Příčiny a léčba úrazů horních stálých řezáků Causation and treatment ofupper permanent incisors injuries

V experimentální části byla hodnocena skupina pacientů s diagnózou úrazu horních stálých řezáků. Summary The aim ofthis study is to emphasize the most frequent etiologicl factors of injuries to upper permanent incisors and present the latest approaches to the therapy ofthe upper permanent incisors injuries from the viewpoint of the orthodontic phase of the treatment. Key Words: injury, upper permanent incisors, etiology, therapy Úvod Úrazy, které si pacienti přivodiv dětství patří k nepříjemným životním zkušenostem. Nejčastějším úrazem u dětí je poranění hlavy. Na druhém místě je ve statistice úrazů poranění horních a dolních končetin (1). Podle Kiliána (2) je na 1. místě v etiologii úrazů zubů pád při chůzi, běhu, uklouznutí/21,3%/. Jako následující etiologické faktory jsou uvedeny sportovní aktivity /18%/, především je udáván hokej. Nejvíce vznikají úrazy zubů u chlapců mezi 9.-10. rokem, u dívek mezi 10.-11. rokem. Nejčastěji poraněné stálé zuby jsou: - horní centrální řezáky 75,0%, - dolní centrální řezáky 10,5%, - horní laterální řezáky 8,7%, - dolní laterální řezáky 4,4%. Nejběžnějším typem úrazů zubů je fraktura skloviny a dentinu bez otevření dřeňové dutiny. Existují podle Štefkové a Kamínka (3), Šubrtové (4) a Kiliána (2) i určité ortodontické anomálie, kde může být pravděpodobnost úrazů horních stálých řezáků vyšší. Introduction Accidents met with and injuries suffered in childhood are amongst the most unfavourable experiences in one's life. The most frequent injury in children is an injury of skuli. The second most frequent injury is the injury of upper and lower limbs (1). According to Kilián (2) the first pláce in etiology of teeth injuries occuppies the fall during walking, running, slip (21.3%). Among other factors we should cite sports activities (18%), ice hockey above all. Mostly, teeth injuries occur in boys of the age between 9 and 10, in girls between 10 and 11. The most often injured teeth are the following: - upper centrál incisors 75.0% - lower centrál incisors 10.5% - upper lateral incisors 8.7% - lower lateral incisors 4.4%. The typical tooth injury is the fracture of enamel and dentine without exposure of pulp cavity. žáků, u All. 2. oddělenía při incizálním schůdku rovnajícím se nebo větším než 9 mm. Všeobecně dělíme úrazy obličeje na úrazy čelistí, úrazy měkkých tkání a úrazy zubů a závěsného aparátu (5). Ošetření úrazů zubů a závěsného aparátu vyžaduje zákrok stomatochiruga, praktického zubního lékaře, protetika a také ortodontisty. Je několik indikací pro komplexní terapii úrazů zubů s ortodontickou intervencí. Cílem této práce je zdůraznit a popsat ortodontickou část léčby při terapii fraktury zubu zasahující do subgingivální oblasti, intruzní luxace zubu a úplné luxacezubu. Ortodontická terapie fraktury zubu. Vznikne-li následkem úrazu zubu fraktura zasahující pod úroveň marginální gingivy, není možné zub ošetřit přímo protetickou náhradou. V rámci komplexní terapie je zde řešením léčba ortodontickým aparátem s následným protetickým ošetřením Léčebný ortodontický postup se nazývá termínem „erupce silou"/„forced eruption"/, nebo jinak také termínem „kontrolovaná extruze"/„controlled extrusion"/ (6.). Přesná definice termínu „forced eruption" je: „aplikace ortodontického tahu na zařazení neeruptovaných nebo impaktovaných zubů do jejich fyziologické pozice v zubním oblouku" (7). Proffit (6) indikuje tento postup u ortodontické terapie defektů zubů v cervikální třetině kořene. Kromě fraktury příčné a šikmé se jedná i o další poškození zubů v této lokalizaci. Jde například o patologickou perforaci, interní nebo externí resorpci. Před léčbou je nutné: 1. Důkladně zhodnotit intraorální rentgenogram s periapikální projekcí. Musíme vyloučit postižení kořene apikální frakturou, dilacerací nebo hypercementózou. Tyto nálezy by mohly být komplikací při terapii. 2. Zhotovit kvalitní definitivní endodontické ošetření zubu. 3. Přesně určit rozsah extruze zubu při léčbě. Velikost extruze zubu je podmíněna: A. Umístěním defektu (zlomeniny,perforace,...). B. Velikostí zbylého zubu. Po dokončené kontrolované extruzi musí být zub lokalizován svým okrajem alespoň 1 mm nad úrovní marginální gingivy . C. Šířkou připojené gingivy, která vznikne po extruzi. Šířka připojené gingivy by měla být po zákroku dostatečná, minimálně 2 mm. Všeobecně má být kontrolovaná extruze prováděna rychlostí maximálně 1 mm za týden, bez poškození periodontální štěrbiny. Průměrná doba aktivní léčby se pohybuje okolo 3-6 týdnů. Příliš rychlá extruze nebo nadměrně velká použitá extruzní síla vede ke vzniku

