytes and macrophages from patients and controls will be used in cell stimulation experiments, using a selection of stimuli based on the findings in part 1 and 2. Pilot experiments indicate that poly I:C (double stranded RNA analog), MDP and flagellin are potent TLR/NLR ligands in keratinocytes. For monocyte/macrophage stimulation we will use LPS. The readout of these experiments will consist of 20-30 genes associated with keratinocyte activation following TLR/NLR stimulation as determined under

part 1B. Processing and secretion of IL-1beta and IL-18 will be measured by ELISA and luminex assays, as described above. The experimental design to run these experiments on the inflammasome and TLR/NLRs in keratinocytes alongside with similar studies in monocytes and macrophages from the same controls and patients, will allow us to investigate if there is a general defect or a defect confined to specific cell types. In addition to our studies on keratinocytes as a model cell system to investigate inflammasome signaling in healthy skin and skin diseases, we also want to investigate whether keratinocytes are directly involved in the skin symptoms of Schnitzler syndrome. To this end we want to perform functional studies that will address the question if mediators produced by keratinocytes can be causally linked to urticaria as seen in these patients. Based on the results obtained under the previous headings, we will select stimuli to which keratinocytes from Schnitzler syndrome patients and healthy controls will be exposed. Although Schnitzler syndrome patients respond poorly to histamine antagonists, we hypothesize that at least part of the urticarial lesions are mast cell dependent, possibly by mast cell derived mediators other than histamine. The culture supernatants will be used to stimulate the HMC-1 mast cell line (collaboration with dr C. Laegue, Mayo Clinic, USA). As readout for activation we will use tryptase and histamine secretion, using published assays. None of these techniques are currently used in our lab and time will be allocated to set up the mast cell line culture and appropriate activation readout assays. 4 Clinical and histopathological studies on Schnitzler syndrome Here we want to further characterize dermatological aspects of Schnitzler syndrome, with the aim of improving diagnosis and distinguishing this disease from disorders such as chronic idiopathic urticaria, a more common inflammatory skin disease. At the moment, the main feature distinguishing Schnitzler syndrome from (the more severe forms of) chronic idiopathic urticaria is the presence of a gammopathy, which is part of the definition of Schnitzler syndrome. This gammopathy seems to be an epi phenomenon, not directly causative, since it can sometimes appear years after the appearance of urticaria, the level of paraprotein is not related to disease severity, and treatment which abolishes all disease symptoms does not necessarily affect the concentration of paraprotein. It is our hypothesis that the present definition may be too restrictive, and that Schnitzler syndrome is not so rare as generally thought. For example, in our Internal Medicine Clinics, we have diagnosed at least 4 patients with Schnitzler syndrome in the past 4 years. We want to look more closely at patients

with chronic idiopathic urticaria, especially those who also have systemic features such as fever and general malaise. At the skin level, the following studies will be performed. Skin biopsies from both patient groups will be collected from involved and uninvolved skin (both for cryosections and for formalin fixed paraffin sections). Histopathological changes will be examined (collaboration with the department of Pathology) and specific cell populations within the cellular infiltrate will be analyzed using immunofluorescence. Activation status of the epidermis will be analyzed using markers established in our lab (SKALP/elafin, hBD-2). Histopathological findings in urticaria are often non-specific, such as dermal edema and a sparse perivascular mixed inflammatory infiltrate. In the case of Schnitzler syndrome the overlap or delineation from idiopathic chronic urticaria and urticarial vasculitis will be made. The patients will also be compared at the clinical level. A detailed examination will be made of the clinical features (with emphasis on skin phenotype) of chronic idiopathic urticaria versus Schnitzler syndrome, as to development of phenotype, response to therapy and prognosis. In 2008, the candidate did a research internship on IL-1 and TNF-receptor in TNF-receptor associated periodic syndrome (TRAPS) and Schnitzler syndrome, supervised by Dr V. Super and Dr S. Man in the National Institute of Skin diseases in Bethesda, USA. The following techniques were used: electrophoresis, Western blot, cell cultures. Earlier, she participated in a HIV project at the Department of Virology, Kyoto, Japan, resulting in a co-authorship of a paper that was recently submitted. The techniques used were: electrophoresis, polymerase chain reaction, plasmid-mediated selective gene cloning, and gene sequencing. She has the ambition to broaden her interest from clinical research to laboratory-oriented and translational research, whilst keeping her focus on the subject of auto-inflammation. During her internships in Japan and the USA she has already worked in research laboratories, which provides her with an excellent background for the proposed project. The candidate has first-class writing skills which is of course a strong asset. Her grades so far were of remarkable distinction, and we think that she belongs to the high-potentials from which future academic dermatologists will be recruited.
