Foreword
Pozitronová emisní tomografie (PET) na začátku nového století představuje nejdynamičtěji se rozvíjející diagnostickou zobrazovací modalitu v celosvětovém kontextu klinické medicíny. Vývoj se přirozeně nevyhnul ani České republice, kde je v pražské Nemocnici Na Homolce od roku 1999 v provozu první PET centrum. Za 3,5 roku jeho činnosti nabyli pracovníci zkušenost s více než 6.000 PET vyšetřeními. Díky těsné spolupráci s řadou renomovaných pracovišť odesílajících pacienty k PET vyšetření získali autoři zpětnou vazbu – cennou zkušenost, která umožňuje kriticky posoudit možnosti a přínos nové metody. Předkládaná kniha představuje završení průkopnické práce zaměstnanců Nemocnice Na Homolce při zavádění PET do české klinické praxe. Shrnuje období, kdy byl v ČR k dispozici jediný specializovaný PET skener. Kniha byla vydána díky finanční podpoře firmy Lacomed s.r.o. jako čtvrté volné pokračování edice atlasů nukleární medicíny. Autoři se pokusili na předurčeném prostoru oslovit širší lékařskou veřejnost s cílem přiblížit a osvětlit běžné pojmy PET a demonstrovat možnosti metody. V úvodní části jsou uvedeny základní historické údaje o PET, stručně jsou vysvětleny principy PET, farmakokinetika FDG a použité parametry vyšetření. Těžiště knihy představuje atlas příkladů klinického využití FDG PET. Autoři si nekladli za cíl postihnout všechny smysluplné indikace ani hlouběji rozebírat všeobecně dostupné literární prameny. Záměrem je prezentování obvyklých i méně běžných případů z každodenní klinické praxe, tak aby si čtenář mohl vytvořit představu o možnostech FDG PET a o průřezu činnosti PET centra v Nemocnici Na Homolce. Veškerá data byla získána v rutinním provozu, tj. nikoliv za speciálních podmínek, jak bývá běžné u výzkumných studií ve většině center.
As this new century begins, Positron Emission Tomography (PET) has become the most dynamically developing diagnostic imaging modality for clinical medicine globally. This development has of course not bypassed the Czech Republic, where the first PET centre entered service in the Na Homolce Hospital in 1999. During its three-and-a-half years of operation, the centre’s personnel have gained in experience by conducting in excess of 6,000 PET examinations. Thanks to the centre’s close cooperation with a number of renowned medical facilities that have referred patients to them for PET examinations, the authors have managed to accumulate a mass of detailed feedback, which has proved invaluable in allowing them to assess both the benefits and scope of the new method. This book provides a summary of the pioneering work involved in introducing PET into Czech clinical practice, an era during which only one dedicated PET scanner existed in the whole of the Czech Republic. The publication of this book has been made possible through the support provided by Lacomed s.r.o., and is the fourth in the series of atlases of nuclear medicine. The authors have attempted to address a broader medical community within space available to them, with the objective of unveiling and explaining common PET terminology while at the same time demonstrating the method’s potential. The first part of this work summarises the basic PET historical data and briefly explains the principles of PET, FDG pharmaco-kinetics and the test parameters used. The second part covers examples of clinical applications of PET FDG. The authors did not aspire to cover all the relevant indications, nor did they analyse in-depth those literary sources that are generally and readily available. The primary aim of the book is to present common as well as less common cases arising in everyday clinical practice, and to enable the reader to gain some idea of the potential of FDG PET, as well as of a section of the Na Homolce Hospital PET Centre’s activities. All the data have been gathered from routine clinical operations, not under special conditions, as is often the case with research studies conducted by many of this type of centre.

A Brief History
1.1 Snímací technika
1.1 Scanning techniques
Využití anihilačního záření pozitronových zářičů pro lékařské zobrazování bylo poprvé popsáno v roce 1951. Tehdy nezávisle na sobě Sweet et al. a Wrenn et al. vyvinuli první skener použitelný k zobrazení mozku. Obsahoval dvě protilehlé sondy na bázi jodidu sodného NaI(Tl). V roce 1962 spatřil světlo světa první vícedetektorový systém. Vhodně uspořádané koincidenčně zapojené detektory umožňovaly dvojrozměrné zobrazení, poloha ve třetím rozměru byla odhadována podle ostrosti zobrazení (1968). V následujících letech byl vyvinut první tomograf PC-I a byl popsán v roce 1972 Brownellem et al. Obsahoval dvě dvourozměrná rotující detekční pole. Ani tento přístroj ještě nebyl navržen na principech počítačové tomografie. V šedesátých letech byly postupně zdokonalovány počítačové rekonstrukční algoritmy (Kuhl, Edwards aj.) V roce 1970 Chesler využil filtrovanou zpětnou projekci pro výpočet emisních a transmisních tomogramů a pro vytvoření emisních tomogramů korigovaných na absorpci záření ve tkáních. V roce 1973 Michael E. Phelps sestavil první pozitronový počítačový tomograf s původním názvem PETT-I (Positron Emission Transaxial Tomography). O rok později spolu s Hoffmanem dokončili PET-III, který byl již vhodný pro humánní medicínu. Tomograf ztratil přídomek „Transaxial“, protože umožňoval rekonstrukci i dalších rovin. Měl 48 NaI(Tl) pohyblivých detektorů hexagonálně uspořádaných po osmi v jediné rovině. Pohyby detektorů i lůžka již řídil počítač. NaI(Tl) detektory, přes svoje nesporné výhody, mají nízkou absorpční schopnost pro anihilační záření s energií 511 keV. V roce 1975 byly poprvé popsány scintilační vlastnosti bismuth-germanátu (BGO). Jeho vysoká hustota jej předurčila pro použití v PET. Již v roce 1978 dva různé týmy sestrojily PET skener s využitím BGO krystalů. Přístroj nazvaný NeuroECAT se stal prvním komerčně dostupným BGO skenerem. Následně jich bylo vyrobeno kolem 600 kusů. Dosavadní koncepce detektorů, kdy ke každému krystalu byl připojen jeden fotonásobič s elektronikou, limitovala konstruktéry při dalším zvyšování prostorového rozlišení skenerů. V letech 1984-5 byl vyvinut koncept bloku detektorů. První vyráběné bloky sestávaly ze čtveřice fotonásobičů připojených k poli 32 krystalů. Velkou výhodou tohoto konceptu bylo snížení nákladů na výrobu detektorů. Prstenčitě uspořádané bloky BGO detektorů jsou dodnes jádrem specializovaných PET skenerů. Prstence bývají řazeny za sebou, čímž se v axiálním směru prodlužuje zorné pole a zvyšuje se citlivost systému. Dalším milníkem ve vývoji klasických PET skenerů je rok 2001, kdy byl poprvé instalován PET skener na bázi LSO krystalů (lutecium oxyorthosilikát). Od BGO se liší především výrazně kratší délkou scintilace, což umožňuje efektivně zpracovávat větší fotonové toky. Začátek století s sebou také přinesl spojení nejvýkonnějších PET a CT kamer do jediného přístroje, schopného zobrazovat funkční děje a anatomické detaily. Při hledání levnějších alternativ PET byla v roce 1990 uvedena na trh kamera specializovaná na PET s hexagonálně uspořádanými silnými velkoplošnými NaI(Tl) krystaly. Para-
The use of annihilation radiation of positron emitters to enable medical imaging was first described as far back as 1951. At that time, and totally independently of each other, Sweet et al. and Wren et al. developed the first scanner that could be used to display images of the human brain. The equipment had two sodium iodide Nal(TI) based probes placed opposite each other. By 1962, the first multiple detector system was born, which, by the arrangement, and in coincidence operating of the instrument’s detectors could enable two-dimensional imaging. Positions in the third dimension were estimated according to the sharpness of the image (1968). Subsequently, the first PC-I tomograph was developed, described in 1972 by Brownell et al. and comprising two rotating two-dimensional detection fields. However, the design of this instrument had still not incorporated the principles of computer tomography. In the nineteen-sixties, computer reconstruction algorithms were gradually being enhanced (Kuhl, Edwards and others). In 1970, Chesler made use of a filtered back projection to compute emission and transmission tomograms and to construct emission tomograms that were corrected for the attenuation of radiation by tissues. In 1973, Michael E. Phelps built the first computer-aided positron tomograph, originally called PETT-I (Positron Emission Transaxial Tomography). A year later he and Hoffman completed the PET-III, which was already suitable for use in human medicine. The new generation tomograph lost the attribute “transaxial”, because it allowed other planes to be reconstructed. It had 48 Nal(TI) moving detectors, hexagonally arranged, eight in each plane. The movements of both detectors and the bed were already computer-controlled. In spite of their unquestionable advantages, NaI(Tl) detectors have low absorption capabilities for annihilation radiation with energy levels of 511 keV. In 1975, the scintillation properties of bismuth-germanate (BGO) were described for the first time and its high density made it seem predestined to be used in PET. Back in 1978, two teams had already built a PET scanner using BGO crystals. The instrument, named the NeuroECAT, became the first commercially available BGO scanner and around 600 units were later produced. The contemporary concept of detectors, according to which each crystal had a photo-multiplier tube (PMT) with electronic circuits connected to it, imposed a limit on the designers, preventing them from further enhancing the spatial resolution of the scanners. Between 1984 and 1985, the concept of detector blocks was developed. These early blocks consisted of four PMT, connected to an array of 32 crystals. The major advantage of this concept lay in the reduction of detector manufacturing costs. Even today, the ring arrangement of BGO detector blocks remains the core of dedicated PET scanners. The rings are usually arranged in series, which lengthens the field of view in the axial direction and increases the system’s sensitivity. Another milestone in the development of classic PET scanners was reached in 2001, when an LSO (lutetium oxy-ortho-silicate) crystalbased PET scanner was installed for the first time ever. It principally differs from BGO-based instruments in the significantly shorter duration of its scintillation, which allows larger photon flows to be processed efficiently. By the beginning of the new century the highest performance PET and CT scanners were being combined into a single instrument, capable of displaying images of functional processes and anatomic details. In the search for cheaper alternatives to PET, a special PET camera was introduced onto the market in 1990, with hexagonally arran7
metry zobrazení však zdaleka nedosahují klasických PET skenerů. V průběhu 90. let byl také koincidenční režim snímání aplikován na běžné vícedetektorové SPECT systémy. Proti konceptu hexagonálně uspořádaných krystalů jsou geometrické poměry podstatně méně příznivé, čemuž odpovídají i horší parametry zobrazení. Klinický přínos těchto hybridních SPECT/PET kamer je doposud sporný.
1.2 Radiofarmaka Pozitronová emisní tomografie je závislá stejnou měrou na snímací technice a na dostupnosti vhodných radiofarmak. Počátky PET se pojí s využíváním 68Ga, 15O molekulárního kyslíku, 15O oxidu uhelnatého, 15O oxidu uhličitého, 15O vody, 13N amoniaku, 11C glukózy a iontu 18 F. Tato radiofarmaka dodnes zůstávají pro výzkumné účely v repertoáru řady PET center. Podíl vyšetření prováděných s jejich pomocí je nyní již malý. Sokoloff et al. popsal v roce 1977 autoradiografické stanovení konzumpce glukózy v mozku krysy pomocí 14C-deoxyglukózy. Později vznikl nápad označit deoxyglukózu pomocí pozitronového zářiče 18F. Výslednou 2-deoxy-2-[18F]-fluoroD-glukózu poprvé popsal Ido et al. v roce 1978. Při své práci vyšel ze zkušeností Pacáka et al., kteří jako první v roce 1968 v Praze tuto molekulu syntetizovali – tehdy ovšem ještě se stabilním 19F. Pro molekulu se později vžil názvoslovně méně správný termín fluoro-deoxy-glukóza (FDG). Později se ukázalo, že vlastnosti FDG předurčují další rozvoj PET. Jen díky ní se koncem 90. let PET začala dostávat z ryze výzkumného pole do běžné klinické praxe. Na výroční schůzi Society of Nuclear Medicine v roce 1996 byla molekula FDG nominována na „molekulu století“. Pro další rozvoj PET bylo zapotřebí zjednodušit a zlevnit výrobu radiofarmak. V roce 1986 byl instalován první minicyklotron, v roce 1989 byl popsán první počítačově řízený automatický modul pro syntézu radiofarmak. Výroba radiofarmak tak mohla být zajištěna jen s minimem pracovníků. V devadesátých letech se začal také uplatňovat satelitní koncept, kdy na jedno PET centrum vybavené cyklotronem bylo navázáno více blízkých pracovišť vybavených jen skenery.
1.3 Klinické vyuÏití První PET skenery umožňovaly axiálně zobrazit pouze velmi omezené zorné pole. Nejčastějším cílem výzkumných prací proto tehdy byl – a dodnes zůstává – mozek. V roce 1986 byly publikovány první práce popisující perfúzi (13N-amoniak) a konzumpci glukózy (18FDG) v myokardu. Od té doby se odvíjí velký zájem o studium viability myokardu pro klinické účely. Další zvětšení zorného pole a hlavně zvýšení výkonu výpočetní techniky umožnilo provádět celotělová PET vyšetření. Ukázalo se, že FDG dosahuje vysokého kontrastu ve zhoubných nádorech vůči krevnímu a tkáňovému pozadí. FDG PET se tak stala vysoce spolehlivou metodou onkologické diagnostiky. V důsledku toho v mnoha vyspělých zemích přistoupili k úhradě PET vyšetření z veřejných zdrojů. Tyto skutečnosti jsou nejpodstatnějším motorem rozvoje PET. Na výročním kongresu Evropské asociace nukleární medicíny 2002 bylo konstatováno, že PET je nejdynamičtěji se rozvíjející zobrazovací modalitou v medicíně vůbec. Onkologická vyšetření pomocí 8

