Klasické rentgenové snímky zaznamenávají trojrozměrný prostor na dvojrozměrný rentgenový film, jednotlivé útvary se tedy vzájemně překrývají a jejich obrazy jsou různou měrou zkresleny. Khodnocenipolohyretinovaného stálého horního špičáku v prostoru čelisti je nutné použít některou z radiologických lokalizačních metod. Ve sdělení je podán přehled existujících radiologických lokalizačních metod s jejich výhodami a nevýhodami. (Ortodoncie 2003,12, č. 3, s. 20-25). Summary One ofthe objectives of radiological diagnostics ofthe impacted permanent maxillary canine is to obtain the most precise Information on its localization within the maxilla and on its relations to the surrounding anatomical structures. omyly rentgenové diagnostiky, které dělí do dvou skupin - na omyly obecné a lokalizační. Jeho závěry jsou důležité i pro diagnostiku retinovaných špičáků. Abychom se vyhnuli obecným omylům, musíme si uvědomit následující. Vlivem rozptylu a divergence rentgenových paprsků se nestejnoměrně zobrazí spongióza kosti a slabé kořeny, resp. jejich hroty. Tedy kořeny prvních premolárů nebo i retinovaných horních špičáků se mohou ztratit z kresby. Také je mylný názor, že v rentgenové kresbě kompakta bezpodmínečně zatlačuje spongiózu, resp. kořeny okolní spongiózu. Poměry jsou komplikovanější, jedním z důležitých činitelů je poměr množství spongiózy a kompakty v dané vrstvě kosti. Proto také hroty kořenů bývají někdy nezřetelné, protože se přes ně klade kresba spongiózy a zastírá je. Zároveň neplatí poznatek, který se traduje ve všeobecné rentgenologii, že je možné lokalizovat podle ostrosti kresby. A sice, že útvary ležící blíže k filmu se zakreslí ostřeji než útvary vzdálenější. To všeobecně platí, ale nikoliv u zubů, které mají korunku velmi hutnou a kořen značně transparentní. Tento poznatek tedy nemůžeme aplikovat pro případy, kdy korunka retinovaného špičáku překrývá kořen sousedního zubu. V horní čelisti jsou lokalizační poměry značně složité. Při lokalizaci se neobejdeme bez použití některé z lokalizačních metod. Radiologické lokalizační metody dělíme: 1. Méně přesné metody: - metoda zvětšení obrazu (obrazové zvětšení) - metoda navrstvení (překrývání) obrazu 2. Přesné metody: - paralaxní metoda (též Clarkova metoda, Clarkovo pravidlo, pravidlo bukálního objektu, metoda posunu tubusu rentgenového přístroje) - pravoúhlá technika - stereoradiografie ad. Metoda zvětšení obrazu (2,3) Tato metoda je založena na principu, že objekt umístěný těsněji k filmu (tj. dále od rentgenového přístroje) vrhá menší stín než objekt lokalizovaný ve větší vzdálenosti od filmu a těsněji k přístroji. Tedy při hodnocení polohy zubu na ortopantomogramu budou zuby umístěné palatinálně od zubního oblouku (resp. dále od filmu) zvětšeny ve srovnání s velikostí stínů zubů v linii oblouku. Stíny zubů umístěných vestibulárně budou zmenšeny. Nedoporučuje se hodnotit velikost např. neprořezaného špičáku s velikostí kontralaterálního špičáku z těchto důvodů: - velikost všech zubů v kontralaterálním kvadrantu může být ovlivněna nevhodným centrováním hlavy pacienta při snímkování - bilaterální přemístění špičáku se vyskytuje v 41,6% případů