which the risk of injury to upper permanent incisors is higher. It is especially the diagnosis of protruding upper permanent incisors, in All. Class 2, and in čase of overjet approximating or higher than 9 mm. In generál, we may differentiate injuries of faceto gnathic injuries, injuries to soft tissues, injuries of teeth and supporting apparatus (5). Teeth injuries and supporting apparatus injuries must by managed by a surgeon, generál dentist, prosthodontist and orthodontist. There are several indications for the complextherapy of teeth injuries accommpanied with orthodontic intervention. The aim of the article is to give a survey and description of the orthodontic part in the management of a tooth fracture involving subgingival area, intrusive tooth luxation and total tooth luxation. Orthodontic therapy of tooth fracture If a tooth injury results in the fracture intervening underthe level of marginal gingivathen the tooth cannot betreated directly with a prosthetic implant. Within the complex therapy the solution is seen in the treatment with orthodontic appliance and the successive prosthetic treatment. The orthodontic method of treatment ušed is called „forced eruption" or „controlled extrusion" (6). Forced eruption is defined as „the application of orthodontic traction to guide an unerupted or impacted tooth into its proper position in the dental arch". (7) Proffit (6) indicates the approach in orthodontic therapy of teeth defectsin the cervicalthirdof the root.Besides lateral and oblique fracture also other defects of teeth in this location aretreated, e.g. pathological perforation, internal or external resorption. Before the treatment starts it is necessary: 1. Detailed assessment of intraoral X-ray with periapical projection. It is necessary to eliminate apical fracture of the root, dilaceration or hypercementosis. These findings could become complications in the therapy. 2. Good quality finál endodontic treatment of the tooth. 3. Define precisely the scope of the tooth extrusion during the treatment. The extent of extrusion is influenced by: A. Location of the defect (fracture, perforation). B. Size of the remaining part of the tooth. After controlled extrusion the tooth must stay with its edge at least 1 mm above the marginal gingiva. C. Width of adjacent gingiva that is formed after extrusion. The width of the adjacent gingiva should be - after the intervention - sufficient, i.e. at least 2.0 mm. In generál, controlled extrusion should be doně at the maximum of 1.0 mm per week, if we want to avoid damage caused to periodontal crevice. The mean length of active therapy is about 3-6 weeks. tkáňového poškození nebo ankylózy. Poměr korunka: kořen by měl být po léčbě minimálně 1:1. Proffit /6/ doporučuje léčbu ortodontickým fixním aparátem. Tento aparát se skládá ze dvou částí. První část slouží jako kotevní. Kotevní část je určena větším počtem zubů opatřených zámky. Zuby by měly být vzájemně spojeny rigidně. Druhou část fixního ortodontického aparátu lze nazvat jako pracovní. Tato část zahrnuje úrazem poškozený zub. Tento zub je opatřen retenčním zařízením na aplikaci tahu. Na zraněný zub by se mělo působit kontinuálníextruznísilou o velikosti 25-40gramů. Je-li dostatečně dlouhá klinická korunka, lze na ní nalepit zámek. U rozsáhleji poškozeného zubu s perforací dřeňové dutiny je nutné nacementovat do dřeňové dutiny intrapulpální čep s očkem nebo závitem. Pracovní část je spojena s kotevníčástí flexibilně. Je možné použít dva typy fixních ortodontických aparátů.

extrusion or excessive extrusion force applied result in the origin of damage to tissues or to ankylosis. After the treatment process the ratio crown:root should be 1:1 atthe minimum. Proffit (6) recommends the treatment with a fixed appliance. The appliance is composed of two parts. The first part should serve as an anchorage. It is defined by a higher number of teeth equipped with brackets. The teeth should be firmly tight together. The other part of the appliance may be called „operative" or „working site". There we also find the damaged tooth. The tooth is equipped with a retention clasp for the traction application. The injured tooth should be continually under the force of 2540gramms. If the clinical crown is sufficient, the bracket can be bonded onto it. In a moreseriously damaged tooth with pulp cavity perforation it is necessary to cement into the cavity an intrapulpal pivot with a loop or a coil. This part is flexibly connected with the anchorage. There are two types of fixed orthodontic appliances ušed.