During the nineteen-nineties, the coincidence scanning mode was also applied to standard multiple-detector SPECT systems. Compared to the hexagonal arrangement concept, its geometric conditions are significantly less favourable, a fact which is reflected in its worse imaging parameters. The clinical benefits provided by these hybrid SPECT/PET cameras still remains a source of controversy.
1.2 Radio-pharmaceuticals Positron emission tomography depends to a large degree on the scanning technique and the availability of suitable radio-pharmaceuticals. The origins of PET were associated with the use of 68Ga, 15 O molecular oxygen, 15O carbon monoxide, 15O carbon dioxide, 15 O water, 13N ammoniac, 11C glucose and 18F ion and these radiopharmaceuticals have remained in the repertoire of many PET centres for research purposes to this day. However, their contribution in terms of numbers of performed clinical examinations remains relatively small. In 1977, Sokoloff et al. described a technique involving the auto-radiographic determination of the consumption of glucose in the brain of a rat, using 14C-deoxyglucose. Later on, the idea of labelling deoxyglucose by means of an 18F positron emitter was born. The resultant 2-deoxy-2-[18F]-fluoro-D-glucose was for the first time described by Ido et al in 1978. He based his work on the experiences of Pacák et al., who first synthesised this molecule in 1968 in Prague – but, of course, still used a stable 19F. Later on, this molecule became known by the less correct name of fluoro-deoxy-glucose (FDG). Still later it was discovered that the properties of FDG were to destine it to play a vital role in the further development of PET. It was thanks to FDG that, in the late 1990s, PET started to move away from the field of pure research and into standard clinical practice. At the 1996 Annual Conference of the Society of Nuclear Medicine, the FDG molecule was nominated the “Molecule of the Century”. It was obvious that, if PET were to develop further, the production of radio-pharmaceuticals had to be simplified and its costs reduced. In 1986 the first mini-cyclotron was installed, and in 1989 the first computer-controlled automatic module for the synthesis of radiopharmaceuticals was described. This created the possibility of producing radio-pharmaceuticals with a reduced number of personnel. In the 1990s, the satellite concept started to be applied, where a single PET centre equipped with a cyclotron was linked to a number of nearby facilities equipped with scanners only.
1.3 Clinical applications The early PET scanners allowed only images with a limited axial field of view to be displayed. Therefore the most common object of contemporary research was – and still remains today – the brain. In 1986 the first works describing the perfusion (13N-ammoniac) and consumption of glucose (18FDG) in the myocardium were published. Ever since then, the interest in conducting studies on the viability of the myocardium for clinical purposes has been on the increase. Further enlargement of the field of view and, even more importantly, improvements in computer performance, allowed PET examinations of the whole body to be carried out. It was discovered that high levels of FDG are accumulated in malignant tumours, detectable against a blood and tissues background. Thus FDG PET has become a highly reliable method for use in oncological diagnostics. As a result, many developed countries started using public funds to reimburse PET examinations, a fact which has come to represent the single most significant driving force behind the development of PET. The
FDG jednoznačně převažují a jejich podíl lze velmi zhruba odhadovat na 90 % všech PET vyšetření. PET byla po velmi dlouhou dobu jen atraktivním výzkumným nástrojem. V databázi MEDLINE je evidováno pod klíčovým slovem „Positron Emission Tomography“ na počátku roku 2003 přes 32.000 prací. Přelom století přenesl těžiště PET do klinické praxe, kde na dlouhou dobu zůstane.
1.4 Situace v âeské republice Po dlouhá léta existovaly marné snahy mnohých pracovníků o instalaci PET v Československu a později v ČR. Nakonec dva projekty výraznou měrou přispěly k zavedení PET do klinické praxe. Prvním byl projekt „Příprava výroby pozitronových radiofarmak značených 18F a 11C pro pozitronovou emisní tomografii – vývoj technologie“, řešený od roku 1996 do r. 1998 Ústavem jaderné fyziky Akademie věd ČR (ÚJF) za finanční podpory Interní grantové agentury Ministerstva zdravotnictví ČR. Konečným výstupem bylo zavedení hromadné výroby FDG a registrace tohoto preparátu 30. června 1999. V té době byla ovšem FDG využitelná pouze ke scintigrafii myokardu pomocí konvenční SPECT kamery – nejednalo se tedy o PET. Druhý projekt byl založen na myšlence Ústavu jaderného výzkumu Řež a.s. (ÚJV) sdružit prostředky se zdravotnickým subjektem a s pomocí Mezinárodní agentury pro atomovou energii (IAEA) vybudovat kompletní PET centrum vybavené specializovaným PET skenerem. IAEA pojala tento projekt za modelový. To znamená, že zkušenosti zde nabyté by měly sloužit při zakládání PET center v dalších zemích na podobném stupni rozvoje jako byla ČR. Původní úmysl realizovat PET centrum v areálu Fakultní nemocnice Bulovka se v průběhu projekčních prací ukázal jako neschůdný. Vesměs z ekonomických důvodů jej odmítly i jiné pražské nemocnice. Nakonec se k jeho realizaci připojila pražská Nemocnice Na Homolce. Projekt byl završen 25. srpna 1999 prvním PET vyšetřením v historii ČR (s pomocí FDG dovážené z Řeže z ÚJF) a 15. října 2001 první dodávkou registrované FDG vyrobené ÚJV v prostorách PET centra. Téměř čtyři roky pracoval specializovaný PET skener v Nemocnici Na Homolce jako jediný v ČR. V roce 2003 byl na témže pracovišti instalován hybridní PET/CT systém poslední generace. Tím se otevřela nová etapa vývoje PET v ČR. Do budoucna lze očekávat instalace PET skenerů i na několika dalších pracovištích a rozšíření nabídky dostupných radiofarmak.

2002 Annual Congress EANM proclaimed PET to be the most dynamically developing imaging modality in the entire field of medicine. FDG oncological examinations are clearly in the majority and their share can be very roughly estimated as constituting 90% of all PET examinations. For a long time, PET was only used as an attractive research tool. At the beginning of 2003, the MEDLINE database registered more than 32,000 works under the keyword “Positron Emission Tomography”. The new century has shifted the PET focal point into clinical practice, where it is destined to remain over the long-term.
1.4 The Situation in the Czech Republic For many years a number of medical professionals fought in vain to have PET installed in Czechoslovakia and later in the Czech Republic. Eventually two projects contributed significantly towards the introduction of PET into clinical practice. The first of these projects was “Preparation for the manufacture of positron radio-pharmaceuticals labelled by 18F and 11C for positron emission tomography – development of the technology”, conducted between 1996 and 1998 by the Nuclear Physics Institute of the Czech Academy of Sciences (NPI), with financial support provided by the Internal Grant Agency of the Czech Ministry of Health. The final outcome was the introduction of mass production of FDG and the registration of this product on 30th June 1999. Of course, at that time, FDG could still only be used for scintigraphy of the myocardium by means of a conventional SPECT camera – i.e. it was not yet PET. The second project was based on the idea of Nuclear Research Institute plc. in Řež (NRI) to join forces and funds with a medical facility and, with an assistance of the International Agency for Atomic Energy (IAEA), to develop a complete PET centre equipped with a dedicated PET scanner. IAEA accepted it as a model project, which meant that the experience gained from this project was to be used when establishing PET centres in other countries with a similar level of development as the Czech Republic. The original idea of building a PET centre within the complex of the Bulovka Teaching Hospital turned out, during the course of the planning works, to be unfeasible. Other Prague hospitals too, declined the proposal, though mainly for financial reasons. Eventually the project was accepted by Na Homolce Hospital. On 25th August 1999, the first PET examination in the history of the Czech Republic was conducted (using FDG supplied by the NPI in Řež), and on 15th October 2001 the first delivery of FDG manufactured by NRI in the PET Centre was made. For nearly four years the dedicated PET scanner in the Na Homolce Hospital was the only one in the Czech Republic. Then, in 2003, a hybrid PET/CT state of the art system was installed in the same department. This has opened up a new era of PET development in the Czech Republic. We now anticipate the installation of PET scanners in other departments, as well as a broader availability of suitable radio-pharmaceuticals in the future.

V nestabilním jádře pozitronového zářiče dochází k přeměně protonu na neutron, což je provázeno vyzářením kladného náboje ve formě pozitronu – částice podobné svými vlastnostmi elektronu s kladným nábojem. Jedná se o jakousi formu antihmoty, která nemůže trvale existovat v hmotném prostředí. Délka dráhy pozitronu závisí na jeho energii. Například 82Rb užívané k posouzení perfúze myokardu má střední délku doletu pozitronu 2,6 mm a maximální dolet 15,6 mm. Nejužívanější 18 F má střední délku doletu jen 0,2 mm a maximální dolet 2,4 mm. Na konci dráhy pozitronu je setkání s elektronem okolní hmoty a jejich zánik – anihilace, provázená emisí dvou kvant záření γ o energii 511 keV. Obě kvanta se z místa anihilace pohybují opačným směrem téměř po přímce.
Inside the unstable positron emitter core, protons are transformed to neutrons, a process that is accompanied by the emission of a positive charge in the form of a positron – a particle similar in its properties to a positively charged electron. It is therefore a type of anti-mass, incapable of existing permanently in a mass environment. The length of the positron’s trajectory depends on its energy. For example 82Rb, used in assessing the perfusion of the myocardium, has a medium positron range of 2.6 mm and a maximum range of 15.6 mm. The most commonly used 18F has a medium range of only 0.2 mm and a maximum range of 2.4 mm. At the end of its trajectory, the positron meets with an electron from the surrounding mass and both are annihilated, a process which is accompanied by the emission of two quanta of γ radiation with an energy of 511 keV. Both quanta move away from the point of annihilation in opposite directions along an almost straight line.
2.2 Princip detekce
2.2 The detection principle
Z uvedeného je zřejmé, že detekční systémy nejsou založeny na detekci samotných pozitronů, ale průvodního anihilačního záření. K detekci může být využita klasická scintilační kamera vybavená vhodným kolimátorem (obr. 1a). V tomto režimu se snímá vždy jen jediný foton (jednofotonová emisní počítačová tomografie – SPECT). Vzhledem k relativně vysoké energii anihilačního záření je ovšem třeba použít speciální kolimátor s dostatečně silnými septy. Přítomný NaI(Tl) krystal absorbuje jen malé procento fotonů s energií 511 keV. Účinnost takového systému je nízká. Získané obrazy ať planární nebo tomografické (SPECT) mají navíc špatné prostorové rozlišení. PET se od scintigrafie či SPECT liší současným snímáním obou fotonů bez použití kolimátorů. Zde se využívá faktu, že oba fotony letí po přímce. Zdánlivě nejjednodušší je použít dvoudetektorovou scintilační kameru, sejmout kolimátory a zapojit tzv. koincidenční obvod, který vyhodnocuje scintilace současně vzniklé na obou detektorech. Poloha obou bodů slouží k proložení přímky (obr. 1b). Znalost velkého množství takových přímek v mnoha směrech (oba detektory rotují kolem pacienta) umožňuje rekonstruovat tomografické řezy. PET je tedy primárně tomografická metoda a na rozdíl od scintigrafie či SPECT nepracuje s planárním zobrazením. Je zřejmé, že použití běžné dvoudetektorové scintilační kamery se jen vzdáleně blíží optimálnímu skeneru. O něco lepších detekčních parametrů pro anihilační záření lze dosáhnout se silnějšími NaI(Tl) krystaly (za cenu zhoršení parametrů snímání 99mTc). Proto je pro PET daleko vhodnější používat specializované systémy. Svojí konstrukcí se konvenčním scintilačním kamerám blíží systém s hexagonálně uspořádanými šesti silnými NaI(Tl) detektory (obr. 1c). Tato geometrie výrazně zvyšuje citlivost. Standard dnes ovšem představují systémy s kruhově uspořádanými bizmut-germanátovými (BGO) detektory s počtem krystalů v řádu desetitisíce (obr. 1d). Vysoká absorpční schopnost BGO a množství krystalů tak systémy činí vysoce citlivými s dobrým prostorovým rozlišením. V poslední době se do popředí zájmu dostávají systémy s krystaly z lutecium oxyorthosilikátu
From what has been said earlier it is obvious that detection systems cannot be based on the detection of positrons themselves, but rather on the accompanying annihilation radiation. A classical scintillation camera equipped with a suitable collimator (Figure 1a) can be used to enable this detection. In this mode, only one single photon is scanned (a Single Photon Emission Computer Tomography – SPECT). Because of the relatively high energy of the annihilation radiation, a special collimator with sufficiently thick septa must be used. The Nal(TI) crystal present absorbs only a small percentage of photons, because of their high energy (511 keV). This type of system has low levels of efficiency and, in addition, the acquired images, planar or tomographical (SPECT), have poor spatial resolution. PET differs from scintigraphy or SPECT in that both photons are simultaneously scanned without the use of collimators. The system makes use of the fact that both photons fly in a straight trajectory. The simplest method entails the use of a double-detector scintillation camera and the replacement of the collimators with what is known as a coincidence circuit, enabling the scintillation generated to be evaluated simultaneously on both detectors. The positions of both points are used and a straight line drawn through them (Figure 1b). Knowing a large quantity of such straight lines in many directions (both detectors rotate around the patient) allows us to reconstruct tomographical sections. Thus PET is primarily a tomographical method, and unlike scintigraphy or SPECT, it does not work with planar images. It is obvious that the performance of a standard double-detector scintillation camera cannot match that of an optimal scanner. Somewhat better detection parameters for annihilation radiation can be achieved by using thicker Nal(TI) crystals (at the cost of worse 99mTc scanning parameters). It is therefore far more advantageous to use specialised systems for PET. The system of six, hexagonally-arranged, thick Nal(TI) detectors is similar to conventional scintillation cameras in design (Figure 1c). This geometry significantly enhances the instrument’s sensitivity. Of course, today’s standard is represented by systems with circularly arranged bismuth-germanate (BGO) detectors with numbers of crystals in the order of magnitude of tens of thousands (Figure 1d). The BGO’s high absorption capability and the quantity of crystals make these systems highly sensitive with good spatial resolution. Lately, systems using lu-

2.3 Disturbing phenomena 2.3 Ru‰ivé jevy 2.3.1 Absorpce Bylo by ideální, kdyby všechny vyzářené fotony byly absorbovány až ve scintilačním krystalu detektoru. Bohužel jejich velké množství je absorbováno již v těle pacienta, což vede k exponenciálnímu poklesu signálu se zvyšující se hloubkou zobrazované struktury v těle. Toto je dobře známé při jednofotonovém snímání. Při koincidenčním snímání (PET) je situace ještě nepříznivější, neboť oba fotony musí být registrovány zároveň – tzn. jedná se o pravděpodobnost, že jeden foton vyvolá scintilaci za situace, že i druhý foton vyvolá scintilaci, tzn. ani jeden z obou fotonů není v těle pacienta absorbován. Proto je u PET významná korekce absorpce, kterou naprostá většina PET systémů nabízí. Při geometrické metodě se korekční koeficienty odhadují z tvaru transaxiálního řezu. Tento postup je v praxi použitelný jen pro zobrazení mozku. Proto jsou moderní PET skenery vybaveny transmisními zdroji, pomocí nichž je pacient prozářen a jsou změřeny absorpční koeficienty. Mapa korekčních koeficientů obráží denzitu zobrazovaných struktur a odpovídá vlastně výsledku CT zobrazení. Konstrukce PET skeneru ovšem z daleka neumožňuje dosáhnout u korekčních map diagnostické kvality CT. Klinická potřeba současného PET i CT vyšetření vedla konstruktéry k vytvoření hybridního PET/CT přístroje, který obsahuje vedle sebe PET i CT skener. Kvalitní diagnostické CT zobrazení je pak využíváno i pro korekci absorpce. Z hlediska korekce je u PET/CT skenerů výhodou podstatně lepší odstup signálu od šumu a kratší čas snímání. Určitou nevýhodou je jiná energie transmisního a emisního záření.
2.3.1 Attenuation It would be ideal if all the emitted photons could be absorbed in the scintillation crystal of the detector. Unfortunately, a large proportion of the photons are absorbed in the patient’s body, resulting in an exponential drop of the signal’s strength as the depth of the imaged structure inside the body increases. This phenomenon is well known from single photon scanning techniques. In coincidence scanning mode (PET), the situation becomes even less favourable, because both photons must be registered simultaneously – and hence there is a probability that one of the photons will trigger a scintillation in a situation when the other photon has also triggered a scintillation, resulting in neither of both photons being absorbed in the patient’s body. Therefore it is important in PET systems to be able to correct the attenuation, a feature offered by a large majority of the PET systems now available. In the geometry method, correction coefficients are estimated from the shape of the transaxial slice. In practice, this procedure can only be used for brain imaging. Therefore modern PET scanners are equipped with transmission sources by means of which the patient is penetrated with radiation, and attenuation coefficients are measured. The correction coefficient map reflects the density of the imaged structures and in fact corresponds with the result of the CT image. Of course the PET scanner design does not allow the correction maps even remotely to achieve the diagnostic quality of CT. The clinical needs of current PET and CT examinations led designers to create a hybrid PET/CT instrument that accommodates PET and CT scanners side-by-side. The quality of CT imaging is also used to correct attenuation. From the correction point of view, the advantage of PET/CT scanners lies in their significantly better signal-to-noise ratio and shorter scanning time. One disadvantage lies in the different energy levels of the transmission and emission radiation. 11
Z fyzikálního pohledu je transmisní snímání s pomocí externích zdrojů optimální provádět ještě před podáním radiofarmaka, aby se transmisní i emisní sken vzájemně neovlivňovaly. Pro tento způsob vyšetření se užívá termín „cold transmission“. Bývá využíván jen pro výzkumné účely; do praxe se nehodí. Po transmisním snímání je podáno radiofarmakon a pacient buď v nezměněné poloze setrvává na lůžku skeneru do zahájení emisního snímání (cca 30-90 min.), nebo je zaznamenána jeho poloha vůči skeneru, poté se může vzdálit a pro emisním snímáním je shodně uložen. První způsob je nepohodlný pro pacienta, vede k velkým prostojům skeneru a skrývá v sobě riziko, že se pacient během dlouhé doby pohne a korekce nebude adekvátní. Druhý způsob je kromě velkého rizika nepřesnosti při repozici pacienta spojen se zvýšeným ozářením personálu. Z těchto důvodů se v praxi téměř výhradně používá režim tzv. „hot transmission“, kdy se transmisní snímání provádí až po aplikaci radiofarmaka.
From the point of view of physics, detection of a transmission generated by external sources is optimally carried out before a radio-pharmaceutical is administered, in order to eliminate mutual interference of the transmission and the emission scan. This type of examination is called “cold transmission”. It is used for research purposes and is not suitable for clinical practice. After a transmission scan, a radio-pharmaceutical is administered and the patient either remains in the same position on the scanner bed until the start of emission scanning (30 to 90 minutes), or his/her position relative to the scanner is recorded, and he/she can leave, and after coming back for emission scanning, is again put into an identical position. The first method is uncomfortable for the patient, the scanner cannot be used for quite a while and there is a risk of the patient moving during the long wait and a wrong correction. The second method, in addition to running a high risk of inaccuracy when re-positioning the patient, carries an increased hazard of personnel irradiation. For these reasons, the so-called “hot transmission” mode is almost exclusively used in clinical practice, where the transmission scanning is carried out after the radio-pharmaceutical has been administered.
2.3.2 Rozptyl

