Obesity LYNN A. FENTON, M.D. Patients are considered to be obese if they weigh >20% above their ideal body weight (IBW) or have a body mass index (BMI) > 30 kg/m2 (see Table 1). Obesity is usually due to overeating. Secondary etiologies include hypothyroidism, Cushing’s disease, insulinoma, and hypothalamic disease. Bariatric surgeries are appropriate for patients with BMI > 40 (or BMI > 35 in the presence of significant health complications).1 Physiologic sequelae of obesity include increased cardiac output (0.1 L/min per kg adipose), hypertension (HTN), coronary artery disease (CAD), pulmonary HTN, type 2 diabetes mellitus (DM), hiatal hernia (HH) with delayed gastric emptying, increased gastric volumes and acid, GERD, fatty infiltration of the liver, venous thrombosis/pulmonary embolism, poly- cythemia, increased oxygen consumption and carbon dioxide (CO2) production, V/Q mismatch, pulmonary compromise (decreased vital capacity, FRC, expiratory reserve volume, and inspiratory capacity), obstructive sleep apnea (OSA), and obesity hypoventilation syndrome or Pickwickian syndrome. 2–5 A. Perform a preoperative evaluation including a thor- ough history, physical examination, and review of clinical or laboratory data. Determine if further workup is needed for any comorbid conditions. If the patient has OSA or Pickwickian syndrome, evaluate pulmonary function and cardiac function (rule out abnormal ventricular function, pulmonary HTN, and cor pulmonale). If asthma is present, consider pulmonary function tests (PFTs). Examine the airway (AW) carefully to determine the type of intubation required. Discuss anesthetic plans, AW management, and the likely postoperative course with the patient. B. Premedicate the patient with H2 antagonists, meto- clopramide, and nonparticulate antacids. Continue all medications (accepted as safe) used to treat asso- ciated morbidities. Use only IV and oral routes to deliver drugs; IM injections usually deposit into adi- pose tissue resulting in unpredictable
absorption. When significant OSA is present, refrain from sedation. Otherwise, titrate sedation carefully to avoid respira- tory depression. Vascular access may be difficult. Ultrasound devices may be helpful in locating and assessing blood vessels for cannulation. Patients weighing more than 350 pounds may need a special OR table. Pressure points may require extra padding to avoid decubitus ulcers and neurological injuries. C. Monitoring the patient can be difficult. To ensure a proper BP cuff fit, use an extra wide BP cuff or select other sites (e.g., leg or forearm). Alternatively, use an arterial catheter to obtain BP. Use a V 5 or equivalent ECG on all patients. If cardiac function is compromised, place a CVP or pulmonary artery (PA) catheter. D. During minimum alveolar concentration (MAC) anes- thesia, titrate all drugs to effect. Standard dosing reg- imens (e.g., mg/kg) risk overdosing and possible loss of the AW. E. Regional anesthesia reduces the need for respiratory depressants and provides postoperative analgesia. Block placement may be technically difficult—consider using the sitting position to facilitate placement of neuraxial blocks. The response to local anesthetics can be unpredictable—titrate epidural and continu- ous spinal drugs carefully. Supine and Trendelenburg positions increase V/Q mismatch and may lead to hypoxia in a spontaneously breathing patient. F. Administration of general anesthesia presents many concerns and challenges—securing and maintaining an AW is paramount. With rare exceptions, deliver GA via endotracheal intubation. If difficulty with intu- bation is anticipated, perform an awake intubation with continuous supplemental oxygen. Position the patient with the head and shoulders elevated to facilitate preoxygenation and maintain FRC above closing capacity. Ensure complete denitrogenation before induction. Obese patients desaturate rapidly (dFRC and Doxygen consumption). Consider an RSI with cricoid pressure. Intraoperative ventilation of the patient may require high peak airway pressures due to poor chest wall and pulmonary compliance. Consider changes in ventilator type, settings, or the mode of ventilation to maintain adequate oxygena- tion and ventilation. Obese patients exhibit altered drug pharmacokinetics and pharmacodynamics. Therefore, choose drugs with short half-lives and low blood-fat solubility. As a general rule, calculate drug doses using IBW, except for the water-soluble muscle relaxants, where slightly higher than IBW should be used. Titrate drugs to effect. Before extubation, assure that the patient is awake, cooperative, and fully reversed from neuromuscular blockade. G. Postoperatively, observe for AW obstruction and hypoxia. Sit the patient upright to improve ventilation.