také kontrolovat zda nedochází k nákusu dolních zubů na extrudovaný zub. Bude-li takto zub traumatizován, pak je vhodné místo předčasného kontaktu zábrusem odstranit z okluze. Nejčastěji dochází k předčasnému nákusu na palatinální straně extrudovaného řezáku. Po aktivní fázi silové extruze se doporučuje zuby stabilizovat. Nejvhodnější je použít silný hranatý rigidní ocelový drát, který bude pasivně ležet ve všech zámcích. Tento drát bude rigidně spojen i s intrapulpálním čepem. V této fázi dochází k reorganizaci periodontálních vláken a remodelaci alveolární kosti. Stabilizační fáze by měla trvat minimálně 3-6 týdnů. Působením ve stabilizační fázi se snažíme snížit riziko recidivy. V další fázi je možné provést definitivní ošetření extrudovaného zubu. Tento zákrok by se měl provádět až za 1 měsíc po kompletní silové extruzi zubu. Jedná se parodontální operace, následné konzervační nebo protetické ošetření.
It is also important to control whether there does not occur bite of lower teeth on the extruded tooth. If this occurs then it is advisable to lap the pláce of the early contactoffocclusion. Our main task during the stage is to eliminate the risk of relapse. The next stage may be devoted to the finál treatment of the extruded tooth. The intervention shoul be carried out not earlierthan a month afterthe complexforced extrusion ofthe tooth. The intervention involves parodontal interventions, following conservative or prosthetic treatment.
Ortodontická terapie traumatické intruze Traumatická intruze nebo repulze je stav těžké subluxace zubu. Zub je vtlačen do lůžka v důsledku působení axiální úrazové síly. Všeobecně v rámci terapie je možné postupovat dvěma způsoby. Dle mnohých autorů je možné zub ponechat zpětně prořezat (8). V případě, že zub spontánně neprořezává, je vhodné zařadit aktivní extruzní ortodontický tah s použitím fixního aparátu. Postup je podobný jako u výše popsané metody „erupce silou" . Jiní autoři doporučují zahájit aktivní extruzní tah na zub bezprostředně po úrazu (9). Proffit (6) udává, že zuby, které byly intrudovány traumaticky, nemají většinou tendenci ke spontánnímu znovuzařazení. Naopak se zvyšuje riziko vzniku ankylózy. Zde doporučuje immediátní ortodontickou léčbu ve smyslu „kontrolované erupce" s použitím fixního aparátu. Andreasen (9) doporučuje zahájit extruzní ortodontický tah na úrazem intrudovaný zub v prvních 5-ti dnech po úrazu zubu. Důvodem k tomuto postupu je snížení incidence vzniku resorpce kořene na 30%40% a snížení možnosti vzniku ankylózy. Skiellerová (10) zahajuje bezprostřední repozici ortodontickým tahem co nejdříve po vzniku úrazu zubu. Taintor (8) zastává názor, že po úrazu je vhodné ponechat zub spontálně reeruptovat. Nebudeme-li úspěšní, doporučuje až ve druhé fázi zařadit ortodontický tah. Délku období, kdy vyčkáváme na spontální reerupci zubu autor neuvádí.
Orthodontic therapy of traumatic intrusion Traumatic intrusion or repulsion is the statě of a serious tooth subluxation. The tooth is crushed into its bed due to the effect of axial traumatic force. In generál, two ways of therapy are ušed. A number of authorssuggest it is possibleto letthetooth erupt regressively (8). In čase the tooth does not erupt unpromptly it is advisable to apply an active extrusive orthodontic traction by means of a fixed appliance. The process is similar to that desribed in the above mentioned „forced eruption". Otherauthorsrecommend active extrusive traction on the tooth immediately after the injury (9). Proffit (6) states the teeth that traumatically intruded do not show the tendency to spontaneous reinsertion. On the other hand, the risk of ankylosis is raised. He suggests an immediate orthodontic therapy of "controlled eruption" with the use of the fixed appliance. Andreasen (9) recommends to start the extrusive orthodontic traction on the intruded tooth within the first five days after the injury. The reason is the lower risk of root resorption incidence (lowered to 30% 40%) and the lower risk of ankylosis. Skieller (10) starts the immediate reposition by means of the orthodontic traction as soon as possible after the tooth injury. Taintor (8) states that it is advisable to let the injured tooth reerupt spontaneously. If we are not successful then we may apply the orthodontic traction in the second phase. The author does not mention the time to spend waiting for the spontaneous tooth reeruption.