Emitované fotony nejsou v těle pacienta jen absorbovány. Řada z nich se interakcí s hmotou odchýlí z původního směru, což je provázeno snížením jejich energie (Comptonův rozptyl). V důsledku toho dochází k chybnému určení místa anihilace (obr. 2). Účinným nástrojem k potlačení tohoto jevu by bylo zvýšení dolního prahu energetického okna, ve kterém se snímá. Rozptýlené fotony se sníženou energií by pak nebyly zaznamenávány. Nedostatečné energetické rozlišení používaných krystalů však umožňuje využít této nabízené možnosti jen z části. Pro korekci je tedy třeba navíc použít sofistikovanější matematické nástroje.
Emitted photons are not only absorbed in the body. Many of them, by interacting with the mass, are scattered from their original trajectory, a phenomenon accompanied by the loss of their energy (Compton’s scatter). As a result, the point of annihilation is incorrectly determined (Fig. 2). An effective tool for suppressing this phenomenon would be to raise the lower threshold of the energy window in which the scanning takes place. This would mean that the scattered photons with reduced energy would not be recorded. However, this solution can be used only partially because of the limited energy resolution of the crystals. Thus more sophisticated mathematical tools have to be used for corrections.
2.3.3 Náhodné koincidence
As has already been mentioned earlier, the coincidence detection is based on the principle of running a connection line through the two points in which scintillation took place simultaneously. In BGO scanners, an identical moment is usually taken to be scintillation within a 12 ns time window. The patient is administered a level of activity where hundreds of millions of decays per second take place. Thus it comes as no surprise that sometimes only one photon can be registered in a particular annihilation (the second photon might for instance have been absorbed in the patient’s body), and another photon from a completely different annihilation can be registered within the same time window. In this case the coincidence lines will be determined incorrectly (Fig. 2). It is obvious that in order to eliminate this phenomenon, the coincidence time window must be shortened. The problem is not the speed of the electronic circuits, but the time duration of the light flash during the crystal’s scintillation. The main drawback of BGO crystals is their relatively long-lasting flash. In this respect the introduction of the “faster” LSO crystals represented a major step forward, as they enabled the time window to be shortened to 6 ns. Jak již bylo zmíněno, koincidenční detekce je postavena na principu proložení spojnice dvou bodů, kde došlo ke scintilaci ve shodný okamžik. Za shodný okamžik se u BGO skenerů obvykle považuje časové okno 12 ns. Pacientovi je podána taková aktivita, že k rozpadům dochází v řádech stovek milionů za sekundu. Nepřekvapí tedy situace, že v daném časovém okně může dojít k registraci jen jediného fotonu z nějaké anihilace (druhý foton byl např. absorbován v těle pacienta) a zároveň k registraci jiného fotonu z úplně jiné anihilace. V takovém případě dojde k určení chybné koincidenční přímky (obr. 2). Je jasné, že k omezení tohoto jevu je zapotřebí zkrátit časové okno koincidence. Problémem není rychlost elektronických obvodů, ale délka světelného záblesku při scintilaci v krystalu. Hlavní nevýhodou BGO krystalů je právě poměrně dlouho trvající záblesk. V tomto smyslu je zavedení „rychlejších“ LSO krystalů velkým pokrokem, neboť časové okno je zkráceno na 6 ns. Obr. 2: Schéma rušivých jevů při detekci. Pokud k němu nebyl v definovaném časovém okně registrován jiný foton, jedná se o tzv. „single“, který je systémem odmítnut. Pokud ovšem v časovém okně byl zaregistrován jiný foton (např. d1), pak systém mylně určí spojnici [c1;d1] za anihilační přímku.

If only one photon is registered within the defined time window, it is deemed to be the “single”, and rejected by the system. 2.4 2D and 3D scanning modes
Celá sestava detektoru moderních PET skenerů se skládá z několika za sebou zařazených prstenců sestávajících z mnoha bloků detektorů. Tím se dosahuje zorného pole v axiálním směru přes 15 cm. Pokud se snímají koincidenční přímky pouze v rovině prstenců, jedná se o tzv. 2D mód snímání. Dosahuje se jej tak, že se mezi pacienta a detektor vkládá řada stínících sept. Geometricky septa představují několik desítek mezikruží, zařazených v definovaných odstupech za sebou. Jejich středy leží na ose shodné s osou těla pacienta. Tak je dosaženo kolimace záření do roviny detekčních prstenců (obr. 3a). Pokud se septa ze skeneru vysunou, pak se jedná o 3D mód snímání. Kromě koincidenčních přímek v rovině prstenců se navíc registrují i přímky mezi rovinami prstenců (obr. 3b). Je zřejmé, že ve 3D režimu je zachyceno mnohem více fotonů než ve 2D režimu. 3D mód je tedy podstatně citlivější. Logicky zde ovšem dochází k četnějším náhodným koincidencím. Výhodu vysoké citlivosti 3D režimu lze tak plně využít až s LSO detektory, které mají kratší koincidenční časové okno. U BGO skenerů je 3D režim vhodný jen pro zobrazení mozku, kde je malá kontaminace fotony z okolních orgánů. Určitou nevýhodou 3D režimu je vyšší podíl rozptýleného záření, neboť delší trajektorie šikmo letících fotonů v těle pacienta zvyšuje pravděpodobnost jejich rozptylu.
The entire assembly of modern PET scanners comprises several rings arranged in-series, each composed of many detector blocks. This arrangement extends the field of view in an axial direction to over 15 cm. When only those coincidence lines lying in the plane of the rings are scanned, it is described as the 2D scanning mode. In this mode, a row of shielding septa is inserted between the patient and the detector. The septa constitute several dozen annuluses arranged at predefined intervals behind each other. Their midpoints lie along a centreline coinciding with the centreline of the patient’s body. This guarantees that radiation collimation takes place in the detection ring plane (Fig. 3a). When the septa are pulled out of the scanner, it is termed the 3D scanning mode. Coincidence lines between ring planes are registered as well as the coincidence lines within the ring plane (Fig. 3b). It is obvious that many more photons are captured in the 3D mode than in the 2D mode, making the 3D mode considerably more sensitive, although of course it is logical that random coincidences occur more frequently in this mode. Thus the advantage of the 3D mode’s high sensitivity can be fully utilised only when combined with LSO detectors, which have a shorter coincidence time window. In BGO scanners, the 3D mode is only suitable for brain imaging, where there is a low contamination of photons from the surrounding organs. A disadvantage of the 3D mode is a higher proportion of scattered radiation, as the longer trajectories of diagonally flying photons in the patient’s body increase the probability of their scatter.

Axiální zorné pole kolem 15 cm je dostatečné pro zobrazení mozku nebo myokardu. Pro obvyklé využití FDG PET v onkologické diagnostice je ovšem příliš krátké. Aby bylo dosaženo potřebného rozsahu, je při snímání řízen pohyb vyšetřovacího stolu. Jednotlivé pozice pacienta vůči skeneru se musí překrývat, aby bylo dosaženo vyrovnané citlivosti v celém rozsahu snímání. Vyšší přesah je zapotřebí ve 3D režimu, neboť fotony vycházející z pacienta v rovině blízké okraji detektoru mohou být zachyceny vlastně jen v této rovině – u šikmo letících fotonů detektor pohltí nejvýše jeden z takových fotonů (3D režim se u okraje detektoru blíží 2D režimu). Výpočetní systém zpracuje postupně nasnímaná data v několika polohách do jediného objemu s odpovídající délkou zorného pole. V praxi lze při celotělovém snímání nejdříve provést „hot transmission“ sken v celém rozsahu a pak emisní snímání nebo naopak. Snížit riziko pohybu pacienta mezi oběma skeny lze střídáním emisního (E) a transmisního (T) snímání v každé poloze lůžka. Zkrácení neproduktivního času potřebného pro vysouvání a zasouvání transmisních zdrojů na polovinu lze dosáhnout střídáním: E-T-posuv-T-E-posuv-E-T-posuv...
An axial field of view of around 15 cm is sufficient for brain or myocardium imaging, but is, of course, too short for the most common use of FDG PET, in oncology diagnostics. In order to achieve the required range, movements of the patient’s table are controlled during the scanning process. The patient’s individual positions relative to the scanner must overlap to enable an evenly balanced sensitivity to be achieved within the entire scanning range. Greater overlapping is required in the 3D mode, because the photons emitted from the patient in the plane close to the detector edge can only be captured in this plane – the detector can only absorb one of the diagonally flying photons (near the detector edge, the 3D mode closely resembles the 2D mode). The computing system gradually processes the scanned data in several positions into a single volume corresponding to the field of view. In practice, when performing a whole body scan, first a “hot transmission” scan can be performed over the full extent, and then emission scanning, or vice versa. The risk that the patient will move between the two scans can be reduced by alternating between emission (E) and transmission (T) scanning in each position of the bed. Shortening the non-productive time needed to shift in and out of the transmission sources to half can be achieved by alternating: E-T-shiftT-E-shift-E-T-shift...

2.6.1 Slice reconstruction
Nasnímané souřadnice koincidenčních přímek jsou rekonstruovány do transaxiálních řezů. Z nich lze prostou reorientací vytvořit koronární, sagitální a jakékoliv šikmé řezy. Pro rekonstrukci se do konce století v praxi využívala téměř výhradně filtrovaná zpětná projekce (FBP). Při PET s FDG je dosahováno značných rozdílů v koncentracích radiofarmaka v nádoru či močovém měchýři oproti okolním tkáním. FBP vede k hvězdicovým artefaktům v okolí ložisek s vysokou akumulací radiofarmaka, což může vést k přehlédnutí drobných ložisek v blízkosti větších. Pro klinickou praxi je tedy FBP méně vhodná. Její výhoda malé výpočetní náročnosti se časem stala nepodstatnou díky rychlému vývoji počítačů. FBP mohla být nahrazena výpočetně náročnými iterativními metodami, které jsou prosté těchto artefaktů. Nejrozšířenější iterativní postup je označován OSEM (ordered subset maximalization expectation).
The coordinates of the acquired coincidence lines are reconstructed into transaxial slices. Coronary, sagittal and any other skew slices can then be created from them by simple re-orientation. Until the end of the last century, the Filtered Back Projection (FBP) was, in practice, almost exclusively used for reconstruction. In PET with FDG, considerable differences in concentrations of the radio-pharmaceutical in a tumour or urinary bladder, as compared to the surrounding tissues, are achieved. The FBP leads to the formation of star-shaped artefacts around focuses, with a high accumulation of the radio-pharmaceutical, which might result in minor focuses being overlooked when in the vicinity of major ones. This means that the FBP method is less suitable for clinical practice, particularly as its advantage in requiring a low level of computation has, over the course of time become irrelevant, thanks to the rapid development of computers. This meant that FBP could be replaced by computationally demanding iterative methods, free of these artefacts. The most commonly used interactive procedure is known as OSEM (Ordered Subset Maximalisation Expectation).

Obvyklým výstupem PET je trojice objemů dat. Objemem dat se rozumí množina voxelů – elementárních objemových jednotek se známými prostorovými souřadnicemi nesoucích příslušnou informaci. První informace představuje naměřenou aktivitu radiofarmaka bez korekce na absorpci, druhá informace je aktivita korigovaná na absorpci a třetí informace představuje absorpční koeficient, podle kterého se korigovalo. Všechny tři objemy je možno zobrazit a využít k diagnostickým účelům. Konzole počítačů umožňují zobrazit na sebe kolmé i šikmé řezy a využít běžné zobrazovací nástroje (barevné škály včetně nastavení prahů, filtrování, zvětšování, oříznutí, definice oblastí zájmů apod.). Oblíbenou formou zobrazení je tzv. Maximum Intensity Projections (MIP).
The usual PET output is a trio of data volumes. By data volume we mean a set of voxels – elementary volume units with known spatial coordinates that carry the relevant information. The first data volume represents the measured activity of the radio-pharmaceutical without correction on attenuation, the second data volume corresponds to the attenuation corrected activity, and the third data volume represents the absorption coefficient used to apply that correction. All three volumes can be displayed and used for diagnostic purposes. Computer consoles allow mutually perpendicular as well as skew sections to be displayed, and standard imaging tools to be used (colour tables including thresholding, filtering, zooming, cropping, defining regions of interest, etc.). A popular form of display is the so-called Maximum Intensity Projection (MIP). This is an illustrative, easy-to-read, three-di-

PET skenery bývají kalibrovány v absolutních jednotkách aktivity, tzn. umožňují určovat aktivitu v jednotkách Bq/cm3. Toho se hojně využívá ve výzkumných aplikacích ke kvantifikaci metabolického obratu podaných radiofarmak s využitím matematického modelování. Tato implementačně náročná a velice pracná procedura obvykle vyžaduje znalost průběhu aktivity v arteriální krvi, což bývá spojeno s nutností velkého množství odběrů krve během vyšetření. Pro praxi je zcela nevhodná. Místo ní se s oblibou stanovuje tzv. SUV (Standardized Uptake Value), což je naměřená aktivita v cílové tkáni vztažená k podané aktivitě a dále buď ke hmotnosti, k povrchu nebo i k jiným parametrům vyšetřované osoby.
PET scanners are usually calibrated in absolute units of activity, which means that they allow activity to be determined in Bq/cm3. This is frequently used in research applications to quantify the metabolic turnaround of the administered radio-pharmaceuticals, using mathematical modelling. This is an implementation-demanding and very elaborate procedure, usually requiring knowledge of the timeactivity curve in the arterial blood, which leads to the need to take large quantities of blood samples during the examination. In practice this method proves quite unsuitable. A popular alternative technique entails determining the so-called SUV (Standardised Uptake Volume), which is the ratio between the measured activity in the target tissue and the administered activity, taking the examined person’s weight, surface area or other parameters into consideration.

Jiným způsobem objektivizace tomografického metabolického vyšetření (PET, SPECT) je matematické testování odchylek mezi dvěma objemy – např. před léčbou a po léčbě u téhož jedince. Nezbytným krokem je tzv. vzájemná registrace obou objemů, tzn. nalezení vektoru posunu ve třech směrech a otočení ve třech rovinách. Existují iterativní algoritmy, které dosti spolehlivě automaticky registraci provedou. Druhým krokem je normalizace naměřených aktivit při obou vyšetřeních, následovaná nalezením odchylek mezi odpovídajícími voxely. Rutinně snadno aplikovatelné je barevné zvýraznění voxelů, které se liší o více než stanovené procento. Vzájemně lze registrovat i vyšetření různých jedinců. Zde k translacím a rotacím přibývají ještě dilatace ve třech směrech. Iteruje se tedy přes devět parametrů. Smyslem vzájemné registrace vyšetření různých jedinců je vytvoření např. normálního vzoru, kdy lze následně stanovit objemy představující střední hodnoty a rozptyly v hodnotách voxelů. Po registraci a normalizaci konkrétního vyšetření s takovýmto vzorem lze zvýraznit voxely, které se liší o více než stanovený počet směrodatných odchylek. Registrace objemů není omezena jen na vyšetření v rámci jediné zobrazovací modality. Podobně lze registrovat např. PET s MRI nebo CT vyšetřeními. Cílem je kombinované funkční (PET, SPECT) a morfologické (MRI, CT) zobrazení. Výsledná tzv. fúze obrazů představuje společné zobrazení, kdy obraz každé modality je zobrazen ve své vlastní barevné škále. Při zobrazení lze dynamicky měnit poměr zastoupení modalit v obraze – tzn. od zobrazení např. jen PET s postupným přidáváním složky CT zobrazení až po úplné potlačení PET a zobrazení jen CT. Jinou možností je prohlížení jedné modality, kdy kurzor představuje „okno“ do druhé modality.
Another method of analysis in tomographic metabolic examinations (PET, SPECT) involves the mathematical testing of deviations between two volumes – e.g. before and after treatment in the same individual. It is necessary to achieve what is termed the mutual registration of both volumes, i.e. to find the shift vector in three directions and the rotation in three planes. Iterative algorithms exist that are able to perform the registration automatically and with a reasonable degree of reliability. The next stage is to normalise the measured activities during both examinations, and this is followed by locating the differences between corresponding voxels. An easily applicable process, used on a routine basis, is the colour highlighting of voxels which differ by more than a specified percentage. Examinations of different individuals can also be mutually registered. Here dilations in three directions are added to the translations and rotations. Thus iterations are calculated using nine parameters. One purpose for the mutual registration of different individuals can be, for instance, the creation of a normal template, representing the median and variance in the voxel values. After the registration and normalisation of a particular examination using this type of template, it is possible to highlight those voxels that differ by more than a specified number of standard deviations. Registration of volumes is not limited only to examinations carried out by a single imaging modality. For instance, PET with an MRI or CT examination can be registered in a similar manner. The target is to achieve a combination of functional (PET, SPECT) and morphological (MRI, CT) imaging. The resultant fusion of images produces a combined image, in which the image of each modality is displayed within its own colour table. When displaying the image, the ratio or representation of the modalities in the image can be dynamically changed, i.e. from, for example. a PET only display, then with the gradual addition of the CT component, up to complete suppression of the PET image, displaying only the CT. Another option is to view a single modality, and to use the cursor to open a “window” to another modality.