diographic examination and divided them into two groups generál errors and localization errors. His conclusions are important for the diagnostics of impacted canines as well. To avoid generál errors we háve to také into consideration the following. Through diffusion and divergency of X-rays the bone spongiosis and weak roots (theircusps) will beprojected unequally. Therefore, the roots of first premolars or even impacted maxillary canines may disappear from the picture. Another erroneous opinion is that in a radiograph the compact bones always push back spongiosis or the roots surrounding spongiosis. The situation is more complicated, one important factor being the proportion of spongiosis and compact bone in the given layer of a bone. Therefore, sometimes apices of roots are faint as the image of spongiosis overlaps them and hide them. In generál radiology it is said, also erroneously, that it is possibleto localizean object according tothesharpness of picture, i.e. thestructures closer to the film are sharper than the more distant ones. This is true in generál, however, this is not true in teeth. Teeth háve a very dense crown and rather transparent roots. This knowledge, therefore, cannot be applied in cases when the crown of the impacted canine overlaps the root of an adjacent tooth.Conditions for localization are very complexin the maxilla. We cannot do without the use of some of the radiological methods of localization. Methods of radiological localization are divided: 1. Less accurate methods: - the method of magnification (magnification of the image)(image magnification) - the method of superimposition (overlapping) of the image (image superimposition) 2. Accurate methods: - parallax method (also Clarks method, Clarks principle, principle of buccal object buchal object rule, method of the radiograph tube shifting tube shift method) - right angletechnique - stereoradiography Ad. Magnification method Image magnification (2,3) The method is based on the principle according to which the object located doseto the film (i.e. more distant from the RTG appratus) casts a smaller shadow than the object located in a more distance from the film and closer to the apparatus. When assessing the location of a tooth in the orthopantomogram the teeth located palatinally from the dental arch (i.e. those more distant from the film) will be magnified in comparison with the size of the teeth located in the line of the arch. The shadowsof the teeth located vestibularly will bediminished.ltisnotadvisabletoassesse.g.thesizeofan unerupted canine by comparison with the size of the contralateral canine. The reasons are the following: - the size of all teeth in the contralateral quadrant may be influenced by the inappropriate centering of a patienfs head during X-rays 21

Platí, že horní špičák je průměrně o 0,5mm širší než první premolar. Wolf a Mattila (4) však shledali metodu zvětšeníobrazu jako nespolehlivou. Chaushu S., Chaushu G. a Becker (3) se snažili zdokonalit metodu lokalizace neprořezaného špičáku pomocí jednoho panoramatického snímku. Hodnotili polohu špičáku pomocí Indexu špičák-řezák = poměr nejširšího mesiodistálního rozměru špičáku k nejširšímu mesiodistálnímu rozměru homolaterálního středního řezáku. Tuto metodu však shledali nespolehlivou pro vysoko umístěné špičáky, tj. v apikálnízóně přilehlých zubů. Polohu špičáku hodnotili také podle Indexu špičák-špičák = poměr nejširšího mesiodistálního rozměru postiženého špičáku k nejširšímu mesiodistálnímu rozměru nepostiženého špičáku. I přestože kladli veliký důraz na správnou polohu kontralaterálního špičáku, přesná diagnóza polohy špičáku byla možná pouze asi v 88% případů. Fox, Fletcher a Horner (5) ve své studii hodnotili přesnost předpovědi polohy neprořezaného špičáku, která je založená na hodnocení panoramatických snímků užívající princip zvětšeníobrazu v horizontální rovině. Dospěli k závěru, že je možné předpovědět polohu neprořezaného špičáku na základě rozdílného zvětšení na panoramatickém snímku asi v 80% případů. Tento nález má být však pouze nedokonalým vodítkem k poloze neprořezaného špičáku a má napovědět, že jsou nutná dalšívyšetření. Protože panoramatické snímky bývajíčasto u pacientů zhotovovány jako snímky první volby, vyslovili naději, že užitím metody zvětšeníobrazu by mohly ektopické špičáky být detekovány dříve a dříve by mohla být zahájena vhodná léčba, resp. interceptivní opatření (např. extrakce dočasného špičáku). Připouštějí, že přesnost panoramatických snímků je velmi závislá na dobré kvalitě obrazu.