V rámci terapie zvažujeme možnost replantace zubu nebo komplexní řešení vzniklé mezery v zubním oblouku. A Replantace (tzv. časná replantace) má šanci na úspěch, pokud se provede do 90 minut po úrazu zubu. Zub musí být do té doby uchováván ve vlhkém prostředí. Pozdní replantace se provádí maximálně do několika hodin po úrazu. Zub je vhodné propláchnout v roztoku antibiotika, endodonticky ošetřit a někdy je nutné sejmout apikální část kořene. Peřinka doporučuje následně fixovat zub po dobu 6-ti týdnů (11). B. Dojde-li ke ztrátě horního stálého řezáku, buď přímo při úrazu nebo neúspěšnou replantací, vznikne v oblasti horního frontálního úseku mezera. Řešenítohoto stavu má dvě možnosti. Můžeme mezeru uzavírat meziálním posunem laterálních zubů nebo ji budeme udržovat popřípadě rozšiřovat na náhradu (12). V rámci řešenítohoto problému je vhodné zhodnotit několik hledisek. Podle práce Štefkové a Špidlena (13) musíme brát v úvahu fakt, že při řešení luxace horního stálého řezáku přizpůsobujeme hornízubníoblouk dolnímu zubnímu oblouku. Při řešení ztráty horních stálých laterálních řezáků se spíše přikláníme: K udržení mezery u převislého skusu, obráceného skusu, skusu „hrana na hranu", pravidelného zubního oblouku bez mezer a stěsnání. Uzavření mezery indikujeme spíše je-li nezaložený nebo hypoplastický druhostranný horní stálý laterální řezák a je-li příznivý tvar horních stálých špičáků (plochý , gracilní, barevně podobný zub). Při řešení ztráty horního stálého centrálního řezáku nejčastěji rozšíříme mezeru na náhradu zubu. Zřídka mezializujeme horní stálý laterální řezák, který pak opatříme fazetou nebo kompozitní dostavbou. Dojde-li ke ztrátě obou horních stálých centrálních řezáků u ortodontické diagnózy distookluze, můžeme mezeru uzavřít mezializací horních stálých laterálních řezáků. Oba postupy mají své indikace a svá úskalí. Budeme-li udržovat mezeru po ztraceném zubu, je nutné si uvědomit, že definitivní protetickou náhradu nelze provést u dětského pacienta. Zároveň však není vhodné, aby pacient nosil fixníaparát až do dospělosti. Vtomto případě lze indikovat provizornířešenímezery. Například můžeme použít adhesivní můstek. Proffit (6) doporučuje pro zvolenísprávného léčebného postupu zohlednit následující kritéria: 1. profil pacienta, 2. pozici řezáků, 3. přítomnost tremat nebo diastematu (což bývá kontraindikací k poúrazovému uzávěru mezery), 24

We consider the tooth replantation or the complex solution of the split in the dental arch. Replantation (the so-called early replantation) is usually successful when carried out within 90 minutes after the tooth injury. Till that time the tooth must be stored in a wet environment. Latě replantation is performed several hours after the injury at the latest. It is recommended to wash the tooth in the antibiotics solution, to treat it endodontically; sometimes it is necessary to remove the apical part of the root. Peřinka suggests to fix the tooth for 6 weeks (11). If an upper permanent incisor is lost dueto the injury or unsuccessful intervention, the split is formed in the area of upper front part. There are two approaches: The split may be dosed by means of shifting lateral teeth in mesial direction or we may maintain or widen the split for a substitute (12). There are few aspects to be taken into account. Štefková and Špidlen (13) mention the fact that in luxation of upper permanent incisor we adjust the upper dental arch in relation to the lower dental arch. In solution of the loss of upper permanent lateral incisors we rather recommend the following: To maintain the split in čase of deep overbite, prognathic occlusion, edge to edge incisor bite, regular dental arch without splits and crowding. Closure of the split is indicated rather in čase when the upper permanent lateral incisor on the other side is not founded or it is hypoplastic, and the shape of upper permanent canines (fiat, gracile tooth of similar colour) is favourable. In solving the loss of the upper permanent centrál incisor the split is widened to obtain the pláce for a substitute. Rarely, we may mesialisethe upper permanent lateral incisor which is then equipped with a facet or a composite structure. In čase both upper permanent centrál incisors are lost in the orthodontic diagnosis of distocclusion, the split may be dosed by means of mesialisation of upper permanent lateral incisors. Both approaches háve their indications and their risks. If we decide to maintain the split in pláce of the lost tooth, we must také into consideration the fact that the prosthetic substitute cannot be applied in a child. At the samé time it is not appropriate to let the patient wear a fixed applianace till their adulthood. We may indicate a temporary solution of the split, e.g. adhesion bridge. Proffit (6) states it is necessary to také into account the following criteria before we decide for the appropriate therapy: 1. a patienfs profile 2. position of incisors,