Farmakokinetika FDG
Jak již bylo zmíněno, pro PET lze využít široké škály radiofarmak. S ohledem na jejich biologické vlastnosti lze zkoumat nejrůznější funkční pochody v lidském těle. I pouhý přehled známých radiofarmak by přesahoval rámec této knihy, která je soustředěna na klinicky zdaleka nejvíce užívanou 2-[18F]-fluoro-2-deoxy-D-glukózu (FDG). Dopravu glukózy a analogicky i FDG do buňky zajišťuje skupina transportních proteinů a to jak proti koncentračnímu gradientu (SGLT), tak i podél něj (Glut). Transportéry mají různou afinitu ke glukóze a jsou na různých buňkách různě vyjádřeny. Například Glut-4 je hojně přítomen na povrchu svalových a tukových buněk a jeho exprese je závislá na hladině inzulínu. V buňce jsou glukóza i FDG ve vzájemné kompetici fosforylovány pomocí enzymu hexokinázy na glukózo-6-fosfát a 2-FDG-6-fosfát. 2-FDG-6-fosfát nemůže být dále metabolizován a v buňce se tedy hromadí. V malé míře však probíhá reverzní reakce za pomoci enzymu glukózo-6-fosfatázy a FDG tak může být z buňky částečně vyplavována. Nádorové buňky mívají zmnoženy inzulín non-dependentní glukózové transportéry (např. Glut-1 a Glut-3). Dále mívají zvýšenou rychlost fosforylace a sníženou hladinu glukózo-6-fosfatázy. Všechny tři zmíněné skutečnosti působí souhlasně. Důsledkem je obvykle výrazně zvýšená akumulace FDG v nádorových buňkách. Z uvedeného vyplývá potřeba správné přípravy na vyšetření. Pro zobrazení nádorů se doporučuje 6 hodinové lačnění. To proto, aby byla co nejnižší hladina glykémie a minimalizovala se kompetice FDG s glukózou – substrátů pro hexokinázovou reakci. Zvýšená hladina glykémie (např. u diabetiků) vede ke zhoršení kontrastních poměrů mezi nádorem a krevním pozadím. Lačnění má za důsledek také nízkou hladinu inzulinémie. Pro akumulaci FDG v nádorových buňkách to nehraje velkou roli, neboť glukózové transportéry nádorových buněk jsou povětšinou inzulín non-dependentní. Naopak to s výhodou zvyšuje kontrast vůči okolním zdravým tkáním, kde při nízké inzulinémii nejsou exprimovány inzulín dependentní glukózové transportéry. Při zobrazení myokardu je ovšem situace odlišná. Jedná se o zobrazení svalových buněk vybavených inzulín dependentními transportéry. To znamená, že zvýšená inzulinémie usnadní vstup FDG do myocytů. Glykémie by ovšem měla být opět co nejnižší, aby nebyly nepříznivé kompetiční poměry pro hexokinázovou reakci. Optimální příprava se nazývá „hyperinzulinemické - euglykemické sevření“, kdy je po dobu cca 1 hodiny před aplikací FDG pacientovi i.v. podáván inzulín a pečlivou monitorací je euglykémie udržována i.v. přívodem glukózy. V denní rutině je schůdnější perorální podání 50 g glukózy 60 minut před aplikací FDG s případným i.v. podáním několika korekčních jednotek inzulínu v okamžiku aplikace FDG. Protože myokard často metabolizuje volné mastné kyseliny na úkor glukózy, je navíc doporučována nemastná dieta krátce před vyšetřením, popř. podání hypolipidemik (Acipimox). Pro kvantifikaci akumulace FDG je obvykle používán dvoukompartmentový model. Vstupní funkci tvoří koncentrace FDG v plasmě, první kompartment představuje volná FDG ve tkáních a druhý kompartment FDG váza16

Pharmaco-kinetics of FDG
As has already been mentioned earlier, a broad spectrum of radiopharmaceuticals can be used for PET. Depending on their properties, various functional processes can be investigated in the human body. Even a simple list of known radio-pharmaceuticals would be beyond the scope of this book, which focuses on 2-[18F]-fluoro-2-deoxy-D-glucose (FDG), most widely used in clinical practice. A group of transportation proteins transport glucose and, analogically also, FDG to the cell, both across a concentration gradient (SGLT), and along the concentration gradient (Glut). Transporters have different affinities to glucose and are expressed differently in different cells. For instance Glut-4 is present in large quantities on the surface of muscle and fat cells, and its expression depends on the level of insulin. In a cell, glucose in mutual competition with FDG, are phosphorylised with the help of a hexokinase enzyme to glucose-6-phosphate and 2-FDG-6-phosphate. The 2-FDG-6-phosphate cannot be metabolised any further and hence accumulates in the cell. However, to a certain degree a reverse reaction is taking place with the help of a glucose-6-phosphatase enzyme, and therefore the FDG can be partially washed out of the cell. Tumour cells often contain excessive insulin-non-dependent glucose transporters (e.g. Glut-1 and Glut-3). They also often have an increased phosphorylation speed and a reduced level of glucose-6-phosphatase. All three of these facts produce a positive effect and the usual consequence is a significantly increased accumulation of FDG in tumour cells. From what has been said above, it is obvious that the examination requires strict preparation. In the case of tumour imaging it is recommended that the patient fast for six hours. The reason for this is to reduce the level of blood sugar and to minimise the competition between FDG and glucose – substrates for the hexokinase reaction. An increased level of blood sugar (e.g. in diabetics) leads to a reduction in the contrast between the tumour and the blood background. Fasting also reduces the level of insulinaemia but does not have a major effect on the accumulation of FDG in tumour cells, because glucose transporters of tumour cells are mostly insulin-non-dependent. On the contrary, it can provide an advantage in increasing the contrast with the surrounding healthy tissues, in cases of low insulinaemia where insulin-non-dependent glucose transporters are not exprimated. Of course the situation is different in myocardium imaging. This is an imaging of muscle cells containing insulin-dependent transporters. It means that increased insulinaemia will ease the uptake of FDG in the myocytes. The level of blood sugar should again of course be as low as possible, to avoid the creation of an environment of unfavourable competition for the hexokinase reaction. An optimal preparation is known as “hyper-insulinaemic-euglycaemic clamping”, and involves the intravenous administration of insulin for approximately one hour before applying FDG to the patient, while, at the same time, the blood sugar level is carefully monitored and corrected by the intravenous administration of glucose. As a daily routine it can prove easier to administer 50 g of glucose orally 60 minutes before FDG is applied, while a few units of insulin can be administered along with FDG. Because the myocardium often metabolises free fat acids at the expense of glucose, it is also recommended that the patient be restricted to a non-fat diet shortly before the examination, or, as an alternative, an hypolipoidaemic (Acipimox) can be administered. A two-compartment model is usually used to quantify the FDG accumulation. The input function is formed by the FDG concentration in the plasma; the first compartment represents free FDG in the tissues

FDG does not have quite the same affinity as glucose has to its various transporters and hexokinase. An affinity ration, known as a "lumped constant" has been empirically determined for different tissues. Using this constant, quantitative parameters applicable to FDG can be converted to glucose. The result is then a quantitative determination of the consumption of glucose in µmol.s-1.g-1.

All the data presented below were acquired from the ECAT EXACT PET scanner produced by Siemens in 1999. The FDG was supplied either by the NRI Řež plc. or by the NPI of the Academy of Sciences. Hermes computers from Nuclear Diagnostics AB were used for data storage, image fusion and screenshots. The basic test parameters are given in the table below:

FDG je fyziologicky zvýšeně akumulována neurony (1) a vylučuje se ledvinami (2) do moče. To zhoršuje citlivost při zobrazení nádorů v mozku a v ledvinách a prakticky znemožňuje zobrazit nádor v močovém měchýři (3). Depa aktivní moči v ureterech naopak mohou simulovat nádorové postižení lymfatických uzlin v oblasti křížení s ilickým cévním svazkem. V lymfatické tkáni Waldayerova okruhu a v laryngu bývá často aktivita zvýšena, někdy i výrazně. Symetrický nález však nebývá klinicky významný. Myokard (4) se nalačno zobrazuje jen minimálně nebo naopak i velmi výrazně. Ani fokální distribuce v něm však nemá nalačno větší klinický význam. Ve svalovině žaludku (5) a především ve střevech (6) bývá akumulace FDG nehomogenně zvýšena, někdy i dost výrazně, což může činit problémy při odlišení nádoru. Díky mírné difúzní akumulaci FDG v kostní dřeni lze obvykle dobře diferencovat skelet. Vychytávání FDG stoupá při aktivaci dřeně, např. po chemoterapii. Po fyzické zátěži (i s odstupem), nebo není-li pacient lačný, se více zobrazuje kosterní svalovina.
Normal pattern of FDG uptake
FDG is accumulated intensively by neurones (1) and then cleared via the kidneys (2), thus making imaging of brain or renal tumours more complicated. Any imaging of tumours within the urinary bladder (3) is practically impossible. Another problem arises where ureters cross the iliac vessels, as depots of “hot” urine can mimic neoplastic involvement of lymph nodes. FDG uptake is often increased in naso- and oropharyngeal lymphatic tissue and in the larynx. Symmetrical findings are usually of no clinical importance. In a fasting state, the myocardium (4) does not usually accumulate FDG, although its uptake can sometimes be very intense. Even focal myocardial uptake is of no clinical importance in a fasting state. FDG uptake is non-homogenous – it can sometimes be very intensive in the stomach (5) and intestine (6). This can make it difficult to differentiate from tumour tissue. Bones are visible due to the mild diffuse uptake of FDG in the bone marrow. Uptake is increased in cases of bone marrow activation, e.g. after chemotherapy. Muscles are well distinguished after physical exercise (even after a delay) or in a non-fasting state.

Zajímavou variantou fyziologického zobrazení vídanou u tenzních pacientů, především mladých žen s nízkým BMI (Body Mass Index) je lehce, ale i výrazně zvýšená akumulace FDG, a to symetricky nuchálně (1), laterálně na krku (2) s propagací retroklavikulárně (3) až pod axily (4) a dále paravertebrálně (5) v rozsahu hrudní páteře a někdy i ventrálně při kaudálním okraji hrudníku (6). Asymetrická akumulace bývá v těchto případech patrná v oblasti horní hrudní apertury s propagací vlevo kaudálně do mediastina (7), dále v oblasti nadledvin, či spíše dorzálně od nich (8, 9). Původní představy, že se jedná o zobrazení drobných svalů či ganglií, byly v poslední době modifikovány díky hybridnímu PET/CT zobrazení a předpokládá se, že se jedná o aktivovanou tukovou tkáň (reziduální hnědý tuk) při termoregulaci či stresu.
An interesting variant of physiological FDG uptake can be seen in anxious patients, predominantly at young females with a lower BMI (Body Mass Index). Mild or even very intense symmetrical uptake can be present in the sub occipital region (1), laterally at the neck (2) with propagation along the clavicles (3) to the axillas (4) and also paravertebrally (5) in the region of the thoracic spine and sometimes at the lower margins of the anterior thorax (6). Asymmetrical uptake in these cases can also be seen in the upper thoracic aperture with propagation to the left part of the upper mediastinum (7), and in the dorsal aspect of the region next to the adrenal glands (8, 9). These uptake sites were previously thought to be small muscles or ganglia. Nowadays, thanks to hybrid PET/CT imaging, this increased uptake has been localised to the fatty tissue (residual brown fat). It can be activated by thermoregulation or psychical stress.

Pfiípad 1: Pacientka s metastázou nemalobuněčného plicního karcinomu do mozku, po ozáření Leksellovým gama nožem, byla odeslána k FDG PET pro odlišení radionekrózy od recidivy metastázy při nespecifickém nálezu na MRI. Nález: Výrazné hypermetabolické ložisko temporálně vlevo (1) představuje recidivu metastázy, rozsáhlá oblast hypometabolizmu (2) v okolí je v důsledku kolaterálního edému. Závûr: Následný vývoj prokázal re1 cidivu metastázy, která regredovala po dalším radiochirurgickém zákroku.
Case 1: A patient with a suspected brain metastasis of non-small cell lung cancer was sent to FDG PET after radiotherapy by Leksell gamma-knife to distinguish tumour recurrence from radionecrosis after a non-specific MRI result. 2 Finding: The intense hypermetabolic focus in the left temporal lobe (1) represents tumour recurrence; the surrounding large hypometabolism (2) is the result of a collateral oedema. Result: Further clinical follow-up and radiological tests confirmed brain metastasis that regressed after subsequent irradiation.
Pfiípad 2: Pacient po nefrektomii pro karcinom ledviny, po chirurgickém odstranění metastázy okcipitálně vpravo, po následném ozáření recidivy Leksellovým gama nožem byl odeslán k FDG PET pro suspekci z další recidivy dle MRI. Nález: Defekt metabolizmu okcipitálně vpravo (1) představuje změny po léčbě, drobné ložisko jen minimálně zvýšené konzumpce glukózy může představovat recidivu (2). Závûr: Další sledování pacienta prokázalo recidivu metastázy.

FDG PET nemůže nahradit MRI při primární diagnostice metastáz do mozku. Důvodem je fyziologicky vysoká konzumpce glukózy v šedé hmotě, zhoršující kontrast. Po léčbě však MRI není mnohdy schopna odlišit radionekrózu od recidivy metastázy. Je-li po léčbě v ložisku přítomna i mírně zvýšená akumulace FDG oproti okolí, je recidiva vysoce pravděpodobná; v opačném případě se pravděpodobnost recidivy snižuje, avšak spolehlivě ji vyloučit nelze. Po léčbě Leksellovým gama nožem někdy bývá kolem ozářeného hypometabolického ložiska patrný lem lehce zvýšené konzumpce glukózy, který představuje benigní reparativní změny.

Case 2: A patient who had undergone a nephrectomy for renal cancer, resection of a metastasis in the right occipital lobe and irradiation by Leksell gamma-knife, was sent to FDG PET when MRI found suspected tumour recurrence. Finding: There is a metabolic defect (1) following therapy in the right occipital lobe. A small focus of faint uptake could represent tumour recurrence (2). Result: Further follow-up confirmed tumour recurrence.
FDG PET cannot replace MRI for primary diagnostics of brain metastases, because any physiologically high level of FDG uptake in the surrounding grey matter worsens the contrast. MRI often cannot distinguish between radionecrosis and tumour recurrence after treatment. Even faint uptake of FDG in contrast to the surroundings provides a strong suspicion of tumour recurrence in this case; a negative result reduces the probability but cannot reliably eliminate the possibility of tumour recurrence. Some hypermetabolic margin can be seen around the necrotic irradiated volume after Leksell gamma-knife treatment. This represents benign reparative changes.
6.1.2 Glioblastom – recidiva
6.1.2 Glioblastoma – recurrence
Pfiípad 3: Pět měsíců po makroskopicky radikálním odstranění glioblastomu frontálně vpravo je dle MRI suspektní recidiva tumoru. Nález: V části povrchu pooperační pseudocysty (1) je patrný výrazně hypermetabolický lem (2) svědčící pro recidivu glioblastomu. Závûr: FDG PET podpořila MRI nález. Histologie po následném paliativním neurochirurgickém výkonu potvrdila recidivu glioblastomu.
Case 3: Five months after macroscopically radical excision of a glioblastoma in the right frontal lobe, MRI found suspected tumour recurrence. Finding: There is intense hypermetabolism (2) in part of the surface of a postoperative pseudocyst (1). This represents a recurrence of the glioblastoma. Result: FDG PET supported the MRI result. The cell histology after subsequent palliative neurosurgical treatment confirmed recurrence of a glioblastoma.