The size of the crown of an unerupted canine should be compared with the size of the crown of the first premolar in the samé quadrant. On the average the maxillary canine is wider by 0.5mm than the first premolar. Nevertheless, Wolf and Mattila (4) found the method of magnification unreliable. Chaushu S., Chaushu G. and Becker (3) tried to improve the method of localization of the unerupted canine by means of one panoramic radiograph. They evaluated the canine position with the help of Index canine-incisor, i.e. the proportion of the widest mesiodistal measure of the canine to the widest mesiodistal measure of homolateral centrál incisor. However, they found the method unreliable for the high located canines, i.e. those located in the apical zone of adjacent teeth. The location of the canine was evaluated also by means of the Index canine-canine, i.e. the proportion of the widest mesiodistal measure of the affected canine to the samé measure of the unaffected canine. Though they put the emphasis on the accurate location ofthe contralateral canine the precisediagnosisof the canine position was possible only in 88% of cases. In their study, Fox, Fletcher and Horner (5) evaluated the accuracy ofthe prediction ofthe unerupted canine location which is based on the assessment of panoramic radiographs magnified in the horizontál plane. They came to the conclusion that the location of an unerupted canine may be predicted on the basis of different magnification in the panoramic radiograph in about 80% of cases. The finding, however, serveš only as a guideline and should indicate that other examinations are necessary. As panoramic radiographs are often the first choice in patients the authors believe the method of magnification may help to detect ectopic canines earlier and thus the appropriate treatment may start earlier, or interceptive measures may be adopted (e.g. deciduous canine extraction). They admit the accuracy of panoramic radiographs is highly dependent on the good quality of resolution.
ad. Metoda překrývání obrazu Wolf a Mattila (4) ve své studii nalezli, že všechny (celkem 47) špičáky, které se na panoramatickém snímku překrývaly s kořenem nebo krčkem středního řezáku byly umístěny palatinálně. Z toho učinili závěr, že překrývání středního řezáku je stálý znak palatinální polohy neprořezaného špičáku. Jacobs (6) jejich tvrzení později jednoznačně vyvrátil.
Ad. Method of overlapping Image superimposition In their study ,Wolf and Mattila (4) found out that all (47) canines which overlapped in the panoramic radiograph with the root or cervix of the centrál incisor were located palatinally. This led to the conclusion that overlap with the centrál incisor is the attribute of the palatinal location of the unerupted canine. Jacobs (6) later refuted their statement.
ad. Paralaxní metoda Je založena na principu paralaxe (Obr. 1). Paralaxe v radiologických termínech znamená přemístění obrazu sledovaného objektu (např. retinovaný zub) vzhledem k obrazu referenčního objektu (např. kořen přilehlého zubu). Toto přemístění je způsobeno změ-

In radiological terms parallax means displacement of the image oftheobjectmonitored (e.g. unerupted tooth) with regard to the image of the reference object (e.g. root of the adjacent tooth). This dislocation is caused by the change in the

nou v angulaci centrálního paprsku, tedy změnou v poloze tubusu rentgenového přístroje mezi dvěma expozicemi. Obraz zubu, který je dále od tubusu než referenčnízub se pohybuje ve stejném směru jako tubus, kdežto obraz zubu, který je blíže, se hýbe v opačném směru k tubusu. Je nutné si uvědomit, že čím je sledovaný zub umístěn těsněji k referenčnímu zubu, tím menší posun obrazu mezi dvěma expozicemi zaznamenáme. V české literatuře se setkáváme s označením „princip jedoucího vlaku" (7). Paralaxní metodu poprvé představil v roce 1910 Clark. Ten používal 2 apikální snímky a posun tubusu v horizontální rovině. V roce 1952 to byl Richards, který si uvědomil, že je možné posouvat tubus také ve vertikální rovině. K dalšímu zlepšení metody došlo v roce 1986, kdy Keur nahradil apikální snímky za okluzní, které zobrazují větší oblast. Keurova modifikace má dvě výhody: 1. Mezi dvěma expozicemi je možné provést větší posun tubusu, tedy i posun obrazu sledovaného zubu na okluzních snímcích bude větší a lépe hodnotitelný. 2. Na okluzních snímcích na rozdíl od apikálních snímků je zobrazen zpravidla celý sledovaný zub a dostatek okolních referenčních struktur pro hodnocení polohy, založeném na principu paralaxe. Keur také poprvé navrhl kombinaci panoramatický snímek (ortopantomogram) a okluzní snímek pro radiologickou lokalizaci neprořezaných zubů. Ortopantomogram bývá často brán jako počátečnísnímek. Potom zhotovení okluzního snímku znamená pouze jednu expozici navíc (expozice okluzního snímku je přitom stejná jako expozice apikálního snímku). Diagnostický přínos této metody je vysoký při minimalizaci dávky radiace. U ortopantomogramu má centrální paprsek angulaci přibližně 0-7°, u okluzního snímku má angulaci 70-75°, to znamená, že tubus rentgenového přístroje se mezi oběmi projekcemi posune ve vertikálním směru kraniálně, resp. apikálně (Obr.2). Kombinace ortopantomogramu a apikálního snímku, u kterého je angulace 0-30° (Obr.2), se nedoporučuje, neboť posun tubusu a tedy i posun obrazů zubů na snímcích je malý nato, abychom mohli spolehlivě určit polohu sledovaného zubu.