4. polohu horního stálého špičáku, (Pokud dojde ke ztrátě horního stálého laterálního řezáku před prořezáním horního stálého špičáku, je velká pravděpodobnost, že se špičák při prořezávání zařadí meziálněji, na místo horního laterálního řezáku. Horní první premolár se současně posune na místo horního stálého špičáku.) 5. symetrie poúrazových mezer, (Je-li mezera po úrazu asymetrická, je vhodnější jí ponechat na náhradu.) 6. vztah zubních oblouků dle Anglea. Kokich (14) doporučuje pečlivě zvážit dva odlišné postupy při řešení chyběni horního laterálního stálého řezáku: uzavřenímezerynebojejíudrženínadefinitivnínáhradu. Statisticky četněji (v 80%) Kokich mezeru udržuje, nejčastěji na implantát. V menším počtu případů(20%) mezeru uzavírá mezializacílaterálních zubů. Pro řešení doporučuje zvážit následující hlediska: 1. Okluze: Je-li II. třída Angle bez stěsnání v dolní čelisti, vhodnějšířešeníje mezeru uzavírat. Je-li l.třída Angle i s mírným stěsnáním v obou čelistech, je lépe mezeru otevřít. 2. Profil pacienta: Nachází-li se mandibula hodně vzadu, tzv. retrogenní profil, lze mezeru otevřít a chirurgickou korekcí posunout mandibulu ventrálně. Když je profil esteticky uspokojivý, je lepší mezeru zavřít. 3. Poměr velikosti horních a dolních zubů: Autor doporučuje zhotovení SET-Upu, nebo výpočet indexů (Boltonův, Tonnův) pro snadnější rozhodnutí terapie. 4. Úroveň rtu: Je-li při úsměvu úroveň rtu níže (není „gummy smile"), lze mezeru uzavřít mezializacílaterálních zubů. 5. Tvar a postavení horního stálého špičáku : Příliš zakřivený a barevně odlišný špičák je indikací k udržení mezery. Je-li z okluzálního pohledu na korunku stálého horního špičáku příliš zakřivená labiální strana zubu, odráží světlo a vypadá tmavší. Jako první variantu uvádí postup udržení mezery, resp. její otevření a náhradu chybějícího zubu. Ortodontická terapie se provádí nasazením plného fixního aparátu na celou horníčelist. Kokich doporučuje metodu „straightwire". Mezeru získáme stlačenou expanzní pružinou, která je navlečená na přímém hranatém rigidním ocelovém oblouku /ideálně 18x25 ocel/, do místa, kde plánujeme nahradit zub. Dostatek místa na náhradu zubu získáme za několik týdnů. Vhodné je mezeru na náhradu zubu o několik desetin milimetru přeexpandovát. Toto provádíme postupnou aktivací expanzní spirály. Budeme-li například otvírat místo na horní stálý laterální řezák je vhodné rozšířit mezeru na 65-75% šířky horního stálého centrálního řezáku. Minimálně by měla tyto mezera být 5,5 mm, ideálně okolo 7 mm. Důležité je zajistit paralelitu kořenů sousedních zubů pro bezpečné defini-

3. presence of tremata or diastema (which is usually contraindication to the split closure), 4. position of the upper permanent canine (if the upper permanent Iaterai incisor is lost before the upper permanent canine erupted, it is highly probable the canine erupts in a more mesial direction, i.e. in the pláce of a Iaterai incisor; the upper first premolár shifts into the position of the upper permanent canine at the samé time), 5. symmetry of traumatogenic splits (if the split is assymmetric, it is better to decide for a substitute), 6. dental arches relation according to Angle. Kokich (4) recommends to consider in detail two different solutions of the missing upper Iaterai permanent incisor: closure of the split or maintenance of the split forthe finál substitute. ln80% of patients Kokich preserves the split, most often forthe implant. ln20% he closes the split through mesial move of Iaterai teeth. He recommends to consider the following criteria: 1. Occlusion: If Angle II without crowding characterizes the mandible then it is better to close the split. If there is Angle I (including moderate crowding) in both jaws then it is better to open the split. 