Z primárních mozkových nádorů bývá FDG zvýšeně akumulována především v gliomech III. a IV. stupně. Nízkostupňové gliomy a meningeomy obvykle akumulují FDG méně než okolní šedá hmota. FDG PET lze využít po léčbě, kdy CT či MRI mnohdy nejsou schopné odlišit poléčebné změny od recidivy neoplazie. Je-li po léčbě v ložisku přítomna zřetelně zvýšená akumulace FDG oproti okolí, je recidiva vysokostupňového gliomu nebo tzv. „upgrading“ nízkostupňového gliomu vysoce pravděpodobný; v opačném případě se pravděpodobnost recidivy snižuje, avšak spolehlivě ji vyloučit nelze.
In the case of primary brain tumours, FDG uptake is increased in IIIrd and IVth grade gliomas. Low-grade gliomas and meningeomas usually accumulate FDG to a lesser extent than the surrounding grey matter and therefore a FDG PET examination is of no clinical relevance. FDG PET can be used after treatment of high-grade gliomas when CT or MRI cannot distinguish between tumour recurrence and post-therapeutical changes. Clearly enhanced FDG uptake shows a high probability of recurrence of high-grade glioma or the upgrading of originally low-grade glioma. A negative result reduces the probability of tumour recurrence, but can not exclude it.

6.2.1 Cancer of the root of the tongue – localisation and staging
Pfiípad 4: U pacienta s rezistencí pod úhlem mandibuly vpravo byla při blokové disekci krčních uzlin zjištěna metastáza středně diferencovaného dlaždicového karcinomu. Primární tumor nebyl nalezen při ORL, CT, MRI a bronchoskopickém vyšetření. Proto byla indikována FDG PET. Nález: V oblasti kořene jazyka (1) a vpravo v oblasti úhlu mandibuly (2) jsou ložiska hypermetabolizmu glukózy svědčící pro neoplazii. Závûr: FDG PET lokalizovala primární tumor do oblasti kořene jazyka a odhalila ještě jednu krční metastázu, což bylo následně operačně verifikováno.
Case 4: The metastasis of a relatively well-differentiated squamous carcinoma was found below the angle of mandible on the right side. Neither clinical head and neck exploration, nor CT, MRI and bronchoscopy were able to locate the primary tumour. The patient was therefore sent for FDG PET. Finding: Two foci of glucose hypermetabolism in the area of the root of the tongue (1) and in the area of the angle of mandible on the right side (2) represent a neoplasm. Result: FDG PET localised the primary tumour into the root of the tongue and discovered another neck metastasis. Subsequent surgery and histology confirmed both foci.

Pfiípad 5: U pacienta po resekci karcinomu dutiny nosní a maxilární vpravo a po odstranění krčních metastáz vpravo byla indikována FDG PET k posouzení vzdálených metastáz při recidivě karcinomu do krčních uzlin vlevo. Nález: Vícečetná ložiska hypermetabolizmu glukózy na krku, periklavikulárně a v mediastinu představují generalizaci neoplazie (jednoduché šipky). Dvojitá šipka ukazuje na místo podání FDG. Závûr: FDG PET odhalila generalizaci karcinomu.
Case 5: Tumour recurrence appeared in the neck lymph nodes on the right side after operating on a cancer of the nasal and maxillary cavities on the right side. FDG PET was indicated to discover possible distant metastases. Finding: Multiple hypermetabolic lesions on the neck, around the clavicles and in the mediastinum represent a generalised neoplasm (single arrow). Double arrow shows the site of FDG administration. Result: FDG PET discovered generalised cancer.

6.3.1 Solitární plicní léze – diferenciální diagnostika
6.3.1 Solitary pulmonary nodule – differential diagnostics
Pfiípad 6: Pacient byl pět let sledován pro stacionární ložisko v levé plíci při silně pozitivní tuberkulinové reakci. Ložisko se v poslední době dle CT zvětšilo, a proto byla indikována FDG PET. Bronchoskopie byla negativní. Nález: Ložisko hypermetabolizmu glukózy v dolním laloku levé plíce. Závûr: Pozitivní FDG PET podpořila rozhodnutí k chirurgické intervenci, při které byl následně nalezen nonargentafinní neuroendokrinní karcinom.
Case 6: A patient with a highly positive tuberculine test was followed up for five years due to a stationary nodule in the left lung. According to the CT scan, the nodule had recently begun to grow, but concurrent bronchoscopy findings were negative. Therefore FDG PET was indicated. Finding: A hypermetabolic nodule is present in the lower lobe of the left lung. Result: A positive FDG PET result supported the decision to undertake surgical exploration. Non-argentaffine neuroendocrine carcinoma was found.
Je všeobecně akceptováno, že FDG PET je pro diferenciální diagnostiku solitárních plicních lézí neobyčejně přínosná. Negativní nález s vysokou pravděpodobností svědčí pro benigní lézi (s výjimkou vzácného bronchioloalveolárního karcinomu, který často FDG neakumuluje). Pozitivní nález může kromě neoplazie představovat i fokální zánětlivé změny a je třeba jej dále specifikovat.
It is generally accepted that FDG PET is extremely helpful in the differential diagnostics of solitary pulmonary nodules. A negative finding indicates a very high probability of the presence of a benign lesion (the exception is rare bronchioloalveolar carcinoma with low FDG uptake). A positive finding does not always indicate a neoplasm but can also reveal focal inflammation and therefore requires further specification.

Pfiípad 7: Při stagingu operabilního nemalobuněčného karcinomu levé plíce byla na CT nalezena suspektní ložiska v levé nadledvině a ve skeletu. FDG PET byla indikována k posouzení jejich biologické povahy. Nález: Kromě hypermetabolizmu v primárním ložisku (1) s centrální nekrózou jsou patrna další ložiska v nadledvině (2) a v lopatě kosti kyčelní (3). Nevýznamným je zobrazení extravazace radiofarmaka při jeho podání (4) a pánvičky ledviny (5). Závûr: FDG PET potvrdila generalizaci nádorového onemocnění a od chirurgického řešení bylo ustoupeno.
Case 7: The CT scan revealed suspicious foci in the left adrenal gland and in the skeleton during preoperative staging of non-small cell lung carcinoma. FDG PET was indicated to assess their biological character. Finding: Besides hypermetabolism in the primary tumour (1) with central necrosis, there are other metastases in the adrenal gland (2) and the iliac bone (3). Visualisation of paravenous leakage of activity at the site of administration (4) and renal pelvis (5) is of no clinical importance. Result: FDG PET confirmed generalised cancer and surgical treatment was cancelled.

Pfiípad 8: Před operací ověřeného bronchogenního karcinomu plic bez známého postižení uzlin byla FDG PET indikována k posouzení cystických útvarů v játrech zjištěných při CT. Nález: Kromě primárního nádoru jsou patrna vícečetná ložiska v játrech, v mediastinu a subfrenicky. Závûr: FDG PET charakterizovala ložiska v játrech jako metastázy a nikoliv cysty, a navíc odhalila neznámé postižení uzlin mediastina a subfrenicky.
Case 8: CT discovered cystic lesions in the liver during preoperative staging of lung cancer. FDG PET was indicated to evaluate its metabolism. Finding: Besides the primary tumour there are clearly apparent multiple metastases in the liver, mediastinum and also below the diaphragm. Result: FDG PET specified the liver lesions as metastases and not benign cysts. Furthermore it discovered previously unknown metastases of the mediastinum and below the diaphragm.
FDG PET je neobyčejně přínosná při stagingu plicního karcinomu. Pro upřesnění operability ložisek v mediastinu je často zapotřebí fúze s CT. FDG PET nemůže nahradit MRI při diagnostice metastáz plicního karcinomu v mozku, stejně jako u jiných typů nádorů.
FDG PET is extremely helpful for staging of lung cancer. Image fusion with CT is often required to enable an exact assessment of operability in the mediastinum. FDG PET cannot replace MRI in diagnostics of brain metastases of lung cancer as well as of other types of neoplasm. 25
6.3.3 Plicní karcinom – efekt léčby
6.3.3 Lung cancer – results of therapy
Pfiípad 9: FDG PET byla indikována k posouzení efektu pěti cyklů chemoterapie u malobuněčného plicního karcinomu. Nález: Ložisko hypermetabolizmu glukózy kraniálně v oblasti pravého plicního hilu svědčí pro perzistenci karcinomu. Závûr: FDG PET prokázala nedostatečnou odpověď nádoru na chemoterapii.
Case 9: FDG PET was indicated to assess the effect of five cycles of chemotherapy in a case of small cell lung carcinoma. Finding: A hypermetabolic focus in the upper aspect of the right lung hilum represents persistent cancer. Result: FDG PET assessed that the tumour was responding insufficiently to treatment.

6.3.4 Plicní karcinom – recidiva
6.3.4 Lung cancer – recurrence
Pfiípad 10: U pacienta čtvrtým rokem po operaci bronchogenního karcinomu a radioterapii nebylo možno na CT odlišit možnou recidivu od poradiačních změn. Bronchoskopie a biopsie tenkou jehlou byly opakovaně negativní, pacient byl proto odeslán k FDG PET. Nález: Rozsáhlé ložisko hypermetabolizmu glukózy v levé plíci, vícečetná ložiska v mediastinu, odkud se šíří podél velkých cév pod bránici, a další ložiska v hrudních obratlích a v žebru představují lokální recidivu s generalizací karcinomu.
Case 10: CT was unable to distinguish between tumour recurrence and benign changes after radiotherapy in the case of one patient who had undergone surgery for lung cancer four years previously. FDG PET was indicated in spite of repeatedly negative bronchoscopy and fine needle biopsy results. Finding: An extensive hyper-metabolic focus in the left lung, multiple foci in the mediastinum, foci along the major vessels spreading below the diaphragm, and other foci in the spine and rib indicate not only a local recurrent but also a generalised neoplasm.
Po chemoterapii je na kontrolní FDG PET 1,5 roku poté zřejmá výrazná regrese původního nádoru. Nicméně rezistentní kmen nádorových buněk v trochu jiné lokalizaci progreduje. Vedlejším nálezem je ložisko presakrálně, které bylo následně verifikováno jako adenom sigmatu.
This patient was checked after chemotherapy one and a half years later. There was an apparent regression of the original tumour. Nevertheless a resistant portion of the tumour had progressed in a slightly different location. A presacral hypermetabolic focus was verified as adenoma of the sigmoid colon.
Závûr: FDG PET specifikovala nejasný CT nález a prokázala generalizaci neoplazie přes falešně negativní výsledek biopsie. Díky tomu mohla být zahájena chemoterapie. Posléze FDG PET identifikovala nádorovou masu rezistentní na chemoterapii a další patologii v kolon.
Result: Despite an equivocal CT and a falsely negative fine needle biopsy, FDG PET discovered the generalised neoplasm and encouraged the initiation of chemotherapy. Later on FDG PET identified a tumour resistant to chemotherapy and another pathology in the colon.
FDG PET je obecně považována za velmi přínosnou metodu pro průkaz recidivy karcinomu plic.
FDG PET is generally considered as very helpful for early diagnostics of recurrent lung cancer.

Pfiípad 11: U pacienta se sarkoidózou byla indikována FDG PET pro upřesnění rozsahu a aktivity onemocnění. Nález: Symetrická splývající ložiska hypermetabolizmu glukózy v plicních hilech a v plicích. Závûr: FDG PET prokázala rozsáhlou výraznou zánětlivou aktivitu.
Case 11: FDG PET was indicated to assess the spread and activity of sarcoidosis. Finding: Symmetrical coalescent foci of intense glucose hypermetabolism are localised in the lungs and lung hila. Result: FDG PET assessed intense, wide-spread inflammatory activity.
Pfiípad 12: U pacientky s vysokou sedimentací erytrocytů a erythema nodosum byla indikována FDG PET pro klinickou suspekci na neoplazii. Nález: Mnohočetná splývající ložiska hypermetabolizmu glukózy v mediastinu a v plicních hilech. Závûr: FDG PET odhalila rozsáhlý nález, který byl dále histologicky specifikován jako sarkoidóza.
Case 12: A patient with very high FW and erythema nodosum present was indicated for FDG PET because of a suspected neoplasm. Finding: Multiple coalescent foci of glucose hypermetabolism are present in the mediastinum and in the lung hila. Result: FDG PET discovered a massive local process in the thorax, which was later specified as sarcoidosis.
Aktivní sarkoidóza výrazně akumuluje FDG. V případech symetrického difúzního postižení plic její rozpoznání při PET obvykle nečiní potíže. V případech ložiskového postižení mediastina je však neodlišitelná od neoplazie (především lymfomu).

An active sarcoidosis has a very intense FDG uptake. There is no problem in recognising it when diffuse lung involvement is present. In the absence of other clinical signs, it is almost impossible to distinguish multifocal involvement of the mediastinum from a neoplasm (e.g. lymphoma).

Pfiípad 13: U pacienta v klinické remisi Wegenerovy granulomatózy při dlouhodobé imunosupresní terapii byla zjištěna ložiska v plicích při CT vyšetření. FDG PET byla indikována pro odlišení neoplazie. Nález: Vícečetná ložiska hypermetabolizmu glukózy v plicích bilaterálně. Závûr: FDG PET neodlišila granulomatózní ložiska od neoplazie. Další sledování pacienta svědčilo pro granulomatózní původ ložisek.
Case 13: CT revealed multiple nodules in the lungs of a patient in clinical remission from Wegener granulomatosis on long-term immunosuppressive therapy. FDG PET was indicated to exclude the presence of a neoplasm. Finding: Multiple foci of glucose hypermetabolism were present in both lungs. Result: FDG PET did not distinguish between the granulomatous foci and a neoplasm. Further follow-up indicated the granulomatous origin of the foci.
Pfiípad 14: Mladá dívka byla vyšetřována pomocí FDG PET pro onkologické onemocnění. Shodou okolností v době vyšetření prodělávala pneumonii. Nález: Klínovitá oblast hypermetabolizmu glukózy při bazi pravé plíce koreluje s RTG nálezem a představuje pneumonii. Závûr: Klínovitý tvar v kombinaci s klinickým stavem pacienta a dalšími vyšetřeními svědčí pro pneumonii, intenzitou metabolizmu je ložisko neodlišitelné od neoplazie.
Case 14: A young girl underwent FDG PET to identify a neoplasm. She was also suffering from pneumonia at the same time. Finding: The wedge-shaped area of glucose hypermetabolism at the base of the right lung corresponds to a plain chest radiograph and represents pneumonia. Result: The wedge-shaped area, in combination with the clinical status of the patient and with other examinations, is typical for pneumonia. Taking only the FDG uptake into consideration, it would be impossible to distinguish the lesion from a neoplasm.

Inflammatory changes in the lung often prove extremely difficult to distinguish from neoplasms. Correlation with other examinations and knowledge of the clinical status can help.

Pfiípad 15: U pacienta s epidermoidním karcinomem jícnu (38 – 42 cm od řezáků) a suspektní solitární metastázou v játrech byla indikována FDG PET k potvrzení, resp. vyloučení jaterního postižení v rámci předoperačního stagingu. Nález: V aborální části jícnu se zobrazuje primární tumor (1), který se propaguje ventrokaudálně do epigastria (2). Laterálně na něj naléhá další ložisko (3) – zřejmě postižená lymfatická uzlina. Další vícečetná hypermetabolická ložiska v játrech odpovídají metastázám – ilustrována jsou zde jen dvě (4, 5). Závûr: Kromě zobrazení primárního tumoru prorůstajícího do okolí, zřejmě i s postiženou lymfatickou uzlinou, FDG PET potvrdila známou jaterní metastázu a navíc odhalila další vícečetné metastázy v játrech.
Case 15: A patient with known epidermoid carcinoma of the oesophagus was sent to FDG PET for specification of a solitary liver lesion on CT and to make a preoperative staging. Finding: The primary tumour (1) can easily be seen in the aboral part of oesophagus. It is spreading in an antero-caudal direction into the epigastrium (2). In the lateral aspect of the primary tumour there is another hypermetabolic focus (3) – probably involving a lymph node. Multiple metastases were located in the liver – only two of them are illustrated here (4, 5). Result: Beside the localisation of the known oesophageal tumour, FDG PET discovered that it was spreading into the epigastrium, as well as the involvement of a lymph node and the previously unknown multiple liver metastases.