angulation of the centrál ray, i.e. by the change in the position of the RTG tube between two exposures. The image of the tooth which is more distant to the tube than the reference tooth moves in the samé direction as the tube, while the image of the tooth which is closer to the tube moves contrary to the tube. We háve to bear in mind that the closer the monitored tooth is to the reference one, the shorter move of the image between two exposures will be recorded. In the Czech literatuře weknowthis as "the principleof a running train" (7). The parallax method was first introduced by Clark in 1910. He ušed two apical images and the tube was moved in a horizontál plane. In 1952 Richards realized that the tube may be moved also in a vertical plane. In 1986 Keur improved the method by use of occlusal images instead of apical ones; occlusal images display a larger area. Keuřs modification has two advantages: 1. In between two exposures it is possible to make a larger move of the tube, i.e. also the move of the image of the tooth monitored will be larger in occlusal images and therefore it will be easier to evaluate it. 2. In contrast to apical images the occlusal images provide usually the projection of the whole tooth monitored as well as enough adjacent reference structures which allow to evaluate the location based on the parallax principle. Keur was the first to suggest the combination of panoramic image (orthopantomogram) and occlusal image to radiographic localization of unerupted teeth. Orthopantomogram is often the method of the first choice. Then the occlusal radiograph means only one additional exposure (the exposure of the occlusal image is the samé as the exposure of the apical one). Benefits of the method for diagnostics are great while the radiation is minimalized at the samé time. The angulation of the centrál X-ray in orthopantomogram is approximately 0-7°, in occlusal radiograph the angulation is 70-75°. That means the tube of RTG apparatus moves during the pause between the two projections in vertical direction cranially, resp. apically (Fig.2). Combination of orthopantomogram and apical radiograph, in which the angulation is 0-30°(Fig.2), is not recommended - the shift of the tube and the shift of teeth images in radiographs is too small for us to be able to reliable statě the location of the monitored tooth.

Metoda zvětšení obrazu může být při znalosti metodiky hodnocení užívána pouze jako orientační metoda, indikující nutnost dalších vyšetření a léčebných opatření. K určení přesné polohy retinovaného špičáku je nutné použít buď paralaxní metodu, nebo pravoúhlou techniku, nebo jejich kombinaci. Praktická aplikace těchto metod bude náplní další části našeho sdělení. 24
Conclusions Inaccurately located impacted canine may result in the choice of inappropriate method of surgical exposure during orthodontic-surgical treatment of the impacted maxillary canine. There is still a number of vague points within the knowledge and utilization of individual methods of radiographic localization of impacted teeth. The presented article gives the survey of all radiographic methods of localization and references on their reliability. Magnification method may be ušed only as an orientation method indicating other examinations and therapeutic measures. To determine the accurate position of the impacted canine we háve to use either parallax method or right-angle technique or the combination of both. Practical application of these methods will be introduced in the following part of our study.