2. Patienťs profile: If the mandible is rather backward, i.e. retrognathic profile, thesplit may be opened and the mandible may be moved in ventral direction through surgery. If the profile is esthetically correct then it is better to close the split. 3. The size of upper and lower teeth: Kokich recommends to prepare a SET-Up or indices calculation (Bolton index, Tonn index). 4. The level of a lip: If the level of the lip is lower in smile (not a „gummy smile") the split may be dosed by means of mesial move of Iaterai teeth. 5. The shape and position of the upper permanent canine: excessively curved canine of different colour indicates the split preservation. If we look from the occlusal plane at the crown of the upper permanent canine and the labial plane of the tooth is tortuous then it reflects the light and looks darker. Kokich cites the split preservation in the first pláce, he speaks about the split opening and lost tooth substitution. Orthodontic management involves mounting of the full fixed appliance in the maxilla. He recommends the „straight wire" approach. We open the split by means of compressed expansion spring which is set in the direct square firm steel arch (ideál -18x25 steel). Enough space forthe tooth substitute is created in afew weeks. The split for the tooth substitute shouid be a bit larger (about several tenths of mm). The space is obtained by means of gradual activationof expansion spirál. tivní řešení mezery. Definitivní náhrada zubu je možná až po úplném ukončení růstu. Lze ji zhotovit následujícími způsoby: autotransplantace vlastního zubu, intraosseální implantát nebo fixní metalokeramický můstek. Kokich doporučuje definitivnířešení mezery implantátem až po úplném dokončení růstu. U dívek k úplnému ukončení růstu dochází průměrně ve věku 17 let, u chlapců až v 21 letech. Jako provizorní náhrada na přechodné období doporučuje lepený adhesivní můstek. Další možností provizorní náhrady je snímatelný mezerník ve formě horní patrové desky s připevněnou pryskyřičnou náhradou chybějícího zubu. Pro dobrou retenci je důležité, aby dolní řezáky nepáčily náhradu. Kamínek a Nováková (15) doporučují po ztrátě stálého zubu ve většině případů ortodonticky paralelizovat kořeny, otevřít mezeru na náhradu. Mezera bude provizorně držena adhesivním můstkem. Řešení definitivní náhradou doporučují autoři odložit až po 18. roce, ale lépe až ve 20.-24. letech. Kde není možné použít adhesivní můstek z důvodu hypoplasie skloviny, hlubokého skusu..., je u mladistvých metodou volby skeletová snímací náhrada. Pro druhý léčebný postup, tj. uzávěr mezery po chybějícím zubu, doporučuje Kokich provádět fixním ortodontickým aparátem, který je nasazen na celý oblouk metodou „straight wire". Musíme si uvědomit, že při ztrátě řezáků musí mít zuby, které budou posouvány na místo jiného zubu, nalepeny na sobě zámky a kroužky příslušející k tomu místu. Po sejmutí fix. ap. je nezbytné provedení „recontouringu" u příslušných zubů. Experimentální část V experimentální části byl hodnocen soubor 48 pacientů, kteří utrpěli úraz horních stálých řezáků. Sledovalo se pohlaví, věk ve, kterém si úraz způsobili a příčinu úrazu. Byla zaznamenána přesná lokalizace a typ úrazu zubů. U každého pacienta byla provedena klasifikace ortodontických anomálií. Ve skeletální diagnostice byla určena na kefalometrickém rentgenovém snímku skeletální třída a vertikální vztah čelistí. Vzájemné postavení zubních oblouků bylo hodnoceno podle jejich vztahu v rovině sagitální, ve smyslu ventrodorzálním, tj. u každého pacienta byla určena Angleova třída. Postavení horních řezáků bylo charakterizováno v několika parametrech na kefalometrickém rentgenovém snímku. Sklon horních řezáků byl hodnocen úhlem, který svírá jeho podélná osa k linii NS (1+1 k NS). Průměrná hodnota tohoto úhlu je 104° se směrodatnou odchylkou 6° (3). Dále bylo určeno postavenířezáků tzv. interincisiválním úhlem (1+1 k 1-1). Průměrná hodnota tohoto úhlu je 127° se směrodatnou odchylkou 8,5°(3). Byla zaznamenána i poloha horních řezáků vzhledem k „faciální rovině", tj. vzdálenost jejich řezací hrany k linii N-Po v mm. Průměrná velikost této hodnoty je 7 mm se směrodatnou odchylkou 2,5 mm (3). Na modelech byl naměřen incizálníschůdeka hloubku 26