Pfiípad 16: U pacienta po resekci distálního jícnu a kardie pro adenokarcinom s následnou chemoterapií byla při kontrolní gastrofibroskopii diagnostikována recidiva v anastomóze. FDG PET byla indikována k vyloučení generalizace onemocnění před eventuální reoperací. Nález: Mnohočetná ložiska vysoké konzumpce glukózy v játrech, ve slezině, v oblasti horní hrudní apertury, v mediastinu, vpravo supraklavikulárně, ve stěně hrudní, v páteři, v levém SI skloubení a v pánevních kostech představují neoplazii. Závûr: FDG PET potvrdila generalizaci nádorového onemocnění a od chirurgického řešení bylo ustoupeno.
Case 16: A patient underwent resection of the distal part of oesophagus including part of the stomach and subsequently underwent chemotherapy for cancer. Regular gastrofibroscopy during the follow-up revealed recurrent cancer in the anastomosis. FDG PET was indicated for restaging before a possible reoperation. Finding: Multiple hypermetabolic foci located in the liver, spleen, in the area of upper thoracic aperture, in the mediastinum, above the right clavicle, in the chest wall, in the spine, in the left sacro-iliac joint and in pelvic bones show a neoplasm. Result: FDG PET revealed a generalised neoplasm and surgery was cancelled.

Pfiípad 17: U pacienta s CT nálezem hypodenzního ložiska o průměru 3 cm v oblasti hlavy pankreatu byla indikována FDG PET ke specifikaci ložiska (zánět versus malignita). Nález: Ložisko vysoké konzumpce glukózy v oblasti předpokládaného uložení hlavy pankreatu je s ohledem na nízkou hladinu CRP vysoce suspektní z neoplazie. Závûr: FDG PET specifikovala ložisko jako neoplazii. Jiná ložiska nebyla nalezena. Histologické vyšetření po následné operaci prokázalo adenokarcinom s angioinvazí.
Case 17: CT revealed a hypodense focus (3 cm in diameter) in the pancreas. FDG PET was indicated to specify the lesion (inflammation versus neoplasm). Finding: A hypermetabolic focus in the pancreas gives a high likelihood of neoplasm because of the low level of CRP. Result: The focus was characterised as neoplasm by FDG PET. No other lesions were found. The histology after the operation revealed adenocarcinoma with angioinvasion.
S odstupem 6 měsíců bylo provedeno kontrolní FDG PET vyšetření, při kterém byly odhaleny mnohočetné drobné metastázy v játrech.
FDG PET was repeated six months later. Multiple small metastatic foci were found in the liver.
FDG PET je obecně považována za vysoce spolehlivou metodu pro odlišení karcinomu pankreatu od chronické pankreatitidy. Pokud jsou ovšem elevovány zánětlivé markery (CRP > 10), FDG PET není schopna odlišit akutní zánětlivé změny.
FDG PET is generally considered to be a very reliable method for differentiation of cancer from chronic pancreatitis. However, in cases of increased inflammatory markers (CRP > 10), FDG PET is not able to distinguish an acute inflammation.

Závûr: FDG PET odhalila metastatické postižení jater a přispěla ke změně terapeutického postupu. Metastázy v játrech byly potvrzeny histologicky.
Case 18: A patient with proven rectal cancer had a negative CT-scan and ultrasonography of the liver. FDG PET was indicated for preoperative staging. Finding: Two hypermetabolic foci in the liver revealed metastases (1). “Hot” urine in the renal pelvis (2) is a normal variant. N.B.: The primary tumour is not illustrated here. Result: FDG PET discovered liver metastases, which caused the extent of surgery to be modified. Metastases were histologically verified.

Finding: An unknown lung metastasis (2) as well as a primary tumour (1) were found. Result: FDG PET revealed a generalised neoplasm and helped to change the therapy that followed.

In our experience, FDG PET routinely provokes changes in the therapy of colorectal cancer in ~15 % of patients who have previously been examined by conventional methods (CT, ultrasonography, plain chest radiograph). Nález: Vícečetně v játrech (1, 2), v plicích (3, 4, 5) a také dorzokaudálně od močového měchýře (6) jsou hypermetabolická ložiska představující neoplazii. Závûr: FDG PET prokázala lokální recidivu karcinomu s generalizací.
Case 20: Tumour markers became elevated in the patient after resection of the rectum and left lobectomy of the liver due to cancer. FDG PET was indicated to localise a suspicious recurrent neoplasm. Finding: Multiple hypermetabolic foci in the liver (1, 2), in the lungs (3, 4, 5) and next to the urinary bladder (6) represent a neoplasm. Result: FDG PET revealed a local recurrence of cancer and its generalisation.

Pfiípad 21: U pacientky s histologicky verifikovaným hepatocelulárním karcinomem a metastázou v pravé plíci dle CT byla indikována FDG PET k určení rozsahu onemocnění. Nález: Ve zvětšených játrech jsou mnohočetná splývající ložiska vysoké konzumpce glukózy (1), další ložiska jsou ve slezině (2) a v pravé plíci (3, 4). Nález představuje generalizaci neoplazie. Závûr: FDG PET potvrdila rozsáhlé postižení jater a metastázu v pravé plíci. Navíc odhalila další metastázu v plíci a ve slezině.
Case 21: CT revealed a lung metastasis of histologically verified hepatocellular carcinoma. FDG PET was indicated for staging. Finding: There are multiple coalescent hypermetabolic foci in the enlarged liver (1), in the spleen (2) and two foci in the right lung (3, 4). These represent a generalised neoplasm. Result: FDG PET confirmed a waste liver involvement and a metastasis in the right lung. It discovered another metastasis in the lung and in the spleen.

Case 22: A patient with cirrhosis and one proven focus of hepatocellular carcinoma underwent FDG PET for staging. Finding: Multiple foci of minimally increased glucose metabolism in the liver (some of them with central hypometabolism) represent neoplasm. Result: FDG PET revealed multifocal liver involvement.
Existuje značná interindividuální variabilita v glukózovém metabolizmu hepatocelulárního karcinomu. S tím je spojena vyšší četnost falešně negativních nálezů při FDG PET. Uvádí se, že hypermetabolizmus glukózy bývá spojen s horší prognózou hepatocelulárního karcinomu.
There is a high variability of glucose metabolism in hepatocellular carcinoma. This results in a higher frequency of false negative findings by FDG PET. Intense glucose hypermetabolism is coupled with worse prognosis.

Pfiípad 23: Mladá žena po opakovaných resekcích GIST žaludku a po ligatuře větve a. hepatica pro metastázu do jater. FDG PET byla indikována k posouzení reziduálního onemocnění před eventuální léčbou Glivecem. Nález: Vlevo v epigastriu v místě předpokládaného uložení žaludku (1) a v játrech (2, 3) jsou ložiska hypermetabolizmu glukózy představující neoplazii. Závûr: FDG PET odhalila recidivu v oblasti primárního tumoru a perzistenci metastáz v játrech. Následné CT vyšetření potvrdilo nádorové ložisko v žaludku, při zobrazení ložisek v játrech však selhalo.
Case 23: A young female underwent repeated surgery for a gastrointestinal stromal tumour and ligature of hepatic artery due to liver metastasis. FDG PET was indicated to asses the residual tumour before eventual treatment by Glivec. Finding: In the assumed location in the stomach (1) and in the liver (2, 3) hypermetabolic foci represent neoplasm. Result: FDG PET revealed a recurrence of the primary tumour and persistence of two liver metastases. A subsequent CT-scan confirmed the tumour in the stomach, but was unable to localise any liver metastases.

Pfiípad 24: Pacientce s diagnostikovaným tumorem levé ledviny byla indikována FDG PET ke stagingu před zvažovanou operací. Nález: Ložisko nepříliš zvýšené konzumpce glukózy nasedající na levou ledvinu (1) představuje primární tumor. Další ložiska ve stěně hrudní (2, 3), v tělech obratlů (4, 5) a v oblasti SI skloubení vlevo (6) jsou metastázami. Závûr: FDG PET prokázala generalizaci nádorového onemocnění. Histologicky byl prokázán karcinom ledviny.
Case 24: FDG PET was indicated for a preoperative staging of a known renal tumour. Finding: A focus of mildly increased glucose metabolism in tight conjunction with the left kidney (1) represented the primary tumour. Further similar foci in the chest wall (2, 3), in the spine (4, 5) and in sacroiliac joint (6) represented metastases. Result: FDG PET revealed a generalised neoplasm. The histology confirmed renal cancer.

In view of the variable presence of “hot” urine in the calyces and the renal pelvis, FDG PET is not dedicated to primary diagnostics of renal tumours. There is a high variability of glucose metabolism in renal cancer and this results in a higher frequency of false negative findings of FDG PET when seeking out distant metastases or local tumour recurrence.

Pfiípad 25: Pacient po nefrektomii vlevo pro adenokarcinom, nyní se suspektní metastázou v pravé plíci, byl indikován k FDG PET za účelem restagingu. Nález: Rozsáhlá ložiska výrazného hypermetabolizmu v oblasti pravé plíce a plicního hilu a dále vícečetná drobná ložiska v retroperitoneu představují generalizaci karcinomu. Závûr: FDG PET potvrdila metastatické postižení pravé plíce a navíc odhalila mnohočetné metastatické postižení lymfatických uzlin retroperitonea.
Case 25: FDG PET was indicated for restaging of adenocarcinoma at the patient after nephrectomy in the left side, with suspected metastasis in the right lung. Finding: Large foci of intense hypermetabolism in the right lung and hilum and also multiple small foci in the retroperitoneum showed a generalised neoplasm. Result: FDG PET confirmed metastases in the right lung and moreover also discovered multiple metastases in the retroperitoneal lymph nodes.

Pfiípad 26: Pacientka po operaci nefroblastomu levé ledviny, po dvou relapsech onemocnění v plicích, s následnou chemoterapií, byla odeslána k FDG PET k vyloučení dalšího možného relapsu v plicích. Nález: Vícečetná ložiska hypermetabolizmu glukózy v obou plicích svědčí pro neoplazii. Závûr: FDG PET prokázala další relaps onemocnění v plicích.
Case 26: After surgery of a nephroblastoma of the left kidney, a patient was treated with chemotherapy for two tumour relapses in the lungs. FDG PET was indicated to assess a suspected third relapse. Finding: Multiple hypermetabolic foci in both lungs represent neoplasm. Result: FDG PET revealed another nephroblastoma lung relapse.

Pfiípad 27: Pacient byl odeslán k FDG PET a scintigrafii skeletu za účelem průkazu suspektní, blíže neurčené neoplazie. Nález: Při planární kostní (1) scintigrafii je na přední (1) i zadní projekci (2) patrný tzv. „supersken“, tj. ložiska výrazně zvýšené metabolické aktivity osteoblastů splývající v rozsahu celého centrálního skeletu včetně postižení femurů, humerů a kalvy. FDG PET vykazuje jen nepříliš zvýšenou konzumpci glukózy v prostatě (3) a nehomogenní konzumpci glukózy v kostní dřeni. Závûr: Příkrý rozpor mezi oběma nálezy je dán biologickou povahou nádoru, který vytváří osteoplastické metastázy a glukózu akumuluje jen málo.
Case 27: FDG PET and bone scintigraphy were indicated to localise a suspicious but unconfirmed neoplasm. Finding: A planar bone scan in the anterior (1) (2) and posterior (2) projections reveal a "superscan“, i.e. foci of extremely increased osteoblastic activity involving the whole central skeleton including parts of the femora, humeri and skull. FDG PET reveals only mild glucose metabolism in the prostate (3) and non-homogenous glucose consumption in the bone marrow. Result: The obvious difference between the two findings is based on the biological behaviour of prostate cancer. It creates osteoplastic metastases and only accumulates a lesser amount of glucose.

Konzumpce glukózy karcinomem prostaty je obvykle nízká. Jeho odlišení od benigních onemocnění prostaty pomocí FDG PET je v praxi nemožné. Ze stejného důvodu není FDG PET vhodná ani ke stagingu onemocnění. K posouzení metastáz ve skeletu je daleko vhodnější kostní scintigrafie nebo ještě lépe PET s pomocí volného [18F] fluoridového iontu.

Glucose uptake by prostate cancer is generally low. It is practically impossible to differentiate it from benign prostate diseases when using FDG PET. For the same reason FDG PET is not suitable for its staging. Bone scan or even [18F]-fluoride PET is much more efficient in searching for bone metastases.

Pfiípad 28: Pacient po orchiektomii vlevo pro embryonální karcinom, se suspektním postižením uzlin v retroperitoneu, byl indikován k FDG PET pro posouzení rozsahu onemocnění. Nález: Ložisko výrazného hypermetabolizmu glukózy v retroperitoneu představuje neoplazii. V oblasti genitálií je obtížné odlišit neoplazii od možné kontaminace aktivní močí.
Case 28: A patient underwent orchidectomy on the left side due to embryonic carcinoma. There was also a suspicion of a metastatic involvement in the retroperitoneal lymph nodes. FDG PET was indicated for staging. Finding: Hypermetabolic focus in the retroperitoneum represents a neoplasm. Increased activity in the scrotal region can either represent contamination by “hot” urine or a neoplasm. It is extremely difficult to distinguish during daily routine.
Při kontrolní FDG PET po 4 sériích chemoterapie došlo k úplné normalizaci nálezu. Pruhovitě výrazně zvýšená konzumpce glukózy bilaterálně na krku, periklavikulárně a paravertebrálně představuje variantu fyziologického zobrazení u tenzního pacienta (tuková tkáň, drobné svaly). Závûr: FDG PET potvrdila postižení uzlin retroperitonea. Vymizení hypermetabolizmu svědčí pro dobrou léčebnou odpověď.
There was complete normalisation of the retroperitoneal finding after four cycles of chemotherapy. Intense glucose uptake in the neck, next to the clavicles and paravertebrally is a frequent finding in an anxious patient (fatty tissue, small muscles). Result: FDG PET confirmed lymph node involvement. The subsequent normalisation of the finding shows that the tumour responded well to chemotherapy.
FDG PET je považována za vhodnou metodu pro primární staging a restaging neseminomových testikulárních nádorů (s výjimkou teratomu) a pro posouzení efektu chemoterapie.
FDG PET can be recommended for the primary staging and restaging of non-seminomatous testicular tumours (except teratoma) and for the evaluation of chemotherapy.

6.5.6 Seminom – restaging a efekt léčby Pfiípad 29: Po léčbě primárního seminomu varlete bylo při kontrolním CT vysloveno podezření na relaps nádoru v retroperitoneálních uzlinách. FDG PET byla indikována k posouzení rozsahu onemocnění. Nález: Nepravidelné ložisko výrazného hypermetabolizmu glukózy v retroperitoneu potvrdilo CT nález.
Při kontrolní FDG PET po 4 sériích chemoterapie došlo k úplné normalizaci nálezu. Na transverzálním řezu je zachyceno depo aktivní moče v ureteru (1).
6.5.6 Seminoma – restaging and effect of therapy Case 29: A CT-scan produced suspicions of a tumour relapse in the retroperitoneal lymph nodes after treatment for seminoma. FDG PET was indicated for restaging. Finding: A irregular hypermetabolic focus in the retroperitoneum represents a neoplasm.
There was complete normalisation of the retroperitoneal finding after four cycles of chemotherapy. The depot of “hot” urine can be clearly seen on the transverse slice (1).

Result: FDG PET confirmed the suspected tumour recurrence and could not find any other foci. The subsequent normalisation of the finding shows that the tumour responded well to chemotherapy.

Case 30: Levels of the AFP tumour marker increased during the third year after primary therapy of testicular tumour. Conventional imaging modalities were not able to localise tumour recurrence. Finding: Multiple, somewhere coalescent hypermetabolic foci in the retroperitoneum represented neoplastic involvement of the lymph nodes. Result: FDG PET was the only imaging modality able to localise tumour recurrence.

6.5.8 Ovariální karcinom – recidiva Pfiípad 31: Pacientka 11 let po oboustranné adnexektomii a hysterektomii s následnou chemoterapií pro karcinom ovaria. Před 3 lety absolvovala chemoterapii pro relaps onemocnění. Nyní je dle gynekologického vyšetření podezření na recidivu onemocnění v malé pánvi, nádorové markery nejsou zvýšeny. Nález: Plošně zvýšená konzumpce glukózy při kraniolaterálním povrchu jater (1), ložiska hypermetabolizmu při kaudálním povrchu jater (2), v oblasti sleziny (3) a především ve středním hypogastriu (4) představují recidivu nádoru. Závûr: FDG PET potvrdila recidivu onemocnění v pánvi a odhalila postižení peritonea, popř. i jater a sleziny.
6.5.8 Ovarian cancer – recurrence Case 31: A patient underwent bilateral adnexectomy and hysterectomy and subsequent chemotherapy for ovarian cancer 11 years ago. She underwent another course of chemotherapy for tumour recurrence 3 years ago. A suspicious recurrence has again appeared in the pelvis, located during a gynaecological examination. Tumour markers were not elevated. Finding: An area of increased glucose consumption at the craniolateral surface of the liver (1) hypermetabolic foci at the caudal margin of the liver (2), in the area of the spleen (3) and in the middle hypogastrium (4) represent tumour recurrence. Result: FDG PET confirmed tumour recurrence in the pelvis and discovered an involvement in the peritoneum (the possible involvement of the liver and spleen could not be distinguished).

Nález: Vícečetná splývající ložiska hypermetabolizmu glukózy vlevo periklavikulárně s propagací do levé axily a mnohočetná splývající ložiska v oblasti středního epi- až mezogastria představují generalizaci neoplazie. Závûr: FDG PET potvrdila metastázy v levé nadklíčkové oblasti a odhalila další metastatická ložiska v levé axile a v dutině břišní.
Case 32: A suspicious metastatic infiltration appeared above the left clavicle one year after radiotherapy for carcinoma of the cervix uteri. No local recurrence was clinically apparent. FDG PET was indicated for restaging. Finding: Multiple coalescent hypermetabolic foci in the region of left clavicle, spreading to the axilla and multiple coalescent foci in the middle epi- and mesogastrium indicate a generalised neoplasm. Result: FDG PET confirmed metastases in the region of the left clavicle and discovered other metastases in the left axilla and the abdomen.
FDG PET je slibnou metodou pro průkaz vzdálených metastáz při stagingu a při včasné diagnostice recidivy karcinomu děložního hrdla. Literárních dokladů je však dosud málo.

Pfiípad 33: U pacientky tři roky po ablaci pravého prsu byla nalezena lokoregionální recidiva karcinomu. FDG PET byla indikována pro posouzení rozsahu onemocnění. Nález: Kromě postižení hrudní stěny vpravo byly nalezeny mnohočetné metastázy ve skeletu a v retroperitoneu.
Case 33: Loco-regional recurrence was found three years after ablation of the right breast. FDG PET was indicated for restaging before therapy. Finding: Beside neoplastic involvement of chest wall on the right side there were multiple hypermetabolic foci in the bones and retroperitoneum.
Po třech měsících, kdy pacientka absolvovala tři cykly chemoterapie, je patrná úplná normalizace rozsáhlého nálezu s výjimkou výrazně regredovaného drobného ložiska v oblasti pravé axily, resp. v humeru. Při následné kontrole po další terapii ložisko již nebylo vůbec patrné.
Three months later, when patient had undergone three cycles of chemotherapy, a regression was clearly apparent in the region of the right axilla or humerus and the complete normalisation of all other foci was noted. After further chemotherapy this residual lesion disappeared.
Závûr: FDG PET odhalila generalizaci nádorového onemocnění a posoudila dobrou odpověď nádoru na léčbu. Po dalších 9 měsících je pacientka bez známek recidivy onemocnění.
Result: FDG PET revealed generalised neoplasm and evaluated the tumour's positive response to chemotherapy. After a further 9 months follow-up, there were no signs of tumour recurrence.
FDG PET může být s výhodou využita jako doplňkové vyšetření při stagingu/restagingu karcinomu prsu – zvláště pro odhalení vzdálených metastáz a také ke sledování léčebné odpovědi.

Nález: Mnohočetná ložiska hypermetabolizmu glukózy na krku, v mediastinu, v obou plicních hilech, v játrech, ve stěně hrudní, v Th a L páteři, v SI skloubeních, ve skeletu pánve a ve femurech představují neoplazii. Závûr: FDG PET na rozdíl od konvenčních vyšetření odhalila masivní generalizaci neoplazie.
Case 34: A patient was suffering from low back pain one year after complex therapy of erysipeloid breast carcinoma. A plain chest radiograph, ultrasonography of the liver and pelvis and bone scintigraphy did not reveal any pathology. FDG PET was indicated for growing levels of tumour markers. Finding: Multiple hypermetabolic foci located in the neck in the mediastinum, in both lung hila, in the liver, chest wall, the thoracic and lumbar spine, the sacroiliac joints, the pelvic bones and the femora show a generalised neoplasm. Result: FDG PET discovered a huge generalisation of neoplasm in contrast to conventional imaging modalities.
Dle vlastních zkušeností je FDG PET neobyčejně vhodnou metodou pro včasnou diagnostiku recidivy karcinomu prsu. Při dynamicky rostoucích (nikoliv staticky elevovaných) nádorových markerech je FDG PET ve většině případů pozitivní, a to i přes negativitu konvenčních zobrazovacích metod. FDG PET velmi citlivě detekuje metastázy většiny nádorů do kostní dřeně (s výjimkou karcinomu prostaty). Předčí scintigrafii skeletu, která se stává pozitivní, až když rostoucí metastázy vedou k remodelaci kostní trámčiny.
According to our experience FDG PET is extremely useful for early diagnostics of recurrent breast cancer. In cases involving dynamically growing tumour markers (not in static elevation) FDG PET is positive in the majority of patients, despite negative findings from conventional imaging modalities. FDG PET is a very sensitive detector of bone marrow metastases of the majority of tumours (except prostate cancer). It outperforms bone scintigraphy, which becomes positive later, when growing metastases have led to the remodelling of the bone trabecular system.

Pfiípad 35: Pacientka po thyreoidektomii pro karcinom a po opakované léčbě radiojódem pro recidivu. FDG PET byla indikována pro podezření na relaps onemocnění (dynamický růst thyreoglobulinu). Oblast krku byla vyšetřena pomocí CT a sonografie, kde nebylo možno odlišit neoplazii od změn po léčbě. Scintigrafie nádoru pomocí MIBI a také scintigrafie skeletu byly negativní. Nález: Mnohočetná ložiska hypermetabolizmu glukózy bilaterálně v plicích a ložisko na krku v místě obvyklého uložení pravého laloku štítné žlázy představují neoplazii. Vedlejším nálezem je difúzně výrazně zvýšená konzumpce glukózy ve střevech, což může odrážet klinicky nevýznamné zánětlivé změny. Závûr: FDG PET prokázala jak lokální recidivu, tak rozsáhlé metastatické postižení.
Case 35: A patient underwent a thyroidectomy and radio-iodine treatment for thyroid cancer as well as for a subsequent relapse. FDG PET was indicated to identify a suspected recurrence because levels of thyroglobulin were increasing dynamically. CT and ultrasonography were unable to distinguish between recurrence at the neck and post-treatment changes. Tumour scintigraphy using MIBI as well as bone scintigraphy were both negative. Finding: Multiple hypermetabolic lesions occurring bilaterally in the lungs and a focus in the usual location of the right thyroid lobe indicate a neoplasm. Diffuse intense uptake in the bowel could represent less clinically important inflammatory changes. Result: FDG PET revealed both local recurrence and multiple metastases.
FDG PET je všeobecně považována za velmi vhodnou metodu ke stagingu/restagingu karcinomu štítné žlázy u lézí neakumulujících radiojód. Typicky vhodnou indikací je růst hladiny thyreoglobulinu a negativní jodový sken. U karcinomů akumulujících radiojód je FDG PET často schopna odhalit větší množství ložisek.
FDG PET is generally considered very helpful for the staging/restaging of thyroid cancer, particularly when lesions do not trap radio-iodine. Increasing levels of thyroglobulin and a negative radio-iodine scan typically represent a correct indication for FDG PET. FDG PET can reveal larger numbers of metastases even with cancers that do accumulate radio-iodine.

FDG PET byla indikována k verifikaci scintigrafického nálezu. Nález: Dvě ložiska hypermetabolizmu glukózy v pravém jaterním laloku (1, 2), ložisko v pánvi mediálně od levého kyčelního kloubu (3) a ložisko v krčním (4) a hrudním obratli (5) představují neoplazii. Závûr: FDG PET jako jediná potvrdila nález zjištěný při scintigrafii s pomocí octreotidu.
Case 36: Octreotide scintigraphy revealed tumour generalisation during the follow-up after surgery of the carcinoid. Neither clinical status nor conventional imaging modalities corresponded to the octreotide scan findings. FDG PET was indicated to confirm these findings. Finding: Two hypermetabolic foci in the right liver lobe (1, 2), a pelvic lesion next to the left hip (3) and foci in the cervical (4) and thoracic spine (5) reveal a neoplasm. Result: FDG PET was the only test to confirm the findings of the octreotide scan.

Velmi zjednodušeně a ne zcela přesně lze hypotetizovat, že lépe diferencované formy karcinoidu mají vyvinuty somatostatinové receptory, a tudíž mohou být zobrazeny scintigraficky. FDG PET naopak může být u tohoto typu negativní pro nízkou konzumpci glukózy. Opačná situace by mohla nastat u dediferencovaných forem. Slibným postupem do budoucna se zdá být PET pomocí 18F nebo 68Ga značených derivátů octreotidu.

In cases involving the carcinoid, FDG PET provides a complementary investigation for scintigraphy using 111 In-octreotide. An unconfirmed hypothesis exists, that well-differentiated forms of carcinoid have developed somatostanine receptors and therefore can be visualised by octreotide. FDG PET could give negative findings for these forms because of their low glucose uptake. The opposite situation could occur with dedifferentiated forms of carcinoid. In future, the use of PET with 18F or 68Ga labelled derivatives of octreotide could be a promising development.

Case 37: A patient with malignant melanoma of the left eye was referred to FDG PET for staging. Finding: Multiple hypermetabolic foci in the lungs, the lung hila, in the liver, the left adrenal gland, in the 7th thoracic vertebra, the left sacro-iliac joint and the sacrum point to a neoplasm. Result: FDG PET revealed a generalised melanoblastoma.
FDG PET je všeobecně považována za velmi vhodnou metodu ke stagingu rizikového maligního melanomu kůže (známé postižení uzlin nebo Breslow > 1,5 mm). Rovněž může pomoci při hledání vzdálených metastáz u melanoblastomu oka.
FDG PET is generally accepted as very helpful for the staging of high risk skin melanoma (with known lymph node involvement or Breslow > 1.5 mm). In the same way it can help to localise distant metastases of a melanoblastoma of the eye.

Závûr: FDG PET byla schopna časně zachytit generalizaci melanoblastomu (CT potvrdilo metastázu v játrech, jinde bylo v té době ještě negativní).
Case 38: FDG PET was indicated for early diagnostics of a recurring melanoblastoma in a patient with a history of excision of a malignant melanoma in the right arm, after chemotherapy and after surgery of a metastasis in the right chest wall. Finding: A hypermetabolic focus in the liver (1), in the chest wall (2, 3), at the margin of the mediastinum and the apex of the left lung (4) and a lesion in the body of the 5th lumbar vertebra (5) represent a neoplasm. N.B.: the small foci in the right lung are not illustrated here. Result: FDG PET was able to detect generalised melanoblastoma early (a subsequent CT-scan confirmed liver metastasis; in other regions it was falsely negative at that time).

Závûr: FDG PET svědčila pro maligní povahu útvaru, který byl posléze histologicky určen jako epiteliální varianta maligního neurofibrosarkomu. Jiná ložiska nebyla nalezena.
Case 39: A patient was referred for FDG PET for a preoperative specification of the biological character of a lesion in the chest wall. Finding: A solitary hypermetabolic lesion in the chest wall indicates a neoplasm. Result: FDG PET testified to a malignancy. It was later confirmed by its histology as an epithelial variant of malignant neurofibrosarcoma. No other foci were found.

Nález: Rozsáhlé kulovité ložisko výrazného hypermetabolizmu glukózy retrokardiálně (1) pravděpodobně zasahuje až do hrudní stěny. Další dvě ložiska v levé (2) a dvě v pravé plíci (3) a v oblasti acetabula (4). Všechna ložiska představují generalizaci neoplazie. Vedlejším nálezem je difúzně zvýšená konzumpce glukózy v aktivované kostní dřeni po chemoterapii a artefakt (5) způsobený extravasací radiofarmaka po aplikaci. Závûr: PET vyšetření potvrdilo rozsáhlé metastatické postižení obou plic a navíc odhalilo ložisko v oblasti acetabula.
Case 40: A patient with persistent lung metastases of leiomyosarcoma after chemotherapy and a hysterectomy was referred to FDG PET for restaging before resection of lung metastases. Finding: A large spherical hypermetabolic lesion retrocardially (1) probably also involved the chest wall. Another two foci are located in the left (2) and two foci in the right lung (3) as well as in the region of the acetabulum (4). All the lesions represent a generalised neoplasm. Diffuse uptake in bone marrow indicated its benign activation as a result of chemotherapy. A reconstruction artefact can be clearly seen due to FDG extravasation during intravenous administration (5). Result: PET investigation confirmed major metastatical involvement in both lungs, moreover it also revealed a metastasis in the region of the acetabulum.

Drobné ložisko v místě křížení ureteru s ilických cévním svazkem je klinicky nevýznamnou retencí aktivní moči u ležícího pacienta (dvojitá šipka). Závûr: FDG PET prokázala perzistenci generalizovaného nádorového onemocnění, které nereagovalo na léčbu.
Case 41: A patient who had undergone chemotherapy for alveolar rabdomyosarcoma was referred to FDG PET with the aim to confirm clinical remission of the disease. Finding: Intense hypermetabolism in lesions of the caput humeri bilaterally, in the pelvis and femora represent a neoplasm – single arrows. A small focus in the region where the ureter crosses the iliac vessels, reveal a clinically unimportant depot of “hot” urine in the prone patient (double arrow). Result: FDG PET revealed a persistent generalised neoplasm that was not responding to chemotherapy.

Pfiípad 42: U pacienta s necharakteristickými abdominálními příznaky bylo sonograficky zjištěno zvětšení retroperitoneálních uzlin a byla vyjádřena suspekce na lymfom. FDG PET byla indikována za účelem stagingu. Nález: Rozsáhlá splývající ložiska intenzivního hypermetabolizmu glukózy na krku, periklavikulárně, v mediastinu a v retroperitoneu svědčí pro neoplazii, suspektní je lymfom – posléze byl histologicky prokázán B non-hodgkinský lymfom.
Case 42: Enlargement of the retroperitoneal lymph nodes was revealed by ultrasonography in a patient suffering from non-specific abdominal symptoms. FDG PET was indicated for the staging of a suspected lymphoma. Finding: Large coalescent foci of intense hypermetabolism at the neck, around the clavicles, in the mediastinum and in the retroperitoneum represented a neoplasm, probably lymphoma. Later on, non-Hodgkin’s B lymphoma was defined histologically.
Již po druhém cyklu chemoterapie byla patrna úplná normalizace původního rozsáhlého nálezu. Difúzně homogenně zvýšená konzumpce glukózy v kostní dřeni představuje fyziologickou aktivaci po chemoterapii.
There was complete normalisation of the originally vast focal accumulation just after the second cycle of chemotherapy. Diffuse uptake in bone marrow represents its benign activation after chemotherapy.
Závûr: FDG PET v rámci stagingu poukázala na značný rozsah onemocnění a již v časné fázi léčby predikovala její dobrý efekt. Doposud během 13 měsíců sledování nedošlo k relapsu onemocnění.
Result: FDG PET revealed extensive neoplastic involvement and was able to predict the positive results from the treatment in its initial phase. There have, as yet, been no signs of a relapse during the 13 months of follow-up.
FDG PET je všeobecně uznávaná jako velmi přínosná metoda pro staging lymfomů a monitorování efektu terapie. Dle vlastních zkušeností změní léčebný postup u ~15 % konvenčně vyšetřených pacientů před léčbou.

Case 43: FDG PET was used to monitor the treatment of Hodgkin disease. Finding: Multiple foci in the mediastinum before treatment indicated the presence of a neoplasm (1). Clinically non-significant FDG uptake in the fatty tissue (2) of an anxious patient can of course mask a neoplasm.

Následné histologické vyšetření mediastinálního ložiska nenalezlo nádorové buňky. Chirurgovi se zřejmě nepodařilo odebrat viabilní nádorovou tkáň. Přes další chemoterapii nález na FDG PET progredoval (4).
A histological verification of the residual mediastinal mass was unable to find any viable tumour on completion of therapy. The surgeon did not succeed in taking a sample of viable neoplasm. FDG PET findings continued during further follow-up (4).

Závûr: FDG PET správně posoudila inkompletní odpověď nádoru na léčbu.
Podle vlastních zkušeností má FDG PET značný prognostický význam v průběhu léčby lymfomů. Již po druhém cyklu chemoterapie lze poměrně spolehlivě odlišit nemocné, u kterých dojde k relapsu onemocnění brzy po léčbě nebo již v jejím průběhu.
Result: FDG PET correctly assessed the incomplete response of the tumour to chemotherapy.