be 65-75% of tne width of tne upper permanent centrál incisor. The split should be 5.5 mm at tne minimum, ideál width is about 7 mm. The adjacent teeth roots must go parallel; this is important for tne finál solution of tne split. The finál substitute of tne tooth may be placed only after tne growth is finished. The substitute may be: autotransplant of onés own tooth, intraosseal implant, fixed metalloceramic bridge. Kokich recommends to solve tne split only after tne growth finished. In girls this is about tne age of 17, in boys in 2 1 . Bonded adhesion bridge may be tne temporary solution as well as removable spacer (upper palatal disk with bonded substitute of tne lost tooth made of resin). To ensure a good retention tne lower incisors must not heave tne substitute. Kamínek and Nováková (15) recommend in most cases to orthodontically parallel tne roots and open tne split for tne substitute. The split will be maintained by means of adhesion bridge. The finál substitute should be applied only after tne age of 18, even better of 20 - 24. If an adhesion bridge cannot be applied due to enamel hypoplasy, deep bite,..., tne method of choice in young patients is skeletal removable substitute. Kokich recommends, in čase of the closure of the split after the lost tooth, to use a fixed orthodontic appliance set on the whole dental arch by means of „straight wire" method. We háve to také into consideration that in čase of lost incisors the teeth which will be moved in the pláce of a different tooth must bear bonded brackets and rings corresponding to that pláce. After the fixed appliance is removed "recountouring" of teeth in question is necessary. Experimental part We made the evaluation of 48 patients who sufferred the injury of upper permanent incisors. We follow their sex, age (at the time of the injury), cause of the injury. We recorded precise localization and the type of the injury. In each patient the classification of orthodontic anomalies was carried out. Within skeletal diagnostics a skeletal class and vertical relation of jaws was determined by means of cephalometric Xray. Relation of dental arches was assessed according to sagittal plane (ventrodorsally), i.e. in each patient Angle class was stated. Position of upper incisors was specified by several parameters given on the cephalometric X-ray. Upper incisors inclination was given by the angle inclined by its longitudinal axis and the line NS (1+1 to NS). The mean value of the angle is 104°, standard deviation is 6°(3). Position of incisors was assessed through the so-called intercisival angle (1+1 to 1-1). The mean value of this angle is 127°, standard deviation 8.5°(3). Position of upper

skusu. Průměrná hodnota incizálního schůdku je 3 mm se směrodatnou odchylkou 0,5 mm (4). Průměrná hodnota hloubky skusu je 2,5 mm se směrodatnou odchylkou 0,5 mm (4). Hodnoty byly měřeny posuvným měřítkem na 0,1 mm. Měření bylo standartně prováděno pouze 1x. Jedná o běžně používaný postup v diagnostice ortodontických anomálií, nebyla počítána chyba měřenía vyšetření se neprovádělo opakovaně. Data byl statisticky vyhodnocena programem "Mathematica" firmy Wolfram Research /USA/. Výsledky byly zpracovány do grafů. Po statistickém vyhodnocení všech výsledků jsem dospěla k následujícím závěrům: 1. V otázce pohlaví došlo ke shodě mé statistiky s většinou autorů (2, 15) a to že u chlapců jsou častěji diagnostikovány úrazy horních stálých řezáků. 2. Jako nejrizikovější věk bylo zjištěno období mezi 8.-10.rokem. (Obrázek č. 3) 3. Nejčastější etiologií úrazu byl dle předpokladu aktivní sport. 4. Mezi nejčastěji postižené horní stálé zuby patří centrální řezáky, více levý/21/, těsně následován pravým /11/. (Obrázek č. 4.) 5. Zlomenina korunky bez perforace dřeňové dutiny byla diagnostikována v nejvyšším procentu případů (56%). (Obrázek č. 5) 6. Nejčastější diagnózou byla 1 .skeletální třída bez výrazné anteriorotace a posteriorotace dolní čelisti. Vztah zubních oblouků byl nejčastěji v I. Angleově třídě. 7. Hodnoty charakterizující postavení 1+1 k linii NS byly u většiny pacientů (54%) v oblasti tolerančního pole průměrné hodnoty. 8. Velikost interincizálního úhlu byla také u většiny pacientů (67%) v oblasti tolerančního pole průměrné hodnoty. 9. Hodnota charakterizující polohu horních řezáků k linii N-Po byla u poloviny pacientů (50%) v oblasti přesahující toleranční pole průměrné hodnoty. (Obrázek č. 6.) 10. Velikost incizálního schůdku byla u většiny pacientů (73%) v oblasti přesahující toleranční pole průměrné hodnoty. (Obrázek č. 7.) 11. Hodnota hloubky skusu byla také u většiny pacientů (73%) v oblasti přesahující toleranční pole průměrné hodnoty. (Obrázek č. 8.)
incisors in relation to "fascial plane" was recorded, i.e. distance of the cutting edge to the line N-Po, in mm. The mean value was 7.0 mm, standard deviation 2.5 mm (3). In models the overjet and depth of bite were measured. The mean value for the overjet is 3 mm, standard deviation 0.5 mm (4). The mean value for the bite is 2.5 mm, standard deviation 0.5 mm (4). Measurements were doně by means of slide caliper rule (accuracy of 0.1 mm), and they were doně just once. This is a common process in diagnostics of orthodontic anomalies; measuring error was not taken into account and the measurement was not repeated. Data were statistically processed with the programme "Mathematica" (by Wolfram Research; USA). Results were presented in diagrams. After statistical processing of the data we came to the following conclusions: 1. Sex: the results obtained agree with majority of authors (2,15), i.e. in boys the most frequent tooth injury is the injury of upper permanent incisors. 2. The most hazardous age is between 8 and 10. (Fig.3) 3. The most frequent etiology is active sports. 4. Among the most frequently affected upper permanent teeth we find centrál incisors - left (21), right (11). (Fig.4) 5. In the highest number of patients (56%) fracture of the crown without pulp cavity perforation was diagnosed. (Fig.5) 6. The most frequent diagnosis: 1st skeletal class, without distinct anteriorotation and posteriorotation of mandible. Angle I was the most frequent for the relation of dental arches. 7. Values of 1 +1 in relation to the NS line were in most patients (54%) within tolerance space of the mean value. 8. Values of interincisal angle were in most patients (67%) within tolerance space of the mean value. 9. Value of position of upper incisors to the N-Po line exceeded in 50% of patients tolerance space of the mean value. (Fig.6) 10. Value of the overjet exceeded in most patients (73%) tolerance space of the mean value. (Fig. 7) 11. Values of bite depth exceeded in most patients (73%) tolerance space of the mean value. (Fig.8)
Závěr Ve své práci jsem se pokusila shromáždit poznatky týkajícíse příčin a terapie úrazů horních stálých řezáků. Z informací týkajících se terapie úrazů horních stálých řezáků je důležité si uvědomit, že léčba této diagnózy je vhodná provádět v několika fázích a v rámci interdisciplinární spolupráce s ošetřujícím praktickým zubním lékařem, chirurgem, protetikem a parodontologem.
Conclusion In my work I strove to give a survey of experinces dealing with causation and therapy of the injured upper permanent incisors. It is important to také into account that the therapy should proceed in several phases in interdisciplinary cooperation with a generál dentist, surgeon, prosthodontist and parodontologist. 27

Všechny úrazy zubů je důležité ošetřit dle aktuálního stavu v rámci první pomoci. U některých typů úrazů : fraktura zubu zasahující subgingiválně, - intruzní luxace zubu, - úplná luxace zubu, je vhodné následující fázi léčby provádět ve spolupráci s ortodontistou. Včasně zahájenou a vhodně indikovanou ortodontickou terapií lze významně zlepšit možnost definitivního ošetření chrupu . Výsledky mého vlastního šetření částečně potvrzují poznatky uvedené v literatuře. Je podstatné upozornit, že můj soubor byl méně početnější než srovnávané soubory, tudíž i výsledky nejsou signifikantně tak významné.

Teeth injuries shouid be treated according to the actual statě within the first aid. In some injuries (tooth fracture intervening subgingivally, intrusivetooth luxation, total luxation of tooth) the successive stage of therapy shouid involve also the cooperation with an orthodontist. The early and appropriate orthodontic therapy may significantly improve the possibilities for the finál management of dentition. Results obtained during my own examination partially proved the experiences described in literatuře. It is important to emphasize that the set of patients I worked with is considerably less numerous than compared sets, therefore the results cannot be so significant either. 7r