Pfiípad 44: U pacientky po antibiotické léčbě bronchopneumonie byla nalezena defigurace mediastina na RTG. Následné CT odhalilo masu v pravém plicním hilu. Thorakotomie s pokusem o biopsii byla předčasně ukončena pro silné krvácení z nádoru. FDG PET byla indikována k biologické charakterizaci ložiska a odhalení případných dalších přístupnějších ložisek pro plánovanou další biopsii (zvažován lymfom). Nález: Solitární ložisko středně zvýšené konzumpce glukózy je patrné v oblasti pravého plicního hilu (1). Vedlejším nálezem je aktivovaná tuková tkáň (2). Závûr: Význam FDG PET tkví v tom, že neodhalila jiná ložiska. Klinicky v úvahu přicházející maligní lymfom je méně pravděpodobný vzhledem k nižšímu stupni metabolizmu, jinou neoplazii však odlišit nelze. Opakovaná úspěšná biopsie prokázala benigní angiofolikulární hyperplazii, tzv. Castlemanův nádor.
Case 44: An enlarged mediastinum was revealed by plain chest radiograph after antibiotic treatment for bronchopneumonia. CT subsequently revealed a mass in the right pulmonary hilum. The biopsy during diagnostical thoracotomy was unsuccessful due to serious bleeding from the tumour. FDG PET was indicated to assess the biological behaviour of the mass and to localise any other more accessible lesions for another biopsy (the possibility of lymphoma was considered). Finding: There is a solitary lesion of intermediate glucose consumption in the region of the right lung hilum (1). Uptake in fatty tissue (2) is clinically non-significant. Result: FDG PET did not reveal any other lesions. The probability of lymphoma was lower because the glucose consumption was only intermediate. No other neoplasm could be differentiated. A second, successful, biopsy revealed benign angiofolicular hyperplasia, also known as Castleman’s tumour.

Pfiípad 45: U pacienta s neurologickými příznaky byla diagnostikována mozková metastáza. FDG PET byla indikována za účelem lokalizování primárního nádoru při negativním nálezu na RTG plic. Nález: Ložiska hypermetabolizmu glukózy v plicích (1), plicním hilu (2), v mediastinu (3), v nadledvině (4), ve sternu (5) a hrudním obratli (6) představují generalizaci neoplazie. Závûr: FDG PET odhalila pravděpodobný plicní karcinom s postižením hilových uzlin a metastázami v mediastinu, nadledvině a kostní dřeni. Později byl prokázán velkobuněčný plicní adenokarcinom s hlenotvorbou.
Case 45: A brain metastasis was diagnosed in a patient with neurological symptoms. A plain chest radiograph was negative. FDG PET was indicated to localise an unknown primary tumour. Finding: Hypermetabolic foci in the right lung (1), lung hilum (2), in the mediastinum (3), in the left adrenal gland (4), the sternum (5) and thoracic spine (6) show a generalised neoplasm. Result: FDG PET revealed probable lung cancer with the involvement of hilar lymph nodes and metastases in the mediastinum, adrenal gland and bone marrow. Later on, large-cell lung adenocarcinoma with mucogenesis was proven.

Pfiípad 46: U asymptomatické pacientky sledované třetím rokem po léčbě kolorektálního karcinomu se začala zvyšovat hladina nádorového markeru CEA. Při lokalizaci pravděpodobné recidivy karcinomu selhala veškerá vyšetření, a proto byla indikována FDG PET. Nález: Při FDG PET bylo nalezeno hypermetabolické ložisko ve štítné žláze vysoce suspektní z neoplazie, jinde byl nález fyziologický. Diferenciálně diagnosticky přicházel v úvahu i benigní adenom. Závûr: FDG PET odhalila díky velkému zornému poli ložisko, které bylo histologicky určeno jako papilární karcinom štítné žlázy. Po jeho adekvátní léčbě došlo k normalizaci hladiny CEA.
Case 46: An asymptomatic patient was monitored after the treatment for colorectal cancer. During the third year, the level of the CEA tumour marker began to increase. Conventional investigations were unsuccessful in localising a recurrent neoplasm. Therefore FDG PET was indicated. Finding: FDG PET revealed a hypermetabolic focus in the thyroid gland giving rise to suspicions of a neoplasm. Benign adenoma was also considered. Everywhere else the findings were normal. Result: Thanks to its large field of view, FDG PET was able to reveal a thyroid lesion. It was histologically evaluated as papillary thyroid carcinoma. After the appropriate treatment, the level of CEA returned to its normal value.
FDG PET je vysoce citlivou metodou pro diagnostiku většiny zhoubných nádorů. Tomu, aby se stala screeningovou metodou, však brání její vysoká cena a omezená dostupnost.
FDG PET is a highly sensitive diagnostic method for the majority of neoplasms. Its limited availability and high price greatly limits its usage as a screening procedure.

Nález: Nehomogenně zvýšená konzumpce glukózy v okolí krčku pět let staré endoprotézy pravého kyčelního kloubu by sama o sobě představovala klinicky nevýznamné reparativní změny. Vzhledem ke zvýšené konzumpci glukózy i v okolí dříku a především v kostní dřeni acetabula je třeba uvažovat o zánětlivé komplikaci. Závûr: Následně byla prokázána TBC v pravém kyčelním kloubu.
Case 47: FDG PET was indicated to localise an inflammatory focus in a patient with sepsis. Finding: The non-homogenous uptake around the neck of the five-year-old hip endoprosthesis is a common, clinically non-significant finding. However the increased uptake in the bone marrow of the acetabulum raises suspicions of an infectious complication. Result: Later on tuberculosis was proven in the right hip.
Zvýšená konzumpce glukózy v okolí krčku totální endoprotézy kyčelního kloubu je běžným nálezem a obráží klinicky nevýznamné reparativní změny. Při odlišné distribuci je však třeba uvažovat o zánětlivých komplikacích.
Increased FDG uptake around the neck of hip endoprosthesis is a common, clinically non-significant finding. It represents reparative changes. A different pattern in uptake can indicate an infection.

Pfiípad 48: Pacientka po hysterektomii a appendektomii pro karcinom endometria byla odeslána k FDG PET pro suspekci z generalizace nádorového onemocnění vzhledem ke známému ložisku v pánvi. Nález: Rozsáhlé ložisko výrazného hypermetabolizmu glukózy s centrální nekrózou v pánvi je vysoce suspektní z neoplazie. Závûr: FDG PET sice spolehlivě lokalizovala hypermetabolické ložisko, jeho povahu však nebyla schopna specifikovat. Následně byl totiž prokázán absces v m. psoas major.
Case 48: There was a known focus in the pelvis of a patient after hysterectomy and appendectomy for carcinoma of the endometrium. FDG PET was indicated for a suspected generalised neoplasm. Finding: A large hypermetabolic lesion with a central necrosis in the pelvis was highly indicative of a neoplasm. Result: FDG PET reliably localised the hypermetabolic lesion, but was unable to specify its histological character. Later on an abscess was proven in the psoas major muscle.
Je třeba mít stále na paměti, že s pomocí FDG PET lze hodnotit metabolizmus glukózy a nikoliv histologii. A proto i zjevné výrazně hypermetabolické ložisko nemusí představovat jen neoplazii, ale i zánět.
It is important always to keep in mind that FDG PET evaluates glucose metabolism and not histology. So a clearly apparent hypermetabolic lesion may not only indicate a neoplasm, but also an inflammation.

Pfiípad 49: Pacientka s farmakorezistentní temporální epilepsií a naznačenou meziotemporální sklerózou vlevo dle MRI byla indikována k FDG PET za účelem lateralizace epileptogenní zóny před epileptochirurgickým výkonem. Nález: Temporálně vlevo je patrný hypometabolizmus glukózy. Vyšetření bylo koregistrováno s databází kontrolních vyšetření a porovnáno „voxel po voxelu“. Zeleně je zvýrazněna oblast, která se liší o více než 2 směrodatné odchylky od kontrolního souboru. Závûr: Interiktálně zachycený hypometabolizmus glukózy výrazně podporuje klinickou suspekci na genezi epileptických záchvatů z levého temporálního laloku.
Case 49: A patient with pharmacoresistant temporal epilepsy and MRI findings of a probable mesiotemporal sclerosis on the left side was referred for FDG PET with the objective of lateralising the seizure focus before neurosurgical treatment. Finding: Hypometabolism was found in the left temporal lobe. The acquired data were registered against the normal template and compared voxel by voxel. Voxels showing an activity lower than two standard deviations from normal are marked in green. Result: Interictal glucose hypometabolism strongly supported the clinical suspicion that the origin of seizures lay in the left temporal lobe.

Pfiípad 50: Pacient tři roky po resekci kortikální dysplazie parietálně vpravo, t.č. s progresí farmakorezistentních epileptických záchvatů. FDG PET byla indikována pro lokalizaci epileptogenní zóny před zvažovanou reoperací. Nález: S interiktálním vyšetřením (A) bylo koregistrováno vyšetření provedené těsně po epileptickém záchvatu (B). Bylo provedeno vzájemné porovnání obou vyšetření a odchylky v akumulaci větší než 30 % byly zeleně zvýrazněny (C). Funkčně odlišná oblast byla superponována na koregistrované MRI vyšetření (D). Při všech vyšetřeních je patrný pooperační defekt parietálně vpravo. Oblast v jeho blízkosti je interiktálně hypometabolická a periiktálně se stává hypermetabolickou (šipky). Závûr: Kombinace interiktálního a periiktálního vyšetření umožnila lokalizovat epileptogenní zónu i v terénu změněném předchozí operací. Fúzí s MRI bylo dosaženo zpřesnění lokalizace nezbytné pro neurochirurgický výkon.
Case 50: Three years after resection of cortical dysplasia in the right parietal lobe, a patient was presented showing a progression in the seizures. FDG PET was indicated to localise the seizure focus before neurosurgical operation. Finding: An interictal investigation (A) was registered and then compared with data acquired just after a seizure (B). Voxels showing a difference in activity greater than 30 % are marked in green (C). A functionally different region is overlaid on the registered MRI investigation (D). There is a postoperative defect apparent in the right parietal lobe. Next to it is an interictal hypometabolism that has become hypermetabolic periictally (arrows). Result: A combination of interictal and periictal investigation enabled the localisation of a seizure focus despite the fact that the previous surgery had resulted in an anatomical lesion. Image fusion with MRI enabled a more precise localisation of the focus before operation.

After previous surgery, the interictal FDG PET investigation does not provide a specific enough result. Hypometabolism in the seizure focus cannot be distinguished from hypometabolism resulting from deafferentation of neurones in the operated brain tissue. Ictal FDG PET can hardly be carried out as a daily routine; therefore we recommend a combination of an interictal PET and an ictal perfusion SPECT.

Pfiípad 51: Pacient po Q infarktu přední stěny před dvěma lety, nyní s akutním uzávěrem RIA a neúspěšnou PTCA, byl indikován k FDG PET před rozhodnutím o provedení bypassu. Nález: Trojice navzájem kolmých řezů vedených osami levé komory reprezentuje vlevo glukózový metabolizmus (FDG PET) a vpravo perfúzi myokardu (MIBI SPECT). Infero-apikálně je patrný výrazný hypometabolizmus a hypoperfúze (1), což reprezentuje neviabilní myokard. Antero-apikálně je v hypoperfundované oblasti přítomen hypermetabolizmus, což svědčí pro hibernovaný, tj. viabilní myokard (2). Závûr: Kombinované perfúzně-metabolické vyšetření myokardu prokázalo viabilní myokard a podpořilo tak rozhodnutí k provedení bypassu včetně určení segmentu vhodného k revaskularizaci.
Case 51: A patient with a history of a Q myocardial infarct of the anterior wall two years previously was referred to FDG PET before deciding on a bypass graft. He had recently undergone an acute occlusion of LAD and an unsuccessful percutaneous angioplasty. Finding: A triad of perpendicular slices through the axes of the left ventricle reveals glucose metabolism (FDG PET) on the left side, and myocardial perfusion (MIBI SPECT) on the right side. Marked hypometabolism and hypoperfusion are recorded in the inferior aspect of the apex (1), representing the non-viable myocardium. In the anterior hypoperfused aspect of the apex glucose hypermetabolism reveals the hibernated, i.e. viable myocardium (2). Result: A combined perfusion-metabolic investigation revealed a viable myocardium and supported bypass grafting, including localisation of the segment requiring revascularisation.

Pfiípad 52: Diabetik na perorálních antidiabeticích po opakovaných infarktech myokardu byl indikován k FDG PET pro průkaz viabilního myokardu před uvažovanou revaskularizací. Nález: Trojice navzájem kolmých řezů vedených osami levé komory reprezentuje vlevo FDG PET a vpravo perfúzi myokardu (MIBI SPECT). Přestože je ve většině myokardu zachovaná perfúze, myokard se pomocí FDG PET vůbec nezobrazil a FDG zůstala v krvi v lumenu komor. Závûr: Přes i.v. podání inzulínu a redukované množství cukru per os nezačaly myocyty zvýšeně utilizovat glukózu a FDG PET je nehodnotitelná.
Case 52: A patient with diabetes treated by per oral antidiabetics had a history of repeated myocardial infarcts. He was referred for FDG PET before revascularisation in order to assess whether the myocardium was viable. Finding: A triad of perpendicular slices through the axes of the left ventricle shows glucose metabolism on the left side (FDG PET) and myocardial perfusion on the right side (MIBI SPECT). From the SPECT images we could see that the perfusion has been saved in most of the myocardium, and PET was unable to identify FDG uptake in the myocardium, because high concentrations of FDG remained in the blood. Result: Despite the intravenous administration of insulin and a reduced per oral dose of sugar, the myocytes were still not utilising the glucose to any greater extent. The FDG PET examination was of no diagnostic value.

Onkologické indikaãní desatero
1) PET je makroskopická zobrazovací metoda, která v praxi nemůže zachytit mikroskopickou lézi. Nádory lze obvykle zobrazit od průměru 5 mm. 2) Citlivost i specifičnost metody je obecně velmi vysoká, nikoliv však stoprocentní. Citlivost je nižší u diabetiků – u dekompenzovaného diabetu indikace postrádá smysl. 3) FDG PET selhává u karcinomu prostaty, u neoplazií lokalizovaných v močových cestách a v některých dalších speciálních případech. 4) Účinná chemoterapie snižuje akumulaci FDG v nádorových buňkách. Čím větší je časový odstup od chemoterapie, tím vyšší je citlivost metody (doporučuje se alespoň dvoutýdenní odstup). 5) Hodnocení v ozářené oblasti může být zhoršeno několik měsíců (vhodný je odstup 3 měsíců, není ovšem podmínkou). 6) Mezi nejvýtěžnější onkologické indikace patří dynamický růst nádorových markerů při neoplazii v anamnéze.

Decalogue of oncological indications
1) PET is a macroscopic imaging modality. It is not capable of visualising microscopic lesions as a daily routine. Neoplasms larger then 5 mm can usually be imaged. 2) Its sensitivity and specificity is generally very high, but not 100 %. Sensitivity is slightly lower in diabetics. Examinations of patients with decompensated diabetes are useless. 3) FDG PET misses prostate cancer and neoplasms within the urinary tract, as well as under certain other special conditions. 4) Effective chemotherapy diminishes FDG uptake in neoplastic cells. A longer time interval between the end of chemotherapy and FDG PET increases the sensitivity of the examination (an interval of at least two weeks is recommended). 5) Any evaluation within an irradiated region within a few months could pose problems (the recommended interval is 3 months, but this is not a strict condition).

6) Any dynamic growth in tumour markers in a patient with a history of neoplasms is one of the best indications for FDG PET.

7) One of the worst indications for FDG PET is,"I have no idea how to make a diagnosis, so I will refer the patient to FDG PET.”
9) FDG PET je nákladná – není však drahá, je-li indikována racionálně (cena odpovídá zhruba jednomu cyklu chemoterapie).
8) When the reference includes the patient's summarised case history, current clinical status and the course of the treatment, it significantly increases the diagnostic certainty of the PET results.
10) PET není jen vyšetření s pomocí FDG. PET lze podobně jako scintigrafii provádět s řadou preparátů a zobrazit tak různé funkční pochody.
9) FDG PET is expensive – but when properly used it does not represent a waste of money (the price can be approximately compared to the cost of one cycle of chemotherapy). 10) PET is not only performed using FDG. In a similar way to scintigraphy, PET can be carried out with many different radio-pharmaceuticals to enable a number of different functional processes to be visualised.
